Hans de Goede 8e04d8056c scsi/sr: add no_read_disc_info scsi_device flag
Some USB devices emulate a usb-mass-storage attached (scsi) cdrom device,
usually this fake cdrom contains the windows software for the device.
While working on supporting Appotech ax3003 based photoframes, which do
this I discovered that they will go of into lala land when ever they see a
READ_DISC_INFO scsi command.

Thus this patch adds a scsi_device flag (which can then be set by the
usb-storage driver through an unsual-devs entry), to indicate this, and
makes the sr driver honor this flag.

I know this sucks, but as discussed on linux-scsi list there is no other
way to make this device work properly.

Looking at usb traces made under windows, windows never sends a
READ_DISC_INFO during normal interactions with a usb cdrom device.  So as
this cdrom emulation thingie becomes more common we might see more of this
problem.

Darrick J. Wong 56dd2c0691 [SCSI] libsas: Don't issue commands to devices that have been hot-removed
sd will get hung up issuing commands to flush write cache if a SAS
device behind the expander is unplugged without warning.  Change libsas
to reject commands to domain devices that have already gone away.

Mike Christie 43ca910a9c [SCSI] fc class: add fc host dev loss sysfs file
This adds a fc host dev loss sysfs file. Instead of
calling into the driver using the get_host_def_dev_loss_tmo
callback, we allow drivers to init the dev loss like is done
for other fc host params, and then the fc class will handle
updating the value if the user writes to the new sysfs file.

Martin K. Petersen 13f05c8d8e block/scsi: Provide a limit on the number of integrity segments
Some controllers have a hardware limit on the number of protection
information scatter-gather list segments they can handle.

Introduce a max_integrity_segments limit in the block layer and provide
a new scsi_host_template setting that allows HBA drivers to provide a
value suitable for the hardware.

Add support for honoring the integrity segment limit when merging both
bios and requests.

Tejun Heo 9cbbdca44a block: remove spurious uses of REQ_HARDBARRIER
REQ_HARDBARRIER is deprecated.  Remove spurious uses in the following
users.  Please note that other than osdblk, all other uses were
already spurious before deprecation.

* osdblk: osdblk_rq_fn() won't receive any request with
  REQ_HARDBARRIER set.  Remove the test for it.

* pktcdvd: use of REQ_HARDBARRIER in pkt_generic_packet() doesn't mean
  anything.  Removed.

* aic7xxx_old: Setting MSG_ORDERED_Q_TAG on REQ_HARDBARRIER is
  spurious.  Removed.

* sas_scsi_host: Setting TASK_ATTR_ORDERED on REQ_HARDBARRIER is
  spurious.  Removed.

* scsi_tcq: The ordered tag path wasn't being used anyway.  Removed.

Mike Christie b8ef3204f4 [SCSI] fc class: add fc host default default dev loss setting
This patch adds a fc_host setting to store the
default dev_loss_tmo. It is used if the driver
has a callack to get the value from the LLD. If
the callback is not set, then we use the fc class
module default value.

Peter Korsgaard 083c8c1e60 scsi: use __uX types for headers exported to user space
Commit 9e4f5e29 ("FC Pass Thru support") exported a number of header files
in include/scsi to user space, but didn't change the uX types to the
userspace-compatible __uX types.  Without that you'll get compile errors

Alan Stern bc4f24014d [SCSI] implement runtime Power Management
This patch (as1398b) adds runtime PM support to the SCSI layer.  Only
the machanism is provided; use of it is up to the various high-level
drivers, and the patch doesn't change any of them.  Except for sg --
the patch expicitly prevents a device from being runtime-suspended
while its sg device file is open.

The implementation is simplistic.  In general, hosts and targets are
automatically suspended when all their children are asleep, but for
them the runtime-suspend code doesn't actually do anything.  (A host's
runtime PM status is propagated up the device tree, though, so a
runtime-PM-aware lower-level driver could power down the host adapter
hardware at the appropriate times.)  There are comments indicating
where a transport class might be notified or some other hooks added.

LUNs are runtime-suspended by calling the drivers' existing suspend
handlers (and likewise for runtime-resume).  Somewhat arbitrarily, the
implementation delays for 100 ms before suspending an eligible LUN.
This is because there typically are occasions during bootup when the
same device file is opened and closed several times in quick
succession.

The way this all works is that the SCSI core increments a device's
PM-usage count when it is registered.  If a high-level driver does
nothing then the device will not be eligible for runtime-suspend
because of the elevated usage count.  If a high-level driver wants to
use runtime PM then it can call scsi_autopm_put_device() in its probe
routine to decrement the usage count and scsi_autopm_get_device() in
its remove routine to restore the original count.

Hosts, targets, and LUNs are not suspended while they are being probed
or removed, or while the error handler is running.  In fact, a fairly
large part of the patch consists of code to make sure that things
aren't suspended at such times.

James Bottomley df64d3caab [SCSI] Unify SAM_ and SAM_STAT_ macros
We have two separate definitions for identical constants with nearly the
same name.  One comes from the generic headers in scsi.h; the other is
an enum in libsas.h ... it's causing confusion about which one is
correct (fortunately they both are).

Fix this by eliminating the libsas.h duplicate

Joe Eykholt 9226115695 [SCSI] libfc: don't require a local exchange for incoming requests
Incoming requests shouldn't require a local exchange if we're
just going to reply with one or two frames and don't expect
anything further.  Don't allocate exchanges for such requests
until requested by the upper-layer protocol.

The sequence is always NULL for new requests, so remove
that as an argument to request handlers.

Also change the first argument to lport->tt.seq_els_rsp_send
from the sequence pointer to the received frame pointer, to
supply the exchange IDs and destination ID info.

Joe Eykholt 24f089e2f2 [SCSI] libfc: add fc_fill_reply_hdr() and fc_fill_hdr()
Add functions to fill in an FC header given a request header.
These reduces code lines in fc_lport and fc_rport and works
without an exchange/sequence assigned.

fc_fill_reply_hdr() fills a header for a final reply frame.

fc_fill_hdr() which is similar but allows specifying the
f_ctl parameter.

Add defines for F_CTL values FC_FCTL_REQ and FC_FCTL_RESP.
These can be used for most request and response sequences.

v2 of patch adds a line to copy the frame encapsulation
info from the received frame.

Joe Eykholt 251748a99e [SCSI] libfc: add fc_frame_sid() and fc_frame_did() functions
To pave the way for eliminating exchanges from incoming requests,
add simple inline fc_frame_sid() and fc_frame_did() functions
which get the FC_IDs from the frame header.  This can be almost
as efficient as getting them from the sequence/exchange.

Move ntohll, htonll, ntoh24 and hton24 to <scsi/fc_frame.h>
since we need them there and that's included by <scsi/libfc.h>

Joe Eykholt 079ecd8cfe [SCSI] libfc: eliminate rport LOGO state
The LOGO state hasn't been used in a while, except in a brief
transition to DELETE state while holding the rport mutex.
All port LOGO responses have been ignored as well as any timeout
if we don't get a response.

So this patch just removes LOGO state and simplifies the response handler.

Joe Eykholt e10f8c667b [SCSI] libfcoe: fcoe: fnic: add FIP VN2VN point-to-multipoint support
The FC-BB-6 committee is proposing a new FIP usage model called
VN_port to VN_port mode.  It allows VN_ports to discover each other
over a loss-free L2 Ethernet without any FCF or Fibre-channel fabric
services.  This is point-to-multipoint.  There is also a variant
of this called point-to-point which provides for making sure there
is just one pair of ports operating over the Ethernet fabric.

We add these new states:  VNMP_START, _PROBE1, _PROBE2, _CLAIM, and _UP.
These usually go quickly in that sequence.  After waiting a random
amount of time up to 100 ms in START, we select a pseudo-random
proposed locally-unique port ID and send out probes in states PROBE1
and PROBE2, 100 ms apart.  If no probe responses are heard, we
proceed to CLAIM state 400 ms later and send a claim notification.
We wait another 400 ms to receive claim responses, which give us
a list of the other nodes on the network, including their FC-4
capabilities.  After another 400 ms we go to VNMP_UP state and
should start interoperating with any of the nodes for whic we
receivec claim responses.  More details are in the spec.j

Add the new mode as FIP_MODE_VN2VN.  The driver must specify
explicitly that it wants to operate in this mode.  There is
no automatic detection between point-to-multipoint and fabric
mode, and the local port initialization is affected, so it isn't
anticipated that there will ever be any such automatic switchover.

It may eventually be possible to have both fabric and VN2VN
modes on the same L2 network, which may be done by two separate
local VN_ports (lports).

When in VN2VN mode, FIP replaces libfc's fabric-oriented discovery
module with its own simple code that adds remote ports as they
are discovered from incoming claim notifications and responses.
These hooks are placed by fcoe_disc_init().

A linear list of discovered vn_ports is maintained under the
fcoe_ctlr struct.  It is expected to be short for now, and
accessed infrequently.  It is kept under RCU for lock-ordering
reasons.  The lport and/or rport mutexes may be held when we
need to lookup a fcoe_vnport during an ELS send.

Change fcoe_ctlr_encaps() to lookup the destination vn_port in
the list of peers for the destination MAC address of the
FIP-encapsulated frame.

Add a new function fcoe_disc_init() to initialize just the
discovery portion of libfcoe for VN2VN mode.

Joe Eykholt f60e12e9c7 [SCSI] libfc: track FIP exchanges
When an exchange is received with a FIP encapsulation, we need
to know that the response must be sent via FIP and what the original
ELS opcode was.  This becomes important for VN2VN mode, where we may
receive FLOGI or LOGO from several peer VN_ports, and the LS_ACC or
LS_RJT must be sent FIP-encapsulated with the correct sub-type.

Add a field to the struct fc_frame, fr_encaps, to indicate the
encapsulation values.  That term is chosen to be neutral and
LLD-agnostic in case non-FCoE/FIP LLDs might find it useful.

The frame fr_encaps is transferred from the ingress frame to the
exchange by fc_exch_recv_req(), and back to the outgoing frame
by fc_seq_send().

This is taking the last byte in the skb->cb array.  If needed,
we could combine the info in sof, eof, flags, and encaps
together into one field, but it'd be better to do that if
and when its needed.

Joe Eykholt a7b12a279f [SCSI] libfc: add FLOGI state to rport for VN2VN
The FIP proposal for VN_port to VN_port point-to-multipoint
operation requires a FLOGI be sent to each remote port.
The FLOGI is sent with the assigned S_ID and D_IDs of the
local and remote ports.  This and the response get
FIP-encapsulated for Ethernet.

Add FLOGI state to the remote port state machine.
This will be skipped if not in point-to-multipoint mode.

To reduce a little duplication between PLOGI and FLOGI
response handling, added fc_rport_login_complete(), which
handles the parameters for the rdata struct.

Joe Eykholt 3726f3584e [SCSI] libfc: Add local port point-to-multipoint flag
For VN_port to VN_port mode, the transport sets the port_id and
there's no lport FLOGI.  This is similar to FC loop mode.

Add a point_to_multipoint flag that indicates the local port is in
point-to-multipoint mode.  This skips FLOGI and discovery.
It also skips resetting the port_id on resets other than link down.

Add function fc_lport_set_local_id() that sets the local port_id.
This is called by libfcoe on behalf of the low-level driver
to set the port_id when the link comes up.

Joe Eykholt 0685230c59 [SCSI] libfc: add discovery-private pointer for LLD
For VN_port to VN_port mode, FIP will do discovery and needs a
way to find its state from the local port or discovery structure.
It seems that any other LLD that implements its own discovery
would also need something like this.

Replace disc->lport with disc->priv, and use container_of to
find the lport.  We could use disc->priv for that, but
container_of is smaller and faster.

Joe Eykholt fdb068c6cd [SCSI] libfcoe: convert FIP to lock with mutex instead of spin lock
It turns out most of the FIP work is now done from worker threads
or process context now, so there's no need to use a spin lock.

Change to use mutex instead of spin lock and delayed_work instead
of a timer.

