PCI hotplug: cpqphp: use pci_dev->subsystem_{vendor|device}

The driver reads PCI subsystem IDs from the PCI configuration registers while
they are already stored by the PCI subsystem in the 'subsystem_{vendor|device}'
fields of 'struct pci_dev'...
