First set of fixes for v4.8-rc cycle. Again, dwc3 is

According to Synopsys Databook 2.60a, section 8.3.4,
it's stated that:

When rndis data transfer is in progress, some Windows7 Host PC is not
sending the GET_ENCAPSULATED_RESPONSE command for receiving the response
for the previous SEND_ENCAPSULATED_COMMAND processed.
The rndis function driver appends each response for the
SEND_ENCAPSULATED_COMMAND in a queue. As the above process got corrupted,
the Host sends a REMOTE_NDIS_RESET_MSG command to do a soft-reset.
As the rndis response queue is not freed, the previous response is sent
as a part of this REMOTE_NDIS_RESET_MSG's reset response and the Host
block any more Rndis transfers.
Hence free the rndis response queue as a part of this soft-reset so that
the correct response for REMOTE_NDIS_RESET_MSG is sent properly during the
response command.

When reading synchronously from a non-zero endpoint, gadgetfs will
return -EFAULT even if the read succeeds, due to a bad check of the
copy_to_iter() return value.
This fix compares the return value of copy_to_iter to the amount of
bytes that was passed, and only fails if they are not the same.

For case 14 and case 21, their correct return value is the number
of bytes transferred, so it is a positive integer. But in usbtest_ioctl,
it takes non-zero as false return value for usbtest_do_ioctl, so
it will treat the correct test as wrong test, then the time on
tests will be the minus value.

This patch fixes an issue that isochronous transfer's data is possible to
be lost as a workaround. Since this driver uses a workqueue to start
the dmac, the transfer is possible to be delayed when system load is high.

This patch fixes an issue that unexpected BRDY interruption happens
when the usb_ep_{enable,disable}() are called with different direction.
In this case, the driver will cause the following message:
renesas_usbhs e6590000.usb: irq_ready run_error 1 : -16
This issue causes the followings:
1) A pipe is enabled as transmission
2) The pipe sent a data
3) The pipe is disabled and re-enabled as reception.
4) The pipe got a queue
Since the driver doesn't clear the BRDYSTS flags after 2) above, the issue
happens. If we add such clearing the flags into the driver, the code will
become complicate. So, this patch clears the BRDYSTS flag of reception in
usbhsg_ep_enable() to avoid complicate.

Since R-Car Gen3 SoC has the USB-DMAC, this driver should set
dparam->has_usb_dmac to 1. Otherwise, behavior of this driver and
the usb-dmac driver will be mismatch, then sometimes receiving data will
be corrupt.

DWC3 has one interesting peculiarity with chained
transfers. If we setup N chained transfers and we
get a short packet before processing all N TRBs,
DWC3 will (conditionally) issue a XferComplete or
XferInProgress event and retire all TRBs from the
one which got a short packet to the last without
clearing their HWO bits.
This means SW must clear HWO bit manually, which
this patch is doing.

Implemented queued response handling. This queue is processed every time the
WDM_READ flag is cleared.
In case of a read error, userspace may not actually read the data, since the
driver returns an error through wdm_poll. After this, the underlying device may
attempt to send us more data, but the queue is not processed. While userspace is
also blocked, because the read error is never cleared.
After this patch, we proactively process the queue on a read error. If there was
an outstanding response to handle, that will clear the error (or go through the
same logic again, if another read error occurs). If there was no outstanding
response, this will bring the queue size back to 0, unblocking a future response
from the underlying device.

1. kernel config
Followings are added.
USBIP_VHCI_HC_PORTS: Number of ports per USB/IP virtual host
controller. The default is 8 - same as current VHCI_NPORTS.
USBIP_VHCI_NR_HCS: Number of USB/IP virtual host controllers. The
default is 1. This paratmeter is replaced with USBIP_VHCI_INIT_HCS and
USBIP_VHCI_MAX_HCS included in succeeding dynamic extension patch.

vhci[.N] is shown for each host controller kobj. vhch.1, vhci.2, ...

of ports is acquired by reading status lines. Status is divided for

has 6 columns so there's no compatibility issue in this change.

Hans de Goede has reported a difficulty in the Linux port of libusb.
When a device is removed, the poll() system call in usbfs starts
returning POLLERR as soon as udev->state is set to
USB_STATE_NOTATTACHED, but the outstanding URBs are not available for
reaping until some time later (after usbdev_remove() has been called).
This is awkward for libusb or other usbfs clients, although not an
insuperable problem.
At any rate, it's easy to change usbfs so that it returns POLLHUP as
soon as the state becomes USB_STATE_NOTATTACHED but it doesn't return
POLLERR until after the outstanding URBs have completed. That's what
this patch does; it uses the fact that ps->list is always on the
dev->filelist list until usbdev_remove() takes it off, which happens
after all the outstanding URBs have been cancelled.

The size could be also clipped by something like:

In ehci_turn_off_all_ports() all EHCI port registers are cleared to zero.
On some hardware, this can lead to an system hang,
when ehci_port_power() accesses the already cleared registers.
This patch changes the order of cleanup.
First call ehci_port_power() which respects the current bits in
port status registers
and afterwards cleanup the hard way by setting everything to zero.

Erroneous or malicious endpoint descriptors may have non-zero bits in
reserved positions, or out-of-bounds values. This patch helps prevent
these from causing problems by bounds-checking the wMaxPacketValue
entries in endpoint descriptors and capping the values at the maximum
allowed.

It's a rather simple controller, we just need to make sure USB is
powered (using GPIO pin) and reset bus core. Once this is done it's

Digging into it it seems to be related to the problem described in the
commit message for cd998ded5c "i2c: designware: Prevent runtime
suspend during adapter registration" as scsi_add_host also calls
device_add and after the call to device_add the parent device is
suspended.
Fix this by using the approach from the mentioned commit and getting
the runtime pm reference before calling scsi_add_host.

