Felipe Balbi 743411b3f3 usb: musb: make all glue layer export struct musb_platform_ops
preparing to a big refactor on musb code. We need
to be able to compile in all glue layers (or at
least all ARM-based ones) together and have a
working binary.

While preparing for that, we move every glue
layer to export only one symbol, which is
a struct musb_platform_ops, and make all
other functions static.

Later patches will come to allow for compiling
all glue layers together and have a working
binary.

Felipe Balbi 3ca8abb845 usb: musb: introduce struct musb_platform_ops
This will be passed to musb_core by platform glue
layer in order to make it easier to compile support
for several HW glue layers.

Later patches will come using this structure and
also moving HW glue layers to its own platform
driver; the idea is to be able to handle platform
peculiarities in a manner which doesn't affect one
another.

Hema Kalliguddi fcf173e451 usb: musb: add names for IRQs in structure resource
Soon resource data will get automatically
populated from a set of autogenerated data
from TI's hardware database for the OMAP
platform.

Such database, might not have resources at
the expected order by the current drivers.

While we could hack in some exceptions to
that tool to generate resources in a specific
order, it seems less fragile to use the
resource name instead. That way, no matter
what order the resources are generated, the
driver still work.

Modified the OMAP, Blackfin and Davinci
architecture files to add the name of the IRQs
in the resource structures and musb driver to
use the platform_get_irq_byname() api to get
the device and dma irq numbers instead of using
the index.

Anand Gadiyar 07a8cdd2bb usb: musb: do not use dma for control transfers
The Inventra DMA engine used with the MUSB controller in many
SoCs cannot use DMA for control transfers on EP0, but can use
DMA for all other transfers.

The USB core maps urbs for DMA if hcd->self.uses_dma is true.
(hcd->self.uses_dma is true for MUSB as well).

Split the uses_dma flag into two - one that says if the
controller needs to use PIO for control transfers, and
another which says if the controller uses DMA (for all
other transfers).

Also, populate this flag for all MUSB by default.

(Tested on OMAP3 and OMAP4 boards, with EHCI and MUSB HCDs
simultaneously in use).

Ming Lei e75df37165 usb: musb: clear RXCSR_AUTOCLEAR before PIO read
If RXCSR_AUTOCLEAR flag is not cleard before PIO reading, one packet
may be recieved by musb fifo, but no chance to notify
software, so cause packet loss, follows the detailed process:

	- PIO read one packet
	- musb fifo auto clear the MUSB_RXCSR_RXPKTRDY
	- musb continue to recieve the next packet, and MUSB_RXCSR_RXPKTRDY
	is set
	- software clear the MUSB_RXCSR_RXPKTRDY, so there is no chance for
	musb to notify software that the 2nd recieved packet.

The patch does fix the g_ether issue below:

	- use fifo_mode 3 to enable double buffer
	- 'ping -s 1024 IP_OF_BEAGLE_XM'
	- one usb packet of 512 byte is lost, so ping failed,
	which can be observed by wireshark

note:
	Beagle xm takes musb rtl1.8 and may fallback to pio mode
	for unaligned buffer.

Hema Kalliguddi 92d2711f5d usb: musb: unmap dma buffer when switching to PIO
Buffer is mapped to dma when dma channel is
allocated. If, for some reason, dma channel
programming fails, musb code will fallback
to PIO mode to transfer that request. In
that case, we need to unmap the buffer
back to CPU.

MUSB RTL1.8 and above cannot handle buffers
which are not 32bit aligned. That happens to
every request sent by g_ether gadget
driver. Since the buffer sent was unaligned,
we need to fallback to PIO.

Because of that, g_ether was failing due
to missing buffer unmapping.

With this patch and [1] g_ether works fine
with all MUSB revisions.

Verified with OMAP3630 board, which has
MUSB RTL1.8 using g_ether and g_zero.

Anand Gadiyar 6e16edfe62 usb: musb: fail unaligned DMA transfers on v1.8 and above
The Inventra DMA engine in version 1.8 and later of the MUSB
controller cannot handle DMA addresses that are not aligned
to a 4 byte boundary. It ends up ignoring the last two bits
programmed in the DMA_ADDR register. This is a deliberate
design change in the controller and is documented in the
programming guide.

Earlier versions of the controller could handle these
accesses just fine.

Fail dma_channel_program if we see an unaligned address when
using the newer controllers, so that the caller can carry out
the transfer using PIO mode.
(Current callers already have this backup path in place).