This will make it nicer for the VN_port to VN_port feature that
will interact more with the libfc layers requiring that
spinlocks not be held.

Vasu Dev 519e5135e2 [SCSI] fcoe: adds src and dest mac address checking for fcoe frames
This is  per FC-BB-5 Annex-D recommendation and per that
if address checking fails then drop the frame.

FIP code paths are already doing this so only needed for fcoe
frames.

The src address checking is limited to only fip mode since
this might break non-fip mode used in p2p due to used OUI
based addressing in some p2p code paths, going forward FIP
will be the only mode, therefore limited this to only FIP
mode so that it won't break non-fip p2p mode for now.

Removes FCOE packet type checking since fcoe_rcv is
registered to receive only FCoE type packets from netdev
and it is already checked by netdev.

Bart Van Assche d058fd31c7 [SCSI] fcoe: make it possible to verify fcoe with sparse
Analyzing fcoe with sparse currently fails. This is because struct
fcoe_rcv_info contains two enum members that have been declared with
__attribute__((packed)). Apparently gcc honors this attribute while sparse
ignores it. The result is that sizeof(struct fcoe_rcv_info)
== sizeof(struct sk_buff::cb) == 48 on a 64-bit system according to gcc, but
not according to sparse. The patch below modifies the definition of
struct fcoe_rcv_info such that gcc and sparse interpret this structure
definition in the same way. The current sparse output is as follows:

Remote ports were restarting indefinitely after getting
rejects in PRLI.

Fix by adding a counter of restarts and limiting that with
the port login retry limit as well.

Joe Eykholt 4b2164d4d2 [SCSI] libfc: Fix remote port restart problem
This patch somewhat combines two fixes to remote port handing in libfc.

The first problem was that rport work could be queued on a deleted
and freed rport.  This is handled by not resetting rdata->event
ton NONE if the rdata is about to be deleted.

However, that fix led to the second problem, described by
Bhanu Gollapudi, as follows:

So, the problem here is that we were tracking the desire for
the rport be restarted by state RESTART, which was otherwise
equivalent to DELETE.  A contributing factor is that we dropped
the lock between steps 6 and 10 in thread T2, which allows the
state to change, and we didn't completely re-evaluate then.

This is hopefully corrected by the following minor redesign:

Simplify the rport restart logic by making the decision to
restart after deleting the transport rport.  That decision
is based on a new STARTED flag that indicates fc_rport_login()
has been called and fc_rport_logoff() has not been called
since then.  This replaces the need for the RESTART state.

Only restart if the rdata is still in DELETED state
and only if it still has the STARTED flag set.

Also now, since we clear the event code much later in the
work thread, allow for the possibility that the rport may
have become READY again via incoming PLOGI, and if so,
queue another event to handle that.

In the problem scenario, the second LOGO received will
cause the LOGO event to occur again.

Unsolicited PRLO request is now handled by sending LS_ACC,
and then relogin to the remote port if an N-port login
session exists for that remote port.

Robert Love 7b2787ec15 [SCSI] libfc: Move the port_id into lport
This patch creates a port_id member in struct fc_lport.
This allows libfc to just deal with fc_lport instances
instead of calling into the fc_host to get the port_id.

This change helps in only using symbols necessary for
operation from the libfc structures. libfc still needs
to change the fc_host_port_id() if the port_id changes
so the presentation layer (scsi_transport_fc) can provide
the user with the correct value, but libfc shouldn't
rely on the presentation layer for operational values.

Vasu Dev da87bfab8a [SCSI] fcoe, fnic, libfc: increased CDB size to 16 bytes for fcoe.
No reason to restrict CDB size to 12 bytes in fcoe, so
increased to 16 so that 16 bytes SCSI CDB doesn't fail.

Uses common define to set max_cmd_len for fcoe and fnic,
fnic is already setting max_cmd_len to 16.

sg_readcap -l fails without this fix.

Christof Schmitt 2f2eb58762 [SCSI] Allow FC LLD to fast-fail scsi eh by introducing new eh return
If the scsi eh is running and then a FC LLD calls
fc_remote_port_delete, the SCSI commands sent from the eh will fail.
To prevent this, a FC LLD can call fc_block_scsi_eh from the eh
callback, blocking the eh thread until the dev_loss_tmo fires or the
remote port is available again.

If (e.g. for a multipathing setup) the dev_loss_tmo is set to a very
large value, thus preventing the scsi device removal , the scsi eh can
block for a long time. For multipathing, the fast_io_fail_tmo is then
set to a low value to detect path problems sooner.

This patch introduces a new return code FAST_IO_FAIL. The function
fc_block_scsi_eh now returns FAST_IO_FAIL when the fast_io_fail_tmo
fires. This indicates that the LLD terminated all pending I/O requests
and there are no more pending SCSI commands for the scsi eh to wait
for. This return code can be passed back to the scsi eh to stop the
escalation and finish the recovery process for this device.

Joe Eykholt f018b73af6 [SCSI] libfc, libfcoe, fcoe: use smp_processor_id() only when preempt disabled
When the kernel is configured for preemption, using smp_processor_id()
when preemption is enabled causes a warning backtrace and is wrong
since we could move off of that CPU as soon as we get the ID,
and we would be referencing the wrong CPU, and possibly an invalid one
if it could be hotswapped out.

Remove the fc_lport_get_stats() function and explicitly use per_cpu_ptr()
to get the statistics.  Where preemption has been disabled by holding
a _bh lock continue to use smp_processor_id(), but otherwise use
get_cpu()/put_cpu().

In fcoe_recv_frame() also changed the cases where we return in the
middle to do a goto to the code which bumps ErrorFrames and does
a put_cpu().  Two of these cases didn't bump ErrorFrames before, but
doing so is harmless because they "can't happen", due to prior length
checks.

Also rearranged code in fcoe_recv_frame() to have only one call to
fc_exch_recv().  It's just as efficient and saves a call to put_cpu().

In fc_fcp.c, adjusted a FIXME comment for code which doesn't need fixing.

longer useful, since they are always set to the same value,
and FIP always calls libfc to pass link information to the lport.

Eliminate those fields and rename link_work to timer_work, since

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

James Bottomley 75c85a0bc1 libsrp: fix compile failure
commit 4546548789 ("kfifo: move struct
kfifo in place") caused a compile failure in ibmvscsitgt.c because it
changed a pointer to kfifo in the libsrp.h structure to a direct
inclusion without including <linux/kfifo.h>.

The fix is simple, just add the include, but how did this happen? This
change, introduced at -rc2, hardly looks like a bug fix, and it clearly
didn't go through linux-next, which would have picked up this compile
failure (it only occurs on ppc because of the ibm virtual scsi target).

[ Apparently all of -mm wasn't in linux-next.. ]

Stefani Seibold 4546548789 kfifo: move struct kfifo in place
This is a new generic kernel FIFO implementation.

The current kernel fifo API is not very widely used, because it has to
many constrains.  Only 17 files in the current 2.6.31-rc5 used it.
FIFO's are like list's a very basic thing and a kfifo API which handles
the most use case would save a lot of development time and memory
resources.

I think this are the reasons why kfifo is not in use:

 - The API is to simple, important functions are missing
 - A fifo can be only allocated dynamically
 - There is a requirement of a spinlock whether you need it or not
 - There is no support for data records inside a fifo

So I decided to extend the kfifo in a more generic way without blowing up
the API to much.  The new API has the following benefits:

 - Generic usage: For kernel internal use and/or device driver.
 - Provide an API for the most use case.
 - Slim API: The whole API provides 25 functions.
 - Linux style habit.
 - DECLARE_KFIFO, DEFINE_KFIFO and INIT_KFIFO Macros
 - Direct copy_to_user from the fifo and copy_from_user into the fifo.
 - The kfifo itself is an in place member of the using data structure, this save an
   indirection access and does not waste the kernel allocator.
 - Lockless access: if only one reader and one writer is active on the fifo,
   which is the common use case, no additional locking is necessary.
 - Remove spinlock - give the user the freedom of choice what kind of locking to use if
   one is required.
 - Ability to handle records. Three type of records are supported:
   - Variable length records between 0-255 bytes, with a record size
     field of 1 bytes.
   - Variable length records between 0-65535 bytes, with a record size
     field of 2 bytes.
   - Fixed size records, which no record size field.
 - Preserve memory resource.
 - Performance!
 - Easy to use!

This patch:

Since most users want to have the kfifo as part of another object,
reorganize the code to allow including struct kfifo in another data
structure.  This requires changing the kfifo_alloc and kfifo_init
prototypes so that we pass an existing kfifo pointer into them.  This
patch changes the implementation and all existing users.

Boaz Harrosh 5d0961fd1f [SCSI] libosd: Fix blk_put_request locking again
So libosd has decided to sacrifice some code simplicity for the sake of
a clean API. One of these things is the possibility for users to call
osd_end_request, in any condition at any state. This opens up some
problems with calling blk_put_request when out-side of the completion
callback but calling __blk_put_request when detecting a from-completion
state.

The current hack was working just fine until exofs decided to operate on
all devices in parallel and wait for the sum of the requests, before
deallocating all osd-requests at once. There are two new possible cases
1. All request in a group are deallocated as part of the last request's
   async-done, request_queue is locked.
2. All request in a group where executed asynchronously, but
   de-allocation was delayed to after the async-done, in the context of
   another thread. Async execution but request_queue is not locked.

The solution I chose was to separate the deallocation of the osd_request
which has the information users need, from the deallocation of the
internal(2) requests which impose the locking problem. The internal
block-requests are freed unconditionally inside the async-done-callback,
when we know the queue is always locked. If at osd_end_request time we
still have a bock-request, then we know it did not come from within an
async-done-callback and we can call the regular blk_put_request.

The internal requests were used for carrying error information after
execution. This information is now copied to osd_request members for
later analysis by user code.

The external API and behaviour was unchanged, except now it really
supports what was previously advertised.

While the target reset task management function has been deprecated in
newer specs, it is still in use by SCSI FC drivers and there is no
real replacement. Add the target reset flag to the FCP header file to
allow usage of this definition in SCSI FC drivers.

When the D bit is set if the FKA_ADV_Period of the FIP Discovery
Advertisement, the ENode should not transmit period ENode FIP Keep Alive and
VN_Port FIP Keep Alive (FC-BB-5 Rev2, 7.8.3.13).

Note that fcf->flags is taken directly from the fip_header, I am claiming one
bit for the purpose of the FIP_FKA_Period D bit as FIP_FL_FK_ADV_B, and use
FIP_HEADER_FLAGS as bitmask for bits used in fip_header.

Yi Zou 6580bbd0af [SCSI] libfc: add FC-BB-5 LESB counters to fcoe_dev_stats
FC-BB-5 Rev2.0, Clause 7.10 extends the FC-LS-3 LESB for FC-BB_E. We are
already tracking Link Failure Count so add the rest in this patch.

For VLinkFailureCount and MissDiscAdvCount, they are part of the per-cpu
fcoe_dev_stats. For SymbolErrorCount, ErroredBlockCount, and FCSErrorCount,
they are defined in IEEE 802.3-2008 and are per LLD. They are expected to
come from LLD.

Administer some love to the osd_req_decode_sense function

* Fix a bad bug with osd_req_decode_sense(). If there was no scsi
  residual, .i.e the request never reached the target, then all the
  osd_sense_info members where garbage.

* Add grossly missing in/out_resid to osd_sense_info and fill them in
  properly.

* Define an osd_err_priority enum which divides the possible errors into
  7 categories in ascending severity. Each category is also assigned a
  Linux return code translation.

  Analyze the different osd/scsi/block returned errors and set the
  proper osd_err_priority and Linux return code accordingly.

* extra check a few situations so not to get stuck with inconsistent
  error view. Example an empty residual with an error code, and other
  places ...

Lots of libosd's osd_req_decode_sense clients had this logic in some
form or another. Consolidate all these into one place that should
actually know about osd returns. Thous translating it to a more
abstract error.