The driver enforces a strict one-to-one relationship between the
received RESPONSE_AVAILABLE notifications and messages read from
the device. At the same time, it will cancel the interrupt URB
when there is no client holding the character device open.
Many devices do not cope well with this behaviour. They maintain
a FIFO queue of messages, and send notifications on a best effort
basis. Messages are queued regardless of whether the notification
is successful or not. So if the driver loses a single notification,
which can easily happen when the interrupt URB is cancelled, then
the device and driver becomes out-of-sync. New messages end up
at the end of the queue, while the associated notification makes
the driver read only the first message from the queue.
This state is permanent from a user point of view. There is no
no way to flush the device queue without resetting the device or
using another driver.
The problem is easy to hit with current QMI and MBIM command line
tools, which typically close the character device after seeing
the reply they expect. Any pending unsolicited messages from the
device will then trigger the driver bug.
Fix by always reading all queued messages from the device when
the notification URB is first submitted. This is expected to
end with an -EPIPE status when there are no more pending
messages, so demote the printk associated with -EPIPE to debug
level.
The workaround has been tested on a large number of different MBIM
and QMI devices, as well as the Ericsson F5521gw and H5321gw modems
with real Device Management functions.

alloc_ordered_workqueue replaces the deprecated
create_singlethread_workqueue.
There are multiple work items on the work queue, which require
ordering. Hence, an ordered workqueue has been used.
The workqueue "wq_otg" is not being used on a memory reclaim path.
Hence, WQ_MEM_RECLAIM has not been set.

The status workqueue is involved in initializing the Uxxx and polling
the Uxxx until a supported PCMCIA CardBus device is detected.
It then starts the command and respond workqueues and then loads the
module that handles the device, after which it just polls the Uxxx
looking for card ejects.
The command and respond workqueues are involved in implementing a command
sequencer for communicating with the firmware on the other side of
the FTDI chip in the Uxxx.
These workqueues have only a single work item each and hence they do not
require ordering. Also, none of the above workqueues are being used on a
memory recliam path. Hence, the singlethreaded workqueues have been
replaced with the use of system_wq.
System workqueues have been able to handle high level of concurrency
for a long time now and hence it's not required to have a singlethreaded
workqueue just to gain concurrency. Unlike a dedicated per-cpu workqueue
created with create_singlethread_workqueue(), system_wq allows multiple
work items to overlap executions even on the same CPU; however, a
per-cpu workqueue doesn't have any CPU locality or global ordering
guarantee unless the target CPU is explicitly specified and thus the
increase of local concurrency shouldn't make any difference.
The work items have been sync cancelled because they are self-requeueing
and need to wait for the in-flight work item to finish before proceeding
with destruction. Hence, they have been sync cancelled in
ftdi_status_cancel_work(), ftdi_command_cancel_work() and
ftdi_response_cancel_work(). These functions are called in
ftdi_elan_exit() to ensure that there are no pending work items while
disconnecting the driver.

This is a minimal driver to support bringing a usb4604 device
from microchip out of reset and into hub mode. The usb4604 device
is related to the usb3503 device, but it didn't seem close enough
to warrant putting both into the same file. This patch borrows
some of the usb3503 structure and trims it down to just handle
the optional reset gpio and adds the i2c command to put the
device into hub mode.

This patch fixes fives off-by-one bugs in the ftdi-elan driver code. The
bug can be triggered by plugging a USB adapter for CardBus 3G cards (model
U132 manufactured by Elan Digital Systems, Ltd), causing a kernel panic.
The fix was tested on Ubuntu 14.04.4 with 4.7.0-rc14.2.0-27-generic+ and
4.4.0-22-generic+ kernel. In the ftdi_elan_synchronize function, an
off-by-one memory corruption occurs when packet_bytes is equal or bigger
than m. After having read m bytes, that is bytes_read is equal to m, "
..\x00" is still copied to the stack variable causing an out bounds write
of 4 bytes, which overwrites the stack canary and results in a kernel
panic.
This off-by-one requires physical access to the machine. It is not
exploitable since we have no control on the overwritten data. Similar
off-by-one bugs have been observed in 4 other functions:
ftdi_elan_stuck_waiting, ftdi_elan_read, ftdi_elan_edset_output and
ftdi_elan_flush_input_fifo.

For case 14 and case 21, their correct return value is the number
of bytes transferred, so it is a positive integer. But in usbtest_ioctl,
it takes non-zero as false return value for usbtest_do_ioctl, so
it will treat the correct test as wrong test, then the time on
tests will be the minus value.

The locking in hub_activate() is not adequate to provide full mutual
exclusion with hub_quiesce(). The subroutine locks the hub's
usb_interface, but the callers of hub_quiesce() (such as
hub_pre_reset() and hub_event()) hold the lock to the hub's
usb_device.
This patch changes hub_activate() to make it acquire the same lock as
those other routines.

The early-exit pathway in hub_activate, added by commit e50293ef97
("USB: fix invalid memory access in hub_activate()") needs
improvement. It duplicates code that is already present at the end of
the subroutine, and it neglects to undo the effect of a
usb_autopm_get_interface_no_resume() call.
This patch fixes both problems by making the early-exit pathway jump
directly to the end of the subroutine. It simplifies the code at the
end by merging two conditionals that actually test the same condition
although they appear different: If type < HUB_INIT3 then type must be
either HUB_INIT2 or HUB_INIT, and it can't be HUB_INIT because in that
case the subroutine would have exited earlier.

Memory leak and unbalanced reference count:
If the hub gets disconnected while the core is still activating it, this
can result in leaking memory of few USB structures.
This will happen if we have done a kref_get() from hub_activate() and
scheduled a delayed work item for HUB_INIT2/3. Now if hub_disconnect()
gets called before the delayed work expires, then we will cancel the
work from hub_quiesce(), but wouldn't do a kref_put(). And so the
unbalance.

Deadlocks:
If the hub gets disconnected early enough (i.e. before INIT2/INIT3 are
finished and the init_work is still queued), the core may call
hub_quiesce() after acquiring interface device locks and it will wait
for the work to be cancelled synchronously. But if the work handler is
already running in parallel, it may try to acquire the same interface
device lock and this may result in deadlock.
Fix both the issues by removing the call to cancel_delayed_work_sync().

phy-sun4i-usb now has proper dr_mode handling, it always registers an
extcon, and sends a notify with the mode (even when in peripheral- /
host-only mode) at least once.
So we can simply the sunxi musb glue by always registering its extcon
notifier and relying on sunxi_musb_work() to enable vbus when in
host-only mode.
This also enables host- and peripheral-only mode with vbus monitoring.

This adds tracepoints to musb register read/write wrappers to get
trace log for register access.
The default tacepoint log prefix here would be musb_readX/writeX(),
which is not much helpful. So this patch let the tracepoints use
__buildin_return_address(0) to print the caller funciton name to
provide more context of the register access.

Make this a proper typed array. Drop the old allocate context code since
that is no longer used.
Note that the memops functions now get a struct device pointer instead of
the struct device ** that was there initially (actually a void pointer to
a struct containing only a struct device pointer).
This code is now a lot cleaner.

