The original bootmem allocator is getting replaced by memblock. To
cover the needs of the s390 kdump implementation the physical memory
list is used.
With this patch the bootmem allocator and its bitmaps are completely
removed from s390.

If the sigp interpretation facility is installed, most SIGP EXTERNAL CALL
operations will be interpreted instead of intercepted. A partial execution
interception will occurr at the sending cpu only if the target cpu is in the
wait state ("W" bit in the cpuflags set). Instruction interception will only
happen in error cases (e.g. cpu addr invalid).
As a sending cpu might set the external call interrupt pending flags at the
target cpu at every point in time, we can't handle this kind of interrupt using
our kvm interrupt injection mechanism. The injection will be done automatically
by the SIE when preparing the start of the target cpu.

The current trace definition doesn't work very well with the perf tool.
Perf shows a "insn_to_mnemonic not found" message. Let's handle the

This patch adds a new decoder of SIE intercepted instructions.
The decoder implemented as a macro and potentially can be used in
both kernelspace and userspace.
Note that this simplified instruction decoder is only intended to be
used with the subset of instructions that may cause a SIE intercept.

Rework the function kvm_arch_fault_in_sync() to become a proper helper
function for faulting-in a guest page. Now it takes the guest address as
a parameter and does not ignore the possible error code from gmap_fault()
anymore (which could cause undetected error conditions before).

If the new PSW for program interrupts is invalid, the VM ends up
in an endless loop of specification exceptions. Since there is not
much left we can do in this case, we should better drop to userspace
instead so that the crash can be reported to the user.

As a program status word is also invalid (and thus generates an
specification exception) if the instruction address is not even,
we should test this in is_valid_psw(), too. This patch also exports
the function so that it becomes available for other parts of the
S390 KVM code as well.

Use the mm semaphore to serialize multiple invocations of s390_enable_skey.
The second CPU faulting on a storage key operation needs to wait for the
completion of the page table update. Taking the mm semaphore writable
has the positive side-effect that it prevents any host faults from
taking place which does have implications on keys vs PGSTE.

bpf_alloc_binary() adds 128 bytes of room to JITed program image
and rounds it up to the nearest page size. If image size is close
to page size (like 4000), it is rounded to two pages:
round_up(4000 + 4 + 128) == 8192
then 'hole' is computed as 8192 - (4000 + 4) = 4188
If prandom_u32() % hole selects a number >= PAGE_SIZE - sizeof(*header)
then kernel will crash during bpf_jit_free():

An additional testcase found an issue with the last
series of patches applied: the fallback solution may
not save the iv value after operation. This very small
fix just makes sure the iv is copied back to the
walk/desc struct.

We replace the old way to configure the scheduler topology with a new method
which enables a platform to declare additionnal level (if needed).
We still have a default topology table definition that can be used by platform
that don't want more level than the SMT, MC, CPU and NUMA ones. This table can
be overwritten by an arch which either wants to add new level where a load
balance make sense like BOOK or powergating level or wants to change the flags
configuration of some levels.
For each level, we need a function pointer that returns cpumask for each cpu,
a function pointer that returns the flags for the level and a name. Only flags
that describe topology, can be set by an architecture. The current topology
flags are:
SD_SHARE_CPUPOWER
SD_SHARE_PKG_RESOURCES
SD_NUMA
SD_ASYM_PACKING
Then, each level must be a subset on the next one. The build sequence of the
sched_domain will take care of removing useless levels like those with 1 CPU
and those with the same CPU span and no more relevant information for
load balancing than its children.

The external interrupt interception can only occur in rare cases, e.g.
when the PSW of the interrupt handler has a bad value. The old handler
for this interception simply ignored these events (except for increasing
the exit_external_interrupt counter), but for proper operation we either
have to inject the interrupts manually or we should drop to userspace in
case of errors.

When copy_from_user() fails, this code returns the number of bytes
remaining instead of a negative error code. The positive number is
returned to the user but otherwise it is harmless.

This patch introduces two new functions to set/clear the CPUSTAT_STOPPED bit and
makes use of it at all applicable places. These functions prepare the additional
execution of code when starting/stopping a vcpu.

The s390 architecture has a special protection mechanism that can
be used to prevent write access to the vital data in the low-core
memory area. This patch adds a new helper function that can be used
to check for such write accesses and in case of protection, it also
sets up the exception data accordingly.

When the guest executes the MVPG instruction with DAT disabled,
and the source or destination page is not mapped in the host,
the so-called partial execution interception occurs. We need to
handle this event by setting up a mapping for the corresponding
user pages.

This merges the patch to fix possible loss of dirty bit on munmap() or
madvice(DONTNEED). If there are concurrent writers on other CPU's that
have the unmapped/unneeded page in their TLBs, their writes to the page
could possibly get lost if a third CPU raced with the TLB flush and did
a page_mkclean() before the page was fully written.
Admittedly, if you unmap() or madvice(DONTNEED) an area _while_ another
thread is still busy writing to it, you deserve all the lost writes you
could get. But we kernel people hold ourselves to higher quality
standards than "crazy people deserve to lose", because, well, we've seen
people do all kinds of crazy things.
So let's get it right, just because we can, and we don't have to worry
about it.
* safe-dirty-tlb-flush:
mm: split 'tlb_flush_mmu()' into tlb flushing and memory freeing parts
The mmu-gather operation 'tlb_flush_mmu()' has done two things: the
actual tlb flush operation, and the batched freeing of the pages that
the TLB entries pointed at.
This splits the operation into separate phases, so that the forced
batched flushing done by zap_pte_range() can now do the actual TLB flush
while still holding the page table lock, but delay the batched freeing
of all the pages to after the lock has been dropped.
This in turn allows us to avoid a race condition between
set_page_dirty() (as called by zap_pte_range() when it finds a dirty
shared memory pte) and page_mkclean(): because we now flush all the
dirty page data from the TLB's while holding the pte lock,
page_mkclean() will be held up walking the (recently cleaned) page
tables until after the TLB entries have been flushed from all CPU's.

caused random memory corruption on my s390 box. Turns out that the
last element of the ccwgroup structure is of dynamic size, so we
must move the newly introduced work structure _before_ the zero
length array.

When a guest is single-stepped, we want to disable timer interrupts. Otherwise,
the guest will continuously execute the external interrupt handler and make
debugging of code where timer interrupts are enabled almost impossible.
The delivery of timer interrupts can be enforced in such sections by setting a
breakpoint and continuing execution.
In order to disable timer interrupts, they are disabled in the control register
of the guest just before SIE entry and are suppressed in the interrupt
check/delivery methods.

