Ronnie Sahlberg 555782aa55 cifs: move smb version mount options into fs_context.c
This and related patches which move mount related
code to fs_context.c has the advantage of
shriking the code in fs/cifs/connect.c (which had
the second most lines of code of any of the files
in cifs.ko and was getting harder to read due
to its size) and will also make it easier to
switch over to the new mount API in the future.

Steve French 3c6e65e679 smb3: do not try to cache root directory if dir leases not supported
To servers which do not support directory leases (e.g. Samba)
it is wasteful to try to open_shroot (ie attempt to cache the
root directory handle).  Skip attempt to open_shroot when
server does not indicate support for directory leases.

Cuts the number of requests on mount from 17 to 15, and
cuts the number of requests on stat of the root directory
from 4 to 3.

Rohith Surabattula 8e670f77c4 Handle STATUS_IO_TIMEOUT gracefully
Currently STATUS_IO_TIMEOUT is not treated as retriable error.
It is currently mapped to ETIMEDOUT and returned to userspace
for most system calls. STATUS_IO_TIMEOUT is returned by server
in case of unavailability or throttling errors.

This patch will map the STATUS_IO_TIMEOUT to EAGAIN, so that it
can be retried. Also, added a check to drop the connection to
not overload the server in case of ongoing unavailability.

Paulo Alcantara e183785f25 cifs: fix check of tcon dfs in smb1
For SMB1, the DFS flag should be checked against tcon->Flags rather

Waiman Long 453431a549 mm, treewide: rename kzfree() to kfree_sensitive()
As said by Linus:

  A symmetric naming is only helpful if it implies symmetries in use.
  Otherwise it's actively misleading.

  In "kzalloc()", the z is meaningful and an important part of what the
  caller wants.

  In "kzfree()", the z is actively detrimental, because maybe in the
  future we really _might_ want to use that "memfill(0xdeadbeef)" or
  something. The "zero" part of the interface isn't even _relevant_.

The main reason that kzfree() exists is to clear sensitive information
that should not be leaked to other future users of the same memory
objects.

Rename kzfree() to kfree_sensitive() to follow the example of the recently
added kvfree_sensitive() and make the intention of the API more explicit.
In addition, memzero_explicit() is used to clear the memory to make sure
that it won't get optimized away by the compiler.

The renaming is done by using the command sequence:

followed by some editing of the kfree_sensitive() kerneldoc and adding
a kzfree backward compatibility macro in slab.h.

Paulo Alcantara 7efd081582 cifs: document and cleanup dfs mount
cifs_mount() for DFS mounts is for a long time way too complex to
follow, mostly because it lacks some documentation, does a lot of
operations like resolving DFS roots and links, checking for path
components, perform failover, crap code, etc.

Besides adding some documentation to it, do some cleanup and ensure
that the following is implemented and supported:

    * non-DFS mounts
    * DFS failover
    * DFS root mounts
        - tcon and cifs_sb must contain DFS path (NOT including prefix)
        - if prefix path, then save it in cifs_sb and it must not be
	  changed
    * DFS link mounts
      - tcon and cifs_sb must contain DFS path (including prefix)
      - if prefix path, then save it in cifs_sb and it may be changed
    * prevent recursion on broken link referrals (MAX_NESTED_LINKS)
    * check every path component of the currently resolved
      target (including prefix), and chase them accordingly
    * make sure that DFS referrals go through newly resolved root
      servers

Paulo Alcantara 11375a59a9 cifs: only update prefix path of DFS links in cifs_tree_connect()
For DFS root mounts that contain a prefix path, do not change them
after failover.

E.g., if the user mounts

	//srvA/root/dir1

and then lost connection to srvA, it will reconnect to

	//srvB/root/dir1

In case of DFS links, which may resolve to different prefix paths
depending on their list of targets, the following must be supported:

	- mount //srvA/root/link/bar
	- connect to //srvA/share
	- set prefix path to "bar"
	- lost connection to srvA
	- reconnect to next target: //srvB/share/foo
	- set new prefix path to "foo/bar"

In cifs_tree_connect(), check the server_type field of the cached DFS
referral to determine whether or not prefix path should be updated.

Colin Ian King c6a80e1ff4 cifs: fix double free error on share and prefix
Currently if the call dfs_cache_get_tgt_share fails we cannot
fully guarantee that share and prefix are set to NULL and the
next iteration of the loop can end up potentially double freeing
these pointers. Since the semantics of dfs_cache_get_tgt_share
are ambiguous for failure cases with the setting of share and
prefix (currently now and the possibly the future), it seems
prudent to set the pointers to NULL when the objects are
free'd to avoid any double frees.