Boaz Harrosh 2cdd6410e5 [SCSI] libosd: osd_dev_info: Unique Identification of an OSD device
Define an osd_dev_info structure that Uniquely identifies an OSD
device lun on the network. The identification is built from unique
target attributes and is the same for all network/SAN machines.

osduld_info_lookup() - NEW
    New API that will lookup an osd_dev by its osd_dev_info.
    This is used by pNFS-objects for cross network global device
    identification. And by exofs multy-device support, the device
    info is specified in the on-disk exofs device table.

osduld_device_info() - NEW
    Given an osd_dev handle returns its associated osd_dev_info.
    The ULD fetches this information at startup and hangs it on
    each OSD device. (This is a fast operation that can be called
    at any condition)

osduld_device_same() - NEW
    With a given osd_dev at one hand and an osd_dev_info
    at another, we would like to know if they are the same
    device.
    Two osd_dev handles can be checked by:
        osduld_device_same(od1, osduld_device_info(od2));

osd_auto_detect_ver() - REVISED
    Now returns an osd_dev_info structure. Is only called once
    by ULD as before. See added comments for how to use.

Boaz Harrosh d6ae4333e6 [SCSI] osduld: Use device->release instead of internal kref
The true logic of this patch will be clear in the next patch where we
use the class_find_device() API. When doing so the use of an internal
kref leaves us a narrow window where a find is started while the actual
object can go away. Using the device's kobj reference solves this
problem because now the same kref is used for both operations. (Remove
and find)

Core changes
* Embed a struct device in uld_ structure and use device_register
  instead of devie_create. Set __remove to be the device release
  function.
* __uld_get/put is just get_/put_device. Now every thing is accounted
  for on the device object. Internal kref is removed.
* At __remove() we can safely de-allocate the uld_ structure. (The
  function has moved to avoid forward declaration)

Some cleanups
* Use class register/unregister is cleaner for this driver now.
* cdev ref-counting games are no longer necessary

I have incremented the device version string in case of new bugs.

Note: Previous bugfix of taking the reference around fput() still
      applies.

Before we're trying to send a PDU we have to check whether a TMF
is active. If so and if the PDU will be affected by the TMF
we should allow only Data-out PDUs to be sent.

If fast_abort is set, no Data-out PDUs will be sent while
a LUN reset is being processed for a affected LUN.

fast_abort is now ingored during a ABORT TASK tmf. We will not
send any Data-outs for a task if the task is being aborted.

Some of our virtual SCSI hosts don't have a proper bus parent at the
top, which can be a problem for doing DMA on them

This patch makes the host device cache a pointer to the physical bus
device and provides an extra API for setting it (the normal API picks
it up from the parent).  This patch also modifies the qla2xxx and lpfc
vport logic to use the new DMA host setting API.

Joe Eykholt ab593b1873 [SCSI] libfc: register FC4 features with the FC switch
Customers and certification tests have pointed out that we don't
show up on the switch management software as an initiator.

On some MDS switches 'show fcns database' command shows libfc
initiators as 'fcp' not 'fcp:init' like other initiators.

On others switches, I think the switch gets the features by doing a PRLI,
but it may be only certain models or under certain configurations.

Fix this by registering our FC4 features with the RFF_ID CT request
after local port login and after the RFT_ID.

Joe Eykholt 386309ce92 [SCSI] libfcoe: fcoe: simplify receive FLOGI response
There was a locking problem where the fip->lock was held during
the call to update_mac().  The rtnl_lock() must be taken before
the fip->lock, not the other way around.  This fixes that.

Now that fcoe_ctlr_recv_flog() is called only from the response handler
to a FLOGI request, some checking can be eliminated.  Instead of calling
update_mac(), just fill in the granted_mac address for the passed-in
frame (skb).

Eliminate the passed-in source MAC address since it is also in the skb.

Also, in fcoe, call fcoe_set_src_mac() directly instead of going thru
the fip function pointer.  This will generate less code.
Then, since fip isn't needed for LOGO response, use lport as the arg.

Joe Eykholt 093bb6a2d3 [SCSI] libfc: add set_fid function to libfc template
This is to notify the LLD when an FC_ID is assigned to the local port.

The fnic driver needs to push the assigned FC_ID to firmware.
It currently does this by intercepting the FLOGI responses, and
in order to make that code more common with FIP and NPIV, it
makes more sense to wait until the local port has completely
handled the FLOGI or FDISC response.  Also, when we fix
point-to-point FC_ID assignment, we'll need this callback as well.

Add a call to the libfc template, which is called whenever
the local port FC_ID is being assigned.  It defaults to
fc_lport_set_fid(), supplied by libfc.

As additional benefit of this function, the LLD may determine
the MAC address that caused the change by looking at the received frame.

We also print the assigned port ID as long as it isn't 0.
Setting port ID to 0 happens often in reset while retrying FLOGI,
and would be uninteresting.  This replaces the previous message
which didn't identify the host adapter instance.

Joe Eykholt dd42dac4ec [SCSI] libfcoe: FIP should report link to libfc whether selected or not
The fnic driver with FIP is reporting link up, even though it's down.

When the interface is shut down by the switch, we receive a clear
virtual link, and set the state reported to libfc as down, although
we still report it up.  Clearly wrong.  That causes the subsequent
link down event not to be reported, and /sys shows the host "Online".

Currently, in FIP mode, if an FCF times out, then link to libfc
is reported as down, to stop FLOGIs.  That interferes with the LLD
link down being reported.

Users really need to know the physical link information, to diagnose
cabling issues, so physical link status should be reported to libfc.

If the selected FCF needs to be reported, that should be done
separately, in a later patch.

This patch makes a variety of cleanup changes to all libfc files.

This patch adds kernel-doc headers to all functions lacking them
and attempts to better format existing headers. It also add kernel-doc
headers to structures.

This patch ensures that the current naming conventions for local ports,
remote ports and remote port private data is upheld in the following
manner.

Chris Leech 11b5618866 [SCSI] libfcoe, fcoe: libfcoe NPIV support
The FIP code in libfcoe needed several changes to support NPIV

1) dst_src_addr needs to be managed per-n_port-ID for FPMA fabrics with NPIV
   enabled.  Managing the MAC address is now handled in fcoe, with some slight
   changes to update_mac() and a new get_src_addr() function pointer.

2) The libfc elsct_send() hook is used to setup FCoE specific response
   handlers for FIP encapsulated ELS exchanges.  This lets the FCoE specific
   handling know which VN_Port the exchange is for, and doesn't require
   tracking OX_IDs.  It might be possible to roll back to the full FIP frame
   in these, but for now I've just stashed the contents of the MAC address
   descriptor in the skb context block for later use.  Also, because
   fcoe_elsct_send() just passes control on to fc_elsct_send(), all transmits
   still come through the normal frame_send() path.

3) The NPIV changes added a mutex hold in the keep alive sending, the lport
   mutex is protecting the vport list.  We can't take a mutex from a timer,
   so move the FIP keep alive logic to the link work struct.

Chris Leech 8faecddb21 [SCSI] libfc: vport link handling and fc_vport state managment
NPIV vports are managed in libfc by changing their virtual link state
when the parent N_Ports internal state changes.  The vport link is only
online when the N_Port is in a ready state (logged into the fabric).

vport_state is updated as needed in this patch as well, currently the states
LINKDOWN, INITIALIZING, ACTIVE, DSIABLED, and NO_FABRIC_SUPP are used.

This also changes the fc_host port_state handling to differentiate between
LINKDOWN and OFFLINE.

I'd like to keep basic initialization together with allocation, which means
this can't just be a tail-call to scsi_host_alloc.

This is needed to create a generic libfc host allocation routine for NPIV
VN_Ports, which will share the exchange ID space (through sharing exchange
manager structures) with the parent lport.  In order to clone the exchange
manager list when the lport is allocated, the list head must be initialized
earlier.

Also, update fnic to use the libfc_host_alloc so that later changes do not break

Robert Love 1a7b75ae71 [SCSI] libfc: Move non-common routines and prototypes out of libfc.h
This patch moves all non-common routines and function prototypes
out of libfc.h and into the appropriate .c files. It makes these
routines 'static' when necessary and removes any unnecessary EXPORT_SYMBOL
statements.

A result of moving the fc_exch_seq_send, fc_seq_els_rsp_send, fc_exch_alloc
and fc_seq_start_next prototypes out of libfc.h is that they were no longer
being imported into fc_exch.c when libfc.h was included. This caused errors
where routines in fc_exch.c were looking for undefined symbols. To fix this
this patch reorganizes fc_seq_alloc, fc_seq_start_next and
fc_seq_start_next_locked. This move also made it so that
fc_seq_start_next_locked did not need to be prototyped at the top of
fc_exch.c.

Vasu Dev 4a84067dbf [SCSI] add queue_depth ramp up code
Current FC HBA queue_depth ramp up code depends on last queue
full time. The sdev already  has last_queue_full_time field to
track last queue full time but stored value is truncated by
last four bits.

So this patch updates last_queue_full_time without truncating
last 4 bits to store full value and then updates its only
current usages in scsi_track_queue_full to ignore last four bits
to keep current usages same while also use this field
in added ramp up code.

Adds scsi_handle_queue_ramp_up to ramp up queue_depth on
successful completion of IO. The scsi_handle_queue_ramp_up will
do ramp up on all luns of a target, just same as ramp down done
on all luns on a target.

The ramp up is skipped in case the change_queue_depth is not
supported by LLD or already reached to added max_queue_depth.

Updates added max_queue_depth on every new update to default
queue_depth value.

The ramp up is also skipped if lapsed time since either last
queue ramp up or down is less than LLD specified
queue_ramp_up_period.

Adds queue_ramp_up_period to sysfs but only if change_queue_depth
is supported since ramp up and queue_ramp_up_period is needed only
in case change_queue_depth is supported first.

Initializes queue_ramp_up_period to 120HZ jiffies as initial
default value, it is same as used in existing lpfc and qla2xxx.

Mike Christie e881a172da [SCSI] modify change_queue_depth to take in reason why it is being called
This patch modifies scsi_host_template->change_queue_depth so that
it takes an argument indicating why it is being called. This will be
used so that if a LLD needs to do some extra processing when
handling queue fulls or later ramp ups, it can do so.

This is a simple port of the drivers setting a change_queue_depth
callback. In the patch I just have these LLDs adjust the queue depth
if the user was requesting it.

Joe Eykholt b4a9c7ede9 [SCSI] libfc: fix free of fc_rport_priv with timer pending
Timer crashes were caused by freeing a struct fc_rport_priv
with a timer pending, causing the timer facility list to be
corrupted.  This was during FC uplink flap tests with a lot
of targets.

After discovery, we were doing an PLOGI on an rdata that was
in DELETE state but not yet removed from the lookup list.
This moved the rdata from DELETE state to PLOGI state.
If the PLOGI exchange allocation failed and needed to be
retried, the timer scheduling could race with the free
being done by fc_rport_work().

When fc_rport_login() is called on a rport in DELETE state,
move it to a new state RESTART.  In fc_rport_work, when
handling a LOGO, STOPPED or FAILED event, look for restart
state.  In the RESTART case, don't take the rdata off the
list and after the transport remote port is deleted and
exchanges are reset, re-login to the remote port.

Note that the new RESTART state also corrects a problem we
had when re-discovering a port that had moved to DELETE state.
In that case, a new rdata was created, but the old rdata
would do an exchange manager reset affecting the FC_ID
for both the new rdata and old rdata.  With the new state,
the new port isn't logged into until after any old exchanges
are reset.

Yi Zou d37322a43e [SCSI] libfc: Fix frags in frame exceeding SKB_MAX_FRAGS in fc_fcp_send_data
In case of sequence offload, in fc_fcp_send_data(), the skb_fill_page_info()
called may end up adding more frags to the skb_shinfo(fp_skb(fp))->frags[],
exceeding SKB_MAX_FRAGS, this eventually corrupts the memory. I am adding the
FR_FRAME_SG_LEN back, but as SKB_MAX_FRAGS -1, leaving 1 for our fcoe_eof_crc
page. And send will be broken into multiple large sends if the frame already
contains more frags than skb handle.