This patch adds support to debug the guest using the PER facility on s390.
Single-stepping, hardware breakpoints and hardware watchpoints are supported. In
order to use the PER facility of the guest without it noticing it, the control
registers of the guest have to be patched and access to them has to be
intercepted(stctl, stctg, lctl, lctlg).
All PER program interrupts have to be intercepted and only the relevant PER
interrupts for the guest have to be given back. Special care has to be taken

host in the guests PER configuration is not fully transparent. PER instruction
nullification can not be used by the guest and too many storage alteration
events may be reported to the guest (if it is activated for special address
ranges only) when the host concurrently debugging it.

When a program interrupt was to be delivered until now, no program interrupt
parameters were stored in the low-core of the target vcpu.
This patch enables the delivery of those program interrupt parameters, takes
care of concurrent PER events which can be injected in addition to any program
interrupt and uses the correct instruction length code (depending on the
interception code) for the injection of program interrupts.

Whenever a program interrupt is intercepted, some parameters are stored in the
sie control block. These parameters have to be extracted in order to be
reinjected correctly. This patch also takes care of intercepted PER events which
can occurr in addition to any program interrupt.

fields, the PER code and the PER Addressing-and-Translation-Mode
Identification (ATMID)

Convert handle_tpi() to new guest access functions.
The code now sets up a structure which is copied with a single call to
guest space instead of issuing several separate guest access calls.
This is necessary since the to be copied data may cross a page boundary.
If a protection exception happens while accessing any of the pages, the
instruction is suppressed and may not have modified any memory contents.

The new guest memory access function write_guest() and read_guest() can be
used to access guest memory in an architecture compliant way.
These functions will look at the vcpu's PSW and select the correct address
space for memory access and also perform correct address wrap around.
In case DAT is turned on, page tables will be walked otherwise access will
happen to real or absolute memory.
Any access exception will be recognized and exception data will be stored
in the vcpu's kvm_vcpu_arch.pgm member. Subsequently an exception can be
injected if necessary.
Missing are:
- key protection checks
- access register mode support
- program event recording support
This patch also adds write_guest_real(), read_guest_real(),
write_guest_absolute() and read_guest_absolute() guest functions which can
be used to access real and absolute storage. These functions currently do
not perform any access checks, since there is no use case (yet?).

put_guest_lc, read_guest_lc and write_guest_lc are guest access
functions which shall only be used to access the lowcore of a vcpu.
These functions should be used for e.g. interrupt handlers where no
guest memory access protection facilities, like key or low address
protection, are applicable.
At a later point guest vcpu lowcore access should happen via pinned
prefix pages, so that these pages can be accessed directly via the
kernel mapping. All of these *_lc functions can be removed then.

Add a 'struct kvm_s390_pgm_info pgm' member to kvm_vcpu_arch. This
structure will be used if during instruction emulation in the context
of a vcpu exception data needs to be stored somewhere.
Also add a helper function kvm_s390_inject_prog_cond() which can inject
vcpu's last exception if needed.

Add kvm_s390_logical_to_effective() helper which converts a guest vcpu's
logical storage address to a guest vcpu effective address by applying the
rules of the vcpu's addressing mode defined by PSW bits 31 and 32
(extendended and basic addressing mode).
Depending on the vcpu's addressing mode the upper 40 bits (24 bit addressing
mode), 33 bits (31 bit addressing mode) or no bits (64 bit addressing mode)
will be zeroed and the remaining bits will be returned.

When userspace reset the guest without notifying kvm, the CMMA state
of the pages might be unused, resulting in guest data corruption.
To avoid this, CMMA must be enabled only if userspace understands
the implications.
CMMA must be enabled before vCPU creation. It can't be switched off
once enabled. All subsequently created vCPUs will be enabled for
CMMA according to the CMMA state of the VM.

For live migration kvm needs to test and clear the dirty bit of guest pages.
That for is ptep_test_and_clear_user_dirty, to be sure we are not racing with
other code, we protect the pte. This needs to be done within
the architecture memory management code.

Switch the user dirty bit detection used for migration from the hardware
provided host change-bit in the pgste to a fault based detection method.
This reduced the dependency of the host from the storage key to a point
where it becomes possible to enable the RCP bypass for KVM guests.
The fault based dirty detection will only indicate changes caused
by accesses via the guest address space. The hardware based method
can detect all changes, even those caused by I/O or accesses via the
kernel page table. The KVM/qemu code needs to take this into account.

Introduce a new function s390_enable_skey(), which enables storage key
handling via setting the use_skey flag in the mmu context.
This function is only useful within the context of kvm.
Note that enabling storage keys will cause a one-time hickup when
walking the page table; however, it saves us special effort for cases
like clear reset while making it possible for us to be architecture
conform.
s390_enable_skey() takes the page table lock to prevent reseting
storage keys triggered from multiple vcpus.

page_table_reset_pgste() already does a complete page table walk to
reset the pgste. Enhance it to initialize the storage keys to
PAGE_DEFAULT_KEY if requested by the caller. This will be used
for lazy storage key handling. Also provide an empty stub for
!CONFIG_PGSTE
Lets adopt the current code (diag 308) to not clear the keys.

As per the existing implementation; implement the new one using
smp_mb().
AFAICT the s390 compare-and-swap does imply a barrier, however there
are some immediate ops that seem to be singly-copy atomic and do not
imply a barrier. One such is the "ni" op (which would be
and-immediate) which is used for the constant clear_bit
implementation. Therefore s390 needs full barriers for the
{before,after} atomic ops.

The whole point of the out-of-line strnlen_user_srst() function was to
avoid corruption of register 0 due to register asm assignment.
However 'somebody' :) forgot to remove the update_primary_asce() function
call, which may clobber register 0 contents.
So let's remove that call and also move the size check to the calling
function.

smp_stop_cpu() should stop the current cpu even for !CONFIG_SMP.
Otherwise machine_halt() will return and and the machine generates a
panic instread of simply stopping the current cpu:

If the renamed symbol is defined lib/iomap.c implements ioport_map and
ioport_unmap and currently (nearly) all platforms define the port
accessor functions outb/inb and friend unconditionally. So
HAS_IOPORT_MAP is the better name for this.
Consequently NO_IOPORT is renamed to NO_IOPORT_MAP.
The motivation for this change is to reintroduce a symbol HAS_IOPORT
that signals if outb/int et al are available. I will address that at
least one merge window later though to keep surprises to a minimum and
catch new introductions of (HAS|NO)_IOPORT.

The main motivation behind this patch is to provide a way to disable THP
for jobs where the code cannot be modified, and using a malloc hook with
madvise is not an option (i.e. statically allocated data). This patch
allows us to do just that, without affecting other jobs running on the
system.
We need to do this sort of thing for jobs where THP hurts performance,
due to the possibility of increased remote memory accesses that can be
created by situations such as the following:
When you touch 1 byte of an untouched, contiguous 2MB chunk, a THP will
be handed out, and the THP will be stuck on whatever node the chunk was
originally referenced from. If many remote nodes need to do work on
that same chunk, they'll be making remote accesses.
With THP disabled, 4K pages can be handed out to separate nodes as
they're needed, greatly reducing the amount of remote accesses to
memory.
This patch is based on some of my work combined with some
suggestions/patches given by Oleg Nesterov. The main goal here is to
add a prctl switch to allow us to disable to THP on a per mm_struct
basis.

