Michael Albaugh afd9970f95 IB/ipath: Allow old and new diagnostic packet formats

Ralph Campbell c4b4d16e09 IB/ipath: Make send buffers available for kernel if not allocated to user
A fixed partitioning of send buffers is determined at driver load time
for user processes and kernel use.  Since send buffers are a scarce
resource, it makes sense to allow the kernel to use the buffers if they
are not in use by a user process.

Also, eliminate code duplication for ipath_force_pio_avail_update().

John Gregor 58411d1c01 IB/ipath: Head of Line blocking vs forward progress of user apps
There's a conflict between our need to quiesce PSM-based applications
to avoid HoL blocking when the IB link goes down and the apps' desire
to remain running so that their quiescence timout mechanism can keep
running.

The compromise is to STOP the processes for a fixed period of time and
then alternate between CONT and STOP until the link is again active.

If there are poor interactions with subnet manager configuration at a
given site, the interval can be adjusted via a module paramter.

Ralph Campbell 210d6ca3db IB/ipath: Performance optimization for CPU differences
Different processors have different ordering restrictions for write
combining.  By taking advantage of this, we can eliminate some write
barriers when writing to the send buffers.

Alexey Dobriyan 4e950f6f01 Remove fs.h from mm.h
Remove fs.h from mm.h. For this,
 1) Uninline vma_wants_writenotify(). It's pretty huge anyway.
 2) Add back fs.h or less bloated headers (err.h) to files that need it.

As result, on x86_64 allyesconfig, fs.h dependencies cut down from 3929 files
rebuilt down to 3444 (-12.3%).

Cross-compile tested without regressions on my two usual configs and (sigh):

Michael Albaugh e8e7ad7115 IB/ipath: Add capability to modify PBC word
During compliance testing and when debugging some interconnect issues,
it is very useful to be able to send malformed packets, without having
the device signal them as malformed (drop, or terminate with EBP). The
hardware supports this, but the driver "diagnostic packet" interface
did not.

Extend capability to send specific malformed packets for testing.

Bryan O'Sullivan 490462c268 IB/ipath: Prevent random program use of diags interface
To prevent random utility reads and writes of the diag interface to the
chip, we first require a handshake of reading from offset 0 and writing
to offset 0 before any other reads or writes can be done through the
diags device.   Otherwise chip errors can be triggered.

Arjan van de Ven 2b8693c061 [PATCH] mark struct file_operations const 3
Many struct file_operations in the kernel can be "const".  Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data.  In addition it'll catch accidental writes at compile time to
these shared resources.

Robert Walsh 6ef93dddfe IB/ipath: Initialize diagpkt file on device init only
Don't attempt to set up the diagpkt device in the module init code.
Instead, wait until a piece of hardware is initialized.  Fixes a
problem when loading the ib_ipath module when no InfiniPath hardware
is present: modprobe would go into the D state and stay there.

Bryan O'Sullivan 34b2aafea3 IB/ipath: simplify layering code
A lot of ipath layer code was only called in one place. Now that the
ipath_core and ib_ipath drivers are merged, it's more sensible to simply
inline the simple stuff that the layer code was doing.

Bryan O'Sullivan 27b678dd04 [PATCH] IB/ipath: namespace cleanup: replace ips with ipath
Remove ips namespace from infinipath drivers.  This renames ips_common.h to
ipath_common.h.  Definitions, data structures, etc.  that were not used by
kernel modules have moved to user-only headers.  All names including ips have
been renamed to ipath.  Some names have had an ipath prefix added.

Bryan O'Sullivan a2acb2ff36 [PATCH] IB/ipath: allow diags on any unit
There is no longer a /dev/ipath_diag file; instead, there's
/dev/ipath_diag0, 1, etc.

It's still not possible to have diags run on more than one unit at a time,
but that's easy to fix at some point.

Bryan O'Sullivan 097709fea0 IB/ipath: chip initialisation code, and diag support
ipath_init_chip.c sets up an InfiniPath device for use.

ipath_diag.c permits userspace diagnostic tools to read and write a
chip's registers.  It is different in purpose from the mmap interfaces
to the /sys/bus/pci resource files.
