Joe Perches 28f65c11f2 treewide: Convert uses of struct resource to resource_size(ptr)
Several fixes as well where the +1 was missing.

Done via coccinelle scripts like:

@@
struct resource *ptr;
@@

- ptr->end - ptr->start + 1
+ resource_size(ptr)

and some grep and typing.

Mostly uncompiled, no cross-compilers.

Santosh Shilimkar 6ac77e469e ARM: GIC: Convert GIC library to use the IO relaxed operations
The GIC register accesses today make use of readl()/writel()
which prove to be very expensive when used along with mandatory
barriers. This mandatory barriers also introduces an un-necessary
and expensive l2x0_sync() operation. On Cortex-A9 MP cores, GIC
IO accesses from CPU are direct and doesn't go through L2X0 write
buffer.

A DSB before writel_relaxed() in gic_raise_softirq() is added to be
compliant with the Barrier Litmus document - the mailbox scenario.

Will Deacon 1a01753ed9 ARM: gic: use handle_fasteoi_irq for SPIs
Currently, the gic uses handle_level_irq for handling SPIs (Shared
Peripheral Interrupts), requiring active interrupts to be masked at
the distributor level during IRQ handling.

On a virtualised system, only the CPU interfaces are virtualised in
hardware. Accesses to the distributor must be trapped by the
hypervisor, adding latency to the critical interrupt path in Linux.

This patch modifies the GIC code to use handle_fasteoi_irq for handling
interrupts, which only requires us to signal EOI to the CPU interface
when handling is complete. Cascaded IRQ handling is also updated to use
the chained IRQ enter/exit functions to honour the flow control of the
parent chip.

Note that commit 846afbd1 ("GIC: Dont disable INT in ack callback")
broke cascading interrupts by forgetting to add IRQ masking. This is
no longer an issue because the unmask call is now unnecessary.

Tested on Versatile Express and Realview EB (1176 w/ cascaded GICs).

Santosh Shilimkar d7ed36a4ea ARM: 6777/1: gic: Add hooks for architecture specific extensions
Few architectures combine the GIC with an external interrupt
controller. On such systems it may be necessary to update both
the GIC registers and the external controller's registers to control
IRQ behavior.

This can be addressed in couple of possible methods.
 1. Export common GIC routines along with 'struct irq_chip gic_chip'
and allow architectures to have custom function by override.
 2. Provide architecture specific function pointer hooks
within GIC library and leave platforms to add the necessary
code as part of these hooks.

First one might be non-intrusive but have few shortcomings like arch
needs to have there own custom gic library. Locks used should be
common since it caters to same IRQs etc. Maintenance point of view
also it leads to multiple file fixes.

The second probably is cleaner and portable. It ensures that all the
common GIC infrastructure is not touched and also provides archs to
address their specific issue.

Abhijeet Dharmapurikar 846afbd1fe GIC: Dont disable INT in ack callback
Masking in the ack callback fails to work with handle_percpu_irq and handle_edge_irq.
The interrupt stays disabled after the first invocation since percpu and edge irq do
not unmask an interrupt after handling it. For handle_level_irq masking in the ack
is redundant because ack is always called after mask in the mask_ack function.

Masking in the ack function is required only when __do_IRQ was used instead of flow
handlers, but using __do_IRQ has been deprecated.

Remove the masking of interrupt from the ack callback.

FUJITA Tomonori 710224fa27 arm: fix "arm: fix pci_set_consistent_dma_mask for dmabounce devices"
This fixes the regression caused by the commit 6fee48cd33
("dma-mapping: arm: use generic pci_set_dma_mask and
pci_set_consistent_dma_mask").

ARM needs to clip the dma coherent mask for dmabounce devices. This
restores the old trick.

Note that strictly speaking, the DMA API doesn't allow architectures to do
such but I'm not sure it's worth adding the new API to set the dma mask
that allows architectures to clip it.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

  http://userweb.kernel.org/~tj/misc/slabh-sweep.py

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
   files.

2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

H Hartley Sweeten c8c3dcb9df [ARM] locomo: fix unpaired spin_lock_irqsave
The function locomo_m62332_senddata sends a three byte i2c message to
a M62332 DAC. This entire function is guarded with a spin_lock_irqsave
at the start of the function and a spin_unlock_irqrestore at the end.

As each byte is transferred, the i2c ACK from the DAC is checked.
Currently, if the ACK is missing the function simply returns without
the unlock. It also leaves the i2c bus in an invalid state since the
last byte transferred did not have a "stop" condition and leave the
bus idle.

Fix this by adding an exit path using goto.

H Hartley Sweeten 1b0d76cb9d [ARM] locomo: fix SPI register offset
The locomo spi registers are all defined in locomo.h as offsets
from the first spi register LOCOMO_SPI (0x60), which is itself
an offset from the locomo base address. To correctly access
these registers LOCOMO_SPI must always be included in the
address calculation.

There are two places in locomo.c where this is not done. The
first one, in locomo_suspend, actually results in a write to
LOCOMO_ST instead of LOCOMO_SPICT. The second is in __locomo_probe
and results in a write to LOCOMO_MCSX2 instead of LOCOMO_SPIIE.

