Julian Wiedmann 5d4f78564c s390/qeth: implement smarter resizing of the RX buffer pool
The RX buffer pool is allocated in qeth_alloc_qdio_queues().
A subsequent pool resizing is then handled in a very simple way:
first free the current pool, then allocate a new pool of the requested
size.

There's two ways where this can go wrong:
1. if the resize action happens _before_ the initial pool was allocated,
   then a subsequent initialization will call qeth_alloc_qdio_queues()
   and fill the pool with a second(!) set of pages. We consume twice the
   planned amount of memory.
   This is easy to fix - just skip the resizing if the queues haven't
   been allocated yet.
2. if the initial pool was created by qeth_alloc_qdio_queues() but a
   subsequent resizing fails, then the device has no(!) RX buffer pool.
   The next initialization will _not_ call qeth_alloc_qdio_queues(), and
   attempting to back the RX buffers with pages in
   qeth_init_qdio_queues() will fail.
   Not very difficult to fix either - instead of re-allocating the whole
   pool, just allocate/free as many entries to match the desired size.

Julian Wiedmann 0e635c2a87 s390/qeth: cancel RX reclaim work earlier
When qeth's napi poll code fails to refill an entirely empty RX ring, it
kicks off buffer_reclaim_work to try again later.

Make sure that this worker is cancelled when setting the qeth device
offline. Otherwise a RX refill action can unexpectedly end up running
concurrently to bigger re-configurations (eg. resizing the buffer pool),
without any locking.

Julian Wiedmann 240c194849 s390/qeth: don't reset default_out_queue
When an OSA device in prio-queue setup is reduced to 1 TX queue due to
HW restrictions, we reset its the default_out_queue to 0.

In the old code this was needed so that qeth_get_priority_queue() gets
the queue selection right. But with proper multiqueue support we already
reduced dev->real_num_tx_queues to 1, and so the stack puts all traffic
on txq 0 without even calling .ndo_select_queue.

Thus we can preserve the user's configuration, and apply it if the OSA
device later re-gains support for multiple TX queues.

Julian Wiedmann 1309982414 s390/qdio: add tighter controls for IRQ polling
Once the call to qdio_establish() has completed, qdio is free to deliver
data IRQs to the device driver's IRQ poll handler.

For qeth (the only qdio driver that currently uses IRQ polling) this is
problematic, since the IRQs can arrive before its NAPI instance is
even registered. Calling napi_schedule() from qeth_qdio_start_poll()
then crashes in various nasty ways.

Until recently qeth checked for IFF_UP to drop such early interrupts,
but that's fragile as well since it doesn't enforce any ordering.

Fix this properly by bringing up the qdio device in IRQS_DISABLED mode,
and have the driver explicitly opt-in to receive data IRQs.
qeth does so from qeth_open(), which kick-starts a NAPI poll and then
calls qdio_start_irq() from qeth_poll().

Also add a matching qdio_stop_irq() in qeth_stop() to switch the qdio
dataplane back into a disabled state.

Alexandra Winter 6f3846f095 s390/qeth: vnicc Fix EOPNOTSUPP precedence
When getting or setting VNICC parameters, the error code EOPNOTSUPP
should have precedence over EBUSY.

EBUSY is used because vnicc feature and bridgeport feature are mutually
exclusive, which is a temporary condition.
Whereas EOPNOTSUPP indicates that the HW does not support all or parts of
the vnicc feature.
This issue causes the vnicc sysfs params to show 'blocked by bridgeport'
for HW that does not support VNICC at all.

qeth_l?_stop_card() is _never_ called while in HARDSETUP state, and
there's no other usage of the card state that relies on the
DOWN -> HARDSETUP -> SOFTSETUP transition.

As related cleanup, remove the check in qeth_realloc_buffer_pool() as it
is already done by the callers.

Julian Wiedmann 7f92316c53 s390/qeth: make cmd/reply matching more flexible
When data is received on the READ channel, the matching logic for cmds
that are waiting for a reply is currently hard-coded into the channel's
main IO callback.
Move this into a per-cmd callback, so that we can apply custom matching
logic for each individual cmd.

This also allows us to remove the coarse-grained check for unexpected
non-IPA replies, since they will no longer match against _all_ pending
cmds.

Note that IDX cmds use _no_ matcher, since their reply is synchronously
received as part of the cmd's IO.

