Greg Kroah-Hartman 039a5dcd2f kset: convert /sys/power to use kset_create
Dynamically create the kset instead of declaring it statically.  We also
rename power_subsys to power_kset to catch all users of the variable and
we properly export it so that people don't have to guess that it really
is present in the system.

The pseries code is wierd, why is it createing /sys/power if CONFIG_PM
is disabled?  Oh well, stupid big boxes ignoring config options...

Greg Kroah-Hartman 3514faca19 kobject: remove struct kobj_type from struct kset
We don't need a "default" ktype for a kset.  We should set this
explicitly every time for each kset.  This change is needed so that we
can make ksets dynamic, and cleans up one of the odd, undocumented
assumption that the kset/kobject/ktype model has.

This patch is based on a lot of help from Kay Sievers.

Paul Mackerras dfbe0d3b6b [POWERPC] Fix boot failure on POWER6
Commit 473980a993 added a call to clear
the SLB shadow buffer before registering it.  Unfortunately this means
that we clear out the entries that slb_initialize has previously set in
there.  On POWER6, the hypervisor uses the SLB shadow buffer when doing
partition switches, and that means that after the next partition switch,
each non-boot CPU has no SLB entries to map the kernel text and data,
which causes it to crash.

This fixes it by reverting most of 473980a9 and instead clearing the
3rd entry explicitly in slb_initialize.  This fixes the problem that
473980a9 was trying to solve, but without breaking POWER6.

Michael Neuling 473980a993 [POWERPC] Fix CPU hotplug when using the SLB shadow buffer
Before we register the SLB shadow buffer, we need to invalidate the
entries in the buffer, otherwise we can end up stale entries from when
we previously offlined the CPU.

This does this invalidate as well as unregistering the buffer with
PHYP before we offline the cpu.  Tested and fixes crashes seen on
970MP (thanks to tonyb) and POWER5.

Benjamin Herrenschmidt bf5e2ba28f [POWERPC] Merge PCI resource fixups
The PCI code in 32 and 64 bits fixes up resources differently.

32 bits uses a header quirk plus handles bridges in pcibios_fixup_bus()
while 64 bits does things in various places depending on whether you
are using OF probing, using PCI hotplug, etc...

This merges those by basically using the 32 bits approach for both,
with various tweaks to make 64 bits work with the new approach.

Milton Miller d7cf0edb8f [POWERPC] Push down or eliminate smp_processor_id calls in xics code
The per-processor interrupt request register and current processor
priority register are only accessed on the current cpu.  In fact the
hypervisor doesn't even let us choose which cpu's registers to access.

The only function to use cpu twice is xics_migrate_irqs_away, not a fast
path.  But we can cache the result of get_hard_processor_id() instead of
calling get_hard_smp_processor_id(cpu) in a loop across the call to rtas.

Years ago the irq code passed smp_processor_id into get_irq, I thought
we might initialize the CPPR third party at boot as an extra measure of
saftey, and it made the code symmetric with the qirr (queued interrupt
for software generated interrupts), but now it is just extra and
sometimes unneeded work to pass it down.

Stephen Rothwell 68d315f597 [POWERPC] iommu_free_table doesn't need the device_node
It only needs the iommu_table address.  It also makes use of the node
name to print error messages.  So just pass it the things it needs.
This reduces the places that know about the pci_dn by one.

to the device driver. Some systems may take many seconds to

Benjamin Herrenschmidt 8d089085a4 [POWERPC] Cleanup SMT thread handling
This cleans up the SMT thread handling, removing some hard coded
assumptions and providing a set of helpers to convert between linux
cpu numbers, thread numbers and cores.

This implementation requires the number of threads per core to be a
power of 2 and identical on all cores in the system, but it's an
implementation detail, not an API requirement and so this limitation
can be lifted in the future if anybody ever needs it.

Paul Mackerras 8f51506164 Revert "[POWERPC] Fix RTAS os-term usage on kernel panic"
This reverts commit a2b51812a4.

It turns out that this change caused some machines to fail to come
back up when being rebooted, and generated an error in the hypervisor
error log on some machines.  The platform architecture (PAPR) is a
little unclear on exactly when the RTAS ibm,os-term function should be
called.  Until that is clarified I'm reverting this commit.

Linas Vepstas a2b51812a4 [POWERPC] Fix RTAS os-term usage on kernel panic
The rtas_os_term() routine was being called at the wrong time.
The actual rtas call "os-term" will not ever return, and so
calling it from the panic notifier is too early.  Instead,
call it from the machine_reset() call.

This splits the rtas_os_term() routine into two: one part to capture
the kernel panic message, invoked during the panic notifier, and
another part that is invoked during machine_reset().

Prior to this patch, the os-term call was never being made,
because panic_timeout was always non-zero.  Calling os-term
helps keep the hypervisor happy!  We have to keep the hypervisor
happy to avoid service, dump and error reporting problems.

Michael Ellerman 1db3e890ae [POWERPC] Read back MSI message in rtas_setup_msi_irqs() so restore works
There are plans afoot to use pci_restore_msi_state() to restore MSI
state after a device reset.  In order for this to work for the RTAS MSI
backend, we need to read back the MSI message from config space after
it has been setup by firmware.

This should be sufficient for restoring the MSI state after a device
reset, however we will need to revisit this for suspend to disk if that
is ever implemented on pseries.

Serge E. Hallyn b460cbc581 pid namespaces: define is_global_init() and is_container_init()
is_init() is an ambiguous name for the pid==1 check.  Split it into
is_global_init() and is_container_init().

A cgroup init has it's tsk->pid == 1.

A global init also has it's tsk->pid == 1 and it's active pid namespace
is the init_pid_ns.  But rather than check the active pid namespace,
compare the task structure with 'init_pid_ns.child_reaper', which is
initialized during boot to the /sbin/init process and never changes.

