Drop the FSF's postal address from the source code files that typically
contain mostly the license text. Of the 628 removed instances, 578 are
outdated.
The patch has been created with the following command without manual edits:

Fix the 0x0x prefix in integer constants.
In this case a padding 0 must also be inserted to make the constants
look like all the other 16 bits ones.

Rename all PCI drivers with their own directory under
drivers/media/video into drivers/media/pci and update the
building system.
