headers and no active developement has been
going on those glues for quite some time.
Apparently, for da8xx glue, only initial commit
3ee076de (usb: musb: introduce DA8xx/OMAP-L1x
glue layer) has been tested. All other patches
seem to have been compile-tested only.
For davinci glue layer, last real commit dates
back from 2010, with commit f405387 (USB: MUSB:
fix kernel WARNING/oops when unloading module
in OTG mode).

MUSB still needs lots of work on the DMA part
if we want to enable multiple DMA engines on
a multiplatform kernel. Meanwhile, we're forcing
PIO-only so that we, at least, have a working
driver.

Currently all UDC drivers are calling
device_register() before calling
usb_add_gadget_udc(). In order to avoid
code duplication, we can allow udc-core.c
register that device.
However that would become a really large patch,
so to cope with the meanwhile and allow us
to write bite-sized patches, we're adding
a flag which will be set by UDC driver once
it removes the code for registering the
gadget device.
Once all are converted, the new flag will
be removed.

Add 2 flags, needs_vcc and needs_reset to platform data.
If the flag is set and the regulator couldn't be found
then we bail out with -EPROBE_DEFER.
For device tree boot we depend on presensce of vcc-supply/
reset-supply properties to decide if we should bail out
with -EPROBE_DEFER or just continue in case the regulator
can't be found.
This is required for proper functionality in cases where the
regulator is needed but is probed later than the PHY device.

We expect the RESET line to be modeled as a regulator with supply
name "reset". The regulator should be modeled such that enabling
the regulator brings the PHY device out of RESET and disabling the
regulator holds the device in RESET.
They PHY will be held in RESET in .shutdown() and brought out of
RESET in .init().

The commit 2e124b4a39 removed the checks
that prevented qt2_process_read_urb() from trying to put chars into
ttys that weren't actually opened. This resulted in 'tty is NULL'
warnings from flush_to_ldisc() when the device was used.
The devices use just one read urb for all ports. As a result
qt2_process_read_urb() may be called with the current port set to a
port number that has not been opened. Add a check if the port is open
before calling tty_flip_buffer_push().

this driver to udc_start/udc_stop, it failed
to consider the fact that stop_activity()
is called from disconnect interrupt.

drivers/usb/phy/ should be compiled everytime
we have CONFIG_USB_OTG_UTILS enabled.
phy/ should've been part of the build process based
on CONFIG_USB_OTG_UTILS, don't know where I had my head
when I allowed CONFIG_USB_COMMON there.
In fact commit c6156328de
tried to fix a previous issue but it made things even
worse.
The real solution is to compile phy/ based on
CONFIG_USB_OTG_UTILS which gets selected by all PHY
drivers.
I only triggered the error recently after accepting a
patch which moved a bunch of code from otg/otg.c to
phy/phy.c and running 100 randconfig cycles.

1bf0cf6040 "usb: gadget: omap_udc: convert to udc_start/udc_stop"
made some trivial changes but was missing a ';' character.
8c4cc00552 "ARM: OMAP1: DMA: Moving OMAP1 DMA channel definitions
to mach-omap1" added a definition for OMAP_DMA_USB_W2FC_TX0 to
the driver while making the header file it was defined in
unavailable for drivers, but this driver actually needs
OMAP_DMA_USB_W2FC_RX0 as well.
Both changes appear trivial, so let's add the missing semicolon
and the macro definition.

This reverts commit 200e0d99 ("USB: storage: optimize to match the
Huawei USB storage devices and support new switch command" and the
followup bugfix commit cd060956 ("USB: storage: properly handle
the endian issues of idProduct").
The commit effectively added a large number of Huawei devices to
the deprecated usb-storage mode switching logic. Many of these
devices have been in use and supported by the userspace
usb_modeswitch utility for years. Forcing the switching inside
the kernel causes a number of regressions as a result of ignoring
existing onfigurations, and also completely takes away the ability
to configure mode switching per device/system/user.
Known regressions caused by this:
- Some of the devices support multiple modes, using different
switching commands. There are existing configurations taking
advantage of this.
- There is a real use case for disabling mode switching and
instead mounting the exposed storage device. This becomes
impossible with switching logic inside the usb-storage driver.
- At least on device fail as a result of the usb-storage switching
command, becoming completely unswitchable. This is possibly a
firmware bug, but still a regression because the device work as
expected using usb_modeswitch defaults.
In-kernel mode switching was deprecated years ago with the
development of the more user friendly userspace alternatives. The
existing list of devices in usb-storage was only kept to prevent
breaking already working systems. The long term plan is to remove