James Bottomley 860dc73608 [SCSI] fix async scan add/remove race resulting in an oops
Async scanning introduced a very wide window where the SCSI device is
up and running but has not yet been added to sysfs.  We delay the
adding until all scans have completed to retain the same ordering as
sync scanning.

This delay in visibility causes an oops if a device is removed before
we make it visible because the SCSI removal routines have an inbuilt
assumption that if a device is in SDEV_RUNNING state, it must be
visible (which is not necessarily true in the async scanning case).

Fix this by introducing an additional is_visible flag which we can use
to condition the tear down so we do the right thing for running but
not yet made visible.

The checksum format is orthogonal to whether the protection information
is being passed on beyond the HBA or not.  It is perfectly valid to use
a non-T10 CRC with WRITE_STRIP and READ_INSERT.

Consequently it no longer makes sense to explicitly refer to the
conversion in the protection operation.  Update sd_dif and lpfc
accordingly.

Mike Christie 661134ad37 [SCSI] libiscsi, bnx2i: make bound ep check common
bnx2i currently has a check for if a ep is properly bound, so if
iscsi_queuecommand/xmit_task is called while there is no ep
we will not queue IO.

be2iscsi sends IO from queuecommand/xmit_task like how bnx2i does
and needs a similar test. This patch has us just use the suspend_bit
test for this.

When ep_poll has succeeed iscsid will call conn_bind, the LLD will
then call iscsi_conn_bind which will clear the suspend bit.
When ep_disconnect is called (or if there is a conn error) we set
the suspend bit. For the ep_disconnect case I am adding a helper
in this patch that will take the session lock to make sure
iscsi_queuecommand/xmit_task is not running and it will set
the suspend bit.

Joe Eykholt 2ab7e1ecb8 [SCSI] libfc: send GPN_ID in reaction to single-port RSCNs.
When an RSCN indicates changes to individual remote ports,
don't blindly log them out and then back in.  Instead, determine
whether they're still in the directory, by doing GPN_ID.

If that is successful, call login, which will send ADISC and reverify,
otherwise, call logoff.  Perhaps we should just delete the rport,
not send LOGO, but it seems safer.

Also, fix a possible issue where if a mix of records in the RSCN
cause us to queue disc_ports for disc_single and then we decide
to do full rediscovery, we leak memory for those disc_ports queued.

So, go through the list of disc_ports even if doing full discovery.
Free the disc_ports in any case.  If any of the disc_single() calls
return error, do a full discovery.

The ability to fill in GPN_ID requests was added to fc_ct_fill().
For this, it needs the FC_ID to be passed in as an arg.
The did parameter for fc_elsct_send() is used for that, since the
actual D_DID will always be 0xfffffc for all CT requests so far.

When rport_login is called on an rport that is already thought
to be logged in, use ADISC.  If that fails, redo PLOGI.
This is less disruptive after fabric changes that don't affect
the state of the target.

Implement the sending of ADISC via fc_els_fill.

Add ADISC state to the rport state machine.  This is entered from READY
and returns to READY after successful completion.  If it fails, the rport
is either logged off and deleted or re-does PLOGI.

When receiving an RSCN, do not log off all rports.  This is
extremely disruptive.  If, after the GPN_FT response, some
rports haven't been listed, delete them.

Add field disc_id to structs fc_rport_priv and fc_disc.
disc_id is an arbitrary serial number used to identify the
rports found by the latest discovery.  This eliminates the need
to go through the rport list when restarting discovery.

Joe Eykholt f211fa514a [SCSI] libfc: make rport structure optional
Allow a struct fc_rport_priv to have no fc_rport associated with it.
This sets up to remove the need for "rogue" rports.

Add a few fields to fc_rport_priv that are needed before the fc_rport
is created.  These are the ids, maxframe_size, classes, and rport pointer.

Remove the macro PRIV_TO_RPORT().  Just use rdata->rport where appropriate.

To take the place of the get_device()/put_device ops that were used to
hold both the rport and rdata, add a reference count to rdata structures
using kref.  When kref_get decrements the refcount to zero, a new template
function releasing the rdata should be called.  This will take care of
freeing the rdata and releasing the hold on the rport (for now).  After
subsequent patches make the rport truly optional, this release function
will simply free the rdata.

Remove the simple inline function fc_rport_set_name(), which becomes
semanticly ambiguous otherwise.  The caller will set the port_name and
node_name in the rdata->Ids, which will later be copied to the rport
when it its created.

Joe Eykholt a46f327aa5 [SCSI] libfc: change elsct to use FC_ID instead of rdata
tt.elsct_send is used by both FCP and by the rport state machine.
After further patches, these two modules will use different
structures for the remote port.

So, change elsct_send to use the FC_ID instead of the fc_rport_priv
as its argument.  It currently only uses the FC_ID anyway.

For CT requests the destination FC_ID is still implicitly 0xfffffc.
After further patches the did arg on CT requests will be used to
specify the FC_ID being inquired about for GPN_ID or other queries.

Joe Eykholt 795d86f55e [SCSI] libfc: change interface for rport_create
The interface for lport->tt.rport_create() takes a fc_disc_port arg,
which is unnatural for most calls.   The only reason for this was
to avoid passing in the local port as an argument, but otherwise
added to complexity.

Simplify by just using lport and fc_rport_identifiers.

Joe Eykholt ab28f1fd3b [SCSI] libfc: prepare to split off struct fc_rport_priv from fc_rport_libfc_priv
While the I/O and LLD interfaces use fc_rport_libfc_priv, the
disc and rport interfaces will use fc_rport_priv, which will
be separately allocated.

Change the disc and rport usage of fc_rport_libfc_priv to fc_rport_priv.

Use #define temporarily to make both names equivalent until a
subsequent patch splits them.

Vasu Dev b2f0091fbf [SCSI] fcoe, libfc: fully makes use of per cpu exch pool and then removes em_lock
1. Updates fcoe_rcv() to queue incoming frames to the fcoe per
   cpu thread on which this frame's exch was originated and simply
   use current cpu for request exch not originated by initiator.
   It is redundant to add this code under CONFIG_SMP, so removes
   CONFIG_SMP uses around this code.

2. Updates fc_exch_em_alloc, fc_exch_delete, fc_exch_find to use
   per cpu exch pools, here fc_exch_delete is rename of older
   fc_exch_mgr_delete_ep since ep/exch are now deleted in pools
   of EM and so brief new name is sufficient and better name.

   Updates these functions to map exch id to their index into exch
   pool using fc_cpu_mask, fc_cpu_order and EM min_xid.
   This mapping is as per detailed explanation about this in
   last patch and basically this is just as lower fc_cpu_mask
   bits of exch id as cpu number and upper bit sum of EM min_xid
   and exch index in pool.

   Uses pool next_index to keep track of exch allocation from
   pool along with pool_max_index as upper bound of exches array
   in pool.

3. Adds exch pool ptr to fc_exch to free exch to its pool in
   fc_exch_delete.

4. Updates fc_exch_mgr_reset to reset all exch pools of an EM,
   this required adding fc_exch_pool_reset func to reset exches
   in pool and then have fc_exch_mgr_reset call fc_exch_pool_reset
   for each pool within each EM for a lport.

5. Removes no longer needed exches array, em_lock, next_xid, and
   total_exches from struct fc_exch_mgr, these are not needed after
   use of per cpu exch pool, also removes not used max_read,
   last_read from struct fc_exch_mgr.

6. Updates locking notes for exch pool lock with fc_exch lock and
   uses pool lock in exch allocation, lookup and reset.

Vasu Dev e4bc50bedf [SCSI] fcoe, libfc: adds per cpu exch pool within exchange manager(EM)
Adds per cpu exch pool for these reasons:-

 1. Currently an EM instance is shared across all cpus to manage
    all exches for all cpus. This required em_lock across all
    cpus for an exch alloc, free, lookup and reset each frame
    and that made em_lock expensive, so instead having per cpu
    exch pool with their own per cpu pool lock will likely reduce
    locking contention in fast path for an exch alloc, free and
    lookup.

 2. Per cpu exch pool will likely improve cache hit ratio since
    all frames of an exch will be processed on the same cpu on
    which exch originated.

This patch is only prep work to help in keeping complexity of next
patch low, so this patch only sets up per cpu exch pool and related
helper funcs to be used by next patch. The next patch fully makes
use of per cpu exch pool in all code paths ie. tx, rx and reset.

Divides per EM exch id range equally across all cpus to setup per
cpu exch pool. This division is such that lower bits of exch id
carries cpu number info on which exch originated, later a simple
bitwise AND operation on exch id of incoming frame with fc_cpu_mask
retrieves cpu number info to direct all frames to same cpu on which
exch originated. This required a global fc_cpu_mask and fc_cpu_order
initialized to max possible cpus number nr_cpu_ids rounded up to 2's
power, this will be used in mapping exch id and exch ptr array
index in pool during exch allocation, find or reset code paths.

Adds a check in fc_exch_mgr_alloc() to ensure specified min_xid
lower bits are zero since these bits are used to carry cpu info.

Adds and initializes struct fc_exch_pool with all required fields
to manage exches in pool.

Allocates per cpu struct fc_exch_pool with memory for exches array
for range of exches per pool. The exches array memory is followed
by struct fc_exch_pool.

Adds fc_exch_ptr_get/set() helper functions to get/set exch ptr in
pool exches array at specified array index.

Increases default FCOE_MAX_XID to 0x0FFF from 0x07EF, so that more
exches are available per cpu after above described exch id range
division across all cpus to each pool.

Mike Christie d1af8a3287 [SCSI] iscsi_tcp: add new conn error to indicate tcp conn closed
If a target closed the connection, we will detect it in the
state_changed or data_ready callout. This adds a new conn
error value to use for this problem, so it is not confused
with when the initiator throws a conn error and drops
the connection.

Chandra Seetharaman 18ee70c9d7 [SCSI] scsi_dh: add the interface scsi_dh_set_params()
When we moved the device handler functionality from dm layer to SCSI layer
we dropped the parameter functionality.

This path adds an interface to scsi dh layer to set device handler
parameters.

Basically, multipath layer need to create a string with all the parameters
and call scsi_dh_set_params() after it called scsi_dh_attach() on a
device.

If a device handler provides such an interface it will handle the parameters
as it expects them.

Yi Zou 537029f8e9 [SCSI] libfc: Remove FC_FRAME_SG_LEN in fc_fcp_send_data
FC_FRAME_SG_LEN is 4 which is too small when offload is enabled. Actually, the
WARN_ON() in fc_fcp_send_data() should be:

	WARN_ON(skb_shinfo(fp_skb(fp))->nr_frags > MAX_SKB_FRAGS);

But since we will not get anything more than 64K anyway, so there is no need
to do this anyway here. Therefore, I am getting rid of FC_FRAME_SG_LEN here
and the WARN_ON here.

Vasu Dev 52ff878c91 [SCSI] fcoe, fnic, libfc: modifies current code paths to use EM anchor list
Modifies current code to use EM anchor list in EM allocation, EM free,
EM reset, exch allocation and exch lookup code paths.

 1. Modifies fc_exch_mgr_alloc to accept EM match function and then
    have allocated EM added to the lport using fc_exch_mgr_add API
    while also updating EM kref for newly added EM.

 2. Updates fc_exch_mgr_free API to accept only lport pointer instead
    EM and then have this API free all EMs of the lport from EM anchor
    list.

 3. Removes single lport pointer link from the EM, which was used in
    associating lport pointer in newly allocated exchange. Instead have
    lport pointer passed along new exchange allocation call path and
    then store passed lport pointer in newly allocated exchange, this
    will allow a single EM instance to be used across more than one
    lport and used in EM reset to reset only lport specific exchanges.

 4. Modifies fc_exch_mgr_reset to reset all EMs from the EM anchor list
    of the lport, adds additional exch lport pointer (ep->lp) check for
    shared EM case to reset exchange specific to a lport requested reset.

 5. Updates exch allocation API fc_exch_alloc to use EM anchor list and
    its anchor match func pointer. The fc_exch_alloc will walk the list
    of EMs until it finds a match, a match will be either null match
    func pointer or call to match function returning true value.

