Christian König 15d3332f31 drm/radeon/kms: add support for semaphores v3
They are used to sync between rings, while fences
sync between a ring and the cpu.

v2 Fix radeon_semaphore_driver_fini when no semaphore were
allocated.

The GCT setup was used on Moorestown. The Oaktrail version uses a normal PC
interface. That means we must also honour the dither info from the BIOS

Alan Cox 0cecdd818c gma500: Final enables for Oaktrail
This switches the ifdef to match the Kconfig so that Oaktrail probing occurs
and adds some additional minor bulletproofing.

Tested on a Fujtisu Stylistic Q550 internal display. HDMI might work but that
remains to be seen.

Patrik Jakobsson fea578b6a1 gma500: SDVO DDC bus guessing isn't working so hardcode it instead
We currently don't have support for parsing SDVO mappings from BIOS so we're
guessing the bus switch parameter. This isn't working so hardcode it to a
configuration known to work on most poulsbo hardware.

Our current SDVO implementation is not working properly, so replace it with
a modified version of the i915. Further testing and debugging is needed to make
sure we can handle the different SDVO setups and wiring.

Patrik Jakobsson 9c8cee4713 gma500: Convert PSB LVDS to new output handling
LVDS for PSB now uses psb_intel_encoder and psb_intel_connectors instead of
psb_intel_output. i2c_bus and ddc_bus are moved to lvds_priv. There was also a
pointer to mode_dev (for no obvious reason) that we now get directly from
dev_priv.

Ville Syrjälä 01f2c7730e drm: Replace pitch with pitches[] in drm_framebuffer
Otherwise each driver would need to keep the information inside
their own framebuffer object structure. Also add offsets[]. BOs
on the other hand are driver specific, so those can be kept in
driver specific structures.

Christian Schmidt 59df7b1771 drm/intel: Fix initialization if startup happens in interlaced mode [v2]
My EFI BIOS starts the graphics card up in my projector's preferred EDID
mode, 1080@60i. The Intel driver does not clear all the interlaced bits.

This patch introduces a new PIPECONF_INTERLACE_MASK define and uses it
to restore progressive mode.

Christian Schmidt 4966b2a935 Fix wrong assumptions in cea_for_each_detailed_block v2
The current logic misunderstands the spec about CEA 18byte descriptors.
First, the spec doesn't state "detailed timing descriptors" but "18 byte
descriptors", so any data record could be stored, mixed timings and
other data, just as in the standard EDID.
Second, the lower four bit of byte 3 of the CEA record do not contain
the number of descriptors, but "the total number of DTDs defining native
formats in the whole EDID [...], starting with the first DTD in the DTD
list (which starts in the base EDID block)." A device can of course
support non-native formats.

As such the number can't be used to determine n, and the existing code
will filter non-timing 18byte descriptors anyway.

This reverts commit eb1711bb94.

It blows up the i915 seqno tracking, resulting in the

	BUG_ON(seqno == 0);

in i915_wait_request() triggering, which will cause lock-ups.

Adam Jackson 3b5c78a35c drm/i915/dp: Dither down to 6bpc if it makes the mode fit
Some active adaptors (VGA usually) only have two lanes at 2.7GHz.
That's a maximum pixel clock of 144MHz at 8bpc, but 192MHz at 6bpc.

Fixes Asus UX31 panel being black at startup due to no valid modes since
dc22ee6fc1.

v2: Rebased to current code, resulting in the fix applying to EDP panels as
    well.  Also changed from spatio-temporal to just spatial dithering on
    pre-ironlake, to be conssitent (and less visual flicker)

Eugeni Dodonov f45b55575c drm/i915: enable semaphores on per-device defaults
This adds a default setting for semaphores parameter, and enables
semaphores by default on IVB.

For now, as semaphores interaction with VTd causes random issues on
SNB, we do not enable them by default. But they can still be enabled
via the semaphores=1 kernel parameter.

v2: enables semaphores on SNB when IO remapping is disabled, with base
on Keith Packard patch.

Jesse Barnes 7317c75e66 drm/i915: don't set unpin_work if vblank_get fails
This fixes a race where we may try to finish a page flip and decrement
the refcount even if our vblank_get failed and we ended up with a
spurious flip pending interrupt.

Keith Packard c0f372b374 drm/i915: By default, enable RC6 on IVB and SNB when reasonable
RC6 should always work on IVB, and should work on SNB whenever IO
remapping is disabled. RC6 never works on Ironlake. Make the default
value for the parameter follow these guidelines. Setting the value
to either 0 or 1 will force the specified behavior.