Paul Aurich 5391b8e1b7 SMB3: Honor 'posix' flag for multiuser mounts
The flag from the primary tcon needs to be copied into the volume info
so that cifs_get_tcon will try to enable extensions on the per-user
tcon. At that point, since posix extensions must have already been
enabled on the superblock, don't try to needlessly adjust the mount
flags.

Steve French 82e9367c43 smb3: Add new parm "nodelete"
In order to handle workloads where it is important to make sure that
a buggy app did not delete content on the drive, the new mount option
"nodelete" allows standard permission checks on the server to work,
but prevents on the client any attempts to unlink a file or delete
a directory on that mount point.  This can be helpful when running
a little understood app on a network mount that contains important
content that should not be deleted.

Paulo Alcantara e4af35fa55 cifs: handle hostnames that resolve to same ip in failover
In order to support reconnect to hostnames that resolve to same ip
address, besides relying on the currently set hostname to match DFS
targets, attempt to resolve the targets and then match their addresses
with the reconnected server ip address.

For instance, if we have two hostnames "FOO" and "BAR", and both
resolve to the same ip address, we would be able to handle failover in
DFS paths like

    \\FOO\dfs\link1 -> [ \BAZ\share2 (*), \BAR\share1 ]
    \\FOO\dfs\link2 -> [ \BAZ\share2 (*), \FOO\share1 ]

so when "BAZ" is no longer accessible, link1 and link2 would get
reconnected despite having different target hostnames.

Paulo Alcantara aaa3aef34d cifs: set up next DFS target before generic_ip_connect()
If we mount a very specific DFS link

    \\FS0.FOO.COM\dfs\link -> \FS0\share1, \FS1\share2

where its target list contains NB names ("FS0" & "FS1") rather than
FQDN ones ("FS0.FOO.COM" & "FS1.FOO.COM"), we end up connecting to
\FOO\share1 but server->hostname will have "FOO.COM".  The reason is
because both "FS0" and "FS0.FOO.COM" resolve to same IP address and
they share same TCP server connection, but "FS0.FOO.COM" was the first
hostname set -- which is OK.

However, if the echo thread timeouts and we still have a good
connection to "FS0", in cifs_reconnect()

    rc = generic_ip_connect(server) -> success

it successfully reconnects to "FS0" server but does not set up next

as reconn_inval_dfs_target() would have it set to "FS0" if called
earlier.

Finally, in __smb2_reconnect(), the reconnect of tcons would fail
because tcon->ses->server->hostname (FS0.FOO.COM) does not match DFS
target's hostname (FS0).

Fix that by calling reconn_inval_dfs_target() before
generic_ip_connect() so server->hostname will get updated correctly
prior to reconnecting its tcons in __smb2_reconnect().

With "cifs: handle hostnames that resolve to same ip in failover"
patch

    - The above problem would not occur.
    - We could save an DNS query to find out that they both resolve to
      the same ip address.

Paulo Alcantara 65303de829 cifs: do not share tcons with DFS
This disables tcon re-use for DFS shares.

tcon->dfs_path stores the path that the tcon should connect to when
doing failing over.

If that tcon is used multiple times e.g. 2 mounts using it with
different prefixpath, each will need a different dfs_path but there is
only one tcon. The other solution would be to split the tcon in 2
tcons during failover but that is much harder.

tcons could not be shared with DFS in cifs.ko because in a
DFS namespace like:

          //domain/dfsroot -> /serverA/dfsroot, /serverB/dfsroot

          //serverA/dfsroot/link -> /serverA/target1/aa/bb

          //serverA/dfsroot/link2 -> /serverA/target1/cc/dd

you can see that link and link2 are two DFS links that both resolve to
the same target share (/serverA/target1), so cifs.ko will only contain a
single tcon for both link and link2.

The problem with that is, if we (auto)mount "link" and "link2", cifs.ko
will only contain a single tcon for both DFS links so we couldn't
perform failover or refresh the DFS cache for both links because
tcon->dfs_path was set to either "link" or "link2", but not both --
which is wrong.

Eric Biggers dc920277f1 cifs: clear PF_MEMALLOC before exiting demultiplex thread
Leaving PF_MEMALLOC set when exiting a kthread causes it to remain set
during do_exit().  That can confuse things.  For example, if BSD process
accounting is enabled and the accounting file has FS_SYNC_FL set and is
located on an ext4 filesystem without a journal, then do_exit() can end
up calling ext4_write_inode().  That triggers the
WARN_ON_ONCE(current->flags & PF_MEMALLOC) there, as it assumes
(appropriately) that inodes aren't written when allocating memory.

