Split the connectivity check and recovery routine into separated
functions based on the types
1. iwl_good_ack_health() - check for ack count
2. iwl_good_plcp_health() - check for plcp error
Based on the type of errors being detected, different recovery methods
will be used to bring the system back to normal operational state.
Because different NIC has different HW and uCode, the behavior is also
different; these functions thus now form part of the ops infrastructure,
so we can have more control on how to monitor and recover from error condition

When the throughput drops, the ratio of the actual_ack_count and the

mac80211 recently implemented two new callbacks that are used to request
station add/remove from the driver. The benefot from these new callbacks
are that they enable the driver to sleep while performing this work.
This is a big patch since a few things need to be coordinated in this move.
First we need to decouple station management from rate scaling, which
caused a lot of code to be moved and/or deleted. Next we needed to tie in
with mac80211's station management callback and let it direct our station
management as well as trigger the rate scaling initialization.

Currently the broadcast station is added after every RXON command. Change
this to only add the broadcast station when interface is added by mac80211.
With this we need some extra work to ensure broadcast station is always
present since station table is cleared when RXON without ASSOC bit set is
sent. To deal with this we re-add all driver known stations to uCode after
such an RXON command is sent.
We also do some cleanup and remove the various calls to clear the station
table. We now only clear the station table in two scenarios:
- only clear uCode portion of station table when RXON command without ASSOC
bit is sent
- clear uCode and driver portion when interface goes down or is removed.
We need to do this clearing when interface goes down to deal with the
device restart/reconfigure routines which do not remove the interface, but
do add the interface during reconfiguration.
Previously the keys were also cleared when station table in driver is
cleared, this is not done anymore since mac80211 will take care that keys
are set and cleared correctly.
There is a known issue with this change. Associating with different AP
without bringing interface down fails with a firmware error. This is
because of the lack of full station notification support and the later
patches in this series that complete the station notification support will
fix this.

Recent patch "iwlwifi: move 3945 clip groups to 3945 data" exposed a memory
corruption problem. When initializing the clip groups the code was
mistakenly using the iwlagn rate count, not the 3945 rate count. This
resulted in more memory being written than was allocated.
"iwlwifi: move 3945 clip groups to 3945 data" moved the location where the
clip groups are stored and the impact is now severe in that the number of
configured TX queues is modified. Previously the
"temperature" field was overwritten, which did not seem to affect the
operation.
Fix this one instance where wrong rate count was used. I also noticed one
more location where the iwlagn rate count was used to index an iwl3945
array, fix this. I also modified one location that modified the iwlagn rate
count to obtain the iwl3945 rate count ... just use the iwl3945 rate count
directly.

4965 hardware has 7 queues reserved and the
remaining ones used for aggregation, 5000
and higher need to have 10 reserved. This
is not very clear in the code right now,
unfortunately.
Introduce a new IWL_TX_FIFO_UNUSED constant
and make the queue/FIFO mapping arrays able
to hold that value, and change the setup
code to reserve all queues in the arrays
(the queue number is the index) and use the
new unused constant to not map those queues
to any FIFO.
Additionally, clear up the AC/queue mapping
code to be more understandable. The mapping
is the identity mapping right now, but with
the mapping function I think it's easier to
understand what happens there.
Finally, HCCA isn't implemented at all and
I think newer microcode removed it, so let's
remove all mention of it in the code, some
comments remain for 4965.

At the wireless summit in Portland we discussed a way of
loading firmware asynchronously from ->probe() before
registration to mac80211, in order to register with the
wireless subsystems with complete information in cases
where firmware is required to know parameters.
This is not yet the case in iwlwifi, but for some new
features we're working on it will be the case since
those will only be supported by new firmware images.
Hence, to start with, convert iwlwifi to load firmware
asynchronously from probe, unbinding the device when
firmware loading fails, and only registering with the
wireless subsystems after firmware has been loaded
successfully.
Future patches will hook into this to register the
new firmware capabilities, depending on the firmware
API version.

The goal of "iwlwifi: Monitor and recover the aggregation TX flow failure"
is to first detect when data transmission stalls and then to recover from
this situation with a reset of the radio or the firmware, depending on how
bad the transmission failures are.
Unfortunately we have found that this change causes excessive resets with
its current detection algorithm. It also performs its recovery action when
none is really needed, like when we are not associated.