 6. Updates fc_exch_recv to accept incoming frame on local port using
    only lport pointer and frame pointer without specifying EM instance
    of incoming frame. Instead modified fc_exch_recv to locate EM for the
    incoming frame by matching xid of incoming frame against a EM xid range.
    This change was required to use EM list in libfc Rx path and after this
    change the lport fc_exch_mgr pointer emp is not needed anymore, so
    removed emp pointer.

 7. Updates fnic for removed lport emp pointer and above modified libfc APIs
    fc_exch_recv, fc_exch_mgr_alloc and fc_exch_mgr_free.

 8. Removes exch_get and exch_put from libfc_function_template as these
    are no longer needed with EM anchor list and its match function use.
    Also removes its default function fc_exch_get.

A defect this patch introduced regarding the libfc initialization order in
the fnic driver was fixed by Joe Eykholt <jeykholt@cisco.com>.

Robert Love d459b7ea1b [SCSI] libfc: Remove the FC_EM_DBG macro
Currently there is a 1:1 relationship between the lport
and exchange manager. This macro takes an EM as an argument
and determines the lport from it. However, later patches
will use an EM list per lport, so we will no longer have
this 1:1 relationship- this macro must change.

The FC_EM_DBG macro is rarely used. There are four callers,
two can use FC_LPORT_DBG instead and two can be removed
since they're not necessary. This patch makes those changes
and removes the macro.

Vasu Dev 96316099ac [SCSI] fcoe, libfc: adds exchange manager(EM) anchor list per lport and related APIs
Adds EM list using a anchor struct fc_exch_mgr_anchor, anchor is used
to allow same EM instance sharing across more than one lport on a eth
device, this implementation is per discussed design posted at
http://www.open-fcoe.org/pipermail/devel/2009-June/002566.html.

The shared EM is required for multiple lports on eth device when
using multiple VLANs or NPIV.

Adds fc_exch_mgr_add API to add a EM to the lport and fc_exch_mgr_del
API to delete previously added EM.

Also adds function fc_exch_mgr_destroy() to destroy allocated EM.
The kref is added to the EM to keep track of EM usage count, the EM is
destroyed when no longer in use upon kref reaching to zero.

The caller can specify match function to fc_exch_mgr_add, this
will be used in determining exchange allocation from its EM or not.

Moved calling of fcoe_em_config below fcoe_libfc_config calling,
so that list head lp->ema_list is initialized before configuring
EM.

Joe Eykholt 141940548c [SCSI] libfc: rename rport state "NONE" to "DELETE".
State RPORT_ST_NONE was intented to be an invalid state (0), never used.
This was a misguided attempt to be sure it was always initialized.
Having an extra state meaning nothing requires switch statements to
have a case covering that state.

State NONE has been used instead to mean the remote port is being deleted.
Changing the name to RPORT_ST_DELETE.

the initial state.  Rename it to be DISABLED, as more descriptive.

Joe Eykholt 7f74549ff6 [SCSI] libfc: change debug messages to give host number.
libfc debug messages currently show 'lport: <fc-id>:'
wher <fc-id> is the hex assigned port-id.  When the lport
is logged off, that will be zero, so its hard to distinguish
which instance is involved.  The FC-ID can change
if the port is re-patched or changes VSANs.

Two lports may even have the same FC-ID if connected to isolated SANs.

Change the debug messages to print the SCSI host number "hostN:",
which will not change for the life of the lport.
Still show the FC_ID on lport messages.

Also, add a macro to FC_RPORT_ID_DBG for rport debugging where there's
no rdata structure involved.  It takes the lport and port_id as parameters.
Use this in fc_rport_recv_plogi_req() and fc_rport_recv_logo_req().

Robert Love 7414705ea4 libfc: Add runtime debugging with debug_logging module parameter
This patch adds the /sys/module/libfc/parameters/debug_logging
file to sysfs as a module parameter. It accepts an integer
bitmask for logging. Currently it supports:

   bit
LSB 0 = general libfc debugging
    1 = lport debugging
    2 = disc debugging
    3 = rport debugging
    4 = fcp debugging
    5 = EM debugging
    6 = exch/seq debugging
    7 = scsi logging (mostly error handling)

the other bits are not used at this time.

The patch converts all of the libfc source files to use
these new macros and removes the old FC_DBG macro.

James Bottomley ea4431906d [SCSI] aic79xx: make driver respect nvram for IU and QAS settings
This patch allows the Adaptec firmware to pass on its values for Packetize and
QAS.  To do this, the settings max_iu and max_qas have been introduced into
the SPI transport class and populated from the adaptec NVram tables.  Domain
validation in the SPI transport class will respect the max settings when
configuring to the highest possible speed for testing.

James Smart 9e4f5e2961 [SCSI] FC Pass Thru support
Attached is the ELS/CT pass-thru patch for the FC Transport. The patch
creates a generic framework that lays on top of bsg and the SGIO v4 ioctl
in order to pass transaction requests to LLDD's.

The interface supports the following operations:
  On an fc_host basis:
    Request login to the specified N_Port_ID, creating an fc_rport.
    Request logout of the specified N_Port_ID, deleting an fc_rport
    Send ELS request to specified N_Port_ID w/o requiring a login, and
      wait for ELS response.
    Send CT request to specified N_Port_ID and wait for CT response.
      Login is required, but LLDD is allowed to manage login and decide
      whether it stays in place after the request is satisfied.
    Vendor-Unique request. Allows a LLDD-specific request to be passed
      to the LLDD, and the passing of a response back to the application.
  On an fc_rport basis:
    Send ELS request to nport and wait for ELS response.
    Send CT request to nport and wait for CT response.

The patch also exports several headers from include/scsi such that
they can be available to user-space applications:

Boaz Harrosh 021e2230d6 [SCSI] osduld: use filp_open() when looking up an osd-device
This patch was inspired by Al Viro, for simplifying and fixing the
retrieval of osd-devices by in-kernel users, eg: file systems.
In-Kernel users, now, go through the same path user-mode does by
opening a file on the osd char-device and though holding a reference
to both the device and the Module.

A file pointer was added to the osd_dev structure which is now
allocated for each user. The internal osd_dev is no longer exposed
outside of the uld. I wanted to do that for a long time so each
libosd user can have his own defaults on the device.

The API is left the same, so user code need not change.

It is no longer needed to open/close a file handle on the osd
char-device from user-mode, before mounting an exofs on it.

Boaz Harrosh 62f469b596 [SCSI] libosd: osd_req_{read,write} takes a length parameter
For supporting of chained-bios we can not inspect the first
bio only, as before. Caller shall pass the total length of the
request, ie. sum_bytes(bio-chain).

Also since the bio might be a chain we don't set it's direction
on behalf of it's callers. The bio direction should be properly
set prior to this call. So fix a couple of write users that now
need to set the bio direction properly

[In this patch I change both library code and user sites at
 exofs, to make it easy on integration. It should be submitted
 via James's scsi-misc tree.]

Michael Chan 43514774ff [SCSI] iscsi class: Add new NETLINK_ISCSI messages for cnic/bnx2i driver.
Add ISCSI_NETLINK messages for iSCSI NICs to get information such as
path from userspace.  Original iscsid messages are now always sent as
multicast to group 1.  The new messages are sent to group 2.

Mike Christie b3cd5050bf [SCSI] libiscsi: add task aborted state
If a task did not complete normally due to a TMF, libiscsi will
now complete the task with the state ISCSI_TASK_ABRT_TMF. Drivers
like bnx2i that need to free resources if a command did not complete normally
can then check the task state. If a driver does not need to send
a special command if we have dropped the session then they can check
for ISCSI_TASK_ABRT_SESS_RECOV.

Mike Christie 3bbaaad95f [SCSI] libiscsi: handle cleanup task races
bnx2i needs to send a hardware specific cleanup command if
a command has not completed normally (iscsi/scsi response from
target), and the session is still ok (this is the case when we
send a TMF to stop the command).

At this time it will need to drop the session lock. The problem
with the current code is that fail_all_commands assumes we
will hold the lock the entire time, so it uses list_for_each_entry_safe.
If while bnx2i drops the session lock multiple cmds complete then
list_for_each_entry_safe will not handle this correctly.

This patch removes the running lists and just has us loop over
the cmds array (in later patches we will then replace that
array with a block tag map at the session level). It also fixes
up the completion path so that if the TMF code and the normal recv
path were completing the same command then they both do not try
to do release the refcount taken when the task is queued.

Mike Christie 10eb0f013c [SCSI] iscsi: pass ep connect shost
When we create the tcp/ip connection by calling ep_connect, we currently
just go by the routing table info.

I think there are two problems with this.

1. Some drivers do not have access to a routing table. Some drivers like
qla4xxx do not even know about other ports.

2. If you have two initiator ports on the same subnet, the user may have
set things up so that session1 was supposed to be run through port1. and
session2 was supposed to be run through port2. It looks like we could
end with both sessions going through one of the ports.

Vasu Dev 184dd3459b fcoe: adds spma mode support
If we can find a type NETDEV_HW_ADDR_T_SAN mac address from the
corresponding netdev for a fcoe interface then sets up added the
fc->ctlr.spma flag and stores spma mode address in ctl_src_addr.

In case the spma flag is set then:-

 1. Adds spma mode MAC address in ctl_src_addr as secondary
    MAC address, the FLOGI for FIP and pre-FIP will go out
    using this address.
 2. Cleans up stored spma MAC address in ctl_src_addr in
    fcoe_netdev_cleanup.
 3. Sets up spma bit in fip_flags for FIP solicitations along
    with exiting FPMA bit setting.
 4. Initialize the FLOGI FIP MAC descriptor to stored spma
    MAC address in ctl_src_addr. This is used as proposed
    FCoE MAC address from initiator along with both SPMA
    and FPMA bit set in FIP solicitation, in response the
    switch may grant any FPMA or SPMA mode MAC address to
    initiator.

Removes FIP descriptor type checking against ELS type
ELS_FLOGI in fcoe_ctlr_encaps to update a FIP MAC descriptor,
instead now checks against FIP_DT_FLOGI.

I've tested this with available FPMA-only FCoE switch but
since data_src_addr is updated using same old code for
both FPMA and SPMA modes with FIP or pre-FIP links, so added
SPMA mode will work with SPMA-only switch also provided that
switch grants a valid MAC address.

The same can happen for iscsi when transitioning from logged in

This patch converts iscsi and fc's transitions back to use DID_IMM_RETRY

Tejun Heo 83096ebf12 block: convert to pos and nr_sectors accessors
With recent cleanups, there is no place where low level driver
directly manipulates request fields.  This means that the 'hard'
request fields always equal the !hard fields.  Convert all
rq->sectors, nr_sectors and current_nr_sectors references to
accessors.

While at it, drop superflous blk_rq_pos() < 0 test in swim.c.

 * Cryptographic keys and signatures are 20 => 32
 * Few new definitions.

(Still missing new standard definitions attribute values, these do not change
 wire format and will be added later when needed)

In OSD2r04 draft, cryptographic key size changed to 32 bytes from
OSD1's 20 bytes. This causes a couple of on-the-wire structures
to change, including the CDB.

In this patch the OSD1/OSD2 handling is separated out in regard
to affected structures, but on-the-wire is still the same. All
on the wire changes will be submitted in one patch for bisect-ability.

Boaz Harrosh 71f32e31e5 [SCSI] libosd: OSD2r05: Prepare for rev5 attribute list changes
In OSD2r05 draft each attribute list element header was changed
so attribute-value would be 8 bytes aligned. In OSD2r01-r04
it was aligned on 2 bytes. (This is because in OSD2r01 the complete
element was 8 bytes padded at end but the header was not adjusted
and caused permanent miss-alignment.)

OSD1 elements are not padded and might be or might not be aligned.
OSD1 is still supported.

In this code we do all the code re-factoring to separate OSD1/OSD2
differences but do not change actual wire format. All wire format
changes will happen in one patch later, for bisect-ability.