Julian Wiedmann 0b698c838e s390/qeth: fix initialization on old HW
I stumbled over an old OSA model that claims to support DIAG_ASSIST,
but then rejects the cmd to query its DIAG capabilities.

In the old code this was ok, as the returned raw error code was > 0.
Now that we translate the raw codes to errnos, the "rc < 0" causes us
to fail the initialization of the device.

The fix is trivial: don't bail out when the DIAG query fails. Such an
error is not critical, we can still use the device (with a slightly
reduced set of features).

During vnicc_init wanted_char should be compared to cur_char and not
to QETH_VNICC_DEFAULT. Without this patch there is no way to enforce
the default values as desired values.

Note, that it is expected, that a card comes online with default values.
This patch was tested with private card firmware.

Alexandra Winter e8a66d8004 s390/qeth: Fix vnicc_is_in_use if rx_bcast not set
Symptom: After vnicc/rx_bcast has been manually set to 0,
	bridge_* sysfs parameters can still be set or written.
Only occurs on HiperSockets, as OSA doesn't support changing rx_bcast.

Vnic characteristics and bridgeport settings are mutually exclusive.
rx_bcast defaults to 1, so manually setting it to 0 should disable
bridge_* parameters.

Instead it makes sense here to check the supported mask. If the card
does not support vnicc at all, bridge commands are always allowed.

Alexandra Winter 68c57bfd52 s390/qeth: fix false reporting of VNIC CHAR config failure
Symptom: Error message "Configuring the VNIC characteristics failed"
in dmesg whenever an OSA interface on z15 is set online.

The VNIC characteristics get re-programmed when setting a L2 device
online. This follows the selected 'wanted' characteristics - with the
exception that the INVISIBLE characteristic unconditionally gets
switched off.

For devices that don't support INVISIBLE (ie. OSA), the resulting
IO failure raises a noisy error message
("Configuring the VNIC characteristics failed").
For IQD, INVISIBLE is off by default anyways.

So don't unnecessarily special-case the INVISIBLE characteristic, and
thereby suppress the misleading error message on OSA devices.

qeth_l3_dev_hsuid_store() initially checks the card state, but doesn't
take the conf_mutex to ensure that the card stays in this state while
being reconfigured.

Rework the code to take this lock, and drop a redundant state check in a
helper function.

Julian Wiedmann 8b5026bc16 s390/qeth: fix qdio teardown after early init error
qeth_l?_set_online() goes through a number of initialization steps, and
on any error uses qeth_l?_stop_card() to tear down the residual state.

The first initialization step is qeth_core_hardsetup_card(). When this
fails after having established a QDIO context on the device
(ie. somewhere after qeth_mpc_initialize()), qeth_l?_stop_card() doesn't
shut down this QDIO context again (since the card state hasn't
progressed from DOWN at this stage).

Even worse, we then call qdio_free() as final teardown step to free the
QDIO data structures - while some of them are still hooked into wider
QDIO infrastructure such as the IRQ list. This is inevitably followed by
use-after-frees and other nastyness.

Fix this by unconditionally calling qeth_qdio_clear_card() to shut down
the QDIO context, and also to halt/clear any pending activity on the
various IO channels.
Remove the naive attempt at handling the teardown in
qeth_mpc_initialize(), it clearly doesn't suffice and we're handling it
properly now in the wider teardown code.

Julian Wiedmann 0f399305cd s390/qeth: fix promiscuous mode after reset
When managing the promiscuous mode during an RX modeset, qeth caches the
current HW state to avoid repeated programming of the same state on each
modeset.

But while tearing down a device, we forget to clear the cached state. So
when the device is later set online again, the initial RX modeset
doesn't program the promiscuous mode since we believe it is already
enabled.
Fix this by clearing the cached state in the tear-down path.

Note that for the SBP variant of promiscuous mode, this accidentally
works right now because we unconditionally restore the SBP role while
re-initializing.

Julian Wiedmann 2e3d7fa5d2 s390/qeth: handle error due to unsupported transport mode
Along with z/VM NICs, there's additional device types that only support
a specific transport mode (eg. external-bridged IQD).
Identify the corresponding error code, and raise a fitting error message
so that the user knows to adjust their device configuration.

On top of that also fix the subsequent error path, so that the rejected
cmd doesn't need to wait for a timeout but gets cancelled straight away.

This is a leftover from back when a recovery action didn't go through
dev_close(), and was meant to shoot down all remaining af_iucv sockets
on the interface.