This was originally reported for another kernel thread, xfsaild() [1].
cifs_demultiplex_thread() also exits with PF_MEMALLOC set, so it's
potentially subject to this same class of issue -- though I haven't been
able to reproduce the WARN_ON_ONCE() via CIFS, since unlike xfsaild(),
cifs_demultiplex_thread() is sent SIGKILL before exiting, and that
interrupts the write to the BSD process accounting file.

Either way, leaving PF_MEMALLOC set is potentially problematic.  Let's
clean this up by properly saving and restoring PF_MEMALLOC.

Steve French ba55344f36 CIFS: Warn less noisily on default mount
The warning we print on mount about how to use less secure dialects
(when the user does not specify a version on mount) is useful
but is noisy to print on every default mount, and can be changed
to a warn_once.  Slightly updated the warning text as well to note
SMB3.1.1 which has been the default which is typically negotiated
(for a few years now) by most servers.

      "No dialect specified on mount. Default has changed to a more
       secure dialect, SMB2.1 or later (e.g. SMB3.1.1), from CIFS
       (SMB1). To use the less secure SMB1 dialect to access old
       servers which do not support SMB3.1.1 (or even SMB3 or SMB2.1)
       specify vers=1.0 on mount."

revalidate the root dentry every time we set a new prefix path.

With the addition of SMB session channels, we introduced new TCP
server pointers that have no sessions or tcons associated with them.

In this case, when we started looking for TCP connections, we might
end up picking session channel rather than the master connection,
hence failing to get either a session or a tcon.

In order to fix that, this patch introduces a new "is_channel" field
to TCP_Server_Info structure so we can skip session channels during
lookup of connections.

in smb2_get_enc_key().

Currently a lot of the code to initialize a connection & session uses
the cifs_ses as input. But depending on if we are opening a new session
or a new channel we need to use different server pointers.

Add a "binding" flag in cifs_ses and a helper function that returns
the server ptr a session should use (only in the sess establishment
code path).

Pavel Shilovsky 7b71843fa7 CIFS: Do not miss cancelled OPEN responses
When an OPEN command is cancelled we mark a mid as
cancelled and let the demultiplex thread process it
by closing an open handle. The problem is there is
a race between a system call thread and the demultiplex
thread and there may be a situation when the mid has
been already processed before it is set as cancelled.

Fix this by processing cancelled requests when mids
are being destroyed which means that there is only
one thread referencing a particular mid. Also set
mids as cancelled unconditionally on their state.

Pavel Shilovsky 86a7964be7 CIFS: Fix NULL pointer dereference in mid callback
There is a race between a system call processing thread
and the demultiplex thread when mid->resp_buf becomes NULL
and later is being accessed to get credits. It happens when
the 1st thread wakes up before a mid callback is called in
the 2nd one but the mid state has already been set to
MID_RESPONSE_RECEIVED. This causes NULL pointer dereference
in mid callback.

Fix this by saving credits from the response before we
update the mid state and then use this value in the mid
callback rather then accessing a response buffer.

Fix this by obtaining extra references for mids being retried
and marking them as MID_DELETED which indicates that such a mid
has been dequeued from the pending list.

Also move mid cleanup logic from DeleteMidQEntry to
_cifs_mid_q_entry_release which is called when the last reference
to a particular mid is put. This allows to avoid any use-after-free
of response buffers.

The patch needs to be backported to stable kernels. A stable tag
is not mentioned below because the patch doesn't apply cleanly
to any actively maintained stable kernel.

Paulo Alcantara (SUSE) d532cc7efd cifs: Handle -EINPROGRESS only when noblockcnt is set
We only want to avoid blocking in connect when mounting SMB root
filesystems, otherwise bail out from generic_ip_connect() so cifs.ko
can perform any reconnect failover appropriately.

This fixes DFS failover/reconnection tests in upstream buildbot.

Steve French 3e7a02d478 smb3: allow disabling requesting leases
In some cases to work around server bugs or performance
problems it can be helpful to be able to disable requesting
SMB2.1/SMB3 leases on a particular mount (not to all servers
and all shares we are mounted to). Add new mount parm
"nolease" which turns off requesting leases on directory
or file opens.  Currently the only way to disable leases is
globally through a module load parameter. This is more
granular.