The HT extension channel settings require priv->staging_rxon.channel to be
accurate. However, iwl_set_rxon_ht was being called before iwl_set_rxon_channel
and thus HT40 could be broken unless another call to iwl_mac_config came in.
This problem was recently introduced by "iwlwifi: Fix to set correct ht
configuration"
The particular setting in which I noticed this was monitor mode:
iwconfig wlan0 mode monitor
ifconfig wlan0 up
./iw wlan0 set channel 64 HT40-
#./iw wlan0 set channel 64 HT40-
tcpdump -i wlan0 -y IEEE802_11_RADIO
would only catch HT40 packets if I issued the IW command twice.
From visual inspection, iwl_set_rxon_channel does not depend on
iwl_set_rxon_ht, so simply swapping them should be safe and fixes this problem.

ack_count also drops. At the same time, the aggregated ba_timeout (the number
of ba timeout retries) also rises. If the actual_ack_count/expected_ack_count
ratio is 0 and the number of ba timeout retries rises to 16, no tx packets
(tcp, udp, or ping - icmp) can be delivered. The driver recovers from this
situation by reseting the uCode firmware. If the actual_ack_count/expected_
ack_count ratio drops below 50% (but not 0) and the aggregated ba_timeout
retries just exceed 5 (but not 16), then the driver can reset the radio to
bring the throughput up.

To ensure that card is in a sane state during probe we add a reset call.
This change was prompted by users of kdump who was not able to bring up the
wireless driver in the kdump kernel. The problem here was that the primary
kernel, which is not running at the time, left the wireless card up and
running. When the kdump kernel starts it is thus possible to immediately
receive interrupts from firmware after registering interrupt, but without
being ready to deal with interrupts from firmware yet.

There is a problem if an "internal short scan" is in progress when a
mac80211 requested scan arrives. If this new scan request arrives within
the "next_scan_jiffies" period then driver will immediately return success
and complete the scan. The problem here is that the scan has not been
fully initialized at this time (is_internal_short_scan is still set to true
because of the currently running scan), which results in the scan
completion never to be sent to mac80211. At this time also, evan though the
internal short scan is still running the state (is_internal_short_scan)
will be set to false, so when the internal scan does complete then mac80211
will receive a scan completion.
Fix this by checking right away if a scan is in progress when a scan
request arrives from mac80211.

When running directed active scans we currently end up sending both the SSID
probe requests and an additional broadcast one.
This is due to the fact that we always leave the probe request template SSID IE
length to 0. Instead we should set it to the first SSID to scan, and fill the
direct_scan array with the remaining SSIDs to scan for. This way we only send
what we've been asked to: a broadcast probe request when no directed scan is
requested, and directed probe requests otherwise.

mac80211: make off-channel work generic
triggered a bug in iwlwifi where HT parameters would not be correctly set in
some mac80211 pathways. The aggregation (and possibly other) station flags were
not being set, which limited the size of aggregation blocks and reduced
throughput at high rates.
>From Johannes:
"""
Due to Wey-Yi's patch to use the set-channel command when the channel changes
while associated, we don't get a full new RXON. Therefore, we don't re-set the
rxon-station either. However, under some circumstances that apparently have
gotten more likely mac80211 will first set up the BSS info, then add the
station and then switch to an HT channel type.
Therefore, the check for "priv->current_ht_config.is_ht" in
iwl_rxon_add_station() will hit false and not fill in the HT information.
However, that check can just be removed, which is the easiest fix for all this,
because the HT capa struct is always there, just could possibly have the
ht_supported member set to false.

In iwlagn the support for spectrum measurement command has been
disabled since v2.6.29 without any requests for it. In addition to this
when this command is indeed enabled it has been found to trigger firmware
SYSASSERT on at least 4965 and 5100 hardware (see
http://bugzilla.intellinuxwireless.org/show_bug.cgi?id=1952 ). Since then
this code has been bitrotting and cannot just be enabled without porting.
Remove support for spectrum measurement command from iwlagn. It can be
added back if there is a future need and the firmware problem it triggers
has been fixed. Support for the spectrim measurement notification remains
as it has been enabled all the time.
In addition to this remove the 3945 spectrum measurement command Kconfig
option and make this command always supported. The code added by this
enabling is minimal and only run when user triggers a spectrum measurement
request via sysfs.