Revert commit 8e72033f2a and add in code to fix up any issues caused
by the revert.
The revert is necessary because hugepage_madvise would return -EINVAL
when VM_NOHUGEPAGE is set, which will break subsequent chunks of this
patch set.
Here's a snip of an e-mail from Gerald detailing the original purpose of
this code, and providing justification for the revert:
"The intent of commit 8e72033f2a was to guard against any future
programming errors that may result in an madvice(MADV_HUGEPAGE) on
guest mappings, which would crash the kernel.
Martin suggested adding the bit to arch/s390/mm/pgtable.c, if
8e72033f2a was to be reverted, because that check will also prevent
a kernel crash in the case described above, it will now send a
SIGSEGV instead.
This would now also allow to do the madvise on other parts, if
needed, so it is a more flexible approach. One could also say that
it would have been better to do it this way right from the
beginning..."

multi buffer instances. That is, you can now trace some functions

buffer (or any tracer for that matter) and have different function tracing

The current uaccess code uses a page table walk in some circumstances,
e.g. in case of the in atomic futex operations or if running on old
hardware which doesn't support the mvcos instruction.
However it turned out that the page table walk code does not correctly
lock page tables when accessing page table entries.
In other words: a different cpu may invalidate a page table entry while
the current cpu inspects the pte. This may lead to random data corruption.
Adding correct locking however isn't trivial for all uaccess operations.
Especially copy_in_user() is problematic since that requires to hold at
least two locks, but must be protected against ABBA deadlock when a
different cpu also performs a copy_in_user() operation.
So the solution is a different approach where we change address spaces:
User space runs in primary address mode, or access register mode within
vdso code, like it currently already does.
The kernel usually also runs in home space mode, however when accessing
user space the kernel switches to primary or secondary address mode if
the mvcos instruction is not available or if a compare-and-swap (futex)
instruction on a user space address is performed.
KVM however is special, since that requires the kernel to run in home
address space while implicitly accessing user space with the sie
instruction.
So we end up with:
User space:
- runs in primary or access register mode
- cr1 contains the user asce
- cr7 contains the user asce
- cr13 contains the kernel asce
Kernel space:
- runs in home space mode
- cr1 contains the user or kernel asce
-> the kernel asce is loaded when a uaccess requires primary or
secondary address mode
- cr7 contains the user or kernel asce, (changed with set_fs())
- cr13 contains the kernel asce
In case of uaccess the kernel changes to:
- primary space mode in case of a uaccess (copy_to_user) and uses
e.g. the mvcp instruction to access user space. However the kernel
will stay in home space mode if the mvcos instruction is available
- secondary space mode in case of futex atomic operations, so that the
instructions come from primary address space and data from secondary
space
In case of kvm the kernel runs in home space mode, but cr1 gets switched
to contain the gmap asce before the sie instruction gets executed. When
the sie instruction is finished cr1 will be switched back to contain the
user asce.
A context switch between two processes will always load the kernel asce
for the next process in cr1. So the first exit to user space is a bit
more expensive (one extra load control register instruction) than before,
however keeps the code rather simple.
In sum this means there is no need to perform any error prone page table
walks anymore when accessing user space.
The patch seems to be rather large, however it mainly removes the
the page table walk code and restores the previously deleted "standard"
uaccess code, with a couple of changes.
The uaccess without mvcos mode can be enforced with the "uaccess_primary"
kernel parameter.

The zEC12 machines introduced the local-clearing control for the IDTE
and IPTE instruction. If the control is set only the TLB of the local
CPU is cleared of entries, either all entries of a single address space
for IDTE, or the entry for a single page-table entry for IPTE.
Without the local-clearing control the TLB flush is broadcasted to all
CPUs in the configuration, which is expensive.
The reset of the bit mask of the CPUs that need flushing after a
non-local IDTE is tricky. As TLB entries for an address space remain
in the TLB even if the address space is detached a new bit field is
required to keep track of attached CPUs vs. CPUs in the need of a
flush. After a non-local flush with IDTE the bit-field of attached CPUs
is copied to the bit-field of CPUs in need of a flush. The ordering
of operations on cpu_attach_mask, attach_count and mm_cpumask(mm) is
such that an underindication in mm_cpumask(mm) is prevented but an
overindication in mm_cpumask(mm) is possible.

The principles of operations states that the CPU is allowed to create
TLB entries for an address space anytime while an ASCE is loaded to
the control register. This is true even if the CPU is running in the
kernel and the user address space is not (actively) accessed.
In theory this can affect two aspects of the TLB flush logic.
For full-mm flushes the ASCE of the dying process is still attached.
The approach to flush first with IDTE and then just free all page
tables can in theory lead to stale TLB entries. Use the batched
free of page tables for the full-mm flushes as well.
For operations that can have a stale ASCE in the control register,
e.g. a delayed update_user_asce in switch_mm, load the kernel ASCE
to prevent invalid TLBs from being created.

"PPC and ARM do not have much going on this time. Most of the cool
stuff, instead, is in s390 and (after a few releases) x86.
ARM has some caching fixes and PPC has transactional memory support in
guests. MIPS has some fixes, with more probably coming in 3.16 as
QEMU will soon get support for MIPS KVM.
For x86 there are optimizations for debug registers, which trigger on
some Windows games, and other important fixes for Windows guests. We
now expose to the guest Broadwell instruction set extensions and also
Intel MPX. There's also a fix/workaround for OS X guests, nested
virtualization features (preemption timer), and a couple kvmclock
refinements.
For s390, the main news is asynchronous page faults, together with
improvements to IRQs (floating irqs and adapter irqs) that speed up

We need to reset the usage state of the pages on kexec/kdump,
which use subcode 0 and 1. We will only do the cmma reset in
the kernel, everything else is done in userspace as before.

When reworking the bitops and atomic ops I missed that those instructions
that got atomic behaviour only perform a "specific-operand-serialization"
instead of a full "serialization".
The compare-and-swap instruction used before performs a full serialization
before and after the instruction is executed, which means it has full
memory barrier semantics.
In order to give the new bitops and atomic ops functions also full memory
barrier semantics add a "bcr 14,0" before and after each of those new
instructions which performs full serialization as well.
This restores memory barrier semantics for bitops and atomic ops functions
which return values, like e.g. atomic_add_return(), but not for functions
which do not return a value, like e.g. atomic_add().
This is consistent to other architectures and what common code requires.

