When we call target_execute_cmd for write commands the command has been
on the state list before an abort might have come in before
target_execute_cmd. Call transport_check_aborted_status to deal with
this case.

Remove the execute_cmd method in struct se_subsystem_api, and always use the
one directly in struct se_cmd. To make life simpler for SBC virtual backends
a struct spc_ops that is passed to sbc_parse_cmd is added. For now it
only contains an execute_rw member, but more will follow with the subsequent
commits.

Since "target: Drop se_device TCQ queue_depth usage from I/O path" we always
submit all commands (or back then, tasks) from __transport_execute_tasks.
That means the the execute list has lots its purpose, as we can simply
submit the commands that are restarted in transport_complete_task_attr
directly while we walk the list. In fact doing so also solves a race
in the way it currently walks to delayed_cmd_list as well.

This patch changes back the pSCSI backend to follow pre 3.6-queue code to
passthrough SPC-3 persistent reservations + SPC-2 legacy reservation
handling to the underlying LLD / physical hardware.
For folks who really need this for their own SPC-3 emulation logic, avoid
changing the functionality of this beyond what is exported for REPORT_LUNS
for existing code, and to avoid problems with SPC-3 PR/ALUA as INQUIRY
EVPD=0x83 emulation needs to be in place in order for this to work as
expected with spc_parse_cdb() code..

Instead of trying to handle all SCSI command sets in one function
(transport_generic_cmd_sequencer) call out to the backend driver to perform
this functionality. For pSCSI a copy of the existing code is used, but for
all virtual backends we can use a new parse_sbc_cdb helper is used to
provide a simple SBC emulation.
For now this setups means a fair amount of duplication between pSCSI and the
SBC library, but patches later in this series will sort out that problem.

We don't need three flags to classifiy the CDB as we can check for a NULL S/G
list for a dataless command, and can infer from the absence of the data flag
that we deal with a control CDB. Also remove the _SG_IO from the data CDB
flag as all I/O is dont on S/G lists now.

When NUMBER OF LOGICAL BLOCKS is 0, WRITE SAME is supposed to write
all the blocks from the specified LBA through the end of the device.
However, dev->transport->get_blocks(dev) (perhaps confusingly) returns
the last valid LBA rather than the number of blocks, so the correct
number of blocks to write starting with lba is
dev->transport->get_blocks(dev) - lba + 1

This patch fixes a crash seen when large reads have their exchange
aborted by either timing out or being reset. Because the exchange
abort results in the seq pointer being set to NULL, because the
sequence is no longer valid, it must not be dereferenced. This
patch changes the function ft_get_task_tag to return ~0 if it is
unable to get the tag for this reason. Because the get_task_tag
interface provides no means of returning an error, this seems
like the best way to fix this issue at the moment.

The error paths in target_emulate_set_target_port_groups() are all
essentially "rc = -EINVAL; goto out;" but the code at "out:" ignores
rc and always returns success. This means that even if eg explicit
ALUA is turned off, the initiator will always see a good SCSI status
for SET TARGET PORT GROUPS.
Fix this by returning rc as is intended. It appears this bug was
added by the following patch:

Convert to use O_DSYNC for all cases at FILEIO backend creation time to
avoid the extra syncing of pure timestamp updates with legacy O_SYNC during
default operation as recommended by hch. Continue to do this independently of
Write Cache Enable (WCE) bit, as WCE=0 is currently the default for all backend
devices and enabled by user on per device basis via attrib/emulate_write_cache.
This patch drops the now unnecessary fd_buffered_io= token usage that was
originally signalling when to explictly disable O_SYNC at backend creation
time for buffered I/O operation. This can end up being dangerous for a number
of reasons during physical node failure, so go ahead and drop this option
for now when O_DSYNC is used as the default.
Also allow explict FUA WRITEs -> vfs_fsync_range() call to function in
fd_execute_cmd() independently of WCE bit setting.

Pull sbp-2 (firewire) target mode support from Nicholas Bellinger:
"The FireWire SBP-2 Target is a driver for using an IEEE-1394
connection as a SCSI transport. This module uses the SCSI Target
framework to expose LUNs to other machines attached to a FireWire bus,
in effect acting as a FireWire hard disk similar to FireWire Target
Disk mode on many Apple computers.
Also included are the two drivers/firewire/ patches required by
sbp-target to access fw_request fabric speed needed for mgt_agent
TCODE_WRITE_BLOCK_REQUEST ops, and exporting fw_card kref logic used
when creating/destroying active session references to individual
endpoints.
A credit goes to Chris in being able to get this code up and running
so quickly w/o any target core changes, and special thanks goes out to
Stefan Richter + Clemens Ladisch + Andy Grover for their help in
getting this driver ready for mainline. Also, one of Chris's goals
was to be able to connect sbp-target to a PowerPC based MacOS-X based

This patch fixes a bug in iscsit_allocate_iovecs() where iov_count was
incorrectly calculated using min(1UL, data_length / PAGE_SIZE) instead of
max(1UL, data_length / PAGE_SIZE), that ends up triggering an OOPs for
large block I/O when the SGL <-> iovec mapping exceeds the bogus iov_count
allocation size.
This is a regression introduced during the iscsi-target conversion back
to using core memory allocation here:

Neither "acceptor_values" nor "proposer_values" can be NULL here when
scanning the value lists for incoming iSCSI login parameters such as
HeaderDigest=CRC32C,None.
Smatch complains because we are not allowed to pass NULL pointers to
strchr(). Also I removed a second later check for "!acceptor_values"
because it gets checked on the next line in the do while condition.

This patch fixes a bug in the handling of FILEIO w/ underlying block_device
resize operations where the original fd_dev->fd_dev_size was incorrectly being
used in fd_get_blocks() for READ_CAPACITY response payloads.
This patch avoids using fd_dev->fd_dev_size for FILEIO devices with
an underlying block_device, and instead changes fd_get_blocks() to
get the sector count directly from i_size_read() as recommended by hch.

This patch adds support for ALUA MI_REPORT_TARGET_PGS extended header
format defined within SPC-4. It changes target core ALUA emulation logic
within target_emulate_report_target_port_groups() to support both the
extended and original length only header formats.
It includes adding a new 'implict_trans_secs' attribute for each ALUA
target port group to control the value returned to the application client
for an recommended implict translation timeout in seconds. By default
this value is currently set to zero, and limited up to 255 by virtue of
using a single byte in the extended header format.
This value is used by target_emulate_report_target_port_groups() within
the extended header logic to set IMPLICIT TRANSITION TIME as defined by
spc4r30.

This patch fixes the MAINTENANCE_IN service action type checks to only
look at the proper lower 5 bits of cdb byte 1. This addresses the case
where MI_REPORT_TARGET_PGS w/ extended header using the upper three bits of
cdb byte 1 was not processed correctly in transport_generic_cmd_sequencer,
as well as the three cases for standby, unavailable, and transition ALUA
primary access state checks.
Also add MAINTENANCE_IN to the excluded list in transport_generic_prepare_cdb()
to prevent the PARAMETER DATA FORMAT bits from being cleared.

This patch addresses a bug in a special case for target core SPC-2 RELEASE
logic where the same physical client (eg: iSCSI InitiatorName) with
differing iSCSI session identifiers (ISID) is allowed to incorrectly release
the same client's SPC-2 reservation from the non reservation holding path.
Note this bug is specific to iscsi-target w/ SPC-2 reservations, and
with the default enforce_pr_isids=1 device attr setting in target-core
controls if a InitiatorName + different ISID reservations are handled
the same as a single iSCSI client entity.

This patch removes some potentially problematic legacy code within
core_clear_initiator_node_from_tpg() that was originally intended to
release left over se_lun_acl setup during dynamic NodeACL+MappedLUN
generate when running with TPG demo-mode operation.
Since we now only ever expect to allocate and release se_lun_acl from
within target_core_fabric_configfs.c:target_fabric_make_mappedlun() and
target_fabric_drop_mappedlun() context respectively, this code for
demo-mode release is incorrect and needs to be removed.

This patch removes the original usage of dev_attr->max_sectors in favor of
dev_attr->hw_max_sectors that is now being enforced by target core from
within transport_generic_cmd_sequencer() for SCF_SCSI_DATA_SG_IO_CDB ops.
After the recent se_task removal patches from hch, this value for IBLOCK
backends being set via configfs by userspace from an saved max_sectors
value that is turning out to be problematic, so it makes sense to go ahead
and remove this now legacy attribute all-together.
This patch also continues to make se_dev_set_default_attribs() do
(sectors / block_size) alignment for what actually get used by
target_core_mod to be safe here, following the same alignment currently
used by fabric_max_sectors.

the hw_max_sectors reported by the backend device in order to determine when
to reject an I/O's who's sector count exceeds what is supported by the backend
with a single se_cmd descriptor.
It addresses a potential case where se_dev_attrib.max_sectors for IBLOCK
backends has already been set via queue_max_sectors() to something small
like max_sectors=32 (LVM, DRBD may do this), resulting typically sized
SCF_SCSI_DATA_SG_IO_CDB to be incorrectly rejected with invalid_cdb_field
in transport_generic_cmd_sequencer().

Simply transport_generic_new_cmd to only allocate a single task. For normal
unidirection commands nothing changes except that the code is a lot simpler
now. Any BIDI support that used to work will stop now for the next few
patches at least.

Now that tasks are always the same size as the command there is no need
to rewrite a CDB in common code. Notw that we keep the separately allocated
CDB in the pscsi and stgt backends for now, to easy reintroducing any
command splitting local to these backends if nessecary.

The SCSI MMC GET_EVENT_STATUS_NOTIFICATION command can be used to find
out about media change, among other things. This patch adds it to the
command sequencer so that PSCSI CD-ROM passthrough works with modern
Linux guests that issue this command.

We can use kcalloc() here instead of kzalloc(). It's better style and
it has overflow checking built in.
Also -ENOMEM is the correct error code for allocation errors. -1 means
-EPERM. None of the callers preserve the error codes so it doesn't
matter except as a cleanup.

We originally changed iscsi to allocate its own buffers just as an
intermediate step to clean up some core buffer allocation mechanisms. Now
we can put it back.
Also had to change allocate_iovecs to use data_length instead of
t_data_nents because iovecs are now allocated before the data buffer, thus
t_data_nents is not yet initialized.

Immediate queue:
Consolidate down to one switch statement by moving send_tx_data and stuff
from second switch into the first switch, or the functions the first switch
calls.
Response queue:
Do not lock istate_lock except directly around i_state modifications.
Put entire ISTATE_SEND_DATAIN path within first switch statement, in prep
for further refactoring.
All other cases set use_misc = 1 and will not be using sendpage, so just
use send_tx_data for these and set use_misc param to 1.
map_sg, sent_status, use_misc, and se_cmd vars no longer needed.
Then put immediate and response handling in separate functions in order
to get iscsi_target_tx_thread down to where it fits on a page.

