Add user-mode implementations of gettimeofday() and clock_gettime() to
the VDSO. This is currently usable with 2 clocksources: the CP0 count
register, which is accessible to user-mode via RDHWR on R2 and later
cores, or the MIPS Global Interrupt Controller (GIC) timer, which
provides a "user-mode visible" section containing a mirror of its
counter registers. This section must be mapped into user memory, which
is done below the VDSO data page.
When a supported clocksource is not in use, the VDSO functions will
return -ENOSYS, which causes libc to fall back on the standard syscall
path.
When support for neither of these clocksources is compiled into the
kernel at all, the VDSO still provides clock_gettime(), as the coarse
realtime/monotonic clocks can still be implemented. However,
gettimeofday() is not provided in this case as nothing can be done
without a suitable clocksource. This causes the symbol lookup to fail
in libc and it will then always use the standard syscall path.
This patch includes a workaround for a bug in QEMU which results in
RDHWR on the CP0 count register always returning a constant (incorrect)
value. A fix for this has been submitted, and the workaround can be
removed after the fix has been in stable releases for a reasonable
amount of time.
A simple performance test which calls gettimeofday() 1000 times in a
loop and calculates the average execution time gives the following
results on a Malta + I6400 (running at 20MHz):
- Syscall: ~31000 ns
- VDSO (GIC): ~15000 ns
- VDSO (CP0): ~9500 ns

Add an initial implementation of a proper (i.e. an ELF shared library)
VDSO. With this commit it does not export any symbols, it only replaces
the current signal return trampoline page. A later commit will add user
implementations of gettimeofday()/clock_gettime().
To support both new toolchains and old ones which don't generate ABI
flags section, we define its content manually and then use a tool
(genvdso) to patch up the section to have the correct name and type.
genvdso also extracts symbol offsets ({,rt_}sigreturn) needed by the
kernel, and generates a C file containing a "struct mips_vdso_image"
containing both the VDSO data and these offsets. This C file is
compiled into the kernel.
On 64-bit kernels we require a different VDSO for each supported ABI,
so we may build up to 3 different VDSOs. The VDSO to use is selected by
the mips_abi structure.
A kernel/user shared data page is created and mapped below the VDSO
image. This is currently empty, but will be used by the user time
function implementations which are added later.

Prior to release 6 of the MIPS architecture it has been implementation
dependent whether masked interrupts cause a wait instruction to return,
so the kernel has effectively had to maintain a whitelist of cores upon
which it is safe to use the r4k_wait_irqoff cpu_wait implementation.
With MIPSr6 this is no longer implementation dependent and
r4k_wait_irqoff can always be used.
Remove the existing I6400 case which will no longer ever be hit, and was
incorrect anyway since I6400 & r6 in general doesn't have the WII bit.

Document that CPC core-other accesses must take place within the bounds
of the CM lock, and begin using the CM lock functions where we access
the GCRs of other cores. This is required because with CM3 the CPC began
using GCR_CL_OTHER instead of CPC_CL_OTHER.

Introduce mips_cm_lock_other & mips_cm_unlock_other, mirroring the
existing CPC equivalents, in order to lock access from the current core
to another via the core-other GCR region. This hasn't been required in
the past but with CM3 the CPC starts using GCR_CL_OTHER rather than
CPC_CL_OTHER and this will be required for safety.

When debugging core bringup it is useful to see the state of the CPC
sequencer, so output that value if the core hasn't started within a
reasonable amount of time (1 second). This avoids simply appearing to
the user to hang if a secondary core fails to start.

Provide support for outputting early debug information, in the form of
various register values should an exception occur, during the early
bringup of secondary cores. This code requires an ns16550-compatible
UART accessible from the secondary core, and is written in assembly due
to the environment in which such early exceptions occur where way may
not have a stack, be coherent or even have initialised caches.

WAIT instruction:
Thread enters wait state. No instructions are executed until an
interrupt occurs. The processor's clocks are stopped if both threads
are in idle mode.
Description:
Execution of this instruction puts the thread into wait state, an idle
mode in which no instructions are fetched or executed. The thread remains
in wait state until an interrupt occurs that is not masked by the
interrupt mask field in the Status register. Then, if interrupts are
enabled by the IE bit in the Status register, the interrupt is serviced.
The ERET instruction returns to the instruction following the WAIT
instruction. If interrupts are disabled, the processor resumes executing
instructions with the next sequential instruction.
Programming notes:
The WAIT instruction should be executed while interrupts are disabled
by the IE bit in the Status register. This avoids a potential timing
hazard, which occurs if an interrupt is taken between testing the counter
and executing the WAIT instruction. In this hazard case, the interrupt
will have been completed before the WAIT instruction is executed, so
the processor will remain indefinitely in wait state until the next
interrupt.

Set the Status.BEV bit throughout the early startup of a secondary core
such that if an exception occurs the core branches to one of the
exception vector entries from cps-vec.S, rather than branching to
whatever is set in EBase.

CM3 has 64 bit GCR_ERROR_* registers, but the code in
mips_cm_error_report was previously only reading 32 bits of it in MIPS32
kernels. Fix by splitting the reads for CM2 & CM3, and making use of the
read64_ variants of the accessor function for CM3.

Commit 3885c2b463 ("MIPS: CM: Add support for reporting CM cache
errors") added cases for decoding errors reported by CM3, but leaves the
buf variable which is printed as a string uninitialised for cause values
other than 1, 2 or 3. Fix by ensuring the buf variable is initialised to
an empty string in such cases.

Commit 3885c2b463 ("MIPS: CM: Add support for reporting CM cache
errors") leads to Malta boards unconditionally reading CM GCRs upon bus
errors, regardless of whether a CM is present. This is incorrect & will
lead to further exceptions. Fix by moving the GCR reads to after the
check for whether a CM is present.