Pull s390 compat wrapper rework from Heiko Carstens:
"S390 compat system call wrapper simplification work.
The intention of this work is to get rid of all hand written assembly
compat system call wrappers on s390, which perform proper sign or zero
extension, or pointer conversion of compat system call parameters.
Instead all of this should be done with C code eg by using Al's
COMPAT_SYSCALL_DEFINEx() macro.
Therefore all common code and s390 specific compat system calls have
been converted to the COMPAT_SYSCALL_DEFINEx() macro.
In order to generate correct code all compat system calls may only
have eg compat_ulong_t parameters, but no unsigned long parameters.
Those patches which change parameter types from unsigned long to
compat_ulong_t parameters are separate in this series, but shouldn't
cause any harm.
The only compat system calls which intentionally have 64 bit
parameters (preadv64 and pwritev64) in support of the x86/32 ABI
haven't been changed, but are now only available if an architecture
defines __ARCH_WANT_COMPAT_SYS_PREADV64/PWRITEV64.
System calls which do not have a compat variant but still need proper
zero extension on s390, like eg "long sys_brk(unsigned long brk)" will
get a proper wrapper function with the new s390 specific
COMPAT_SYSCALL_WRAPx() macro:

Given that the C file which contains all the COMPAT_SYSCALL_WRAP lines
includes both linux/syscall.h and linux/compat.h, it will generate
build errors, if the declaration of sys_brk() doesn't match, or if
there exists a non-matching compat_sys_brk() declaration.
In addition this will intentionally result in a link error if
somewhere else a compat_sys_brk() function exists, which probably
should have been used instead. Two more BUILD_BUG_ONs make sure the
size and type of each compat syscall parameter can be handled
correctly with the s390 specific macros.
I converted the compat system calls step by step to verify the
generated code is correct and matches the previous code. In fact it
did not always match, however that was always a bug in the hand
written asm code.
In result we get less code, less bugs, and much more sanity checking"

This patch adds a jited flag into sk_filter struct in order to indicate
whether a filter is currently jited or not. The size of sk_filter is
not being expanded as the 32 bit 'len' member allows upper bits to be
reused since a filter can currently only grow as large as BPF_MAXINSNS.
Therefore, there's enough room also for other in future needed flags to
reuse 'len' field if necessary. The jited flag also allows for having
alternative interpreter functions running as currently, we can only
detect jit compiled filters by testing fp->bpf_func to not equal the
address of sk_run_filter().

The packet hash can be considered a property of the packet, not just
on RX path.
This patch changes name of rxhash and l4_rxhash skbuff fields to be
hash and l4_hash respectively. This includes changing uses of the
field in the code which don't call the access functions.

When kvm_get_vcpu() returned NULL for the destination CPU in
__sigp_emergency() or __sigp_external_call(), the memory for the
"inti" structure was not released anymore. This patch fixes this
issue by moving the check for !dst_vcpu before the kzalloc() call.

We need BITS_TO_LONGS, not sizeof(long) to calculate
the correct size.
idle_mask is a bitmask, each bit representing the state
of a cpu. The desired outcome is an array of unsigned long
fields that can fit KVM_MAX_VCPUS bits. We should not use
sizeof(long) which returnes the size in bytes, but BITS_TO_LONGS

We allocate a page for the 2k sca, so lets use the space to improve
hit rate of some internal cpu caches. No need to change the freeing
of the page, as this will shift away the page offset bits anyway.

Add a new interface to register/deregister sources of adapter interrupts
identified by an unique id via the flic. Adapters may also be maskable
and carry a list of pinned pages.

The generic pci_enable_resources() does essentially the same thing as the
code in the s390 version of pcibios_enable_device().
There are differences, but I don't think any of them are a problem. The
generic code:
- Checks everything up to PCI_NUM_RESOURCES, not PCI_BAR_COUNT (6), so
we'll now check the ROM resource, IOV resources, and bridge windows.
- Checks for res->flags & IORESOURCE_UNSET. The s390 code never sets
IORESOURCE_UNSET, so this isn't a problem.
- Checks res->parent. The s390 pcibios_add_device() calls
pci_claim_resource() on all BARs (except ROM, IOV, and bridge windows)
so this isn't a problem either.

Limit the number of bits to the maximum number of cpus a machine
can have.
possible_cpu_mask typically will have more bits set than a machine
may physically have. This results in wasted memory during per-cpu
memory allocations, if the possible mask contains more cpus than
physically possible for a given configuration.

Since commit 7c470539c9
(s390/kvm: avoid automatic sie reentry) we will run through the C code
of KVM on host interrupts instead of just reentering the guest. This
will result in additional ucontrol exits (at least HZ per second). Let
handle a 0 intercept in the kernel and dont return to userspace,
even if in ucontrol mode.

The BUG_ON in kvm-s390.c is unreachable, as we get the vcpu per common code,
which itself does this from the private_data field of the file descriptor,
and there is no KVM_UNCREATE_VCPU.
The __{set,unset}_cpu_idle BUG_ONs are not triggerable because the vcpu
creation code already checks against KVM_MAX_VCPUS.

As the data parameter is not really used by any ftrace_dyn_arch_init,
remove that from ftrace_dyn_arch_init. This also removes the addr
local variable from ftrace_init which is now unused.
Note the documentation was imprecise as it did not suggest to set
(*data) to 0.

No architecture uses the "data" parameter in ftrace_dyn_arch_init() in any
way, it just sets the value to 0. And this is used as a return value
in the caller -- ftrace_init, which just checks the retval against
zero.
Note there is also "return 0" in every ftrace_dyn_arch_init. So it is
enough to check the retval and remove all the indirect sets of data on
all archs.

The whole compat exec domain code doesn't make any difference.
From the registered s390_exec_domain:
- exec domain name is only displayed in /proc/execdomains
- handler is unused
- pers_low and pers_high are only used internally to find this specific
exec domain otherwise the default exec domain will be used
- all other fields match the default exec domain
So let's get rid of this.

The cpu array size calculation uses the NR_CPUS config option, which
was recently increased from 64 to 256. With a value of 256, the cpu
array will no longer fit into one APPLDATA record and loading the
appldata_os module fails with the following error:
could not insert 'appldata_os': Cannot allocate memory
Use num_possible_cpus() instead of NR_CPUS. For z/VM, this will still
result in a value of 64. This is not true for LPAR, but the appldata
feature is not available for LPAR.

Enforce 32 bit types for all compat syscall argument types.
This way we can make sure that all arguments get correct sign
or zero extension. Otherwise incorrect code would be generated.
E.g. for a 'long' type the COMPAT_SYSCALL_DEFINE macro wouldn't
generate code that would cause sign extension of the passed in 32
bit user space parameter.
This can cause quite subtle bugs like e.g. the one that was fixed
with dfd948e32a "fs/compat: fix parameter handling for compat
readv/writev syscalls".

