Alexander Graf b9e3e20893 KVM: PPC: E500: Remove kvmppc_e500_tlbil_all usage from guest TLB code
The guest TLB handling code should not have any insight into how the host
TLB shadow code works.

kvmppc_e500_tlbil_all() is a function that is used for distinction between
e500v2 and e500mc (E.HV) on how to flush shadow entries. This function really
is private between the e500.c/e500mc.c file and e500_mmu_host.c.

Instead of this one, use the public kvmppc_core_flush_tlb() function to flush
all shadow TLB entries. As a nice side effect, with this we also end up
flushing TLB1 entries which we forgot to do before.

Alexander Graf 483ba97c0f KVM: PPC: E500: Make clear_tlb_refs and clear_tlb1_bitmap static
Host shadow TLB flushing is logic that the guest TLB code should have
no insight about. Declare the internal clear_tlb_refs and clear_tlb1_bitmap
functions static to the host TLB handling file.

Instead of these, we can use the already exported kvmppc_core_flush_tlb().
This gives us a common API across the board to say "please flush any
pending host shadow translation".

Alexander Graf c015c62b13 KVM: PPC: e500: Implement TLB1-in-TLB0 mapping
When a host mapping fault happens in a guest TLB1 entry today, we
map the translated guest entry into the host's TLB1.

This isn't particularly clever when the guest is mapped by normal 4k
pages, since these would be a lot better to put into TLB0 instead.

This patch adds the required logic to map 4k TLB1 shadow maps into
the host's TLB0.

When emulating tlbwe, we want to automatically map the entry that just got
written in our shadow TLB map, because chances are quite high that it's
going to be used very soon.

Today this happens explicitly, duplicating all the logic that is in
kvmppc_mmu_map() already. Just call that one instead.

When shadow mapping a page, mapping this page can fail. In that case we
don't have a shadow map.

Take this case into account, otherwise we might end up writing bogus TLB
entries into the host TLB.

While at it, also move the write_stlbe() calls into the respective TLBn
handlers.

Vincent Bernat d59577b6ff sk-filter: Add ability to lock a socket filter program
While a privileged program can open a raw socket, attach some
restrictive filter and drop its privileges (or send the socket to an
unprivileged program through some Unix socket), the filter can still
be removed or modified by the unprivileged program. This commit adds a
socket option to lock the filter (SO_LOCK_FILTER) preventing any
modification of a socket filter program.

This is similar to OpenBSD BIOCLOCK ioctl on bpf sockets, except even
root is not allowed change/drop the filter.

The state of the lock can be read with getsockopt(). No error is
triggered if the state is not changed. -EPERM is returned when a user
tries to remove the lock or to change/remove the filter while the lock
is active. The check is done directly in sk_attach_filter() and
sk_detach_filter() and does not affect only setsockopt() syscall.

Jason Gunthorpe 023f333a99 NTP: Add a CONFIG_RTC_SYSTOHC configuration
The purpose of this option is to allow ARM/etc systems that rely on the
class RTC subsystem to have the same kind of automatic NTP based
synchronization that we have on PC platforms. Today ARM does not
implement update_persistent_clock and makes extensive use of the class
RTC system.

When enabled CONFIG_RTC_SYSTOHC will provide a generic
rtc_update_persistent_clock that stores the current time in the RTC and
is intended complement the existing CONFIG_RTC_HCTOSYS option that loads
the RTC at boot.

Like with RTC_HCTOSYS the platform's update_persistent_clock is used
first, if it works. Platforms with mixed class RTC and non-RTC drivers
need to return ENODEV when class RTC should be used. Such an update for
PPC is included in this patch.

Long term, implementations of update_persistent_clock should migrate to
proper class RTC drivers and use CONFIG_RTC_SYSTOHC instead.

Tested on ARM kirkwood and PPC405

Alexey Kardashevskiy 1715a826a5 powerpc: Add DSCR support to ptrace
The DSCR (aka Data Stream Control Register) is supported on some
server PowerPC chips and allow some control over the prefetch
of data streams.

The kernel already supports DSCR value per thread but there is also
a need in a ability to change it from an external process for
the specific pid.

The patch adds new register index PT_DSCR (index=44) which can be
set/get by:
  ptrace(PTRACE_POKEUSER, traced_process, PT_DSCR << 3, dscr);
  dscr = ptrace(PTRACE_PEEKUSER, traced_process, PT_DSCR << 3, NULL);

The patch does not increase PT_REGS_COUNT as the pt_regs struct has not
been changed.

Alexander Graf 1c81063655 KVM: PPC: BookE: Implement EPR exit
The External Proxy Facility in FSL BookE chips allows the interrupt
controller to automatically acknowledge an interrupt as soon as a
core gets its pending external interrupt delivered.

Today, user space implements the interrupt controller, so we need to
check on it during such a cycle.

This patch implements logic for user space to enable EPR exiting,
disable EPR exiting and EPR exiting itself, so that user space can
acknowledge an interrupt when an external interrupt has successfully
been delivered into the guest vcpu.