It's clear to see that the list_empty() check means it can never be
uninitialized, however it still looks wrong:
When ep->cancelled_td_list contains more than one entry, the
ep_ring variable will point to the ring that was retrieved
from the last urb, and we have to look it up again in the
second loop instead, which fixes the behavior and gets rid of the
warning too.

This patch fixes an issue that the CFIFOSEL register value is possible
to be changed by usbhsg_ep_enable() wrongly. And then, a data transfer
using CFIFO may not work correctly.

This patch fixes an issue that the xfer_work() is possible to cause
NULL pointer dereference if the usb cable is disconnected while data
transfer is running.
In such case, a gadget driver may call usb_ep_disable()) before
xfer_work() is actually called. In this case, the usbhs_pkt_pop()
will call usbhsf_fifo_unselect(), and then usbhs_pipe_to_fifo()
in xfer_work() will return NULL.

The driver selects NOP_USB_XCEIV, which can only be built-in if
USB_GADGET is either disabled or also built-in, so with USB_DWC2_PCI=y
and USB_GADGET=m, NOP_USB_XCEIV is also built-in and we get this link

The USB core contains a bug that can show up when a USB-3 host
controller is removed. If the primary (USB-2) hcd structure is
released before the shared (USB-3) hcd, the core will try to do a
double-free of the common bandwidth_mutex.
The problem was described in graphical form by Chung-Geol Kim, who

This happens because hcd_release() frees the bandwidth_mutex whenever
it sees a primary hcd being released (which is not a very good idea
in any case), but in the course of releasing the primary hcd, it
changes the pointers in the shared hcd in such a way that the shared
hcd will appear to be primary when it gets released.
This patch fixes the problem by changing hcd_release() so that it
deallocates the bandwidth_mutex only when the _last_ hcd structure
referencing it is released. The patch also removes an unnecessary
test, so that when an hcd is released, both the shared_hcd and
primary_hcd pointers in the hcd's peer will be cleared.

The Kconfig currently controlling compilation of this code is:

Lets remove the couple traces of modularity so that when reading the

inc_deq() is called both for rings with link trbs and the event ring
without link trbs.
The last_trb() check in inc_deq() has a off by one error, going beyond
allocated array when checking if trb == [TRBS_PER_SEGMENT], and the whole
inc_deq() depend on this.
Rewrite the inc_deq() funciton, remove the faulty last_trb() helper, add
new last_trb_on_seg() and last_trb_on_ring() helpers

If the last trb before a link is not packet size aligned, and is not
splittable then use a bounce buffer for that chunk of max packet size
unalignable data.
Allocate a max packet size bounce buffer for every segment of a bulk
endpoint ring at the same time as allocating the ring.
If we need to align the data before the link trb in that segment then
copy the data to the segment bounce buffer, dma map it, and enqueue it.
Once the td finishes, or is cancelled, unmap it.
For in transfers we need to first map the bounce buffer, then queue it,
after it finishes, copy the bounce buffer to the original sg list, and
finally unmap it

TD fragments section 4.11.7.1 in xhci specs have additional requirements
on how trbs in TDs must be organized.
TD fragments shall not span transfer ring segments and TD fragments must
be packet aligned. Normally we don't care about TD fragments, on TD is one
big fragment, but if a TD spans ring segments it will be treated as two
fragments, and we need to comply with the alignment requirements.
For us this means that the payload data must be packet aligned in the
last trb before a link trb.
In most mass storage bulk tranfers we are lucky as the block size aligns
nicely with packet size, and there are no issues.
However, usb network adapters using scatterlists can hit this alignment
issue, and usbtest in kernel triggers this in minutes.
This patch is a partial solution, it solves the easy case when the last
trb before the link trb contains a packet boundary.
If that is the case then just split the trb at the boundary.
If not, then just print a debug message and continue as we have always
done, hoping for the best

Queue trbs until all payload data in the urb is tranferred.
The actual number of trbs might need to change from the pre-calculated
number when the packet alignment restrictions for td fragments in
xhci 4.11.7.1 are taken into account.
Long term plan is to get rid of calculating the needed trbs in advance
all together. It's an unnecessary extra walk through the scatterlist.
This change also allows some bulk queue function simplifications

We only need to know if we are queuing the last trb for a TD when
calculating the td remainder field.
The total number of trbs left is not used.
We won't be able to trust the pre-calculated number of trbs used if we
need to align trb data by splitting or merging trbs in order to satisfy
comply with data alignment requirements in xhci specs section 4.11.7.1.

If a zero-length packet is needed after a bulk transfer, then an
additional zero length TD was prepared before enqueueing the bulk transfer
This set up the zero packet TD structure with incorrect td->start_seg
and td->first_trb pointers.
Prepare the zero packet TD after the data bulk TD is enqueued instead.
It sets these pointers correctly.
This change also simplifies unnecessary complexity related to keeping
track of the last trb when enqueuing trbs.

The above commit reordered spin_lock/unlock and now `&dev->lock' is acquired
(rather than released) before calling `dev->driver->disconnect',
`dev->driver->setup', `dev->driver->suspend', `usb_gadget_giveback_request', and
`usb_gadget_udc_reset'.
But this *may* not be the right way to fix the problem pointed by d3cb25a121.
Note that the other usb/gadget/udc drivers do release the lock before calling
these functions. There are also inconsistencies within pch_udc.c, where
`dev->driver->disconnect' is called while holding `&dev->lock' in lines 613 and
1184, but not in line 2739.
Finally, commit d3cb25a121 may have introduced several potential deadlocks.

f_fs rounds up read(2) requests to a multiple of a max packet size
which means that host may provide more data than user has space for.
So far, the excess data has been silently ignored.
This introduces a buffer for a tail of such requests so that they are
returned on next read instead of being ignored.

Add a pr_err when host sent more data then the size of the buffer user
space gave us. This may happen on UDCs which require OUT requests to
be aligned to max packet size. The patch includes a description of the
situation.

Implementations might use different IRQs for
host, gadget so use named interrupt resources
to allow device tree to specify the interrupts.
Following are the interrupt names
Peripheral Interrupt - peripheral
HOST Interrupt - host
Maintain backward compatibility for a single named
interrupt ("dwc3_usb3") for all interrupts as well as
unnamed interrupt at index 0 for all interrupts.
As platform_get_irq() variants are used, tackle
the -EPROBE_DEFER case as well.