Chris Wilson 5222008580 drm/i915/sdvo: Include LVDS panels for the IS_DIGITAL check
We were checking whether the supplied edid matched the connector it was
read from. We do this in case a DDC read returns an EDID for another
device on a multifunction or otherwise interesting card. However, we
failed to include LVDS as a digital device and so rejecting an otherwise
valid EDID.

Fixes the detection of the secondary SDVO LVDS panel on the Libretto
W105.

Eugeni Dodonov 4ed0b57745 drm/i915: prevent division by zero when asking for chipset power
This prevents an in-kernel division by zero which happens when we are
asking for i915_chipset_val too quickly, or within a race condition
between the power monitoring thread and userspace accesses via debugfs.

The issue can be reproduced easily via the following command:
while ``; do cat /sys/kernel/debug/dri/0/i915_emon_status; done

This is particularly dangerous because it can be triggered by
a non-privileged user by just reading the debugfs entry.

This issue was also found independently by Konstantin Belousov
<kostikbel@gmail.com>, who proposed a similar patch.

Konrad Rzeszutek Wilk 63a741757d xen/swiotlb: Use page alignment for early buffer allocation.
This fixes an odd bug found on a Dell PowerEdge 1850/0RC130
(BIOS A05 01/09/2006) where all of the modules doing pci_set_dma_mask
would fail with:

ata_piix 0000:00:1f.1: enabling device (0005 -> 0007)
ata_piix 0000:00:1f.1: can't derive routing for PCI INT A
ata_piix 0000:00:1f.1: BMDMA: failed to set dma mask, falling back to PIO

The issue was the Xen-SWIOTLB was allocated such as that the end of
buffer was stradling a page (and also above 4GB). The fix was
spotted by Kalev Leonid  which was to piggyback on git commit
e79f86b2ef "swiotlb: Use page alignment
for early buffer allocation" which:

	We could call free_bootmem_late() if swiotlb is not used, and
	it will shrink to page alignment.

	So alloc them with page alignment at first, to avoid lose two pages

And doing that fixes the outstanding issue.

Bjørn Mork 02a551c975 USB: option: Removing one bogus and adding some new Huawei combinations
Huawei use the product code HUAWEI_PRODUCT_E353 (0x1506) for a
number of different devices, which each can appear with a number
of different descriptor sets.  Different types of interfaces
can be identified by looking at the subclass and protocol fields

Subclass 1 protocol 8 is actually the data interface of a CDC
ECM set, with subclass 1 protocol 9 as the control interface.
Neither support serial data communcation, and cannot therefore
be supported by this driver.

At the same time, add a few other sets which appear if the
device is configured in "Windows mode" using this modeswitch
message:

Yinghai Lu 935a9fee51 ibft: Fix finding IBFT ACPI table on UEFI
Found one system with UEFI/iBFT, kernel does not detect the iBFT during
iscsi_ibft module loading.

Root cause: on x86 (UEFI), we are calling of find_ibft_region() much earlier
- specifically in setup_arch() before ACPI is enabled.

Try to split acpi checking code out and call that later

At that time ACPI iBFT already get permanent mapped with ioremap.
So isa_virt_to_bus() will get wrong phys from right virt address.
We could just skip that phys address printing.

For legacy one, print the found address early.

Benjamin Herrenschmidt b302545744 block/swim3: Locking fixes
The old PowerMac swim3 driver has some "interesting" locking issues,
using a private lock and failing to lock the queue before completing
requests, which triggered WARN_ONs among others.

This rips out the private lock, makes everything operate under the
block queue lock, and generally makes things simpler.

We used to also share a queue between the two possible instances which
was problematic since we might pick the wrong controller in some cases,
so make the queue and the current request per-instance and use
queuedata to point to our private data which is a lot cleaner.

We still share the queue lock but then, it's nearly impossible to actually
use 2 swim3's simultaneously: one would need to have a Wallstreet
PowerBook, the only machine afaik with two of these on the motherboard,
and populate both hotswap bays with a floppy drive (the machine ships
only with one), so nobody cares...

While at it, add a little fix to clear up stale interrupts when loading
the driver or plugging a floppy drive in a bay.

Sujit Reddy Thumma 49df780749 mmc: core: Fix deadlock when the CONFIG_MMC_UNSAFE_RESUME is not defined
mmc_suspend_host() tries to claim host during suspend
and release it only when the bus suspend operation is
compeleted. If CONFIG_MMC_UNSAFE_RESUME is defined and
the host is flagged as removable, mmc_suspend_host()
tries to remove the card. In this process, the file system
sync can get blocked trying to acquire host which is already
claimed by mmc_suspend_host() causing deadlock.