Adding "bt_coex_active" module parameter for iwlcore to enable/disable
BT coexist; if bt_coex_active is true (default), uCode will do kill/defer
every time the priority line is asserted (BT is sending signals on the
priority line in the PCIx). By disable the bt_coex_active, uCode will
ignore the BT activity and perform the normal operation.
Users might experience transmit issue on some platform due to this
WiFi/BT co-exist problem. The possible symptoms are: NetworkManager and
other similar programs can scan and find all the available APs, but will
timeout and unable to associate with any of the APs; no out-going frames
can be found with wireless sniffer tools.
On those platforms, WiFi communication can be restored by set "bt_coex_active"
module parameter to "false"

Add function pointer for broadcast station addition so that we can call it
in from iwlcore at a later time. We only distinguish between iwlagn and
iwl3945 broadcast station addition. For the iwl3945 station addition we add
that function to iwlcore since that is where most station functionality
resides, making it part of iwl3945 will require significant code
reorganization that will dilute station management functionality. This
seems to be an efficient solution.
It may seem as though we are removing error checking when adding the 3945
broadcast station but this error checking was never really necessary since

We have seen the throughput dropped due to external noisy environment
and the radio is out of tune. There are lot of plcp errors indicating
this condition. Eventually the station can get de-authenticated by the
Access Point. By resetting and tuning the radio, the plcp errors are
reduced or eliminated and the throughput starts to rise.
To prevent unexpected behavior such as drop in throughput or deauthentication,
- The change provides the driver feature to monitor and tune the radio base on
the statistics notification from the uCode.
- It also allows the setting of the plcp error rate threshold via
the plcp_delta under debugfs interface.

Scan mechanism is that mac80211/cfg80211 requests driver to scan G band
first and then immediately to scan A band. Original code require

Adding "radio reset" function to help reset and stabilize the radio.
During normal operation, sometime for unknown reason, radio encounter
problem and can not recover by itself; the best way to
recover from it is to reset and re-tune the radio. Currently, there is
no RF reset command available, but since radio will get reset when
switching channel, use internal hw scan request to force radio
reset and get back to normal operation state.
The internal hw scan will only perform passive scan on the first
available channel (not the channel being used) in associated state. The
request should be ignored if already performing scan operation or STA is
not in associated state.

On beacon change update notification from mac we are not disabling

Functionality to indicate uCode is duplicated across

as the the in-tree iwlwifi driver version. However the inclusion
of generated/utsrelease.h makes it a unpleasant behaviour to
recompile the driver everytime when utsrelease.h is updated. In
fact, the driver module is already built with the UTS_RELEASE
information via vermagic of modinfo. Mark the in-tree driver
with the version string "in-tree" to distinguish with those old

In order to help uCode debugging, adding the capability to provide
continuous uCode event logging function.
uCode events is located in round-robin event queue and filled by uCode,
by enable continuous event logging, driver check the write pointer
and log the newly added events in iwl_bg_ucode_trace() timer function.
There is still possibility of missing events if event queue being
wrapped before next event dump; but with this capability, we can have
much better understanding of the uCode behavior during runtime; it can
help to debug the uCode related issues.
Methods to enable/disable the continuous event log:
step 1: enable ucode trace timer
"echo 1 >
/sys/kernel/debug/ieee80211/phyX/iwlagn/debug/ucode_tracing"
step 2: start ftrace
sudo ./trace-cmd record -e iwlwifi_ucode:* sleep 1d
step 3: stop ftrace
sudo ./trace-cmd report trace.dat
step 4: disable ucode trace timer
"echo 0 >
/sys/kernel/debug/ieee80211/phyX/iwlagn/debug/ucode_tracing"
use "ucode_tracing" debugfs file to display number of event
queue wrapped when driver attempt the continuous event logging. If event
queue being wrapped more than once when driver has opportunity to log
the event; it indicated there are events missing in the event log trace.
This continuous event log function only available for 4965 and newer

I've also for a long time had a problem with the
temperature calculation code, which I had fixed
by byte-swapping the values, and now it turns out
that was the correct fix after all.
Also, any use of iwl_eeprom_query_addr() that is
for more than a u8 must be cast to little endian,
and some structs as well.
Fix all this. Again, no real impact on platforms

The construct "le16_to_cpu((__force __le16)(r >> 16))" has
always bothered me when looking through the iwlwifi code,
it shouldn't be necessary to __force anything, and before
this code, "r" was obtained with an ioread32, which swaps
each of the two u16 values in it properly when swapping the
entire u32 value. I've had arguments about this code with
people before, but always conceded they were right because
removing it only made things not work at all on big endian
platforms.
However, analysing a failure of the OTP reading code, I now
finally figured out what is going on, and why my intuition
about that code being wrong was right all along.
It turns out that the 'priv->eeprom' u8 array really wants
to have the data in it in little endian. So the force code
above and all really converts *to* little endian, not from
it. Cf., for instance, the function iwl_eeprom_query16() --
it reads two u8 values and combines them into a u16, in a
little-endian way. And considering it more, it makes sense
to have the eeprom array as on the device, after all not
all values really are 16-bit values, the MAC address for
instance is not.
Now, what this really means is that all the annotations are
completely wrong. The eeprom reading code should fill the
priv->eeprom array as a __le16 array, with __le16 values.
This also means that iwl_read_otp_word() should really have
a __le16 pointer as the data argument, since it should be
filling that in a format suitable for priv->eeprom.
Propagating these changes throughout, iwl_find_otp_image()
is found to be, now obviously visible, defective -- it uses
the data returned by iwl_read_otp_word() directly as if it
was CPU endianness. Fixing that, which is this hunk of the
patch:
- next_link_addr = link_value * sizeof(u16);
+ next_link_addr = le16_to_cpu(link_value) * sizeof(u16);
is the only real change of this patch. Everything else is
just fixing the sparse annotations.
Also, the bug only shows up on big endian platforms with a
1000 series card. 5000 and previous series do not use OTP,
and 6000 series has shadow RAM support which means we don't
ever use the defective code on any cards but 1000.