Then while suspending, the gpio_vrsel will change from high to low which

Ming Lei 31c9909b51 USB: musb: gadget: fix MUSB_TXMAXP and MUSB_RXMAXP configuration
Commit 9f445cb29918dc488b7a9a92ef018599cce33df7[USB: musb: disable
double buffering for older RTL versions] tries to disable double
buffer mode by writing endpoint hw max packet size to TXMAP/RXMAP.

First the approach can break full speed and cause overflow problems.
We should always set those registers with the actual max packet size
from endpoint descriptor.

Second, the problem describe by commit 9f445cb299
was caused by musb gadget driver; nothing to do with RTL revision as
originaly suspected.

The real fix to the problem is to always use actual max packet
size from endpoint descriptor to config TXMAP/RXMAP registers.

Grazvydas Ignotas 120d074c58 usb: musb: don't leave PHY enabled on shutdown()
Some actions like musb_platform_exit are only performed on module
removal and not on shutdown, which results in PHY being left enabled
on reboot at least. This is sometimes causing strange failures after
reboot (observed on OMAP3 pandora board), when DEVCTL does not report
VBUS state correctly due to unknown reasons (possibly because of
communication issues between musb IP and PHY). Running
musb_platform_exit before reset seems to resolve that issue.

Move some exit code from musb_remove() to musb_shutdown() so that it
is performed on both module removal and shutdown/reset. Also convert
the host check so that it doesn't need #ifdef.

Ming Lei 9001d80df9 usb: musb: gadget: fix dma mode 0 in double buffer Rx case
1, In Rx double buffer case, FIFO may have two packets, so
rxstate should be called to unload fifo if RXPKTRDY is set
even the current request has not been completed.

2, Commit 633ba7876b96ec339ef685357e2f7c60b5a8ce85
introduces autoclear to support double buffer in dma mode 0,
so remove clearing RXPKTRDY manually for dma mode 0.

3, Commit c7af6b29ffeffbeb28caf39e5b2ce29b11807c7d may break
dma mode 1 for non-doublebuffer endpoint, fix it.

With this patch, either usbtest #5 or g_file_storage(writing
file to device in usb host) or g_ether have been tested OK in
double buffer case(using fifo mode 3). Also, this patch has
been verified that single buffer case can't be broken.

Ajay Kumar Gupta eb83092c2b USB: musb: add musb support for AM35x
AM35x has musb interface and uses CPPI4.1 DMA engine.
Current patch supports only PIO mode. DMA support can be
added later once basic CPPI4.1 DMA patch is accepted.

Also added USB_MUSB_AM35X which is required to differentiate musb ips
between OMAP3x and AM35x. This config would be used to for below
purposes,
        - Select am35x.c instead of omap2430.c for compilation
          at drivers/usb/musb directory. Please note there are
          significant differneces in these two files as musb ip
          in quite different on AM35x.

	  Please note that in multi omap configuration only omap2430.c
	  file will get compiled and we would require to select only
	  AM35x based board config to compile am35x.c

        - Select workaround codes applicable for AM35x musb issues.
          one such workaround is for bytewise read issue on AM35x.

Sergei Shtylyov f405387435 USB: MUSB: fix kernel WARNING/oops when unloading module in OTG mode
Since commit 461972d8a4 (musb_core: don't call
musb_platform_exit() twice), unloading the driver module results in a WARNING
"kobject: '(null)' (c73de788): is not initialized, yet kobject_put() is being
called." (or even kernel oops) on e.g. DaVincis, though only in the OTG mode.
There exists dubious and unbalanced put_device() call in musb_free() which
takes place only in the OTG mode.  As this commit caused musb_platform_exit()
to be called (and so unregister the NOP transceiver) before this put_device()
call, this function references already freed memory.

On the other hand, all the glue layers miss the otg_put_transceiver() call,
complementary to the otg_get_transceiver() call that they do.  So, I think
the solution is to get rid of the strange put_device() call, and instead
call otg_put_transceiver() in the glue layers...

Felipe Balbi e0c43476c1 usb: musb: gadget: only enable AUTOCLEAR in double buffered case
commit 633ba7876b96ec339ef685357e2f7c60b5a8ce85 broke
g_file_storage functionality by enabling AUTOCLEAR on
all cases without caring for all gadget drivers.

This patch will only enable AUTOCLEAR if our endpoint's
FIFO was configured with double buffering support. Note
this is not a complete fix, double buffered case still
doesn't work always, but that hasn't been working for
quite some time. Other than reverting the entire commit
and breaking testusb with double buffered case again,
I decided it was better to fix the single buffered case
and spend more time fixing double buffered case properly.