Alexander Graf b8c649a99d KVM: PPC: BookE: Allow irq deliveries to inject requests
When injecting an interrupt into guest context, we usually don't need
to check for requests anymore. At least not until today.

With the introduction of EPR, we will have to create a request when the
guest has successfully accepted an external interrupt though.

So we need to prepare the interrupt delivery to abort guest entry
gracefully. Otherwise we'd delay the EPR request.

On mfspr/mtspr emulation path Book3E's MMUCFG SPR with value 1015 clashes
with G4's MSSSR0 SPR. Move MSSSR0 emulation from generic part to Books3S.
MSSSR0 also clashes with Book3S's DABRX SPR. DABRX was not explicitly
handled so Book3S execution flow will behave as before.

Alexander Graf 50c7bb80b5 KVM: PPC: Book3S: PR: Enable alternative instruction for SC 1
When running on top of pHyp, the hypercall instruction "sc 1" goes
straight into pHyp without trapping in supervisor mode.

So if we want to support PAPR guest in this configuration we need to
add a second way of accessing PAPR hypercalls, preferably with the
exact same semantics except for the instruction.

So let's overlay an officially reserved instruction and emulate PAPR
hypercalls whenever we hit that one.

The FWNMI region is fixed at 0x7000 and the vector are now
overflowing that with some configurations. Fix that by moving
some hash management code out of that region as it doesn't need
to be that close to the call sites (isn't accessed using
conditional branches).

Michael Neuling e13e895f84 powerpc/perf: Fix for PMCs not making progress
On POWER7 when we have really small counts left before overflow, we can take a
PMU IRQ, but the PMC gets wound back to just before the overflow.

If the kernel is setting the PMC to a value just before the overflow, we can
get interrupted again without the PMC making any progress (ie another buggy
overflow).  In this case, we can end up making no forward progress, with the
PMC interrupt returning us to the same count over and over.

The below detects when we are making no forward progress (ie. delta = 0) and
then increases the amount left before the overflow.  This stops us from locking
up.

Michael Neuling bc09c219b2 powerpc/perf: Fix finding overflowed PMC in interrupt
If a PMC is about to overflow on a counter that's on an active perf event
(ie. less than 256 from the end) and a _different_ PMC overflows just at this
time (a PMC that's not on an active perf event), we currently mark the event as
found, but in reality it's not as it's likely the other PMC that caused the
IRQ.  Since we mark it as found the second catch all for overflows doesn't run,
and we don't reset the overflowing PMC ever.  Hence we keep hitting that same
PMC IRQ over and over and don't reset the actual overflowing counter.

This is a rewrite of the perf interrupt handler for book3s to get around this.
We now check to see if any of the PMCs have actually overflowed (ie >=
0x80000000).  If yes, record it for active counters and just reset it for
inactive counters.  If it's not overflowed, then we check to see if it's one of
the buggy power7 counters and if it is, record it and continue.  If none of the
PMCs match this, then we make note that we couldn't find the PMC that caused
the IRQ.

Thadeu Lima de Souza Cascardo 6a040ce725 powerpc/eeh: Fix crash when adding a device in a slot with DDW
The DDW code uses a eeh_dev struct from the pci_dev. However, this is
not set until eeh_add_device_late is called.

Since pci_bus_add_devices is called before eeh_add_device_late, the PCI
devices are added to the bus, making drivers' probe hooks to be called.
These will call set_dma_mask, which will call the DDW code, which will
require the eeh_dev struct from pci_dev. This would result in a crash,
due to a NULL dereference.

Calling eeh_add_device_late after pci_bus_add_devices would make the
system BUG, because device files shouldn't be added to devices there
were not added to the system. So, a new function is needed to add such
files only after pci_bus_add_devices have been called.

Thadeu Lima de Souza Cascardo 7f966d394d powerpc/iommu: Prevent false TCE leak message
When a device DMA window includes the address 0, it's reserved in the
TCE bitmap to avoid returning that address to drivers.

When the device is removed, the bitmap is checked for any mappings not
removed by the driver, indicating a possible DMA mapping leak. Since the
reserved address is not cleared, a message is printed, warning of such a
leak.

Check for the reservation, and clear it before checking for any other
standing mappings.

Michael Neuling 9422de3e95 powerpc: Hardware breakpoints rewrite to handle non DABR breakpoint registers
This is a rewrite so that we don't assume we are using the DABR throughout the
code.  We now use the arch_hw_breakpoint to store the breakpoint in a generic
manner in the thread_struct, rather than storing the raw DABR value.

The ptrace GET/SET_DEBUGREG interface currently passes the raw DABR in from
userspace.  We keep this functionality, so that future changes (like the POWER8
DAWR), will still fake the DABR to userspace.

Anton Blanchard 98679fb092 powerpc: Avoid load of static chain register when calling nested functions through a pointer on 64bit
The ppc64 ABI has a static chain register (r11) which is only used
when calling nested functions through a pointer. Considering that
we take a dim view of nested functions in the kernel, we have a lot
of unnecessary overhead here.

gcc 4.7 has an option to disable loading of r11 so lets use it.