Displayed in /proc/fs/cifs/Stats once for each
socket we are connected to.

This allows us to find out what the maximum number of
requests that had been in flight (at any one time). Note that
/proc/fs/cifs/Stats can be reset if you want to look for
maximum over a small period of time.

Steve French 563317ec30 smb3: enable offload of decryption of large reads via mount option
Disable offload of the decryption of encrypted read responses
by default (equivalent to setting this new mount option "esize=0").

Allow setting the minimum encrypted read response size that we
will choose to offload to a worker thread - it is now configurable
via on a new mount option "esize="

Depending on which encryption mechanism (GCM vs. CCM) and
the number of reads that will be issued in parallel and the
performance of the network and CPU on the client, it may make
sense to enable this since it can provide substantial benefit when
multiple large reads are in flight at the same time.

Steve French 4f5c10f1ad smb3: allow skipping signature verification for perf sensitive configurations
Add new mount option "signloosely" which enables signing but skips the
sometimes expensive signing checks in the responses (signatures are
calculated and sent correctly in the SMB2/SMB3 requests even with this
mount option but skipped in the responses).  Although weaker for security
(and also data integrity in case a packet were corrupted), this can provide
enough of a performance benefit (calculating the signature to verify a
packet can be expensive especially for large packets) to be useful in
some cases.

Eric W. Biederman 33da8e7c81 signal: Allow cifs and drbd to receive their terminating signals
My recent to change to only use force_sig for a synchronous events
wound up breaking signal reception cifs and drbd.  I had overlooked
the fact that by default kthreads start out with all signals set to
SIG_IGN.  So a change I thought was safe turned out to have made it
impossible for those kernel thread to catch their signals.

Reverting the work on force_sig is a bad idea because what the code
was doing was very much a misuse of force_sig.  As the way force_sig
ultimately allowed the signal to happen was to change the signal
handler to SIG_DFL.  Which after the first signal will allow userspace
to send signals to these kernel threads.  At least for
wake_ack_receiver in drbd that does not appear actively wrong.

So correct this problem by adding allow_kernel_signal that will allow
signals whose siginfo reports they were sent by the kernel through,
but will not allow userspace generated signals, and update cifs and
drbd to call allow_kernel_signal in an appropriate place so that their
thread can receive this signal.

Fixing things this way ensures that userspace won't be able to send
signals and cause problems, that it is clear which signals the
threads are expecting to receive, and it guarantees that nothing
else in the system will be affected.

This change was partly inspired by similar cifs and drbd patches that
added allow_signal.

order to not impact the rest of the merge window.

Steve French 412094a8fb smb3: add new mount option to retrieve mode from special ACE
There is a special ACE used by some servers to allow the mode
bits to be stored.  This can be especially helpful in scenarios
in which the client is trusted, and access checking on the
client vs the POSIX mode bits is sufficient.

Add mount option to allow enabling this behavior.
Follow on patch will add support for chmod and queryinfo
(stat) by retrieving the POSIX mode bits from the special
ACE, SID: S-1-5-88-3

Ronnie Sahlberg f2caf901c1 cifs: Fix a race condition with cifs_echo_request
There is a race condition with how we send (or supress and don't send)
smb echos that will cause the client to incorrectly think the
server is unresponsive and thus needs to be reconnected.

Summary of the race condition:
 1) Daisy chaining scheduling creates a gap.
 2) If traffic comes unfortunate shortly after
    the last echo, the planned echo is suppressed.
 3) Due to the gap, the next echo transmission is delayed
    until after the timeout, which is set hard to twice
    the echo interval.

This is fixed by changing the timeouts from 2 to three times the echo interval.

Steve French 43cdae88de Fix match_server check to allow for auto dialect negotiate
When using multidialect negotiate (default or specifying vers=3.0 which
allows any smb3 dialect), fix how we check for an existing server session.
Before this fix if you mounted a second time to the same server (e.g. a
different share on the same server) we would only reuse the existing smb
session if a single dialect were requested (e.g. specifying vers=2.1 or vers=3.0
or vers=3.1.1 on the mount command). If a default mount (e.g. not
specifying vers=) is done then would always create a new socket connection
and SMB3 (or SMB3.1.1) session each time we connect to a different share
on the same server rather than reusing the existing one.

David Howells 2e12256b9a keys: Replace uid/gid/perm permissions checking with an ACL
Replace the uid/gid/perm permissions checking on a key with an ACL to allow
the SETATTR and SEARCH permissions to be split.  This will also allow a
greater range of subjects to represented.