Commit 80ab72e1 (usb: musb: omap2430: fix the readiness check
in omap_musb_mailbox) made the check incorrect, as we will lose the
glue/link status during the normal built-in probe order (twl4030_usb is
probed after musb omap2430, but before musb core is ready).
As a result, if you boot with USB cable on and load g_ether, the
connection does not work as the code thinks the cable is off and the
phy gets powered down immediately. This is a major regression in 3.9-rc1.
So the proper check should be: exit if _glue is NULL, but if it's
initialized we memorize the status, and then check if the musb core
is ready.

This patch (as1661) fixes a rather obscure bug in ehci-hcd. In a
couple of places, the driver compares the DMA address stored in a QH's
overlay region with the address of a particular qTD, in order to see
whether that qTD is the one currently being processed by the hardware.
(If it is then the status in the QH's overlay region is more
up-to-date than the status in the qTD, and if it isn't then the
overlay's value needs to be adjusted when the QH is added back to the
active schedule.)
However, DMA address in the overlay region isn't always valid. It
sometimes will contain a stale value, which may happen by coincidence
to be equal to a qTD's DMA address. Instead of checking the DMA
address, we should check whether the overlay region is active and
valid. The patch tests the ACTIVE bit in the overlay, and clears this
bit when the overlay becomes invalid (which happens when the
currently-executing URB is unlinked).

(if not all) Intel EHCI controllers. After a QH has been unlinked
from the async schedule and the corresponding IAA interrupt has
occurred, the controller is not supposed access the QH and its qTDs.
There certainly shouldn't be any more DMA writes to those structures.
Nevertheless, Intel's controllers have been observed to perform a
final writeback to the QH's overlay region and to the most recent qTD.

This patch works around the problem by always waiting for two IAA
cycles when unlinking an async QH. The extra IAA delay gives the
controller time to perform its final writeback.
Surprisingly enough, the effects of this silicon bug have gone
undetected until quite recently. More through luck than anything
else, it hasn't caused any apparent problems. However, it does
interact badly with the path that follows this one, so it needs to be
addressed.

USB uses the .find_bridge() callback from struct acpi_bus_type
incorrectly, because as a result of the way it is used by USB every
device in the system that doesn't have a bus type or parent is
passed to usb_acpi_find_device() for inspection.
What USB actually needs, though, is to call usb_acpi_find_device()
for USB ports that don't have a bus type defined, but have
usb_port_device_type as their device type, as well as for USB
devices.
To fix that replace the struct bus_type pointer in struct
acpi_bus_type used for matching devices to specific subsystems
with a .match() callback to be used for this purpose and update
the users of struct acpi_bus_type, including USB, accordingly.
Define the .match() callback routine for USB, usb_acpi_bus_match(),
in such a way that it will cover both USB devices and USB ports
and remove the now redundant .find_bridge() callback pointer from
usb_acpi_bus.

Smatch complains that "len" could be larger than the sizeof(value)

In patch "5d3c28b usb: otg: add device tree support to otg library"
devm_usb_get_phy_by_phandle() was added. It uses try_module_get() to lock the
phy driver in memory. The corresponding module_put() is missing in that patch.
This patch adds try_module_get() to usb_get_phy() and usb_get_phy_dev().
Further the missing module_put() is added to usb_put_phy().

