Delete legacy hack for broken bootloaders. The warning has been in kernel
for several years, and if there are still users using such bootloaders,
they can fix the boot by supplying a proper DTB.

CN68XX requires a different PKO configuration. This patch provides

Configure the pkinds of XAUI interfaces on Octeon models that have
them. This simple configuration uses 1:1 mapping between the PIP input
port number and the selected pkind.

Some CN68XX series Octeon II chips seem to hang if a reset is issued on
XAUI initialization. Avoid the hang by disabling the reset on affected
models. Tested on Cavium EBB6800 evaluation board and Kontron S1901 board.

Reintroduce run-time check for crypto features. The old one was deleted

This prevents PHY not found types of errors for PHY drivers that are
probed after the Ethernet driver is probed, because the ifconfig UP is
done from userspace after all drivers have been probed.
Also avoid the cvmx-helper-board.c PHY code if a real PHY driver is
present, this allows a bootloader supplied device tree to specify the
PHY information rather than having to modify the code for each
different board.

Add interface mode detection for Octeon II. This is necessary to detect
the interface modes correctly on the UBNT E200 board. Code is taken
from the UBNT GPL source release, with some alterations: SRIO, ILK and
RXAUI interface modes are removed and instead return disabled as these
modes are not currently supported.

When booting with in-kernel DTBs, the pruning code will enumerate
interfaces 0-4. However, there is memory reserved only for 4 so some
other data will get overwritten by cvmx_helper_interface_enumerate().

Enable interfaces on EdgeRouter Lite. Tested with cavium_octeon_defconfig
and busybox shell. DHCP & ping works with eth0, eth1 and eth2.
The board type "UBNT_E100" is taken from the sources of the vendor kernel
shipped with the product.

Cong Ding <dinggnu@gmail.com> reports correctly that the variable dummy
is being used without initialization. That said, I can't reproduce this
warning with GCC 4.7.1. However, since the variable dummy servces no
real purpose, I'm going for a different fix. This fix
includes https://patchwork.linux-mips.org/patch/4801/ plus Geert's
suggestion to use ACCESS_ONCE().

The "else clause" of most functions in bitops.h invoked
raw_local_irq_{save,restore}() and in doing so had a dependency on
irqflags.h. This fix moves said code to bitops.c, removing the
dependency.