============
WHY DO THIS?
============

The problem is that SETATTR and SEARCH cover a slew of actions, not all of
which should be grouped together.

For SETATTR, this includes actions that are about controlling access to a
key:

 (1) Changing a key's ownership.

 (2) Changing a key's security information.

 (3) Setting a keyring's restriction.

And actions that are about managing a key's lifetime:

 (4) Setting an expiry time.

 (5) Revoking a key.

and (proposed) managing a key as part of a cache:

 (6) Invalidating a key.

Managing a key's lifetime doesn't really have anything to do with
controlling access to that key.

Expiry time is awkward since it's more about the lifetime of the content
and so, in some ways goes better with WRITE permission.  It can, however,
be set unconditionally by a process with an appropriate authorisation token
for instantiating a key, and can also be set by the key type driver when a
key is instantiated, so lumping it with the access-controlling actions is
probably okay.

As for SEARCH permission, that currently covers:

 (1) Finding keys in a keyring tree during a search.

 (2) Permitting keyrings to be joined.

 (3) Invalidation.

But these don't really belong together either, since these actions really
need to be controlled separately.

Finally, there are number of special cases to do with granting the
administrator special rights to invalidate or clear keys that I would like
to handle with the ACL rather than key flags and special checks.

WHAT IS CHANGED
===============

The SETATTR permission is split to create two new permissions:

 (1) SET_SECURITY - which allows the key's owner, group and ACL to be
     changed and a restriction to be placed on a keyring.

 (2) REVOKE - which allows a key to be revoked.

The SEARCH permission is split to create:

 (1) SEARCH - which allows a keyring to be search and a key to be found.

 (2) JOIN - which allows a keyring to be joined as a session keyring.

 (3) INVAL - which allows a key to be invalidated.

The WRITE permission is also split to create:

 (1) WRITE - which allows a key's content to be altered and links to be
     added, removed and replaced in a keyring.

 (2) CLEAR - which allows a keyring to be cleared completely.  This is
     split out to make it possible to give just this to an administrator.

 (3) REVOKE - see above.


Keys acquire ACLs which consist of a series of ACEs, and all that apply are
unioned together.  An ACE specifies a subject, such as:

 (*) Possessor - permitted to anyone who 'possesses' a key
 (*) Owner - permitted to the key owner
 (*) Group - permitted to the key group
 (*) Everyone - permitted to everyone

Note that 'Other' has been replaced with 'Everyone' on the assumption that
you wouldn't grant a permit to 'Other' that you wouldn't also grant to
everyone else.

Further subjects may be made available by later patches.

The ACE also specifies a permissions mask.  The set of permissions is now:

	VIEW		Can view the key metadata
	READ		Can read the key content
	WRITE		Can update/modify the key content
	SEARCH		Can find the key by searching/requesting
	LINK		Can make a link to the key
	SET_SECURITY	Can change owner, ACL, expiry
	INVAL		Can invalidate
	REVOKE		Can revoke
	JOIN		Can join this keyring
	CLEAR		Can clear this keyring


The KEYCTL_SETPERM function is then deprecated.

The KEYCTL_SET_TIMEOUT function then is permitted if SET_SECURITY is set,
or if the caller has a valid instantiation auth token.

The KEYCTL_INVALIDATE function then requires INVAL.

The KEYCTL_REVOKE function then requires REVOKE.

The KEYCTL_JOIN_SESSION_KEYRING function then requires JOIN to join an
existing keyring.

The JOIN permission is enabled by default for session keyrings and manually
created keyrings only.

BACKWARD COMPATIBILITY
======================

To maintain backward compatibility, KEYCTL_SETPERM will translate the
permissions mask it is given into a new ACL for a key - unless
KEYCTL_SET_ACL has been called on that key, in which case an error will be
returned.

It will convert possessor, owner, group and other permissions into separate
ACEs, if each portion of the mask is non-zero.

SETATTR permission turns on all of INVAL, REVOKE and SET_SECURITY.  WRITE
permission turns on WRITE, REVOKE and, if a keyring, CLEAR.  JOIN is turned
on if a keyring is being altered.

The KEYCTL_DESCRIBE function translates the ACL back into a permissions
mask to return depending on possessor, owner, group and everyone ACEs.

It will make the following mappings:

 (1) INVAL, JOIN -> SEARCH

 (2) SET_SECURITY -> SETATTR

 (3) REVOKE -> WRITE if SETATTR isn't already set

 (4) CLEAR -> WRITE

Note that the value subsequently returned by KEYCTL_DESCRIBE may not match
the value set with KEYCTL_SETATTR.