The current ordering in makefile makes gadget
drivers be loaded before usb functions which
causes usb_get_function_instance() to fail when
gadget modules are statically linked to the
kernel binary.
Changed the ordering here so that USB functions
are loaded before gadget drivers.
Note that this is only a temporary solution and
a more robust fix is needed in the long run.

commit 3921426 (usb: dwc3: core: move
event buffer allocation out of
dwc3_core_init()) introduced a memory leak
of the coherent memory we use as event
buffers on dwc3 driver.
If the driver is compiled as a dynamically
loadable module and use constantly loads
and unloads the driver, we will continue
to leak the coherent memory allocated during
->probe() because dwc3_free_event_buffers()
is never called during ->remove().

Modify the request_module to prefix the file system type with "fs-"
and add aliases to all of the filesystems that can be built as modules
to match.
A common practice is to build all of the kernel code and leave code
that is not commonly needed as modules, with the result that many
users are exposed to any bug anywhere in the kernel.
Looking for filesystems with a fs- prefix limits the pool of possible
modules that can be loaded by mount to just filesystems trivially
making things safer with no real cost.
Using aliases means user space can control the policy of which
filesystem modules are auto-loaded by editing /etc/modprobe.d/*.conf
with blacklist and alias directives. Allowing simple, safe,
well understood work-arounds to known problematic software.
This also addresses a rare but unfortunate problem where the filesystem
name is not the same as it's module name and module auto-loading
would not work. While writing this patch I saw a handful of such
cases. The most significant being autofs that lives in the module
autofs4.
This is relevant to user namespaces because we can reach the request
module in get_fs_type() without having any special permissions, and
people get uncomfortable when a user specified string (in this case
the filesystem type) goes all of the way to request_module.
After having looked at this issue I don't think there is any
particular reason to perform any filtering or permission checks beyond
making it clear in the module request that we want a filesystem
module. The common pattern in the kernel is to call request_module()
without regards to the users permissions. In general all a filesystem
module does once loaded is call register_filesystem() and go to sleep.
Which means there is not much attack surface exposed by loading a
filesytem module unless the filesystem is mounted. In a user
namespace filesystems are not mounted unless .fs_flags = FS_USERNS_MOUNT,
which most filesystems do not set today.

This patch (as1649) reverts commit
55bcdce8a8 (USB: EHCI: remove ASS/PSS
polling timeout). That commit was written under the assumption that
some controllers may take a very long time to turn off their async and
periodic schedules. It now appears that in fact the schedules do get
turned off reasonably quickly, but some controllers occasionally leave
the schedules' status bits turned on and consequently ehci-hcd can't
tell that the schedules are off.
VIA controllers in particular have this problem. ehci-hcd tells the
hardware to turn off the async schedule, the schedule does get turned
off, but the status bit remains on. Since the EHCI spec requires that
the schedules not be re-enabled until the previous disable has taken
effect, with an unlimited timeout the async schedule never gets turned
back on. The resulting symptom is that the system is unable to
communicate with USB devices.

If one storage interface or usb network interface(iSCSI case) exists in
current configuration, memory allocation with GFP_KERNEL during
usb_device_reset() might trigger I/O transfer on the storage interface
itself and cause deadlock because the 'us->dev_mutex' is held in
.pre_reset() and the storage interface can't do I/O transfer when the
reset is triggered by other interface, or the error handling can't be
completed if the reset is triggered by the storage itself (error
handling path).

The static lock initializers want to be fed the proper name of the
lock and not some random string. In mainline random strings are
obfuscating the readability of debug output, but for RT they prevent

The current entry in unusual_cypress.h for the Super TOP SATA bridge devices
seems to be causing corruption on newer revisions of this device. This has
been reported in Arch Linux and Fedora. The original patch was tested on
devices with bcdDevice of 1.60, whereas the newer devices report bcdDevice
as 2.20. Limit the UNUSUAL_DEV entry to devices less than 2.20.