Changelog:

	- Use 'init_pid_ns.child_reaper' to determine if a given task is the
	  global init (/sbin/init) process. This would improve performance
	  and remove dependence on the task_pid().

	  ppc,avr32}/traps.c for the _exception() call to is_global_init().
	  This way, we kill only the cgroup if the cgroup's init has a

Paul Mackerras 1189be6508 [POWERPC] Use 1TB segments
This makes the kernel use 1TB segments for all kernel mappings and for
user addresses of 1TB and above, on machines which support them
(currently POWER5+, POWER6 and PA6T).

We detect that the machine supports 1TB segments by looking at the
ibm,processor-segment-sizes property in the device tree.

We don't currently use 1TB segments for user addresses < 1T, since
that would effectively prevent 32-bit processes from using huge pages
unless we also had a way to revert to using 256MB segments.  That
would be possible but would involve extra complications (such as
keeping track of which segment size was used when HPTEs were inserted)
and is not addressed here.

Parts of this patch were originally written by Ben Herrenschmidt.

Olof Johansson 64f2758514 [POWERPC] Don't enable cpu hotplug on pSeries machines with MPIC
Don't allow cpu hotplug on systems lacking XICS interrupt controller
(i.e. with an MPIC interrupt controller), since the current pSeries
platform code is hardcoded for XICS.

This works around the bug reported by Paul Mackerras where the
disable_nonboot_cpus() call recently added to the shutdown path will
cause an oops on older pSeries machines.

Tony Breeds a0c7ce9c87 [POWERPC] Fix panic in RTAS code
Some older pSeries machines were panicking in pSeries_log_error
because it was getting called before it was ready.  This is a result
of commit "[POWERPC] pseries: Fix jumbled no_logging flag."
(79c0108d1b).

This fixes it by explicitly enabling RTAS error logging when it has
been initialized, and also makes the code clearer by renaming the
"no_more_logging" variable to "logging_enabled".

Michael Ellerman d385366a9b [POWERPC] Simplify rtas_change_msi() error semantics
Currently rtas_change_msi() returns either the error code from RTAS, or if
the RTAS call succeeded the number of irqs that were configured by RTAS.
This makes checking the return value more complicated than it needs to be.

Instead, have rtas_change_msi() check that the number of irqs configured by
RTAS is equal to what we requested - and return an error otherwise. This makes
the return semantics match the usual 0 for success, something else for error.

Linas Vepstas a7fb7ea76e [POWERPC] pseries: device node status can be "ok" or "okay"
It seems that some versions of firmware will report a device
node status as the string "okay". As we are not expecting this
string, the device node will be ignored by the EEH subsystem.
Which means EEH will not be enabled.

When EEH is not enabled, PCI errors will be converted into
Machine Check exceptions, and we'll have a very unhappy system.

Anton Blanchard e48395f175 [POWERPC] Fix xics set_affinity code
On a POWER6 machine running 2.6.23-rc8 I sometimes see the following error:

xics_set_affinity: No online cpus in the mask 00000000,00000000,00000000,00000001 for irq 20

In a desperate attempt to get a changelog entry in 2.6.23, I took a look
into it.

It turns out we are passing a real and not a virtual irq into
get_irq_server.  This works for the case where hwirq < NR_IRQS and we
set virq = hwirq.  In my case however hwirq = 590082 and we try and
access irq_desc[590082], slightly past the end at 512 entries.

Lucky we ship lots of memory with our machines.

for video devices.  The removed code is a left-over from the
olden days where there was concern over how video devices
worked in Linux. We are never going to go that way again,
so kill this.

Get rid of the jumbled usage of the no_logging flag. Its use
spans several directories, and is incorrectly/misleadingly
documented. Instead, two changes:
1) nvram will accept error log as soon as its ready.
2) logging to nvram stops on the first fatal error reported.

Linas Vepstas 093eda3ce5 [POWERPC] EEH: Fix PCI bridge handling bug
The EEH code needs to ignore PCI bridges; sort-of.  It was ignoring
them in the wrong place, and thus failing to set up the
PCI_DN(dn)->pcidev pointer.  Imprudent dereferencing of this pointer
would lead to a crash on cards with bridges.

Michael Neuling ca8ffc974d [POWERPC] Fix future firmware feature fixups function failure
Move firmware feature initialisation from pSeries_init_early to the
earlier pSeries_probe_hypertas so they are initialised before firmware
feature fixups are applied.

Currently firmware feature sections are only used for iSeries which
initialises the these features much earlier.  This is a bug in waiting
on pSeries.

Also adds some whitespace fixups.

Sachin P. Sant b7abc5c53e [POWERPC] Fix Kexec/Kdump for power6
On Power machines supporting VRMA, Kexec/Kdump does not work.
VRMA (virtual real-mode area) means that accesses with IR/DR = 0
(i.e. the MMU "off") actually still go through the hash table,
using entries put there by the hypervisor.

This means that when we clear out the hash table on kexec, we need to
make sure these entries are left untouched.

This also adds plpar_pte_read_raw() on the lines of
plpar_pte_remove_raw().

Mohan Kumar M 7ccb4a6624 [POWERPC] Fix interrupt distribution in ppc970
In some of the PPC970 based systems, interrupt would be distributed to
offline cpus also even when booted with "maxcpus=1".  So check whether
cpu online map and cpu present map are equal or not.  If they are equal
default_distrib_server is used as interrupt server otherwise boot cpu
(default_server) used as interrupt server.

In addition to this, if an interrupt is assigned to a specific cpu (ie
smp affinity) and if that cpu is not online, the earlier code used to
return the default_distrib_server as interrupt server.  This
introduces an additional parameter to the get_irq function, called
strict_check.  Based on this parameter, if the cpu is not online
either default_distrib_server or -1 is returned.

Jake Moilanen d8c391a559 [POWERPC] Donate idle CPU cycles on dedicated partitions
A Power6 can give up CPU cycles on a dedicated CPU (as opposed to a
shared CPU) to other shared processors if the administrator asks for it
(via the HMC).

This enables that to work properly on P6.

This just involves setting a bit in the CAS structure as well as the
VPA.  To donate cycles, a CPU has to have all SMT threads idle and
have the donate bit set in the VPA.  Then call H_CEDE.