Some SoCs have a single phy-hw-block with multiple phys, this is
modelled by a single phy dts node, so we end up with multiple
controller nodes with a phys property pointing to the phy-node
of the otg-phy.
Only one of these controllers typically is an otg controller, yet we
were checking the first controller who uses a phy from the block and
then end up looking for a dr_mode property in e.g. the ehci controller.
This commit fixes this by adding an arg0 parameter to
of_usb_get_dr_mode_by_phy and make of_usb_get_dr_mode_by_phy
check that this matches the phandle args[0] value when looking for
the otg controller.

Reimplemented EP disabled interrupt handler and moved to
corresponding function.
This interrupt indicates that the endpoint has been disabled per
the application's request.
For IN endpoints flushes txfifo, in case of BULK clears DCTL_CGNPINNAK,
in case of ISOC completes current request.
For ISOC-OUT endpoints completes expired requests. If there is
remaining request starts it. This is the part of ISOC-OUT transfer
drop flow. When ISOC-OUT transfer expired we must disable ep to drop
ongoing transfer.

Incomplete ISO IN interrupt indicates one of the following conditions
occurred while transmitting an ISOC transaction.
- Corrupted IN Token for ISOC EP.
- Packet not complete in FIFO.
Incomplete ISO OUT indicates that there is at least one isochronous OUT
endpoint on which the transfer is not completed in the current
microframe.
The following actions will be taken:
In case of EP-IN
- Determine the EP
- Disable EP directly from this handler; when "Endpoint Disabled"
interrupt is received flush FIFO
In case of EP-OUT
- Determine the EP
- If target frame elapsed set DCTL_SGOUTNAK, unmask GOUTNAKEFF and
proceed as described in section 7.5.1 of DWC-HSOTG Programming Guide
Also added dwc2_gadget_target_frame_elapsed() helper function which
will be used in Incomplete ISO IN/OUT Interrupt handlers.

NAKINTRPT interrupt is starting point for isoc-in transfer,
synchronization done with first in token received from host,
core asserts this interrupt when responds with 0 length data
to in token, received from host.
The first IN token is asynchronous for device - device does not
know when first one token will arrive from host. On first token
arrival HW generates 2 interrupts: 'in token received while FIFO
empty' and 'NAK'. NAK interrupt for ISOC in means that token has
arrived and ZLP was sent in response to that as there was no data
in FIFO. SW is basing on this interrupt to obtain frame in which
token has come and then based on the interval calculates next
frame for transfer.
OUTTKNEPDIS interrupt is starting point for isoc-out transfer,
synchronization done with first out token received from host
while corresponding ep is disabled.
For OUTs the reason is same - device does not know initial frame
in which out token will come. For this HW generates OUTTKNEPDIS
- out token is received while EP is disabled. Upon getting this
interrupt SW starts calculation for next transfer frame.

According DWC-OTG databook, "GOUTNakEff" is read only and can be
cleared only by "DCTL.CGOUTNak", but here we do not need to clear
it because DWC-OTG programming guide says that before disabling
any OUT endpoint, the application must enable Global OUT NAK mode,
so if this mode is enabled we can continue without this step.

Because of recent changes to transfer handling on
DWC3, we will not get XferComplete unless we
completely fill up our TRB ring. This means that we
might get a Reset or Disconnect without getting a
XferComplete first.
In order to correctly release our allocated Transfer
Resource, we must issue ENDTRANSFER command whenever
dep->resource_index is valid.

If there is a failure after pm_runtime_enable/get_sync()
we need to call pm_runtime_disable/put_sync().
Otherwise it will lead to an unbalanced pm_runtime_enable() on the
subsequent probe if the earlier probe bailed out due to -EPROBE_DEFER.
pm_runtime_get_sync() can fail as well so deal with that case too.

it's clear now that when is_on=true, we must loop
until DWC3_DSTS_DEVCTRLHLT clears; while when
is_on=false we must loop until DWC3_DSTS_DEVCTRLHLT
gets set.
Instead of adding actual if() statements, we can
rely on XOR operation to evaluate to true only when
the above conditions apply. Then, we can move the
break condition back to the while() statement
together with our timeout check and the resulting
code is very compact and simpler to read.

It's know that Intel's SoCs' dwc3 integration is
peripheral-only since Intel implements its own
portmux for role-swapping. In order to prevent dwc3
from ever registering and XHCI platform_device,
let's just set dr-mode to peripheral-only on Intel
SoCs.

This new set of tracepoints will help all gadget
drivers and UDC drivers when problem appears. Note
that, in order to be able to add tracepoints to
udc-core.c we had to rename that to core.c and
statically link it with trace.c to form
udc-core.o. This is to make sure that module name
stays the same.

instead of defining all functions as static inlines,
let's move them to udc-core and export them with
EXPORT_SYMBOL_GPL, that way we can make sure that
only GPL drivers will use them.
As a side effect, it'll be nicer to add tracepoints
to the gadget API.
While at that, also fix Kconfig dependencies to
avoid randconfig build failures.

Don't make any decisions regarding VBUS session based on ID
status. That is best left to the OTG core.
Pass ID and VBUS events independent of each other so that OTG
core knows exactly what to do.
This makes dual-role with extcon work with OTG irq on OMAP platforms.

We intend to share this interrupt with the OTG driver an to ensure
that irqflags match for the shared interrupt handlers we use
request_threaded_irq()
If we don't use request_treaded_irq() then forced threaded irq will
set IRQF_ONESHOT and this won't match with the OTG IRQ handler's
IRQ flags.
NOTE: OTG IRQ handler is yet to be added. This is a preparatory step.

GUCTL1 reg has some useful functions which can be
written by user. For rockchip platform, we set
GUCTL1.DEV_FORCE_20_CLK_FOR_30_CLK (bit26, applicable
for the core is programmed to operate in 2.0 device
only) to 1 in bootrom, and after start the kernel,
we want to check whether this bit can be reset to
default 0 after the core reset. Dump GUCTL1 reg from
debugfs is more convenient for us.

If the trb->enqueue == trb->dequeue, then it could be full or empty.
This could also happen at TRB index 0, so modify the check to handle
that condition. At index 0, the previous TRB is the one just before the
link TRB.