The USB device descriptor of one identity presented by a few
Huawei morphing devices have serial functions with class codes
02/02/ff, indicating CDC ACM with a vendor specific protocol. This
combination is often used for MSFT RNDIS functions, and the CDC
ACM class driver will therefore ignore such functions.
The CDC ACM class driver cannot support functions with only 2
endpoints. The underlying serial functions of these modems are
also believed to be the same as for alternate device identities
already supported by the option driver. Letting the same driver
handle these functions independently of the current identity
ensures consistent handling and user experience.
There is no need to blacklist these devices in the rndis_host
driver. Huawei serial functions will either have only 2 endpoints
or a CDC ACM functional descriptor with bmCapabilities != 0, making
them correctly ignored as "non RNDIS" by that driver.

Make sure serial-driver dtr_rts is called with disc_mutex held after
checking the disconnected flag.
Due to a bug in the tty layer, dtr_rts may get called after a device has
been disconnected and the tty-device unregistered. Some drivers have had
individual checks for disconnect to make sure the disconnected interface
was not accessed, but this should really be handled in usb-serial core
(at least until the long-standing tty-bug has been fixed).
Note that the problem has been made more acute with commit 0998d06310
("device-core: Ensure drvdata = NULL when no driver is bound") as the
port data is now also NULL when dtr_rts is called resulting in further
oopses.

Let's have a single platform data structure for the OMAP's High-Speed
USB host subsystem instead of having 3 separate ones i.e. one for
board data, one for USB Host (UHH) module and one for USB-TLL module.
This makes the code much simpler and avoids creating multiple copies of
platform data.

The Tegra USB driver has a number of issues:
1) The PHY driver isn't a true platform device, and doesn't implement
the standard USB PHY API.
2) struct device instance numbers were used to make decisions in the
driver, rather than being parameterized by DT or platform data.
This pull request solves issue (2), and lays the groundwork for solving
issue (1). The work on issue (1) involved introducing new DT nodes for
the USB PHYs, which in turn interacted with the Tegra common clock
framework changes, due to the move of clock lookups into device tree.
Hence, these USB driver changes are taken through the Tegra tree with
acks from USB maintainers.
This pull request is based on the previous pull request, with tag

The Tegra USB driver has a number of issues:
1) The PHY driver isn't a true platform device, and doesn't implement
the standard USB PHY API.
2) struct device instance numbers were used to make decisions in the
driver, rather than being parameterized by DT or platform data.
This pull request solves issue (2), and lays the groundwork for solving
issue (1). The work on issue (1) involved introducing new DT nodes for
the USB PHYs, which in turn interacted with the Tegra common clock
framework changes, due to the move of clock lookups into device tree.
Hence, these USB driver changes are taken through the Tegra tree with
acks from USB maintainers.
This pull request is based on the previous pull request, with tag

This patch (as1654) fixes a very old bug in ehci-hcd, connected with
scheduling of periodic split transfers. The calculations for
full/low-speed bus usage are all carried out after the correction for
bit-stuffing has been applied, but the values in the max_tt_usecs
array assume it hasn't been. The array should allow for allocation of
up to 90% of the bus capacity, which is 900 us, not 780 us.
The symptom caused by this bug is that any isochronous transfer to a
full-speed device with a maxpacket size larger than about 980 bytes is
always rejected with a -ENOSPC error.

This patch (as1653) fixes a bug in ehci-hcd. Unlike iTD entries, an
siTD entry in the periodic schedule may not complete until the frame
after the one it belongs to. Consequently, when scanning the periodic
schedule it is necessary to start with the frame _preceding_ the one
where the previous scan ended.
Not doing this properly can result in memory leaks and failures to
complete isochronous URBs.

This patch (as1656) adds a module parameter to the net2280 UDC driver
to force full-speed operation. It is intended for testing purposes,
where one wants to check how well a full-speed device performs when
attached to a high-speed bus. Without this parameter it would be
necessary to interpose a full-speed hub; otherwise the net2280 would
connect at high speed.