The MIPS syscall handler code used to return -ENOSYS on invalid
syscalls. Whilst this is expected, it caused problems for seccomp
filters because the said filters never had the change to run since
the code returned -ENOSYS before triggering them. This caused
problems on the chromium testsuite for filters looking for invalid
syscalls. This has now changed and the seccomp filters are always
run even if the syscall is invalid. We return -ENOSYS once we
return from the seccomp filters. Moreover, similar codepaths have
been merged in the process which simplifies somewhat the overall
syscall code.

Commit 1a3d59579b ("MIPS: Tidy up FPU context switching") removed FP
context saving from the asm-written resume function in favour of reusing
existing code to perform the same task. However it only removed the FP
context saving code from the r4k_switch.S implementation of resume.
Remove it from the r2300_switch.S implementation too in order to prevent
attempting to save the FP context twice, which would likely lead to an
exception from the second save because the FPU had already been disabled
by the first save.
This patch has only been build tested, using rbtx49xx_defconfig.

The CONFIG_MIPS_MT symbol can be selected by CONFIG_MIPS_VPE_LOADER in
addition to CONFIG_MIPS_MT_SMP. We only want MT code in the CPS SMP boot
vector if we're using MT for SMP. Thus switch the config symbol we ifdef
against to CONFIG_MIPS_MT_SMP.

The MT-specific code in mips_cps_boot_vpes can safely be omitted from
kernels which don't support MT, with the default VPE==0 case being used
as it would be after the has_mt (Config3.MT) check failed at runtime.
Discarding the code entirely will save us a few bytes & allow cleaner
handling of MT ASE instructions by later patches.

The has_mt macro ended with a branch, leaving its callers with a delay
slot that would be executed if Config3.MT is not set. However it would
not be executed if Config3 (or earlier Config registers) don't exist
which makes it somewhat inconsistent at best. Fill the delay slot in the
macro & fix the mips_cps_boot_vpes caller appropriately.

MAARs should be initialised on each CPU (or rather, core) in the system
in order to achieve consistent behaviour & performance. Previously they
have only been initialised on the boot CPU which leads to performance
problems if tasks are later scheduled on a secondary CPU, particularly
if those tasks make use of unaligned vector accesses where some CPUs
don't handle any cases in hardware for non-speculative memory regions.
Fix this by recording the MAAR configuration from the boot CPU and
applying it to secondary CPUs as part of their bringup.

R6 removed the Config4.MMUExtDef field, with the low 16 bits only
allowed to contain FTLB fields, and commit e87569cd6c ("MIPS:
cpu-probe: Fix VTLB/FTLB configuration for R6") updated the probing of
this field to assume an FTLB is always present for R6.
However the FTLB may still be absent. The presence of those fields is
actually specified by the MMU type in the Config.MT field, so use that
(the new cpu_has_ftlb) to determine whether the FTLB is actually
present.

Add cpu_has_ftlb, which specifies that an FTLB is present in addition to
the VTLB, probed based on whether Config.MT == 4 (rather than 1 for
standard JTLB).
This is necessary since MIPS release 6 removes Config4.MMUExtDef, so the
presence of the FTLB fields in Config4 must be determined from Config.MT
instead.

Commit a6335fa1 fixed the case with gap between initrd and next usable PFN zone,
but broken the case when initrd is combined with usable memory into one region

Rather than saving the scalar FP or vector context in the assembly
resume function, reuse the existing C code we have in fpu.h to do
exactly that. This reduces duplication, results in a much easier to read
resume function & should allow the compiler to optimise out more MSA
code due to is_msa_enabled()/cpu_has_msa being known-zero at compile
time for kernels without MSA support.

In order for userland to determine whether various features are safe to
use, it will need to know both that the hardware supports those features
and that the kernel is recent enough & configured appropriately to
support them. For example under the O32 modeless FP proposal the dynamic
linker & ifunc resolvers will need this information. The kernel is the
only thing in a position to know availability accurately, so the kernel
needs to provide the information to userland. This patch introduces the
infrastructure to provide the AT_HWCAP aux vector to userland in order
to provide that information. It also defines the 2 currently specified
flags, which indicate MIPSr6 & MSA support.

It is desirable for signal handlers to be allowed to make use of MSA,
particularly if auto vectorisation is used when compiling a program.
The MSA context must therefore be saved & restored before & after
invoking the signal handler. Make use of the extended context structs
defined in the preceding patch to save MSA context after the sigframe
when appropriate.

The context introduced by MSA needs to be saved around signals. However,
we can't increase the size of struct sigcontext because that will change
the offset of the signal mask in struct sigframe or struct ucontext.
This patch instead places the new context immediately after the struct
sigframe for traditional signals, or similarly after struct ucontext for
RT signals. The layout of struct sigframe & struct ucontext is identical
from their sigcontext fields onwards, so the offset from the sigcontext
to the extended context will always be the same regardless of the type
of signal.
Userland will be able to search through the extended context by using
the magic values to detect which types of context are present. Any
unrecognised context can be skipped over using the size field of struct
extcontext. Once the magic value END_EXTCONTEXT_MAGIC is seen it is
known that there are no further extended context structures to examine.
This approach is somewhat similar to that taken by ARM to save VFP &
other context at the end of struct ucontext.
Userland can determine whether extended context is present by checking
for the USED_EXTCONTEXT bit in the sc_used_math field of struct
sigcontext. Whilst this could potentially change the historic semantics
of sc_used_math if further extended context which does not imply FP
context were to be introduced in the future, I have been unable to find
any userland code making use of sc_used_math at all. Using one of the
fields described as unused in struct sigcontext was considered, but the
kernel does not already write to those fields so there would be no
guarantee of the field being clear on older kernels. Other alternatives
would be to have userland check the kernel version, or to have a HWCAP
bit indicating presence of extended context. However there is a desire
to have the context & information required to decode it be self
contained such that, for example, debuggers could decode the saved
context easily.