The reason why shared processors just aren't used is because dedicated
CPUs are guaranteed an actual processor, yet the system is still able to
increase the capacity of the shared CPU pool.

Also rename the VPA's cpuctls_task_attrs field to a more accurate name.

Benjamin Herrenschmidt 3d5134ee83 [POWERPC] Rewrite IO allocation & mapping on powerpc64
This rewrites pretty much from scratch the handling of MMIO and PIO
space allocations on powerpc64.  The main goals are:

 - Get rid of imalloc and use more common code where possible
 - Simplify the current mess so that PIO space is allocated and
   mapped in a single place for PCI bridges
 - Handle allocation constraints of PIO for all bridges including
   hot plugged ones within the 2GB space reserved for IO ports,
   so that devices on hotplugged busses will now work with drivers
   that assume IO ports fit in an int.
 - Cleanup and separate tracking of the ISA space in the reserved
   low 64K of IO space. No ISA -> Nothing mapped there.

I booted a cell blade with IDE on PIO and MMIO and a dual G5 so
far, that's it :-)

With this patch, all allocations are done using the code in
mm/vmalloc.c, though we use the low level __get_vm_area with
explicit start/stop constraints in order to manage separate
areas for vmalloc/vmap, ioremap, and PCI IOs.

This greatly simplifies a lot of things, as you can see in the
diffstat of that patch :-)

A new pair of functions pcibios_map/unmap_io_space() now replace
all of the previous code that used to manipulate PCI IOs space.
The allocation is done at mapping time, which is now called from
scan_phb's, just before the devices are probed (instead of after,
which is by itself a bug fix). The only other caller is the PCI
hotplug code for hot adding PCI-PCI bridges (slots).

imalloc is gone, as is the "sub-allocation" thing, but I do beleive
that hotplug should still work in the sense that the space allocation
is always done by the PHB, but if you unmap a child bus of this PHB
(which seems to be possible), then the code should properly tear
down all the HPTE mappings for that area of the PHB allocated IO space.

I now always reserve the first 64K of IO space for the bridge with
the ISA bus on it. I have moved the code for tracking ISA in a separate
file which should also make it smarter if we ever are capable of
hot unplugging or re-plugging an ISA bridge.

This should have a side effect on platforms like powermac where VGA IOs
will no longer work. This is done on purpose though as they would have
worked semi-randomly before. The idea at this point is to isolate drivers
that might need to access those and fix them by providing a proper
function to obtain an offset to the legacy IOs of a given bus.

MMIO to the adapter, and grab the contents of the PCI status

It turns out many/most versions of firmware enable MMIO when
the slto-error-detail rtas call is made (in violation of the
architecture). Thus, it would be best to call slot-error-detail
only after notifying device drivers of a freeze, as otherwise,
a variety of strange and unexpected things may happen.

Michael Ellerman 85f2bf9f60 [POWERPC] RTAS MSI implementation
Implement MSI support via RTAS (RTAS = run-time firmware on pSeries
machines).  For now we assumes that if the required RTAS tokens for
MSI are present, then we want to use the RTAS MSI routines.

When RTAS is managing MSIs for us, it will/may enable MSI on devices that
support it by default. This is contrary to the Linux model where a device
is in LSI mode until the driver requests MSIs.

To remedy this we add a pci_irq_fixup call, which disables MSI if they've
been assigned by firmware and the device also supports LSI. Devices that
don't support LSI at all will be left as is, drivers are still expected
to call pci_enable_msi() before using the device.

At the moment there is no pci_irq_fixup on pSeries, so we can just set it
unconditionally. If other platforms use the RTAS MSI backend they'll need
to check that still holds.

Brian King 00c2ae35bd [POWERPC] Add powerpc PCI-E reset API implementation
Adds the pSeries platform implementation for a new PCI API
which can be used to issue various types of PCI-E reset,
including PCI-E warm reset and PCI-E hot reset. This is needed
for an ipr PCI-E adapter which does not properly implement BIST.
Running BIST on this adapter results in PCI-E errors. The only
reliable reset mechanism that exists on this hardware is PCI
Fundamental reset (warm reset).

Jean Delvare 6473d160b4 PCI: Cleanup the includes of <linux/pci.h>
I noticed that many source files include <linux/pci.h> while they do
not appear to need it. Here is an attempt to clean it all up.

In order to find all possibly affected files, I searched for all
files including <linux/pci.h> but without any other occurence of "pci"
or "PCI". I removed the include statement from all of these, then I
compiled an allmodconfig kernel on both i386 and x86_64 and fixed the
false positives manually.

My tests covered 66% of the affected files, so there could be false

Linas Vepstas 650f7b3b2f [POWERPC] pseries: Handle null iommu dma-window property correctly
Some versions of pSeries firmware fail to set up a
dma-window property for PCI slots that are unoccupied.
As a result, the loop searching for this propery, in
pci_dma_dev_setup_pSeriesLP(), can run to the end, resulting
in a NULL pointer dereference later in the routine. This
patch prevents the crash, and prints a warning message.

This is theoretically a rare error, as it occurs on what
is hopefully just beta levels of firmware. But just in case
this firmware escapes into the wild, this patch will avoid
the crash.

Anton Blanchard 4f5fa2fb12 [POWERPC] Bypass hcall stats until cpu features have run
I noticed that we execute hcalls before cpu feature code has run (eg
for setting up the bolted kernel region).  This means that we may be
executing code that is not appropriate for the processor we have.
Create an unconditional branch that we nop out all the time to fix this.

Mohan Kumar M b4aea36b79 [POWERPC] Avoid hypervisor statistics calculation in real mode
kexec invokes plpar_hcall hypervisor call in real mode.  plpar_hcall
refers to per cpu variables for accounting hypervisor statistics.
These variables may not be in the RMO region, so accesses to them
in real mode may result in a data storage exception.

This fixes this problem by using a new plpar_hcall_raw function which
does not update the hypervisor call statistics.  Thanks to Anton for
suggesting this idea.