Now that the offline path always calls dev_close(), the
NETDEV_GOING_DOWN event from __dev_close_many() is sufficient and this
hack can be removed.

Julian Wiedmann e6b1b7da24 s390/qeth: overhaul L3 IP address dump code
The current code that dumps the RXIP/VIPA/IPATO addresses via sysfs
first checks whether the buffer still provides sufficient space to hold
another formatted address.
But the maximum length of an formatted IPv4 address is 15 characters,
not 12. So we underestimate the max required length and if the buffer
was previously filled to _just_ the right level, a formatted address can
end up being truncated.

Revamp these code paths to use the _actually_ required length of the
formatted IP address, and while at it suppress a gratuitous newline.

Also use scnprintf() to format the output. In case of a truncation, this
would allow us to return the number of characters that were actually
written.

Julian Wiedmann 7359393f3c s390/qeth: wake up all waiters from qeth_irq()
card->wait_q is shared by different users, for different wake-up
conditions. qeth_irq() can potentially trigger multiple of these
conditions:
1) A change to channel->irq_pending, which qeth_send_control_data() is
   waiting for.
2) A change to card->state, which qeth_clear_channel() and
   qeth_halt_channel() are waiting for.

As qeth_irq() does only a single wake_up(), we might miss to wake up
a second eligible waiter. Luckily all waiters are guarded with a
timeout, so this situation should recover on its own eventually.

To make things work robustly, add an additional wake_up() for changes
to channel->state. And extract a helper that updates
channel->irq_pending along with the needed wake_up().

Julian Wiedmann f9e50b02a9 s390/qeth: fix dangling IO buffers after halt/clear
The cio layer's intparm logic does not align itself well with how qeth
manages cmd IOs. When an active IO gets terminated via halt/clear, the
corresponding IRQ's intparm does not reflect the cmd buffer but rather
the intparm that was passed to ccw_device_halt() / ccw_device_clear().
This behaviour was recently clarified in
commit b91d9e67e5 ("s390/cio: fix intparm documentation").

As a result, qeth_irq() currently doesn't cancel a cmd that was
terminated via halt/clear. This primarily causes us to leak
card->read_cmd after the qeth device is removed, since our IO path still
holds a refcount for this cmd.

For qeth this means that we need to keep track of which IO is pending on
a device ('active_cmd'), and use this as the intparm when calling
halt/clear. Otherwise qeth_irq() can't match the subsequent IRQ to its
cmd buffer.
Since we now keep track of the _expected_ intparm, we can also detect
any mismatch; this would constitute a bug somewhere in the lower layers.
In this case cancel the active cmd - we effectively "lost" the IRQ and
should not expect any further notification for this IO.

When the RX path builds non-linear skbs, the packet headers can
currently spill over into page fragments. Depending on the packet type
and what fields we need to access in the headers, this could cause us
to go past the end of skb->data.

So for non-linear packets, copy precisely the length of the necessary
headers ('linear_len') into skb->data.
And don't copy more, upper-level protocols will peel whatever additional
packet headers they need.

Depending on a packet's type, the RX path needs to access fields in the
packet headers and thus requires a minimum packet length.
Enforce this length when building the skb.

On the other hand a single runt packet is no reason to drop the whole
RX buffer. So just skip it, and continue processing on the next packet.

Julian Wiedmann c8183f5489 s390/qeth: fix potential deadlock on workqueue flush
The L2 bridgeport code uses the coarse 'conf_mutex' for guarding access
to its configuration state.
This can result in a deadlock when qeth_l2_stop_card() - called under the
conf_mutex - blocks on flush_workqueue() to wait for the completion of
pending bridgeport workers. Such workers would also need to aquire
the conf_mutex, stalling indefinitely.

Introduce a lock that specifically guards the bridgeport configuration,
so that the workers no longer need the conf_mutex.
Wrapping qeth_l2_promisc_to_bridge() in this fine-grained lock then also
fixes a theoretical race against a concurrent qeth_bridge_port_role_store()
operation.

Ursula Braun 42bfba9eaa net/smc: immediate termination for SMCD link groups
SMCD link group termination is called when peer signals its shutdown
of its corresponding link group. For regular shutdowns no connections
exist anymore. For abnormal shutdowns connections must be killed and
their DMBs must be unregistered immediately. That means the SMCR method
to delay the link group freeing several seconds does not fit.