Recent commits "iwlwifi: remove power-wasting calls to apm_ops.init()" and
"iwlagn: power up device before initializing EEPROM" had the goal of
reducing device power consumption from the time the module is loaded until
the interface is brought up and the device's power saving mechanisms kick
in. The idea is that once the module is loaded there is no need for the
device to consume power until the interface is brought up.
With the current solution the device is only powered up during EEPROM read,
and then so also only if the EEPROM type is OTP. We have found that on
certain platforms even non-OTP devices require power to be up during EEPROM
read. On these platforms the driver never loads and the system log contains
the following:
iwlagn 0000:03:00.0: MAC is in deep sleep!. CSR_GP_CNTRL = 0x080403D8
We thus now power up all devices during EEPROM read.

mac80211: A-MPDU Tx add delBA from recipient support
The first problem is that I moved a BUG_ON before various
checks -- thereby making it possible to hit. As the comment
indicates, the BUG_ON can be removed since the ampdu_action
callback must already exist when the state is != IDLE.
The second problem isn't easily exploitable but there's a
race condition due to unconditionally setting the state to
OPERATIONAL when a delBA frame is received, even when no
aggregation session was ever initiated. All the drivers
accept stopping the session even then, but that opens a
race window where crashes could happen before the driver
accepts it. Right now, a WARN_ON may happen with non-HT
drivers, while the race opens only for HT drivers.
For this case, there are two things necessary to fix it:
1) don't process spurious delBA frames, and be more careful
about the session state; don't drop the lock
2) HT drivers need to be prepared to handle a session stop
even before the session was really started -- this is
true for all drivers (that support aggregation) but
iwlwifi which can be fixed easily. The other HT drivers
(ath9k and ar9170) are behaving properly already.

LED blink rate is based on the traffic load, when tx/rx traffic counts
got reset, we also need to reset the led_tpt to prevent incorrect
blink rate being calculated.
Merge both clear_tx_statistics() and clear_rx_statistics() into
single clear_traffic_statistics() function, when reset the traffic byte
counts, both tx and rx need to be reset at the same time, to make
sure calculated the correct led blink rate.

CSR_INT_COALESCING previously had only one, but now has two single-byte fields.
With only one single-byte field (lowest order byte) it was okay to write via
iwl_write32(), but now with two, an iwl_write32() to the lower order field
clobbers the other field (odd-address CSR_INT_PERIODIC_REG, offset 0x5), and an
iwl_write32() to CSR_INT_PERIODIC_REG could clobber the lowest byte of the
next-higher register (CSR_INT, offset 0x8).
Fortunately, no bad side effects have been produced by the iwl_write32()
usage, due to order of execution (low order byte was always written before
higher order byte), and the fact that writing "0" to the low byte of the
next higher register has no effect (only action is when writing "1"s).
Nonetheless, this cleans up the accesses so no bad side effects might occur
in the future, if execution order changes, or more bit fields get added to
CSR_INT_COALESCING.
Add some comments regarding periodic interrupt usage.

Using the new mac80211 functionality, this makes
iwlwifi handle unicast PS buffering correctly.
The device works like this:
* when a station goes to sleep, the microcode notices
this and marks the station as asleep
* when the station is marked asleep, the microcode
refuses to transmit to the station and rejects all
frames queued to it with the failure status code
TX_STATUS_FAIL_DEST_PS (a previous patch handled
this correctly)
* when we need to send frames to the station _although_
it is asleep, we need to tell the ucode how many,
and this is asynchronous with sending so we cannot
just send the frames, we need to wait for all other
frames to be flushed, and then update the counter
before sending out the poll response frames. This
is handled partially in the driver and partially in
mac80211.
In order to do all this correctly, we need to
* keep track of how many frames are pending for each
associated client station (avoid doing it for other
stations to avoid the atomic ops)
* tell mac80211 that we driver-block the PS status
while there are still frames pending on the queues,
and once they are all rejected (due to the dest sta
being in PS) unblock mac80211

A recent change optimized the power usage by the device by only powering it

We know that the routines in this trace, iwlcore_eeprom_acquire_semaphore
and iwlcore_eeprom_verify_signature, only access CSR registers and thus do
not need the device to be awake if it is EEPROM. But for OTP it is required
for the device to be awake to read these registers. Ensure device is awake
before accessing these registers.