The current calculation takes dep->trb_dequeue - dep->trb_enqueue to
find the TRB space left.
If you enqueue 1, that results in:
(u8) 0 - (u8) 1 = 0xff = 255 TRBs left.
This is correct if DWC3_TRB_NUM == 256.
If DWC3_TRB_NUM is less than 256 (but still a power of 2) you need to
mod the result by DWC3_TRB_NUM.
For example the same calculation with DWC3_TRB_NUM = 8, results in:
255 % 6 = 7 TRBs left.

this patch implements the most basic pm_runtime
support for dwc3. Whenever USB cable is dettached,
then we will allow core to runtime_suspend.
Runtime suspending will involve completely tearing
down event buffers and require a full soft-reset of
the IP.
Note that a further optimization could be
implemented once we decide to support hibernation,
which is to allow runtime_suspend with cable
connected when bus is in U3. That's subject to a
separate patch, however.

when we call dwc3_gadget_giveback(), we end up
releasing our controller's lock. Another thread
could get scheduled and disable the endpoint,
subsequently setting dep->endpoint.desc to NULL.
In that case, we would end up dereferencing a NULL
pointer which would result in a Kernel Oops. Let's
avoid the problem by simply returning early if we
have a NULL descriptor.

commit f3af36511e ("usb: dwc3: gadget: always
enable IOC on bulk/interrupt transfers") ended up
regressing Isochronous endpoints by clearing
DWC3_EP_BUSY flag too early, which resulted in
choppy audio playback over USB.
Fix that by partially reverting original commit and
making sure that we check for isochronous endpoints.

As a micro-power optimization, let's only resume the
USB2 PHY if we're working on <=HIGHSPEED. If we're
gonna work on SUPERSPEED or SUPERSPEED+, there's no
point in resuming the USB2 PHY.

The idea of this patch is for dwc3_core_init() to
abstract all the details about how to initialize
dwc3 and dwc3_core_exit() to do the same for
teardown.
With this, we can simplify suspend/resume operations
by a large margin and always know that we're going
to start dwc3 from a known starting point.

By adding a pointer to endpoint registers' base
address, we can avoid using our controller-wide
struct dwc3 pointer for everything. At some point
this will allow us to have per-endpoint locks which
will, in turn, let us queue requests to separate
endpoints in parallel.
Because of this change our debugfs interface and io
accessors need to be changed accordingly.

In all call sites of dwc3_send_gadget_ep_cmd() we
already had a valid dep pointer, so instead of
passing dwc and dep->number, which would be used to
fetch the same pointer we already had, just pass dep
directly.
In other words, we're changing:
struct dwc3_ep *dep = dwc[dep->number];
to just passing struct dwc3_ep *dep as argument.

Instead of returning -EINVAL when someone calls
__dwc3_gadget_wakeup() in speeds > highspeed, let's
return 0. There are no problems for the driver for
calling it in superspeed as we cleanly just return.
This avoids an annoying WARN_ONCE() always
triggering during superspeed enumeration with LPM
enabled.

If we're updating transfers, we can also prepare as
many TRBs as we can fit in the ring. Let's start
doing that.
This patch 'solves' a limitation of how many TRBs we
can prepare when we're getting close the end of the
ring. Instead driver to prepare only up to end of
the ring, we check if we have space to wrap around
the ring properly.
Note that this only happens when our enqueue and
dequeue pointers are equal (which is the case for
bulk endpoints after an XferComplete event).

Instead of trying hard to stay connected to the
host, it's best (and far easier) to disconnect from
the host already.
Anything relying on KEEP_CONNECT will just have that
ignored, but we don't have proper hibernation
implementation yet, so there are no regressions.
In any case, hibernation is only useful for runtime
PM, not system sleep.
While at that, also remove dwc3.dcfg which has been
rendered unnecessary.

USB338x but without the USB3.0 superspeed support.

With a default size of 16kiB and with maximum of 32
buffers, we can transfer up to 512kiB, however Linux
can transfer up to 1MiB in a single mass storage
block transfer to USB3 storage devices.
Because of this, 1MiB block transfers end up being
slower than 512kiB block transfers. Let's increase
maximum number of storage buffers to a ridiculous
amount (256) so that anybody wanting to test maximum
achievable throughput can do so.

The USB LED driver exposes a undocumented sysfs interface and doesn't
use the standard kernel LED subsystem. It supports three devices:
Delcom Visual Signal Indicator
The driver supports generation 1 of the device only which was
manufactured until 2008. Remove support for this device completely.
Riso Kagaku RGB LED + Dream Cheeky Webmail Notifier
These devices are HID compliant and are supported by a new USB LED
driver under drivers/hid utilizing the kernel LED subsystem.
So let's remove the old USB LED driver.

Stop using the return value of platform_device_register_full() to get to
the struct musb in sunxi_musb_work(). If a gadget has been registered
(insmod-ed) before the musb driver, then musb_start will get called
from the musb_core probe function and sunxi_musb_work() may run before
platform_device_register_full() has returned.
Instead store a pointer to struct musb in struct sunxi_glue when
sunxi_musb_enable gets called. Note that sunxi_musb_enable always gets
called before sunxi_musb_work() can run.

Currently the Linux PCI core does not touch power state of PCI bridges and
PCIe ports when system suspend is entered. Leaving them in D0 consumes
power unnecessarily and may prevent the CPU from entering deeper C-states.
With recent PCIe hardware we can power down the ports to save power given
that we take into account few restrictions:
- The PCIe port hardware is recent enough, starting from 2015.
- Devices connected to PCIe ports are effectively in D3cold once the port
is transitioned to D3 (the config space is not accessible anymore and
the link may be powered down).
- Devices behind the PCIe port need to be allowed to transition to D3cold
and back. There is a way both drivers and userspace can forbid this.
- If the device behind the PCIe port is capable of waking the system it
needs to be able to do so from D3cold.
This patch adds a new flag to struct pci_device called 'bridge_d3'. This
flag is set and cleared by the PCI core whenever there is a change in power
management state of any of the devices behind the PCIe port. When system
later on is suspended we only need to check this flag and if it is true
transition the port to D3 otherwise we leave it in D0.
Also provide override mechanism via command line parameter
"pcie_port_pm=[off|force]" that can be used to disable or enable the
feature regardless of the BIOS manufacturing date.

Add support for shared platform controllers by using
devm_reset_control_get_shared_by_index instead of
of_reset_control_get_by_index.
Note we use the devm function because there is no
of_reset_control_get_shared_by_index, this also leads
to a nice cleanup of the cleanup code.
This brings the ehci-platform reset handling code inline
with ohci-platform.

At least the EHCI/OHCI found on the Allwinnner H3 SoC needs multiple
reset lines, the controller will not initialize while the reset for
its companion is still asserted, which means we need to de-assert
2 resets for the controller to work.