Maulik Mankad 496dda704b usb: musb: host: unmap the buffer for PIO data transfers
The USB stack maps the buffer for DMA if the controller supports DMA.
MUSB controller can perform DMA as well as PIO transfers.
The buffer needs to be unmapped before CPU can perform
PIO data transfers.

Export unmap_urb_for_dma() so that drivers can perform
the DMA unmapping in a sane way.

Sergei Shtylyov d613746d8b USB: musb: make DBG() calls actually depend on CONFIG_USB_MUSB_DEBUG
Enabling CONFIG_USB_MUSB_DEBUG option causes -DDEBUG to be added to gcc's
command line, however the DBG() macro doesn't depend on DEBUG, so that the
debugging messages get printed regardless of the option, and I don't think
that this was intended. Get rid of otherwise unused xprintk() macro and make
DBG() macro directly call pr_debug() which only results in the actual code
generated if DEBUG is defined.

This change makes musb_hdrc.o ~30% less in size with CONFIG_USB_MUSB_DEBUG
disabled (in host mode).

To accomplish this the function to register a gadget driver takes the bind
function as a second argument.  To make things clearer rename the function
to resemble platform_driver_probe.

This fixes many section mismatches like

It was found that CDB field of CBW packet was set to 0x0. This was
due to missing a barrier before DMA engine starts transfer.
This  buffer is  allocated using dma_alloc_coherent which gives
non-cacheble but bufferable memory and hence needed a write
memory barrier to flush the write buffer.

Ming Lei 490e5fbe8c usb: musb: gadget: enable autoclear for OUT transfer in both DMA 0 and DMA 1
This patch fixes one bugs of OUT transfer in double buffer case:

	-the current code only enable autoclear for dma mode 1, and not
	for dma mode 0

Without this patch, test #5 of usbtest can't be passed if we
configure musb as g_zero and use fifo mode 3 to enable double
buffer mode.

With this patch and the following patch(fix dma length),
on my beagle B5, test#5(queued bulk out) may go beyond
18Mbyte/s(seems dma mode 0 is quicker in double buffer case)
if musb is configured as g_zero and fifo mode 3 is taken, follows
the test command:

    #./testusb -D DEV_NAME -c 1024 -t 5 -s 32768 -g 8   [1]

Also I have tested this patch can't make g_ether broken.

Ming Lei eeb1b2a4a9 usb: musb: gadget: fix bulk IN infinit hangs in double buffer case
This patch fixes one infinite hang of bulk IN transfer in double buffer
case, the hang can be observed easily by test #6 of usbtest if musb is
configured as g_zero and fifo mode 3 is taken to enable double fifo.

In fact, the patch only removes the check for non-empty fifo before
loading data from new request into fifo since the check is not correct:

	-in double buffer case, fifo may accommodate more than one packet,
	even though it has contained one packet already and is non-empty

	-since last DMA is completed before calling musb_g_tx, it is sure
	that fifo may accommodate at least one packet

Without applying the patch, new requst enqueued from .complte may not
have a chance to be loaded into fifo, then will never be completed and
cause infinite hangs.

With the patch, on my beagle B5, test#6(queued bulk in) can be passed and
test result may go beyond 33Mbyte/s if musb is configured as g_zero and
fifo mode 3 is taken, follows the test command:

	#testusb -D DEV_NAME -c 1024 -t 6 -s 32768 -g 8   [1]

For shared fifo hw endpoint(with FIFO_TXRX style), only ep_in
field of musb_hw_ep is intialized in musb_g_init_endpoints, and
ep_out is not initialized, but musb_g_rx and rxstate may access
ep_out field of musb_hw_ep by the method below:

	musb_ep = &musb->endpoints[epnum].ep_out

which can cause the kernel panic[1] below, this patch fixes the issue
by getting 'musb_ep' from '&musb->endpoints[epnum].ep_in' for shared fifo

Alan Stern 541c7d432f USB: convert usb_hcd bitfields into atomic flags
This patch (as1393) converts several of the single-bit fields in
struct usb_hcd to atomic flags.  This is for safety's sake; not all
CPUs can update bitfield values atomically, and these flags are used
in multiple contexts.

The flag fields that are set only during registration or removal can
remain as they are, since non-atomic accesses at those times will not
cause any problems.

(Strictly speaking, the authorized_default flag should become atomic
as well.  I didn't bother with it because it gets changed only via
sysfs.  It can be done later, if anyone wants.)