In both 6x00 and 6x50 series, the enhanced/extended tx power table in
EEPROM is used to set the max. tx power limit.
This new tx power table is in 1/2 dBm format, which creates an issue of
possibility of 1/2 dBm loss when driver set the tx power limit; because
of driver keep track and report the tx power in dBm format.
In order to prevent the 1/2 dBm loss, keep track of the true max tx
power in 1/2 dBm format in driver; do the comparison and adjust the tx
power if needed when send tx power command to uCode.

Follow-up to "update write pointers for all tx queues after wakeup"; that
patch changed iwl_irq_tasklet_legacy(), but not iwl_irq_tasklet(), so
newer devices were not covered.
Comments from original patch:
Wakeup interrupt has been updating write pointers (indexes, actually) only
for tx queues 0-5. This is adequate just for 3945, but inadequate for other
devices, all of which have more tx queues. Now updating all tx/command queues,
so device can be aware of all new tx and host commands enqueued while
device was asleep.
This can potentially improve data traffic bandwidth and/or latency.

When I added that code setting the swq_id, I evidently
did not understand the distinction between FIFO and TX
queue yet and added code to compare a queue ID and a
FIFO number, which is bogus.
However, the code there need not be this specific, it
can just set all queues to the identity mapping which
will be overwritten by the aggregation queue code. As

When 802.11g was introduced, we had RTS/CTS and CTS-to-Self protection
mechanisms. In an HT Beacon, HT stations use the "Operating Mode" field
in the HT Information Element to determine whether or not to use
protection.
The Operating Mode field has 4 possible settings: 0-3:
Mode 0: If all stations in the BSS are 20/40 MHz HT capable, or if the
BSS is 20/40 MHz capable, or if all stations in the BSS are 20 MHz HT
stations in a 20 MHz BSS
Mode 1: used if there are non-HT stations or APs using the primary or
secondary channels
Mode 2: if only HT stations are associated in the BSS and at least one
20 MHz HT station is associated.
Mode 3: used if one or more non-HT stations are associated in the BSS.
When in operating modes 1 or 3, and the Use_Protection field is 1 in the
Beacon's ERP IE, all HT transmissions must be protected using RTS/CTS or
CTS-to-Self.
By default, CTS-to-self is the preferred protection mechanism for less
overhead and higher throughput; but using the full RTS/CTS will better
protect the inner exchange from interference, especially in
highly-congested environment.
For 6000 series WIFI NIC, RTS/CTS protection mechanism is the
recommended choice for HT traffic based on the HW design.

Since we have a TODO item to make all station
management dependent on virtual interfaces, I
figured I'd start with pushing such a change
to drivers before more drivers start using the
ieee80211_find_sta() API with a hw pointer and
cause us grief later on.
For now continue exporting the old API in form
of ieee80211_find_sta_by_hw(), but discourage
its use strongly.

For 6x50 series, it is wimax/wifi combo device, so driver need to enable
the wimax/wifi co-exist function and send the coexist event priority
table to uCode for operation.
The priority table will be used by uCode to determine what is the proper
action it should take when co-exist with WiMAX.
For example, when WiFi runs a scan, it must own radio exclusively, therefore
will disconnect WiMAX if WiMAX is connected.

Currently the rate scale information is pointed to from the mac80211
station structure but since that is the only member we might as well make
it part of the structure. Also move the rate scaling initialization to the
init function, no need to do it when we allocate the structure. This fits
with how mac80211 deals with rate scaling information (it always calls init
after allocation) and makes it easier for us to later call initialization
directly.

To save power, don't run apm_ops.init() until needed at "up" time.
EEPROM (5000 and earlier devices) may be read without running apm_ops.init(),
but OTP reads (6000 and newer devices) require a powered-up chip.
Therefore, remove apm_ops.init() from the general path in XXXX_pci_probe(),
and call it only if device uses OTP. Once done with OTP read, call
apm_ops.stop() to reset chip and save power until "up" time comes around.