Abhijeet Joglekar b4c6f54632 [SCSI] libfc: Track rogue remote ports
Rogue ports are currently not tracked on any list. The only reference
to them is through any outstanding exchanges pending on the rogue ports.
If the module is removed while a retry is set on a rogue port
(say a Plogi retry for instance), this retry is not cancelled because there
is no reference to the rogue port in the discovery rports list. Thus the
local port can clean itself up, delete the exchange pool, and then the
rogue port timeout can fire and try to start up another exchange.

This patch tracks the rogue ports in a new list disc->rogue_rports. Creating
a new list instead of using the disc->rports list keeps remote port code
change to a minimum.

1)  Whenever a rogue port is created, it is immediately added to the
disc->rogue_rports list.

2) When the rogues port goes to ready, it is removed from the rogue list
and the real remote port is added to the disc->rports list

3) The removal of the rogue from the disc->rogue_rports list is done in
the context of the fc_rport_work() workQ thread in discovery callback.

4) Real rports are removed from the disc->rports list like before. Lookup
is done only in the real rports list. This avoids making large changes
to the remote port code.

5) In fc_disc_stop_rports, the rogues list is traversed in addition to the
real list to stop the rogue ports and issue logoffs on them. This way, rogue
ports get cleaned up when the local port goes away.

6) rogue remote ports are not removed from the list right away, but
removed late in fc_rport_work() context, multiple threads can find the same
remote port in the list and call rport_logoff(). Rport_logoff() only
continues with the logoff if port is not in NONE state, thus preventing
multiple logoffs and multiple list deletions.

7) Since the rport is removed from the disc list at a later stage
(in the disc callback), incoming frames can find the rport even if
rport_logoff() has been called on the rport. When rport_logoff() is called,
the rport state is set to NONE, and we are trying to cancel all exchanges
and retries on that port. While in this state, if an incoming
Plogi/Prli/Logo or other frames match the rport, we should not reply
because the rport is in the NONE state. Just drop the frame, since the
rport will be deleted soon in the disc callback (fc_rport_work)

8)  In fc_disc_single(), remove rport lookup and call to fc_disc_del_target.
fc_disc_single() is called from recv_rscn_req() where rport lookup
and rport_logoff is already done.

Rafael J. Wysocki c751085943 PM/Hibernate: Wait for SCSI devices scan to complete during resume
There is a race between resume from hibernation and the asynchronous
scanning of SCSI devices and to prevent it from happening we need to
call scsi_complete_async_scans() during resume from hibernation.

In addition, if the resume from hibernation is userland-driven, it's
better to wait for all device probes in the kernel to complete before
attempting to open the resume device.

Joe Eykholt 97c8389d54 [SCSI] fcoe, libfcoe: Add support for FIP. FCoE discovery and keep-alive.
FIP is the new standard way to discover Fibre-Channel Forwarders (FCFs)
by sending solicitations and listening for advertisements from FCFs.

It also provides for keep-alives and period advertisements so that both
parties know they have connectivity.  If the FCF loses connectivity to
the storage fabric, it can send a Link Reset to inform the E_node.

This version is also compatible with pre-FIP implementations, so no
configured selection between FIP mode and non-FIP mode is required.

We wait a couple seconds after sending the initial solicitation
and then send an old-style FLOGI.  If we receive any FIP frames,
we use FIP only mode.  If the old FLOGI receives a response,
we disable FIP mode.  After every reset or link up, this
determination is repeated.

The foce_softc mem was reserved by libfc_host_alloc as well as
by fcoe_host_alloc.

Removes one liner fcoe_host_alloc completely, instead directly calls
libfc_host_alloc to alloc scsi_host with libfc for just one fcoe_softc
as fcoe private data.

Moves libfc_host_alloc to libfc.h since it is a libfc API, placed
lport_priv API adjacent to libfc_host_alloc since this is related
to scsi_host priv data.

Vasu Dev 7f34914295 [SCSI] fcoe: removes default sw transport code file fcoe_sw.c
Moves only required code from fcoe_sw.c to libfcoe.c towards having
just one source file for fcoe module, this gets rid off default sw
transport code in a separate fcoe_sw.c file.

Very minor renaming along this move, dropped _sw_ or _SW_ use
in names and replaced them by _if_ as a auxiliary interface
functions. Now some of these funcs can be removed or merged with
other func after fcoe transport is gone, but that should be
in another patch to keep this patch simple.

Now the libfcoe.c file name for fcoe module doesn't go along well,
so the libfcoe.c file renaming to fcoe.c as the only single fcoe
module file is done in next patch to keep this patch clean
and small for review.

Vasu Dev 5919a59503 [SCSI] fcoe: prep work to completely remove fc_transport_fcoe code
The fcoe transport code was added for generic FCoE transport
infrastructure to allow additional offload related module loading
on demand, this is not required anymore after recently added
different offload approach by having offload related func ops
in netdev.

This patch removes fcoe transport related code use, calls functions
directly between existing libfcoe.c and fcoe_sw.c for now, for
example fcoe_sw_destroy and fcoe_sw_create calling.

The fcoe_sw.c and libfcoe.c code will be further consolidated in
later patches and then also the default fcoe sw transport code
file fcoe_sw.c will be completely removed.

The fcoe transport code files are completely removed in next
patch to keep this patch simple for reviewing.

[This patch is an update to a previous patch. This update

Robert Love 38eccabd10 [SCSI] fcoe: Initialize all possilbe skb_queue(s) when module is loaded
Currently the skb_queue is initialized every time the associated
CPU goes online. This patch has libfcoe initializing the skb_queue
for all possible CPUs when the module is loaded.

This patch also re-orders some declarations in the fcoe_rcv()
function so the structure declarations are grouped before
the primitive declarations.

Lastly, this patch converts all CPU indicies to use unsigned int
since CPU indicies should not be negative.

The qdepth setting was useful when we needed libiscsi to verify
the setting. Now we just need to make sure if older tools
passed in zero then we need to set some default.

So this patch just has us use the sht->cmd_per_lun or if
for LLD does a host per session then we can set it on per
host basis.

When LLD supports direct data placement (ddp) for large receive of an scsi
i/o coming into fc_fcp, we call into libfc_function_template's ddp_setup()
to prepare for a ddp of large receive for this read I/O. When I/O is complete,
we call the corresponding ddp_done() to get the length of data ddped as well
as to let LLD do clean up.

fc_fcp_ddp_setup()/fc_fcp_ddp_done() are added to setup and complete a ddped
read I/O described by the given fc_fcp_pkt. They would call into corresponding
ddp_setup/ddp_done implemented by the fcoe layer. Eventually, fcoe layer calls
into LLD's ddp_setup/ddp_done provided through net_device

This checks if net_devices supports FCoE offload ops in netdev_ops and it
if it does, then sets up the corresponding flags in the associated fc_lport.

For large send offload, the maximum length supported in one large send is now
described by the added lso_max in fc_lport, which is setup initially from
netdev->gso_max_size.

Implementation of the osd_req_decode_sense() API. Can be called by
library users to decode what failed in command executions.

Add SCSI_OSD_DPRINT_SENSE Kconfig variable. Possible values are:
0 - Do not print any errors to messages file <KERN_ERR>
1 - (Default) Print only decoded errors that are not recoverable.
    Recoverable errors are those that the target has complied with
    the request but with a warning. For example read passed end of
    object will return zeros after the last valid byte.
2- Print all errors.

Boaz Harrosh b799bc7da0 [SCSI] osd_uld: API for retrieving osd devices from Kernel
Kernel clients like exofs can retrieve struct osd_dev(s)
by means of below API.

+ osduld_path_lookup() - given a path (e.g "/dev/osd0") locks and
returns the corresponding struct osd_dev, which is then needed
for subsequent libosd use.

+ osduld_put_device() - free up use of an osd_dev.

Devices can be shared by multiple clients. The osd_uld_device's
life time is governed by an embedded kref structure.

The osd_uld_device holds an extra reference to both it's
char-device and it's scsi_device, and will release these just
before the final deallocation.

There are three possible lock sources of the osd_uld_device
1. First and for most is the probe() function called by
  scsi-ml upon a successful login into a target. Released in release()
  when logout.
2. Second by user-mode file handles opened on the char-dev.
3. Third is here by Kernel users.
All three locks must be removed before the osd_uld_device is freed.

The MODULE has three lock sources as well:
1. scsi-ml at probe() time, removed after release(). (login/logout)
2. The user-mode file handles open/close.
3. Import symbols by client modules like exofs.

TODO:
  This API is not enough for the pNFS-objects LD. A more versatile
  API will be needed. Proposed API could be:
  struct osd_dev *osduld_sysid_lookup(const char id[OSD_SYSTEMID_LEN]);

Vasu Dev c826a31457 [SCSI] fcoe: Out of order tx frames was causing several check condition SCSI status
frames followed by these errors in log.

	[sdp] Result: hostbyte=DID_OK driverbyte=DRIVER_SENSE,SUGGEST_OK
	[sdp] Sense Key : Aborted Command [current]
	[sdp] Add. Sense: Data phase error

This was causing some test apps to exit due to write failure under heavy
load.

This was due to a race around adding and removing tx frame skb in
fcoe_pending_queue, Chris Leech helped me to find that brief unlocking
period when pulling skb from fcoe_pending_queue in various contexts
(fcoe_watchdog and fcoe_xmit) and then adding skb back into fcoe_pending_queue
up on a failed fcoe_start_io could change skb/tx frame order in
fcoe_pending_queue. Thanks Chris.

This patch allows only single context to pull skb from fcoe_pending_queue
at any time to prevent above described ordering issue/race by use of
fcoe_pending_queue_active flag.

This patch simplified fcoe_watchdog with modified fcoe_check_wait_queue by
use of FCOE_LOW_QUEUE_DEPTH instead previously used several conditionals
to clear and set lp->qfull.

I think FCOE_MAX_QUEUE_DEPTH with FCOE_LOW_QUEUE_DEPTH  will work better
in re/setting lp->qfull and these could be fine tuned for performance.

Chris Leech 6755db1cd4 [SCSI] libfc: rport retry on LS_RJT from certain ELS
This allows any rport ELS to retry on LS_RJT.

The rport error handling would only retry on resource allocation failures
and exchange timeouts.  I have a target that will occasionally reject PLOGI
when we do a quick LOGO/PLOGI.  When a critical ELS was rejected, libfc would
fail silently leaving the rport in a dead state.

The retry count and delay are managed by fc_rport_error_retry.  If the retry
count is exceeded fc_rport_error will be called.  When retrying is not the
correct course of action, fc_rport_error can be called directly.

Vasu Dev bc0e17f691 [SCSI] libfc, fcoe: fixed locking issues with lport->lp_mutex around lport->link_status
The fcoe_xmit could call fc_pause in case the pending skb queue len is larger
than FCOE_MAX_QUEUE_DEPTH, the fc_pause was trying to grab lport->lp_muex to
change lport->link_status and that had these issues :-

1. The fcoe_xmit was getting called with bh disabled, thus causing
"BUG: scheduling while atomic" when grabbing lport->lp_muex with bh disabled.

2. fc_linkup and fc_linkdown function calls lport_enter function with
lport->lp_mutex held and these enter function in turn calls fcoe_xmit to send
lport related FC frame, e.g. fc_linkup => fc_lport_enter_flogi to send flogi
req. In this case grabbing the same lport->lp_mutex again in fc_puase from
fcoe_xmit would cause deadlock.

The lport->lp_mutex was used for setting FC_PAUSE in fcoe_xmit path but
FC_PAUSE bit was not used anywhere beside just setting and clear this
bit in lport->link_status, instead used a separate field qfull in fc_lport
to eliminate need for lport->lp_mutex to track pending queue full condition
and in turn avoid above described two locking issues.

Also added check for lp->qfull in fc_fcp_lport_queue_ready to trigger
SCSI_MLQUEUE_HOST_BUSY when lp->qfull is set to prevent more scsi-ml cmds
while lp->qfull is set.

This patch eliminated FC_LINK_UP and FC_PAUSE and instead used dedicated
fields in fc_lport for this, this simplified all related conditional
code.

Also removed fc_pause and fc_unpause functions and instead used newly added
lport->qfull directly in fcoe.

