isert has an issue of trying to create a CQ with more CQEs than are
supported by the hardware, that currently results in failures during
isert_device creation during first session login.
This is the isert version of the patch that Minh Tran submitted for
iser, and is simple a workaround required to function with existing
ocrdma hardware.

Previously, the driver queried the firmware in order to get the number
of supported EQs. Under SRIOV, since this was done before the driver
notified the firmware how many VFs it actually needs, the firmware had
to take into account a worst case scenario and always allocated four EQs
per VF, where one was used for events while the others were used for completions.
Now, when the firmware supports the asymmetric allocation scheme, denoted
by exposing num_sys_eqs > 0 (--> MLX4_DEV_CAP_FLAG2_SYS_EQS), we use the
QUERY_FUNC command to query the firmware before enabling SRIOV. Thus we
can get more EQs and MSI-X vectors per function.
Moreover, when running in the new firmware/driver mode, the limitation
that the number of EQs should be a power of two is lifted.

At least LID reassignment can trigger a race condition in the SRP
initiator driver, namely the receive completion handler trying to
post a request on a QP during or after QP destruction and before
the CQ's have been destroyed. Avoid this race by modifying a QP
into the error state and by waiting until all receive completions
have been processed before destroying a QP.

Improve performance by using multiple RDMA/RC channels per SCSI
host for communication with an SRP target. About the
implementation:
- Introduce a loop over all channels in the code that uses
target->ch.
- Set the SRP_MULTICHAN_MULTI flag during login for the creation
of the second and subsequent channels.
- RDMA completion vectors are chosen such that RDMA completion
interrupts are handled by the CPU socket that submitted the I/O
request. As one can see in this patch it has been assumed if a
system contains n CPU sockets and m RDMA completion vectors
have been assigned to an RDMA HCA that IRQ affinity has been
configured such that completion vectors [i*m/n..(i+1)*m/n) are
bound to CPU socket i with 0 <= i < n.
- Modify srp_free_ch_ib() and srp_free_req_data() such that it
becomes safe to invoke these functions after the corresponding
allocation function failed.
- Add a ch_count sysfs attribute per target port.

Since the block layer already contains functionality to assign
a tag to each request, use that functionality instead of
reimplementing that functionality in the SRP initiator driver.
This change makes the free_reqs list superfluous. Hence remove
that list.

If a cable is pulled during LUN scanning it can happen that the
SRP rport and the SCSI host have been created but no LUNs have been
added to the SCSI host. Since multipathd only sends SCSI commands
to a SCSI target if one or more SCSI devices are present and since
there is no keepalive mechanism for IB queue pairs this means that
after a LUN scan failed and after a reconnect has succeeded no
data will be sent over the QP and hence that a subsequent cable
pull will not be detected. Avoid this by not creating an rport or
SCSI host if a cable is pulled during a SCSI LUN scan.
Note: so far the above behavior has only been observed with the
kernel module parameter ch_count set to a value >= 2.

Attempting to connect three times may be insufficient after an
initiator system tries to relogin, especially if the relogin
attempt occurs before the SRP target service ID has been
registered. Since the srp_daemon retries a failed login attempt
anyway, remove the stale connection retry mechanism.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Various patches have ended up changing the style of the symbolic macros/register
defines to different style.
As a result, the current kernel.org files are a mix of different macro styles.
Since this macro/register defines is used by different drivers a
few patch series have ended up adding duplicate macro/register define entries
with different styles. This makes these register define/macro files a complete
mess and we want to make them clean and consistent. This patch cleans up a part
of it.

In this case the cm_id->context is the isert_np, and the cm_id->qp
is NULL, so use that to distinct the cases.
Since we don't expect any other events on this cm_id we can
just return -1 for explicit termination of the cm_id by the
cma layer.

This patch adds a max_send_sge=2 minimum in isert_conn_setup_qp()
to ensure outgoing control PDU responses with tx_desc->num_sge=2
are able to function correctly.
This addresses a bug with RDMA hardware using dev_attr.max_sge=3,
that in the original code with the ConnectX-2 work-around would
result in isert_conn->max_sge=1 being negotiated.

If mlx4_ib_create_flow() attempts to create > 1 rules with the
firmware, and one of these registrations fail, we leaked the
already created flow rules.
One example of the leak is when the registration of the VXLAN ghost
steering rule fails, we didn't unregister the original rule requested
by the user, introduced in commit d2fce8a906 "mlx4: Set
user-space raw Ethernet QPs to properly handle VXLAN traffic".
While here, add dump of the VXLAN portion of steering rules
so it can actually be seen when flow creation fails.

It is not guaranteed to that srp_sq_size is supported
by the HCA. So if we failed to create the QP with ENOMEM,
try with a smaller srp_sq_size. Keep it up until we hit
MIN_SRPT_SQ_SIZE, then fail the connection.

In ib_uverbs_open_qp, the sharable xrc target qp is created as a
"pseudo" qp and added to a list of qp's sharing the same physical
QP. This is done before the "pseudo" qp is assigned a uobject.
There is a race condition here if an async event arrives at the
physical qp. If the event is handled after the pseudo qp is added to
the list, but before it is assigned a uobject, the kernel crashes in
ib_uverbs_qp_event_handler, due to trying to dereference a NULL
uobject pointer.
Note that simply checking for non-NULL is not enough, due to error
flows in ib_uverbs_open_qp. If the failure is after assigning the
uobject, but before the qp has fully been created, we still have a
problem.
Thus, in ib_uverbs_qp_event_handler, we test that the uobject is
present, and also that it is live.