Hema HK c0f1f8e38f USB: musb: Enable the maximum supported burst mode for DMA
Setting MUSB Burst Mode 3 automatically enables support for
lower burst modes (BURST4, BURST8, BURST16 or bursts of unspecified
length). There is no need to set these burst modes based on the
packet size. Also enable the burst mode for both mode1 and mode0.

This is a fix for buggy hardware - having the lower burst modes
enabled can potentially cause lockups of the DMA engine used in
OMAP2/3/4 chips.

OMAP4 support.

Alan Stern 749da5f82f USB: straighten out port feature vs. port status usage
This patch (as1349b) clears up the confusion in many USB host
controller drivers between port features and port statuses.  In mosty
cases it's true that the status bit is in the position given by the
corresponding feature value, but that's not always true and it's not
guaranteed in the USB spec.

There's no functional change, just replacing expressions of the form
(1 << USB_PORT_FEAT_x) with USB_PORT_STAT_x, which has the same value.

Eric Lescouet 27729aadd3 USB: make hcd.h public (drivers dependency)
The usbcore headers: hcd.h and hub.h are shared between usbcore,
HCDs and a couple of other drivers (e.g. USBIP modules).
So, it makes sense to move them into a more public location and
to cleanup dependency of those modules on kernel internal headers.
This patch moves hcd.h from drivers/usb/core into include/linux/usb/

Tony Lindgren 1d0f11b397 usb: Fix tusb6010 for DMA API
Commit 18eabe2347 introduced
DMA buffer ownership. Fix tusb6010 accordingly. To compile,
also dummy musb_platform_save and restore functions need to
be added.

Also change the order of musb_read_fifo() to happen after
dma_cache_maint to have the DMA operations completed before
moving the remaining unaligned bytes with PIO. The DMA
access and PIO touch different areas of the FIFO, so this
change only makes the code a bit easier to follow.

Tested on n810 and g_ether with variable size ping test.
The test seems to fail for some ping sizes, but that seems to
be a different problem.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

The USB PHY on current Blackfin processors is a UTMI+ level 2 PHY.
However, it has no ULPI support - so there are no registers at all.
That means accesses to ULPI_BUSCONTROL have to be abstracted away
like other MUSB registers.

This fixes building for Blackfin parts again.

All current Blackfin parts are using RTL v1.9, but they don't expose the
hardware registers to probe this dynamically.  So hardcode the version to
v1.9 for now.

Need to move the local hwvers related defines higher up in the header so
that sub-musb headers may utilize them.

Swaminathan S 565969237a usb: musb: Fix cppi_channel_abort() function to handle Tx abort correctly
This patch fixes the Tx abort/teardown logic. We now wait for the teardown
completion interrupt and acknowledge the same by setting the tx_complete
register to 0.

This change is needed to ensure that abort processing works on DM365 platform.
Without this change after completion of abort processing the system is
overwhelmed with continuous stream of abort interrupts.

This change has been tested on all CPPI3.x platforms (DM644x, DM646x, DM35x,
DM36x).

Swaminathan S 5274dab6cb usb: musb: workaround toggle bug when doing bulk transfer after isoc
This patch implements the work around for a Mentor controller related
bug where it's observed a BULK Tx toggle error on the bus when a
BULK IO gets scheduled on an endpoint that was earlier used for
handling ISOC transaction and needed to start on 1 toggle.  When such
a situation arises even if the TXCSR toggle bits are programmed
correctly by the musb driver the data gets transmitted with 0 toggle
which leads to toggle error on the bus and the BULK transaction fails.
In case of MSC write, the device gets reset by the Host.

This Mentor bug is observed on almost all Mentor versions (1.3, 1.5,
1.8).  Confirmed on DM644x, DM355, DM365, OMAPL13x platforms.

Anand Gadiyar f933a0c0fe usb: musb: workaround MUSB DMA_INTR sometimes reads zero
MUSB DMA_INTR register may sometimes read zero when infact there
was a pending interrupt. Workaround this by reading the DMA_COUNT
values for all enabled channels when this condition occurs.
Flag these channels as the ones needing to be serviced.

Additionally, the absence of a debug print meant we would never
catch a spurious DMA interrupt in MUSB. So this patch adds a
debug print in the IRQ handler.

Ajay Kumar Gupta 3b151526ed USB: musb: Add new fifo table for a OMAP3 errata
We have observed MSC data read corruption when USB LAN device is
also connected and it's interface is up.

Silicon team has confirmed an errata where in all the active
transfers should use FIFO space either in first 8K or next 8K.
So far we have observed the issue in above use case scenario.

As a workaround to it, adding a new FIFO config (5) fitting well
within first 8K which can be used for such use cases.