Now that all fabrics are converted over to using se_cmd->t_data_sg
directly, we can drop the task sg chaining support. With the modern
memory allocation in target core, task sg chaining is needless
overhead -- we would split up the main cmd sglist into pieces, and
then splice those pieces back together instead of just using the
original list directly.

With the modern target core, se_cmd->t_data_sg already points to a
sglist that covers the whole command. So task_sg chaining is needless
overhead and obfuscation -- instead of splicing the split up task
sglists back into one list, we can just use the original list directly.

The high-performance backends (iblock and rd) support tasks of unlimited
size. With that there is no reason to keep a complex infrastructure for
splitting up commands in place. Stop doing so and only submit a single
task per data direction. Once this is in place we can slowly remove fields
from the task that duplicate things in the command, or move other fields
into the command.
From nab:
The benefit to IBLOCK performance by removing the additional
fast-path allocation overhead + SGL mapping to se_task->task_sg[] is now
greater than transparently supporting an received CDB I/O length that
exceeds what is allowed by backend pSCSI LLD hardware max_sectors, that
was originally supported for all backend export cases.
This change may effect some users of pSCSI users on legacy hardware, but
I think most folks are now using TYPE_DISK struct scsi_device export
with IBLOCK. The only other place where this may can issues that cannot
be resolved with IBLOCK TYPE_DISK is using TYPE_ROM, TYPE_TAPE or other
pSCSI non TYPE_DISK export with an SCSI LLDs using a smaller
max_sectors.

The UASP protocol does not inform the target device upfront how much
data it should expect so we have to learn in from the CDB. So in order
to handle this case, add a TARGET_SCF_UNKNOWN_SIZE to target_submit_cmd()
and perform an explictly assignment for se_cmd->data_length from the
extracted CDB size in transport_generic_cmd_sequencer().

This was used at one time as a hack by FILEIO backend registration to
allow a struct block_device that was claimed with blkdev_get (by a local
filesystem mount for example) to be exported as read-only (SCSI WP=1).
Since FILEIO backend registration will no longer attempt to obtain
exclusive access to an underlying struct block_device here, this flag is
now obsolete.

Avoid freeing a registered tpg structure if an alloc_workqueue call
fails. This fixes a bug where the failure was leaking memory associated
with se_portal_group setup during the original core_tpg_register() call.

Add abort flag and use it to terminate processing when an exchange
is timed out or is reset. The abort flag is used in place of the
transport_generic_free_cmd function call in the reset and timeout
cases, because calling that function in that context would free
memory that was in use. The aborted flag allows the lifetime to
be managed in a more normal way, while truncating the processing.
This change eliminates a source of memory corruption which
manifested in a variety of ugly ways.

This patch addresses a tcm_loop bug with transport_generic_allocate_tasks()
return checking in tcm_loop_submission_work() where other non zero return
codes (including -EBUSY for reservation conflicts) are incorrectly falling
through to transport_generic_map_mem_to_cmd() -> transport_handle_cdb_direct().
This bug was introduced into target-pending/for-next-merge with the following

Remove the hack that has READ CAPACITY(10) return 0xFFFFFFFF as the
number of sectors when thin provisioning is enabled. This is supposed
to trigger the initiator to use READ CAPACITY(16) in this case so that
it finds out about thin provisioning. But an initiator that cares about
thin provisioning is going to ask anyway, and an initiator that doesn't
know about READ CAPACITY(16) is going to get the wrong capacity. So
just have READ CAPACITY(10) return the size it's supposed to.

This patch addresses a iscsi-target specific bug related to reservation conflict
handling in iscsit_handle_scsi_cmd() that has been causing reservation conflicts
to complete and not fail as expected due to incorrect errno checking. The problem
occured with the change to return -EBUSY from transport_generic_cmd_sequencer() ->
transport_generic_allocate_tasks() failures, that broke iscsit_handle_scsi_cmd()
checking for -EINVAL in order to invoke a non GOOD status response.
This was manifesting itself as data corruption with legacy SPC-2 reservations,
but also effects iscsi-target LUNs with SPC-3 persistent reservations.

This patch addresses a bug with target_check_scsi2_reservation_conflict()
return checking in target_scsi2_reservation_[reserve,release]() that was
preventing CRH=1 operation from silently succeeding in the two special
cases defined by SPC-3, and not failing with reservation conflict status
when dealing with legacy RESERVE/RELEASE + active SPC-3 PR logic.
Also explictly set cmd->scsi_status = SAM_STAT_RESERVATION_CONFLICT during
the early non reservation holder failure from pr_ops->t10_seq_non_holder()
check in transport_generic_cmd_sequencer() for fabrics that already expect
it to be set.

This patch fixes a bug in tcm_fc where fc_exch memory from fc_exch_mgr->ep_pool
is currently being leaked by ft_send_resp_status() usage. Following current
code in ft_queue_status() response path, using lport->tt.seq_send() needs to be
followed by a lport->tt.exch_done() in order to release fc_exch memory back into
libfc_em kmem_cache.
ft_send_resp_status() code is currently used in pre submit se_cmd ft_send_work()
error exceptions, TM request setup exceptions, and main TM response callback
path in ft_queue_tm_resp(). This bugfix addresses the leak in these cases.

This patch converts iscsi-target session code to use se_sess->sess_kref
counting for iscsi session shutdown. The following cases include:
*) last iscsit_close_connection() shutdown path to invoke close session
*) iscsit_logout_post_handler_closesession() for explict logout
*) iscsit_free_session() caller for explict shutdown
It also moves iscsit_stop_session() call from lio_tpg_close_session()
into lio_tpg_shutdown_session() TFO callbacks to invoke an explict

This patch converts core_tpg_del_initiator_node_acl() shutdown from configfs
context to use se_node_acl->acl_kref and ->acl_free_comp in order to wait for
outstanding fabric callbacks to complete via transport_deregister_session()
callbacks before waking ->acl_free_comp from the last ->acl_kref put.
It also changes core_tpg_del_initiator_node_acl() to setup a local sess_list
with target_get_session() + acl->acl_stop = 1 for active sessions that will
be shutdown, and changes transport_deregister_session_configfs() to check
for ->acl_stop usage.

Add se_node_acl->acl_free_comp for NodeACL release path to wait for outstanding
fabric session shutdown to complete in transport_deregister_session() before
finishing NodeACL release from configfs process context.
Also make transport_deregister_session() clear the comp_nacl bit

status TMR_LUN_DOES_NOT_EXIST and invoke TFO->queue_tm_rsp(), instead of

The old code did (MSB << 8) & 0xff, which always evaluates to 0. Just use
get_unaligned_be16() so we don't have to worry about whether our open-coded
version is correct or not.

cdb_offset is always equal to offset - 8, so remove that one. More
importantly, the existing code only worked correct if
se_cmd->data_length is a multiple of 8. Pass in a length of, say, 9 and
we will happily overwrite 7 bytes of "unallocated" memory.
Now, afaics this bug is currently harmless, as allocations will
implicitly be padded to multiples of 8 bytes. But depending on such a
fact wouldn't qualify as sound engineering practice.

If the target core signals an over- or under-run, tcm_loop should call
scsi_set_resid() to tell the SCSI midlayer about the residual data length.
The difference can be seen by doing something like
strace -eioctl sg_raw -r 1024 /dev/sda 8 0 0 0 1 0 > /dev/null
and looking at the "resid=" part of the SG_IO ioctl -- after this patch,
the field is correctly reported as 512.

se_dev_attrib.max_sectors currently has two independent meanings:
- It is reported in the block limits VPD page as the maximum transfer
length, ie the largest IO that the front-end (fabric) can handle.
Also the target core doesn't enforce this maximum transfer length.
- It is used to hold the size of the largest IO that the back-end can
handle, so we know when to split SCSI commands into multiple tasks.
Fix this by adding a new se_dev_attrib.fabric_max_sectors to hold the
maximum transfer length, and checking incoming IOs against that limit.

SPC-4 says about the WBUS16 and SYNC bits:
The meanings of these fields are specific to SPI-5 (see 6.4.3).
For SCSI transport protocols other than the SCSI Parallel
Interface, these fields are reserved.
We don't have a SPI fabric module, so we should never set these bits.
(The comment was misleading, since it only mentioned Sync but the
actual code set WBUS16 too).

Current code sets the peripheral device type to 0x3f == "not present
unknown" for virtual LUN 0 for standard INQUIRY commands, but leaves it
as 0 == "connected direct access block" for VPD INQUIRY commands. This
is just because the check for LUN 0 only happens in some code paths.
Make our peripheral device type consistent by moving the LUN 0 check
into the common emulate_inquiry() code.

If the initiator sends us an INQUIRY command with an allocation length
that's shorter than what we want to return, we're simply supposed to
truncate our response and return what the initiator gave us space for,
without signaling any error. Current target code has various tests that
don't fill out the full response if the buffer is too short and
sometimes return errors incorrectly.
Fix this up by allocating a bounce buffer for INQUIRY responses if we
need to, ie if we have cmd->data_length too small as well as
SCF_PASSTHROUGH_SG_TO_MEM_NOALLOC set in cmd->se_cmd_flags -- for most
fabrics, we always allocate at least a full page, but for tcm_loop we
may have a small buffer coming directly from the SCSI stack.
This lets us delete a lot of cmd->data_length checking, and also makes
our INQUIRY handling correct per SPC in a lot more cases.

This patch adds initial support for TMR_ABORT_TASK ops for se_cmd
descriptors using se_sess->sess_cmd_list and se_cmd->cmd_kref counting.
It will perform an explict abort for all outstanding se_cmd ops based
upon tmr->ref_task_tag that have not been set CMD_T_COMPLETE.
It will cancel se_cmd->work and wait for backing I/O to complete before
attempting to send SAM_STAT_TASK_ABORTED and perform
target_put_sess_cmd() to release the referenced descriptor.
It also adds a CMD_T_ABORTED check into transport_complete_task() to
catch the completion from backend I/O that has been aborted, and
updates transport_wait_for_tasks() to allow CMD_T_ABORTED usage with
core_tmr_abort_task() context.

transport_generic_request_failure() is a wrapper around calling
transport_send_check_condition_and_sense() that is required once
an se_cmd->cmd_kref has been obtained via target_submit_cmd() ->
target_get_sess_cmd().
tcm_qla2xxx currently requires this, and since it's necessary for
other callers using target_submit_cmd() make it exportable now.