of the station's existence. The driver dealt with this by not relying on

This switches the iwlwifi driver to use paged skb from linear skb for Rx
buffer. So that it relieves some Rx buffer allocation pressure for the
memory subsystem. Currently iwlwifi (4K for 3945) requests 8K bytes for
Rx buffer. Due to the trailing skb_shared_info in the skb->data,
alloc_skb() will do the next order allocation, which is 16K bytes. This
is suboptimal and more likely to fail when the system is under memory
usage pressure. Switching to paged Rx skb lets us allocate the RXB
directly by alloc_pages(), so that only order 1 allocation is required.
It also adjusts the area spin_lock (with IRQ disabled) protected in the
tasklet because tasklet guarentees to run only on one CPU and the new
unprotected code can be preempted by the IRQ handler. This saves us from
spawning another workqueue to make skb_linearize/__pskb_pull_tail happy
(which cannot be called in hard irq context).
Finally, mac80211 doesn't support paged Rx yet. So we linearize the skb
for all the management frames and software decryption or defragmentation
required data frames before handed to mac80211. For all the other frames,
we __pskb_pull_tail 64 bytes in the linear area of the skb for mac80211
to handle them properly.

iwl_power_initialize() initializes driver data (not device hardware), and does
not need to execute more than once (when the driver initializes). Therefore, it
does not belong in iwl3945_apm_init(), which initializes hardware, and may run
more than once.

The sleep_level_override debugfs file is used by the user to request a
static power index instead of the dynamic sleep values. Users are expected
to provide value from 1 to 5 as an index or -1 to disable it.
The problem at the moment is that users can also provide 0 to this file
which, together with the value 1, is translated to index 1. This is
confusing and even more so when users write 0 to sleep_level_override and
then read 1 from it afterwards.
Modify checking to treat 0 as invalid.

In some cases (e.g. when mac80211 calls iwl_mac_stop() for suspend or user
no longer wants device active), device has not been going into low power state
via __iwl_down(). apm_ops.reset() does not put device into low power state;
instead it resets the device, then puts it into a powered-up state ready to be
re-loaded with uCode and re-started. This has needlessly warmed up user's
laptops and drained batteries.
With current architecture in which mac80211 controls device up/down (including
resetting device after firmware errors), there is no need for apm_ops.reset()
any more; apm_ops.reset() is basically a combination of apm_ops.stop() and
apm_ops.init().
Instead, __iwl_down() now unconditionally places the device into a low-power
state via apm_ops.stop(). Device may be re-started via __iwl_up() calling
apm_ops.init() as soon as it may be needed (e.g. quickly for firmware errors),
but in the meantime, device will stop wasting energy.
Note that, even in this low power state, if driver re-enables interrupts,
the device retains the ability to sense the hardware RF-KILL switch, and
(except for 3945) interrupt the host when it changes.

Both 1000 & 6000 series NICs contain on-chip OTP memory that
replaces the off-chip EEPROM memory. The nature of OTP means
there is a limited number of times a particular board can go through the
factory flow and be (re)calibrated. As a consequence there will be some boards
that contain EEPROM memory because OTP blocks were full.
In the signature validation routine, iwlwifi needs to make sure
"select bit" and "EEPROM/OTP signature" agree on the type of
NVM to be used to configure the system.

The iwlwifi drivers have LED blinking requirements that
mac80211 cannot fulfill due to the use of just a single
LED instead of different ones for TX, RX, radio etc.
Instead, the single LED blinks according to transfers
and is solid on the rest of the time. As such, having
LED class devices registered that mac80211 triggers are
connected to is pointless as we don't use the triggers
anyway.
Remove all the useless code and add hooks into the
driver itself. At the same time, make the LED code
abstracted so the core code that determines blink rate
etc. can be shared between 3945 and agn in iwlcore.
At the same time, the fact that we removed the use of
the mac80211 LED triggers means we can also remove the
IWLWIFI_LEDS Kconfig symbol since the LED support is
now self-contained.

When uCode detects critical temperature it should send "card state
notification" interrupt to driver and then shut itself down to prevent
overheating. There is a race condition where uCode shuts down before it
can deliver the interrupt to driver.
Additional method provided here for driver to enter CT_KILL state based
on temperature reading.
How it works:
Method 1:
If driver receive "card state notification" interrupt from uCode; it
enters "CT_KILL" state immediately
Method 2:
If the last temperature report by Card reach Critical temperature,
driver will send "statistic notification" request to uCode to verify the
temperature reading, if driver can not get reply from uCode within
300ms, driver will enter CT_KILL state automatically.
Method 3:
If the last temperature report by Card did not reach Critical
temperature, but uCode already shut down due to critical temperature.
All the host commands send to uCode will not get process by uCode;
when command queue reach the limit, driver will check the last reported
temperature reading, if it is within pre-defined margin, enter "CT_KILL"
state immediately. In this case, when uCode ready to exit from "CT_KILL" state,
driver need to restart the adapter in order to reset all the queues and
resume normal operation.
One additional issue being address here, when system is in CT_KILL
state, both tx and rx already stopped, but driver still can send host
command to uCode, it will flood the command queue since card was not
responding; adding STATUS_CT_KILL flag to reject enqueue host commands
to uCode if it is in CT_KILL state, when uCode is ready to come out of
CT_KILL, driver will clear the STATUS_CT_KILL bit and allow enqueue the host
commands to uCode to recover from CT_KILL state.

