Currently the null check for wqe is incorrect and lets a null wqe
be passed to set_64bit_val and this indexes into the null pointer
causing a null pointer dereference. Fix this by fixing the null
pointer check to return an error if wqe is null.

The driver uses a hard-coded value for FW version and reports an
inconsistent FW version between ibv_devinfo and
/sys/class/infiniband/i40iw/fw_ver.
Retrieve the FW version via a Control QP (CQP) operation and report it
consistently across sysfs and query device.

The flush CQP OP can be used to optionally generate
Asynchronous Events (AEs) in addition to QP flush.
Consolidate all HW AE generation code under a new
function i40iw_gen_ae which use the flush CQP OP
to only generate AEs.

Currently, if the number of processed Asynchronous Event Queue (AEQ)
entries exceeds 255, they are not returned to HW for re-use. During
scale-up, the unreturned AEQ entries can grow to the max AEQ size and
cause the HW to report an AEQ overflow.
Remove the check which limits the number of processed AEQ entries returned
to HW.

Currently there is only one sdbuf per Control QP (CQP) for
programming Segment Descriptor (SD). If multiple SD work
requests are posted simultaneously, the sdbuf is reused
by all WQEs and new WQEs can corrupt previous WQEs sdbuf
leading to incorrect SD programming.
Fix this by allocating one sdbuf per CQP SQ WQE. When an
SD command is posted, it will use the corresponding sdbuf
for the WQE.

Control QP (CQP) command backlog list is initialized at
device initialization time. It is not reinitialized in
the reset flow. Move the initialization to CQP creation
time so the list can be initialized correctly for reset as well.

If User-space Direct Access (UDA) QP creation fails,
the QP entry is not removed from QoS list. Fix this

Clear Control Queue Pair (CQP) Head/Tail on CQP
initialization as during an adapter reset, these
values are not reinitialized. Tail is cleared by
writing 0 to CQP's tail register. Head is cleared
by writing 0 to CQP's doorbell register.

On a netdev MTU change event, the iWARP
Exception Queue (IEQ) buffers may not be
sized properly to handle the new MTU.
Reinitialize the IEQ with new MTU size on MTU
change event.
Also, add define for the max ethernet frame size
field in IEQ QP context instead of the snd_mss
define which is for iWARP QPs' MSS field.

The IPv6 header size is not subtracted from MTU when MSS is
set for QPs.
Save MTU opposed to MSS in the vsi struct during
initialization and calculate the MSS based on IPv4 vs
IPv6 connection.

The AE source field in Asynchronous Event Queue Entry (AEQE) is
not set by the hardware for some AEs, but the code assumes it does.
This results in incorrect processing of some AEs.
Fix this by setting ae_src to I40IW_AE_SOURCE_RSVD for those AEs.

Replace some instances of set_64bit_val with

The current timeout logic for Control Queue-Pair (CQP) OPs
does not take into account whether CQP makes progress but
rather blindly waits for a large timeout value, 100000 jiffies
for the completion event. Improve this by setting the timeout
based on whether the CQP is making progress or not. If the CQP
is hung, the timeout will happen sooner, in 5000 jiffies. Each
time the CQP progress is detetcted, the timeout extends by 5000
jiffies.

Parsing of commit/query Host Memory Cache Function Private Memory
is not skipping over reserved fields and incorrectly assigning
those values into object's base/cnt/max_cnt fields. Skip over
reserved fields and set correct values. Also correct memory
alignment requirement for commit/query FPM buffers.

RQ WQE size other than 128B is not supported. Correct
RQ size calculation to use 128B only.

The flush_code variable in i40iw_bld_terminate_hdr() is obsolete and
the check to set qp->sq_flush is unreachable. Currently flush code is
populated in setup_term_hdr() and both SQ and RQ are flushed always
as part of the tear down flow.

Passed in page_size was used as encoded value for writing
the WQE and passed in value was usually 4096. This was
working out since bit 0 was 0 and implies 4KB pages,
but would not work for other page sizes.

Add support for QoS on QPs. Upon device initialization,
a map is created from user priority to queue set
handles. On QP creation, use ToS to look up the queue
set handle for use with the QP.