The sc_used_math field of struct sigcontext & its variants has
traditionally been used as a boolean value indicating only whether or
not floating point context is saved within the sigcontext. With various
supported FP modes & the ability to switch between them this information
will no longer be enough to decode the meaning of the data stored in the
sc_fpregs fields of struct sigcontext.
To make that possible 3 bits are defined within sc_used_math:
- Bit 0 (USED_FP) represents whether FP was used, essentially
providing the boolean flag which sc_used_math as a whole provided
previously.
- Bit 1 (USED_FR1) provides the value of the Status.FR bit at the time
the FP context was saved.
- Bit 2 (USED_HYBRID_FPRS) indicates whether the FP context was saved
under the hybrid FPR scheme. Essentially, when set the odd singles
are located in bits 63:32 of the preceding even indexed sc_fpregs
element.
Any userland that tests whether the sc_used_math field is zero or
non-zero will continue to function as expected. Having said that, I
could not find any userland which uses the sc_used_math field at all.

When a task uses 32 bit floating point, the odd indexed 32b register
values are stored in bits 63:32 of the preceding even indexed 64b
FP register field in saved context. Thus there is no point in
preserving the odd indexed 64b register fields since they hold no
valid context. This patch will cause them to be skipped, as is
already done in arch/mips/kernel/signal32.c.

When saving FP state to struct sigcontext, make use of the offsets
provided by struct mips_abi to obtain appropriate addresses for the
sc_fpregs & sc_fpc_csr fields of the sigcontext. This is done only for
the native struct sigcontext in this patch (ie. for O32 in CONFIG_32BIT
kernels or for N64 in CONFIG_64BIT kernels) but is done in preparation
for sharing this code with compat ABIs in further patches.

The protected_{save,restore}_fp_context functions had effectively
different implementations for EVA. Simplify & unify the code somewhat
such that EVA configurations simply guarantee the FPU-not-owned path
through the standard code path.

The TLB registers are dumped in a couble of places:
- sysrq_tlbdump_single() - when dumping TLB state.
- do_mcheck() - in response to a machine check error.
The main TLB registers also differ between r3k and r4k, but r4k appears
to be assumed.
Refactor this code into a dump_tlb_regs() function, implemented for both
r3k and r4k, and used by both of the above functions.

The MSA architecture specification allows for hardware to not implement
unaligned vector memory accesses in some or all cases. A typical example
of this is the I6400 core which does not implement unaligned vector
memory access when the memory crosses a page boundary. The architecture
also requires that such memory accesses complete successfully as far as
userland is concerned, so the kernel is required to emulate them.
This patch implements support for emulating unaligned MSA ld & st
instructions by copying between the user memory & the tasks FP context
in struct thread_struct, updating hardware registers from there as
appropriate in order to avoid saving & restoring the entire vector
context for each unaligned memory access.
Tested both using an I6400 CPU and with a QEMU build hacked to produce
AdEL exceptions for unaligned vector memory accesses.

- Stop recalculating the address of the unaligned vector memory access
and rely upon the value read from BadVAddr as we do for other
unaligned memory access instructions.

Introduce accessor functions allowing the kernel to access arbitrary
vector registers using an arbitrary data format. The accessors are
implemented in assembly, using macros to avoid massive duplication, in
order to make use of the existing support for MSA with & without
toolchain support. The accessors will be used in a later patch.

Weak header file declarations are error-prone because they make every
definition weak, and the linker chooses one based on link order (see
10629d711e ("PCI: Remove __weak annotation from pcibios_get_phb_of_node
decl")).
get_c0_compare_int() is defined in several files. Each definition is weak,
so I assume Kconfig prevents two or more from being included. The caller
contains default code used when get_c0_compare_int() isn't defined at all.
Add a weak get_c0_compare_int() definition with the default code and remove
the weak annotation from the declaration.
Then the platform implementations will be strong and will override the weak
default. If multiple platforms are ever configured in, we'll get a link
error instead of calling a random platform's implementation.

Commit b677bc03d7 ("MIPS: cps-vec: Use macros for various arithmetics
and memory operations") replaced various load & store instructions
through cps-vec.S with the PTR_L & PTR_S macros. However it was somewhat
overzealous in doing so for CM GCR accesses, since the bit width of the
CM doesn't necessarily match that of the CPU. The registers accessed
(GCR_CL_COHERENCE & GCR_CL_ID) should be safe to simply always access
using 32b instructions, so do so in order to avoid issues when using a
32b CM with a 64b CPU.

That's not a problem for vpe_run() because Kconfig ensures there's never
more than one definition:
- vpe_run() is defined in arch/mips/kernel/vpe-mt.c if

- vpe_run() is defined in arch/mips/mti-malta/malta-amon.c if
CONFIG_MIPS_CMP=y
- CONFIG_MIPS_VPE_LOADER_MT cannot be set if CONFIG_MIPS_CMP=y
But it's simpler to verify correctness if we remove "weak" from the picture
and test the config symbols directly.
Remove "weak" from the vpe_run() declaration and use #if to test whether a
definition should be present.

vpe_run() is a weak symbol. If there's no definition of it, its value is
zero.
If vpe_run is zero, return failure early. We're going to fail anyway, so
there's no point in getting a VPE and attempting to load it.

R6 has dropped the MMUExtDef field from the config4 register and it
now returns 0. However, the return value means nothing in that case
and the only supported configuration for R6 is the VTLB+FTLB
(MMUextDef == 3). As a result, rework the code so that the correct
value is set for R6 cores.

Add a default case for the FTLB enable/disable code. This will be used
to detect that something went wrong in the set_ftlb_enable() function
either because that function knows nothing about the running core, or
simply because the core can't turn its FTLB on/off.