If hell freezes over and hipsters manage to litter the kernel
with nested functions, gcc will give us an error message and
won't simply compile bad code:

    You cannot take the address of a nested function if you use
    the -mno-pointers-to-nested-functions option.

Furthermore our kernel module trampolines don't setup the static
chain register so adding this option and forcing gcc to error out
makes even more sense.

[PATCH 6/6] powerpc: Implement PPR save/restore

When the task enters in to kernel space, the user defined priority (PPR)
will be saved in to PACA at the beginning of first level exception
vector and then copy from PACA to thread_info in second level vector.
PPR will be restored from thread_info before exits the kernel space.

P7/P8 temporarily raises the thread priority to higher level during
exception until the program executes HMT_* calls. But it will not modify
PPR register. So we save PPR value whenever some register is available
to use and then calls HMT_MEDIUM to increase the priority. This feature
supports on P7 or later processors.

We save/ restore PPR for all exception vectors except system call entry.
GLIBC will be saving / restore for system calls. So the default PPR
value (3) will be set for the system call exit when the task returned
to the user space.

David Woodhouse fe3955cb29 powerpc: Enable ARCH_USE_BUILTIN_BSWAP
By using the compiler intrinsics instead of hand-crafted opaque inline
assembler for byte-swapping, we let the compiler see what's actually
happening and it gets to use lwbrx/stwbrx instructions instead of a
normal load/store coupled with a sequence of rlwimi instructions to
move bits around.

Compiled-tested only. It gave a code size reduction of almost 4% for
ext2, and more like 2.5% for ext3/ext4.

Ian Munsie a413f474a0 powerpc: Disable relocation on exceptions whenever PR KVM is active
For PR KVM we allow userspace to map 0xc000000000000000. Because
transitioning from userspace to the guest kernel may use the relocated
exception vectors we have to disable relocation on exceptions whenever
PR KVM is active as we cannot trust that address.

This issue does not apply to HV KVM, since changing from a guest to the
hypervisor will never use the relocated exception vectors.

Currently the hypervisor interface only allows us to toggle relocation
on exceptions on a partition wide scope, so we need to globally disable
relocation on exceptions when the first PR KVM instance is started and
only re-enable them when all PR KVM instances have been destroyed.

It's a bit heavy handed, but until the hypervisor gives us a lightweight
way to toggle relocation on exceptions on a single thread it's only real
option.

Jimi Xenidis 96f013fe1b powerpc/kexec: Add kexec "hold" support for Book3e processors
Motivation:
IBM Blue Gene/Q comes with some very strange firmware that I'm trying to get out
of using in the kernel.  So instead I spin all the threads in the boot wrapper
(using the firmware) and have them enter the kexec stub, pre-translated at the
virtual "linear" address, never touching firmware again.

This works strategy works wonderfully, but I need the following patch in the
kexec stub. I believe it should not effect Book3S and Book3E does not appear
to be here yet so I'd love to get any criticisms up front.

This patch adds two items:

1) Book3e requires that GPR4 survive the "hold" process, so we make
   sure that happens.
2) Book3e has no real mode, and the hold code exploits this.  Since
   these processors ares always translated, we arrange for the kexeced
   threads to enter the hold code using the normal kernel linear mapping.

Finally remove the two level TOC and build with -mcmodel=medium.

Unfortunately we can't build modules with -mcmodel=medium due to
the tricks the kernel module loader plays with percpu data:

# -mcmodel=medium breaks modules because it uses 32bit offsets from
# the TOC pointer to create pointers where possible. Pointers into the
# percpu data area are created by this method.
#
# The kernel module loader relocates the percpu data section from the
# original location (starting with 0xd...) to somewhere in the base
# kernel percpu data space (starting with 0xc...). We need a full
# 64bit relocation for this to work, hence -mcmodel=large.

On older kernels we fall back to the two level TOC (-mminimal-toc)

Anton Blanchard 5ac47f7a6e powerpc: Relocate prom_init.c on 64bit
The ppc64 kernel can get loaded at any address which means
our very early init code in prom_init.c must be relocatable. We do
this with a pretty nasty RELOC() macro that we wrap accesses of
variables with. It is very fragile and sometimes we forget to add a
RELOC() to an uncommon path or sometimes a compiler change breaks it.

32bit has a much more elegant solution where we build prom_init.c
with -mrelocatable and then process the relocations manually.
Unfortunately we can't do the equivalent on 64bit and we would
have to build the entire kernel relocatable (-pie), resulting in a
large increase in kernel footprint (megabytes of relocation data).
The relocation data will be marked __initdata but it still creates
more pressure on our already tight memory layout at boot.

