Frank Pavlic 6cbed91ab7 [S390] qdio: output queue stall on FCP and network devices
When running QIOASSIST enabled qdio devices in a z/VM environment
the output queue for such devices stall in heavy workload situations.
When SQBS and EQBS instructions returns CCQ=96 qdio does not reissue
the instruction again with the register settings done by millicode
but processed the returned qdio buffer. This is wrong. qdio has to
reissue the instruction once again on CCQ=96, as we already do it
for CCQ=97.

Zhang Rui 91a6902958 sysfs: add parameter "struct bin_attribute *" in .read/.write methods for sysfs binary attributes
Well, first of all, I don't want to change so many files either.

What I do:
Adding a new parameter "struct bin_attribute *" in the
.read/.write methods for the sysfs binary attributes.

In fact, only the four lines change in fs/sysfs/bin.c and
include/linux/sysfs.h do the real work.
But I have to update all the files that use binary attributes
to make them compatible with the new .read and .write methods.
I'm not sure if I missed any. :(

Why I do this:
For a sysfs attribute, we can get a pointer pointing to the
struct attribute in the .show/.store method,
while we can't do this for the binary attributes.
I don't know why this is different, but this does make it not
so handy to use the binary attributes as the regular ones.
So I think this patch is reasonable. :)

Who benefits from it:
The patch that exposes ACPI tables in sysfs
requires such an improvement.
All the table binary attributes share the same .read method.
Parameter "struct bin_attribute *" is used to get
the table signature and instance number which are used to
distinguish different ACPI table binary attributes.

Without this parameter, we need to offer different .read methods
for different ACPI table binary attributes.
This is impossible as there are various ACPI tables on different
platforms, and we don't know what they are until they are loaded.

Tejun Heo 7b595756ec sysfs: kill unnecessary attribute->owner
sysfs is now completely out of driver/module lifetime game.  After
deletion, a sysfs node doesn't access anything outside sysfs proper,
so there's no reason to hold onto the attribute owners.  Note that
often the wrong modules were accounted for as owners leading to
accessing removed modules.

This patch kills now unnecessary attribute->owner.  Note that with
this change, userland holding a sysfs node does not prevent the
backing module from being unloaded.

Michael Holzheu dc4123303b [S390] Program check in diag 210 under 31 bit
If a virtual address is passed to the diag210 function under 31 bit, we get a
programming exception, since diag 210 only works with physical addresses. To
fix this, the content of the diag210 data structure is copied to a local
structure and the physical address of that structure is passed to diagnose 210.

Cornelia Huck 9a92fe48b9 [S390] cio: Get rid of _ccw_device_get_device_number().
The function shouldn't have existed in the first place (not MSS-aware).
Introduce a new function ccw_device_get_id() that extracts the
ccw_dev_id structure of a ccw device and convert all users of

Cornelia Huck fa1a8c23eb s390: cio: Delay uevents for subchannels
We often have the situation that we register a subchannel and start device
recognition, only to find out that the device is not usable after all, which
triggers an unregister of the subchannel.  This often happens on hundreds of
subchannels on a LPAR, leading to a storm of events which aren't of any use. 
Therefore, use uevent_suppress to delay the KOBJ_ADD uevent for a subchannel
until we know that its ccw_device is to be registered.

Cornelia Huck ef99516c96 [S390] cio: Unregister ccw devices directly.
We used to unregister ccw devices not directly from the I/O
subchannel remove function in order to avoid lifelocks on the
css bus semaphore. This semaphore is gone, and there is no reason
to not unregister the ccw device directly (it is even better since
it is more in keeping with the goal of immediate disconnect).

Peter Oberparleiter 83b3370c79 [S390] cio: replace subchannel evaluation queue with bitmap
Use a bitmap for indicating which subchannels require evaluation
instead of allocating memory for each evaluation request. This
approach reduces memory consumption during recovery in case of
massive evaluation request occurrence and removes the need for
memory allocation failure handling.