Manish Ahuja 5d30bf3097 [POWERPC] pseries: Enabling auto poweron after power is restored.
During power outages, the UPS notifies the system for a shutdown.
In the current setup, it isn't possible to poweron when power is
restored.  This patch fixes the issue by calling the right
ibm,power-off-ups token during such events.  It also adds a sysfs
interface so userspace can specify whether or not to power on when
power is restored.

Provide helpers for setting up ppc_md.kexec_cpu_down, so that we don't
have to have #ifdef CONFIG_KEXEC in setup.c

Move the initialisation of the ppc_md kexec callbacks into an init routine.
This is well and truly early enough to cause no change in behaviour, we
can't kexec until userspace has given us a kernel to kexec into.

Many struct file_operations in the kernel can be "const".  Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data.  In addition it'll catch accidental writes at compile time to

The previous patch changing pSeries to use H_BULK_REMOVE broke the
JS20 blade, where the firmware doesn't support H_BULK_REMOVE.  This
adds a firmware check so that on machines that don't have H_BULK_REMOVE,
we just use the H_REMOVE call as before.

Linas Vepstas 25c4a46f0e [POWERPC] pSeries: EEH improperly enabled for some Power4 systems
It appears that EEH is improperly enabled for some Power4 systems.
On these systems, the ibm,set-eeh-option returns a value of success
even when EEH is not supported on the given node. Thus, an explicit
check for support is required.

During boot, on power4, without this patch, one sees messages
similar to:

EEH: event on unsupported device, rc=0 dn=/pci@400000000110/pci@2,2
etc.

The patch makes these go away.

Without this patch, EEH recovery does seem to work correctly for
at least some devices (I tested ethernet e1000), but fails to
recover others (the Emulex LightPulse LPFC, most notably).
Off the top of my head, I don't remember why some devices are
affected, but not others.

The PAPR indicates that the correct way to test for EEH is as
done in this patch; its not clear to me if this was in the PAPR
all along, or recently added; if it was there all along, its not
clear to me why this hadn't been fixed long ago. I suspect only
certain firmware levels are affected.

It appears that the iommu table address is never stored, and thus
never found, on non-lpar systems. Thus, for example, during boot:

No iommu table? How can that be? Well, circa line 471 of
arch/powerpc/platforms/pseries/iommu.c we see the code:

      dn = dn->parent;

and a few lines later is the surprising print statement about
the missing table.  Seems that this loop ran unto the end, never
once finding a non-null PCI_DN(dn)->iommu_table.

The problem can be found a few lines earlier: it sems that the
value of PCI_DN(dn)->iommu_table is never ever set. Thus, the
patch sets it.

The patch was tested on a Power4 system running in full system
partition mode, which is where I saw the problem. It works; I've
not done any wider testing. Had a brief discussion on this on irc.

Anton Blanchard dc40127ca5 [POWERPC] Fix bugs in the hypervisor call stats code
There were a few issues with the HCALL_STATS code:

- PURR cpu feature checks were backwards
- We iterated one entry off the end of the hcall_stats array
- Remove dead update_hcall_stats() function prototype

I noticed one thing while debugging, and that is we call H_ENTER (to set
up the MMU hashtable in early init) before we have done the cpu fixups.
This means we will execute the PURR SPR reads even on a CPU that isnt
capable of it. I wonder if we can move the CPU feature fixups earlier.

Anton Blanchard ab87e8dc88 [POWERPC] Fix corruption in hcall9
It looks to me like we are corrupting r12 in the hcall9 function.
Although we have r0 free we cant use offsets against it, so save
away r12 in there instead.  r12 holds the ninth return value from
the hypervisor call, so without this fix, the caller will see the
wrong value for the ninth element in the array that gets the return
values.

Anton Blanchard a223535425 [POWERPC] dont allow pSeries_probe to succeed without initialising MMU
pSeries_probe can decide that we are a pseries but then fail to
initialise the MMU. If an rtas node doesnt exist, we continually fall
out of pSeries_probe_hypertas early and never get to the MMU init code.

While pseries without RTAS is an illegal combination, the way we
currently fail is a pain to track down, and can happen if your flattened
device tree code has issues (like mine did :).

With the following patch we init the MMU, come up and print some
warnings about RTAS not existing, instead of looping on 0x400 exceptions.

Mohan Kumar M a5715d6dfc [POWERPC] pSeries/kexec: Fix for interrupt distribution
This allows any secondary CPU thread also to become boot cpu for
POWER5.  The patch is required to solve kdump boot issue when the
kdump kernel is booted with parameter "maxcpus=1".  XICS init code
tries to match the current boot cpu id with "reg" property in each CPU
node in the device tree.  But CPU node is created only for primary
thread CPU ids and "reg" property only reflects primary CPU ids.  So
when a kernel is booted on a secondary cpu thread above condition will
never meet and the default distribution server is left as zero.  This
leads to route the interrupts to CPU 0, but which is not online at
this time.

We use ibm,ppc-interrupt-server#s to check for both primary and
secondary CPU ids.  Accordingly default distribution server value is
initialized from "ibm,ppc-interrupt-gserver#s" property.  We loop
through ibm,ppc-interrupt-gserver#s property to find the global
distribution server from the last entry that matches with boot cpuid.

Benjamin Herrenschmidt 4c9d2800be [POWERPC] Generic OF platform driver for PCI host bridges.
When enabled in Kconfig, it will pick up any of_platform_device
matching it's match list (currently type "pci", "pcix", "pcie",
or "ht" and setup a PHB for it.

Platform must provide a ppc_md.pci_setup_phb() for it to work
(for doing the necessary initialisations specific to a given PHB
like setting up the config space ops).

It's currently only available on 64 bits as the 32 bits PCI code
can't quite cope with it in it's current form. I will fix that
later.

Benjamin Herrenschmidt 12d04eef92 [POWERPC] Refactor 64 bits DMA operations
This patch completely refactors DMA operations for 64 bits powerpc. 32 bits
is untouched for now.