Alan Modra pointed out that the 64bit ABI is relocatable even
if we don't build with -pie, we just need to relocate the TOC.
This patch implements that idea and relocates the TOC entries of
prom_init.c. An added bonus is there are very few relocations to
process which helps keep boot times on simulators down.

gcc does not put 64bit integer constants into the TOC but to be
safe we may want a build time script which passes through the
prom_init.c TOC entries to make sure everything looks reasonable.

This patch actually hooks up doorbell interrupts on POWER8:

- Select the PPC_DOORBELL Kconfig option from PPC_PSERIES

- Add the doorbell CPU feature bit to POWER8

- We define a new pSeries_cause_ipi_mux() function that issues a
  doorbell interrupt if the recipient is another thread within the same
  core as the sender. If the recipient is in a different core it falls
  back to using XICS to deliver the IPI as before.

- During pSeries_smp_probe() at boot, we check if doorbell interrupts
  are supported. If they are we set the cause_ipi function pointer to
  the above mentioned function, otherwise we leave it as whichever XICS
  cause_ipi function was determined by xics_smp_probe().

Ian Munsie fe9e1d54e3 powerpc: Add code to handle soft-disabled doorbells on server
This patch adds the logic to properly handle doorbells that come in when
interrupts have been soft disabled and to replay them when interrupts
are re-enabled:

- masked_##_H##interrupt is modified to leave interrupts enabled when a
  doorbell has come in since doorbells are edge sensitive and as such
  won't be automatically re-raised.

- __check_irq_replay now tests if a doorbell happened on book3s, and
  returns either 0xe80 or 0xa00 depending on whether we are the
  hypervisor or not.

- restore_check_irq_replay now tests for the two possible server
  doorbell vector numbers to replay.

- __replay_interrupt also adds tests for the two server doorbell vector
  numbers, and is modified to use a compare instruction rather than an
  andi. on the single bit difference between 0x500 and 0x900.

The last two use a CPU feature section to avoid needlessly testing
against the hypervisor vector if it is not the hypervisor, and vice
versa.

Ian Munsie 42d02b81f2 powerpc: Define differences between doorbells on book3e and book3s
There are a few key differences between doorbells on server compared
with embedded that we care about on Linux, namely:

- We have a new msgsndp instruction for directed privileged doorbells.
  msgsnd is used for directed hypervisor doorbells.
- The tag we use in the instruction is the Thread Identification
  Register of the recipient thread (since server doorbells can only
  occur between threads within a single core), and is only 7 bits wide.
- A new message type is introduced for server doorbells (none of the
  existing book3e message types are currently supported on book3s).

Michael Ellerman 323a6bf1d6 powerpc: Add a powerpc implementation of SHA-1
This patch adds a crypto driver which provides a powerpc accelerated
implementation of SHA-1, accelerated in that it is written in asm.

Original patch by Paul, minor fixups for upstream by moi.

Lightly tested on 64-bit with the test program here:

Seems to work, and is "not slower" than the generic version.

Needs testing on 32-bit.

Greg Kroah-Hartman cad5cef62a POWERPC: drivers: remove __dev* attributes.
CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Philippe De Muyter 9a32299394 powerpc, dma: move bestcomm driver from arch/powerpc/sysdev to drivers/dma
The bestcomm dma hardware, and some of its users like the FEC ethernet
component, is used in different FreeScale parts, including non-powerpc
parts like the ColdFire MCF547x & MCF548x families.  Don't keep the
driver hidden in arch/powerpc where it is inaccessible for other arches.
.c files are moved to drivers/dma/bestcomm, while .h files are moved to
include/linux/fsl/bestcomm.  Makefiles, Kconfigs and #include directives
are updated for the new file locations.

Tested by recompiling for MPC5200 with all bestcomm users enabled.

Shan Hai ce73ec6db4 powerpc/vdso: Remove redundant locking in update_vsyscall_tz()
The locking in update_vsyscall_tz() is not only unnecessary because the vdso
code copies the data unproteced in __kernel_gettimeofday() but also
introduces a hard to reproduce race condition between update_vsyscall()
and update_vsyscall_tz(), which causes user space process to loop
forever in vdso code.

The following patch removes the locking from update_vsyscall_tz().

Locking is not only unnecessary because the vdso code copies the data
unprotected in __kernel_gettimeofday() but also erroneous because updating
the tb_update_count is not atomic and introduces a hard to reproduce race
condition between update_vsyscall() and update_vsyscall_tz(), which further
causes user space process to loop forever in vdso code.

The below scenario describes the race condition,

	    timer interrupt
		update_vsyscall

					    update_vsyscall_tz

Because the ++ operator would be implemented as three instructions and not
atomic on powerpc.

Catalin Marinas 0ad50c3896 compat: generic compat_sys_sched_rr_get_interval() implementation
This function is used by sparc, powerpc tile and arm64 for compat support.
 The patch adds a generic implementation with a wrapper for PowerPC to do
the u32->int sign extension.

The reason for a single patch covering powerpc, tile, sparc and arm64 is
to keep it bisectable, otherwise kernel building may fail with mismatched
function declarations.