This patch adds immediate termination of a link group and its SMCD
connections and makes sure all SMCD link group related cleanup steps
are finished.

Julian Wiedmann 8311c7a252 s390/qeth: clean up error path in qeth_core_probe_device()
qeth_core_free_card() is meant to be the counterpart of
qeth_alloc_card() - but unfortunately was also picked as the place
to free the QDIO queues.

This gets messy when qeth_core_probe_device() fails during
qeth_add_dbf_entry(). At this point the card->qdio.state is not initialized
yet, so qeth_free_qdio_queues() ends up operating on uninitialized data.

Luckily for now, the whole qeth_card struct is zero-allocated and the value
of the QETH_QDIO_UNINITIALIZED enum is 0 as well. So there's no real impact
from this bug at the moment, it's just really fragile.

Clean this up by moving the qeth_free_qdio_queues() call up one level in
the hierarchy. This way it doesn't get called from the error path.

Packets with an unexpected HW format are currently first extracted from
the RX buffer, passed upwards to the layer-specific driver and only then
finally dropped.

Enhance the RX path so that we can drop such packets before even
allocating an skb. For this, add some additional logic so that when a
packet is meant to be dropped, we can still walk along the packet's data
chunks in the RX buffer. This allows us to extract the following
packet(s) from the buffer.

Each RX buffer may contain up to 64KB worth of data. In case the device
needs to discard a packet _after_ already having reserved space for it
in the buffer, the whole buffer gets set to ERROR state. As the buffer
might contain any number of good packets, this can result in collateral
packet loss.

qeth can provide relief by enabling per-frame invalidation. The RX
buffer is then presented as usual, we just need to spot & drop any
individual packet that was flagged as invalid.

(b) qeth_core_hardsetup_card(), which is only called while the card is
    still in DOWN state. No change in behaviour here.

Julian Wiedmann e53edf743d s390/qeth: add TX NAPI support for IQD devices
Due to their large MTU and potentially low utilization of TX buffers,
IQD devices in particular require fast TX recycling. This makes them
a prime candidate for a TX NAPI path in qeth.

qeth_tx_poll() uses the recently introduced qdio_inspect_queue() helper
to poll the TX queue for completed buffers. To avoid hogging the CPU for
too long, we yield to the stack after completing an entire queue's worth
of buffers.
While IQD is expected to transfer its buffers synchronously (and thus
doesn't support TX interrupts), a timer covers for the odd case where a
TX buffer doesn't complete synchronously. Currently this timer should
only ever fire for
(1) the mcast queue,
(2) the occasional race, where the NAPI poll code observes an update to
    queue->used_buffers while the TX doorbell hasn't been issued yet.

tx_dropped, to be consistent with dev->tx_dropped.

Julian Wiedmann 292a50e3fc s390/qeth: reject oversized SNMP requests
Commit d4c08afafa ("s390/qeth: streamline SNMP cmd code") removed
the bounds checking for req_len, under the assumption that the check in
qeth_alloc_cmd() would suffice.

But that code path isn't sufficiently robust to handle a user-provided
data_length, which could overflow (when adding the cmd header overhead)
before being checked against QETH_BUFSIZE. We end up allocating just a
tiny iob, and the subsequent copy_from_user() writes past the end of
that iob.

Special-case this path and add a coarse bounds check, to protect against
maliciuous requests. This let's the subsequent code flow do its normal
job and precise checking, without risk of overflow.

Julian Wiedmann 308946b074 s390/qeth: merge qeth_reply struct into qeth_cmd_buffer
Except for card->read_cmd, every cmd we issue now passes through
qeth_send_control_data() and allocates a qeth_reply struct. The way we
use this struct requires additional refcounting, and pointer tracking.

Clean up things by moving most of qeth_reply's content into the main
cmd struct. This keeps things in one place, saves us the additional
refcounting and simplifies the overall code flow.
A nice little benefit is that we can now match incoming replies against
the pending requests themselves, without caching the requests' seqnos.

The qeth_reply struct stays around for a little bit longer in a shrunk
form, to avoid touching every single callback.

Julian Wiedmann 12fc286f84 s390/qeth: propagate length of processed cmd IO data to callback
When an cmd IO completes in qeth_irq(), calculate how much data was
processed by the device and pass this value to the cmd's callback.

This allows cmds that retrieve data from the device to check whether
sufficient data was received, so we do that in qeth_read_conf_data_cb().