The usbtest driver includes some rather simple-minded logic for
selecting an altsetting to test. It doesn't work well for the g_zero
gadget, because it selects altsetting 0 (which doesn't have
isochronous endpoints) rather than altsetting 1 (which does have them,
if the gadget's hardware supports them). This prevents usbtest's
isochronous tests (15, 16, 22, and 23) from working with g_zero.
Since g_zero is one of the most common gadget drivers used for USB
testing, usbtest should do a better job of supporting it. But since
some programs may rely on the current scheme for selecting
altsettings, I didn't want to change it.
Instead, this patch (as1655) adds a module parameter to usbtest, which
can be used to override the default altsetting. Since usbtest is
never used by normal users (most distributions probably don't even
build it), the new module parameter won't inconvenience anybody. In
any case, it is entirely optional -- leaving it unset preserves the
existing behavior.
The patch also fixes a related bug in usbtest: After selecting an
altsetting, the driver neglects to store its selection.

This patch (as1652) fixes a long-standing bug in ehci-hcd. The driver
relies on status polls to know when to stop port-resume signalling.
It uses the root-hub status timer to schedule these status polls. But
when the driver for the root hub is resumed, the timer is rescheduled
to go off immediately -- before the port is ready. When this happens
the timer does not get re-enabled, which prevents the port resume from
finishing until some other event occurs.
The symptom is that when a new device is plugged in, it doesn't get
recognized or enumerated until lsusb is run or something else happens.
The solution is to re-enable the root-hub status timer after every
status poll while a port resume is in progress.
This bug hasn't surfaced before now because we never used to try to
suspend the root hub in the middle of a port resume (except by
coincidence).

This patch (as1649) adds a mechanism for host controller drivers to
inform usbcore when they have begun or ended resume signalling on a
particular root-hub port. The core will then make sure that the root
hub does not get runtime-suspended while the port resume is going on.
Since commit 596d789a21 (USB: set hub's
default autosuspend delay as 0), the system tries to suspend hubs
whenever they aren't in use. While a root-hub port is being resumed,
the root hub does not appear to be in use. Attempted runtime suspends
fail because of the ongoing port resume, but the PM core just keeps on
trying over and over again. We want to prevent this wasteful effort.

This patch (as1648) fixes a regression affecting nVidia EHCI
controllers. Evidently they don't like to have more than one async QH
unlinked at a time. I can't imagine how they manage to mess it up,
but at least one of them does.
The patch changes the async unlink logic in two ways:
Each time an IAA cycle is started, only the first QH on the
async unlink list is handled (rather than all of them).
Async QHs do not all get unlinked as soon as they have been
empty for long enough. Instead, only the last one (i.e., the
one that has been on the schedule the longest) is unlinked,
and then only if no other unlinks are in progress at the time.
This means that when multiple QHs are empty, they won't be unlinked as
quickly as before. That's okay; it won't affect correct operation of
the driver or add an excessive load. Multiple unlinks tend to be
relatively rare in any case.

This patch (as1647) attempts to work around a problem that seems to
affect some nVidia EHCI controllers. They sometimes take a very long
time to turn off their async or periodic schedules. I don't know if
this is a result of other problems, but in any case it seems wise not
to depend on schedule enables or disables taking effect in any
specific length of time.
The patch removes the existing 20-ms timeout for enabling and
disabling the schedules. The driver will now continue to poll the
schedule state at 1-ms intervals until the controller finally decides
to obey the most recent command issued by the driver. Just in case
this hides a problem, a debugging message will be logged if the
controller takes longer than 20 polls.
I don't know if this will actually fix anything, but it can't hurt.

This patch is to set power.async_suspend for usb port in order
to allow it to be suspended and resumed asynchronously during
system sleep transitions.
The power.async_suspend flag is also set for devices that don't have
suspend or resume callbacks, because otherwise they would make the
main suspend/resume thread wait for their "asynchronous" children
(during suspend) or parents (during resume), effectively negating the
possible gains from executing these devices' suspend and resume
callbacks asynchronously.