This code isn't broken per se, but it's scary to look at! It looks like
in the t_data_nents==1 case we're doing both a kunmap and a vunmap,
what's saving us is that t_data_vmap in this case is 0, so vunmap
doesn't do anything.
Return after kunmap, so the handling of the three cases does not overlap.

There is no real limit for task sizes in the iblock driver given that we
can chain bios. Increase the maximum size to UINT_MAX, and change the
code to submit bios in a smaller batch size to avoid deadlocks when
having more bios in flight than the pool supports. Also increase the
pool size to always allow multiple tasks to be in flight.
I also had to change the task refcounting to include one reference for
the submission task, which is a standard practice in this kind of code
in Linux (e.g. XFS I/O submission). This was wrong before, but couldn't
be hit easily.

a debug printk in the detach path. I've simply removed the verbose

Replace various atomic_ts used as flags in struct se_cmd with a single
transport_state bitmap that requires t_state_lock to be held for modifications.
In the target core that assumption generally is true, but some recently added
code in the SRP target had to grow new lock calls. I can't say I like the way
how it messes with the command state directly, but let's leave that for later.
(Re-add missing ib_srpt.c changes that nab dropped..)

The call_rcu() in ft_tport_delete() invokes ft_tport_rcu_free(),
which just does a kfree(). So convert the call_rcu() to kfree_rcu(),
allowing ft_tport_rcu_free() to be eliminated.

This patch fixes a bug in target-core where unsupported WRITE_SAME ops
from a target_check_write_same_discard() failure was incorrectly
returning CHECK_CONDITION w/ TCM_INVALID_CDB_FIELD sense data.
This was causing some clients to not properly fall back, so go ahead
and use the correct TCM_UNSUPPORTED_SCSI_OPCODE sense for this case.

Use IP_FREEBIND socket option so that iscsi portal configuration with
explicit IP addresses can happen during boot, before network interfaces
have been assigned IPs.
This is especially important on systemd based Linux boxes where system
boot happens asynchronously and non-trivial configuration must be done
to get targetcli.service to start synchronously after the network is
configured.

The block layer keeps q->limits.discard_granularity in bytes, but iblock
(and the SCSI Block Limits VPD page) keep unmap_granularity in blocks.
Report the correct value when exporting block devices by dividing to
convert bytes to blocks.

This patch fixes a bug in target_submit_cmd() where the failure path
for transport_generic_allocate_tasks() made a direct call to
transport_send_check_condition_and_sense() and not calling the
final target_put_sess_cmd() release callback.
For transport_generic_allocate_tasks() failures, use the proper call to
transport_generic_request_failure() to handle kref_put() along
with potential internal queue full response processing.
It also makes transport_lookup_cmd_lun() failures in
target_submit_cmd() use transport_send_check_condition_and_sense() and
target_put_sess_cmd() directly to avoid se_cmd->se_dev reference in
transport_generic_request_failure() handling.
Finally it drops the out_check_cond: label and use direct reference for
allocate task failures, and per-se_device queue_full handling is
currently not supported for transport_lookup_cmd_lun() failure
descriptors due to se_device dependency.

Retval not very useful, and may even be harmful. Once submitted, fabrics
should expect a sense error if anything goes wrong. All fabrics checking
of this retval are useless or broken:
fc checks it just to emit more debug output.
ib_srpt trickles retval up, then it is ignored.
qla2xxx trickles it up, which then causes a bug because the abort goto
in qla_target.c thinks cmd hasn't been sent to target.
Just returning nothing is best.

WindowsXP+BOT issues a MODE_SENSE request with page 0x1c which is not
suppoerted by target. Target rejects that command with
TCM_INVALID_CDB_FIELD, so far so good. On BOT I can't send the SENSE
response back, instead I can only reply that an error occured. The next
thing happens is a REQUEST_SENSE request with 18 bytes length. Since the
check here is more than 18 bytes I have to NACK that request as well.
This is not really required: We check for some additional room, but we
never use it. The additional length is set to 0xa so the total length is
0xa + 8 = 18 which is fine with my 18 bytes.

This patch addresses a bug with sendtargets discovery where INADDR_ANY (0.0.0.0)
+ IN6ADDR_ANY_INIT ([0:0:0:0:0:0:0:0]) network portals where incorrectly being
reported back to initiators instead of the address of the connecting interface.
To address this, save local socket ->getname() output during iscsi login setup,
and makes iscsit_build_sendtargets_response() return these TargetAddress keys
when INADDR_ANY or IN6ADDR_ANY_INIT portals are in use.

We need to handle >1 page control cdbs, so extend the code to do a vmap
if bigger than 1 page. It seems like kmap() is still preferable if just
a page, fewer TLB shootdowns(?), so keep using that when possible.

A statement such as
struct iscsi_node_attrib *na = na = iscsit_tpg_get_node_attrib(sess);
has undefined behaviour since there are two assignments to 'na', strictly
speaking (the order in which side-effects from the assignments take place
is undefined since there's no intervening sequence point), and it looks
unintentional in any case.

It may happen that uasp will free the request in irq conntext, the
callchain:
uasp_cmd_release() -> transport_generic_free_cmd() -> core_dec_lacl_count()
where the last function enables the IRQ. Those irqs are re-disabled
later (due to the spin.*irq_restore) but in between we could get hurt.

This patch adds a work-around for handling zero allocation length
control CDBs (type SCF_SCSI_CONTROL_SG_IO_CDB) that was causing an
OOPs with the following raw calls:
# sg_raw -v /dev/sdd 3 0 0 0 0 0
# sg_raw -v /dev/sdd 0x1a 0 1 0 0 0
This patch will follow existing zero-length handling for data I/O
and silently return with GOOD status. This addresses the zero length
issue, but the proper long-term resolution for handling arbitary
allocation lengths will be to refactor out data-phase handling in
individual CDB emulation logic within target_core_cdb.c

According to SPC-4, the sense key for commands that are failed with
INVALID FIELD IN PARAMETER LIST and INVALID FIELD IN CDB should be
ILLEGAL REQUEST (5h) rather than ABORTED COMMAND (Bh). Without this
patch, a tcm_loop LUN incorrectly gives:

Doing alloc_page(GFP_KERNEL | __GFP_ZERO) to get pages used for data
buffers wastes a lot of CPU clearing pages that will be quickly be
overwritten by the actual data. However, for emulated control
commands such as INQUIRY and so on, the code does assume that the
buffer is zeroed.
To avoid this CPU overhead, skip the __GFP_ZERO for commands that are
actually moving data, ie cmds that have SCF_SCSI_DATA_SG_IO_CDB set.

This patch removes a legacy se_dev_check_online() check from within
transport_execute_tasks() that should no longer be necessary as
transport_lookup_cmd_lun() is already making this call.
Using transport_cmd_check_stop() from transport_execute_tasks() should
already be checking per se_cmd context for each descriptor upon active
I/O shutdown, so no need to acquire dev->dev_status_lock again while
executing se_task submission.

This patch removes the original usage of __transport_execute_tasks() ahead
of every transport_get_cmd_from_queue() call in transport_processing_thread().
This helps reduce se_device->execute_task_lock contention between qla2xxx wq
with target_submit_cmd() for READs and transport_processing_thread()
context servicing WRITEs with full payloads for I/O submission.
It also adds a __transport_execute_tasks() to kick the task queue again
without a *se_cmd descriptor with existing queue full logic, but this may
end up not being necessary.

This patch makes __transport_execute_tasks() perform the addition of
tasks to dev->execute_task_list via __transport_add_tasks_from_cmd()
while holding dev->execute_task_lock during normal I/O fast path
submission.
It effectively removes the unnecessary re-acquire of dev->execute_task_lock
during transport_execute_tasks() -> transport_add_tasks_from_cmd() ahead
of calling __transport_execute_tasks() to queue tasks for the passed
*se_cmd descriptor.

Historically, pSCSI devices have been the ones that required target-core
to enforce a per se_device->depth_left. This patch changes target-core
to no longer (by default) enforce a per se_device->depth_left or sleep in
transport_tcq_window_closed() when we out of queue slots for all backend
export cases.

This patch adds a target_submit_cmd() caller that can be used by fabrics
to submit an uninitialized se_cmd descriptor to an struct se_session +
unpacked_lun from workqueue process context. This call will invoke the
following steps:
- transport_init_se_cmd() to setup se_cmd specific pointers
- Obtain se_cmd->cmd_kref references with target_get_sess_cmd()
- set se_cmd->t_tasks_bidi
- transport_lookup_cmd_lun() to setup struct se_cmd->se_lun from
the passed unpacked_lun
- transport_generic_allocate_tasks() to setup the passed *cdb, and
- transport_handle_cdb_direct() handle READ dispatch or WRITE
ready-to-transfer callback to fabric

This patch moves target_put_sess_cmd() to use a se_cmd->cmd_kref
callback target_release_cmd_kref when performing driver release of
fabric->se_cmd descriptor memory. It sets the default cmd_kref
count value to '2' within target_get_sess_cmd() setup, and
currently assumes TFO->check_stop_free() usage.
It drops se_tfo->check_release_cmd() usage in the main
transport_release_cmd codepath.

There is not reason to artifically limit max_sectors in tcm_loop, set
it to UINT_MAX to allow stressing the large I/O handling in the target
core using the loopback driver. Also remove various superflous defines
hiding the values set in the host template.

A reader should spend an extra moment whenever noticing a cast,
because either something special is going on that deserves extra
attention or, as is all too often the case, the code is wrong.
These casts, afaics, have all been useless. They cast a foo* to a
foo*, cast a void* to the assigned type, cast a foo* to void*, before
assigning it to a void* variable, etc.
In a few cases I also removed an additional &...[0], which is equally
useless.
Lastly I added three FIXMEs where, to the best of my judgement, the
code appears to have a bug. It would be good if someone could check
these.

The command
| echo rd_pages=32768 > ramdisk/control
Does not work because it writes "rd_pages=32768\n" and the parser which
matches for "rd_pages=%d" does not recognize it due to the \n. One way
of fixing this would be using "echo -n" instead.
This patch adds \n to the list of separators so we don't have to use the
-n argument which I find is more convinient.

There is no need to make task_state_active an atomic_t given that it is
always set under the execute_task_lock so we can make it a simple bool.
Also rename it to t_state_active to be closer to the list it guards,
and make sure all checks before the list addion/removal actually happen
under execute_task_lock.

This reorganized the headers under include/target into:
- target_core_base.h stays as is with all target-wide data stuctures and defines
- target_core_backend.h contains the whole interface to I/O backends
- target_core_fabric.h contains the whole interface to fabric modules

Create a new headers, drivers/target/target_core_internal.h that is supposed
to hold all target_core-internal prototypes. Move all non-exported includes
from include/target to it, and merge the smaller prototype-only includes
inside drivers/target into it as well. Mark functions that were found to
not be called outside their implementation file static.