In order to the save power consumption, as a workaround, suspend
forcibly the USB PORTA/B/C via set the SUSPEND_A/B/C bits of OHCI
Interrupt Configuration Register in the SFRs while OHCI USB suspend.
This suspend operation must be done before the USB clock is disabled,
resume after the USB clock is enabled.

stub_disconnect() calls stub_device_reset() during usb_unbind_device() when
usb device is locked. So usb_lock_device_for_reset() in stub_device_reset()
in that case polls for one second and returns -EBUSY anyway.

Some ehci compatible controllers have more than one reset signal lines,
e.g., Synopsys DWC USB2.0 Host-AHB Controller has two resets hreset_i_n
and phy_rst_i_n. Two more resets are added in this patch in order for
this kind of controller to use this driver directly.

Starting with commit 0b52297f22 ("reset: Add support for shared reset
controls") there is a reference count for reset control assertions. The
goal is to allow resets to be shared by multiple devices and an assert
will take effect only when all instances have asserted the reset.
In order to preserve backwards-compatibility, all reset controls become
exclusive by default. This is to ensure that reset_control_assert() can
immediately assert in hardware.
However, this new behaviour triggers the following warning in the EHCI
driver for Tegra:

The reason is that Tegra SoCs have three EHCI controllers, each with a
separate reset line. However the first controller contains UTMI pads
configuration registers that are shared with its siblings and that are
reset as part of the first controller's reset. There is special code in
the driver to assert and deassert this shared reset at probe time, and
it does so irrespective of which controller is probed first to ensure
that these shared registers are reset before any of the controllers are
initialized. Unfortunately this means that if the first controller gets
probed first, it will request its own reset line and will subsequently
request the same reset line again (temporarily) to perform the reset.
This used to work fine before the above-mentioned commit, but now
triggers the new WARN.
Work around this by making sure we reuse the controller's reset if the
controller happens to be the first controller.

There are three EHCI controllers on Tegra SoCs, each with its own reset
line. However, the first controller contains a set of UTMI configuration
registers that are shared with its siblings. These registers will only
be reset as part of the first controller's reset. For proper operation
it must be ensured that the UTMI configuration registers are reset
before any of the EHCI controllers are enabled, irrespective of the
probe order.
Commit a47cc24cd1 ("USB: EHCI: tegra: Fix probe order issue leading to
broken USB") introduced code that ensures the first controller is always
reset before setting up any of the controllers, and is never again reset
afterwards.
This code, however, grabs the wrong reset. Each EHCI controller has two
reset controls attached: 1) the USB controller reset and 2) the UTMI
pads reset (really the first controller's reset). In order to reset the
UTMI pads registers the code must grab the second reset, but instead it
grabbing the first.

parport subsystem has introduced parport_del_port() to delete a port
when it is going away. Without parport_del_port() the registered port
will not be unregistered.
To reproduce and verify the error:
Command to be used is : ls /sys/bus/parport/devices
1) without the device attached there is no output as there is no
registered parport.
2) Attach the device, and the command will show "parport0".
3) Remove the device and the command still shows "parport0".
4) Attach the device again and we get "parport1".
With the patch applied:
1) without the device attached there is no output as there is no
registered parport.
2) Attach the device, and the command will show "parport0".
3) Remove the device and there is no output as "parport0" is now
removed.
4) Attach device again to get "parport0" again.

Since ed_schedule begins with marking the ED as "operational",
the ED may be left in such state even if scheduling actually
fails.
This allows future submission attempts to smuggle this ED to the
hardware behind the scheduler's back and without linking it to
the ohci->eds_in_use list.
The former causes bandwidth saturation and data loss on isoc
endpoints, the latter crashes the kernel when attempt is made
to unlink such ED from this list.
Fix ed_schedule to update ED state only on successful return.

Ensure that the endpoint is stopped by clearing REQPKT before
clearing DATAERR_NAKTIMEOUT before rotating the queue on the
dedicated bulk endpoint.
This addresses an issue where a race could result in the endpoint
receiving data before it was reprogrammed resulting in a warning
about such data from musb_rx_reinit before it was thrown away.
The data thrown away was a valid packet that had been correctly
ACKed which meant the host and device got out of sync.

shared_fifo endpoints would only get a previous tx state cleared
out, the rx state was only cleared for non shared_fifo endpoints
Change this so that the rx state is cleared for all endpoints.
This addresses an issue that resulted in rx packets being dropped
silently.

If the session bit was not set in the backup of devctl register,
restoring devctl would clear the session bit. Therefor, only restore
devctl register when the session bit was set in the backup.
This solves the device enumeration failure in otg mode exposed by commit
56f487c (PM / Runtime: Update last_busy in rpm_resume).

At least on n900 we have phy-twl4030-usb only generating cable
interrupts, and then have a separate USB PHY.
In order for musb to know the real cable status, we need to
clear any cached state until musb is ready. Otherwise the cable
status interrupts will get just ignored if the status does
not change from the initial state.
To do this, let's add a return value to musb_mailbox(), and
reset cached linkstat to MUSB_UNKNOWN on error. Sorry to cause
a bit of churn here, I should have added that already last time
patching musb_mailbox().

At least 2430 glue layer pulls d+ high on start up even if there are
no gadgets configured. This is bad at least for anything using a separate
battery charger chip as it can confuse the charger detection.
Let's fix the issue by removing the bogus glue layer code tinkering
with the SESSION bit. As pointed out Bin Liu <b-liu@ti.com> and
Sergei Shtylyov <sergei.shtylyov@cogentembedded.com>, the SESSION
bit just starts host mode if ID pin is grounded, and starts the
srp is ID pin is floating. So without the ID pin changing, it's
unable to force musb mode to anything. And just for starting a
host mode, things work fine without this code.

We can now rely only on PM runtime and cable detection.
The other glue layers can probably remove try_idle too, but that

We may have drivers loaded but no configured gadgets and MUSB may be in
host mode. If gadgets are configured during host mode, PM runtime will
get confused.
Disable PM runtime from gadget state, and do it based on the cable
and last state.
Note that we may get multiple cable events, so we need to keep track
of the power state.

The conditional use of PM runtime does not work properly
for musb gadget. On cable disconnect we may not get any
USB_EVENT_NONE leaving the PM runtime call unpaired.
Let's fix the issue by making sure the PM runtime calls are
paired within the functions. The glue layer will take care
of the rest.

Let's not tinker with the PM runtime of musb core from the omap2430
wrapper. This allows us to initialize PM runtime for musb core later
on instead of doing it in stages. And omap2430 wrapper has no need
to for accessing musb core at this point.
Note that this does not remove all the PM runtime calls from the
glue layer, those will get removed in a later patch.

