Matthew Wilcox (Oracle) 5f7136db82 block: Add bio_max_segs
It's often inconvenient to use BIO_MAX_PAGES due to min() requiring the
sign to be the same.  Introduce bio_max_segs() and change BIO_MAX_PAGES to
be unsigned to make it easier for the users.

Gustavo A. R. Silva b18ae8dd9d dm: replace zero-length array with flexible-array
The current codebase makes use of the zero-length array language
extension to the C90 standard, but the preferred mechanism to declare
variable-length types such as these ones is a flexible array member[1][2],
introduced in C99:

};

By making use of the mechanism above, we will get a compiler warning
in case the flexible array does not occur last in the structure, which
will help us prevent some kind of undefined behavior bugs from being
inadvertently introduced[3] to the codebase from now on.

Also, notice that, dynamic memory allocations won't be affected by
this change:

"Flexible array members have incomplete type, and so the sizeof operator
may not be applied. As a quirk of the original implementation of
zero-length arrays, sizeof evaluates to zero."[1]

sizeof(flexible-array-member) triggers a warning because flexible array
members have incomplete type[1]. There are some instances of code in
which the sizeof operator is being incorrectly/erroneously applied to
zero-length arrays and the result is zero. Such instances may be hiding
some bugs. So, this work (flexible-array member conversions) will also
help to get completely rid of those sorts of issues.

This issue was found with the help of Coccinelle.

Currently, although we submit super bios in order (and super.nr_entries
is incremented by each logged entry), submit_bio() is async so each
super sector may not be written to log device in order and then the
final nr_entries may be smaller than it should be.

This problem can be reproduced by the xfstests generic/455 with ext4:

  QA output created by 455
 -Silence is golden
 +mark 'end' does not exist

Fix this by serializing submission of super sectors to make sure each
is written to the log disk in order.

Dan Williams b3a9a0c36e dax: Introduce a ->copy_to_iter dax operation
Similar to the ->copy_from_iter() operation, a platform may want to
deploy an architecture or device specific routine for handling reads
from a dax_device like /dev/pmemX. On x86 this routine will point to a
machine check safe version of copy_to_iter(). For now, add the plumbing
to device-mapper and the dax core.

  Half of the branch up to commit d2c997c0f1 ("fs, dax: use
  page->mapping to warn...") have been in -next for several releases.
  The of_pmem driver and the address range scrub rework were late
  arrivals, and the dax work was scaled back at the last moment.

  The of_pmem driver missed a previous merge window due to an oversight.
  A sense of obligation to rectify that miss is why it is included for
  4.17. It has acks from PowerPC folks. Stephen reported a build failure
  that only occurs when merging it with your latest tree, for now I have
  fixed that up by disabling modular builds of of_pmem. A test merge
  with your tree has received a build success report from the 0day robot
  over 156 configs.

  An initial version of the ARS rework was submitted before the merge
  window. It is self contained to libnvdimm, a net code reduction, and
  passing all unit tests.

  The filesystem-dax changes are based on the wait_var_event()
  functionality from tip/sched/core. However, late review feedback
  showed that those changes regressed truncate performance to a large
  degree. The branch was rewound to drop the truncate behavior change
  and now only includes preparation patches and cleanups (with full acks
  and reviews). The finalization of this dax-dma-vs-trnucate work will
  need to wait for 4.18.

  Summary:

Mike Snitzer 5bd5e8d891 dm: remove fmode_t argument from .prepare_ioctl hook
Use the fmode_t that is passed to dm_blk_ioctl() rather than
inconsistently (varies across targets) drop it on the floor by
overriding it with the fmode_t stored in 'struct dm_dev'.

All the persistent reservation functions weren't using the fmode_t they
got back from .prepare_ioctl so remove them.

Dan Williams 976431b02c dax, dm: allow device-mapper to operate without dax support
Change device-mapper's DAX dependency to require the presence of at
least one DAX_DRIVER. This allows device-mapper to be built without
bringing the DAX core along which is especially wasteful when there are
no DAX drivers, like BLK_DEV_PMEM, configured.

Ross Zwisler 98d82f48f1 dm log writes: add support for DAX
Now that we have the ability log filesystem writes using a flat buffer, add
support for DAX.

The motivation for this support is the need for an xfstest that can test
the new MAP_SYNC DAX flag.  By logging the filesystem activity with
dm-log-writes we can show that the MAP_SYNC page faults are writing out
their metadata as they happen, instead of requiring an explicit
msync/fsync.

Unfortunately we can't easily track data that has been written via
mmap() now that the dax_flush() abstraction was removed by commit
c3ca015fab ("dax: remove the pmem_dax_ops->flush abstraction").
Otherwise we could just treat each flush as a big write, and store the
data that is being synced to media.  It may be worthwhile to add the
dax_flush() entry point back, just as a notifier so we can do this
logging.

Ross Zwisler e5a20660a1 dm log writes: add support for inline data buffers
Currently dm-log-writes supports writing filesystem data via BIOs, and
writing internal metadata from a flat buffer via write_metadata().

For DAX writes, though, we won't have a BIO, but will instead have an
iterator that we'll want to use to fill a flat data buffer.