If an attribute is present (but not yet supported) it should be OK
to write 0 (a no-op) to the attribute.
This is an issue because userspace should be able to save and restore all
set attribute values without error.

So the code assumes that the sg list is only a array while in reality
loopback SGL memory via scsi_cmnd into target-core may be already
chained. This patch converts ramdisk code to use sg_miter logic from
scatterlist.h in order to properly support passthrough SGL usage with
transport_generic_map_mem_to_cmd() via loopback.
With this patch the bug goes away. However after umount/mount of the
device my files are gone. So something is still not right. After looking
at it for a while I decided to rewrite the that part of the code and now
things do work for me.

memory passed into from loopback LLD struct scsi_cmnd scatterlist memory.

SBC-3 says:
A TRANSFER LENGTH field set to zero specifies that 256 logical
blocks shall be written. Any other value specifies the number
of logical blocks that shall be written.
The old code was always just returning the value in the TRANSFER LENGTH
byte. Fix this to return 256 if the byte is 0.

IO commands with a TRANSFER LENGTH of 0 are not an error; for example,
for READ (10) and WRITE (10), SBC-3 says:
A TRANSFER LENGTH field set to zero specifies that no logical blocks
shall be read. This condition shall not be considered an error.
In case we have nothing to do, just complete the command with good status.

This patch fixes iscsi-target handling of underflow where residual data is
causing an OOPs by using the incorrect iscsi_cmd_t->data_length initially
assigned in iscsit_allocate_se_cmd(). It resets iscsi_cmd_t->data_length
from se_cmd_t->data_length after transport_generic_allocate_tasks()
has been invoked in iscsit_handle_scsi_cmd() RX context, and converts
iscsi_cmd->residual_count usage to access iscsi_cmd->se_cmd.residual_count
to get the proper residual count set by target-core.

This patch changes transport_generic_map_mem_to_cmd() to reject SCSI data
overflow and to send exception status with CHECK_CONDITION + TCM_INVALID_CDB_FIELD
for fabrics that are passing a pre-populated struct scatterlist (eg: tcm_loop
and iscsi-target) being mapped into se_cmd->t_data_sg and se_cmd->t_data_nents.
This addresses an OOPs where transport_allocate_data_tasks() would walk
the incorrect post OVERFLOW cmd->data_length value beyond the end of
the passed scatterlist.

This patch removes config_item_name() informational usage of
TFO->free_wwn() treewide in loopback, tcm_fc, ib_srpt and
tcm_vhost module code.
Using v4 target_core_fabric_configfs.c logic, a fabric call for
config_item_name() in TFO->drop_wwn() context returns NULL as
target_fabric_drop_wwn() invoking config_item_put() ->
config_group_put() will release fabric_port->port_wwn.wwn_group
before the last config_item_put() -> TFO->drop_wwn() is
invoked.

While testing ib_srpt I noticed that the target system became
rather unresponsive during intensive I/O. The patch below made

This patch removes legacy usage of PYX_TRANSPORT_* return codes in a number
of locations and addresses cases where transport_generic_request_failure()
was returning the incorrect sense upon CHECK_CONDITION status after the
v3.1 converson to use errno return codes.
This includes the conversion of transport_generic_request_failure() to
process cmd->scsi_sense_reason and handle extra TCM_RESERVATION_CONFLICT
before calling transport_send_check_condition_and_sense() to queue up
response status. It also drops PYX_TRANSPORT_OUT_OF_MEMORY_RESOURCES legacy
usgae, and returns TCM_LOGICAL_UNIT_COMMUNICATION_FAILURE w/ a response
for these cases.
transport_generic_allocate_tasks(), transport_generic_new_cmd(), backend
SCF_SCSI_DATA_SG_IO_CDB ->do_task(), and emulated ->execute_task() have
all been updated to set se_cmd->scsi_sense_reason and return errno codes
universally upon failure. This includes cmd->scsi_sense_reason assignment
in target_core_alua.c, target_core_pr.c and target_core_cdb.c emulation code.
Finally it updates fabric modules to remove the legacy usage, and for
TFO->new_cmd_map() callers forwards return values outside of fabric code.
iscsi-target has also been updated to remove a handful of special cases
related to the cleanup and signaling QUEUE_FULL handling w/ ft_write_pending()
(v2: Drop extra SCF_SCSI_CDB_EXCEPTION check during failure from
transport_generic_new_cmd, and re-add missing task->task_error_status
assignment in transport_complete_task)

All ->execute_task instances now need to complete the I/O explicitly,
which can either happen synchronously or asynchronously.
Note that a lot of the CDB emulations appear to return success even if
some lowlevel operations failed. Given that this is an existing issue
this patch doesn't change that fact.

This patch adds the initial pieces of generic active I/O shutdown logic.
This is intended to be a 'opt-in' feature for fabric modules that
includes the following functions to provide a mechinism for fabric
modules to track se_cmd via se_session->sess_cmd_list:
*) target_get_sess_cmd() - Add se_cmd to sess->sess_cmd_list, called
from fabric module incoming I/O path.
*) target_put_sess_cmd() - Check for completion or drop se_cmd from
->sess_cmd_list
*) target_splice_sess_cmd_list() - Splice active I/O list from
->sess_cmd_list to ->sess_wait_list, can called with HW fabric
lock held.
*) target_wait_for_sess_cmds() - Walk ->sess_wait_list waiting on
individual ->cmd_wait_comp. Optional transport_wait_for_tasks()
call.
target_splice_sess_cmd_list() is allowed to be called under HW fabric
lock, and performs the splice into se_sess->sess_wait_list and set
se_cmd->cmd_wait_set. Then target_wait_for_sess_cmds() walks the list
waiting for individual target_put_sess_cmd() fabric callbacks to
complete.
It also adds TFO->check_release_cmd() to split the completion and memory
release calls, where a fabric module uses target_put_sess_cmd() to check
for I/O completion during session shutdown. This is currently pushed out
into fabric modules as current fabric code may sleep here waiting for
TFO->check_stop_free() to complete in main response path, and because
target_wait_for_sess_cmds() calling TFO->release_cmd() to free fabric
descriptor memory directly.

accidentally removed setting t_tasks_failed in transport_complete_task.
Add it back in a slightly cleaner way; now it is set for every failed task
instead of special casing the last one completing by using the success
argument directly for it.

This patch drops TRANSPORT_FREE_CMD_INTR usage from target core, which
includes the removal of transport_generic_free_cmd_intr() symbol,
TRANSPORT_FREE_CMD_INTR usage in transport_processing_thread(), and
special case LUN_RESET handling to skip TRANSPORT_FREE_CMD_INTR processing
in core_tmr_drain_cmd_list(). We now expect that fabric modules will
use an internal workqueue to provide process context when releasing
se_cmd descriptor resources via transport_generic_free_cmd().

This patch converts target_core_fabric_ops->check_stop_free() usage in
transport_cmd_check_stop() and associated fabric module usage to
return '1' when the passed se_cmd has been released directly within
->check_stop_free(), or return '0' when the passed se_cmd has not
been released.
This addresses an issue where transport_cmd_finish_abort() ->
transport_cmd_check_stop_to_fabric() was leaking descriptors during
LUN_RESET for modules using ->check_stop_free(), but not directly
releasing se_cmd in all cases.

This patch addresses two issues with non immediate TMR handling in
iscsit_handle_task_mgt_cmd(). The first involves breakage due to
v3.1-rc conversion of iscsit_sequence_cmd(), which upon good status
would hit the iscsit_add_reject_from_cmd() block of code. This patch
adds an explict check for CMDSN_ERROR_CANNOT_RECOVER.
The second adds a check to return when non immediate TMR operation is
detected after iscsit_ack_from_expstatsn(), as iscsit_sequence_cmd()
-> iscsit_execute_cmd() will have called transport_generic_handle_tmr()
for the non immediate TMR case already.

This patch fixes a bug in core_tmr_drain_tmr_list() where drain_tmr_list
was using the wrong se_tmr_req for cmd assignment due to a typo during the
LUN_RESET re-org. This was resulting in general protection faults while
using the leftover bogus *tmr_p pointer from list_for_each_entry_safe().

This patch changes target core to also check for -ENOMEM from fabric callbacks
to signal QUEUE_FULL status, instead of just -EAGAIN in order to catch a
larger set of fabric failure cases that want to trigger QUEUE_FULL logic.
This includes the callbacks for ->write_pending(), ->queue_data_in() and
->queue_status().
It also makes transport_generic_write_pending() return zero upon QUEUE_FULL,
and removes two unnecessary -EAGAIN checks to catch write pending QUEUE_FULL
cases from transport_generic_new_cmd() failures in transport_handle_cdb_direct()
and transport_processing_thread():TRANSPORT_NEW_CMD_MAP state.

This patch addresses an issue with buggy userspace code sending I/O
via scsi-generic that does not explictly clear their associated read
buffers. It adds an explict memset of the first SGL entry within
tcm_loop_new_cmd_map() for SCF_SCSI_CONTROL_SG_IO_CDB payloads that
are currently guaranteed to be a single SGL by target-core code.
This issue is a side effect of the v3.1-rc1 merge to remove the
extra memcpy between certain control CDB types using a contigious
+ cleared buffer in target-core, and performing a memcpy into the
SGL list within tcm_loop.
It was originally mainfesting itself by udev + scsi_id + scsi-generic
not properly setting up the expected /dev/disk/by-id/ symlinks because
the INQUIRY payload was containing extra bogus data preventing the
proper NAA IEEE WWN from being parsed by userspace.

This patch removes the legacy usage of se_task->task_timer and associated
infrastructure that originally was used as a way to help manage buggy backend
SCSI LLDs that in certain cases would never return back an outstanding task.
This includes the removal of target_complete_timeout_work(), timeout logic
from transport_complete_task(), transport_task_timeout_handler(),
transport_start_task_timer(), the per device task_timeout configfs attribute,
and all task_timeout associated structure members and defines in
target_core_base.h
This is being removed in preparation to make transport_complete_task() run
in lock-less mode.

This patch converts target-core to use se_cmd->t_transport_sent instead of
a duplicated se_cmd->transport_sent member in a handful of locations.
It also updates iscsi_target to properly use ->t_transport_sent instead of
it's own iscsi_cmd_t->transport_sent value that was not being assigned.

If we only have a single task per command (which at least in my testing
is the by far most common case) we do not have to allocate a new per-task
S/G list but can reuse the one from the command.

There were only two callers, and one of them always wants the call
to transport_allocate_data_tasks anyway. Also drop the constant
lba argument to transport_allocate_data_tasks and move the variables
inside it into the minimum required scope.