Abhijeet Joglekar 1f6ff364ce [SCSI] libfc: Pass lport in exch_mgr_reset
fc_exch_mgr structure is private to fc_exch.c. To export exch_mgr_reset to
transport, transport needs access to the exch manager. Change
exch_mgr_reset to use lport param which is the shared structure between
libFC and transport.

Alternatively, fc_exch_mgr definition can be moved to libfc.h so that lport
can be accessed from mp*.

Robert Love 42e9a92fe6 [SCSI] libfc: A modular Fibre Channel library
libFC is composed of 4 blocks supported by an exchange manager
and a framing library. The upper 4 layers are fc_lport, fc_disc,
fc_rport and fc_fcp. A LLD that uses libfc could choose to
either use libfc's block, or using the transport template
defined in libfc.h, override one or more blocks with its own
implementation.

The EM (Exchange Manager) manages exhcanges/sequences for all
commands- ELS, CT and FCP.

The framing library frames ELS and CT commands.

The fc_lport block manages the library's representation of the
host's FC enabled ports.

The fc_disc block manages discovery of targets as well as
handling changes that occur in the FC fabric (via. RSCN events).

The fc_rport block manages the library's representation of other
entities in the FC fabric. Currently the library uses this block
for targets, its peer when in point-to-point mode and the
directory server, but can be extended for other entities if
needed.

The fc_fcp block interacts with the scsi-ml and handles all
I/O.

Mike Christie 6df19a791b [SCSI] libiscsi_tcp: support padding offload
cxgb3i does not offload the processing of the header,
but it will always process the padding. This patch
adds a padding offload flag to detect when the LLD
supports this.

The patch also modifies the header processing so that
we do not try to read/bypass the header dugest in the
skb. cxgb3i will not include it with the header like
with other offload cards.

Mike Christie 262ef63627 [SCSI] libiscsi: allow drivers to modify the itt sent to the target
bnx2i and cxgb3i need to encode LLD info in the itt so that
the firmware/hardware can process the pdu. This patch allows
the LLDs to encode info in the task->hdr->itt that they
setup in the alloc_pdu callout (any resources that are allocated
can be freed with the pdu in the cleanup_task callout). If
the LLD encodes info in the itt they should implement a
parse_pdu_itt callout. If parse_pdu_itt is not implemented
libiscsi will do the right thing for the LLD.

Mike Christie 577577da6d [SCSI] libiscsi: prepare libiscsi for new offload engines by modifying unsol data code
cxgb3i offloads data transfers. It does not offload the entire scsi/iscsi
procssing like qla4xxx and it does not offload the iscsi sequence
processing like how bnx2i does. cxgb3i relies on iscsi_tcp for the
seqeunce handling so this changes how we transfer unsolicitied data by
adding a common r2t struct and helpers.

Mike Christie 63c62f1cb9 [SCSI] iscsi_tcp: prepare helpers for LLDs that can offload some operations
cxgb3i is unlike qla4xxx and bnx2i in that it does not offload entire
scsi commands or iscsi sequences. Instead it only offloads the transfer
of a ISCSI DATA_IN pdu's data, the digests and padding. This patch fixes up the
iscsi tcp recv path so that it exports its skb recv processing so
cxgb3i and other drivers can call them. All they have to do is pass
the function the skb with the hdr or data pdu header and this function
will do the rest.

I had this in my patchset to add target reset support, but
it got dropped due to patching conflicts. This initial patch
just renames the function and users. We are actually just
dropping the session, and so this does not have anything to do
with the host exactly. It does for software iscsi because
we allocate a host per session, but for cxgb3i this makes no
sense.

Mike Christie e5bd7b54e9 [SCSI] libiscsi: Support drivers initiating session removal
If the driver knows when hardware is removed like with cxgb3i,
bnx2i, qla4xxx and iser then we will want to remove the sessions/devices
that are bound to that device before removing the host.

cxgb3i and in the future bnx2i will remove the host and that will
remove all the sessions on the hba. iser can call iscsi_kill_session
when it gets an event that indicates that a hca is removed.
And when qla4xxx is hooked in to the lib (it is only hooked into
the class right now) it can call iscsi remove host like the
partial offload card drivers.

Mike Christie a4dfaa6f2e [SCSI] scsi: add transport host byte errors (v3)
Currently, if there is a transport problem the iscsi drivers will return
outstanding commands (commands being exeucted by the driver/fw/hw) with
DID_BUS_BUSY and block the session so no new commands can be queued.
Commands that are caught between the failure handling and blocking are
failed with DID_IMM_RETRY or one of the scsi ml queuecommand return values.
When the recovery_timeout fires, the iscsi drivers then fail IO with
DID_NO_CONNECT.

For fcp, some drivers will fail some outstanding IO (disk but possibly not
tape) with DID_BUS_BUSY or DID_ERROR or some other value that causes a retry
and hits the scsi_error.c failfast check, block the rport, and commands
caught in the race are failed with DID_IMM_RETRY. Other drivers, may
hold onto all IO and wait for the terminate_rport_io or dev_loss_tmo_callbk
to be called.

The following patches attempt to unify what upper layers will see drivers
like multipath can make a good guess. This relies on drivers being
hooked into their transport class.

This first patch just defines two new host byte errors so drivers can
return the same value for when a rport/session is blocked and for
when the fast_io_fail_tmo fires.

The idea is that if the LLD/class detects a problem and is going to block
a rport/session, then if the LLD wants or must return the command to scsi-ml,
then it can return it with DID_TRANSPORT_DISRUPTED. This will requeue
the IO into the same scsi queue it came from, until the fast io fail timer
fires and the class decides what to do.

When using multipath and the fast_io_fail_tmo fires then the class
can fail commands with DID_TRANSPORT_FAILFAST or drivers can use
DID_TRANSPORT_FAILFAST in their terminate_rport_io callbacks or
the equivlent in iscsi if we ever implement more advanced recovery methods.
A LLD, like lpfc, could continue to return DID_ERROR and then it will hit
the normal failfast path, so drivers do not have fully be ported to
work better. The point of the patches is that upper layers will
not see a failure that could be recovered from while the rport/session is
blocked until fast_io_fail_tmo/recovery_timeout fires.

Mike Christie fff9d40ce0 [SCSI] fc class: unblock target after calling terminate callback (take 2)
When we block a rport and the driver implements the terminate
callback we will fail IO that was running quickly. However
IO that was in the scsi_device/block queue sits there until
the dev_loss_tmo fires, and this can make it look like IO is
lost because new IO will get executed but that IO stuck in
the blocked queue sits there for some time longer.

With this patch when the fast io fail tmo fires, we will
fail the blocked IO and any new IO. This patch also allows
all drivers to partially support the fast io fail tmo. If the
terminate io callback is not implemented, we will still fail blocked
IO and any new IO, so multipath can handle that.

This patch also allows the fc and iscsi classes to implement the
same behavior. The timers are just unfornately named differently.

This patch also fixes the problem where drivers were unblocking
the target in their terminate callback, which was needed for
rport removal, but for fast io fail timeout it would cause
IO to bounce arround the scsi/block layer and the LLD queuecommand.
And it for drivers that could have IO stuck but did not have
a terminate callback the unblock calls in the class will fix
them.

Mike Christie f0c0a376d0 [SCSI] Add helper code so transport classes/driver can control queueing (v3)
SCSI-ml manages the queueing limits for the device and host, but
does not do so at the target level. However something something similar
can come in userful when a driver is transitioning a transport object to
the the blocked state, becuase at that time we do not want to queue
io and we do not want the queuecommand to be called again.

The patch adds code similar to the exisiting SCSI_ML_*BUSY handlers.
You can now return SCSI_MLQUEUE_TARGET_BUSY when we hit
a transport level queueing issue like the hw cannot allocate some
resource at the iscsi session/connection level, or the target has temporarily
closed or shrunk the queueing window, or if we are transitioning
to the blocked state.

bnx2i, when they rework their firmware according to netdev
developers requests, will also need to be able to limit queueing at this
level. bnx2i will hook into libiscsi, but will allocate a scsi host per
netdevice/hba, so unlike pure software iscsi/iser which is allocating
a host per session, it cannot set the scsi_host->can_queue and return
SCSI_MLQUEUE_HOST_BUSY to reflect queueing limits on the transport.

The iscsi class/driver can also set a scsi_target->can_queue value which
reflects the max commands the driver/class can support. For iscsi this
reflects the number of commands we can support for each session due to
session/connection hw limits, driver limits, and to also reflect the
session/targets's queueing window.

Jens Axboe 242f9dcb8b block: unify request timeout handling
Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.

Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

Andrew Vasquez a30c3f69e6 [SCSI] fc_transport: Add an API to allow an LLD to create vports
There's already a fc_vport_termintate() call exported by
the transport.  This patch adds a symmetric call to the API to allow
an NPIV-capable LLD to instantiate vports sans user intervention.

James Bottomley 6f4267e3bd [SCSI] Update the SCSI state model to allow blocking in the created state
Brian King <brking@linux.vnet.ibm.com> reported that fibre channel
devices can oops during scanning if their ports block (because the
device goes from CREATED -> BLOCK -> RUNNING rather than CREATED ->
BLOCK -> CREATED).

Fix this by adding a new state: CREATED_BLOCK which can only transition
back to CREATED and disallow the CREATED -> BLOCK transition.  Now both
the created and blocked states that the mid-layer recognises can include
CREATED_BLOCK.

This patch adds scsi netlink recieve and event support for transport
and scsi LLDD's.  It is a reimplementation of the patch posted last
week by David Somayajulu.

There are a few things done differently:

- Transport support is included

- Event delivery is included

- The vendor message is now its own unique message type, considered
  part of the generic "SCSI Transport".

- LLDD entry points are now registered rather than included in the
  scsi_host_template.

  Background: When I started to implement the event handler via template,
  I had to either: muck up scsi_add_host and scsi_remove_host;  or have
  the event handler search all possible shosts. Neither was acceptable.
  Moving to a registration solves this, and also limits the scope of
  the changes to something that could be backported to a distro without
  breaking an already-released-distro kabi. However, I admit it isn't
  as elegant, as the passing of the LLDD host template in the
  registration and the complexity around dynamic add/remove shows.

- The receive path was augmented to require a unique identifier for
  the LLDD before the message was allowed to be handed off to the
  driver. Given how quickly very fatal errors occur if there's msg
  mismatches (which I saw in testing my own tools :), I believe this
  to be a very good thing. The id plays off the vendor id scheme already
  introduced for the vendor unique event messages used by FC.
  Additionally, the id use as the basis of the registration/deregistration.

- Send assist functions, for both the transport and LLDDs are included.

James Bottomley 01b291bd66 [SCSI] fix check of PQ and PDT bits for WLUNs
For IBM z series certain LUNs can no longer be accessed.

This is because kernel version 2.6.19 a check was introduced not to
create a generic SCSI device for devices that return PQ=1 and
PDT=0x1f. For WLUNs (see SAM-3, p. 41ff) generic SCSI devices should
be created unconditionally without looking at the PQ bit, so add a
check for WLUNs in with this test.

Hugh Dickins d211f052fa [SCSI] sd: fix USB devices incorrectly reporting DIF support
Some USB devices set the protect bit in the INQUIRY data which
currently causes the DIF code in sd to assume (incorrectly) that they
support READ_CAPACITY(16).  Fix this (only for the time being) by
making sure we only believe the protect bit in the inquiry data if the
device claims conformance to SCSI-3 or above.

Alan Jenkins 2b14290078 [SCSI] extend the last_sector_bug flag to cover more sectors
The last_sector_bug flag was added to work around a bug in certain usb
cardreaders, where they would crash if a multiple sector read included the
last sector. The original implementation avoids this by e.g. splitting an 8
sector read which includes the last sector into a 7 sector read, and a single
sector read for the last sector.  The flag is enabled for all USB devices.

This revealed a second bug in other usb cardreaders, which crash when they
get a multiple sector read which stops 1 sector short of the last sector.
Affected hardware includes the Kingston "MobileLite" external USB cardreader
and the internal USB cardreader on the Asus EeePC.