Stefan Bader 387b734fc2 [S390] cio: Re-start path verification after aborting internal I/O.
Path verification triggered by changes to the available CHPIDs will be
interrupted by another change but not re-started. This results in an
invalid path mask.
To solve this make sure to completely re-start path verification when
changing the available paths.

Alan Stern d9a9cdfb07 [PATCH] sysfs and driver core: add callback helper, used by SCSI and S390
This patch (as868) adds a helper routine for device drivers that need
to set up a callback to perform some action in a different process's
context.  This is intended for use by attribute methods that want to
unregister themselves or their parent device.  Attribute method calls
are mutually exclusive with unregistration, so such actions cannot be
taken directly.

Two attribute methods are converted to use the new helper routine: one
for SCSI device deletion and one for System/390 ccwgroup devices.

Heiko Carstens 615b04b301 [S390] nss: Free unused memory in kernel image.
With CONFIG_SHARED_KERNEL the kernel text segment that might be in a
read only memory sections starts at 1MB. Memory between 0x12000 and
0x100000 is unused then. Free this, so we have appr. an extra MB
of memory available.

Arjan van de Ven d54b1fdb1d [PATCH] mark struct file_operations const 5
Many struct file_operations in the kernel can be "const".  Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data.  In addition it'll catch accidental writes at compile time to
these shared resources.

Cornelia Huck 4dd3cc5caf [S390] cio: Fixup interface for setting options on ccw devices.
The current ccw_device_set_options() sets a specified mask of options
and clears those not specified, but there is no way to find out which
options have already been set.

In order to fix this up, introduce the following interface changes:

ccw_device_set_options() now only sets the specified bits, but does
not clear those that are not specified.

ccw_device_clear_options() clears the specified bits.

ccw_device_set_options_mask() provides the old semantics (setting only
the specified bits and clearing the others).

Device drivers now work as expected. qdio has been adapted.

Heiko Carstens ab14de6c37 [S390] Convert memory detection into C code.
Hopefully this will make it more maintainable and less error prone.
Code makes use of search_exception_tables(). Since it calls this
function before the kernel exeception table is sorted, there is an
early call to sort_main_extable().

This way it's easy to use the already present infrastructure of fixup
sections. Also this would allows to easily convert the rest of
head[31|64].S into C code.

Martin Schwidefsky d54853ef8c [S390] ETR support.
This patch adds support for clock synchronization to an external time
reference (ETR). The external time reference sends an oscillator
signal and a synchronization signal every 2^20 microseconds to keep
the TOD clocks of all connected servers in sync. For availability
two ETR units can be connected to a machine. If the clock deviates
for more than the sync-check tolerance all cpus get a machine check
that indicates that the clock is out of sync. For the lovely details
how to get the clock back in sync see the code below.

Heiko Carstens 6faf4444f2 [S390] cio: use barrier() in stsch_reset.
Use barrier() in stsch_reset() instead of duplicating the stsch()
inline assembly and adding "memory" to the clobberlist.
Pointed out by Chuck Ebbert.

Real fix would be to add a fixup section to the stsch() and extend the
basic program check handler so it searches the exception tables in case
of a program check.

Michael Holzheu a45e14148f [S390] Fix reboot hang on LPARs
Reboot hangs on LPARs without diag308 support. The reason for this is,
that before the reboot is done, the channel subsystem is shut down.
During the reset on each possible subchannel a "store subchannel" is
done. This operation can end in a program check interruption, if the
specified subchannel set is not implemented by the hardware. During
the reset, currently we do not have a program check handler, which
leads to the described kernel bug. We install now a new program check
handler for the reboot code to fix this problem.

Cornelia Huck 24cb5b4846 [S390] cio: Use path verification for last path gone after vary off.
If the last path to a device is gone after a chpid has been varied
off, putting it on the slow queue doesn't prevent a device driver
from still attempting to use it (it may stay on the slow queue for a
long time). Instead, trigger a verify event which will prevent I/O
attempts from the device driver immediately.