We use the new dev_archdata structure to add the dma operations pointer
and associated data to struct device. While at it, we also add the OF node
pointer and numa node. In the future, we might want to look into merging
that with pci_dn as well.

The old vio, pci-iommu and pci-direct DMA ops are gone. They are now replaced
by a set of generic iommu and direct DMA ops (non PCI specific) that can be
used by bus types. The toplevel implementation is now inline.

Benjamin Herrenschmidt f90bb153b1 [POWERPC] Make pci_read_irq_line the default
This patch reworks the way IRQs are fixed up on PCI for arch powerpc.

It makes pci_read_irq_line() called by default in the PCI code for
devices that are probed, and add an optional per-device fixup in
ppc_md for platforms that really need to correct what they obtain
from pci_read_irq_line().

It also removes ppc_md.irq_bus_setup which was only used by pSeries
and should not be needed anymore.

I've also removed the pSeries s7a workaround as it can't work with
the current interrupt code anyway. I'm trying to get one of these
machines working so I can test a proper fix for that problem.

I also haven't updated the old-style fixup code from 85xx_cds.c
because it's actually buggy :) It assigns pci_dev->irq hard coded
numbers which is no good with the new IRQ mapping code. It should
at least use irq_create_mapping(NULL, hard_coded_number); and possibly
also set_irq_type() to set them as level low.

Linas Vepstas 5d2efba64b [POWERPC] Use 4kB iommu pages even on 64kB-page systems
The 10Gigabit ethernet device drivers appear to be able to chew
up all 256MB of TCE mappings on pSeries systems, as evidenced by
numerous error messages:

 iommu_alloc failed, tbl c0000000010d5c48 vaddr c0000000d875eff0 npages 1

Some experimentation indicates that this is essentially because
one 1500 byte ethernet MTU gets mapped as a 64K DMA region when
the large 64K pages are enabled. Thus, it doesn't take much to
exhaust all of the available DMA mappings for a high-speed card.

This patch changes the iommu allocator to work with its own
unique, distinct page size. Although the patch is long, its
actually quite simple: it just #defines a distinct IOMMU_PAGE_SIZE
and then uses this in all the places that matter.

As a side effect, it also dramatically improves network performance
on platforms with H-calls on iommu translation inserts/removes (since
we no longer call it 16 times for a 1500 bytes packet when the iommu HW
is still 4k).

In the future, we might want to make the IOMMU_PAGE_SIZE a variable
in the iommu_table instance, thus allowing support for different HW
page sizes in the iommu itself.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around.  On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable.  On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions.  Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller.  A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386.  I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs.  Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely.  The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking.  It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

Olaf Hering 873ef76bdc [PATCH] restore parport_pc probing on powermac
The last change for partport_pc did fix the common case for all PowerMacs,
but it broke the case for PCI multiport IO cards.  In fact, the config
option CONFIG_PARPORT_PC_SUPERIO=y lead to a hard crash when cups probed
the parport driver.  It enables the winbond and smsc probing.

Remove the PARPORT_BASE check again, parport_pc_find_nonpci_ports() will
take care of it.  All powerpc configs should have
CONFIG_PARPORT_PC_SUPERIO=n, the code did not find anything on the chrp
boards we tested it on.

Olaf Hering 30cbc22217 [PATCH] update legacy io handling for pmac
ppc can boot one single binary on prep, chrp and pmac boards.  ppc64 can
boot one single binary on pseries and G5 boards.  pmac has no legacy io,
probing for PC style legacy hardware (or accessing the legacy io area
regulary) may lead to a hard crash:

* add check for parport_pc, exit on pmac.  32bit chrp has no
  ->check_legacy_ioport, the probe is always called.  64bit chrp has
  check_legacy_ioport, check for a "parallel" node

* add check for isapnp, only PReP boards may have real ISA slots.  32bit
  PReP will have no ->check_legacy_ioport, the probe is always called.

* update code in i8042_platform_init.  Run ->check_legacy_ioport first,

Sukadev Bhattiprolu f400e198b2 [PATCH] pidspace: is_init()
This is an updated version of Eric Biederman's is_init() patch.
(http://lkml.org/lkml/2006/2/6/280).  It applies cleanly to 2.6.18-rc3 and
replaces a few more instances of ->pid == 1 with is_init().

Further, is_init() checks pid and thus removes dependency on Eric's other
patches for now.

Eric's original description:

	There are a lot of places in the kernel where we test for init
	because we give it special properties.  Most  significantly init
	must not die.  This results in code all over the kernel test
	->pid == 1.

	Introduce is_init to capture this case.

	With multiple pid spaces for all of the cases affected we are
	looking for only the first process on the system, not some other
	process that has pid == 1.

Theodore Ts'o 8e18e2941c [PATCH] inode_diet: Replace inode.u.generic_ip with inode.i_private
The following patches reduce the size of the VFS inode structure by 28 bytes
on a UP x86.  (It would be more on an x86_64 system).  This is a 10% reduction
in the inode size on a UP kernel that is configured in a production mode
(i.e., with no spinlock or other debugging functions enabled; if you want to
save memory taken up by in-core inodes, the first thing you should do is
disable the debugging options; they are responsible for a huge amount of bloat
in the VFS inode structure).

This patch:

The filesystem or device-specific pointer in the inode is inside a union,
which is pretty pointless given that all 30+ users of this field have been
using the void pointer.  Get rid of the union and rename it to i_private, with
a comment to explain who is allowed to use the void pointer.  This is just a
cleanup, but it allows us to reuse the union 'u' for something something where
the union will actually be used.

Bug fix: when marking a slot as frozen, we forgot to mark
pci device itself as frozen. (we did manage to mark the
pci children, but forget the parent itself.)

This is needed so that some device drivers can check the
pci status in critical sections (e.g. in spin loops with
interrupts disabled).

Anton Blanchard 0ddd3e7d07 [POWERPC] Always call cede in pseries dedicated idle loop
The smt_snooze_delay logic changed a bit when the idle loops were
consolidated. A value of 0 used to mean we always polled, now it means
we always sleep. Instead of restoring the old behaviour, lets put a
reasonable default in smt_snooze_delay. This means we spin for a bit
(in case an external interrupt comes in) and then sleep.