Extend the last_sector_bug workaround to ensure that any access which touches
the last 8 hardware sectors of the device is a single sector long.  Requests
are shrunk as necessary to meet this constraint.

This gives us a safety margin against potential unknown or future bugs
affecting multi-sector access to the end of the device.  The two known bugs
only affect the last 2 sectors.  However, they suggest that these devices
are prone to fencepost errors and that multi-sector access to the end of the
device is not well tested.  Popular OS's use multi-sector accesses, but they
rarely read the last few sectors.  Linux (with udev & vol_id) automatically
reads sectors from the end of the device on insertion.  It is assumed that
single sector accesses are more thoroughly tested during development.

Alan Stern 12265709ac [SCSI] scsi_eh_prep_cmnd should save scmd->underflow
This patch (as1116) fixes a bug in scsi_eh_prep_cmnd() and
scsi_eh_restore_cmnd().  These routines are supposed to save any
values they change and restore them later, but someone forgot to
save & restore scmd->underflow.

This fixes part of the problem reported in Bugzilla #9638.

Martin K. Petersen db007fc5e2 [SCSI] Command protection operation
Controllers that support DMA of protection information must be told
explicitly how to handle the I/O.  The controller has no knowledge of
the protection capabilities of the target device so this information
must be passed in the scsi_cmnd.

 - The protection operation tells the HBA whether to generate, strip or
   verify protection information.

 - The protection type tells the HBA which layout the target is
   formatted with.  This is necessary because the controller must be
   able to correctly interpret the included protection information in
   order to verify it.

 - When a scsi_cmnd is reused for error handling the protection
   operation must be cleared and saved while error handling is in
   progress.

 - prot_op and prot_type are placed in an existing hole in scsi_cmnd
   and don't cause the structure to grow.

Martin K. Petersen 4469f98780 [SCSI] Host protection capabilities
Controllers that support protection information must indicate this to
the SCSI midlayer so that the ULD can prepare scsi_cmnds accordingly.

This patch implements a host mask and various types of protection:

 - DIF Type 1-3 (between HBA and disk)
 - DIX Type 0-3 (between OS and HBA)

The patch also allows the HBA to set the guard type to something
different than the T10-mandated CRC.

Hannes Reinecke ae11b1b36d [SCSI] scsi_dh: attach to hardware handler from dm-mpath
multipath keeps a separate device table which may be
more current than the built-in one.
So we should make sure to always call ->attach whenever
a multipath map with hardware handler is instantiated.
And we should call ->detach on removal, too.

Hannes Reinecke b6ff1b14cd [SCSI] scsi_dh: Update EMC handler
This patch converts the EMC device handler to use a proper
state machine. We now also parse the extended INQUIRY
information to determine if long trespass commands are
supported. And we're now using the long trespass command
correctly. And finally there's now an check at init time
to refuse to attach to devices not supporting EMC-specific
VPD pages.

Instead of having each and every driver implement its own
device table scanning code we should rather implement a common
routine and scan the device tables there.
This allows us also to implement a general notifier chain
callback for all device handler instead for one per handler.

Chandra Seetharaman fe9233fb69 [SCSI] scsi_dh: fix kconfig related build errors
Do not automatically "select" SCSI_DH for dm-multipath. If SCSI_DH
doesn't exist,just do not allow  hardware handlers to be used.

Handle SCSI_DH being a module also. Make sure it doesn't allow DM_MULTIPATH
to be compiled in when SCSI_DH is a module.

Stefan Richter d2886ea368 scsi: sd: optionally set power condition in START STOP UNIT
Adds a new scsi_device flag, start_stop_pwr_cond:  If enabled, the sd
driver will not send plain START STOP UNIT commands but ones with the
power condition field set to 3 (standby) or 1 (active) respectively.

Some FireWire disk firmwares do not stop the motor if power condition is
zero.  Or worse, they become unresponsive after a START STOP UNIT with
power condition = 0 and start = 0.

This patch only adds the necessary code to sd_mod but doesn't activate
it.  Follow-up patches to the FireWire drivers will add detection of
affected devices and enable the code for them.

I did not add power condition values to scsi_error.c::scsi_eh_try_stu()
for now.  The three firmwares which suffer from above mentioned problems
do not need START STOP UNIT in the error handler, and they are not
adversely affected by START STOP UNIT with power condition = 0 and start
= 1 (like scsi_eh_try_stu() sends it if scsi_device.allow_restart is
enabled).

Mike Christie 913e5bf435 [SCSI] libiscsi, iser, tcp: remove recv_lock
The recv lock was defined so the iscsi layer could block
the recv path from processing IO during recovery. It
turns out iser just set a lock to that pointer which was pointless.

We now disconnect the transport connection before doing recovery
so we do not need the recv lock. For iscsi_tcp we still stop
the recv path incase older tools are being used.

This patch also has iscsi_itt_to_ctask user grab the session lock
and has the caller access the task with the lock or get a ref
to it in case the target is broken and sends a tmf success response
then sends data or a response for the command that was supposed to
be affected bty the tmf.

Mike Christie 88dfd340b9 [SCSI] iscsi class: Add session initiatorname and ifacename sysfs attrs.
This adds two new attrs used for creating initiator ports and
binding sessions to hardware.

The session level initiatorname:

Since bnx2i does a scsi_host per host device, we need to add the
iface initiator port settings on the session, so we can create
multiple initiator ports (each with different inames) per device/scsi_host.

The current iname reflects that qla4xxx can have one iname per hba, and we are
allocating a host per session for software. The iname on the host will
remain so we can export and set the hba level qla4xxx setting.

The ifacename attr:

To bind a session to a some peice of hardware in userspace we maintain
some mappings, but during boot or iscsid restart (iscsid contains the user
space part of the driver) we need to be able to figure out which of those
host mappings abstractions maps to certain sessions. This patch adds
a ifacename attr, which userspace can set to id the host side of the
endpoint across pivot_roots and iscsid restarts.

Currently we duplicate the list of sessions, because we were using the
test for if a session was on the host list to indicate if the session
was bound or unbound. We can instead use the target_id and fix up

There is no need to have the mgmt and cmd tasks separate
structs. It used to save a lot of memory when we overprealocated
memory for tasks, but the next patches will set up the

common scsi command allocator and common tagging.

Currently to get a ctask from the session cmd array, you have to
know to use the itt modifier. To make this easier on LLDs and
so in the future we can easilly kill the session array and use
the host shared map instead, this patch adds a nice wrapper
to strip the itt into a session->cmds index and return a ctask.

iscsi offload (bnx2i and qla4xx) allocate a scsi host per hba,
so the session creation path needs a shost/host_no argument.
Software iscsi/iser will follow the same behabior as before
where it allcoates a host per session, but in the future iser
will probably look more like bnx2i where the host's parent is
the hardware (rnic for iser and for bnx2i it is the nic), because
it does not use a socket layer like how iscsi_tcp does.

Chandra Seetharaman a6a8d9f87e [SCSI] scsi_dh: add infrastructure for SCSI Device Handlers
Some of the storage devices (that can be accessed through multiple paths),
do need some special handling for
	1. Activating the passive path of the storage access.
	2. Decode and handle the special sense codes returned by the devices.
	3. Handle the I/Os being sent to the passive path, especially
           during the device probe time.
when accessed through multiple paths.

As of today this special device handling is done at the dm-multipath
layer using dm-handlers. That works well for (1); for (2) to be handled
at dm layer, scsi sense information need to be exported from SCSI to dm-layer,
which is not very attractive; (3) cannot be done at all at the dm layer.

Device handler has been moved to SCSI mainly to handle (2) and (3) properly.

Boaz Harrosh db4742dd8f [SCSI] add support for variable length extended commands
Add support for variable-length, extended, and vendor specific
CDBs to scsi-ml. It is now possible for initiators and ULD's
to issue these types of commands. LLDs need not change much.
All they need is to raise the .max_cmd_len to the longest command
they support (see iscsi patch).

- clean-up some code paths that did not expect commands to be
  larger than 16, and change cmd_len members' type to short as
  char is not enough.

Boaz Harrosh 64a87b244b [SCSI] Let scsi_cmnd->cmnd use request->cmd buffer
- struct scsi_cmnd had a 16 bytes command buffer of its own.
   This is an unnecessary duplication and copy of request's
   cmd. It is probably left overs from the time that scsi_cmnd
   could function without a request attached. So clean that up.

 - Once above is done, few places, apart from scsi-ml, needed
   adjustments due to changing the data type of scsi_cmnd->cmnd.

 - Lots of drivers still use MAX_COMMAND_SIZE. So I have left
   that #define but equate it to BLK_MAX_CDB. The way I see it
   and is reflected in the patch below is.
   MAX_COMMAND_SIZE - means: The longest fixed-length (*) SCSI CDB
                      as per the SCSI standard and is not related
                      to the implementation.
   BLK_MAX_CDB.     - The allocated space at the request level

 - I have audit all ISA drivers and made sure none use ->cmnd in a DMA
   Operation. Same audit was done by Andi Kleen.

(*)fixed-length here means commands that their size can be determined
   by their opcode and the CDB does not carry a length specifier, (unlike
   the VARIABLE_LENGTH_CMD(0x7f) command). This is actually not exactly
   true and the SCSI standard also defines extended commands and
   vendor specific commands that can be bigger than 16 bytes. The kernel
   will support these using the same infrastructure used for VARLEN CDB's.
   So in effect MAX_COMMAND_SIZE means the maximum size command
   scsi-ml supports without specifying a cmd_len by ULD's

James Bottomley 643eb2d932 [SCSI] rework scsi_target allocation
The current target allocation code registeres each possible target
with sysfs; it will be deleted again if no useable LUN on this target
was found. This results in a string of 'target add/target remove' uevents.

Based on a patch by Hannes Reinecke <hare@suse.de> this patch reworks
the target allocation code so that only uevents for existing targets
are sent. The sysfs registration is split off from the existing
scsi_target_alloc() into a in a new scsi_add_target() function, which
should be called whenever an existing target is found. Only then a
uevent is sent, so we'll be generating events for existing targets
only.

Matthew Wilcox 5a6483feb0 include: Remove unnecessary inclusions of asm/semaphore.h
None of these files use any of the functionality promised by
asm/semaphore.h.  It's possible that they (or some user of them) rely
on it dragging in some unrelated header file, but I can't build all
these files, so we'll have to fix any build failures as they come up.

FUJITA Tomonori 9ac16b616a [SCSI] scsi: add wrapper functions for sg buffer copy helper functions
LLDs need to copies data between the SG table in struct scsi_cmnd and
liner buffer. So they use the helper functions like

sg_copy_from_buffer(scsi_sglist(sc), scsi_sg_count(sc), buf, buflen)
sg_copy_to_buffer(scsi_sglist(sc), scsi_sg_count(sc), buf, buflen)

This patch just adds wrapper functions:

scsi_sg_copy_from_buffer(sc, buf, buflen)
scsi_sg_copy_to_buffer(sc, buf, buflen)

Mike Christie 30bd7df8ce [SCSI] scsi_error: add target reset handler
The problem is that serveral drivers are sending a target reset from the
device reset handler, and if we have multiple devices a target reset gets
sent for each device when only one would be sufficient. And if we do a target
reset it affects all the commands on the target so the device reset handler
code only cleaning up one devices's commands makes programming the driver a
little more difficult than it should be.

This patch adds a target reset handler, which drivers can use to send
a target reset. If successful it cleans up the commands for a devices
accessed through that starget.

Provide a facility to use the request_firmware() interface to get a SAS
address from userspace.  This can be used by SAS LLDDs that cannot
obtain the address from the host adapter.

Mike Christie 45ab33b6c1 [SCSI] iscsi class: regression - fix races with state manipulation and blocking/unblocking
For qla4xxx, we could be starting a session, but some error (network,
target, IO from a device that got started, etc) could cause the session
to fail and curring the block/unblock and state manipulation could race
with each other. This patch just has those operations done in the
single threaded iscsi eh work queue, so that way they are serialized.