The CM cache error reporting code is not Malta specific and as such it
should live in the mips-cm.c file. Moreover, CM2 and CM3 differ in the
way cache errors are being recorded to the registers so extend the
previous code to add support for the CM3 as well.

Previously, the CM accessors were only accessing CM registers as u32
types instead of using the native CM register with. However, newer CMs
may actually be 64-bit on MIPS64 cores. Fortunately, current 64-bit CMs
(CM3) hold all the useful configuration bits in the lower half of the
64-bit registers (at least most of them) so they can still be accessed
using the current 32-bit accessors.

The genex.S file appears to mix the case of a macro between its definition and
use. A cut down example of this is below. The macro __build_clear_none has
lower case 'build' but ends up being instantiated with upper case BUILD. Can
this be fixed on master. It has been picked up by the LLVM integrated assembler
which is currently case sensitive. We are likely to fix the assembler as well
but the code is currently inconsistent in the kernel.

Commit 4c21b8fd8f ("MIPS: seccomp: Handle indirect system calls (o32)")
fixed indirect system calls on O32 but it also introduced a bug for MIPS64
where it erroneously modified the v0 (syscall) register with the assumption
that the sycall offset hasn't been taken into consideration. This breaks
seccomp on MIPS64 n64 and n32 ABIs. We fix this by replacing the addition
with a move instruction.

"Another round of MIPS fixes for 4.2. No area does particularly stand
out but we have a two unpleasant ones:
- Kernel ptes are marked with a global bit which allows the kernel to
share kernel TLB entries between all processes. For this to work
both entries of an adjacent even/odd pte pair need to have the
global bit set. There has been a subtle race in setting the other
entry's global bit since ~ 2000 but it take particularly
pathological workloads that essentially do mostly vmalloc/vfree to
trigger this.
This pull request fixes the 64-bit case but leaves the case of 32
bit CPUs with 64 bit ptes unsolved for now. The unfixed cases
affect hardware that is not available in the field yet.
- Instruction emulation requires loading instructions from user space
but the current fast but simplistic approach will fail on pages
that are PROT_EXEC but !PROT_READ. For this reason we temporarily
do not permit this permission and will map pages with PROT_EXEC |
PROT_READ.
The remainder of this pull request is more or less across the field

This function can leak kernel stack data when the user siginfo_t has a
positive si_code value. The top 16 bits of si_code descibe which fields
in the siginfo_t union are active, but they are treated inconsistently
between copy_siginfo_from_user32, copy_siginfo_to_user32 and
copy_siginfo_to_user.
copy_siginfo_from_user32 is called from rt_sigqueueinfo and
rt_tgsigqueueinfo in which the user has full control overthe top 16 bits
of si_code.
This fixes the following information leaks:
x86: 8 bytes leaked when sending a signal from a 32-bit process to
itself. This leak grows to 16 bytes if the process uses x32.
(si_code = __SI_CHLD)
x86: 100 bytes leaked when sending a signal from a 32-bit process to
a 64-bit process. (si_code = -1)
sparc: 4 bytes leaked when sending a signal from a 32-bit process to a
64-bit process. (si_code = any)
parsic and s390 have similar bugs, but they are not vulnerable because
rt_[tg]sigqueueinfo have checks that prevent sending a positive si_code
to a different process. These bugs are also fixed for consistency.

Since we've already stepped away from ENABLE is a JMP and DISABLE is a
NOP with the branch_default bits, and are going to make it even worse,
rename it to make it all clearer.
This way we don't mix multiple levels of logic attributes, but have a
plain 'physical' name for what the current instruction patching status
of a jump label is.
This is a first step in removing the naming confusion that has led to
a stream of avoidable bugs such as:

The show_stack() function deals exclusively with kernel contexts, but if
it gets called in user context with EVA enabled, show_stacktrace() will
attempt to access the stack using EVA accesses, which will either read
other user mapped data, or more likely cause an exception which will be
handled by __get_user().
This is easily reproduced using SysRq t to show all task states, which
results in the following stack dump output:

Fix by setting the current user access mode to kernel around the call to
show_stacktrace(). This causes __get_user() to use normal loads to read
the kernel stack.
Now we get the correct output, like this:

If a machine check exception is raised in kernel mode, user context,
with EVA enabled, then the do_mcheck handler will attempt to read the
code around the EPC using EVA load instructions, i.e. as if the reads
were from user mode. This will either read random user data if the
process has anything mapped at the same address, or it will cause an
exception which is handled by __get_user, resulting in this output:
Code: (Bad address in epc)
Fix by setting the current user access mode to kernel if the saved
register context indicates the exception was taken in kernel mode. This
causes __get_user to use normal loads to read the kernel code.

The majority of SMP platforms handle their IPIs through do_IRQ()
which calls irq_{enter/exit}(). When a call function IPI is received,
smp_call_function_interrupt() is called which also calls
irq_{enter,exit}(), meaning irq_count is raised twice.
When tick broadcasting is used (which is implemented via a call
function IPI), this incorrectly causes all CPU idle time on the core
receiving broadcast ticks to be accounted as time spent servicing
IRQs, as account_process_tick() will account as such if irq_count is
greater than 1. This results in 100% CPU usage being reported on a
core which receives its ticks via broadcast.
This patch removes the SMP smp_call_function_interrupt() wrapper which
calls irq_{enter,exit}(). Platforms which handle their IPIs through
do_IRQ() now call generic_smp_call_function_interrupt() directly to
avoid incrementing irq_count a second time. Platforms which don't
(loongson, sgi-ip27, sibyte) call generic_smp_call_function_interrupt()
wrapped in irq_{enter,exit}().

p->thread.user_cpus_allowed is zero-initialized and is only filled on
the first sched_setaffinity call.
To avoid adding overhead in the task initialization codepath, simply OR
the returned mask in sched_getaffinity with p->cpus_allowed.