Let's make the PM runtime use the standard autosuspend calls.
Commit 5de85b9d57 ("PM / runtime: Re-init runtime PM states at probe
error and driver unbind") means we must pair use_autosuspend with
dont_use_autosuspend and then use put_sync to properly idle the
device.
Note that we'll be removing the PM runtime calls from the glue
layer to the MUSB core in the next patch. And we can also remove
the pointless FIXME comment now.

Looks like at least 2430 glue won't idle reliably with the 200 ms
autosuspend delay. This causes deeper idle states being blocked for
the whole SoC when disconnecting OTG A cable.
Increasing the delay to 500 ms seems to idle both MUSB and the PHY
reliably. This is probably because of time needed by the hardware
based negotiation between MUSB and the PHY.

When the board is powering attached usb devices via the otg port
sometimes / on some devices it takes slightly too long for the Vbus
detection code in phy-sun4i-usb.c to signal that Vbus is high after
enabling Vbus and the musb hardware signals a MUSB_INTR_VBUSERROR
interrupt.
This commit sets the otg state to A_WAIT_VRISE upon enabling Vbus
making musb_stage0_irq() ignore the first VBUSERR_RETRY_COUNT
VBUSERROR interrupts, fixing connection issues in these cases.

urb->status is set when endpoint csr RXSTALL, H_ERROR, DATAERROR or
INCOMPRX bit is set. Those bits mean a broken pipe, so don't start next
urb when any of these bits is set by checking urb->status.
To minimize the risk of regression, only do so for RX, until we have a
test case to understand the behavior of TX.
The patch fixes system freeze issue caused by repeatedly invoking RX ISR
while removing a usb uart device connected to a hub, in which case the
hub has no chance to report the disconnect event due to the kernel is
busy in processing the RX interrupt flooding.

I got one of these cards for testing uas with, it seems that with streams
it dma-s all over the place, corrupting memory. On my first tests it
managed to dma over the BIOS of the motherboard somehow and completely
bricked it.
Tests on another motherboard show that it does work with streams disabled.

Several people have reported that UBSAN doesn't like the pointer
arithmetic in ehci_hub_control():

u32 __iomem *hostpc_reg = &ehci->regs->hostpc[(wIndex & 0xff) - 1];
If wIndex is 0 (and it often is), these calculations underflow and
UBSAN complains.
According to the C standard, pointer computations leading to locations
outside the bounds of an array object (other than 1 position past the
end) are undefined. In this case, the compiler would be justified in
concluding the wIndex can never be 0 and then optimizing away the
tests for !wIndex that occur later in the subroutine. (Although,
since ehci->regs->port_status and ehci->regs->hostpc are both 0-length
arrays and are thus GCC extensions to the C standard, it's not clear
what the compiler is really allowed to do.)
At any rate, we can avoid all these difficulties, at the cost of
making the code slightly longer, by not decrementing the index when it
is equal to 0. The runtime effect is minimal, and anyway
ehci_hub_control() is not on a hot path.

Commit 198de51dbc ("USB: uas: Limit qdepth at the scsi-host level")
removed the scsi_change_queue_depth() call from uas_slave_configure()
assuming that the slave would inherit the host's queue_depth, which
that commit sets to the same value.
This is incorrect, without the scsi_change_queue_depth() call the slave's
queue_depth defaults to 1, introducing a performance regression.
This commit restores the call, fixing the performance regression.

The Acer C120 LED Projector is a USB-3 connected pico projector which
takes both its power and video data from USB-3.
In combination with some hubs this device does not play well with
lpm, so disable lpm for it.

On some platforms, the clocks might be registered by a platform
driver. When this is the case, the clock platform driver may very well
be probed after xhci-plat, in which case the first probe() invocation
of xhci-plat will receive -EPROBE_DEFER as the return value of
devm_clk_get().
The current code handles that as a normal error, and simply assumes
that this means that the system doesn't have a clock for the XHCI
controller, and continues probing without calling
clk_prepare_enable(). Unfortunately, this doesn't work on systems
where the XHCI controller does have a clock, but that clock is
provided by another platform driver. In order to fix this situation,
we handle the -EPROBE_DEFER error condition specially, and abort the
XHCI controller probe(). It will be retried later automatically, the
clock will be available, devm_clk_get() will succeed, and the probe()
will continue with the clock prepared and enabled as expected.
In practice, such issue is seen on the ARM64 Marvell 7K/8K platform,
where the clocks are registered by a platform driver.

If commands timeout we mark them for abortion, then stop the command
ring, and turn the commands to no-ops and finally restart the command
ring.
If the host is working properly the no-op commands will finish and
pending completions are called.
If we notice the host is failing, driver clears the command ring and
completes, deletes and frees all pending commands.
There are two separate cases reported where host is believed to work
properly but is not. In the first case we successfully stop the ring
but no abort or stop command ring event is ever sent and host locks up.
The second case is if a host is removed, command times out and driver
believes the ring is stopped, and assumes it will be restarted, but
actually ends up timing out on the same command forever.
If one of the pending commands has the xhci->mutex held it will block
xhci_stop() in the remove codepath which otherwise would cleanup pending
commands.
Add a check that clears all pending commands in case host is removed,
or we are stuck timing out on the same command. Also restart the
command timeout timer when stopping the command ring to ensure we
recive an ring stop/abort event.

Under stress occasions some TI devices might not return early when
reading the status register during the quirk invocation of xhci_irq made
by usb_hcd_pci_remove. This means that instead of returning, we end up
handling this interruption in the middle of a shutdown. Since
xhci->event_ring has already been freed in xhci_mem_cleanup, we end up
accessing freed memory, causing the Oops below.
commit 8c24d6d7b0 ("usb: xhci: stop everything on the first call to
xhci_stop") is the one that changed the instant in which we clean up the
event queue when stopping a device. Before, we didn't call
xhci_mem_cleanup at the first time xhci_stop is executed (for the shared
HCD), instead, we only did it after the invocation for the primary HCD,
much later at the removal path. The code flow for this oops looks like
this:

The fix modifies xhci_stop to only cleanup the xhci data when releasing
the primary HCD. This way, we still have the event_queue configured
when invoking xhci_irq. We still halt the device on the first call to
xhci_stop, though.
I could reproduce this issue several times on the mainline kernel by
doing a bind-unbind stress test with a specific storage gadget attached.
I also ran the same test over-night with my patch applied and didn't
observe the issue anymore.