Fix this deadlock by releasing host before ->remove() is called.

Girish K S a80f162763 mmc: core: Fix power_off_notify during suspend
The eMMC 4.5 devices respond to only RESET and AWAKE command in the
sleep state. Hence the mmc switch command to notify power off state
should be sent before the device enters sleep state.

This patch fixes the same.

Stefan Nilsson XK 6de5fc9cf7 mmc: core: Add quirk for long data read time
Adds a quirk that sets the data read timeout to a fixed value instead
of relying on the information in the CSD. The timeout value chosen
is 300ms since that has proven enough for the problematic cards found,
but could be increased if other cards require this.

This patch also enables this quirk for certain Micron cards known to
have this problem.

Sascha Hauer e58f516ff4 mmc: mxcmmc: fix falling back to PIO
When we can't configure the dma channel we want to fall
back to PIO. We do this by setting host->do_dma to zero.
This does not work as do_dma is used to see whether dma
can be used for the current transfer. Instead, we have
to set host->dma to NULL.

In ssb_pcicore_fix_sprom_core_index() the sprom on the PCI core is
accessed, but the sprom only exists when the ssb bus is connected over
a PCI bus to the rest of the system and not when the SSB Bus is the
main system bus. SoCs sometimes have a PCI host controller and there
this code will not be executed, but there are some old SoCs with an PCI
controller in client mode around and ssb_pcicore_fix_sprom_core_index()
should not be called on these devices too. The PCI controller on these
devices are unused, but without this fix it results in an Data bus
error when it gets initialized.

NeilBrown 9283d8c5af md/raid5: never wait for bad-block acks on failed device.
Once a device is failed we really want to completely ignore it.
It should go away soon anyway.

In particular the presence of bad blocks on it should not cause us to
block as we won't be trying to write there anyway.

So as soon as we can check if a device is Faulty, do so and pretend
that it is already gone if it is Faulty.

When we mark blocks as bad we need them to be acknowledged by the
metadata handler promptly.

For an in-kernel metadata handler that was already being done.  But
for an external metadata handler we need to alert it of the change by
sending a notification through the sysfs file.  This adds that
notification.

NeilBrown 52c64152a9 md: bad blocks shouldn't cause a Blocked status on a Faulty device.
Once a device is marked Faulty the badblocks - whether acknowledged or
not - become irrelevant.  So they shouldn't cause the device to be
marked as Blocked.

Without this patch, a process might write "-blocked" to clear the
Blocked status, but while that will correctly fail the device, it
won't remove the apparent 'blocked' status.

NeilBrown af8a24347f md: take a reference to mddev during sysfs access.
When we are accessing an mddev via sysfs we know that the
mddev cannot disappear because it has an embedded kobj which
is refcounted by sysfs.
And we also take the mddev_lock.
However this is not enough.

The final mddev_put could have been called and the
mddev_delayed_delete is waiting for sysfs to let go so it can destroy
the kobj and mddev.
In this state there are a lot of changes that should not be attempted.

To to guard against this we:
 - initialise mddev->all_mddevs in on last put so the state can be
   easily detected.
 - in md_attr_show and md_attr_store, check ->all_mddevs under
   all_mddevs_lock and mddev_get the mddev if it still appears to
   be active.

This means that if we get to sysfs as the mddev is being deleted we
will get -EBUSY.

rdev_attr_store and rdev_attr_show are similar but already have
sufficient protection.  They check that rdev->mddev still points to
mddev after taking mddev_lock.  As this is cleared  before delayed
removal which can only be requested under the mddev_lock, this
ensure the rdev and mddev are still alive.

NeilBrown 1d23f178d5 md: refine interpretation of "hold_active == UNTIL_IOCTL".
We like md devices to disappear when they really are not needed.
However it is not possible to tell from the current state whether it
is needed or not.  We can only tell from recent history of changes.

In particular immediately after we create an md device it looks very
similar to immediately after we have finished with it.

So we always preserve a newly created md device until something
significant happens.  This state is stored in 'hold_active'.

The normal case is to keep it until an ioctl happens, as that will
normally either activate it, or explicitly de-activate it.  If it
doesn't then it was probably created by mistake and it is now time to
get rid of it.