This patch is to add usb port auto power off mechanism.
When usb device is suspending, usb core will suspend usb port and
usb port runtime pm callback will clear PORT_POWER feature to
power off port if all conditions were met. These conditions are
remote wakeup disable, pm qos NO_POWER_OFF flag clear and persist
enable. When it resumes, power on port again.
Add did_runtime_put in the struct usb_port to ensure
pm_runtime_get/put(portdev) to be called pairedly. Set did_runtime_put
to true when call pm_runtime_put(portdev) during suspending. The
pm_runtime_get(portdev) only will be called when did_runtime_put
is set to true during resuming. Set did_runtime_put to false after
calling pm_runtime_get(portdev).
Make clear_port_feature() and hdev_to_hub() as global symbol.
Rename clear_port_feature() to usb_clear_port_feature() and
hdev_to_hub() to usb_hub_to_struct_hub().
Extend hub_port_debounce() with the fuction of debouncing to
be connected. Add two wraps: hub_port_debounce_be_connected()
and hub_port_debouce_be_stable().
Increase HUB_DEBOUNCE_TIMEOUT to 2000 because some usb ssds
needs around 1.5 or more to make the hub port status to be
connected steadily after being powered off and powered on.

Multiple dwc3 probe calls try to allocate no_op_xceive platform
device. Having static IDs for these will throw sysfs error -EEXIST.
Changing these static platform device IDs to AUTO to enable
multiple dwc3 controller support on a SoC.

New PHY lib APIs like usb_add_phy_dev() and devm_usb_get_phy_dev() are
used in MUSB (OMAP), in order to make use of the binding information
provided in the board file (of OMAP platforms).

Added a new driver for the usb part of control module. This has an API
to power on the USB2 phy and an API to write to the mailbox depending on
whether MUSB has to act in host mode or in device mode.
Writing to control module registers for doing the above task which was
previously done in omap glue and in omap-usb2 phy will be removed.

Added an API devm_usb_get_phy_by_phandle(), to get usb phy by passing a
device node phandle value. This function will return a pointer to
the phy on success, -EPROBE_DEFER if there is a device_node for the phandle,
but the phy has not been added, or a ERR_PTR() otherwise.

In order to add support for multipe PHY's of the same type, new API's
for adding PHY and getting PHY has been added. Now the binding
information for the PHY and controller should be done in platform file
using usb_bind_phy API. And for getting a PHY, the device pointer of the
USB controller and an index should be passed. Based on the binding
information that is added in the platform file, usb_get_phy_dev will return the
appropriate PHY.
Already existing API's to add and get phy by type is not removed. These
API's are deprecated and will be removed once all the platforms start to
use the new API.

In order to support platforms which has multiple PHY's (of same type) and
which has multiple USB controllers, a new design is adopted wherin the binding
information (between the PHY and the USB controller) should be passed to the
PHY library from platform specific file (board file).
So added a new API to pass the binding information.

Add an API in the omap glue layer to write to the mailbox register which
can be used by comparator driver(twl). To pass the detection of the attached
device (signified by VBUS, ID) to the dwc3 core, dwc3 core has to write

This patch (as1646) fixes a long-standing bug in the USB hub driver.
Upon conversion from char to unsigned long, the bytes in the status
buffer are subject to unwanted sign extension. The bytes should be
declared as u8 rather than char, to prevent this.
This effects of this bug are minimal. The hub driver may end up doing
a little unnecessary extra work because it thinks events have occurred
on some ports when they really haven't.

usally we will use udc->tranceiver == NULL or
udc->tranceiver != NULL.
So when failed to get the udc->tranceiver by usb_get_phy(), we
directly set udc->tranceiver to be NULL.
Then the source code will not need macro IS_ERR_OR_NULL() for
udc->tranceiver judgement. It can reduce the line size and make
the judgement simple.

Usb3.0 device defines function remote wakeup which is only for interface
recipient rather than device recipient. This is different with usb2.0 device's
remote wakeup feature which is defined for device recipient. According usb3.0
spec 9.4.5, the function remote wakeup can be modified by the SetFeature()
requests using the FUNCTION_SUSPEND feature selector. This patch is to use
correct way to disable usb3.0 device's function remote wakeup after suspend
error and resuming.
This should be backported to kernels as old as 3.4, that contain the
commit 623bef9e03 "USB/xhci: Enable remote
wakeup for USB3 devices."