Some fs compat system calls have unsigned long parameters instead of
compat_ulong_t.
In order to allow the COMPAT_SYSCALL_DEFINE macro generate code that
performs proper zero and sign extension convert all 64 bit parameters
their corresponding 32 bit counterparts.
compat_sys_io_getevents() is a bit different: the non-compat version
has signed parameters for the "min_nr" and "nr" parameters while the
compat version has unsigned parameters.
So change this as well. For all practical purposes this shouldn't make
any difference (doesn't fix a real bug).
Also introduce a generic compat_aio_context_t type which can be used
everywhere.
The access_ok() check within compat_sys_io_getevents() got also removed
since the non-compat sys_io_getevents() should be able to handle
everything anyway.

Implement the new CCW_CMD_SET_IND_ADAPTER command and try to enable
adapter interrupts for every device on the first startup. If the host
does not support adapter interrupts, fall back to normal I/O interrupts.
virtio-ccw adapter interrupts use the same isc as normal I/O subchannels
and share a summary indicator for all devices sharing the same indicator
area.
Indicator bits for the individual virtqueues may be contained in the same
indicator area for different devices.

When SIGP SET_PREFIX is called with an illegal CPU id, it must return
the condition code 3 ("not operational") instead of 1. Also fixed the
order in which the checks are done - CC3 has a higher priority than CC1.
And while we're at it, this patch also get rid of the floating interrupt
lock here by using kvm_get_vcpu() to get the local_int struct of the
destination CPU.

Parameter conversion within the system call wrappers is only needed
for parameters which differ in size and have a size of eight bytes on
64 bit.
For system call parameters with a size of less than eight byte the
called system call itself will perform parameter conversion anyway.
So we can save the double conversion of e.g. int parameters.
The only types which need to be converted are therefore pointer and
(unsigned) long parameters.

Instead of explicitly changing compat system call parameters from e.g.
unsigned long to compat_ulong_t let the COMPAT_SYSCALL_WRAP macros
automatically detect (unsigned) long parameters and zero and sign
extend them automatically.
The resulting binary is completely identical.
In addition add a sys_[system call name] prototype for each system call
wrapper. This will cause compile errors if the prototype does not match
the prototype in include/linux/syscall.h.
Therefore we should now always get the correct zero and sign extension
of system call parameters. Pointers are handled like before.

The compat syscall wrappers for sync_file_range and fallocate merged 32 bit
parameters into 64 bit parameters. Therefore they did more than just the
usual zero and/or sign extension of system call parameters.
So convert these two wrappers to full s390 specific compat sytem calls.

Introduce a new compat_wrap.c file which contains the s390 specific compat
system call wrappers.
The s390 specific system call wrappers only perform sign, zero and pointer
conversion of system call arguments before actually calling the non-compat
system call.
Therefore introduce COMPAT_SYSCALL_WRAPx macros which generate C code that
is nearly identical to the assembly code. This has the advantage that the
compile will generate correct code, and we avoid the frequent copy-paste
errors seen in the compat_wrapper.S file.

If an architecture has futex_atomic_cmpxchg_inatomic() implemented and there
is no runtime check necessary, allow to skip the test within futex_init().
This allows to get rid of some code which would always give the same result,
and also allows the compiler to optimize a couple of if statements away.

Commit "kvm: Record the preemption status of vcpus using preempt notifiers"
caused a performance regression on s390. It turned out that in the case that
if a former sleeping cpu, that was woken up, this cpu is not a yield candidate
since it gave up the cpu voluntarily. To retain this candiate its preempted
flag is set during wakeup and interrupt delivery time.
Significant performance measurement work and code analysis to solve this
issue was provided by Mao Chuan Li and his team in Beijing.

The maximum usable buffer size of the s390 debug feature (when using
the sprintf_view) is 11 * sizeof(long) (1 pointer for the format
string + 10 arguments). When a larger buffer size is specified the
additional memory is unused and wasted per debug entry. So reducing
the buffer size to its maximum (or to the actual buffer size used)
will make more precious debug feature space usable.
For pci_msg, chsc_msg, and cio_crw we use the additional usable dbf
space to reduce the number of allocated pages.

This patch enables Collaborative Memory Management (CMM) for kvm
on s390. CMM allows the guest to inform the host about page usage
(see arch/s390/mm/cmm.c). The host uses this information to avoid
swapping in unused pages in the page fault handler. Further, a CPU
provided list of unused invalid pages is processed to reclaim swap
space of not yet accessed unused pages.

Git commit 050eef364a "[S390] fix tlb flushing vs. concurrent
/proc accesses" introduced the attach counter to avoid using the
mm_users value to decide between IPTE for every PTE and lazy TLB
flushing with IDTE. That fixed the problem with mm_users but it
introduced another subtle race, fortunately one that is very hard
to hit.
The background is the requirement of the architecture that a valid
PTE may not be changed while it can be used concurrently by another
cpu. The decision between IPTE and lazy TLB flushing needs to be
done while the PTE is still valid. Now if the virtual cpu is
temporarily stopped after the decision to use lazy TLB flushing but
before the invalid bit of the PTE has been set, another cpu can attach
the mm, find that flush_mm is set, do the IDTE, return to userspace,
and recreate a TLB that uses the PTE in question. When the first,
stopped cpu continues it will change the PTE while it is attached on
another cpu. The first cpu will do another IDTE shortly after the
modification of the PTE which makes the race window quite short.
To fix this race the CPU that wants to attach the address space of a
user space thread needs to wait for the end of the PTE modification.
The number of concurrent TLB flushers for an mm is tracked in the
upper 16 bits of the attach_count and finish_arch_post_lock_switch
is used to wait for the end of the flush operation if required.

The uaccesspt kernel parameter allows to enforce using the uaccess page
table walk variant. This is mainly for debugging purposes, so this mode
can also be enabled on machines which support the mvcos instruction.

The types 'size_t' and 'unsigned long' have been used randomly for the
uaccess functions. This looks rather confusing.
So let's change all functions to use unsigned long instead and get rid
of size_t in order to have a consistent interface.
The only exception is strncpy_from_user() which uses 'long' since it
may return a signed value (-EFAULT).

There are only two uaccess variants on s390 left: the version that is used
if the mvcos instruction is available, and the page table walk variant.
So there is no need for expensive indirect function calls.
By default the mvcos variant will be called. If the mvcos instruction is not
available it will call the page table walk variant.
For minimal performance impact the "if (mvcos_is_available)" is implemented
with a jump label, which will be a six byte nop on machines with mvcos.

