The aacraid controller is a RAID controller and the driver will never see
any SCSI messages. Plus it's quite pointless to set the message byte if the
host byte is already set, as the latter takes precedence during error
recovery. Drop the message byte values for the final result.

Drivers should do only device-specific jobs. But in general, drivers using
legacy PCI PM framework for .suspend()/.resume() have to manage many PCI
PM-related tasks themselves which can be done by PCI Core itself. This
brings extra load on the driver and it directly calls PCI helper functions
to handle them.
Switch to the new generic framework by updating function signatures and
define a "struct dev_pm_ops" variable to bind PM callbacks. Also, remove
unnecessary calls to the PCI Helper functions along with the legacy
.suspend & .resume bindings.

The driver calls pci_enable_wake(...., false) in aac_resume(), and there is
no corresponding pci_enable_wake(...., true) in aac_suspend(). Either it
should do enable-wake the device in .suspend() or should not invoke
pci_enable_wake() at all.
Concluding that this is a bug and PCI core calls pci_enable_wake(pci_dev,
PCI_D0, false) during resume, drop it from aac_resume().

The use of compat_alloc_user_space() can be easily replaced by handling
compat arguments in the regular handler, and this will make it work for
big-endian kernels as well, which at the moment get an invalid indirect
pointer argument.
Calling aac_ioctl() instead of aac_compat_do_ioctl() means the compat and
native code paths behave the same way again, which they stopped when the
adapter health check was added only in the native function.

The method struct pci_error_handlers.error_detected() is defined and
documented as taking an 'enum pci_channel_state' for the second argument,
but most drivers use 'pci_channel_state_t' instead.
This 'pci_channel_state_t' is not a typedef for the enum but a typedef for
a bitwise type in order to have better/stricter typechecking.
Consolidate everything by using 'pci_channel_state_t' in the method's
definition, in the related helpers and in the drivers.
Enforce use of 'pci_channel_state_t' by replacing 'enum pci_channel_state'
with an anonymous 'enum'.
Note: Currently, from a typechecking point of view this patch changes
nothing because only the constants defined by the enum are bitwise, not the
enum itself (sparse doesn't have the notion of 'bitwise enum'). This may
change in some not too far future, hence the patch.

If 'scsi_host_alloc()' or 'kcalloc()' fail, 'error' is known to be 0. Set
it explicitly to -ENOMEM before branching to the error handling path.
While at it, remove 2 useless assignments to 'error'. These values are
overwridden a few lines later.

This is a batch of changes that didn't make it in the initial pull
request because the lpfc series had to be rebased to redo an incorrect
split. It's basically driver updates to lpfc, target, bnx2fc and ufs
with the rest being minor updates except the sr_block_release one
which fixes a use after free introduced by the removal of the global
mutex in the first patch set.

struct partition is the on-disk format of a MSDOS partition table entry.
Move it out of genhd.h into a new msdos_partition.h header and give it
a msdos_ prefix to avoid confusion.
Also move the magic number from block/partitions/msdos.h to the new
header so that it can be used by the SCSI drivers looking at the DOS
partition tables.

Since snprintf() returns the would-be-output size instead of the actual
output size, the succeeding calls may go beyond the given buffer limit.
Fix it by replacing with scnprintf().

Fixes the occasional adapter panic when sg_reset is issued with -d, -t, -b
and -H flags. Removal of command type HBA_IU_TYPE_SCSI_TM_REQ in
aac_hba_send since iu_type, request_id and fib_flags are not populated.
Device and target reset handlers are made to send TMF commands only when
reset_state is 0.

Use scsi_host_block() and scsi_host_unblock() instead of
scsi_block_requests()/scsi_unblock_requests() to block and unblock I/O.
This has the advantage that the block layer will stop sending I/O to the
adapter instead of having the SCSI midlayer requeueing I/O internally.

_aac_reset_adapter() only has one caller, and that one already calls
scsi_block_requests(). Move the calls out of _aac_reset_adapter() to avoid

After IOP reset completion, AIF request command is not issued to the
controller. Driver schedules a worker thread to issue a AIF request command

Currently driver waits for the command IOCTL from the firmware and if the
firmware enters nonresponsive state, the driver doesn't respond till the
firmware is responsive again.
Check that firmware is alive, otherwise return -EBUSY.