TESTING
=======

This passes the keyutils testsuite for all but a couple of tests:

 (1) tests/keyctl/dh_compute/badargs: The first wrong-key-type test now
     returns EOPNOTSUPP rather than ENOKEY as READ permission isn't removed
     if the type doesn't have ->read().  You still can't actually read the
     key.

 (2) tests/keyctl/permitting/valid: The view-other-permissions test doesn't
     work as Other has been replaced with Everyone in the ACL.

Eric W. Biederman 72abe3bcf0 signal/cifs: Fix cifs_put_tcp_session to call send_sig instead of force_sig
The locking in force_sig_info is not prepared to deal with a task that
exits or execs (as sighand may change).  The is not a locking problem
in force_sig as force_sig is only built to handle synchronous
exceptions.

Further the function force_sig_info changes the signal state if the
signal is ignored, or blocked or if SIGNAL_UNKILLABLE will prevent the
delivery of the signal.  The signal SIGKILL can not be ignored and can
not be blocked and SIGNAL_UNKILLABLE won't prevent it from being
delivered.

So using force_sig rather than send_sig for SIGKILL is confusing
and pointless.

Because it won't impact the sending of the signal and and because
using force_sig is wrong, replace force_sig with send_sig.

assumes that the MID callback should not take srv_mutex, this may not always
be true. SMB Direct requires the MID callback completed before calling
transport so all pending memory registration can be freed. So restore the
original calling sequence so TCP transport will use the same code, but moving
smbd_destroy() after all MID has been called.

Long Li 050b8c3740 smbd: Make upper layer decide when to destroy the transport
On transport recoonect, upper layer CIFS code destroys the current
transport and then recoonect. This code path is not used by SMBD, in that
SMBD destroys its transport on RDMA disconnect notification independent of
CIFS upper layer behavior.

This approach adds some costs to SMBD layer to handle transport shutdown
and restart, and to deal with several racing conditions on reconnecting
transport.

Re-work this code path by introducing a new smbd_destroy. This function is
called form upper layer to ask SMBD to destroy the transport. SMBD will no
longer need to destroy the transport by itself while worrying about data
transfer is in progress. The upper layer guarantees the transport is
locked.

Steve French ca567eb2b3 SMB3: Allow persistent handle timeout to be configurable on mount
Reconnecting after server or network failure can be improved
(to maintain availability and protect data integrity) by allowing
the client to choose the default persistent (or resilient)
handle timeout in some use cases.  Today we default to 0 which lets
the server pick the default timeout (usually 120 seconds) but this
can be problematic for some workloads.  Add the new mount parameter
to cifs.ko for SMB3 mounts "handletimeout" which enables the user
to override the default handle timeout for persistent (mount
option "persistenthandles") or resilient handles (mount option
"resilienthandles").  Maximum allowed is 16 minutes (960000 ms).
Units for the timeout are expressed in milliseconds. See
section 2.2.14.2.12 and 2.2.31.3 of the MS-SMB2 protocol
specification for more information.

Pavel Shilovsky 335b7b62ff CIFS: Respect reconnect in MTU credits calculations
Every time after a session reconnect we don't need to account for
credits obtained in previous sessions. Introduce new struct cifs_credits
which contains both credits value and reconnect instance of the
time those credits were taken. Modify a routine that add credits
back to handle the reconnect instance by assuming zero credits
if the reconnect happened after the credits were obtained and
before we decided to add them back due to some errors during sending.

This patch fixes the MTU credits cases. The subsequent patch
will handle non-MTU ones.

Steve French e8506d25f7 smb3: make default i/o size for smb3 mounts larger
We negotiate rsize mounts (and it can be overridden by user) to
typically 4MB, so using larger default I/O sizes from userspace
(changing to 1MB default i/o size returned by stat) the
performance is much better (and not just for long latency
network connections) in most use cases for SMB3 than the default I/O
size (which ends up being 128K for cp and can be even smaller for cp).
This can be 4x slower or worse depending on network latency.

By changing inode->blocksize from 32K (which was perhaps ok
for very old SMB1/CIFS) to a larger value, 1MB (but still less than
max size negotiated with the server which is 4MB, in order to minimize
risk) it significantly increases performance for the
noncached case, and slightly increases it for the cached case.
This can be changed by the user on mount (specifying bsize=
values from 16K to 16MB) to tune better for performance
for applications that depend on blocksize.