For some unknown reason the indirect uaccess functions on s390 implement a
different parameter order than what is usual.
e.g.:

Drivers for ccw consoles use ccw_device_probe_console to receive
an initialized ccw device which is already enabled for interrupts.
After that the device driver does the initialization of its private
data. This can race with unsolicited interrupts which can happen
once the device is enabled for interrupts.
Split ccw_device_probe_console into ccw_device_create_console and
ccw_device_enable_console and reorder the initialization of the ccw
console drivers.
While at it mark these functions as __init.

ccw consoles are in use before they can be properly registered with
the driver core. For devices which are in use by a device driver we
rely on the ccw_device's pointer to the driver callbacks to be valid.
For ccw consoles this pointer is NULL until they are registered later
during boot and we dereferenced this pointer. This worked by
chance on 64 bit builds (cdev->drv was NULL but the optional callback
cdev->drv->path_event was also NULL by coincidence) and was unnoticed
until we received reports about boot failures on 31 bit systems.
Fix it by initializing the driver pointer for ccw consoles.

The boundary size for iommu_area_alloc() is currently set to a constant
value. This is wrong, we shouldn't use a constant value but rather the
return value of dma_get_seg_boundary(), since a device driver can override
the default.

driver-core now supports synchrnous self-deletion of attributes and
the asynchrnous removal mechanism is scheduled for removal. Use it
instead of device_schedule_callback().
* Conversions in arch/s390/pci/pci_sysfs.c and
drivers/s390/block/dcssblk.c are straightforward.
* drivers/s390/cio/ccwgroup.c is a bit more tricky because
ccwgroup_notifier() was (ab)using device_schedule_callback() to
purely obtain a process context to kick off ungroup operation which
may block from a notifier callback.
Rename ccwgroup_ungroup_callback() to ccwgroup_ungroup() and make it
take ccwgroup_device * instead. The new function is now called
directly from ccwgroup_ungroup_store().
ccwgroup_notifier() chain is updated to explicitly bounce through
ccwgroup_device->ungroup_work. This also removes possible failure
from memory pressure.
Only compile-tested.

The kernel currently crashes with a low-address-protection exception
if a user space process executes an instruction that tries to use the
linkage stack. Set the base-ASTE origin and the subspace-ASTE origin
of the dispatchable-unit-control-table to point to a dummy ASTE.
Set up control register 15 to point to an empty linkage stack with no
room left.
A user space process with a linkage stack instruction will still crash
but with a different exception which is correctly translated to a
segmentation fault instead of a kernel oops.

Dumps created by kdump or zfcpdump can contain invalid memory holes when
dumping z/VM systems that have memory pressure.
For example:

The memory detection function find_memory_chunks() issues tprot to
find valid memory chunks. In case of CMM it can happen that pages are
marked as unstable via set_page_unstable() in arch_free_page().
If z/VM has released that pages, tprot returns -EFAULT and indicates
a memory hole.
So fix this and switch off CMM in case of kdump or zfcpdump.

In s390 des and 3des ctr mode there is one preallocated page
used to speed up the en/decryption. This page is not protected
against concurrent usage and thus there is a potential of data
corruption with multiple threads.
The fix introduces locking/unlocking the ctr page and a slower
fallback solution at concurrency situations.

In s390 des and des3_ede cbc mode the iv value is not protected
against concurrency access and modifications from another running
en/decrypt operation which is using the very same tfm struct
instance. This fix copies the iv to the local stack before
the crypto operation and stores the value back when done.

The aes-ctr mode uses one preallocated page without any concurrency
protection. When multiple threads run aes-ctr encryption or decryption
this can lead to data corruption.
The patch introduces locking for the page and a fallback solution with
slower en/decryption performance in concurrency situations.

To enable pfault after live migration we need to expose pfault_token,
pfault_select and pfault_compare, as one reg registers to userspace.
So that qemu is able to transfer this between the source and the target.

This patch enables async page faults for s390 kvm guests.
It provides the userspace API to enable and disable_wait this feature.
The disable_wait will enforce that the feature is off by waiting on it.
Also it includes the diagnose code, called by the guest to enable async page faults.
The async page faults will use an already existing guest interface for this
purpose, as described in "CP Programming Services (SC24-6084)".

In the case of a fault, we will retry to exit sie64 but with gmap fault
indication for this thread set. This makes it possible to handle async
page faults.

Userspace can flood the kernel with interrupts as of now, so let's
limit the number of pending floating interrupts injected via either
the floating interrupt controller or the KVM_S390_INTERRUPT ioctl.
We can have up to 4*64k pending subchannels + 8 adapter interrupts,
as well as up to ASYNC_PF_PER_VCPU*KVM_MAX_VCPUS pfault done interrupts.
There are also sclp and machine checks. This gives us
(4*65536+8+64*64+1+1) = 266250 interrupts.

This patch adds a floating irq controller as a kvm_device.
It will be necessary for migration of floating interrupts as well
as for hardening the reset code by allowing user space to explicitly
remove all pending floating interrupts.

Change parameter types of s390's compat ipc syscall from unsigned long
to compat_ulong_t to enforce zero extension of these parameters.
This is not really a bug, since s390_ipc compat syscall is only a
wrapper to the generic compat_sys_ipc() syscall, which performs correct
zero and sign extension.
This was introduced with commit 56e41d3c5a ("merge compat sys_ipc
instances").

inject every kind of interrupt as defined in the z/Architecture. Add

With b8668fd0a7 "s390/uapi: change struct statfs[64] member types
to unsigned values" the size of a couple of struct statfs64 member got
incorrectly changed from 64 to 32 bit for 32 bit builds.
Fix this by changing the type of couple of struct statfs64 members from
unsigned long to unsigned long long.
The definition of struct compat_statfs64 was correct however.

If get_fs() == USER_DS we better test if current->mm is not zero before
walking page tables.
The page table walk code would try to lock mm->page_table_lock, however
if mm is zero this might crash.
Now it is arguably incorrect trying to access userspace if current->mm
is zero, however we have seen that and s390 would be the only architecture
which would crash in such a case.
So we better make the page table walk code a bit more robust and report
always a fault instead.

Seems that commit 210b160701
(KVM: s390: Removed SIE_INTERCEPT_UCONTROL) lost a hunk when we
reworked our patch queue to rework the async_fp code. We now
ignore faults on the sie instruction (guest accesses non-existing
memory) instead of sending a fault into the guest. This leads to
hang situations with the old virtio transport that checks for
descriptor memory after guest memory. Instead of bailing out this
code now goes wild...
Lets re-add the check.

For user space packet capturing libraries such as libpcap, there's
currently only one way to check which BPF extensions are supported
by the kernel, that is, commit aa1113d9f8 ("net: filter: return
-EINVAL if BPF_S_ANC* operation is not supported"). For querying all
extensions at once this might be rather inconvenient.
Therefore, this patch introduces a new option which can be used as
an argument for getsockopt(), and allows one to obtain information
about which BPF extensions are supported by the current kernel.
As David Miller suggests, we do not need to define any bits right
now and status quo can just return 0 in order to state that this
versions supports SKF_AD_PROTOCOL up to SKF_AD_PAY_OFFSET. Later
additions to BPF extensions need to add their bits to the
bpf_tell_extensions() function, as documented in the comment.