Swaminathan S 93aa3dab00 USB: musb: fix for crash in DM646x USB when (CPPI)DMA is enabled
Race condition exists between the cppi_interrupt handler and
davinci_interrupt handler w.r.t completing a TX IO.  Since DM646x
has seperate DMA and USB endpoint interrupts cppi_interrupt handler
needs to hold the lock while operating on the endpoint.

Update over previous patch to avoid taking the lock if already
taken.  Tested on DM644x, DM355 and DM646x platforms.

state machine to "idle, waiting for SETUP" phase prematurely."

So, the majority of the cases of unhandled IRQs is still unfixed...

Cliff Cai 6bd03e7b9d USB: musb: clear the Blackfin interrupt pending bits early in the ISR
If we clear the interrupt pending bits at the end, we sometimes return too
fast and have the same interrupt assert itself.  There is no way in a
Blackfin system to force a sync of this state, so the hardware manual
instructs people to clear interrupt flags early in their ISR.

Daniel Glöckner 8d6499e5bd USB: musb: Fix CPPI IRQs not being signaled
On tx channel abort a cppi interrupt is generated for a short time by
setting the lowest bit of the TCPPICOMPPTR register. It is then reset
immediately by clearing the bit. When the interrupt handler is run,
it does not detect an interrupt in the TCPPIMSKSR or RCPPIMSKSR
registers and thus exits early without writing the TCPPIEOIR register.
It appears that this inhibits further cppi interrupts until the handler
is called by chance, f.ex. from davinci_interrupt().

By moving the unmasking of the interrupt below the writes to
TCPPICOMPPTR, no interrupt is generated and no write to TCPPIEOIR is
necessary.

Daniel Glöckner 5542bc2ac7 USB: musb: respect usb_request->zero in control requests
In gadget mode the answer to a control request should be followed by
a zero-length packet if the amount transferred is an exact multiple of
the endpoint's packet size and the requests has its "zero" flag set.

This patch prevents the request from being immediately removed from the
queue when a control IN transfer ends on a full packet and "zero" is set.
The next time ep0_txstate is entered, a zero-length packet is queued and
the request is removed as fifo_count is 0.

Sergei Shtylyov cea83241b3 USB: musb_gadget: fix STALL handling
The driver incorrectly cancels the mass-storage device CSW request
(which leads to device reset) due to giving back URB at the head of
endpoint's queue after sending each STALL handshake; stop doing that
and start checking for the queue being non-empty before stalling an
endpoint and disallowing stall in such case in musb_gadget_set_halt()
like the other gadget drivers do.

Moreover, the driver starts Rx request despite of the endpoint being
halted -- fix this by moving the SendStall bit check from musb_g_rx()
to rxstate().  And we also sometimes get into rxstate() with DMA still
active after clearing an endpoint's halt (not clear why), so bail out
in this case, similarly to what txstate() does...

While at it, also do the following changes :

- in musb_gadget_set_halt(), remove pointless Tx FIFO flushing (the
  driver does not allow stalling with non-empty Tx FIFO anyway);

- in rxstate(), stop pointlessly zeroing the 'csr' variable;

- in musb_gadget_set_halt(), move the 'done' label to a more proper
  place;

- in musb_g_rx(), eliminate the 'done' label completely...

INDEX register has to be set to '0' before reading
CONFIGDATA register which is only present in TI musb
platforms.

Currently the default register access mode is set to
FLAT_MODE thus INDEX register is not getting set
properly with musb_ep_select() which is just a nop
operation in FLAT_MODE.This invalid register read is
causing module reinset failure.

Fixing the issue by moving INDEX register write part to
musb_read_configdata() function itself.

David Brownell 89368d3d11 USB: musb: silence "suspend as a_wait_vrise is_active" msgs
Get rid of some obnoxious and inappropriate messaging, mostly on
DaVinci, when usbcore tries to autosuspend a root hub if just a
mini/micro-A connector is connected.  Symptom: endless stream of
messages reading like:

 musb_bus_suspend 2221: trying to suspend as a_wait_vrise is_active=1

Improve that musb bus suspend primitive a bit.  Take advantage of
this call to update the OTG state machine if appropriate, moving
the device out of the A_WAIT_VRISE state.  There's basically no
timer for that state transition just now, except with tusb6010;
that can make trouble.

Kevin Hilman 30899ca7f2 USB: musb: davinci dm6446evm GPIO renumbering
Numbering for GPIOs on the pcf857x chips on the dm644x EVM board
changed when DaVinci chips with more GPIOs were supported.  Update
the GPIO number used for nVBUS_DRV.