Heiko Carstens 15e9b586e0 [S390] Reset infrastructure for re-IPL.
In case of re-IPL and diag308 doesn't work we have to reset all devices
manually and wait synchronously that each reset finished.
This patch adds the necessary infrastucture and the first exploiter of it.

Subsystems that need to add a function that needs to be called at re-IPL
may register/unregister this function via

struct reset_call {

void register_reset_call(struct reset_call *reset);
void unregister_reset_call(struct reset_call *reset);

When the registered function get called the context is:

- all cpus beside the current one are stopped
- all machine checks and interrupts are disabled
- prefixing is disabled
- a default machine check handler is available for use

The registered functions may not take any locks are sleep.

For the common I/O layer part of this patch:

Introduce a reset_call css_reset that does the following:
- clear all subchannels
- perform a rchp on all channel paths and wait for the resulting
  machine checks
This replaces the calls to clear_all_subchannels() and
cio_reset_channel_paths() for kexec and ccw reipl. reipl_ccw_dev() now
uses reipl_find_schid() to determine the subchannel id for a given
device id.
Also remove cio_reset_channel_paths() and friends since they are not
needed anymore.

Cornelia Huck e7769b48a0 [S390] cio: Remove grace period for vary off chpid.
The grace period handling introduced needless complexity. It didn't
help the dasd driver (which can handle terminated I/O just well),
and it doesn't help for long running channel programs (which won't
complete during the grace period anyway). Terminating I/O using a
path that just disappeared immediately is much more consistent with
what the user expects.

Martin Schwidefsky 94c12cc7d1 [S390] Inline assembly cleanup.
Major cleanup of all s390 inline assemblies. They now have a common
coding style. Quite a few have been shortened, mainly by using register
asm variables. Use of the EX_TABLE macro helps  as well. The atomic ops,
bit ops and locking inlines new use the Q-constraint if a newer gcc
is used.  That results in slightly better code.

CHPIDs that are logically varied off will not be removed from
a CCW device's path group because resign-from-pathgroup command is
issued with invalid path mask of 0 because internal CCW operations

Do not apply logical path mask to internal operations.

Heiko Carstens 81388d2a45 [S390] Missing initialization in common i/o layer.
Previous patch that was intended to reduce stack usage within common
i/o layer didn't consider implicit memset(..., 0, ...) used with the
initializations used before.
Add these missing memsets wherever it's not obvious that the
concerned memory region is zeroed. This should give the same semantics
as before.

Michael Holzheu ff6b8ea68f [S390] ipl/dump on panic.
It is now possible to specify a ccw/fcp dump device which is used to
automatically create a system dump in case of a kernel panic. The dump
device can be configured under /sys/firmware/dump.
In addition it is now possible to specify a ccw/fcp device which is used
for the next reboot of Linux. The reipl device can be configured under
/sys/firmware/reipl.

Devices enter no-path state after disabling a channel path
via the SE even though another path has been reenabled at the SE.
The devices are set into no-path state before triggering path
verification even though other paths may have become available.
To fix this trigger path verification before setting a device into

Use different kind of assignment to make sure gcc doesn't create code
that creates temp variables on the stack, assigns values to it and
copies the content of the whole temp variable to the destination.
This reduces stack usage of e.g. ccwgroup_driver_register from 976

Peter Oberparleiter 65200c291c [S390] lost interrupt after chpid vary off/on cycle.
I/O on a CCW device may stall if a channel path to that device is
logicaly varied off/on. A user I/O interrupt can get misinterpreted
as interrupt for an internal path verification operation due to a
missing check and is therefore never reported to the device driver.

Correct check for pending interruptions before starting path
verification.