These are two fairly small functions, and merging them gives a much
more readable control flow, and opportunities for more useful comments.
It also moves all code related to resources allocation closer together
and allows to remove a forward declaration for transport_allocate_tasks.

This field is never used given that BIDI handling happens at the
command and not the task level. Remove it and the dead code in
pscsi that tries to work on it.
It also prevents pSCSI passthrough for the two currently enabled BIDI
commands now that task->task_sg_bidi support has been removed.

Instead of abusing the target processing thread for offloading I/O
completion in the backends to user context add a new workqueue. This means
completions can be processed as fast as available CPU time allows it,
including in parallel with other completions and more importantly I/O
submission or QUEUE FULL retries. This should give much better performance
especially on loaded systems.
As a fallout we can merge all the completed states into a single
one.
On the downside this change complicates lun reset handling a bit by
requiring us to cancel a work item only for those states that have it
initialized. The alternative would be to either always initialize the work
item to a dummy handler, or always use the same handler and do a switch on
the state. The long term solution will be a flag that says that the command
has an initialized work item, but that's only going to be useful once we
have more users.

We only need to decrement dev->depth_left if failing a command from
__transport_execute_tasks. Instead of doing it first thing in
transport_generic_request_failure and requiring a pseudo-flag argument
for it just opencode the decrement in the two callers (which should
be factored into a single one anyway)

Currently we stop the timers for all tasks in a command fairly late during
I/O completion, which is fairly pointless and requires all kinds of safety
checks.
Instead delete pending timers early on in transport_complete_task, thus
ensuring no new timers firest after that. We take t_state_lock a bit later
in that function thus making sure currenly running timers are out of the
criticial section. To be completely sure the timer has finished we also
add another del_timer_sync call when freeing the task.
This also allows removing TF_TIMER_RUNNING as it would be equivalent
to TF_ACTIVE now.

TF_TIMER_STOP is useless as it only helps to mitigate a tiny race during
deleting the timer. But given that we have cleared TF_ACTIVE at this point
we already have another mitigation a few lines down the function.

list_for_each_entry_safe only protects against deletions from the list,
but not against any concurrent modifications. Given that we drop
t_state_lock inside the loop it is not safe in transport_free_dev_tasks.
Instead of use a local dispose_list that we move all tasks that are
to be deleted to. This is safe because we never do list_emptry checks
on t_list to check if a command is on the list anywhere.

This patch fixes a bug with the handling of REPORT TARGET PORT GROUPS
containing a smaller allocation length than the payload requires causing
memory writes beyond the end of the buffer. This patch checks for the
minimum 4 byte length for the response payload length, and also checks
upon each loop of T10_ALUA(su_dev)->tg_pt_gps_list to ensure the Target
port group and Target port descriptor list is able to fit into the
remaining allocation length.
If the response payload exceeds the allocation length length, then rd_len
is still increments to indicate to the initiator that the payload has
been truncated.

Instead of calling out to the backends from the core to get a per-task
CDB and then modify it for the LBA/len pair used for this CDB provide
a helper that writes the adjusted CDB into a provided buffer and call
this method from ->do_task in pscsi.

Instead of using a separate flag we can simply do list_emptry checks
on t_execute_list if we make sure to always use list_del_init to remove
a task from the list. Also factor some duplicate code into a new
__transport_remove_task_from_execute_queue helper.
target: remove the read-only task_no field in se_task
The task_no field never was initialized and only used in debug printks,
so kill it.
target: remove the task_padded_sg field in se_task
This field is only check in one place and not actually needed there.
Rationale:
- transport_do_task_sg_chain asserts that we have task_sg_chaining
set early on
- we only make use of the sg_prev_nents field we calculate based on it
if there is another sg list that gets chained onto this one, which
never happens for the last (or only) task.

This patch removes the legacy device active I/O shutdown code that was
originally called from transport_processing_thread() context during shutdown
including transport_processing_shutdown() and transport_release_all_cmds().
This is due to the fact that in modern configfs control plane code by the
time shutdown of an se_device instance in transport_processing_thread()
is allowed to occur via:
rmdir /sys/kernel/config/target/core/$HBA/$DEV
all active I/O will already have been ceased while removing active configfs
fabric Port/LUN symlinks. Eg: the removal of an active se_device is protected
by inter-module VFS references from active Port/LUN symlinks.
Two WARN_ON() checks have been added in their place before exiting
transport_processing_thread() to watch out for any leaked descriptors.

This patch removes a number of SCF_SE_LUN_CMD flag abuses within iscsi-target
code to determine when iscsit_release_cmd() or transport_generic_free_cmd()
should be called while releasing an individual iscsi_cmd descriptor.
In the place of SCF_SE_LUN_CMD checks, this patch converts existing code to
use a new iscsit_free_cmd() that inspects iscsi_cmd->iscsi_opcode types to
determine which of the above functions should be invoked. It also removes the
now unnecessary special case checking in iscsit_release_commands_from_conn().

Testing in_interrupt() to know when sleeping is allowed is not really
reliable (since eg it won't be true if the caller is holding a spinlock).
Instead have the caller tell core_tmr_alloc_req() what GFP_xxx to use;
every caller except tcm_qla2xxx can use GFP_KERNEL.

b) simplifies the code greatly
Note that the code seems to have various error handling issues, mostly

Push session reinstatement out of transport_generic_free_cmd into the only
caller that actually needs it. Clean up transport_generic_free_cmd a bit,
and remove the useless comment. I'd love to add a more useful kerneldoc
comment for it, but as this point I'm still a bit confused in where it
stands in the command release stack.

Inline two simple functions only used by it, and replace a goto
with a simple if else construct.
Note that the code moved from transport_dec_and_check seems fairly
buggy - the atomic_read check on a variable where we'd do an
atomic_dec_and_test looks racy if we'll ever get someone increment
it without the lock held around them (which it looks like we do),
and not decrementing the second counter if the first one doesn't
hit zero also at least needs an explanation.

Instead of duplicating the code from transport_release_fe_cmd re-use it by
allowing transport_release_fe_cmd to return wether it actually freed the
command or not. Also rename transport_release_fe_cmd to transport_put_cmd
and add a kerneldoc comment for it to make the use case more obvious.

This patch fixes a bug where transport_send_task_abort() could be called
during LUN_RESET to return SAM_STAT_TASK_ABORTED + tfo->queue_status(), when
SCF_SENT_CHECK_CONDITION -> tfo->queue_status() has already been sent from
within another context via transport_send_check_condition_and_sense().

This patch fixes a bug in LUN_RESET operation with transport_cmd_finish_abort()
where transport_remove_cmd_from_queue() was incorrectly being called, causing
descriptors with t_state == TRANSPORT_FREE_CMD_INTR to be incorrectly removed
from qobj->qobj_list during process context release. This change ensures the
descriptor is only removed via transport_remove_cmd_from_queue() when doing a
direct release via transport_generic_remove().

This patch contains a bugfix for TMR LUN_RESET related to TRANSPORT_FREE_CMD_INTR
operation, where core_tmr_drain_cmd_list() will now skip processing for this
case to prevent an ABORT_TASK status from being returned for descriptors that
are already queued up to be released by processing thread context.

This patch is a re-orginzation of core_tmr_lun_reset() logic to properly
scan the active tmr_list, dev->state_task_list and qobj->qobj_list w/ the
relivent locks held, and performing a list_move_tail onto seperate local
scope lists before performing the full drain.
This involves breaking out the code into three seperate list specific
functions: core_tmr_drain_tmr_list(), core_tmr_drain_task_list() and
core_tmr_drain_cmd_list().

This patch addresses a bug with the lio-core-2.6.git conversion of
transport_add_cmd_to_queue() to use a single embedded list_head, instead
of individual struct se_queue_req allocations allowing a single se_cmd to

The problem is that some target code still assumes performing multiple
adds is allowed via transport_add_cmd_to_queue(), which ends up causing
list corruption in qobj->qobj_list code. This patch addresses this
by removing an existing struct se_cmd from the list before the add, and
removes an unnecessary list walk in transport_remove_cmd_from_queue()
It also changes cmd->t_transport_queue_active to use explict sets intead
of increment/decrement to prevent confusion during exception path handling.

This patch fixes a bug in the iscsit_fe_sendpage_sg() transmit codepath that
was originally introduced with the v3.1 iscsi-target merge that incorrectly
uses hardcoded cmd->iov_data_count values to determine cmd->iov_data[] offsets
for extra outgoing padding and DataDigest payload vectors.
This code is obviously incorrect for the DataDigest enabled case with sendpage
offload, and this fix ensures correct operation for padding + DataDigest,
padding only, and DataDigest only cases. The bug was introduced during a
pre-merge change in iscsit_fe_sendpage_sg() to natively use struct scatterlist
instead of the legacy v3.0 struct se_mem logic.

This patch makes iscsi-target explictly disable OFMarker=Yes and IFMarker=yes
parameter key usage during iscsi login by setting IFMarkInt_Reject and
OFMarkInt_Reject values in iscsi_enforce_integrity_rules() to effectively
disable iscsi marker usage. With this patch, an initiator proposer asking
to enable either marker parameter keys will be issued a 'No' response, and
the target sets OFMarkInt + IFMarkInt parameter key response to 'Irrelevant'.
With markers disabled during iscsi login, this patch removes the problematic
on-stack local-scope array for marker intervals in iscsit_do_rx_data() +
iscsit_do_tx_data(), and other related marker code in iscsi_target_util.c.
This fixes a potentional stack smashing scenario with small range markers
enabled and a large MRDSL as reported by DanC here:

This patch adds target_parse_naa_6h_vendor_specific() to address a bug where the
conversion of PRODUCT SERIAL NUMBER to use hex2bin() in target_emulate_evpd_83()
was not doing proper isxdigit() checking. This conversion of the vpd_unit_serial
configifs attribute is done while generating a VPD=0x83 NAA IEEE Registered
Extended DESIGNATOR format's 100 bits of unique VENDOR SPECIFIC IDENTIFIER +
VENDOR SPECIFIC IDENTIFIER EXTENSION area.
This patch allows vpd_unit_serial (VPD=0x80) and the T10 Vendor ID DESIGNATOR
format (VPD=0x83) to continue to use free-form variable length ASCII values,
and now skips any non hex characters for fixed length NAA IEEE Registered Extended
DESIGNATOR format (VPD=0x83) requring the binary conversion.
This was originally reported by Martin after the v3.1-rc1 change to use hex2bin()
in commit 11650b8596 where the use of non hex
characters in vpd_unit_serial generated different values than the original
v3.0 internal hex -> binary code. This v3.1 change caused a problem with
filesystems who write a NAA DESIGNATOR onto it's ondisk metadata, and this patch
will (again) change existing values to ensure that non hex characters are not
included in the fixed length NAA DESIGNATOR.
Note this patch still expects vpd_unit_serial to be set via existing userspace
methods of uuid generation, and does not do strict formatting via configfs input.

