Under uAP mode mwifiex may stop all net tx queues on forwarding
packets. This may stop some tx queues and they never have chance
to be waked up. There is also no need to check tx_pending and
stop queues here. Because local host has such kind of check when
transmitting packets and it's not proper to have forwarding affect
local transmitting.

Steinar reported reallocations of skb->head with IPv6, leading to
a warning in skb_try_coalesce()
It turns out iwl3945 has several problems :
1) skb->truesize is underestimated.
We really consume PAGE_SIZE bytes for a fragment,
not the frame length.
2) 128 bytes of initial headroom is a bit low and forces reallocations.
3) We can avoid consuming a full page for small enough frames.

During tear down (e.g. mwifiex_sdio_remove during system suspend),
mwifiex left IRQs enabled for a significant period of time when it was
unable to handle them correctly. This caused interrupt storms and
interfered with the bluetooth interface on the same SDIO card.
Solve this by disabling interrupts at the point when they can no longer
be handled correctly, which is at the start of mwifiex_remove_card().
For cleanliness, we now enable interrupts in the mwifiex_add_card() path,
to be symmetrical with the disabling of interrupts. We also couple the
registration of the sdio IRQ handler with the actual enable/disable of
interrupts at the hardware level.
I also removed a write to this register in mwifiex_init_sdio which seemed
pointless and won't cause any ill effects now that we only register
the SDIO IRQ handler when we are ready to accept interrupts.

The present build configuration for the rtlwifi family of drivers will
fail under two known conditions:
(1) If rtlwifi is selected without selecting any of the dependent drivers,
there are errors in the build.
(2) If the PCI drivers are built into the kernel and the USB drivers are modules,
or vice versa, there are missing globals.
The first condition is fixed by never building rtlwifi unless at least one
of the device drivers is selected. The second failure is fixed by splitting
the PCI and USB codes out of rtlwifi, and creating their own mini drivers.
If the drivers that use them are modules, they will also be modules.
Although a number of files are touched by this patch, only Makefile and Kconfig
have undergone significant changes. The only modifications to the other files
were to export entry points needed by the new rtl_pci and rtl_usb units, or to
rename two variables that had names that were likely to cause namespace collisions.

ath9k_hif_usb_probe() requests firmware asynchronically and
there is some initialization postponed till firmware is ready.
In particular, ath9k_hif_usb_firmware_cb() callback initializes
hif_dev->tx.tx_buf and hif_dev->tx.tx_pending lists.
At the same time, ath9k_hif_usb_suspend() iterates that lists through
ath9k_hif_usb_dealloc_urbs(). If suspend happens before request_firmware_nowait()
callback is called, it can lead to oops.
Similar issue could be in ath9k_hif_usb_disconnect(), but it is prevented
using hif_dev->fw_done completion and HIF_USB_READY flag. The patch extends
this approach to suspend() as well.

Commit a269913c5 entitled "rtlwifi: Rework rtl_lps_leave() and
rtl_lps_enter() to use work queue" has two bugs for USB drivers.
Firstly, the work queue in question was not initialized. Secondly,
the callback routine used by this queue is contained within the
file used for PCI devices. As a result, it is not available for
architectures without PCI hardware.

rtnl_is_locked() doesn't check who holds this lock, it just tells that it's
locked right now. if caif::ldisc_close really can be called under rtrnl_lock
then it should release net device in other context because there is no way
to grab rtnl_lock without deadlock.
This patch adds work which releases these devices. Also this patch fixes calling
dev_close/unregister_netdevice without rtnl_lock from caif_ser_exit().

virtio net called virtqueue_enable_cq on RX path after napi_complete, so
with NAPI_STATE_SCHED clear - outside the implicit napi lock.
This violates the requirement to synchronize virtqueue_enable_cq wrt
virtqueue_add_buf. In particular, used event can move backwards,
causing us to lose interrupts.
In a debug build, this can trigger panic within START_USE.
Jason Wang reports that he can trigger the races artificially,
by adding udelay() in virtqueue_enable_cb() after virtio_mb().
However, we must call napi_complete to clear NAPI_STATE_SCHED before
polling the virtqueue for used buffers, otherwise napi_schedule_prep in
a callback will fail, causing us to lose RX events.
To fix, call virtqueue_enable_cb_prepare with NAPI_STATE_SCHED
set (under napi lock), later call virtqueue_poll with
NAPI_STATE_SCHED clear (outside the lock).