Julian Wiedmann 072f794000 s390/qeth: serialize cmd reply with concurrent timeout
Callbacks for a cmd reply run outside the protection of card->lock, to
allow for additional cmds to be issued & enqueued in parallel.

When qeth_send_control_data() bails out for a cmd without having
received a reply (eg. due to timeout), its callback may concurrently be
processing a reply that just arrived. In this case, the callback
potentially accesses a stale reply->reply_param area that eg. was
on-stack and has already been released.

To avoid this race, add some locking so that qeth_send_control_data()
can (1) wait for a concurrently running callback, and (2) zap any
pending callback that still wants to run.

Julian Wiedmann eca1d5c22d s390/qeth: move cast type selection into fill_header()
The cast type currently gets selected in .ndo_start_xmit, and is then
piped through several layers until it's stored into the HW header.
Push the selection down into qeth_l?_fill_header() to (1) reduce the
number of xmit-wide parameters, and (2) merge the two route validation
checks into just one.

Julian Wiedmann c097cc9228 s390/qeth: consolidate skb RX processing in L3 driver
Use napi_gro_receive() to pass up all types of packets that a L3 device
may receive.
1) For proper L2 packets received by the IQD sniffer, this is the
   obvious thing to do.
2) For af_iucv (which doesn't provide a GRO assist), the GRO code will
   transparently fall back to netif_receive_skb(). So there's no need to
   special-case this traffic in our code.

The VNICC code is somewhat quirky in that it defers the whole cmd setup
to a common helper qeth_l2_vnicc_request(). Some of the cmd specifics
are then passed in via parameter, while others are simply hard-coded.

Split the whole machinery up into the usual format: one helper that
allocates the cmd & fills in the common fields, while all the cmd
originators take care of their sub-cmd type specific work.
This makes it much easier to calculate the cmd's precise length, and
reduces code complexity.

This patch converts the adapter, assist and bridgeport cmd paths to
dynamic allocation. Most of the work is about re-organizing the cmd
headers, calculating the correct cmd length, and filling in the right
value in the sub-cmd's length field.

Since we now also set the correct length for cmds that are not reflected
by a fixed struct (ie SNMP), we can remove the work-around from
qeth_snmp_command().

For code that uses qeth_send_simple_setassparms_prot(), we currently
can't differentiate whether the cmd should contain (1) no parameter, or
(2) a 4-byte parameter with value 0.
At the moment this doesn't cause any trouble. But when using dynamically
allocated cmds, we need to know whether to allocate & transmit an
additional 4 bytes of zeroes.
So instead of the raw parameter value, pass a parameter pointer
(or NULL) to qeth_send_simple_setassparms_prot().

by dynamically allocating all simple IPA cmds (eg. STARTLAN, SETVMAC).
It also converts the OSN path.

Doing so requires some changes to how we calculate the cmd length.
Currently when building IPA cmds, we're quite generous in how much data
we send down to the device (basically the size of the biggest cmd we
know). This is no real concern at the moment, since the static cmd
buffers are backed with zeroed pages. But for dynamic allocations, the
exact length matters. So this patch also adds the needed length
calculations to each cmd path.

Commands that have multiple subtypes (eg. SETADP) of differing length
will be converted with follow-up patches.

IO left that's still using them is the long-running READ.
Replace this with a single allocated cmd, that gets restarted whenever
the READ completed.

survive the IO completion.

qeth currently uses a fixed set of statically allocated cmd buffers for
the read and write IO channels. This (1) doesn't play well with the single
RCD cmd we need to issue on the data channel, (2) doesn't provide the
necessary flexibility for certain IDX improvements, and (3) is also rather
wasteful since the buffers are idle most of the time.

Add a new type of cmd buffer that is dynamically allocated, and keeps
its ccw chain in the DMA data area. Since this touches most callers of
qeth_setup_ccw(), also add a new CCW flags parameter for future usage.

OSN currently provides a custom code path to submit IPA cmds, without
waiting for the cmd response. Replace it with qeth_send_ipa_cmd(), which
uses the common qeth_send_control_data() IO infrastructure.

By setting a custom iob->callback, we can now provide feedback to the
caller about whether the cmd has been successfully submitted to HW.
Since the callback then immediately wakes up the reply-waiter object, we
maintain the old behaviour of returning early without waiting for the
response.