We were using the native syscall and that results in subtle breakage.
This is the same issue as fixed in 077d0e6561
(MIPS: N32: Use compat getsockopt syscall) but that commit did fix it only
for N32.

MT_SMP is not the only SMP option for MT cores. The MT_SMP option
allows more than one VPE per core to appear as a secondary CPU in the
system. Because of how CM works, it propagates the address-based
cache ops to the secondary cores but not the index-based ones.
Because of that, the code does not use IPIs to flush the L1 caches on
secondary cores because the CM would have done that already. However,
the CM functionality is independent of the type of SMP kernel so even in
non-MT kernels, IPIs are not necessary. As a result of which, we change
the conditional to depend on the CM presence. Moreover, since VPEs on
the same core share the same L1 caches, there is no need to send an
IPI on all of them so we calculate a suitable cpumask with only one
VPE per core.

The cps-vec code assumes O32 ABI and uses t4-t7 in quite a few places. This
breaks the build on 64-bit. As a result of which, use the pseudo-registers
ta0-ta3 to make the code compatible with 64-bit.

The PTR_LA macro will pick the correct "la" or "dla" macro to
load an address to a register. This gets rids of the following

Commit 1d8f1f5a78 ("MIPS: smp-cps: hotplug support") added hotplug
support in the SMP/CPS implementation but it introduced a few build problems
on 64-bit kernels due to pointer being casted to and from 'int' C types. We
fix this problem by using 'unsigned long' instead which should match the size
of the pointers in 32/64-bit kernels. Finally, we fix the comment since the
CM base address is loaded to v1($3) instead of v0.

Commits f1b44067c1 ("MIPS: Emulate the
new MIPS R6 B{L,G}T{Z,}{AL,}C instructions") and commit
a8ff66f52d ("MIPS: Emulate the new MIPS
R6 B{L,G}E{Z,}{AL,}C instructions") added support for emulating various
branch compact instructions. However, it missed the case for those which
use the old BLEZL and BGTZL opcodes leading to random crashes when the R6
emulator is disabled. We fix this by ensuring that the 'rt' field is not
zero which is always true for these branch compact instructions.

Commit f9a7febd leads to a fact that mapstart and therefore a page bitmap for
bootmem allocator immediately follows initrd_end. This doesn't always work
well on Octeon, where there are holes in PFN ranges (refer to 5b3b1688 and
4MB-aligned PFN allocation). Depending on the inird location it could happen,
that mapstart would be in an area not allocated by plat_mem_setup() in
arch/mips/cavium-octeon/setup.c, but in the alignment hole between initrd and
the next PFN area. Later on this memory will be unconditionally made available
to buddy allocator at the end of free_all_bootmem_core() (mm/bootmem.c).
All of this results in Linux using the memory not designated for Linux in
Octeon's plat_mem_setup(), which in turn means corruption of the memory used
by another OS/baremetal code on the same SoC.
It doesn't look to me as a problem of Octeon platform code, but rather as an
inability of f9a7febd to deal correctly with the fragmented memory-mappings.
Proposed fix moves the check for initrd address to the same calculation-loop
in bootmem_init() (arch/mips/kernel/setup.c), which also accounts for kernel
code location. This should result in mapstart located starting from the first
PFN area after kernel code AND initrd.

Commit 46e12c07b3 (MIPS: O32 / 32-bit:
Always copy 4 stack arguments.) change the O32 syscall handler to always
load four arguments from the userspace stack even for syscalls that
require fewer or no arguments to be copied. This removes a large table
from kernel space and need to maintain it. It appeared that it was ok
the implementation chosen requires 16 bytes of readable stack space
above the user stack pointer.
Turned out a few threading implementations munmap the user stack before
the thread exits resulting in errors due to the unreadable stack.
We now treat any failed load as a if the loaded value was zero and let
the actual syscall deal with the situation.

allocate_irqno, free_irqno and alloc_legacy_irqno are a simple allocator
for interrupt numbers from the days when the numer of interrupts was still
fixed to NR_IRQS. This was necessary for the SGI IP27 architecture which
with its flexible architecture and possibly large number of interrupts
doesn't easily fit into the old pattern. These days there are better
alternatives.
Move the allocation code from the arch generic code to the only platform
using it, the SGI IP27 aka Origin 200/2000, Onyx 2.

Add support for detecting a vmlinux.bin appended dtb and overriding
the boot arguments to match the UHI interface.
Due to the PERCPU section being empty for !SMP, but still modifying
the current address by aligning it to the page size, do not define
it for !SMP builds to allow __appended_dtb to still point to
the actual end of the data.

In preparation to allow users to enable DeviceTree without arch or
machine selecting it, we need to fix build errors on MIPS. When
CONFIG_OF is enabled, device_tree_init cannot be resolved. This is
trivially fixed by using CONFIG_USE_OF instead of CONFIG_OF for prom.h.

The R12000 added a new feature to enhance branch prediction called
"global history". Per the Vr10000 Series User Manual (U10278EJ4V0UM),

Commit 854700115ecf ([MIPS] kgdb: add arch support for the kernel's kgdb core)
added the 'kgdb_early_setup' flag to avoid calling trap_init() and init_IRQ()
the second time, however the code that called these functions earlier, from
kgdb_arch_init(), had been already removed by that time, so the flag never
served any useful purpose. Remove the related code along with ugly #ifdef'ery
at last.

Ingenic have actually varied the vendor/company ID of the XBurst cores
across their range of SoCs, whilst keeping the product ID & revision
constant... Add definitions for vendor IDs known to be used in some of
Ingenic's newer SoCs, and handle them in the same way as the existing
Ingenic vendor ID from the JZ4740.