Also the pseries dedicated idle loop currently does not cede both
threads in an SMT pair. The hypervisor wants us to call in so it can
power manage, so lets do that.

As part of the new irq code pseries_kexec_cpu_down() was split into a
xics and mpic version. The vpa unregister logic is now only done in the
xics routine, and although that's ok in practice (we don't have SPLPAR
machines with mpic), I'd rather have the two concepts stay separate.

So move the vpa unregister into pseries_kexec_cpu_down(), which gets called
by both the xics and mpic routines. This also gives us an obvious place to
put any new kexec-down logic needed in future.

Cleanup CPU inits a bit more, Geoff Levand already did some earlier.

* Move CPU state save to cpu_setup, since cpu_setup is only ever done
  on cpu 0 on 64-bit and save is never done more than once.
* Rename __restore_cpu_setup to __restore_cpu_ppc970 and add
  function pointers to the cputable to use instead. Powermac always
  has 970 so no need to check there.
* Rename __970_cpu_preinit to __cpu_preinit_ppc970 and check PVR before
  calling it instead of in it, it's too early to use cputable.
* Rename pSeries_secondary_smp_init to generic_secondary_smp_init since
  everyone but powermac and iSeries use it.

This adds a shadow buffer for the SLBs and regsiters it with PHYP.
Only the bolted SLB entries (top 3) are shadowed.

The SLB shadow buffer tells the hypervisor what the kernel needs to
have in the SLB for the kernel to be able to function.  The hypervisor
can use this information to speed up partition context switches.

To fix this issue, rtas_set_indicator_fast() is added so that will not
wait for RTAS 'busy' delay and this new function is used for kdump (in
xics_teardown_cpu()) and for CPU hotplug ( xics_migrate_irqs_away() and
xics_setup_cpu()).

Note that the platform architecture spec says that set-indicator
on the indicator we're using here is not permitted to return the
busy or extended busy status codes.

We should not be calling power4_enable_pmcs() in
pseries_lpar_enable_pmcs(); just doing the hypercall is sufficient.
Prior to 2.6.15 we did not call power4_enable_pmcs() for an lpar.

power4_enable_pmcs() tries to read the hid0 register which is no
longer legal for an lpar in newer Power processors.

Jake Moilanen 9b9b72fe70 [POWERPC] Use H_CEDE on non-SMT
On the JS21 systems, they have the SPLPAR hypertas set, but are not SMT
capable.  So, they are not making the H_CEDE call.  This is causing the
hypervisor to have to queue up work for the hdecr, taking an excessive
amount of time in maintenance code, and causing jitter on the box.

Making the H_CEDE call helps alleviate that problem.

Benjamin Herrenschmidt 6e99e45828 [PATCH] powerpc: fix trigger handling in the new irq code
This patch slightly reworks the new irq code to fix a small design error.  I
removed the passing of the trigger to the map() calls entirely, it was not a
good idea to have one call do two different things.  It also fixes a couple of
corner cases.

Mapping a linux virtual irq to a physical irq now does only that.  Setting the
trigger is a different action which has a different call.

The main changes are:

- I no longer call host->ops->map() for an already mapped irq, I just return
  the virtual number that was already mapped.  It was called before to give an
  opportunity to change the trigger, but that was causing issues as that could
  happen while the interrupt was in use by a device, and because of the
  trigger change, map would potentially muck around with things in a racy way.
   That was causing much burden on a given's controller implementation of
  map() to get it right.  This is much simpler now.  map() is only called on
  the initial mapping of an irq, meaning that you know that this irq is _not_
  being used.  You can initialize the hardware if you want (though you don't
  have to).

- Controllers that can handle different type of triggers (level/edge/etc...)
  now implement the standard irq_chip->set_type() call as defined by the
  generic code.  That means that you can use the standard set_irq_type() to
  configure an irq line manually if you wish or (though I don't like that
  interface), pass explicit trigger flags to request_irq() as defined by the
  generic kernel interfaces.  Also, using those interfaces guarantees that
  your controller set_type callback is called with the descriptor lock held,
  thus providing locking against activity on the same interrupt (including
  mask/unmask/etc...) automatically.  A result is that, for example, MPIC's
  own map() implementation calls irq_set_type(NONE) to configure the hardware
  to the default triggers.

- To allow the above, the irq_map array entry for the new mapped interrupt
  is now set before map() callback is called for the controller.

- The irq_create_of_mapping() (also used by irq_of_parse_and_map()) function
  for mapping interrupts from the device-tree now also call the separate
  set_irq_type(), and only does so if there is a change in the trigger type.

- While I was at it, I changed pci_read_irq_line() (which is the helper I
  would expect most archs to use in their pcibios_fixup() to get the PCI
  interrupt routing from the device tree) to also handle a fallback when the
  DT mapping fails consisting of reading the PCI_INTERRUPT_PIN to know wether
  the device has an interrupt at all, and the the PCI_INTERRUPT_LINE to get an
  interrupt number from the device.  That number is then mapped using the
  default controller, and the trigger is set to level low.  That default
  behaviour works for several platforms that don't have a proper interrupt
  tree like Pegasos.  If it doesn't work for your platform, then either
  provide a proper interrupt tree from the firmware so that fallback isn't
  needed, or don't call pci_read_irq_line()

- Add back a bit that got dropped by my main rework patch for properly
  clearing pending IPIs on pSeries when using a kexec

Benjamin Herrenschmidt 0ebfff1491 [POWERPC] Add new interrupt mapping core and change platforms to use it
This adds the new irq remapper core and removes the old one.  Because
there are some fundamental conflicts with the old code, like the value
of NO_IRQ which I'm now setting to 0 (as per discussions with Linus),
etc..., this commit also changes the relevant platform and driver code
over to use the new remapper (so as not to cause difficulties later
in bisecting).