Longer term, we need a better abstraction of board-specific setup in
this code so we're not hard-coding board specific GPIOs into the
driver, but for now this at least gets it back to working with
mainline davinci core code.

Ajay Kumar Gupta a483d7068f musb: add high bandwidth ISO support
Tested on OMAP3 host side with Creative (Live! Cam Optia) USB camera
which uses high bandwidth isochronous IN endpoints.  FIFO mode 4 is
updated to provide the needed 4K endpoint buffer without breaking
the g_nokia composite gadget configuration.  (This is the only
gadget driver known to use enough endpoints to notice the change.)

Anand Gadiyar d1043a2697 musb: use dma mode 1 for TX if transfer size equals maxpacket (v2)
Currently, with Inventra DMA, we use Mode 0 if transfer size is less
than or equal to the endpoint's maxpacket size.  This requires that
we explicitly set TXPKTRDY for that transfer.

However the musb_g_tx code will not set TXPKTRDY twice if the last
transfer is exactly equal to maxpacket, even if request->zero is set.
Using Mode 1 will solve this; a better fix might be in musb_g_tx().

Without this change, musb will not correctly send out a ZLP if the
last transfer is the maxpacket size and request->zero is set.

David Brownell ab983f2a1b musb: support disconnect after HNP roleswitch
Adjust HNP state machines in MUSB driver so that they handle the
case where the cable is disconnected.  The A-side machine was
very wrong (unrecoverable); the B-Side was much less so.

 - A_PERIPHERAL ... as usual, the non-observability of the ID
   pin through Mentor's registers makes trouble.  We can't go
   directly to A_WAIT_VFALL to end the session and start the
   disconnect processing.  We can however sense link suspending,
   go to A_WAIT_BCON, and from there use OTG timeouts to finally
   trigger that A_WAIT_VFALL transition.  (Hoping that nobody
   reconnects quickly to that port and notices the wrong state.)

 - B_HOST ... actually clear the Host Request (HR) bit as the
   messages say, disconnect the peripheral from the root hub,
   and don't detour through a suspend state.  (In some cases
   this would eventually have cleaned up.)

Also adjust the A_SUSPEND transition to respect the A_AIDL_BDIS
timeout, so if HNP doesn't trigger quickly enough the A_WAIT_VFALL
transition happens as it should.

     * from A_PERIPHERAL, disconnect transitions to A_WAIT_BCON

 - B-Device:
     * kill OTG timer on entry to B_HOST state (HNP succeeded)
     * once HNP succeeds, clear "ignore_disconnect" flag
     * kick the root hub only _after_ the state is adjusted

Other state transitions are left alone.  Notably, exit paths from
the "roles have switched" state ... A_PERIPHERAL handling of that
stays seriously broken.

And tighten use and setup of T(a_wait_bcon) so that if it's used,
it's always valid.  (If that timer expires, the A-device will
stop powering VBUS.  For non-OTG systems, that will be a surprise.)
No behavioral changes, other than more consistency when applying
that core HNP timeout.

Let the otg_transceiver in MUSB be managed by an external driver;
don't assume it's integrated.  OMAP3 chips need it to be external,
and there may be ways to interact with the transceiver which add
functionality to the system.

Platform init code is responsible for setting up the transeciver,
probably using the NOP transceiver for integrated transceivers.
External ones will use whatever the board init code provided,
such as twl4030 or something more hands-off.

The existance of the scheduling list shouldn't matter in
determining whether there's currectly an URB executing on a
hardware endpoint. What should actually matter is the 'in_qh'
or 'out_qh' fields of the 'struct musb_hw_ep' -- those are
set in musb_start_urb() and cleared in musb_giveback() when
the endpoint's URB list drains. Hence we should be able to
replace the big *switch* statements in musb_urb_dequeue()
and musb_h_disable() with mere musb_ep_get_qh() calls...

While at it, do some more changes:

 - add 'is_in' variable to musb_urb_dequeue();

 - remove the unnecessary 'epnum' variable from musb_h_disable();

 - fix the comment style in the vicinity.

Sergei Shtylyov a5073b5283 musb_gadget: fix unhandled endpoint 0 IRQs
The gadget EP0 code routinely ignores an interrupt at end of
the data phase because of musb_g_ep0_giveback() resetting the
state machine to "idle, waiting for SETUP" phase prematurely.

The driver also prematurely leaves the status phase on
receiving the SetupEnd interrupt.

As there were still unhandled endpoint 0 interrupts happening
from time to time after fixing these issues, there turned to
be yet another culprit: two distinct gadget states collapsed
into one.

