Rename it to mc_tmp_ptrs to denote better what it is - a temporary array
for saving pointers to microcode blobs. And "initrd" is not accurate
anymore since initrd is not the only source for early microcode.
Therefore, rename copy_initrd_ptrs() to copy_ptrs() simply and
"initrd_start" to "offset".
And then do the following convention: the global variable is called
"mc_tmp_ptrs" and the local function arguments "mc_ptrs" for
differentiation.

If we're going to BUG_ON() because we're running on the wrong CPU, we
better do it as the first thing we do when entering that function. And
also, turn it into a WARN_ON() because it is not worth to panic the
system if we apply the microcode on the wrong CPU - we're simply going
to exit early.

We do parse for the disable microcode loader chicken bit very early.
After the driver merge, the __setup() param parsing method is not needed
anymore so get rid of it.

Set the initrd @start depending on the presence of an initrd. Otherwise,
builtin microcode loading doesn't work as the start is wrong and we're
using it to compute offset to the microcode blobs.

Thomas Voegtle reported that doing oldconfig with a .config which has
CONFIG_MICROCODE enabled but BLK_DEV_INITRD disabled prevents the
microcode loading mechanism from being built.
So untangle it from the BLK_DEV_INITRD dependency so that oldconfig
doesn't turn it off and add an explanatory text to its Kconfig help what
the supported methods for supplying microcode are.

Running microcode_init() from setup_arch() is a bad idea because
not even kmalloc() is ready at that point and the loader does
all kinds of allocations and init/registration with various
subsystems.
Make it a late initcall when required facilities are initialized
so that the microcode driver initialization can succeed too.

Make CONFIG_MICROCODE a bool. It was practically a bool already anyway,
since early loader was forcing it to =y.
Regardless, there's no real reason to have something be a module which
gets built-in on the majority of installations out there. And its not
like there's noticeable change in functionality - we still can load late
microcode - just the module glue disappears.

A certain number of patch levels of applied microcode should not
be overwritten by the microcode loader, otherwise bad things
will happen.
Check those and abort update if the current core has one of
those final patch levels applied by the BIOS. 32-bit needs
special handling, of course.

Pave the way for checking the current patch level of the
microcode in a core. We want to be able to do stuff depending on
the patch level - in this case decide whether to update or not.
But that will be added in a later patch.
Drop unused local var uci assignment, while at it.
Integrate a fix for 32-bit and CONFIG_PARAVIRT from Takashi Iwai:
Use native_rdmsr() in check_current_patch_level() because with
CONFIG_PARAVIRT enabled and on 32-bit, where we run before
paging has been enabled, we cannot deref pv_info yet. Or we
could, but we'd need to access its physical address. This way of
fixing it is simpler. See:

The __ref / __refdata annotations used to be needed because of
referencing functions / variables annotated __cpuinit /
__cpuinitdata.
But those annotations vanished during the development of v3.11.
Therefore most of the __ref / __refdata annotations are not needed
anymore. As they may hide legitimate sections mismatches, we
better get rid of them.

Andy Shevchenko reported machine freezes when booting latest tip
on 32-bit setups. Problem is, the builtin microcode handling cannot
really work that early, when we haven't even enabled paging.
A proper fix would involve handling that case specially as every
other early 32-bit boot case in the microcode loader and would
require much more involved changes for which it is too late now,
more than a week before the upcoming merge window.
So, disable the builtin microcode loading on 32-bit for now.

Apparently, people do build microcode into the kernel image, i.e.
CONFIG_FIRMWARE_IN_KERNEL=y.
Make that work in the early loader which is where microcode should be
preferably loaded anyway.
Note that you need to specify the microcode filename with the path
relative to the toplevel firmware directory (the same like the late
loading method) in CONFIG_EXTRA_FIRMWARE=y so that early loader can
find it.
I.e., something like this (Intel variant):

Don't compute start and end from start and size in order to compute size
again down the path in scan_microcode(). So pass size directly instead

We do not check the input data bounds containing the microcode before
copying a struct microcode_intel_header from it. A specially crafted
microcode could cause the kernel to read invalid memory and lead to a
denial-of-service.

mc_saved_tmp is a static array allocated on the stack, we need to make
sure mc_saved_count stays within its bounds, otherwise we're overflowing
the stack in _save_mc(). A specially crafted microcode header could lead
to a kernel crash or potentially kernel execution.