Commit 334c86c494 ("MIPS: IRQ: Add stackoverflow detection") added
kernel stack overflow detection, however it only enabled it conditional
upon the preprocessor definition DEBUG_STACKOVERFLOW, which is never
actually defined. The Kconfig option is called DEBUG_STACKOVERFLOW,
which manifests to the preprocessor as CONFIG_DEBUG_STACKOVERFLOW, so
switch it to using that definition instead.

Use the generic mechanism to declare a bitmap instead of unsigned long.
This could fix an overwrite defect of whatever follows irq_map.
Not all "#define NR_IRQS <value>" are a multiple of BITS_PER_LONG so
using DECLARE_BITMAP allocates the proper number of longs required
for the possible bits.

cpu_set was removed (along with a bunch of cpumask helpers) by
commit 2f0f267ea0 ("cpumask: remove deprecated functions.").
Fix this by replacing cpu_set with cpumask_set_cpu. Without this
fix the following error is triggered when CONFIG_MIPS_MT_FPAFF=y.

Fix "BUG: using smp_processor_id() in preemptible" reported in accesses
to thread's FPU defaults: the value to initialise FSCR to at program
startup, the FCSR r/w mask and the contents of FIR in full FPU
emulation, removing a regression introduced with 9b26616c [MIPS: Respect
the ISA level in FCSR handling] and f6843626 [MIPS: math-emu: Set FIR
feature flags for full emulation].
Use `boot_cpu_data' to obtain the data from, following the approach that
`cpu_has_*' macros take and avoiding the call to `smp_processor_id' made
in the reference to `current_cpu_data'. The contents of FSCR have to be
consistent across processors in an SMP system, the settings there must
not change as a thread is migrated across processors. And the contents
of FIR are guaranteed to be consistent in FPU emulation, by definition.

Commit 46490b5725 ("MIPS: kernel: elf: Improve the overall ABI and FPU
mode checks") reworked the ELF FP ABI mode selection logic, but when
CONFIG_MIPS_O32_FP64_SUPPORT is enabled it breaks the use of binaries
which have no PT_MIPS_ABIFLAGS program header & associated
.MIPS.abiflags section.
A default mode is selected based upon whether the ELF contains MIPS32 or
MIPS64 code, but that selection is made in arch_elf_pt_proc.
arch_elf_pt_proc only executes when a PT_MIPS_ABIFLAGS program header is
found. If one is not found then arch_elf_pt_proc is never called, and no
default overall_fp_mode value is selected. When arch_check_elf is
called, both abi0 & abi1 are MIPS_ABI_FP_UNKNOWN which leads to both
prog_req & interp_req being set to none_req. none_req matches none of
the conditions for mode selection at the end of arch_check_elf, so
overall_fp_mode is left untouched. Finally once mips_set_personality_fp
is called the BUG() in the default case is then hit & the kernel likely
panics.
Fix this by moving the selection of a default overall mode to the start
of arch_check_elf, which runs once per ELF executed regardless of
whether it has a PT_MIPS_ABIFLAGS program header.

Commit 6ebb496ffc7e("MIPS: kernel: entry.S: Add MIPS R6 related
definitions") added the MIPSR6 definition but it did not update the
ISA level of the actual assembly code so a pre-MIPSR6 jr.hb instruction
was generated instead. Fix this by using the MISP_ISA_LEVEL_RAW macro.

When emulating a regular lh/lw/lhu/sh/sw we need to use the appropriate
instruction if we are in EVA mode. This is necessary for userspace
applications which trigger alignment exceptions. In such case, the
userspace load/store instruction needs to be emulated with the correct
eva/non-eva instruction by the kernel emulator.

Commit c1771216ab ("MIPS: kernel: unaligned: Handle unaligned
accesses for EVA") allowed unaligned accesses to be emulated for
EVA. However, when emulating regular load/store unaligned accesses,
we need to use the appropriate "address space" instructions for that.
Previously, an unaligned load/store instruction in kernel space would
have used the corresponding EVA instructions to emulate it which led to
segmentation faults because of the address translation that happens
with EVA instructions. This is now fixed by using the EVA instruction
only when emulating EVA unaligned accesses.

Define the central place the default FCSR value is set from, initialised
in `cpu_probe'. Determine the FCSR mask applied to values written to
the register with CTC1 in the full emulation mode and via ptrace(2),
according to the ISA level of processor hardware or the writability of
bits 31:18 if actual FPU hardware is used.
Software may rely on FCSR bits whose functions our emulator does not
implement, so it should not allow them to be set or software may get
confused. For ptrace(2) it's just sanity.

Implement FIR feature flags in the FPU emulator according to features
supported and architecture level requirements. The W, L and F64 bits
have only been added at level #2 even though the features they refer to
were also included with the MIPS64r1 ISA and the W fixed-point format
also with the MIPS32r1 ISA.
This is only relevant for the full emulation mode and the emulated CFC1
instruction as well as ptrace(2) accesses.

Correct an ISA level determination problem introduced with 8b8aa636
[MIPS: kernel: cpu-probe.c: Add support for MIPS R6], reverting explicit
masking against individual `MIPS_CPU_ISA_*' macros in FPU feature
determination.
Feature macros such as `cpu_has_mips_r' cannot be used here, because
they operate on CPU #0 and we want to refer to the current CPU instead.
They cannot be used for masking against the current CPU either because
they mask against CPU #0 too, e.g.:

Rework `process_fpemu_return' and move IEEE 754 exception interpretation
there, from `do_fpe'. Record the cause bits set in FCSR before they are
cleared and pass them through to `process_fpemu_return' so as to set
`si_code' correctly too for SIGFPE signals sent from emulation rather
than those issued by hardware with the FPE processor exception only.
For simplicity `mipsr2_decoder' assumes `*fcr31' has been preinitialised
and only sets it to anything if an FPU instruction has been emulated,
which in turn is the only case SIGFPE can be issued for here.