The driver is comprised of two kernel modules: mlx5_ib and mlx5_core.
This partitioning resembles what we have for mlx4, except that mlx5_ib
is the pci device driver and not mlx5_core.
mlx5_core is essentially a library that provides general functionality
that is intended to be used by other Mellanox devices that will be
introduced in the future. mlx5_ib has a similar role as any hardware
device under drivers/infiniband/hw.

Commit 2768935a46 ('sfc: reuse pages to avoid DMA mapping/unmapping
costs') did not fully take account of DMA scattering which was
introduced immediately before. If a received packet is invalid and
must be discarded, we only drop a reference to the first buffer's
page, but we need to drop a reference for each buffer the packet
used.
I think this bug was missed partly because efx_recycle_rx_buffers()
was not renamed and so no longer does what its name says. It does not
change the state of buffers, but only prepares the underlying pages
for recycling. Rename it accordingly.

This branch contains the following changes:
- Removal of CONFIG_OF_DEVICE, it is always enabled by CONFIG_OF
- Remove #ifdef from linux/of_platform.h to increase compiler syntax
coverage
- Bug fix for address decoding on Bimini and js2x powerpc platforms.
- miscellaneous binding changes
One note on the above. The binding changes going in from all kinds of
different trees has gotten rather out of hand. I picked up some during
this cycle, but even going though my tree isn't a great fit. Ian
Campbell has prototyped splitting the bindings and .dtb files into a
separate repository. The plan is to migrate to using that sometime in
the next few kernel releases which should get rid of a lot of the churn
on binding docs and .dts files.

Commit 55257d72bd (virtio-net: fill only rx queues
which are being used) tries to refill on demand when changing the number of
channels by call try_refill_recv() directly, this may race:
- the refill work who may do the refill in the same time
- the try_refill_recv() called in bh since napi was not disabled
Which may led guest complain during setting channels:
virtio_net virtio0: input.1:id 0 is not a head!
Solve this issue by scheduling a refill work which can guarantee the
serialization of refill.

Rework probe/remove routines to prevent rionet driver from monopolizing
target RapidIO devices. Fix conflict with modular RapidIO switch drivers.
Using one of RapidIO messaging channels rionet driver provides a service
layer common to all endpoint devices in a system's RapidIO network. These
devices may also require their own specific device driver which will be
blocked from attaching to the target device by rionet (or block rionet if
loaded earlier). To avoid conflict with device-specific drivers, the
rionet driver is reworked to be registered as a subsystem interface on the
RapidIO bus.
The reworked rio_remove_dev() and rionet_exit() routines also include
handling of individual rionet peer device removal which was not supported
before.

Pull workqueue changes from Tejun Heo:
"Surprisingly, Lai and I didn't break too many things implementing
custom pools and stuff last time around and there aren't any follow-up
changes necessary at this point.
The only change in this pull request is Viresh's patches to make some
per-cpu workqueues to behave as unbound workqueues dependent on a boot
param whose default can be configured via a config option. This leads
to higher processing overhead / lower bandwidth as more work items are
bounced across CPUs; however, it can lead to noticeable powersave in
certain configurations - ~10% w/ idlish constant workload on a
big.LITTLE configuration according to Viresh.
This is because per-cpu workqueues interfere with how the scheduler
perceives whether or not each CPU is idle by forcing pinned tasks on
them, which makes the scheduler's power-aware scheduling decisions
less effective.
Its effectiveness is likely less pronounced on homogenous
configurations and this type of optimization can probably be made
automatic; however, the changes are pretty minimal and the affected
workqueues are clearly marked, so it's an easy gain for some
configurations for the time being with pretty unintrusive changes."