Commits 65cef1311d ("x86, microcode: Add a disable chicken bit") and
a18a0f6850 ("x86, microcode: Don't initialize microcode code on
paravirt") allow microcode driver skip initialization when microcode
loading is not permitted.
However, they don't prevent the driver from being loaded since the
init code returns 0. If at some point later the driver gets unloaded
this will result in an oops while trying to deregister the (never
registered) device.
To avoid this, make init code return an error on paravirt or when
microcode loading is disabled. The driver will then never be loaded.

I'm such a moron! The simple solution of saving the BSP patch
for use on resume was too simple (and wrong!), hint:
sizeof(struct microcode_intel).
What needs to be done instead is to fish out the microcode patch
we have stashed previously and apply that on the BSP in case the
late loader hasn't been utilized.
So do that instead.

Normally, we do reapply microcode on resume. However, in the cases where
that microcode comes from the early loader and the late loader hasn't
been utilized yet, there's no easy way for us to go and apply the patch
applied during boot by the early loader.
Thus, reuse the patch stashed by the early loader for the BSP.

Paravirtual guests are not expected to load microcode into processors
and therefore it is not necessary to initialize microcode loading
logic.
In fact, under certain circumstances initializing this logic may cause
the guest to crash. Specifically, 32-bit kernels use __pa_nodebug()
macro which does not work in Xen (the code path that leads to this macro
happens during resume when we call mc_bp_resume()->load_ucode_ap()
->check_loader_disabled_ap())

In the situation when we apply early microcode but do *not* apply late
microcode, we fail to update the BSP's microcode on resume because we
haven't initialized the uci->mc microcode pointer. So, in order to
alleviate that, we go and dig out the stashed microcode patch during
early boot. It is basically the same thing that is done on the APs early
during boot so do that too here.

Save the patch while we're running on the BSP instead of later, before
the initrd has been jettisoned. More importantly, on 32-bit we need to
access the physical address instead of the virtual.
This way we actually do find it on the APs instead of having to go
through the initrd each time.

Konrad triggered the following splat below in a 32-bit guest on an AMD
box. As it turns out, in save_microcode_in_initrd_amd() we're using the
*physical* address of the container *after* we have enabled paging and
thus we #PF in load_microcode_amd() when trying to access the microcode
container in the ramdisk range.
Because the ramdisk is exactly there:

and we fault at 0x35e04304.
And since this guest doesn't relocate the ramdisk, we don't do the
computation which will give us the correct virtual address and we end up
with the PA.
So, we should actually be using virtual addresses on 32-bit too by the
time we're freeing the initrd. Do that then!

Add a cmdline param which disables the microcode loader. This is useful
mostly in debugging situations where we want to turn off microcode
loading, both early from the initrd and late, as a means to be able to
rule out its influence on the machine.

For additional coverage, BorisO and friends unknowlingly did swap AMD
microcode with Intel microcode blobs in order to see what happens. What
did happen on 32-bit was

is a bit tricky.

Here's the big driver core and sysfs patch set for 3.14-rc1.
There's a lot of work here moving sysfs logic out into a "kernfs" to
allow other subsystems to also have a virtual filesystem with the same
attributes of sysfs (handle device disconnect, dynamic creation /
removal as needed / unneeded, etc. This is primarily being done for
the cgroups filesystem, but the goal is to also move debugfs to it when
it is ready, solving all of the known issues in that filesystem as well.
The code isn't completed yet, but all should be stable now (there is a
big section that was reverted due to problems found when testing.)
There's also some other smaller fixes, and a driver core addition that
allows for a "collection" of objects, that the DRM people will be using
soon (it's in this tree to make merges after -rc1 easier.)
All of this has been in linux-next with no reported issues.

We've grown a bunch of microcode loader files all prefixed with
"microcode_". They should be under cpu/ because this is strictly
CPU-related functionality so do that and drop the prefix since they're
in their own directory now which gives that prefix. :)
While at it, drop MICROCODE_INTEL_LIB config item and stash the
functionality under CONFIG_MICROCODE_INTEL as it was its only user.