state.
The supported LED modes after this are:
IWL_LED_BLINK (current/default) - blink rate based on current Tx/Rx
traffic
IWL_LED_RF_STATE (new) -
LED OFF: No power/RF disabled, the LED is emitting no light
LED ON: Powered/RF enabled, the LED is emitting light

Current rate scale algorithm fluctuates between different MIMO modes fairly
rapidly, causing widely varying performance. These fluctuations occur because in
the rate_scale tables for expected throughput the values are not very different
for different modes.
However, when aggregation is turned on and MAC overhead is reduced, the
expected throughput for different MIMO modes grows and different modes have
vastly different performance. Add expected throughput tables for this case.
We also need to keep track of aggregation status per-station, so we add the
"is_agg" field to struct lq_sta.

The chain settings we currently use in iwlwifi are
rather confusing -- and we also go by the wrong
settings entirely under certain circumstances. To
clean it up, create a new variable in the current
HT config -- single_chain_sufficient -- that tells
us whether we need more than one chain. Calculate
that based on the AP and operating mode (no IBSS
HT implemented -- so no need for multiple chains,
for station mode we use the AP's capabilities).
Additionally, since APs always send disabled SM PS
mode, keeping track of their sm_ps mode isn't very
useful -- doubly not so for our _own_ RX config
since that should depend on our, not the AP's, SM
PS mode.
Finally, document that our configuration of the
number of RX chains used is currently wrong when
in powersave (by adding a comment).
All together this removes the two remaining items
in struct iwl_ht_config that were done wrong there.
For the future, the number of RX chains and some
SM PS handshaking needs to be added to mac80211,
which then needs to tell us, and the new variable
current_ht_config.single_chain_sufficient should
also be calculated by mac80211.

The address stored in the next link address is a word address but when
reading the OTP blocks, a byte address is used. Also if the blocks are
full and the last link pointer is not zero, then none of the blocks are
valid so return an error.
The algorithm is simply valid blocks have a next address and that
address's contents is zero.
Using the wrong address for the next link address gets arbitrary data,
obviously. In cases seen, the first block is considered valid when it is not.
If the block has in fact been invalidated there may be old data or
there may be no data, bad data, or partial data, there is no way of
telling. Without this patch it is possible that a device with valid OTP data
is unable to work.

When hardware or uCode problem occurs driver captures significant
information from device to enable debugging. The format of this information
is different between 3945 and 4965 and later devices, yet currently the
3945 uses the 4965 and later format. Fix this by adding a new library call
that is initialized to the correct formatting routine based on device.
This moves the iwlagn event and error log handling back to iwl-agn.c to
make it part of iwlagn module.
Also remove the 3945 sysfs file that triggers dump of event log - there is
already a debugfs file that can do it for all drivers.

We keep track of where to write into a buffer by keeping a count of how
much has been written so far. When writing to the buffer we thus take the
buffer pointer and adding the count of what has been written so far.
Keeping track of what has been written so far is done by incrementing
this number every time something is written to the buffer with how much has
been written at that time.
Currently this number is incremented incorrectly when using the
"hex_dump_to_buffer" call to add data to the buffer. Fix this by only
adding what has been added to the buffer in that call instead of what has
been added since beginning of buffer.

Replenishment of receive buffers is done in the tasklet handling
received frames as well as in a workqueue. When we are in the tasklet
we cannot sleep and thus attempt atomic skb allocations. It is generally
not a big problem if this fails since iwl_rx_allocate is always followed
by a call to iwl_rx_queue_restock which will queue the work to replenish
the buffers at a time when sleeping is allowed.
We thus add the __GFP_NOWARN to the skb allocation in iwl_rx_allocate to
reduce the noise if such an allocation fails while we still have enough
buffers. We do maintain the warning and the error message when we are low
on buffers to communicate to the user that there is a potential problem with
memory availability on system

commit 10c994ca70e8e94bbc85a5bf13de5911ee8de4d2 "iwlwifi: fix remove key
error" fixed an error reported by mac80211 during interface down. The fix
involved changing an async command to synchronous. Unfortunately this was
inside a spinlock section in which we cannot sleep.
Modify the sending of the command back to async. This causes the mac80211
error "mac80211-phy0: failed to remove key (0, ff:ff:ff:ff:ff:ff) from
hardware (-16)." to return. This error is not serious since this occurs
during interface down and the keys will be cleared anyway when ucode is
unloaded. Having this error message is thus less serious than a potential
deadlock introduced when sleeping while holding a spinlock. We will have to
find another fix for that error.
This is a revert of the abovementioned commit.