Matthew Leach 7929d407e4 include/linux/init.h: use the stringify operator for the __define_initcall macro
Currently the __define_initcall() macro takes three arguments, fn, id and
level.  The level argument is exactly the same as the id argument but
wrapped in quotes.  To overcome this need to specify three arguments to
the __define_initcall macro, where one argument is the stringification of
another, we can just use the stringification macro instead.

  containers in general and user namespaces in particular.  The user
  space interface is now complete.

  This set of changes adds support for unprivileged users to create user
  namespaces and as a user namespace root to create other namespaces.
  The tyranny of supporting suid root preventing unprivileged users from
  using cool new kernel features is broken.

  This set of changes completes the work on setns, adding support for
  the pid, user, mount namespaces.

  This set of changes includes a bunch of basic pid namespace
  cleanups/simplifications.  Of particular significance is the rework of
  the pid namespace cleanup so it no longer requires sending out
  tendrils into all kinds of unexpected cleanup paths for operation.  At
  least one case of broken error handling is fixed by this cleanup.

  The files under /proc/<pid>/ns/ have been converted from regular files
  to magic symlinks which prevents incorrect caching by the VFS,
  ensuring the files always refer to the namespace the process is
  currently using and ensuring that the ptrace_mayaccess permission
  checks are always applied.

  The files under /proc/<pid>/ns/ have been given stable inode numbers
  so it is now possible to see if different processes share the same
  namespaces.

  Through the David Miller's net tree are changes to relax many of the
  permission checks in the networking stack to allowing the user
  namespace root to usefully use the networking stack.  Similar changes
  for the mount namespace and the pid namespace are coming through my
  tree.

  Two small changes to add user namespace support were commited here adn
  in David Miller's -net tree so that I could complete the work on the
  /proc/<pid>/ns/ files in this tree.

  Work remains to make it safe to build user namespaces and 9p, afs,
  ceph, cifs, coda, gfs2, ncpfs, nfs, nfsd, ocfs2, and xfs so the
  Kconfig guard remains in place preventing that user namespaces from
  being built when any of those filesystems are enabled.

  Future design work remains to allow root users outside of the initial
  user namespace to mount more than just /proc and /sys."

David Rientjes c2d23f919b mm, oom: remove statically defined arch functions of same name
out_of_memory() is a globally defined function to call the oom killer.
x86, sh, and powerpc all use a function of the same name within file scope
in their respective fault.c unnecessarily.  Inline the functions into the
pagefault handlers to clean the code up.

     kernel_execve() is a wrapper for do_execve() - it doesn't need to
     do transition to user mode anymore.

     As a result kernel_thread() and kernel_execve() are
     arch-independent now - they live in kernel/fork.c and fs/exec.c
     resp.  sys_execve() is also in fs/exec.c and it's completely
     architecture-independent.

   - daemonize() is gone, along with its parts in fs/*.c

   - struct pt_regs * is no longer passed to do_fork/copy_process/
     copy_thread/do_execve/search_binary_handler/->load_binary/do_coredump.

   - sys_fork()/sys_vfork()/sys_clone() unified; some architectures
     still need wrappers (ones with callee-saved registers not saved in
     pt_regs on syscall entry), but the main part of those suckers is in
     kernel/fork.c now."

Joonsoo Kim 81df9bff26 bootmem: fix wrong call parameter for free_bootmem()
It is strange that alloc_bootmem() returns a virtual address and
free_bootmem() requires a physical address.  Anyway, free_bootmem()'s
first parameter should be physical address.

There are some call sites for free_bootmem() with virtual address.  So fix
them.

Wen Congyang 8732794b16 numa: convert static memory to dynamically allocated memory for per node device
We use a static array to store struct node.  In many cases, we don't have
too many nodes, and some memory will be unused.  Convert it to per-device
dynamically allocated memory.

 and just send you one with the other fixes/updates and then, after
 3.8-rc1 is out, do the rest of the removals to ensure we catch them all,
 it's up to you.  The merges should all be trivial, and Stephen has been
 doing them all in linux-next for a few weeks now quite easily.
 
 Other than the __dev* marking removals, there's nothing major here, some
 firmware loading updates and other minor things in the driver core.

In BookE, EPCR is defined and valid when either the HV or the 64bit
category are implemented. Reflect this in the field definition.

Today the only KVM target on 64bit is HV enabled, so there is no

Paul Mackerras b4072df407 KVM: PPC: Book3S HV: Handle guest-caused machine checks on POWER7 without panicking
Currently, if a machine check interrupt happens while we are in the
guest, we exit the guest and call the host's machine check handler,
which tends to cause the host to panic.  Some machine checks can be
triggered by the guest; for example, if the guest creates two entries
in the SLB that map the same effective address, and then accesses that
effective address, the CPU will take a machine check interrupt.