We can also modify an array via sysfs (instead of via ioctl) and we
currently treat any change via sysfs like an ioctl as a sign that if
it now isn't more active, it should be destroyed.
However this is not appropriate as changes made via sysfs are more
gradual so we should look for a more definitive change.

So this patch only clears 'hold_active' from UNTIL_IOCTL to clear when
the array_state is changed via sysfs.  Other changes via sysfs
are ignored.

In case of an error dma_unmap() is issued in omap_hsmmc_dma_cleanup()
and then again in omap_hsmmc_post_req(). Resolve this by clearing the
host_cookie to indicate there is no DMA mapped memory to unmap.

Alan Stern fe6b91f470 PM / Driver core: leave runtime PM enabled during system shutdown
Disabling all runtime PM during system shutdown turns out not to be a
good idea, because some devices may need to be woken up from a
low-power state at that time.

The whole point of disabling runtime PM for system shutdown was to
prevent untimely runtime-suspend method calls.  This patch (as1504)
accomplishes the same result by incrementing the usage count for each
device and waiting for ongoing runtime-PM callbacks to finish.  This
is what we already do during system suspend and hibernation, which
makes sense since the shutdown method is pretty much a legacy analog
of the pm->poweroff method.

This fixes a recent regression on some OMAP systems introduced by
commit af8db1508f (PM / driver core:
disable device's runtime PM during shutdown).

So we forbid CONFIG_SPI_FSL_ESPI to be tristate here.

Andy Fleming 28d8ea2d56 fsl_pq_mdio: Clean up tbi address configuration
The code for setting the address of the internal TBI PHY was
convoluted enough without a maze of ifdefs. Clean it up a bit
so we allow the logic to fail down to -ENODEV at the end of
the if/else ladder, rather than using ifdefs to repeat the same
failure code over and over.

Also, remove the support for the auto-configuration. I'm not aware of
anyone using it, and it ends up using the bus mutex before it's been
initialized.

Shawn Guo 43af940c54 net/fec: fix the use of pdev->id
The pdev->id is used in several places for different purpose.  All
these uses assume it's always the id of fec device which is >= 0.
However this is only true for non-DT case.  When DT plays, pdev->id
is always -1, which will break these pdev->id users.

Instead of fixing all these users one by one, this patch introduces
a new member 'dev_id' to 'struct fec_enet_private' for holding the
correct fec device id, and replaces all the existing uses of pdev->id
with this dev_id.

Anton Vorontsov 77a7300aba of/irq: Get rid of NO_IRQ usage
PPC32/64 defines NO_IRQ to zero, so no problems expected.
ARM defines NO_IRQ to -1, but OF code relies on IRQ domains support,
which returns correct ('0') value in 'no irq' case. So everything
should be fine.

Other arches might break if some of their OF drivers rely on NO_IRQ
being not 0. If so, the drivers must be fixed, finally.

Daniel Vetter eb1711bb94 drm/i915: fix infinite recursion on unbind due to ilk vt-d w/a
The recursion loop goes retire_requests->unbind->gpu_idle->retire_reqeusts.

Every time we go through this we need a
- active object that can be retired
- and there are no other references to that object than the one from
  the active list, so that it gets unbound and freed immediately.
Otherwise the recursion stops. So the recursion is only limited by the
number of objects that fit these requirements sitting in the active list
any time retire_request is called.

Issue exercised by tests/gem_unref_active_buffers from i-g-t.

There's been a decent bikeshed discussion whether it wouldn't be
better to pass around a flag, but imo this is o.k. for such a limited
case that only supports a w/a.

Jerome Glisse dc97b3409a drm/ttm: callback move_notify any time bo placement change v4
Previously we were calling back move_notify in error path when the
bo is returned to it's original position or when destroy the bo.
When destroying the bo set the new mem placement as NULL when calling
back in the driver.

Updating nouveau to deal with NULL placement properly.

v2: reserve the object before calling move_notify in bo destroy path
    at that point ttm should be the only piece of code interacting
    with the object so atomic_set is safe here.
v3: callback move notify only once the bo is in its new position
    call move notify want swaping out the buffer
v4:- don't call move_notify when swapin out bo, assume driver should
     do what is appropriate in swap notify
   - move move_notify call back to ttm_bo_cleanup_memtype_use for
     destroy path

Provide helper function to compute the kernel memory size needed
for each buffer object. Move all the accounting inside ttm, simplifying
driver and avoiding code duplication accross them.