As of core revision 2.60a the recommended programming model is to set
the ClearPendIN bit when issuing a Clear Stall EP command for IN
endpoints. This is to prevent an issue where some (non-compliant) hosts
may not send ACK TPs for pending IN transfers due to a mishandled error

Set USB3_FORCE_VBUSVALID when configured for USB_DR_MODE_PERIPHERAL
mode, as it is required to have a working setup.
This worked on the internal driver by relying on the reset
value of the syscfg register as the bits aren't explicity cleared
and set like the upstream driver.
Also add a comment about what setting this bit means.

In OS descriptors handling, if ctrl->bRequestType is
USB_RECIP_DEVICE and w_index != 0x4 or (w_value >> 8)
is true, it will not assign a valid value to req->length,
but use the default value(-EOPNOTSUPP), and queue an
OS desc request with the invalid req->length. It always
happens on the platforms which use os_desc (for example:
rk3366, rk3399), and cause kernel panic as follows

If c->cdev->use_os_string flag is not set,
don't need to invoke ffs_do_os_descs() in _ffs_func_bind.
So uninitialized ext_compat_id pointer won't be accessed by
__ffs_func_bind_do_os_desc to cause kernel panic.

A patch that went into Linux-4.4 to fix big-endian mode on a Lantiq
MIPS system unfortunately broke big-endian operation on PowerPC
APM82181 as reported by Christian Lamparter, and likely other
systems.
It actually introduced multiple issues:
- it broke big-endian ARM kernels: any machine that was working
correctly with a little-endian kernel is no longer using byteswaps
on big-endian kernels, which clearly breaks them.
- On PowerPC the same thing must be true: if it was working before,
using big-endian kernels is now broken. Unlike ARM, 32-bit PowerPC
usually uses big-endian kernels, so they are likely all broken.
- The barrier for dwc2_writel is on the wrong side of the __raw_writel(),
so the MMIO no longer synchronizes with DMA operations.
- On architectures that require specific CPU instructions for MMIO
access, using the __raw_ variant may turn this into a pointer
dereference that does not have the same effect as the readl/writel.
This patch is a simple revert for all architectures other than MIPS,
in the hope that we can more easily backport it to fix the regression
on PowerPC and ARM systems without breaking the Lantiq system again.
We should follow this up with a more elaborate change to add runtime
detection of endianness, to make sure it also works on all other
combinations of architectures and implementations of the usb-dwc2
device. That patch however will be fairly large and not appropriate
for backports to stable kernels.
Felipe suggested a different approach, using an endianness switching
register to always put the device into LE mode, but unfortunately
the dwc2 hardware does not provide a generic way to do that. Also,
I see no practical way of addressing the problem more generally by
patching architecture specific code on MIPS.

Current __ffs_data_do_os_desc() of f_fs.c will check reserved1 field
of OS_DESC_EXT_COMPAT and return -EINVAL if it's 1.
But MS OS 1.0 Descriptors

The gadget API function usb_ep_set_halt() expects the gadget to return
-EAGAIN if the ep is active. Add support for this behavior.
Otherwise this may break mass storage protocol if a STALL is attempted
on the endpoint.

dwc3-exynos has two problems during init if the regulators are slow
to come up (for instance if the I2C bus driver is not on the initramfs)
and return probe deferral. First, every time this happens, the driver
leaks the USB phys created; they need to be deallocated on error.
Second, since the phy devices are created before the regulators fail,
this means that there's a new device to re-trigger deferred probing,
which causes it to essentially go into a busy loop of re-probing the
device until the regulators come up.
Move the phy creation to after the regulators have succeeded, and also
fix cleanup on failure. On my ODROID XU4 system (with Debian's initramfs
which doesn't contain the I2C driver), this reduces the number of probe
attempts (for each of the two controllers) from more than 2000 to eight.

By default user could store only valid UDC name in configfs UDC
attr by doing:
echo $UDC_NAME > UDC
Commit (855ed04 "usb: gadget: udc-core: independent registration of
gadgets and gadget drivers") broke this behavior and allowed to store
any arbitrary string in UDC file and udc core was waiting for such
controller to appear.
echo "any arbitrary string here" > UDC
This commit fix this by adding a flag which prevents configfs
gadget from being added to list of pending drivers if UDC with
given name has not been found.

Driver updates for ARM SoCs, these contain various things that touch
the drivers/ directory but got merged through arm-soc for practical
reasons. For the most part, this is now related to power management
controllers, which have not yet been abstracted into a separate
subsystem, and typically require some code in drivers/soc or arch/arm
to control the power domains.
Another large chunk here is a rework of the NVIDIA Tegra USB3.0

The ohci-platform driver can control the clock, while usb-nop-xceiv
as the PHY can control the vbus regulator. So this JZ4740-specific

Turns out the template and thus many drivers got the return value wrong:
0 means the fabrics driver needs to put a session reference, which no
driver except for the iSCSI target drivers did. Fortunately none of these
drivers supports explicit Node ACLs, so the bug was harmless.
Even without that only qla2xxx and iscsi every did real work in
shutdown_session, so get rid of the boilerplate code in all other
drivers.

Due to a missing upper bound, invalid minor numbers could be assigned to
ports. Such devices would later fail to register, but let's catch this
early as intended and avoid having devices with only a subset of their
ports registered (potentially the empty set).

The firmware of R-Car USB 3.0 host controller will control the reset.
So, if the xhci driver doesn't do firmware downloading (e.g. kernel
configuration is CONFIG_USB_XHCI_PLATFORM=y and CONFIG_USB_XHCI_RCAR
is not set), the reset of USB 3.0 host controller doesn't work
correctly. Then, the host controller will cause long wait in
xhci_reset() because the CMD_RESET bit of op_regs->command is not
cleared for 10 seconds.
So, this patch modifies the Kconfig to enable both CONFIG_USB_XHCI_PLATFORM
and CONFIG_USB_XHCI_RCAR.

The CRTSCTS flag code cleared (and inconsistently) bits unrelated to
CRTSCTS functionality. It was also harder than necessary to read.

A bug in the CRTSCTS handling caused RTS to alternate between
CRTSCTS=0 => "RTS is transmit active signal" and
CRTSCTS=1 => "RTS is used for receive flow control"
instead of
CRTSCTS=0 => "RTS is statically active" and
CRTSCTS=1 => "RTS is used for receive flow control"
This only happened after first having enabled CRTSCTS.