Problem: Changed from wake_up_interruptible -> wake_up_process and
wait_event_interruptible-> schedule_timeout_interruptible broke the FCoE
target. Earlier approach of wake_up_interruptible was also looking at
'queue_cnt' which is not necessary, because it increment of 'queue_cnt'
with wake_up_inetrriptible / waker_up_process introduces race condition.
Fix: Instead of fixing the code which used wake_up_process and remove
'queue_cnt', using work_queue based approach is cleaner and acheives
same result. As well, work queue based approach has less programming
overhead and OS manages threads which processes work queues.

Problem: HW DDP context wasn;t invalidated in case of ABORTS, etc...
This leads to the problem where memory pages which are used for DDP
as user descriptor could get reused for some other purpose (such as to
satisfy new memory allocation request either by kernel or user mode threads)
and since HW DDP context was not invalidated, HW continue to write to
those pages, hence causing memory corruption.
Fix: Either on incoming ABORTS or due to exchange time out, allowed the
target to cleanup HW DDP context if it was setup for respective ft_cmd.
Added new function to perform this cleanup, furthur it can be enhanced
for other cleanup activity.
Additinal Notes: To avoid calling ddp_done from multiple places, composed
the functionality in helper function "ft_invl_hw_context" and it is being
called from multiple places. Cleaned up code in function "ft_recv_write_data"
w.r.t DDP.

When work is scheduled with schedule_work(), the work can end up
running on multiple CPUs at the same time -- this happens if
the work is already running on one CPU and schedule_work() is called
on another CPU. This leads to list corruption with target_qf_do_work(),
which is roughly doing:
spin_lock(...);
list_for_each_entry_safe(...) {
list_del(...);
spin_unlock(...);
// do stuff
spin_lock(...);
}
With multiple CPUs running this code, one CPU can end up deleting the
list entry that the other CPU is about to work on.
Fix this by splicing the list entries onto a local list and then
operating on that in the work function. This way, each invocation of
target_qf_do_work() operates on its own local list and so multiple
invocations don't corrupt each other's list. This also avoids dropping
and reacquiring the lock for each list entry.

which is taken in hardirq context. This means acl_node_lock must become

At least the tcm_qla2xxx fabric driver calls into transport_deregister_session()
while holding an IRQ-disabled spinlock, so the inner locking needs to
use spin_lock_irqsave() instead of spin_lock_bh().

This patch fixes two bugs in allocation failure handling in
iscsit_allocate_se_cmd_for_tmr():
This first reported by DanC is a free-after call to transport_free_se_cmd(), this
patch drops the transport_free_se_cmd() call all together, as iscsit_release_cmd()
will release existing allocations as expected.
The second is a bug where iscsi_cmd_t was being leaked on a cmd->tmr_req allocation
failure, so make this jump to iscsit_release_cmd() as well.

This patch fixes two bugs associated with transport_do_task_sg_chain()
operation where transport_allocate_data_tasks() was incorrectly setting
task_padded_sg for all tasks, and causing bogus task->task_sg_nents
assignments + OOPsen with fabrics depending upon this code. The first bit
here adds a task_sg_nents_padded check in transport_allocate_data_tasks()
to include an extra SGL vector when necessary for tasks that expect to
be linked using sg_chain().
The second change involves making transport_do_task_sg_chain() properly
account for the extra SGL vector when task->task_padded_sg is set for
the non trailing ->task_sg or single ->task_sg allocations. Note this
patch also removes the BUG_ON(!task->task_padded_sg) check within
transport_do_task_sg_chain() as we expect this to happen normally
with the updated logic in transport_allocate_data_tasks(), along with
being bogus for CONTROL_SG_IO_CDB type payloads.
So far this bugfix has been tested with tcm_qla2xxx and iblock backends
in (task_count > 1)( and (task_count == 1) operation.

This patch addresses recent breakage with multiple se_task (task_count > 1)
operation following backend dev->se_sub_dev->se_dev_attrib.max_sectors in new
transport_allocate_data_tasks() code. The initial bug here was a bogus
task->task_sg_nents assignment in transport_allocate_data_tasks() based on
the passed parameter, which now uses DIV_ROUND_UP(task_size, PAGE_SIZE) to
determine the proper number of per task SGL entries for the (task_count > 1)
case.
This also means we now need to enforce a PAGE_SIZE aligned max_sector count
value for this to work as expected without bringing back the pre v3.1
transport_map_mem_to_sg() logic to handle SGL offsets across multiple tasks.
So this patch adds se_dev_align_max_sectors() to round down max_sectors as
necessary to ensure this alignment via se_dev_set_default_attribs() and
se_dev_align_max_sectors() and keeps it simple for (task_count > 1)
operation.
So far this bugfix has been tested with (task_count > 1) operation
using iscsi-target and iblock backends.

This patch adds the missing transport_cmd_get_valid_sectors() check for
SCF_SCSI_DATA_SG_IO_CDB type payloads to ensure that a received LBA + range
does not exeed past the end of associated backend struct se_device.
This patch also fixes a bug in the failure path of transport_new_cmd_obj()
where this check can fail, so change to use a signed 'rc' and return '-EINVAL'
to signal proper transport_generic_request_failure() handling.

This patch fixes a SYNCHRONIZE_CACHE CDB handling bug with IBLOCK/FILEIO
backends where transport_cmd_get_valid_sectors() was incorrectly rejecting
a zero LBA + range CDB from being processed, and returning CHECK_CONDITION.
This includes changing transport_cmd_get_valid_sectors() to return '0' on
success and '-EINVAL' on failure (this makes more sense than sectors),
and to only check transport_cmd_get_valid_sectors() when a non zero LBA +
range SYNCHRONIZE_CACHE operation has been receieved for the non passthrough
case.

This patch removes a duplicate set of transport_complete_task() calls in
target_emulate_unmap() and target_emulate_write_same() as the completion
call is already done within transport_emulate_control_cdb()
This patch also adds a check in transport_emulate_control_cdb() for the
existing SCF_EMULATE_CDB_ASYNC flag currently used by SYNCHRONIZE_CACHE
in order to handle IMMEDIATE processing.

For all flavours of WRITE_SAME, we only expect to handle a single block
of data-out buffer payload, regardless of the number of logical blocks
presented in the CDB. This patch changes all flavours of WRITE_SAME in
transport_generic_cmd_sequencer() to pass '1' into transport_get_size()
instead of the extracted 'sectors' to properly handle the default usage
of sg_write_same without the --xferlen parameter.

This patch adds initial WRITE_SAME (10) w/ UNMAP=1 support following updates in
sbcr26 to allow UNMAP=1 for the non 16 + 32 byte CDB case. It also refactors
current pSCSI passthrough passthrough checks into target_check_write_same_discard()
ahead of UNMAP=0 w/ write payload support into target_core_iblock.c.
This includes the support for handling WRITE_SAME in transport_emulate_control_cdb(),
and converts target_emulate_write_same to accept num_blocks directly for
WRITE_SAME, WRITE_SAME_16 and WRITE_SAME_32.

This patch fixes a bug for fabrics using tfo->new_cmd_map() that

We leak memory if the allocations for 'new_param->name' or
'new_param->value' fail in iscsi_target_parameters.c::iscsi_copy_param_list()
We also do a lot of variable assignments that are completely pointless
if the allocations fail.
So, let's move the allocations before the assignments and also make
sure that we free whatever was allocated to one if the allocation fail.
There's also some small CodingStyle fixups in there (curly braces on
both branches of if statement, only one variable per line) since I was
in the area anyway. And finally, error messages in the function are
put on a single line for easy grep'abillity.

This patch changes target_emulate_inquiry_std() to set the 'not connected'
(0x35) bit in standard INQUIRY response data when we are processing a
request to a virtual LUN=0 mapping from struct se_device *g_lun0_dev that
have been setup for us in transport_lookup_cmd_lun().
This addresses an issue where qla2xxx FC clients need to be able
to create demo-mode I_T FC Nexuses by default, but should not be
exposing the default set of TPG LUNs to all FC clients. This includes
adding an new optional target_core_fabric_ops->tpg_check_demo_mode_login_only()
caller to allow demo_mode nexuses to skip the old default of bulding
a demo-mode MappedLUNs list via core_tpg_add_node_to_devs().

This patch changes transport_send_check_condition_and_sense() for
TCM_NON_EXISTENT_LUN emulation to use 0x25 (LOGICAL UNIT NOT SUPPORTED)
instead of the original 0x20 (INVALID COMMAND OPERATION CODE). This is
helpful to distinguish between TCM_UNSUPPORTED_SCSI_OPCODE ASC=0x20
exceptions.

Problem: HW DDP context was not invalidated in case of ABORTS, etc...
This leads to the problem where memory pages which are used for DDP
as user descriptor could get reused for some other purpose (such as to
satisfy new memory allocation request either by kernel or user mode threads)
and since HW DDP context was not invalidated, HW continue to write to
those pages, hence causing memory corruption.
Fix: Either on incoming ABORTS or due to exchange time out, allowed the
target to cleanup HW DDP context if it was setup for respective ft_cmd.
Added new function to perform this cleanup, furthur it can be enhanced
for other cleanup activity. Fix ft_recv_write_data() to properly handle
fc_frame_payload_get to return pointer to payload if it exist. If there is
no payload which is most common case (+ve case in case if DDP is working
as expected, it will return NULL. Yes, scope of buf is limited to printk.
Invalidation of HW context (which is done inside ft_invl_hw_context() is
necessary in SUCCESS and FAILURE case of DDP. Hence invalidation is DONE
as long as there was DDP setup (whether it worked correctly or not,
NOTE: For some reason, if there is any error w.r.t DDP such as out of
order packet reception, HW simply post the full packet in rx queue.

The Linux-iSCSI.org target module is a full featured in-kernel
software implementation of iSCSI target mode (RFC-3720) for the
current WIP mainline target v4.1 infrastructure code for the v3.1

This patch adds the default 'Unrestricted reordering allowed' for SCSI
control mode page QUEUE ALGORITHM MODIFIER on a per se_device basis in
target_modesense_control() following spc4r23. This includes a new
emuluate_rest_reord configfs attribute that currently (only) accepts
zero to signal 'Unrestricted reordering allowed' in control mode page
usage by the backend target device.