Clear any FCSR cause bits recorded in the saved FPU context after
emulation in all cases rather than in `do_fpe' only, so that any
unmasked IEEE 754 exception left from emulation does not cause a fatal
kernel-mode FPE hardware exception with the CTC1 instruction used by the
kernel to subsequently restore FCSR hardware from the saved FPU context.

Respect the FCSR exception mask when interpreting the IEEE 754 exception
condition to report with SIGFPE in `si_code', so as not to use one that
has been masked where a different one set in parallel caused the FPE
hardware exception to trigger. As per the IEEE Std 754 the Inexact
exception can happen together with Overflow or Underflow.

Changes applied to `do_cpu' over time reduced the use of the SIGILL
issued with `force_sig' at the end to a single CU3 case only in the
switch statement there. Move that `force_sig' call over to right where
required then and toss out the pile of gotos now not needed to skip over
the call, replacing them with regular breaks out of the switch.

This allows the kernel to correctly detect an R16000 MIPS CPU on systems that
have those. Otherwise, such systems will detect the CPU as an R14000, due to
similarities in the CPU PRId value.

If a machine-specific hook is not implemented for restart, poweroff,
or halt, fall back to halting secondary CPUs, disabling interrupts,
and spinning. In the case of restart, attempt to restart the system
via do_kernel_restart() (which will call any registered restart
handlers) before halting.

Since cpu_wait() enables interrupts upon return, CPUs which have
entered stop_this_cpu() may still end up handling interrupts.
This can lead to the softlockup detector firing on a panic or
restart/poweroff/halt. Just disable interrupts and spin to ensure
nothing else runs on the CPU once it has entered stop_this_cpu().

Remove a cast to the 'mips16e_instruction' union inside an if
condition and instead do an assignment to a local
'union mips16e_instruction' variable's 'full' member before the if
statement and use this variable in the if condition.

On certain cores (namely proAptiv and P5600) incoming data via a Fast
Debug Channel (FDC) while the core is blocked on a wait instruction will
cause the wait not to wake up even when another interrupt is received.
This makes an idle target stop as soon as you send FDC data to it, until
the debug probe interrupts it and restarts the wait instruction.
This is worked around by avoiding using r4k_wait on these cores if
CONFIG_MIPS_EJTAG_FDC_TTY is enabled (which would imply the user intends
to use the FDC).

Add architectural definitions and probing for the MIPS Common Device
Memory Map (CDMM) region. When supported and enabled at a particular
physical address, this region allows some number of per-CPU devices to
be discovered and controlled via MMIO.
A bit exists in Config3 to determine whether the feature is present, and
a CDMMBase CP0 register allows the region to be enabled at a particular
physical address.

Before release 2 of the architecture there weren't separate interrupt
pending bits for the local CPU interrupts (timer & perf counter
overflow), so when they were connected to the same interrupt line the
timer handler had to call the performance counter handler before knowing
whether a timer interrupt was actually pending.
Now another CPU local interrupt, for the Fast Debug Channel (FDC), can
also be routed to an arbitrary interrupt line. It isn't scalable to keep
adding cross-calls between handlers for these cases of shared interrupt
lines, especially since the FDC could in theory share its interrupt line
with the performance counter, timer, or both.
Fortunately since release 2 of the architecture separate interrupt
pending bits do exist in the Cause register. This allows local
interrupts which share an interrupt line to have separate handlers using
IRQF_SHARED. Unfortunately they can't easily have their own irqchip as
there is no generic way to individually mask them.
Enable this sharing to happen by removing the special case for when the
perf count shares an IRQ with the timer. cp0_perfcount_irq and
cp0_compare_irq can then be set to the same value with shared interrupt
handlers registered for both of them.
Pre-R2 code should be unaffected. cp0_perfcount_irq will always be -1
and the timer handler will contnue to call into the perf counter
handler.

When requesting the performance counter overflow interrupt, pass flags
which are compatible with the cevt-r4k driver, in particular
IRQF_SHARED so that the two handlers can share the same IRQ. This is
possible since release 2 of the architecture where there are separate
pending interrupt bits for the timer interrupt and the performance
counter interrupt.
This will be necessary since the FDC interrupt can also be arbitrarily
routed to a CPU interrupt, possibly sharing with the timer, the
performance counters, or both, and it isn't scalable to have all the
handlers able to call other handlers that may be on the same IRQ line.
Shared handlers must also have a unique device pointer so they can be
individually removed, so &mipspmu is now passed in for that instead of
NULL.

Make the cevt-r4k interrupt handler shared so that other interrupt
handlers (specifically the performance counter overflow handler and fast
debug channel interrupt handler) can share the same interrupt line.
This simply imvolves returning IRQ_NONE when no timer interrupt has been
handled to allow other handlers to run, and passing IRQF_SHARED when
setting up the IRQ handler so that other handlers (with compatible
flags) can be registered.

The situation where the timer interrupt is on the same line as the
performance counter interrupt is handled in per_cpu_trap_init() by
setting cp0_perfcount_irq to -1, so there is no need to duplicate the
logic conditional upon cp0_perfcount_irq >= 0 in perf
(init_hw_perf_events()) and oprofile (mipsxx_init()).

Long ago, commit 8531a35e5e ("[MIPS] SMTC: Fix SMTC dyntick support.")
moved handle_perf_irq() out of cevt-r4k.c into a header so it could be
shared with cevt-smtc.c.
Slightly less long ago, commit b633648c5a ("MIPS: MT: Remove SMTC
support") removed all traces of SMTC support, including cevt-smtc.c,
leaving cevt-r4k.c once again the sole user of handle_perf_irq(),
therefore move it back into cevt-r4k.c from the header.

