staging: dgap: ifdef HAVE_UNLOCKED_IOCTL conditionals

This patch removes the HAVE_UNLOCKED_IOCTL conditional
statements from driver.c, mgmt.c and mgmt.h. This was
used to support older kernels. It isn't needed now.