This patch breaks up the ->map_task_SG() backend call into two seperate
->map_control_SG() and ->map_data_SG() in order to better address
IBLOCK and pSCSI. IBLOCK only allocates bios for ->map_data_SG(), and
pSCSI will allocate a struct request for both cases.
This patch fixes incorrect usage of ->map_task_SG() for all se_cmd descriptors
in transport_generic_new_cmd() by moving the call into it's proper location
directly inside of transport_allocate_data_tasks()

from Andy and Christoph following the post heavy lifting in the preceeding:

changes. This also includes a conversion of target core and the v3.0

Both backstores and fabrics use arrays of struct scatterlist to describe
data buffers. However TCM used struct se_mems, basically a linked list
of scatterlist entries. We are able to simplify the code by eliminating
this intermediate data structure and just using struct scatterlist[]
throughout.
Also, moved attachment of task to cmd out of transport_generic_get_task
and into allocate_control_task and allocate_data_tasks. The reasoning
is that it's nonintuitive that get_task should automatically add it to
the cmd's task list -- it should just return an allocated, initialized
task. That's all it should do, based on the function's name, so either the
function shouldn't do it, or the name should change to encapsulate the
entire essence of what it does.

Due to all cdbs' data buffers being referenced by scatterlists, buffers
of more than a page are not contiguous. Instead of handling this in all
control command handlers, we may be able to get away with just limiting
control cdb data buffers to one page. The only control CDBs we handle that
have potentially large data buffers are REPORT LUNS and UNMAP, so if we
didn't want to live with this limitation, they would need to be modified
to walk the pages in the data buffer's sgl.

Previously, some control CDBs did not allocate memory in pages for their
data buffer, but just did a kmalloc. This patch makes all cdbs allocate
pages.
This has the benefit of streamlining some paths that had to behave
differently when we used two allocation methods. The downside is that
all accesses to the data buffer need to kmap it before use, and need to
handle data in page-sized chunks if more than a page is needed for a given
command's data buffer.
Finally, note that cdbs with no data buffers are handled a little
differently. Before, SCSI_NON_DATA_CDBs would not call get_mem at all
(they'd be in the final else in transport_allocate_resources) but now
these will make it into generic_get_mem, but just not allocate any
buffers.

Implement page B1h, Block Device Characteristics, so that we can report
a medium rotation rate of 1 (non-rotating / solid state) if the
is_nonrot device attribute is set; we update the iblock backend to set
this attribute if the underlying Linux block device has its nonrot
flag set.

The current handling of VPD page 00h (Supported VPD Pages) for INQUIRY
commands has a couple of problems:
- The page length field is incorrectly set to 3, so the entry for 86h
(Extended INQUIRY Data) is ignored since it is in the fourth slot.
- Even though the code handles pages B0h and B2h, those pages aren't
mentioned in the Supported VPD Pages list, so eg the Linux SCSI stack
won't actually try to use them.
Fix these problems and make things more robust to avoid future problems
by moving to a table of supported VPD pages, which means that any added
VPD page support will automatically get reported on page 0.

In target_fabric_configfs_init(), we should allow fabric_mod to be NULL,
since THIS_MODULE is NULL for built-in modules. The main method of
using the target code may be as modules, but having everything built-in
is useful eg to be able to do quick testing with "qemu -kernel".
In any case, we shouldn't bomb out fabric registration for a perfectly
valid configuration, so simply drop the check of fabric_mod.

This patch converts ft_queue_cmd() to use wake_up_process() and
ft_thread() to use schedule_timeout_interruptible(MAX_SCHEDULE_TIMEOUT)
instead of wait_event_interruptible(). This fixes a potential race with
the wait_event_interruptible() conditional with qobj->queue_cnt in
ft_thread().
This patch also drops the unnecessary set_user_nice(current, -20) in
ft_thread(), and drops extra () around two if (!(acl)) conditionals in
tfc_conf.c.

There is a memory leak in tcm_loop_make_scsi_hba().
If all the strstr() calls return NULL and we end up at return ERR_PTR(-EINVAL);
then we'll be leaking the memory previously allocated to tl_hba as
that variable goes out of scope.
This patch should fix the leak.

transport_lookup_tmr_lun() can be called from interrupt context and
therefore needs to use IRQ-safe spinlock functions. Fix this up, and
to make the locking work, convert the other uses of se_tmr_lock to be
IRQ-disabling.

se_dev_check_online() is called from transport_lookup_cmd_lun(), which
as discussed before may be called from interrupt context. So it needs
to use spin_lock_irqsave() instead of spin_lock_irq() to avoid
enabling interrupts at the wrong time.

transport_lookup_cmd_lun() may be called from interrupt context (eg
tcm_loop_allocate_core_cmd() calls it, and it has a comment that says,
"Can be called from interrupt context"), so it needs to use
spin_lock_irqsave() instead of spin_lock_irq() to avoid enabling
interrupts at the wrong time.
(And indeed the last set of lock operations, on lun_cmd_lock, were
already using spin_lock_irqsave(), so we just need to fix the other
two locks we take)

This patch adds SCF_EMULATE_QUEUE_FULL support using -EAGAIN failures
via transport_handle_queue_full() to signal queue full in completion
path TFO->queue_data_in() and TFO->queue_status() callbacks.
This is done using a new se_cmd->transport_qf_callback() to handle
the following queue full exception cases within target core:
*) TRANSPORT_COMPLETE_OK (for completion path queue full)
*) TRANSPORT_COMPLETE_QF_WP (for TRANSPORT_WRITE_PENDING queue full)
*) transport_send_check_condition_and_sense() failure paths in
transport_generic_request_failure() and transport_generic_complete_ok()
All logic is driven using se_device->qf_work_queue -> target_qf_do_work()
to to requeue outstanding se_cmd at the head of se_dev->queue_obj->qobj_list
for transport_processing_thread() execution.
Tested using tcm_qla2xxx with MAX_OUTSTANDING_COMMANDS=128 for FCP READ
to trigger the TRANSPORT_COMPLETE_OK queue full cases, and a simulated
TFO->write_pending() -EAGAIN failure to trigger TRANSPORT_COMPLETE_QF_WP.

This patch adds a transport_handle_cdb_direct() optimization for mapping
and queueing tasks directly from within fabric processing context by calling
the newly exported transport_generic_new_cmd(). This currently expects to
be called from process context only, and will fail if called within interrupt
context.
This patch also leaves transport_generic_handle_cdb() unmodified for the
moment to function as expected with existing tcm_fc and ib_srpt fabrics,
and will be removed once these have been converted and tested with v4.1
code using transport_handle_cdb_direct().

This patch fixes the handling of WRITE_SAME_[16,32] emulation where a
WRITE_SAME_* CDB with number of blocks=0 was being rejected by SCSI
expected data transfer length overflow checking in target core.
It changes both CDB cases in transport_generic_cmd_sequencer() to use
dev->se_sub_dev->se_dev_attrib.block_size to match what sg_write_same
is sending us with --num=0. It also fixes target_emulate_write_same()
to properly determine the num_blocks with --num=0 case to determine the
remaining range for dev->transport->do_discard().

Problem: After fixing the issue in TCM core w.r.t LUN Reset (Task
Management request) , ran into issue where during the completing of
this LUN Reset command, reference count of "ft_sess" drops to zero
which caused "sess" to be deleted.
Fix: As part of handling task management request (e.g. LUN Reset), TCM
core function "transport_generic_do_tmr" ends up calling ft_free_cmd
which in turn calls "ft_sess_put" (which drops session's reference
count by 1) and then frees ft_cmd. Then function
"transport_generic_do_tmr" calls "transport_cmd_check_stop" which in
turn also calls ft_free_cmd (which calls ft_sess_put - which drops
reference count of sess by 1, hence reference count of sess becomes
zero and session gets deleted). Fix is to just send response in case
of tmr from function "ft_queue_resp_code" and not delete "ft_cmd"
(means don't call ft_free_cmd). Earlier code was to send the response
code and also free ft_cmd. ft_free_cmd will be freed later after
sending response code as a result of "transport_cmd_check_stop" (which
calls ft_release_cmd -> ft_free_cmd) being called from
"transport_generic_do_tmr" after sening TMR response code.
Notes/Dependencies: This bug was found after fixing NULL pointer
access issue in TCM core (in LUN Reset codepath)

The fcoe driver can implement ddp_targ() similarly to ddp_setup() when fcoe
stack works with existing target frame, e.g., tcm, where the ddp_targ() would
eventually point to the underlying hardware driver's implementation of
ndo_fcoe_ddp_targ() through net_device_ops. This new API sets up DDP context
for target appropriately by setting required bits for DDP context.

This patch fixes a bug in ft_send_tm() that was incorrectly calling
ft_get_lun_for_cmd() -> transport_get_lun_for_cmd(), instead of using
transport_get_lun_for_tmr() for the proper struct se_lun lookup
that was triggering an OOPs in the se_cmd->tmr_req failure path.
This patch fixes the issue by re-arranging the codepath where
transport_get_lun_for_tmr() is called after tmr request is allocated and
made it available as part of se_cmd.
It also drops the now unnecessary ft_get_lun_for_cmd() unpacking code, and
uses scsilun_to_int() directly ahead of transport_get_lun_for_cmd() and
transport_get_lun_for_tmr() usage.

This patch fixes a number of cases in target core using an incorrectly
if (strlen(foo) > SOME_MAX_SIZE)
As strlen() returns the number of characters in the string not counting
the NULL character at the end. So if you do something like:
char buf[10];
if (strlen("0123456789") > 10)
return -ETOOLONG;
snprintf(buf, 10, "0123456789");
printf("%s\n", buf);
then the last "9" gets chopped off and only "012345678" is printed.
Plus I threw in one small related cleanup.

In the original code, there were several places inside the
target_fabric_configfs_init() function that returned NULL on error
and one place the returned an ERR_PTR. There are two places that
call this function and they only check for NULL returns; they don't
check for ERR_PTRs. So I've changed the ERR_PTR so now the function
only returns NULL on error.

This patch fixes a bug where task->task_execute_queue=1 was not being
cleared once se_task had been removed from se_device->execute_task_list,
resulting in an OOPs in core_tmr_lun_reset() for the task->task_active=0
case where transport_remove_task_from_execute_queue() was incorrectly
being called.
This patch fixes two cases in transport_get_task_from_execute_queue()
and transport_remove_task_from_execute_queue() to properly clear
task->task_execute_queue=0 once list_del(&task->t_execute_list) has
been called.
It also adds an explict check in transport_remove_task_from_execute_queue()
to dump_stack + return if called with task->task_execute_queue=0.

This patch addresses a bug in the target core release path for HW
operation where transport_free_dev_tasks() was incorrectly being called
from transport_lun_remove_cmd() while releasing a se_cmd reference and
calling struct target_core_fabric_ops->queue_data_in().
This would result in a OOPs with HW target mode when the release of
se_task->task_sg[] would happen before pci_unmap_sg() can be called in
HW target mode fabric module code. This patch addresses the issue by
moving transport_free_dev_tasks() from transport_lun_remove_cmd() into
transport_generic_free_cmd(), and adding TRANSPORT_FREE_CMD_INTR and
transport_generic_free_cmd_intr() to allow se_cmd descriptor release
to happen fromfrom within transport_processing_thread() process context
when release of se_cmd is not possible from HW interrupt context.