To handle this better, when a machine check happens inside the guest,
we call a new function, kvmppc_realmode_machine_check(), while still in
real mode before exiting the guest.  On POWER7, it handles the cases
that the guest can trigger, either by flushing and reloading the SLB,
or by flushing the TLB, and then it delivers the machine check interrupt
directly to the guest without going back to the host.  On POWER7, the
OPAL firmware patches the machine check interrupt vector so that it
gets control first, and it leaves behind its analysis of the situation
in a structure pointed to by the opal_mc_evt field of the paca.  The
kvmppc_realmode_machine_check() function looks at this, and if OPAL
reports that there was no error, or that it has handled the error, we
also go straight back to the guest with a machine check.  We have to
deliver a machine check to the guest since the machine check interrupt
might have trashed valid values in SRR0/1.

If the machine check is one we can't handle in real mode, and one that
OPAL hasn't already handled, or on PPC970, we exit the guest and call
the host's machine check handler.  We do this by jumping to the
machine_check_fwnmi label, rather than absolute address 0x200, because
we don't want to re-execute OPAL's handler on POWER7.  On PPC970, the
two are equivalent because address 0x200 just contains a branch.

Then, if the host machine check handler decides that the system can
continue executing, kvmppc_handle_exit() delivers a machine check
interrupt to the guest -- once again to let the guest know that SRR0/1
have been modified.

Paul Mackerras 1b400ba0cd KVM: PPC: Book3S HV: Improve handling of local vs. global TLB invalidations
When we change or remove a HPT (hashed page table) entry, we can do
either a global TLB invalidation (tlbie) that works across the whole
machine, or a local invalidation (tlbiel) that only affects this core.
Currently we do local invalidations if the VM has only one vcpu or if
the guest requests it with the H_LOCAL flag, though the guest Linux
kernel currently doesn't ever use H_LOCAL.  Then, to cope with the
possibility that vcpus moving around to different physical cores might
expose stale TLB entries, there is some code in kvmppc_hv_entry to
flush the whole TLB of entries for this VM if either this vcpu is now
running on a different physical core from where it last ran, or if this
physical core last ran a different vcpu.

There are a number of problems on POWER7 with this as it stands:

- The TLB invalidation is done per thread, whereas it only needs to be
  done per core, since the TLB is shared between the threads.
- With the possibility of the host paging out guest pages, the use of
  H_LOCAL by an SMP guest is dangerous since the guest could possibly
  retain and use a stale TLB entry pointing to a page that had been
  removed from the guest.
- The TLB invalidations that we do when a vcpu moves from one physical
  core to another are unnecessary in the case of an SMP guest that isn't
  using H_LOCAL.
- The optimization of using local invalidations rather than global should
  apply to guests with one virtual core, not just one vcpu.

(None of this applies on PPC970, since there we always have to
invalidate the whole TLB when entering and leaving the guest, and we
can't support paging out guest memory.)

To fix these problems and simplify the code, we now maintain a simple
cpumask of which cpus need to flush the TLB on entry to the guest.
(This is indexed by cpu, though we only ever use the bits for thread
0 of each core.)  Whenever we do a local TLB invalidation, we set the
bits for every cpu except the bit for thread 0 of the core that we're
currently running on.  Whenever we enter a guest, we test and clear the
bit for our core, and flush the TLB if it was set.

On initial startup of the VM, and when resetting the HPT, we set all the
bits in the need_tlb_flush cpumask, since any core could potentially have
stale TLB entries from the previous VM to use the same LPID, or the
previous contents of the HPT.

Then, we maintain a count of the number of online virtual cores, and use
that when deciding whether to use a local invalidation rather than the
number of online vcpus.  The code to make that decision is extracted out
into a new function, global_invalidates().  For multi-core guests on
POWER7 (i.e. when we are using mmu notifiers), we now never do local
invalidations regardless of the H_LOCAL flag.

Paul Mackerras 28c483b62f KVM: PPC: Book3S PR: Fix VSX handling
This fixes various issues in how we were handling the VSX registers
that exist on POWER7 machines.  First, we were running off the end
of the current->thread.fpr[] array.  Ultimately this was because the
vcpu->arch.vsr[] array is sized to be able to store both the FP
registers and the extra VSX registers (i.e. 64 entries), but PR KVM
only uses it for the extra VSX registers (i.e. 32 entries).

Secondly, calling load_up_vsx() from C code is a really bad idea,
because it jumps to fast_exception_return at the end, rather than
returning with a blr instruction.  This was causing it to jump off
to a random location with random register contents, since it was using
the largely uninitialized stack frame created by kvmppc_load_up_vsx.

In fact, it isn't necessary to call either __giveup_vsx or load_up_vsx,
since giveup_fpu and load_up_fpu handle the extra VSX registers as well
as the standard FP registers on machines with VSX.  Also, since VSX
instructions can access the VMX registers and the FP registers as well
as the extra VSX registers, we have to load up the FP and VMX registers
before we can turn on the MSR_VSX bit for the guest.  Conversely, if
we save away any of the VSX or FP registers, we have to turn off MSR_VSX
for the guest.