Peter Oberparleiter ba4ba8a69d [S390] permanent subchannel busy conditions may cause I/O stall
In special conditions where a subchannel rejects the HALT I/O-
instruction with a busy indication (cc 2), I/O may stall.
I/O request termination logic retries HALT I/O indefinitely
because it expects HALT I/O to alter the subchannel status which
is not true when cc 2 is returned.
In case of a busy indication, try CLEAR I/O instruction immediately.

Cornelia Huck 7e560814de [S390] path grouping and path verifications fixes.
1. Multipath devices for which SetPGID is not supported are not handled well.
   Use NOP ccws for path verification (sans path grouping) when SetPGID is not
   supported.
2. Check for PGIDs already set with SensePGID on _all_ paths (not just the
   first one) and try to find a common one. Moan if no common PGID can be
   found (and use NOP verification). If no PGIDs have been set, use the css
   global PGID (as before). (Rationale: SetPGID will get a command reject if
   the PGID it tries to set does not match the already set PGID.)
3. Immediately before reboot, issue RESET CHANNEL PATH (rcp) on all chpids. This
   will remove the old PGIDs. rcp will generate solicited CRWs which can be
   savely ignored by the machine check handler (all other actions create
   unsolicited CRWs).

registered. Since 2.6.17, we've seen oopses in kslowcrw when a device is

observed with earlier kernels...

Cornelia Huck 94bb063312 [S390] rework of channel measurement facility.
Fixes for several channel measurement facility bugs:
* Blocks copied from the hardware might not be consistent. Solve this
  by moving the copying into idle state and repeating the copying.
* avg_sample_interval changed with every read, even though no new block
  was available. Solve this by storing a timestamp when the last new
  block was received.
* Several locking issues.
* Measurements were not reenabled after a disconnected device became
  available again.
* Remove #defines for ioctls that were never implemented.

Cornelia Huck b4f7b1ee57 [S390] cio chpid offline.
After setting a path to a dasd offline at the SE, I/O hangs on that
dasd for 5 minutes, then continues.
I/O for which an interrupt will not be reported after the channel
path has been disabled was not terminated by the common I/O layer,
causing the dasd MIH to hit after 5 minutes.

Be more aggressive in terminating I/O after setting a channel path
offline. Also make sure to generate a fake irb if the device
driver issues an I/O request after being notified of the killed
I/O and clear residual information from the irb before trying to
start the delayed verification.

Cornelia Huck 887ab59929 [S390] ccwgroup device unregister.
Work around the problem that a device cannot be unregistered from
driver_for_each_device() because of klist node refcounting: Get device
after device owned by the driver to be unregistered with driver_find_device()
and then unregister it. This works because driver_get_device() gets us out of
the region of the elevated klist node refcount. driver_find_device() will
always get the next device in the list after the found one has been
unregistered.

Peter Oberparleiter da039bd1e8 [S390] cio long busy in read configuration data.
Trying to set a DASD root device online can fail under some circumstances
with the message "Read configuration data returned error -5". The cause
is that read configuration data incorrectly aborts with -EIO when it
encounters a temporary busy condition at a storage server.
Perform retry when encountering temporary busy conditions.

Christian Borntraeger 40ac6b204c [PATCH] s390: fix slab debugging
With CONFIG_SLAB_DEBUG=y networking over qeth doesn't work.  The problem is
that the qib structure embedded in the qeth_irq structure needs an alignment
of 256 but kmalloc only guarantees an alignment of 8.  When using SLAB
debugging the alignment of qeth_irq is not sufficient for the embedded qib
structure which causes all users of qdio (qeth and zfcp) to stop working.
Allocate qeth_irq structure with __get_free_page.  That wastes a small amount
of memory (~2500 bytes) per online adapter.

Frank Pavlic 15c7369178 [PATCH] s390: V=V qdio fixes
Using FCP devices with V=V support, the input queue stalled when CCQ 97 had
been returned in qdio_do_eqbs.  When this happen we have to reissue the eqbs
instruction.