Konrad Rzeszutek Wilk 2334b75ffb drm/ttm: provide dma aware ttm page pool code V9
In TTM world the pages for the graphic drivers are kept in three different
pools: write combined, uncached, and cached (write-back). When the pages
are used by the graphic driver the graphic adapter via its built in MMU
(or AGP) programs these pages in. The programming requires the virtual address
(from the graphic adapter perspective) and the physical address (either System RAM
or the memory on the card) which is obtained using the pci_map_* calls (which does the
virtual to physical - or bus address translation). During the graphic application's
"life" those pages can be shuffled around, swapped out to disk, moved from the
VRAM to System RAM or vice-versa. This all works with the existing TTM pool code
- except when we want to use the software IOTLB (SWIOTLB) code to "map" the physical
addresses to the graphic adapter MMU. We end up programming the bounce buffer's
physical address instead of the TTM pool memory's and get a non-worky driver.
There are two solutions:
1) using the DMA API to allocate pages that are screened by the DMA API, or
2) using the pci_sync_* calls to copy the pages from the bounce-buffer and back.

This patch fixes the issue by allocating pages using the DMA API. The second
is a viable option - but it has performance drawbacks and potential correctness
issues - think of the write cache page being bounced (SWIOTLB->TTM), the
WC is set on the TTM page and the copy from SWIOTLB not making it to the TTM
page until the page has been recycled in the pool (and used by another application).

The bounce buffer does not get activated often - only in cases where we have
a 32-bit capable card and we want to use a page that is allocated above the
4GB limit. The bounce buffer offers the solution of copying the contents
of that 4GB page to an location below 4GB and then back when the operation has been
completed (or vice-versa). This is done by using the 'pci_sync_*' calls.
Note: If you look carefully enough in the existing TTM page pool code you will
notice the GFP_DMA32 flag is used  - which should guarantee that the provided page
is under 4GB. It certainly is the case, except this gets ignored in two cases:
 - If user specifies 'swiotlb=force' which bounces _every_ page.
 - If user is using a Xen's PV Linux guest (which uses the SWIOTLB and the
   underlaying PFN's aren't necessarily under 4GB).

To not have this extra copying done the other option is to allocate the pages
using the DMA API so that there is not need to map the page and perform the
expensive 'pci_sync_*' calls.

This DMA API capable TTM pool requires for this the 'struct device' to
properly call the DMA API. It also has to track the virtual and bus address of
the page being handed out in case it ends up being swapped out or de-allocated -
to make sure it is de-allocated using the proper's 'struct device'.

Implementation wise the code keeps two lists: one that is attached to the
'struct device' (via the dev->dma_pools list) and a global one to be used when
the 'struct device' is unavailable (think shrinker code). The global list can
iterate over all of the 'struct device' and its associated dma_pool. The list
in dev->dma_pools can only iterate the device's dma_pool.

The maximum amount of dma pools a device can have is six: write-combined,
uncached, and cached; then there are the DMA32 variants which are:
write-combined dma32, uncached dma32, and cached dma32.

Currently this code only gets activated when any variant of the SWIOTLB IOMMU
code is running (Intel without VT-d, AMD without GART, IBM Calgary and Xen PV
with PCI devices).

pool code. Remove it. Note further cleanup would change the
ttm page allocation helper to actualy take an array instead

Each of these error messages can be caused by a broken or malicious
userspace wanting to spam the dmesg with useless info.  They're really
not worthy of DRM_DEBUG statements either; those are generally only
useful during bringup of new hardware or versions, and ought to be
removed before going upstream anyway.

Alan Cox aa0c45fdca gma500: Fix oaktrail probing part 1
The Oaktrail platform does not use the GCT/VBT format that is used by the
Moorestowm (non PC legacy) equivalent device. It uses the BIOS tables which
means an opregion and the like.

The current code uses the wrong table which breaks things like the Fujitsu
q550 tablets. Fix the table usage as a first step.

The problem was found and diagnosed by Chia-I Wu

Alan Cox a6ba582d26 gma500: gtt based hardware scrolling console
Add support for GTT based scrolling. Instead of pushing bits around we simply
use the GTT to change the mappings. This provides us with a very fast way to
scroll the display providing we have enough memory to allocate on 4K line
boundaries. In practice this seems to be the case except for very big displays
such as HDMI, and the usual configurations are netbooks/tablets.

If we are the console then a printk can hit us with a spin lock held (and
in fact the kernel will do its best to take the console printing lock).

In that case we cannot politely sleep when synching after an accelerated op
but must behave obnoxiously to be sure of getting the bits out.