The s390 bpf jit compiler emits the signed divide instructions "dr" and "d"
for unsigned divisions.
This can cause problems: the dividend will be zero extended to a 64 bit value
and the divisor is the 32 bit signed value as specified A or X accumulator,
even though A and X are supposed to be treated as unsigned values.
The divide instrunctions will generate an exception if the result cannot be
expressed with a 32 bit signed value.
This is the case if e.g. the dividend is 0xffffffff and the divisor either 1
or also 0xffffffff (signed: -1).
To avoid all these issues simply use unsigned divide instructions.

This patch enables transactional execution for KVM guests
on s390 systems zec12 or later.
We rework the allocation of the page containing the sie_block
to also back the Interception Transaction Diagnostic Block.
If available the TE facilities will be enabled.
Setting bit 73 and 50 in vfacilities bitmask reveals the HW
facilities Transactional Memory and Constraint Transactional
Memory respectively to the KVM guest.
Furthermore, the patch restores the Program-Interruption TDB
from the Interception TDB in case a program interception has
occurred and the ITDB has a valid format.

The patch "s390/perf: add support for the CPU-Measurement Sampling
Facility" added a new instance of the __cpuinit macro usage.
We removed this a couple versions ago; we now want to remove
the compat no-op stubs. Introducing new users is not what
we want to see at this point in time, as it will break once
the stubs are gone.

PSW32_USER_BITS should define the primary address space for user space
instead of the home address space.
Symptom of this bug is that gdb doesn't work in compat mode.
The bug was introduced with e258d719ff "s390/uaccess: always run the kernel
in home space" and f26946d7ec "s390/compat: make psw32_user_bits a constant
value again".

A number of situations currently require the heavyweight smp_mb(),
even though there is no need to order prior stores against later
loads. Many architectures have much cheaper ways to handle these
situations, but the Linux kernel currently has no portable way
to make use of them.
This commit therefore supplies smp_load_acquire() and
smp_store_release() to remedy this situation. The new
smp_load_acquire() primitive orders the specified load against
any subsequent reads or writes, while the new smp_store_release()
primitive orders the specifed store against any prior reads or
writes. These primitives allow array-based circular FIFOs to be
implemented without an smp_mb(), and also allow a theoretical
hole in rcu_assign_pointer() to be closed at no additional
expense on most architectures.
In addition, the RCU experience transitioning from explicit
smp_read_barrier_depends() and smp_wmb() to rcu_dereference()
and rcu_assign_pointer(), respectively resulted in substantial
improvements in readability. It therefore seems likely that
replacing other explicit barriers with smp_load_acquire() and
smp_store_release() will provide similar benefits. It appears
that roughly half of the explicit barriers in core kernel code
might be so replaced.

When using the CLP interface to enable or disable a pci device a
valid function handle needs to be delivered. So far our assumption
was that we always have an up-to-date version of the function handle
(since it doesn't change when the device is in use). This assumption
is incorrect if the pci device is enabled or disabled outside of our
control. When we are notified about such a change we already receive
the new function handle. Just use it.

Replace remaining occurences (just as we did in crypto/) under arch/*/crypto/
that make use of memcmp() for comparing keys or authentication tags for
usage with crypto_memneq(). It can simply be used as a drop-in replacement

Since under z/VM we cannot have more than 64 cpus, make sure the
cpu_possible_mask does not contain more bits.
This avoids wasting memory for dynamic per-cpu allocations if
CONFIG_NR_CPUS is larger than 64.

Add the PERF_CPUM_SF_FULL_BLOCKS flag to process only sample-data-blocks that
have the block-full-indicator bit set. Sample-data-blocks that are partially
filled are discarded. Use this flag if the sampling buffer is likely to be
shared among perf events that use different sampling modes. In such
environments, flushing sample-data-blocks that are not completely filled, might
cause invalid-data-formats.
Setting PERF_CPUM_SF_FULL_BLOCKS prevents potentially invalid sampling data to
be processed but, in contrast, also discards valid samples in partially filled
sample-data-blocks. Note that sample-data-blocks might not become full for
small sampling frequencies or for workload that is scheduled for tiny intervals.
To sample with the PERF_CPUM_SF_FULL_BLOCKS flag, set the perf->attr.config1
to 0x0004. For example:

Also support the diagnostic-sampling function in addition to the basic-sampling
function. Diagnostic-sampling data entries contain hardware model specific
sampling data and additional programs are required to analyze the data.
To deliver diagnostic-sampling, as well, as basis-sampling data entries to user
space, introduce support for sampling "raw data". If this particular perf
sampling type (PERF_SAMPLE_RAW) is used, sampling data entries are copied
to user space. External programs can then analyze these data.

The host-program-parameter (hpp) value of basic sample-data-entries designates
a SIE control block that is set by the LPP instruction in sie64a().
Non-zero values indicate guest samples, a value of zero indicates a host sample.
For perf samples, host and guest samples are distinguished using particular
PERF_MISC_* flags. The perf layer calls perf_misc_flags() to set the flags
based on the pt_regs content. For each sample-data-entry, the cpum_sf PMU
creates a pt_regs structure with the sample-data information. An additional
flag structure is added to easily distinguish between host and guest samples.

The trailer entry contains a timestamp of the time when the sample-data-block
became full. The timestamp specifies a TOD (time-of-day) value in either the
STCK or STCKE format.
Provide a helper function to return the TOD value depending on the setting of
time format indicator.

Improve the sampling buffer allocation and add a function to reallocate and
increase the sampling buffer structure. The number of allocated buffer elements
(sample-data-blocks) are accounted. You can control the minimum and maximum
number these sample-data-blocks through the cpum_sfb_size kernel parameter.
The number hardware sample overflows (if any) are also accounted and stored
per perf event. During the PMU disable/enable calls, the accumulated overflow
counter is analyzed and, if necessary, the sampling buffer is dynamically
increased.

If we receive a notification that a pci function became unavailable we clean
up by removing the pci device. This can confuse the driver since the function
is already unaccessible. Improve this situation by setting an appropriate
error_state.

Initialization and scanning of the pci bus is omitted on older
machines without pci support or if pci=off was specified. Remember
the fact that we ran without pci support and prevent further bus

The cpum_cf (counter facility) PMU does not support sampling events.
With cpum_sf (sampling facility), a PMU for sampling CPU cycles is
available.
Make cpum_sf the "default" PMU for PERF_COUNT_HW_CPU_CYCLES sampling
events but use the more precise cpum_cf PMU for non-sampling events.

Introduce a perf PMU, "cpum_sf", to support the CPU-Measurement
Sampling Facility. You can control the sampling facility through
this perf PMU interfaces. Perf sampling events are created for
hardware samples.