Another bug was when V=V was enabled we checked if hardware has SIGA-sync
support.  If not we returned with 0 from tiqdio_is_inbound_q_done.  Thus qdio
lost initiative on FCP devices and input queue stalled.  Running devices in
V=V there is no SIGA-sync support but nevertheless we have to process
tiqdio_is_inbound_q_done either.

Frank Pavlic 8129ee1642 [PATCH] s390: qdio V=V pass-through
New feature V=V qdio pass-through.

QDIO and HiperSockets processing in z/VM V=V guest environments (as well as
V=R with z/VM running in LPAR mode) requires shadowing of all QDIO
architecture queue elements.  Especially the shadowing of SBALs and SLSBs
structures in the hypervisor, and the need to issue SIGA SYNC operations to
observe state changes, eventually causes significant CPU processing overhead
in the hypervisor.

The QDIO pass-through support for V=V guests avoids the shadowing of SBALs and
SLSBs.  This significantly reduces the hypervisor overhead for QDIO based I/O.

Fix the broken atomic_cmpxchg primitive.  Add atomic_sub_and_test,

atomic64_add_unless and atomic64_inc_not_zero.  Replace old style
atomic_compare_and_swap by atomic_cmpxchg.  Shorten the whole header by
defining most primitives with the two inline functions atomic_add_return and
atomic_sub_return.

In addition this patch contains the s390 related fixes of Hugh's "mm: fill
arch atomic64 gaps" patch.

Tim Schmielau 4e57b68178 [PATCH] fix missing includes
I recently picked up my older work to remove unnecessary #includes of
sched.h, starting from a patch by Dave Jones to not include sched.h
from module.h. This reduces the number of indirect includes of sched.h
by ~300. Another ~400 pointless direct includes can be removed after
this disentangling (patch to follow later).
However, quite a few indirect includes need to be fixed up for this.

In order to feed the patches through -mm with as little disturbance as
possible, I've split out the fixes I accumulated up to now (complete for
i386 and x86_64, more archs to follow later) and post them before the real
patch.  This way this large part of the patch is kept simple with only
adding #includes, and all hunks are independent of each other.  So if any
hunk rejects or gets in the way of other patches, just drop it.  My scripts
will pick it up again in the next round.

Close a small window where a device may be not operational again after senseid
finished and the "same device" check fails due to dev=0000 by checking for dnv
after stsch() by then setting the device to not operational.  (No need to
check for dnv in ccw_device_handle_oper() again since we don't do stsch() into
the subchannel's schib in the meantime and will get a crw anyway if the device
becomes not oper again).

Jesper Juhl 77933d7276 [PATCH] clean up inline static vs static inline
`gcc -W' likes to complain if the static keyword is not at the beginning of
the declaration.  This patch fixes all remaining occurrences of "inline
static" up with "static inline" in the entire kernel tree (140 occurrences in
47 files).

While making this change I came across a few lines with trailing whitespace
that I also fixed up, I have also added or removed a blank line or two here
and there, but there are no functional changes in the patch.

Michael Holzheu 66a464dbc8 [PATCH] s390: debug feature changes
This patch changes the memory allocation method for the s390 debug feature.
Trace buffers had been allocated using the get_free_pages() function before.
Therefore it was not possible to get big memory areas in a running system due
to memory fragmentation.  Now the trace buffers are subdivided into several
subbuffers with pagesize.  Therefore it is now possible to allocate more
memory for the trace buffers and more trace records can be written.

In addition to that, dynamic specification of the size of the trace buffers is
implemented.  It is now possible to change the size of a trace buffer using a
new debugfs file instance.  When writing a number into this file, the trace
buffer size is changed to 'number * pagesize'.

In the past all the traces could be obtained from userspace by accessing files
in the "proc" filesystem.  Now with debugfs we have a new filesystem which
should be used for debugging purposes.  This patch moves the debug feature
from procfs to debugfs.

Since the interface of debug_register() changed, all device drivers, which use
the debug feature had to be adjusted.

Linus Torvalds 1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