Fix these by including LOCOMO_SPI in the calculation.

The code now has the following flow:

1) struct vic_device definition, static variables, and to_vic()

2) common code (vic_init2)
3) vic power management callbacks
4) vic power management initialization/registration

Nicolas Pitre 58edb51572 [ARM] make page_to_dma() highmem aware
If a machine class has a custom __virt_to_bus() implementation then it
must provide a __arch_page_to_dma() implementation as well which is
_not_ based on page_address() to support highmem.

This patch fixes existing __arch_page_to_dma() and provide a default
implementation otherwise. The default implementation for highmem is
based on __pfn_to_bus() which is defined only when no custom
__virt_to_bus() is provided by the machine class.

That leaves only ebsa110 and footbridge which cannot support highmem
until they provide their own __arch_page_to_dma() implementation.
But highmem support on those legacy platforms with limited memory is
certainly not a priority.

For example, when asked for device D connection C1 and we only find

Change the algorithm such that:
An entry with a NULL ID is assumed to be a wildcard.
If an entry has a device ID, it must match
If an entry has a connection ID, it must match

However, we maintain the order of precidence while still only doing
a single pass over all entries: dev+con > dev only > con only.

Rusty Russell 0de26520c7 cpumask: make irq_set_affinity() take a const struct cpumask
Impact: change existing irq_chip API

Not much point with gentle transition here: the struct irq_chip's
setaffinity method signature needs to change.

Fortunately, not widely used code, but hits a few architectures.

Note: In irq_select_affinity() I save a temporary in by mangling
irq_desc[irq].affinity directly. Ingo, does this break anything?

Nicolas Pitre 3bca103a1e [ARM] 5295/1: make ZONE_DMA optional
Most ARM machines don't need a special "DMA" memory zone, and
when configured out, the kernel becomes a bit smaller:

| text data bss dec hex filename
|3826182 102384 111700 4040266 3da64a vmlinux
|3823593 101616 111700 4036909 3d992d vmlinux.nodmazone

This is because the system now has only one zone total which effect is
to optimize away many conditionals in page allocation paths.

So let's configure this zone only on machines that need split zones.

Russell King 125ab12acf [ARM] dma: fix dmabounce dma_sync_xxx() implementations
The dmabounce dma_sync_xxx() implementation have been broken for
quite some time; they all copy data between the DMA buffer and
the CPU visible buffer no irrespective of the change of ownership.
(IOW, a DMA_FROM_DEVICE mapping copies data from the DMA buffer
to the CPU buffer during a call to dma_sync_single_for_device().)

Fix it by getting rid of sync_single(), moving the contents into
the recently created dmabounce_sync_for_xxx() functions and adjusting
appropriately.

This also makes it possible to properly support the DMA range sync
functions.

FUJITA Tomonori 8d8bb39b9e dma-mapping: add the device argument to dma_mapping_error()
Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:

This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).

I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread). So I
CC'ed this to KVM camp. Comments are appreciated.

A pointer to dma_mapping_ops to struct dev_archdata is added. If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it. If it's
NULL, the system-wide dma_ops pointer is used as before.

If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging). It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.

The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations. So x86 can't have dma_mapping_ops per
device. Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.

The first patch adds the device argument to dma_mapping_error. The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.

This patch:

dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations. So we can't have dma_mapping_ops per device.

Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function. x86 IOMMUs use device
argument.

Matthew Wilcox 950e4da324 arch: Remove unnecessary inclusions of asm/semaphore.h
None of these files use any of the functionality promised by
asm/semaphore.h. It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have
fix any build failures as they come up.

Dmitry Baryshkov b43a9e6087 [ARM] 4962/1: Introduce standard gpio interface for Scoop2.
This deprecates old set/reset_scoop_gpio interfacein favour of
support for generic gpio interface.

It requires gpiolib, so it depends on the previous patch
(gpiolib for SA-1100).

Rafael J. Wysocki 26398a70ea PM: Rename struct pm_ops and related things
The name of 'struct pm_ops' suggests that it is related to the power
management in general, but in fact it is only related to suspend.   Moreover,
its name should indicate what this structure is used for, so it seems
reasonable to change it to 'struct platform_suspend_ops'.   In that case, the
name of the global variable of this type used by the PM core and the names of
related functions should be changed accordingly.

Rafael J. Wysocki 95d9ffbe01 PM: Move definition of struct pm_ops to suspend.h
Move the definition of 'struct pm_ops' and related functions from <linux/pm.h>
to <linux/suspend.h> .

There are, at least, the following reasons to do that:
* 'struct pm_ops' is specifically related to suspend and not to the power
  management in general.
* As long as 'struct pm_ops' is defined in <linux/pm.h>, any modification of it
causes the entire kernel to be recompiled, which is unnecessary and annoying.
* Some suspend-related features are already defined in <linux/suspend.h>, so it
is logical to move the definition of 'struct pm_ops' into there.
* 'struct hibernation_ops', being the hibernation-related counterpart of
  'struct pm_ops', is defined in <linux/suspend.h> .