There's a bug in 4965 powersave that appears to
be related to the way it keeps track of its data
during sleep, but we haven't found it yet. Due to
that, using powersave may spontaneously cause the
device to SYSASSERT when transitioning from sleep
to wake. Therefore, disable powersave for 4965,

and fix the problem.

RX handling maintains a few lists that keep track of the RX buffers.
Buffers move from one list to the other as they are used, replenished, and
again made available for usage. In one such instance, when a buffer is used
it enters the "rx_used" list. When buffers are replenished an skb is
attached to the buffer and it is moved to the "rx_free" list. The problem
here is that the buffer is first removed from the "rx_used" list _before_ the
skb is allocated. Thus, if the skb allocation fails this buffer remains
removed from the "rx_used" list and is thus lost for future usage.
Fix this by first allocating the skb before trying to attach it to a list.
We add an additional check to not do this unnecessarily.

Significant literature suggests users use debug flags 0x43fff - this causes
the debug flags to be set that causes information to be printed for every
received frame - including beacons. In the best case it fills up the logs,
at worst it slows driver down and causes failures due to timeouts.
In the RX handler, print debugging only if user requested RX debugging.

Some concerns were raised about the automatic adjustment
of sleep intervals to all the same, potentially high,
value, and I can imagine the hardware behaving better
when we don't ask too much of it.
So let's convert to use a succession of sleep levels
when requesting to go to deeper sleeps (which can only
happen with large DTIM intervals), using the succession
values from power level three, which have the benefit of
also having been tested extensively already.
As a result, the automatic sleep level adjustment will
now be mostly equivalent to power level three, except
for the RX/TX timeouts and possibly using smaller sleep
vectors to account for networking latency.

For HT packets, mac80211 expects the rate_idx to be an MCS number, which is the
lower byte of rate_n_flags. However, iwl_hwrate_to_plcp_idx takes the MCS
number and reduces it down to the range 0-8 (6 to 60 Mbps), removing the bits
that signify multiply streams, HT40 Duplicate mode, or unequal modulation.
This version is used for various internal purposes through the driver.
Add the function iwl_hwrate_get_mac80211_idx, an alternate version which takes
the rate and the band and returns the mac80211 index (MCS, for HT packets, and
PLCP rate, for legacy packets).

Refactor and correct rate selection for outgoing transmitted
packets.
First, note that HT rates in the mac80211 rate table do not provide valid
indices when ieee80211_get_tx_rate is called; the check to see if we could to
abort a transmission early in iwl_tx_skb() would thus occasionally read invalid
memory and occasionally stall transmission (if the erroneous byte was 0xff).
We remove that code; the check wasn't valid anyway.
Second, iwl_tx_cmd_build_rate() also called ieee80211_get_tx_rate to be used
for sending management packets, which do not use the uCode station table. This
patch refactors that function and adds comments to enhance legibility, replaces
the call to ieee80211_get_tx_rate() with a direct lookup, and adds error
handling in case the table entry is invalid.

Commit 1ccb84d87d04df3c76cd4352fe69786d8c7cf016 by Wey-Yi Guy
("iwlwifi: clean up unused NL80211_IFTYPE_MONITOR for Monitor mode")
broke injection of non-broadcast frames to unassociated stations
(causing a SYSASSERT for all such injected frames), due to injected
frames no longer automatically getting a broadcast station ID assigned.
This patch restores the old behavior, fixing the aforementioned
regression.
Also, consistently check for IEEE80211_TX_CTL_INJECTED instead of
iwl_is_monitor_mode in the TX path, as TX_CTL_INJECTED specifically
means that a given packet is coming from a monitor interface, while
iwl_is_monitor_mode only shows whether a monitor interface exists
on the device.

Over time, a whole bunch of drivers have come up
with their own scheme to delay the configure_filter
operation to a workqueue. To be able to simplify
things, allow configure_filter to sleep, and add
a new prepare_multicast callback that drivers that
need the multicast address list implement. This new
callback must be atomic, but most drivers either
don't care or just calculate a hash which can be
done atomically and then uploaded to the hardware
non-atomically.
A cursory look suggests that at76c50x-usb, ar9170,
mwl8k (which is actually very broken now), rt2x00,
wl1251, wl1271 and zd1211 should make use of this
new capability.