The (missing) state that comes after STATUS IN/OUT states was
typically indiscernible from them since the corresponding
interrupts tend to happen within too little period of time
(due to only a zero-length status packet in between) and so
they got coalesced; yet this state is not the same as the next
one which is associated with the reception of a SETUP packet.

Adding this extra state seems to have fixed the rest of the
unhandled interrupts that generic_interrupt() and
davinci_interrupt() hid by faking their result and only
emitting a debug message -- so, stop doing that.

Sergei Shtylyov 6b6e97107f USB: musb: fix isochronous TXDMA (take 2)
Multi-frame isochronous TX URBs transfers in DMA mode never
complete with CPPI DMA because musb_host_tx() doesn't restart
DMA on the second frame, only emitting a debug message.
With Inventra DMA they complete, but in PIO mode.  To fix:

 - Factor out programming of the DMA transfer from
   musb_ep_program() into musb_tx_dma_program();

 - Reorder the code at the end of musb_host_tx() to
   facilitate the fallback to PIO iff DMA fails;

 - Handle the buffer offset consistently for both
   PIO and DMA modes;

 - Add an argument to musb_ep_program() for the same
   reason (it only worked correctly with non-zero
   offset of the first frame in PIO mode);

 - Set the completed isochronous frame descriptor's
   'actual_length' and 'status' fields correctly in
   DMA mode.

Also, since CPPI reportedly doesn't like sending isochronous
packets in the RNDIS mode, change the criterion for this
mode to be used only for multi-packet transfers.  (There's
no need for that mode in the single-packet case anyway.)

Sergei Shtylyov b6e434a540 USB: musb: sanitize clearing TXCSR DMA bits (take 2)
The MUSB code clears TXCSR_DMAMODE incorrectly in several
places, either asserting that TXCSR_DMAENAB is clear (when
sometimes it isn't) or clearing both bits together.  Recent
versions of the programmer's guide require DMAENAB to be
cleared first, although some older ones didn't.

Fix this and while at it:

 - In musb_gadget::txstate(), stop clearing the AUTOSET
   and DMAMODE bits for the CPPI case since they never
   get set anyway (the former bit is reserved on DaVinci);
   but do clear the DMAENAB bit on the DMA error path.

 - In musb_host::musb_ep_program(), remove the duplicate
   DMA controller specific code code clearing the TXCSR
   previous state, add the code to clear TXCSR DMA bits
   on the Inventra DMA error path, to replace such code
   (executed late) on the PIO path.

 - In musbhsdma::dma_channel_abort()/dma_controller_irq(),
   add/use the 'offset' variable to avoid MUSB_EP_OFFSET()
   invocations on every RXCSR/TXCSR access.

Sergei Shtylyov c7bbc056a9 USB: musb: bugfixes for multi-packet TXDMA support
We really want to use DMA mode 1 for all multi-packet transfers;
that's one IRQ on DMA completion, instead of one per packet.

There is an important issue with such transfers, especially on
the host side:  when such transfers end with a full-size packet,
we must defer musb_dma_completion() calls until the FIFO empties.
Else we report URB completions too soon, and may clobber data in
the FIFO fifo when writing the next packet (losing data).

The Inventra DMA support uses DMA mode 1, but it ignores that
issue.  The CPPI DMA support uses mode 0, but doesn't handle
its TXPKTRDY interrupts quite right either; it can get stale
"packet ready" interrupts, and report transfer completion too
early using slightly different code paths, also losing data.

So I'm solving it in a generic way -- by adding a sort of the
"interrupt filter" into musb_host_tx(), catching these cases
where a DMA completion IRQ doesn't suffice and removing some
needlessly controller-specific logic.  When a TXDMA interrupt
happens and DMA request mode 1 is active, that filter resets
to mode 0 and defers URB completion processing until TXPKTRDY,
unless the FIFO is already empty.  Related filtering logic in
Inventra and CPPI code gets removed.

Since it should be competely safe now to use the DMA request
mode 1 for host side transfers with the CPPI DMA controller,
set it in musb_h_tx_dma_start() ... now renamed (and shared).

David Brownell 78322c1a64 USB: musb_host, fix ep0 fifo flushing
The MUSB host side can't share generic TX FIFO flush logic
with EP0; the EP0 TX status register bits are different
from those for other entpoints.

Resolve this issue by providing a new EP0-specific routine
to flush and reset the FIFO, which pays careful attention to
restrictions listed in the latest programmer's guide.  This
gets rid of an open issue whereby the usbtest control write
test (#14) failed.