Add base code for supporting the MIPS SIMD Architecture (MSA) in MIPS
KVM guests. MSA cannot yet be enabled in the guest, we're just laying
the groundwork.
As with the FPU, whether the guest's MSA context is loaded is stored in
another bit in the fpu_inuse vcpu member. This allows MSA to be disabled
when the guest disables it, but keeping the MSA context loaded so it
doesn't have to be reloaded if the guest re-enables it.
New assembly code is added for saving and restoring the MSA context,
restoring only the upper half of the MSA context (for if the FPU context
is already loaded) and for saving/clearing and restoring MSACSR (which
can itself cause an MSA FP exception depending on the value). The MSACSR
is restored before returning to the guest if MSA is already enabled, and
the existing FP exception die notifier is extended to catch the possible
MSA FP exception and step over the ctcmsa instruction.
The helper function kvm_own_msa() is added to enable MSA and restore
the MSA context if it isn't already loaded, which will be used in a
later patch when the guest attempts to use MSA for the first time and
triggers an MSA disabled exception.
The existing FPU helpers are extended to handle MSA. kvm_lose_fpu()
saves the full MSA context if it is loaded (which includes the FPU
context) and both kvm_lose_fpu() and kvm_drop_fpu() disable MSA.
kvm_own_fpu() also needs to lose any MSA context if FR=0, since there
would be a risk of getting reserved instruction exceptions if CU1 is
enabled and we later try and save the MSA context. We shouldn't usually
hit this case since it will be handled when emulating CU1 changes,
however there's nothing to stop the guest modifying the Status register
directly via the comm page, which will cause this case to get hit.

When handling floating point exceptions (FPEs) and MSA FPEs the Cause
bits of the appropriate control and status register (FCSR for FPEs and
MSACSR for MSA FPEs) are read and cleared before enabling interrupts,
presumably so that it doesn't have to go through the pain of restoring
those bits if the process is pre-empted, since writing those bits would
cause another immediate exception while still in the kernel.
The bits aren't normally ever restored again, since userland never
expects to see them set.
However for virtualisation it is necessary for the kernel to be able to
restore these Cause bits, as the guest may have been interrupted in an
FP exception handler but before it could read the Cause bits. This can
be done by registering a die notifier, to get notified of the exception
when such a value is restored, and if the PC was at the instruction
which is used to restore the guest state, the handler can step over it
and continue execution. The Cause bits can then remain set without
causing further exceptions.
For this to work safely a few changes are made:
- __build_clear_fpe and __build_clear_msa_fpe no longer clear the Cause
bits, and now return from exception level with interrupts disabled
instead of enabled.
- do_fpe() now clears the Cause bits and enables interrupts after
notify_die() is called, so that the notifier can chose to return from
exception without this happening.
- do_msa_fpe() acts similarly, but now actually makes use of the second
argument (msacsr) and calls notify_die() with the new DIE_MSAFP,
allowing die notifiers to be informed of MSA FPEs too.

The basic premise of the patch was incorrect since MSA context
(including FP state) is saved using st.d which stores two consecutive
64-bit words in memory rather than a single 128-bit word. This means
that even with big endian MSA, the FP state is still in the first 64-bit
word.

If a ptracee has not used the FPU and the ptracer sets its FP context
using PTRACE_POKEUSR, PTRACE_SETFPREGS or PTRACE_SETREGSET then that
context will be discarded upon either the ptracee using the FPU or a
further write to the context via ptrace. Prevent this loss by recording
that the task has "used" math once its FP context has been written to.
The context initialisation code that was present for the PTRACE_POKEUSR
case is reused for the other 2 cases to provide consistent behaviour
for the different ptrace requests.

When running the emulator to handle an instruction that raised an FP
unimplemented operation exception, the FCSR cause bits were being
cleared. This is done to ensure that the kernel does not take an FP
exception when later restoring FP context to registers. However, this
was not being done when the emulator is invoked in response to a
coprocessor unusable exception. This happens in 2 cases:
- There is no FPU present in the system. In this case things were
OK, since the FP context is never restored to hardware registers
and thus no FP exception may be raised when restoring FCSR.
- The FPU could not be configured to the mode required by the task.
In this case it would be possible for the emulator to set cause
bits which are later restored to hardware if the task migrates
to a CPU whose associated FPU does support its mode requirements,
or if the tasks FP mode requirements change.
Consistently clear the cause bits after invoking the emulator, by moving
the clearing to process_fpemu_return and ensuring this is always called
before the tasks FP context is restored. This will make it easier to
catch further paths invoking the emulator in future, as will be
introduced in further patches.

Much like for traditional scalar FP exceptions, the cause bits in the
MSACSR register need to be cleared following an MSA FP exception.
Without doing so the exception will simply be raised again whenever
the kernel restores MSACSR from a tasks saved context, leading to
undesirable spurious exceptions. Clear the cause bits from the
handle_msa_fpe function, mirroring the way handle_fpe clears the
cause bits in FCSR.

The {save,restore}_fp_context{,32} functions require that the assembler
allows the use of sdc instructions on any FP register, and this is
acomplished by setting the arch to mips64r2 or mips64r6
(using MIPS_ISA_ARCH_LEVEL_RAW).
However this has the effect of enabling the assembler to use mips64
instructions in the expansion of pseudo-instructions. This was done in
the (now-reverted) commit eec43a224c "MIPS: Save/restore MSA context
around signals" which led to my mistakenly believing that there was an
assembler bug, when in reality the assembler was just emitting mips64
instructions. Avoid the issue for future commits which will add code to
r4k_fpu.S by pushing the .set MIPS_ISA_ARCH_LEVEL_RAW directives into
the functions that require it, and remove the spurious assertion
declaring the assembler bug.

Add support for extended physical addressing (XPA) so that
32-bit platforms can access equal to or greater than 40 bits
of physical addresses.
NOTE:
1) XPA and EVA are not the same and cannot be used
simultaneously.
2) If you configure your kernel for XPA, the PTEs
and all address sizes become 64-bit.
3) Your platform MUST have working HIGHMEM support.