To handle all this, it is more convenient for a single call to
kvmppc_giveup_ext() to handle all the state saving that needs to be done,
so we make it take a set of MSR bits rather than just one, and the switch
statement becomes a series of if statements.  Similarly kvmppc_handle_ext
needs to be able to load up more than one set of registers.

permissions on a HPT entry be changed to allow writing, we make the
requested change even if the page is marked read-only in the host
Linux page tables.  This is a problem since it would for instance
allow a guest to modify a page that KSM has decided can be shared
between multiple guests.

To fix this, if the new permissions for the page allow writing, we need
to look up the memslot for the page, work out the host virtual address,
and look up the Linux page tables to get the PTE for the page.  If that
PTE is read-only, we reduce the HPTE permissions to read-only.

With HV-style KVM, we maintain reverse-mapping lists that enable us to
find all the HPT (hashed page table) entries that reference each guest
physical page, with the heads of the lists in the memslot->arch.rmap
arrays.  When we reset the HPT (i.e. when we reboot the VM), we clear
out all the HPT entries but we were not clearing out the reverse
mapping lists.  The result is that as we create new HPT entries, the
lists get corrupted, which can easily lead to loops, resulting in the
host kernel hanging when it tries to traverse those lists.

This fixes the problem by zeroing out all the reverse mapping lists
when we zero out the HPT.  This incidentally means that we are also
zeroing our record of the referenced and changed bits (not the bits
in the Linux PTEs, used by the Linux MM subsystem, but the bits used
by the KVM_GET_DIRTY_LOG ioctl, and those used by kvm_age_hva() and
kvm_test_age_hva()).

Paul Mackerras a2932923cc KVM: PPC: Book3S HV: Provide a method for userspace to read and write the HPT
A new ioctl, KVM_PPC_GET_HTAB_FD, returns a file descriptor.  Reads on
this fd return the contents of the HPT (hashed page table), writes
create and/or remove entries in the HPT.  There is a new capability,
KVM_CAP_PPC_HTAB_FD, to indicate the presence of the ioctl.  The ioctl
takes an argument structure with the index of the first HPT entry to
read out and a set of flags.  The flags indicate whether the user is
intending to read or write the HPT, and whether to return all entries
or only the "bolted" entries (those with the bolted bit, 0x10, set in
the first doubleword).

This is intended for use in implementing qemu's savevm/loadvm and for
live migration.  Therefore, on reads, the first pass returns information
about all HPTEs (or all bolted HPTEs).  When the first pass reaches the
end of the HPT, it returns from the read.  Subsequent reads only return
information about HPTEs that have changed since they were last read.
A read that finds no changed HPTEs in the HPT following where the last
read finished will return 0 bytes.

The format of the data provides a simple run-length compression of the
invalid entries.  Each block of data starts with a header that indicates
the index (position in the HPT, which is just an array), the number of
valid entries starting at that index (may be zero), and the number of
invalid entries following those valid entries.  The valid entries, 16
bytes each, follow the header.  The invalid entries are not explicitly
represented.

This uses a bit in our record of the guest view of the HPTE to record
when the HPTE gets modified.  We use a reserved bit for this, and ensure
that this bit is always cleared in HPTE values returned to the guest.

The recording of modified HPTEs is only done if other code indicates
its interest by setting kvm->arch.hpte_mod_interest to a non-zero value.
The reason for this is that when later commits add facilities for
userspace to read the HPT, the first pass of reading the HPT will be
quicker if there are no (or very few) HPTEs marked as modified,
rather than having most HPTEs marked as modified.

reverse map entry, thus preserving the reference and change bits.
We were also unnecessarily setting the reference bit, and this
fixes that too.

This restructures the code that creates HPT (hashed page table)
entries so that it can be called in situations where we don't have a
struct vcpu pointer, only a struct kvm pointer.  It also fixes a bug
where kvmppc_map_vrma() would corrupt the guest R4 value.

Most of the work of kvmppc_virtmode_h_enter is now done by a new
function, kvmppc_virtmode_do_h_enter, which itself calls another new
function, kvmppc_do_h_enter, which contains most of the old
kvmppc_h_enter.  The new kvmppc_do_h_enter takes explicit arguments
for the place to return the HPTE index, the Linux page tables to use,
and whether it is being called in real mode, thus removing the need
for it to have the vcpu as an argument.

Currently kvmppc_map_vrma creates the VRMA (virtual real mode area)
HPTEs by calling kvmppc_virtmode_h_enter, which is designed primarily
to handle H_ENTER hcalls from the guest that need to pin a page of
memory.  Since H_ENTER returns the index of the created HPTE in R4,
kvmppc_virtmode_h_enter updates the guest R4, corrupting the guest R4
in the case when it gets called from kvmppc_map_vrma on the first
VCPU_RUN ioctl.  With this, kvmppc_map_vrma instead calls
kvmppc_virtmode_do_h_enter with the address of a dummy word as the
place to store the HPTE index, thus avoiding corrupting the guest R4.