Alexandra Winter 335726195e s390/qeth: fix VLAN attribute in bridge_hostnotify udev event
Enabling sysfs attribute bridge_hostnotify triggers a series of udev events
for the MAC addresses of all currently connected peers. In case no VLAN is
set for a peer, the device reports the corresponding MAC addresses with
VLAN ID 4096. This currently results in attribute VLAN=4096 for all
non-VLAN interfaces in the initial series of events after host-notify is
enabled.

Instead, no VLAN attribute should be reported in the udev event for
non-VLAN interfaces.

Only the initial events face this issue. For dynamic changes that are
reported later, the device uses a validity flag.

This also changes the code so that it now sets the VLAN attribute for
MAC addresses with VID 0. On Linux, no qeth interface will ever be
registered with VID 0: Linux kernel registers VID 0 on all network
interfaces initially, but qeth will drop .ndo_vlan_rx_add_vid for VID 0.
Peers with other OSs could register MACs with VID 0.

While qeth_l3 uses netif_keep_dst() to hold onto the dst, a skb's dst
may still have been obsoleted (via dst_dev_put()) by the time that we
end up using it. The dst then points to the loopback interface, which
means the neighbour lookup in qeth_l3_get_cast_type() determines a bogus
cast type of RTN_BROADCAST.
For IQD interfaces this causes us to place such skbs on the wrong
HW queue, resulting in TX errors.

Fix-up the various call sites to first validate the dst entry with
dst_check(), and fall back accordingly.

When selecting the cast type of a neighbourless IPv4 skb (eg. on a raw
socket), qeth_l3 falls back to the packet's destination IP address.

This fixes DHCP requests, which were misclassified as unicast
(and for IQD interfaces thus ended up on the wrong HW queue).

When building the L3 HW header for non-IP packets, trust the cast type
that was passed as parameter. qeth_l3_get_cast_type() has most likely
also used h_dest to determine the cast type, so we get the same
result, and can remove that duplicated code.
In the unlikely case that we would get a _different_ cast type, then
that's based off a route lookup and should be considered authoritative.

On the first initialization of a queue, its Output Buffers are in a
clean state with no attached resources. On every subsequent
initialization, qeth_l?_stop_card() has previously put them in a clean
state via qeth_drain_output_queues(). So the call to
qeth_clear_output_buffer() is redundant and can be removed.

While at it, move the initialization of the queue's card pointer into
the queue allocation. It never changes afterwards.

Julian Wiedmann 54a50941b7 s390/qeth: stop/wake TX queues based on their fill level
Current xmit code only stops the txq after attempting to fill an
IO buffer that hasn't been TX-completed yet. In many-connection
scenarios, this can result in frequent rejected TX attempts, requeuing
of skbs with NETDEV_TX_BUSY and extra overhead.

Now that we have a proper 1-to-1 relation between stack-side txqs and
our HW Queues, overhaul the stop/wake logic so that the xmit code
stops the txq as needed.
Given that we might map multiple skbs into a single buffer, it's crucial
to ensure that the queue always provides an _entirely_ empty IO buffer.
Otherwise large skbs (eg TSO) might not fit into the last available
buffer. So whenever qeth_do_send_packet() first utilizes an _empty_
buffer, it updates & checks the used_buffers count.

This now ensures that an skb passed to qeth_xmit() can always be mapped
into an IO buffer, so remove all of the -EBUSY roll-back handling in the
TX path. We preserve the minimal safety-checks ("Is this IO buffer
really available?"), just in case some nasty future bug ever attempts to
corrupt an in-use buffer.

Julian Wiedmann 3a18d75400 s390/qeth: add TX multiqueue support for IQD devices
qeth has been supporting multiple HW Output Queues for a long time. But
rather than exposing those queues to the stack, it uses its own queue
selection logic in .ndo_start_xmit... with all the drawbacks that
entails.
Start off by switching IQD devices over to a proper mqs net_device,
and converting all the netdev_queue management code.

One oddity with IQD devices is the requirement to place all mcast
traffic on the _highest_ established HW queue. Doing so via
.ndo_select_queue seems straight-forward - but that won't work if only
some of the HW queues are active
(ie. when dev->real_num_tx_queues < dev->num_tx_queues), since
netdev_cap_txqueue() will not allow us to put skbs on the higher queues.

To make this work, we
1. let .ndo_select_queue() map all mcast traffic to netdev_queue 0, and
2. later re-map the netdev_queue and HW queue indices in
   .ndo_start_xmit and the TX completion handler.