This patch fixes two bugs wrt to the interrupt context usage of target
core with HW target mode drivers. It first converts the usage of struct
se_device->stats_lock in transport_get_lun_for_cmd() and core_tmr_lun_reset()
to properly use spin_lock_irq() to address an BUG with CONFIG_LOCKDEP_SUPPORT=y
enabled.
This patch also adds a 'in_interrupt()' check to allow GFP_ATOMIC usage from
core_tmr_alloc_req() to fix a 'sleeping in interrupt context' BUG with HW
target fabrics that require this logic to function.

This patch fixes a bug in transport_do_task_sg_chain() used by HW target
mode modules with sg_chain() to provide a single sg_next() walkable memory
layout for use with pci_map_sg() and friends. This patch addresses an
issue with mapping multiple small block max_sector tasks across multiple
struct se_task->task_sg[] mappings for HW target mode operation.
This was causing OOPs with (cmd->t_task->t_tasks_no > 1) I/O traffic for
HW target drivers using transport_do_task_sg_chain(), and has been tested
so far with tcm_fc(openfcoe), tcm_qla2xxx, and ib_srpt fabrics with
t_tasks_no > 1 IBLOCK backends using a smaller max_sectors to trigger the
original issue.

This is a comprehensive patch for FC-FC4 provider. tcm_fc is a FC-FC4
provider which glues target core (TCM) with Fiber channel library
(libfc). tcm_fc uses existing FC4 provider hooks from Fiber channel
library. This Fiber channel library is used by FCoE (transport - FC
over Ethernet) protocol driver as well.
Combination of modules such as Fiber channel library, tcm_fc, TCM
target core, and FCoE protocol driver enables functional FCoE target.
This patch includes initial commit for tcm_fc plus additional
enhancement, bug fixes.
This tcm_fc module essentially contains 3 entry points such as "prli",
"prlo", "recv". When process login request (ELS_PRLI) request is
received, Fiber channel library (libfc) module calls passive providers
(FC-FC4, tcm_fc) (if any registered) "prli" function. Likewise when
LOGO request is received, "prlo" function of passive provider is
invoked by libfc. For all other request (e.g. any read/write, task
management, LUN inquiry commands), "recv" function of passiver
provider is invoked by libfc. Those passive providers "prli, prlo,
recv" functions interact with TCM target core for requested operation.

Fix up conflicts in fs/{aio.c,super.c}
This patch adds the TCM_Loop Linux/SCSI LLD fabric module for
accessing TCM device backstores as locally accessable SCSI LUNs in
virtual SAS, FC, and iSCSI Target ports using the generic fabric
TransportID and Target Port WWN naming handlers from TCM's
target_core_fabric_lib.c The TCM_Loop module uses the generic fabric
configfs infratructure provided by target_core_fabric_configfs.c and
adds a module dependent attribute for the creation/release of the
virtual I_T Nexus connected the TCM_Loop Target and Initiator Ports.
TCM_Loop can also be used with scsi-generic and BSG drivers so that
STGT userspace fabric modules, QEMU-KVM and other hypervisor SCSI
passthrough support can access TCM device backstore and control CDB
emulation.

This patch fixes the follownig section mismatch warning:
WARNING: vmlinux.o(.text+0x21617a): Section mismatch in reference from the
function target_core_init_configfs() to the function .init.text:rd_module_init()
The function target_core_init_configfs() references the function __init
rd_module_init(). This is often because target_core_init_configfs() lacks
a __init annotation or the annotation of rd_module_init is wrong.

This patch removes a bogus conditional+return check within the failure path
of transport_add_device_to_core_hba(). This breakage was introduced during
the v4 conversion to remove struct se_cmd passthrough ops for INQUIRY /
READ_CAPCITY during struct se_device creation and registration process.

In drivers/target/target_core_transport.c::transport_generic_get_mem()
there are a few potential memory leaks in the error paths. This patch
makes sure that we free previously allocated memory when other allocations
fail. It also moves some work (INIT_LIST_HEAD() and assignment to
se_mem->se_len) below all the allocations so that if something fails we
don't do the work at all.

This patch addresses the majority of sparse warnings and adds
proper locking annotations. It also fixes the dubious one-bit signed
bitfield, for which the signed one-bit types can be 0 or -1 which can
cause a problem if someone ever checks if (foo->lu_gp_assoc == 1).
The current code is fine because everyone just checks zero vs non-zero.
But Sparse complains about it so lets change it. The warnings look like
this:

This patch removes a legacy struct se_hba->hba_dev_list -> se_release_device_for_hba()
list walk in core_delete_hba(), which is no longer required while using configfs
VFS level parent/child struct config_group dependency referencing. The reason
is because any struct se_hba->hba_dev_list-> struct se_device members are going
to have to be released via:
rmdir /sys/kernel/config/target/core/$HBA/*
before rmdir release of struct se_hba via target_core_configfs.c:
target_core_call_delhbafromtarget() -> core_delete_hba()
rmdir /sys/kernel/config/target/core/$HBA
to release struct se_hba in core_delete_hba().
This patch also removes the legacy se_clear_dev_ports() function, which is
left-over pre-configfs shutdown logic for when se_free_virtual_device()
was responsible for walking struct se_device->dev_sep_list and calling
core_dev_del_lun() for each individual active struct se_port->se_lun.
The reason this can be removed is because all struct se_device->dev_sep_list
-> struct se_port communication is done via configfs symlinks, which
means that an target fabric module's endpoints containg active struct
se_port(s) will have to be released via target_core_fabric_configfs.c:
target_fabric_port_unlink() via:
unlink /sys/kernel/config/target/$FABRIC_MOD/$ENDPOINT/tpgt_$TPGT/lun/lun_$LUN_ID/<symlink>
before rmdir release of struct se_device in target_core_configfs.c:
target_core_drop_subdev() -> se_free_virtual_device() can happen via:

This patch converts the target_core_store_dev_enable() -> struct
se_subsystem_api->create_virtdevice() call to return proper ERR_PTR values
back up to configfs logic during backend dependent struct se_device ENABLE
exception conditions.
Along with the change to target_core_configfs.c, this includes converting IBLOCK,
FILEIO, pSCSI, and RAMDISK_* backend subsystem plugins to obtain upper level
PTR_ERR return codes (where available), and return via ERR_PTR during a
*_create_virtdev() failure.

match_strdup() dynamically allocates memory and it is the responsabillity
of the caller to free that memory. The following three cases:
drivers/target/target_core_file.c:fd_set_configfs_dev_params()
drivers/target/target_core_iblock.c:iblock_set_configfs_dev_params()
drivers/target/target_core_configfs.c:target_core_dev_pr_store_attr_res_aptpl_metadata()
should be kfree()'ing the allocated memory once it is no longer needed.
It also makes sure to return -ENOMEM if the memory allocation in match_strdup()
should fail. For target_core_configfs.c, this patch adds kfree()'s around
Opt_initiator_fabric, Opt_initiator_node, Opt_initiator_sid, Opt_sa_res_key,
Opt_target_fabric, and Opt_target_node for the Persistent Reservations
Activate Persistence across Target Power Loss (APTPL=1) token parsing.

the target infrastructure fails to send the correct conventional size
to READ_CAPACITY that force a retry with READ_CAPACITY_16, which reads

This patch addresses two outstanding bugs related to
T_TASK(cmd)->t_transport_aborted handling during TMR LUN_RESET and
active I/O shutdown.
This first involves adding two explict t_transport_aborted=1
assignments in core_tmr_lun_reset() in order to signal the task has
been aborted, and updating transport_generic_wait_for_tasks() to skip
sleeping when t_transport_aborted=1 has been set. This fixes an issue
where transport_generic_wait_for_tasks() would end up sleeping
indefinately when called from fabric module context while TMR
LUN_RESET was happening with long outstanding backend struct se_task
not yet being completed.
The second adds a missing call to
transport_remove_task_from_execute_queue() when
task->task_execute_queue=1 is set in order to fix an OOPs when
task->t_execute_list has not been dropped. It also fixes the same
case in transport_processing_shutdown() to prevent the issue from
happening during active I/O struct se_device shutdown.

This patch moves a large number of memory release paths inside of the
configfs callback target_core_hba_item_ops->release() called from
within fs/configfs/item.c: config_item_cleanup() context. This patch
resolves the SLUB 'Poison overwritten' warnings.

This patch removes the legacy procfs based target_core_mib.c code,
and moves the necessary scsi_index_tables functions and defines into
target_core_transport.c and target_core_base.h code to allow existing
fabric independent statistics to function.
This includes the removal of a handful of 'atomic_t mib_ref_count'
counters used in struct se_node_acl, se_session and se_hba to prevent
removal while using seq_list procfs walking logic.

This patch fixes a bug introduced during the v4 control CDB emulation
refactoring that broke SCF_SCSI_CONTROL_SG_IO_CDB operation within
transport_map_control_cmd_to_task(). It moves the BUG_ON() into
transport_do_se_mem_map() after the TRANSPORT(dev)->do_se_mem_map()
RAMDISK_DR special case, and adds the proper struct se_mem assignment
when !list_empty() for normal non RAMDISK_DR backend device cases.

This patch fixes two bugs uncovered during testing with
slub_debug=FPUZ during module_exit() -> target_core_exit_configfs()
with release of configfs subsystem consumer default groups, namely how
this should be working with
fs/configfs/dir.c:configfs_unregister_subsystem() release logic for
struct config_group->default_group.
The first issue involves configfs_unregister_subsystem() expecting to
walk+drain the top-level subsys->su_group.default_groups directly in
unlink_group(), and not directly from the configfs subsystem consumer
for the top level struct config_group->default_groups. This patch
drops the walk+drain of subsys->su_group.default_groups from TCM
configfs subsystem consumer code, and moves the top-level
->default_groups kfree() after configfs_unregister_subsystem() has
been called.
The second issue involves calling
core_alua_free_lu_gp(se_global->default_lu_gp) to release the
default_lu_gp->lu_gp_group before configfs_unregister_subsystem() has
been called. This patches also moves the core_alua_free_lu_gp() call
to release default_lu_group->lu_gp_group after the subsys has been
unregistered.
Finally, this patch explictly clears the
[lu_gp,alua,hba]_cg->default_groups pointers after kfree() to ensure
that no stale memory is picked up from child struct
config_group->default_group[] while configfs_unregister_subsystem() is
called.