Ajay Kumar Gupta 743821717c USB: musb: only turn off vbus in OTG hosts
Except on DaVinci, VBUS is now switched off as part of idling the
USB link (after a_wait_bcon) whenever a device is disconnected
from host.  This is correct for OTG hosts, where either SRP or
an ID interrupt could turn VBUS on again.

However, for non-OTG hosts there's no way to turn VBUS on again,
so the host becomes unusable.  And the procfs entry which once
allowed a manual workaround for this is now gone.

This patch adds an is_otg_enabled() check before scheduling the
switch-off timer in disconnect path, supporting a "classic host"
mode where SRP is unavailable.

Ajay Kumar Gupta 1e0320f0d4 USB: musb: NAK timeout scheme on bulk RX endpoint
Fixes endpoint starvation issue when more than one bulk QH is
multiplexed on the reserved bulk RX endpoint, which is normal
for cases like serial and ethernet adapters.

This patch sets the NAK timeout interval for such QHs, and when
a timeout triggers the next QH will be scheduled.  (This resembles
the bulk scheduling done in hardware by EHCI, OHCI, and UHCI.)

This scheme doesn't work for devices which are connected to a
high to full speed tree (transaction translator) as there is
no NAK timeout interrupt from the musb controller from such
devices.

Tested with PIO, Inventra DMA, CPPI DMA.

Sergei Shtylyov 3ecdb9acf3 USB: musb: be careful with 64K+ transfer lengths, host side
Feeding 32-bit length cast down to 'u16' to min() to calculate the FIFO
count in musb_host_tx() risks sending a short packet prematurely for
transfer sizes over 64 KB.

Similarly, although data transfer size shouldn't exceed 65535 bytes for
the control endpoint, making musb_h_ep0_continue() more robust WRT URBs
with possibly oversized buffer will not hurt either...

Sergei Shtylyov 51d9f3e100 USB: musb: fix data toggle saving with shared FIFO
For some strange reason the host side musb_giveback() decides
that it's always got an IN transfer when the hardware endpoint
is using a shared FIFO.  This causes musb_save_toggle() to read
the toggle state from the RXCSR register instead of TXCSR, and
may also cause unneeded reloading of RX endpoint registers.

Sergei Shtylyov dc61d238b8 USB: musb: host endpoint_disable() oops fixes
The musb_h_disable() routine can oops in some cases:

 - It's not safe to read hep->hcpriv outside musb->lock,
   since it gets changed on completion IRQ paths.

 - The list iterators aren't safe to use in that way;
   just remove the first element while !list_empty(),
   so deletions on other code paths can't make trouble.

We need two "scrub the list" loops because only one branch
should touch hardware and advance the schedule.

Sergei Shtylyov a2fd814e6a USB: musb: fix urb_dequeue() method
The urb_dequeue() method forgets to unlink 'struct musb_qh' from the
control or bulk schedules when the URB being cancelled is the only
one queued to its endpoint.  That will cause musb_advance_schedule()
to block once it reaches 'struct musb_qh' with now empty URB list, so
URBs queued for other endpoints after the one being dequeued will not
be served.

Fix by unlinking the QH from the list except when it's already being
handled (typically by musb_giveback).  Since a QH with an empty URB
list is now supposed to be freed, do that.  And remove a now-useless
check from musb_advance_schedule().

Dmitry Krivoschekov b7bdcb79de USB: musb: fix musb_host_tx() for shared endpoint FIFO
The input queue should be used for TX on endpoints which
share FIFO hardware.  The host TX path wasn't doing that.

Shared FIFOs are most often configured for periodic endpoints,
which are mostly used for RX/IN transfers ... that's probably
how this bug managed to linger for a long time.

David Brownell 34f32c9701 usb: musb: make Davinci *work* in mainline
Now that the musb build fixes for DaVinci got merged (RC3?), kick in
the other bits needed to get it finally *working* in mainline:

 - Use clk_enable()/clk_disable() ... the "always enable USB clocks"
   code this originally relied on has since been removed.

 - Initialize the USB device only after the relevant I2C GPIOs are
   available, so the host side can properly enable VBUS.

 - Tweak init sequencing to cope with mainline's relatively late init
   of the I2C system bus for power switches, transceivers, and so on.

Sanity tested on DM6664 EVM for host and peripheral modes; that system
won't boot with CONFIG_PM enabled, so OTG can't yet be tested.  Also
verified on OMAP3.

(Unrelated:  correct the MODULE_PARM_DESC spelling of musb_debug.)