Timur Tabi 6baf11906e powerpc/512x: don't compile any platform DIU code if the DIU is not enabled
If the DIU framebuffer driver is not enabled, then there's no point in
compiling any platform DIU code, because it will never be used.  Most of
the platform code was protected in the appropriate #ifdef, but not all.
This caused a break in some randconfig builds.

This is only a problem on the 512x platforms.  The P1022DS and MPC8610HPCD
platforms are already correct.

This patch reverts commit 12e36309f8 ("powerpc:
Option FB_FSL_DIU is not really optional for mpc512x") and restores the
ability to configure DIU support.

Grant Likely 499b42c3e4 powerpc: Fix fallout from device_node->name constification
Commit c22618a1, "drivers/of: Constify device_node->name and
->path_component_name" changes device_node name to a const value, but
the PowerPC scom code still assigns it to a non-void field in
debugfs_blob_wrapper. The /right/ solution might be to change the
debugfs_blob_wrapper->data to also be const, but that is a bit
risky. Instead, cast the value to (void*). It is a bit ugly, but it
is the safest change until it can be investigated where
debugfs_blob_wrapper can be modified.

Julius Werner a474a51549 cpuidle: Measure idle state durations with monotonic clock
Many cpuidle drivers measure their time spent in an idle state by
reading the wallclock time before and after idling and calculating the
difference. This leads to erroneous results when the wallclock time gets
updated by another processor in the meantime, adding that clock
adjustment to the idle state's time counter.

If the clock adjustment was negative, the result is even worse due to an
erroneous cast from int to unsigned long long of the last_residency
variable. The negative 32 bit integer will zero-extend and result in a
forward time jump of roughly four billion milliseconds or 1.3 hours on
the idle state residency counter.

This patch changes all affected cpuidle drivers to either use the
monotonic clock for their measurements or make use of the generic time
measurement wrapper in cpuidle.c, which was already working correctly.
Some superfluous CLIs/STIs in the ACPI code are removed (interrupts
should always already be disabled before entering the idle function, and
not get reenabled until the generic wrapper has performed its second
measurement). It also removes the erroneous cast, making sure that
negative residency values are applied correctly even though they should
not appear anymore.

Gavin Shan e716e01438 powerpc/eeh: Do not invalidate PE properly
While the EEH does recovery on the specific PE that has PCI errors,
the PCI devices belonging to the PE will be removed and the PE will
be marked as invalid since we still need the information stored in
the PE. We only invalidate the PE when it doesn't have associated
EEH devices and valid child PEs. However, the code used to check
that is wrong. The patch fixes that.

York Sun bc15236fbe powerpc/mpc85xx: Change spin table to cached memory
ePAPR v1.1 requires the spin table to be in cached memory. So we need
to change the call argument of ioremap to enable cache and coherence.
We also flush the cache after writing to spin table to keep it compatible
with previous cache-inhibit spin table. Flushing before and after
accessing spin table is recommended by ePAPR.

Function fsl_pcibios_fixup_bus() is available only if PCI is enabled.  The
MPC8610 HPCD platform file was not protecting the assigned with an #ifdef,

already has this #ifdef.

Frederic Weisbecker e3942ba040 vtime: Consolidate a bit the ctx switch code
On ia64 and powerpc, vtime context switch only consists
in flushing system and user pending time, plus a few
arch housekeeping.

Consolidate that into a generic implementation. s390 is
a special case because pending user and system time accounting
there is hard to dissociate. So it's keeping its own implementation.

Frederic Weisbecker fd25b4c2f2 vtime: Remove the underscore prefix invasion
Prepending irq-unsafe vtime APIs with underscores was actually
a bad idea as the result is a big mess in the API namespace that
is even waiting to be further extended. Also these helpers
are always called from irq safe callers except kvm. Just
provide a vtime_account_system_irqsafe() for this specific
case so that we can remove the underscore prefix on other
vtime functions.

The expressions tsk->nsproxy->pid_ns and task_active_pid_ns
aka ns_of_pid(task_pid(tsk)) should have the same number of
cache line misses with the practical difference that
ns_of_pid(task_pid(tsk)) is released later in a processes life.

Furthermore by using task_active_pid_ns it becomes trivial
to write an unshare implementation for the the pid namespace.

So I have used task_active_pid_ns everywhere I can.

In fork since the pid has not yet been attached to the
process I use ns_of_pid, to achieve the same effect.

fix up the users that try to modify it in-place. In one case
kmalloc+memcpy is replaced with kstrdup() to avoid modifying the string.

Since we don't know if they new kernel we are kexecing into has been
built to support relocation on exceptions, we disable them before we
kexec.

We do NOT disable them if we are execing a kdump kernel, because we
want to change as little state as possible and it is likely that we are
execing ourselves and will be able to handle them anyway.

Ian Munsie fc8effa4e4 powerpc: Enable relocation on during exceptions at boot
We currently do this synchronously at boot from setup_arch. On a large
system this could hypothetically take a little while to complete, so
currently we will give up if we are asked to wait for more than a second
in total.

If we actually start hitting that timeout in practice we can always move
this code into a kernel thread to take care of it in the background.