Provide PMU event attributes for supported counters and export their symbolic
names to the sysfs "events" directory.
See the /sys/devices/cpum_cf/events/ directory for a list of available counters.
Note that you might require counter set authorizations for the LPAR to use them.

Extract and move the oprofile hwsampler data structures and interfaces to
the cpu_mf.h header file which contains common interface definitions
for the various CPU-measurement facilities. This change is necessary for
a new perf PMU.
Few interface names have been revised to fit to the latest CPU-measurement
facilities documentation. Also declare the data structures as __packed and
correct checkpatch findings.

Add SCLP console detect functions to encapsulate detection of SCLP console
capabilities, for example, VT220 support. Reuse the sclp_send/receive masks

Remove the embedded struct cpu from struct pcpu and replace it with a
pointer instead. The struct cpu now gets allocated when a new cpu gets
detected.
The size of the pcpu_devices array (NR_CPUS * sizeof(struct pcpu)) gets
reduced by nearly 120KB.

The user_enable_single_step() and user_disable_sindle_step() functions

If the per cpu ec_mask bit of the receiving cpu is already set there is
no need to send an ipi, since a different cpu has already sent an ipi
and the receiving cpu has not yet executed the external call ipi handler.

The diagnose 500 subcode 3 contains the 32 bit subchannel id in bits 32-63
(counting from the left). As for other I/O instructions, bits 0-31 should be
ignored and thus not be passed to kvm_io_bus_write_cookie().
This fixes a bug where the guest passed non-zero bits 0-31 which the
host tried to interpret, leading to ioeventfd notification failures.

When SIGP RESTART detected an illegal CPU address, there is no need to
drop to userspace, we can return CC3 to the guest directly instead.
Also renamed __sigp_restart() to sigp_check_callable() (since this
is a better description of what the function is really doing) and
moved a string specific to RESTART to the calling place instead, so
that this function gets usable by other SIGP orders, too.

and wall_to_monotonic. The old update_vsyscall function only

The code to use the ECTG instruction to calculate the cputime for the
current thread is currently used only for the per-thread CPU-clock
with the clockid -2 (PID=0, VIRT=1). Use the same code for the clockid
CLOCK_THREAD_CPUTIME_ID to speed up the more common clockid as well.

The access-list entry is supposed to have the fetch-only bit set, however
a reserved bit got set instead.
Userspace isn't able to write to the page anyway since the accessed page
has the read-only bit set. So this saves us only for bad surprises in the
future if the reserved bit gets used.

struct sclp_cpu_info contains entries only for 255 cpus, while the new
smp fallback sigp detection code will fill up to 256 entries.
Even though there is no machine available which has 256 cpus and where
in addition the fallback sigp cpu detection code will be used we better
fix this, to prevent out of bound accesses.

Some s390 crypto algorithms incorrectly use the crypto_tfm structure to
store private data. As the tfm can be shared among multiple threads, this
can result in data corruption.
This patch fixes aes-xts by moving the xts and pcc parameter blocks from
the tfm onto the stack (48 + 96 bytes).

The SIGP order STOP_AND_STORE_STATUS is defined to stop a CPU and store
its status. However, we only stored the status if the CPU was still
running, so make sure that the status is now also stored if the CPU was
already stopped. This fixes the problem that the CPU information was
not stored correctly in kdump files, rendering them unreadable.

The EQBS and SQBS instructions do not set CC3 for invalid channels, but
should throw an operation exception instead when not available. Thus they
should not be handled by the handle_io_inst() wrapper but drop to userspace
instead (which will then inject the operation exception).

The TPI handler currently uses vcpu->run->s.regs.crs[6] to get the current
value of CR6. I think this is wrong, because vcpu->run->s.regs.crs is
only updated when kvm_arch_vcpu_ioctl_run() drops back to userspace.
So let's change the TPI handler to use vcpu->arch.sie_block->gcr[6] instead.

"s390/mm,tlb: tlb flush on page table upgrade fixup" removed the
exception handler for the asce-type exception. This is incorrect
as the user-copy with MVCOS can cause asce-type exceptions in
the kernel if a user pointer is too large. Those need to be
handled with do_no_context to branch to the fixup in the
user-copy code.
The simplest fix for this problem is to call do_dat_exception for
asce-type excpetions, as there is no vma for the address the code
will handle the exception correctly.

"s390: Use direct ktime path for s390 clockevent device" makes use
of the CLOCK_EVT_FEAT_KTIME clockevent option to avoid the delta
calculation with ktime_get() in clockevents_program_event and the
get_tod_clock() in s390_next_event. This is based on the assumption
that the difference between the internal ktime and the hardware
clock is reflected in the wall_to_monotonic delta. But this is not
true, the ntp corrections are applied via changes to the tk->mult
multiplier and this is not reflected in wall_to_monotonic.
In theory this could be solved by using the raw monotonic clock
but it is simpler to switch back to the standard clock delta
calculation.

When translating a user space address, the address must be checked against
the ASCE limit of the process. If the address is larger than the maximum
address that is reachable with the ASCE, an ASCE type exception must be
generated.
The current code simply ignored the higher order bits. This resulted in an
address wrap around in user space instead of an exception in user space.

Always use the mvcl instruction to copy a page instead of mvpg or a
couple of mvc instructions.
Copying a huge page is 25% faster this way. Also bypass caches when
copying pages since only parts of a page will be used afterwards.
Especially when copying a huge page this would kick everything out
of the L1 and L2 data caches on a zEC12 machine.

Commit "s390: fix handling of runtime instrumentation psw bit" (5ebf250dab)
changed the behavior of setting the runtime instrumentation psw bit. This
commit restores the original logic:
1. When returning from the signal handler, the runtime instrumentation psw bit
is restored to its saved state.
2. If the runtime instrumentation psw bit is enabled during the signal handler,
it is always turned off when leaving the signal handler. The saved state
is restored as described in 1. That also implies that turning on runtime
instrumentation in the signal handler is only effective while running in the
signal context.

When the availability of a pci function has changed by means
outside of our control we receive an availability event.
Implement/improve the handling of these notifications.

Stop hiding scm_block's dependency to the eadm subchannel driver
(by using functions provided by the eadm subchannel instead of
wrappers provided by the scm bus).
This will help userspace recognizing module dependencies (e.g. for
building a ramdisk). As a side effect we can get rid of some code
reimplementing refcounting between those modules.

The new function calls the old ones. The sclp_event_mask_early() is removed
and replaced by one invocation of sclp_set_event_mask(0, 0).

The early SCLP driver code in sclp_cmd.c belongs to sclp_early.c
because it is independent from the 'normal' SCLP driver. So move
it to sclp_early.c

Currently we have hardcoded the HSA size to 32 MiB. With this patch the
HSA size is determined dynamically via SCLP in early.c.