With this patch we default to a fixed set of 1 ucast and 1 mcast queue.
Support for dynamic reconfiguration is added at a later time.

As the documentation for netif_trans_update() says, netdev_start_xmit()
already updates the last-tx time after every good xmit. So don't
duplicate that effort.

One odd case is that qeth_flush_buffers() also gets called from our
TX completion handler, to flush out any partially filled buffer when
we switch the queue to non-packing mode. But as the TX completion
handler will _always_ wake the txq, we don't have to worry about
the TX watchdog there.

This converts the IDX code to use qeth_send_control_data(), replacing
a bunch of duplicated IO code and unbounded waits. It also allows the
IDX sequence to benefit from the improved timeout & notify
infrastructure, so that we can eliminate the DOWN -> ACTIVATING -> UP
transition in the channel state machine.

The patch looks rather big, but most of it is a straight-forward
conversion of the old IDX cmd setup & callbacks to the new model.

The L2 and L3 .ndo_set_rx_mode callbacks maintain an address cache
to decide which addresses have changed since the last modeset.

When the card is set offline, qeth_l?_stop_card() drains this cache.
This happens only after 1) the net_device has been detached, and
2) any pending RX modeset has completed. Consequently we can access the
cache lock-free.

.ndo_set_rx_mode gets called in process context, but while holding the
addr_list spinlock. Which means we currently can't sleep while
re-programming the HW, and need to poll for IO completion. That's bad,
in particular since receiving the cmd response can fail silently and
we're then polling until the timeout hits.

As a first step towards eliminating the IO completion polling, run the
RX modeset from a work element and only take the addr_list lock while
updating the RX mode address cache.

As part of the TX completion path, qeth_release_skbs() frees the completed
skbs with __skb_queue_purge(). This ends in kfree_skb(), reporting every
completed skb as dropped.
On the other hand when dropping an skb in .ndo_start_xmit, we end up
calling consume_skb()... where we should be using kfree_skb() so that
drop monitors get notified.

Switch the drop/consume logic around, and also don't accumulate dropped
packets in the tx_errors statistics.

Now that qeth always uses dev_close() to shutdown the interface, we can
trust the locking and remove some custom state checks.
qeth_l?_stop_card() is no longer called for a card in UP state, so remove
the checks there too. This basically makes the UP state obsolete, so rip
out the whole thing (except for the sysfs-visible string).

Julian Wiedmann d4560150cb s390/qeth: call dev_close() during recovery
When resetting an interface ("recovery"), qeth currently attempts to
elide the call to dev_close(). We initially only call .ndo_close to
quiesce the data path, and then offline & online the ccwgroup device.
If the reset succeeded, a call to .ndo_open then resumes the data path
along with some internal setup (dev_addr validation, RX modeset) that
dev_open() would have usually triggered.
dev_close() only gets called (via the close_dev worker) if the reset
action fails.

It's unclear whether this was initially done due to locking concerns, or
rather to execute the reset transparently. Either way, temporarily
closing the interface without dev_close() is fragile, and means we're
susceptible to various races and unexpected behaviour. For instance:

- Bypassing dev_deactivate_many() means that the qdiscs are not set to
__QDISC_STATE_DEACTIVATED. Consequently any intermittent TX completion
can wake up the txq, resulting in calls to .ndo_start_xmit while the
data path is down. We have custom state checking to detect this case
and drop such packets.

- Because the IFF_UP flag doesn't reflect the interface's actual state
during a reset, we have custom state checking in .ndo_open and
.ndo_close to guard against invalid calls.

- Considering that the reset might take a considerable amount of time
(in particular if an IO fails and we end up waiting for its timeout), we
_do_ want NETDEV_GOING_DOWN and NETDEV_DOWN events so that components
like bonding, team, bridge, macvlan, vlan, ... can take appropriate
action.

In its attempt to run only the minimal amount of tear down steps,
qeth_l2_stop_card() fails to reset the "is dev_addr registered?" flag
in some rare scenarios. But a future change to the tear down sequence
would cause us to _always_ hit this issue, so patch it up before that
code lands.

Fix it by unconditionally clearing the flag bit. This also allows us to
remove the additional cleanup step in qeth_dev_layer2_store().

Sebastian Ott 0ff06c44ef s390/ism: ignore some errors during deregistration
Prior to dma unmap/free operations the ism driver tries to ensure
that the memory is no longer accessed by the HW. When errors
during deregistration of memory regions from the HW occur the ism
driver will not unmap/free this memory.