So, create write_inline_data() which allows us to write filesystem data
using a flat buffer as a source, and wire it up in log_one_block().

Josef Bacik 228bb5b260 dm log writes: fix >512b sectorsize support
512b sectors vs device's physical sectorsize was not maintained
consistently and as such the support for >512b sector devices has bugs.
The log metadata expects native sectorsize but 512b sectors were being
stored.  Also, device's sectorsize was assumed when assigning the
bi_sector for blocks that were being logged.

Fix this up by adding two helpers to convert between bio and dev
sectors, and use these in the appropriate places to fix the problem and
make it clear which units go where.  Doing so allows dm-log-writes use
with 4k devices.

Josef Bacik 0c79c62021 dm log writes: don't use all the cpu while waiting to log blocks
The check to see if the logging kthread needs to go to sleep is wrong,
it checks lc->pending_blocks, which will be non-0 if there are any
blocks that are pending, whether they are ready to be logged or not.
What we really want is to go to sleep until it's time to log blocks, so
change this check so we do actually go to sleep in between flushes.

Christoph Hellwig 74d46992e0 block: replace bi_bdev with a gendisk pointer and partitions index
This way we don't need a block_device structure to submit I/O.  The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open.  Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).

For the actual I/O path all that we need is the gendisk, which exists
once per block device.  But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.

Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

Christoph Hellwig 846785e6a5 dm: don't return errnos from ->map
Instead use the special DM_MAPIO_KILL return value to return -EIO just
like we do for the request based path.  Note that dm-log-writes returned
-ENOMEM in a few places, which now becomes -EIO instead.  No consumer
treats -ENOMEM special so this shouldn't be an issue (and it should
use a mempool to start with to make guaranteed progress).

Mikulas Patocka 7efb367320 dm log writes: fix bug with too large bios
bio_alloc() can allocate a bio with at most BIO_MAX_PAGES (256) vector
entries.  However, the incoming bio may have more vector entries if it
was allocated by other means.  For example, bcache submits bios with
more than BIO_MAX_PAGES entries.  This results in bio_alloc() failure.

To avoid the failure, change the code so that it allocates bio with at
most BIO_MAX_PAGES entries.  If the incoming bio has more entries,
bio_add_page() will fail and a new bio will be allocated - the code that
handles bio_add_page() failure already exists in the dm-log-writes
target.

Mikulas Patocka a5d60783df dm log writes: move IO accounting earlier to fix error path
Move log_one_block()'s atomic_inc(&lc->io_blocks) before bio_alloc() to
fix a bug that the target hangs if bio_alloc() fails.  The error path
does put_io_block(lc), so atomic_inc(&lc->io_blocks) must occur before
invoking the error path to avoid underflow of lc->io_blocks.

Jens Axboe 1eff9d322a block: rename bio bi_rw to bi_opf
Since commit 63a4cc2486, bio->bi_rw contains flags in the lower
portion and the op code in the higher portions. This means that
old code that relies on manually setting bi_rw is most likely
going to be broken. Instead of letting that brokeness linger,
rename the member, to force old and out-of-tree code to break
at compile time instead of at runtime.

No intended functional changes in this commit.

Christoph Hellwig e56f81e0b0 dm: refactor ioctl handling
This moves the call to blkdev_ioctl and the argument checking to DM core
code, and only leaves a callout to find the block device to operate on
in the targets.  This simplifies the code and allows us to pass through
ioctl-like command using other methods in the next patch.

Also split out a helper around calling the prepare_ioctl method that
will be reused for persistent reservation handling.

Kent Overstreet 8ae126660f block: kill merge_bvec_fn() completely
As generic_make_request() is now able to handle arbitrarily sized bios,

Vivek Goyal e80d1c805a dm: do not override error code returned from dm_get_device()
Some of the device mapper targets override the error code returned by
dm_get_device() and return either -EINVAL or -ENXIO.  There is nothing
gained by this override.  It is better to propagate the returned error
code unchanged to caller.

This work was motivated by hitting an issue where the underlying device
was busy but -EINVAL was being returned.  After this change we get
-EBUSY instead and it is easier to figure out the problem.

Christoph Hellwig 4246a0b63b block: add a bi_error field to struct bio
Currently we have two different ways to signal an I/O error on a BIO:

 (1) by clearing the BIO_UPTODATE flag
 (2) by returning a Linux errno value to the bi_end_io callback

The first one has the drawback of only communicating a single possible
error (-EIO), and the second one has the drawback of not beeing persistent
when bios are queued up, and are not passed along from child to parent
bio in the ever more popular chaining scenario.  Having both mechanisms
available has the additional drawback of utterly confusing driver authors
and introducing bugs where various I/O submitters only deal with one of
them, and the others have to add boilerplate code to deal with both kinds
of error returns.

So add a new bi_error field to store an errno value directly in struct
bio and remove the existing mechanisms to clean all this up.

Josef Bacik 0e9cebe724 dm: add log writes target
Introduce a new target that is meant for file system developers to test file
system integrity at particular points in the life of a file system.  We capture
all write requests and associated data and log them to a separate device
for later replay.  There is a userspace utility to do this replay.  The
idea behind this is to give file system developers a tool to verify that
the file system is always consistent.