Yet one more pull request for wireless updates intended for 3.11...
For the mac80211 bits, Johannes says:
"Here we have a few memory leak fixes related to BSS struct handling
mostly from Ben, including a fix for a more theoretical problem
(associating while a BSS struct times out) from myself, a compilation
warning fix from Arend, mesh fixes from Thomas, tracking the beacon
bitrate (Alex), a bandwidth change event fix (Ilan) and some initial
work for 5/10 MHz channels from Simon."
Regarding the iwlwifi bits, Johannes says:
"Emmanuel removed some unneeded/unsupported module parameters and adds a
Bluetooth 1x1 lookup-table for some upcoming products. From Alex I have
an older patch to add low-power receive support, this depended on a
mac80211 commit that only just came in with the merge from wireless-next
I did. Ilan made beacon timings better, and Eytan added some debug
statements for thermal throttling. I have a few cleanups, a fix for a
long-standing but rare warning, and, arguably the most important patch
here, the firmware API version bump for the 7260/3160 devices."
Also included is a Bluetooth pull -- Gustavo says:
"Here goes a set of patches to 3.11. The biggest work here is from Andre Guedes
on the move of the Discovery to use the new request framework. Other than that
Johan provided a bunch of fixes to the L2CAP code. The rest are just small
fixes and clean ups."
On top of all that, there are a variety of updates and fixes to
brcmfmac, rt2x00, wil6210, ath9k, ath10k, and a few others here and
there. This also includes a pull of the wireless tree, in order to
prevent some merge conflicts.

warning: "drivers/net/nlmon.c:31 nlmon_is_valid_mtu()
warn: always true condition '(new_mtu <= ((~0 >> 1))) =>
(s32min-s32max <= s32max)'"
Thus, we should simply remove the test against INT_MAX. Next to that
we also need to explicitly cast the sizeof() case as the comparison
is type promoted to unsigned long so negative values are then
valid instead of invalid. While at it, this also adds a comment about
Netlink and MTUs.

A simple semantic change, when a slave's MAC is cloned by the bond
master then set addr_assign_type to NET_ADDR_STOLEN instead of
NET_ADDR_SET. Also use bond_set_dev_addr() in BOND_FOM_ACTIVE mode

set properly.

In struct bonding there's a member called dev_addr_from_first which is
used to denote when the bond dev should clone the first slave's MAC
address but since we have netdev's addr_assign_type variable that is not
necessary. We clone the first slave's MAC each time we have a random MAC
set to the bond device. This has the nice side-effect of also fixing an
inconsistency - when the MAC address of the bond dev is set after its
creation, but prior to having slaves, it's not kept and the first slave's
MAC is cloned. The only way to keep the MAC was to create the bond device
with the MAC address set (e.g. through ip link). In all cases if the
bond device is left without any slaves - its MAC gets reset to a random
one as before.

Used NL80211_NUM_ACS to indicate the BCMC fifo used in the driver
which has the same value now, but it is a bad idea relying on that.

When getting a transmit packet from the networking layer simply
enqueue the packet unconditional and have it handled by the dequeue
worker. The transfer of the packet to the bus-specific driver part
is now done from one context.

Apparently HW doesn't require us to generate MMIC
for TKIP suite.
Each frame was 8 bytes longer than it should be
and some APs would drop frames that exceed 1520
bytes of 802.11 payload. This could be observed
during throughput tests or fragmented IP traffic.

Nonsense channel flags were being set.
Although it doesn't seem this was visible to the
user the patch makes sure that channel
availability won't be crippled in the future if
ath_common behaviour changes.

The code writes the default_power2 value into the TX field
of the RFCSR50 register, however the condition in the if
statement uses default_power1. Due to this, wrong TX power
value might be written into the register.
Use the correct value in the condition to fix the issue.
Compile tested only.

The 3T/3R devices are using the tertiary PAs/LNAs
however those are never turned on. Fix the code to
turn on those on for such devices.
Also modify the code to use switch statements to
improve readability.

The secondary PAs/LNAs are turned on only for 2T/2R
devices, however these are used for 3T/3R devices as
well. Always turn those on if the device uses more
than one tx/rx chains.

Ralink 3T chipsets are using a different EEPROM
layout than the others. The EEPROM on these devices
contain more data than the others which does not fit
into 272 byte which the rt2800 driver actually uses.
The Ralink reference driver defines EEPROM_SIZE to
512/1024 bytes for PCI/USB devices respectively.
Increase the EEPROM_SIZE constant to 512 bytes, in
order to make room for EEPROM data of 3T devices.