This patch removes the old pre-parsing of the open firmware interrupt
tree along with all the bogus assumptions it made to try to renumber
interrupts according to the platform. This is all to be handled by the
new code now.

For the pSeries XICS interrupt controller, a single remapper host is
created for the whole machine regardless of how many interrupt
presentation and source controllers are found, and it's set to match
any device node that isn't a 8259.  That works fine on pSeries and
avoids having to deal with some of the complexities of split source
controllers vs. presentation controllers in the pSeries device trees.

The powerpc i8259 PIC driver now always requests the legacy interrupt
range. It also has the feature of being able to match any device node
(including NULL) if passed no device node as an input. That will help
porting over platforms with broken device-trees like Pegasos who don't
have a proper interrupt tree.

Ingo Molnar d1bef4ed5f [PATCH] genirq: rename desc->handler to desc->chip
This patch-queue improves the generic IRQ layer to be truly generic, by adding
various abstractions and features to it, without impacting existing
functionality.

While the queue can be best described as "fix and improve everything in the
generic IRQ layer that we could think of", and thus it consists of many
smaller features and lots of cleanups, the one feature that stands out most is
the new 'irq chip' abstraction.

The irq-chip abstraction is about describing and coding and IRQ controller
driver by mapping its raw hardware capabilities [and quirks, if needed] in a
straightforward way, without having to think about "IRQ flow"
(level/edge/etc.) type of details.

This stands in contrast with the current 'irq-type' model of genirq
architectures, which 'mixes' raw hardware capabilities with 'flow' details.
The patchset supports both types of irq controller designs at once, and
converts i386 and x86_64 to the new irq-chip design.

As a bonus side-effect of the irq-chip approach, chained interrupt controllers
(master/slave PIC constructs, etc.) are now supported by design as well.

The end result of this patchset intends to be simpler architecture-level code
and more consolidation between architectures.

We reused many bits of code and many concepts from Russell King's ARM IRQ
layer, the merging of which was one of the motivations for this patchset.

This patch:

rename desc->handler to desc->chip.

Originally i did not want to do this, because it's a big patch.  But having
both "desc->handler", "desc->handle_irq" and "action->handler" caused a
large degree of confusion and made the code appear alot less clean than it
truly is.

I have also attempted a dual approach as well by introducing a
desc->chip alias - but that just wasnt robust enough and broke
frequently.

So lets get over with this quickly.  The conversion was done automatically
via scripts and converts all the code in the kernel.

This renaming patch is the first one amongst the patches, so that the
remaining patches can stay flexible and can be merged and split up
without having some big monolithic patch act as a merge barrier.

Haren Myneni 5f50867b4f [POWERPC] kdump: Reserve the existing TCE mappings left by the first kernel
During kdump boot, noticed some machines checkstop on dma protection
fault for ongoing DMA left in the first kernel. Instead of initializing
TCE entries in iommu_init() for the kdump boot, this patch fixes this
issue by walking through the each TCE table and checks whether the
entries are in use by the first kernel. If so, reserve those entries by
setting the corresponding bit in tbl->it_map such that these entries
will not be available for the kdump boot.

However it could be possible that all TCE entries might be used up due
to the driver bug that does continuous mapping. My observation is around
1700 TCE  entries are used on some systems (Ex: P4) at some point of
time during kdump boot and saving dump (either write into the disk or
sending to remote machine). Hence, this patch will make sure that
minimum of 2048 entries will be available such that kdump boot could be
successful in some cases.

Linas Vepstas 4240545661 [PATCH] powerpc/pseries: Increment fail counter in PCI recovery
When a PCI device driver does not support PCI error recovery,
the powerpc/pseries code takes a walk through a branch of code
that resets the failure counter. Because of this, if a broken
PCI card is present, the kernel will attempt to reset it an
infinite number of times. (This is annoying but mostly harmless:
each reset takes about 10-20 seconds, and uses almost no CPU time).

This patch preserves the failure count across resets.

The powerpc code is currently performing PCI setup before memory
initialization.  PCI setup touches PCI config space registers.  If the PCI
card is bad, this will evoke an error, which currrently can't be handled,
as the PCI error recovery code expects kmalloc() to be functional.  This
patch will cause the system to punt instead of crashing with

Linas Vepstas ac325acd50 [PATCH] powerpc/pseries: clear PCI failure counter if no new failures
The current PCI error recovery system keeps track of the number of PCI card
resets, and refuses to bring a card back up if this number is too large.
The goal of doing this was to avoid an infinite loop of resets if a card is
obviously dead.  However, if the failures are rare, but the machine has a
high uptime, this mechanism might still be triggered; this is too harsh.

This patch will avoids this problem by decrementing the fail count after an
hour.  Thus, as long as a pci card BSOD's less than 6 times an hour, it
will continue to be reset indefinitely.  If it's failure rate is greater
than that, it will be taken off-line permanently.

This patch is larger than it might otherwise be because it changes
indentation by removing a pointless while-loop.  The while loop is not
needed, as the handler is invoked once fo each event (by schedule_work());
the loop is leftover cruft from an earlier implementation.

Haren Myneni 81bbbe9294 [PATCH] powerpc: clear IPIs on kdump
In some crash scenarios, the kexec CPU is not responding to an IPI sent by
secondary CPU after init thread is forked, causing the system to drop into
xmon during kdump boot.  This problem can be reproduced each time when the
debugger is enabled and soft-reset is used to invoke kdump boot. The first
CPU sends an IPI - setting the IPI priority for all secondary cpus
(xics_cause_ipi()). But some CPUs will enter into the xmon via soft-reset,
i.e, not executing xics_ipi_action(). Hence, IPI is not cleared. When
exited from the debugger, one of these CPUs could become the primary kexec
CPU. Since the IPI is not cleared, causing this issue in kdump boot. This
patch clears and EOI IPI for kexec CPU as well before the kdump boot
started.

in the past where people were using for_each_cpu() where they should have been

future.