When the xHCI driver is not available, actively switch the ports to EHCI
mode since some BIOSes leave them in xHCI mode where they would
otherwise appear dead. This was discovered on a Dell Optiplex 7010,
but it's possible other systems could be affected.
This should be backported to kernels as old as 3.0, that contain the
commit 69e848c209 "Intel xhci: Support
EHCI/xHCI port switching."

This patch (as1640) fixes a memory leak in xhci-hcd. The urb_priv
data structure isn't always deallocated in the handle_tx_event()
routine for non-control transfers. The patch adds a kfree() call so
that all paths end up freeing the memory properly.

To calculate the TD size for a particular TRB in an isoc TD, we need
know the endpoint's max packet size. Isochronous endpoints also encode
the number of additional service opportunities in their wMaxPacketSize
field. The TD size calculation did not mask off those bits before using
the field. This resulted in incorrect TD size information for
isochronous TRBs when an URB frame buffer crossed a 64KB boundary.
For example:
- an isoc endpoint has 2 additional service opportunites and
a max packet size of 1020 bytes
- a frame transfer buffer contains 3060 bytes
- one frame buffer crosses a 64KB boundary, and must be split into
one 1276 byte TRB, and one 1784 byte TRB.
The TD size is is the number of packets that remain to be transferred
for a TD after processing all the max packet sized packets in the
current TRB and all previous TRBs.
For this TD, the number of packets to be transferred is (3060 / 1020),
or 3. The first TRB contains 1276 bytes, which means it contains one
full packet, and a 256 byte remainder. After processing all the max
packet-sized packets in the first TRB, the host will have 2 packets left
to transfer.
The old code would calculate the TD size for the first TRB as:
total packet count = DIV_ROUND_UP (TD length / endpoint wMaxPacketSize)
total packet count - (first TRB length / endpoint wMaxPacketSize)
The math should have been:
total packet count = DIV_ROUND_UP (3060 / 1020) = 3
3 - (1276 / 1020) = 2
Since the old code didn't mask off the additional service interval bits
from the wMaxPacketSize field, the math ended up as
total packet count = DIV_ROUND_UP (3060 / 5116) = 1
1 - (1276 / 5116) = 1
Fix this by masking off the number of additional service opportunities
in the wMaxPacketSize field.
This patch should be backported to stable kernels as old as 3.0, that

An isochronous TD is comprised of one isochronous TRB chained to zero or
more normal TRBs. Only the isoc TRB has the TBC and TLBPC fields. The
normal TRBs must set those fields to zeroes. The code was setting the
TBC and TLBPC fields for both isoc and normal TRBs. Fix this.
This should be backported to stable kernels as old as 3.0, that contain
the commit b61d378f2d " xhci 1.0: Set
transfer burst last packet count field."

First of all, that 28 value makes no sense as
HIRD threshold is a 4-bit value, second of all
it's causing issues for OMAP5.
Using 12 because commit cbc725b3 (usb: dwc3:
keep default hird threshold value as 4b1100)
had the intention of setting the maximum allowed
value of 0xc.
Also, original code has been wrong forever, so
this should be backported as far back as
possible.

This patches converts the driver into the new style start/stop
interface. As a result the driver no longer uses the static
global the_conroller variable.

The fix is to convert ehci-mxc over to the new "ehci-hcd is a library"
scheme so that it can coexist peacefully with the ehci-platform
driver. As part of the conversion the ehci_mxc_priv data structure,
which was allocated dynamically, is now placed where it belongs: in
the private area at the end of struct ehci_hcd.

Convert all uses of devm_request_and_ioremap() to the newly introduced
devm_ioremap_resource() which provides more consistent error handling.
devm_ioremap_resource() provides its own error messages so all explicit
error messages can be removed from the failure code paths.
