Marc Zyngier 33b5c38852 arm: KVM: Allow unaligned accesses at HYP
We currently have the HSCTLR.A bit set, trapping unaligned accesses
at HYP, but we're not really prepared to deal with it.

Since the rest of the kernel is pretty happy about that, let's follow
its example and set HSCTLR.A to zero. Modern CPUs don't really care.

Marc Zyngier d9118c87d2 ARM: hyp-stub: Zero r0 on successful stub handling
We now return HVC_STUB_ERR when a stub hypercall fails, but we
leave whatever was in r0 on success. Zeroing it on return seems
like a good idea.

Another missing stub hypercall is HVC_SOFT_RESTART. It turns out
that it is pretty easy to implement in terms of HVC_RESET_VECTORS
(since it needs to turn the MMU off).

Marc Zyngier bc845e4fbb ARM: KVM: Implement HVC_RESET_VECTORS stub hypercall in the init code
In order to restore HYP mode to its original condition, KVM currently
implements __kvm_hyp_reset(). As we're moving towards a hyp-stub
defined API, it becomes necessary to implement HVC_RESET_VECTORS.

This patch adds the HVC_RESET_VECTORS hypercall to the KVM init
code, which so far lacked any form of hypercall support.

Marc Zyngier e537ecd7ef arm: KVM: Allow hyp teardown
So far, KVM was getting in the way of kexec on 32bit (and the arm64
kexec hackers couldn't be bothered to fix it on 32bit...).

With simpler page tables, tearing KVM down becomes very easy, so
let's just do it.

Pranavkumar Sawargaonkar f6edbbf36d ARM/ARM64: KVM: Nuke Hyp-mode tlbs before enabling MMU
X-Gene u-boot runs in EL2 mode with MMU enabled hence we might
have stale EL2 tlb enteris when we enable EL2 MMU on each host CPU.

This can happen on any ARM/ARM64 board running bootloader in
Hyp-mode (or EL2-mode) with MMU enabled.

This patch ensures that we flush all Hyp-mode (or EL2-mode) TLBs
on each host CPU before enabling Hyp-mode (or EL2-mode) MMU.

Linus Torvalds 66bb0aa077 Here are the PPC and ARM changes for KVM, which I separated because
they had small conflicts (respectively within KVM documentation,
and with 3.16-rc changes). Since they were all within the subsystem,
 I took care of them.

Stephen Rothwell reported some snags in PPC builds, but they are all
fixed now; the latest linux-next report was clean.

 New features for ARM include:
 - KVM VGIC v2 emulation on GICv3 hardware
 - Big-Endian support for arm/arm64 (guest and host)
 - Debug Architecture support for arm64 (arm32 is on Christoffer's todo list)

 And for PPC:
 - Book3S: Good number of LE host fixes, enable HV on LE
 - Book3S HV: Add in-guest debug support

This release drops support for KVM on the PPC440. As a result, the
PPC merge removes more lines than it adds. :)

I also included an x86 change, since Davidlohr tied it to an independent
bug report and the reporter quickly provided a Tested-by; there was no
 reason to wait for -rc2.

Russell King 6ebbf2ce43 ARM: convert all "mov.* pc, reg" to "bx reg" for ARMv6+
ARMv6 and greater introduced a new instruction ("bx") which can be used
to return from function calls. Recent CPUs perform better when the
"bx lr" instruction is used rather than the "mov pc, lr" instruction,
and this sequence is strongly recommended to be used by the ARM
architecture manual (section A.4.1.1).

We provide a new macro "ret" with all its variants for the condition
code which will resolve to the appropriate instruction.

Rather than doing this piecemeal, and miss some instances, change all
the "mov pc" instances to use the new macro, with the exception of
the "movs" instruction and the kprobes code. This allows us to detect
the "mov pc, lr" case and fix it up - and also gives us the possibility
of deploying this for other registers depending on the CPU selection.

Victor Kamensky 19b0e60a63 ARM: KVM: handle 64bit values passed to mrcc or from mcrr instructions in BE case
In some cases the mcrr and mrrc instructions in combination with the ldrd
and strd instructions need to deal with 64bit value in memory. The ldrd
and strd instructions already handle endianness within word (register)
boundaries but to get effect of the whole 64bit value represented correctly,
rr_lo_hi macro is introduced and is used to swap registers positions when
the mcrr and mrrc instructions are used. That has the effect of swapping
two words.

Marc Zyngier 5a677ce044 ARM: KVM: switch to a dual-step HYP init code
Our HYP init code suffers from two major design issues:
- it cannot support CPU hotplug, as we tear down the idmap very early
- it cannot perform a TLB invalidation when switching from init to
runtime mappings, as pages are manipulated from PL1 exclusively

The hotplug problem mandates that we keep two sets of page tables
(boot and runtime). The TLB problem mandates that we're able to
transition from one PGD to another while in HYP, invalidating the TLBs
in the process.

To be able to do this, we need to share a page between the two page
tables. A page that will have the same VA in both configurations. All we
need is a VA that has the following properties:
- This VA can't be used to represent a kernel mapping.
- This VA will not conflict with the physical address of the kernel text

The vectors page seems to satisfy this requirement:
- The kernel never maps anything else there
- The kernel text being copied at the beginning of the physical memory,
it is unlikely to use the last 64kB (I doubt we'll ever support KVM
on a system with something like 4MB of RAM, but patches are very
  welcome).

Let's call this VA the trampoline VA.

Now, we map our init page at 3 locations:
- idmap in the boot pgd
- trampoline VA in the boot pgd
- trampoline VA in the runtime pgd

The init scenario is now the following:
- We jump in HYP with four parameters: boot HYP pgd, runtime HYP pgd,
  runtime stack, runtime vectors
- Enable the MMU with the boot pgd
- Jump to a target into the trampoline page (remember, this is the same
  physical page!)
- Now switch to the runtime pgd (same VA, and still the same physical
  page!)
- Invalidate TLBs
- Set stack and vectors
- Profit! (or eret, if you only care about the code).

Note that we keep the boot mapping permanently (it is not strictly an
idmap anymore) to allow for CPU hotplug in later patches.

Christoffer Dall 342cd0ab0e KVM: ARM: Hypervisor initialization
Sets up KVM code to handle all exceptions taken to Hyp mode.

When the kernel is booted in Hyp mode, calling an hvc instruction with r0
pointing to the new vectors, the HVBAR is changed to the the vector pointers.
This allows subsystems (like KVM here) to execute code in Hyp-mode with the
MMU disabled.

We initialize other Hyp-mode registers and enables the MMU for Hyp-mode from
the id-mapped hyp initialization code. Afterwards, the HVBAR is changed to
point to KVM Hyp vectors used to catch guest faults and to switch to Hyp mode
to perform a world-switch into a KVM guest.

Also provides memory mapping code to map required code pages, data structures,
and I/O regions accessed in Hyp mode at the same virtual address as the host
kernel virtual addresses, but which conforms to the architectural requirements
for translations in Hyp mode. This interface is added in arch/arm/kvm/arm_mmu.c
and comprises:
 - create_hyp_mappings(from, to);
 - create_hyp_io_mappings(from, to, phys_addr);
 - free_hyp_pmds();