Benjamin Herrenschmidt e8222502ee [PATCH] powerpc: Kill _machine and hard-coded platform numbers
This removes statically assigned platform numbers and reworks the
powerpc platform probe code to use a better mechanism.  With this,
board support files can simply declare a new machine type with a
macro, and implement a probe() function that uses the flattened
device-tree to detect if they apply for a given machine.

We now have a machine_is() macro that replaces the comparisons of
_machine with the various PLATFORM_* constants.  This commit also
changes various drivers to use the new macro instead of looking at
_machine.

Alan Stern e041c68341 [PATCH] Notifier chain update: API changes
The kernel's implementation of notifier chains is unsafe.  There is no
protection against entries being added to or removed from a chain while the
chain is in use.  The issues were discussed in this thread:

We noticed that notifier chains in the kernel fall into two basic usage
classes:

	"Blocking" chains are always called from a process context
	and the callout routines are allowed to sleep;

	"Atomic" chains can be called from an atomic context and
	the callout routines are not allowed to sleep.

We decided to codify this distinction and make it part of the API.  Therefore
this set of patches introduces three new, parallel APIs: one for blocking
notifiers, one for atomic notifiers, and one for "raw" notifiers (which is
really just the old API under a new name).  New kinds of data structures are
used for the heads of the chains, and new routines are defined for
registration, unregistration, and calling a chain.  The three APIs are
explained in include/linux/notifier.h and their implementation is in
kernel/sys.c.

With atomic and blocking chains, the implementation guarantees that the chain
links will not be corrupted and that chain callers will not get messed up by
entries being added or removed.  For raw chains the implementation provides no
guarantees at all; users of this API must provide their own protections.  (The
idea was that situations may come up where the assumptions of the atomic and
blocking APIs are not appropriate, so it should be possible for users to
handle these things in their own way.)

There are some limitations, which should not be too hard to live with.  For
atomic/blocking chains, registration and unregistration must always be done in
a process context since the chain is protected by a mutex/rwsem.  Also, a
callout routine for a non-raw chain must not try to register or unregister
entries on its own chain.  (This did happen in a couple of places and the code
had to be changed to avoid it.)

Since atomic chains may be called from within an NMI handler, they cannot use
spinlocks for synchronization.  Instead we use RCU.  The overhead falls almost
entirely in the unregister routine, which is okay since unregistration is much
less frequent that calling a chain.

Paul Mackerras fbd7740fdf powerpc: Simplify pSeries idle loop
Since pSeries only wants to do something different in the idle loop when
there is no work to do, we can simplify the code by implementing
ppc_md.power_save functions instead of complete idle loops.  There are
two versions: one for shared-processor partitions and one for dedicated-
processor partitions.

With this we also do a cede_processor() call on dedicated processor
partitions if the poll_pending() call indicates that the hypervisor
has work it wants to do.

Anton Blanchard 4df20460a3 [PATCH] powerpc: Allow non zero boot cpuids
We currently have a hack to flip the boot cpu and its secondary thread
to logical cpuid 0 and 1. This means the logical - physical mapping will
differ depending on which cpu is boot cpu. This is most apparent on
kexec, where we might kexec on any cpu and therefore change the mapping
from boot to boot.

The patch below does a first pass early on to work out the logical cpuid
of the boot thread. We then fix up some paca structures to match.

Ive also removed the boot_cpuid_phys variable for ppc64, to be
consistent we use get_hard_smp_processor_id(boot_cpuid) everywhere.

It has been decreed that platform numbers are evil, so as a step in that
direction, replace platform_is_lpar() with a FW_FEATURE_LPAR bit.

Currently FW_FEATURE_LPAR really means i/pSeries LPAR, in the future we might
have to clean that up if we need to be more specific about what LPAR actually
means. But that's another patch ...

Paul Mackerras 82dfdcae0d powerpc: Disallow lparcfg being a module
The lparcfg code needs several things which are pretty arcane internal
details and which we don't want to export, which means that lparcfg
doesn't work when built as a module.  This makes it a bool instead of
a tristate in the Kconfig so that users can't try to build it as a
module.

John Rose 827c1a6c1a [PATCH] powerpc: fix dynamic PCI probe regression
Some hotplug driver functions were migrated to the kernel for use by EEH
in commit 2bf6a8fa21.

Previously, the PCI Hotplug module had been changed to use the new
OFDT-based PCI probe when appropriate:

When rpaphp_pci_config_slot() was moved from the rpaphp driver to the
new kernel function pcibios_add_pci_devices(), the OFDT-based probe
stuff was dropped.  This patch restores it.

Michael Ellerman 496b7a5159 [PATCH] powerpc: Fix bug in spinup of renumbered secondary threads
If the logical and physical cpu ids of a secondary thread don't match, we will
fail to spin the thread up on pSeries machines due to a bug in pseries/smp.c

We call the RTAS "start-cpu" method with the physical cpu id, the address of
pSeries_secondary_smp_init and the value to pass that function in r3. Currently
we pass "lcpu", the logical cpu id, but pSeries_secondary_smp_init expects
the physical cpu id in r3.

We should be passing pcpu instead.

David Gibson 3356bb9f7b [PATCH] powerpc: Remove lppaca structure from the PACA
At present the lppaca - the structure shared with the iSeries
hypervisor and phyp - is contained within the PACA, our own low-level
per-cpu structure.  This doesn't have to be so, the patch below
removes it, making a separate array of lppaca structures.

This saves approximately 500*NR_CPUS bytes of image size and kernel
memory, because we don't need aligning gap between the Linux and
hypervisor portions of every PACA.  On the other hand it means an
extra level of dereference in many accesses to the lppaca.

The patch also gets rid of several places where we assign the paca
address to a local variable for no particular reason.

Linas Vepstas 257ffc64a6 [PATCH] powerpc: get rid of per_cpu EEH counters
242-eeh-no-percpu-counters.patch

Remove per-cpu counters from the EEH code.  These statistics counters
are incremented at a very low frequency, and the performance gains of
per-cpu variables are negligable.  By contrast, the counters weren't
safe against cpu off/online operations, and its not worth the effort
to make them so (other than to turn them into plain globals).