When we receive notification from the hypervisor that a PCI function
has been detached we can no longer access the device and would never
unmap/free these memory regions which led to complaints by the DMA
debug API.

Treat this kind of errors during the deregistration of memory regions
from the HW as success since it is already ensured that the memory
is no longer accessed by HW.

Julian Wiedmann 8024cc9e85 s390/qeth: split out OSN netdev ops
Rather than special-casing OSN in a number of places, just give this
device type its own netdev_ops structure.

When setting up the OSN net_device, also skip the handling of the
various HW offloads (eg TSO). The device shouldn't be advertising any of
them, and the OSN code paths in qeth don't have support for them.
In particular RX VLAN filtering is not supported, so don't hook up those
callbacks in the netdev_ops.

qeth dynamically allocates an array for storing pointers to its
Output Queue structures. Switch this to a static array - we are
currently limited to 4 Output Queues, so shrinking the qeth_qdio_info
struct by just a few bytes doesn't justify the additional complexity.

Julian Wiedmann 742d4d4083 s390/qeth: convert remaining legacy cmd callbacks
This calls the existing errno translation helpers from the callbacks,
adding trivial wrappers where necessary. For cmds that have no
sophisticated errno translation, default to -EIO.

For IPA cmds with no callback, fall back to a minimal default. This is
currently being used by qeth_l3_send_setrouting().

Thus having all converted all callbacks, remove the legacy path in
qeth_send_control_data_cb().

Julian Wiedmann 4b7ae12216 s390/qeth: allow cmd callbacks to return errnos
Error propagation from cmd callbacks currently works in a way where
qeth_send_control_data_cb() picks the raw HW code from the response,
and the cmd's originator later translates this into an errno.
The callback itself only returns 0 ("done") or 1 ("expect more data").

This is
1. limiting, as the only means for the callback to report an internal
error is to invent pseudo HW codes (such as IPA_RC_ENOMEM), that
the originator then needs to understand. For non-IPA callbacks, we
even provide a separate field in the IO buffer metadata (iob->rc) so
the callback can pass back a return value.
2. fragile, as the originator must take care to not translate any errno
that is returned by qeth's own IO code paths (eg -ENOMEM). Also, any
originator that forgets to translate the HW codes potentially passes
garbage back to its caller. For instance, see
commit 2aa4867198 ("s390/qeth: translate SETVLAN/DELVLAN errors").

Introduce a new model where all HW error translation is done within the
callback, and the callback returns
>  0, if it expects more data (as before)
== 0, on success
<  0, with an errno

Start off with converting all callbacks to the new model that either
a) pass back pseudo HW codes, or b) have a dependency on a specific
HW error code. Also convert c) the one callback that uses iob->rc, and
d) qeth_setadpparms_change_macaddr_cb() so that it can pass back an
error back to qeth_l2_request_initial_mac() even when the cmd itself
was successful.

The old model remains supported: if the callback returns 0, we still
propagate the response's HW error code back to the originator.

Julian Wiedmann 54daaca702 s390/qeth: cancel cmd on early error
When sending cmds via qeth_send_control_data(), qeth puts the request
on the IO channel and then blocks on the reply object until the response
has been received.

If the IO completes with error, there will never be a response and we
block until the reply-wait hits its timeout. For this case, connect the
request buffer to its reply object, so that we can immediately cancel
the wait.

Julian Wiedmann 0951c6babf s390/qeth: simplify reply object handling
Current code enqueues & dequeues a reply object from the waiter list
in various places. In particular, the dequeue & enqueue in
qeth_send_control_data_cb() looks fragile - this can cause
qeth_clear_ipacmd_list() to skip the active object.
Add some helpers, and boil the logic down by giving
qeth_send_control_data() the sole responsibility to add and remove
objects.

qeth_send_control_data_cb() and qeth_clear_ipacmd_list() will now only
notify the reply object to interrupt its wait cycle. This can cause
a slight delay in the removal, but that's no concern.

Julian Wiedmann 4386e34fab s390/qeth: align csum offload with TSO control logic
csum offload and TSO have similar programming requirements. The TSO code
was reworked with commit "s390/qeth: enhance TSO control sequence",
adjust the csum control flow accordingly. Primarily this means replacing
custom helpers with more generic infrastructure.

Also, change the LP2LP check so that it warns on TX offload (not RX).
This is where reduced csum capability actually matters.