During create-ah from userspace, uverbs is sending garbage data in
attr.dmac and attr.vlan_id. This patch sets attr.dmac and
attr.vlan_id to zero.

The kernel used to contain two functions for length-delimited,
case-insensitive string comparison, strnicmp with correct semantics and
a slightly buggy strncasecmp. The latter is the POSIX name, so strnicmp
was renamed to strncasecmp, and strnicmp made into a wrapper for the new
strncasecmp to avoid breaking existing users.
To allow the compat wrapper strnicmp to be removed at some point in the
future, and to avoid the extra indirection cost, do
s/strnicmp/strncasecmp/g.

"The big thing in this pile is Eric's unmount-on-rmdir series; we
finally have everything we need for that. The final piece of prereqs
is delayed mntput() - now filesystem shutdown always happens on
shallow stack.
Other than that, we have several new primitives for iov_iter (Matt
Wilcox, culled from his XIP-related series) pushing the conversion to
->read_iter()/ ->write_iter() a bit more, a bunch of fs/dcache.c
cleanups and fixes (including the external name refcounting, which
gives consistent behaviour of d_move() wrt procfs symlinks for long
and short names alike) and assorted cleanups and fixes all over the
place.
This is just the first pile; there's a lot of stuff from various
people that ought to go in this window. Starting with
unionmount/overlayfs mess... ;-/"

In some platforms, when iommu is enabled, the bus address returned by
dma_alloc_coherent is different than the physical address. ocrdma
should use physical address for mmap-ing the queue memory for the
applications.
This patch adds the use of virt_to_phys() at all such places where
kernel buffer is mapped to user process context.

Expose more signature setting parameters. We modify the signature API
to allow usage of some new execution parameters relevant to data
integrity feature.
This patch modifies ib_sig_domain structure by:
- Deprecate DIF type in signature API (operation will
be determined by the parameters alone, no DIF type awareness)
- Add APPTAG check bitmask (for input domain)
- Add REFTAG remap (increment) flag for each domain
- Add APPTAG/REFTAG escape options for each domain
The mlx5 driver is modified to follow the new parameters in HW
signature setup.
At the moment the callers (iser/isert) hard-code new parameters (by
DIF type). In the future, callers will retrieve them from the scsi
command structure.

Rather than using the basic BSF layout which utilizes a pre-configured
signature settings (sufficient for current DIF implementation), we use
the extended BSF layout to expose advanced signature settings. These
settings will also be exposed to the user later.

In the future this will be a per-command parameter so we can lose it,

When dealing with umem objects, the driver assumed host page sizes
defined by PAGE_SHIFT. Modify the code to use arbitrary page shift
provided by umem->page_shift to support different page sizes.

Singal completion of every 32 scsi commands and suppress all the rest.
We don't do anything upon getting the completion so no need to "just
consume" it. Cleanup of scsi command is done in cleanup_task callback.
Still keep dataout and control send completions as we may need to
cleanup there. This helps reducing the amount of interrupts/completions
in the IO path.

Avoid post_send counting (atomic) in the IO path just to keep track of
how many completions we need to consume. Use a beacon post to indicate
that all prior posts completed.

This will solve a possible condition where we might miss TX completion
(flush error) during session teardown. Since we are using a single
CQ, we don't need to actively drain the TX CQ, instead just wait for
flush_completion (when counters reach zero) and remove iser_poll_for_flush_errors().
This patch might introduce a minor performance regression on its own,
but the next patches will enhance performance using a single CQ for RX
and TX.

We need a way to guarentee that we don't stay in soft-IRQ context for
too long. We might starve other pending CQ tasklets or worse lock
against application trying to issue IO on the running CPU.

In case iscsid was violently killed (SIGKILL) during its error
recovery stage, we may never get a connection teardown sequence for
some of the old connections. No harm done, but when we try to unload
the module we will need to cleanup all these connections. So we
actually may end-up here - so it's not a BUG_ON(), just give a relaxed
warning that this happened and continue with normal unload. BUG_ON()
will cause segfault on module_exit and we don't want that.

Previously we notified iscsi layer about the connection layer when
we consumed all of our flush errors. This was racy as there
was no guarentee that iscsi_conn wasn't terminated by then (which ends
up in an invalid memory access). In case we got a non FLUSH error
completion, we are guarenteed that iscsi_conn is still alive. We should
notify iSCSI layer with iscsi_conn_failure to initiate error handling.
While we are at it, add a nice kernel-doc style documentation.

Bailout in case a task cleanup (iscsi_iser_cleanup_task) is called
after the IB device was removed (DEVICE_REMOVAL CM event). We also
call iscsi_conn_stop with a lock taken to prevent DEVICE_REMOVAL and
tasks cleanup from racing.

Previously we didn't need to unbind the iser_conn and iscsi_conn since
we always relied on iscsi daemon to teardown the connection and never
let it finish before we cleanup all that is needed in iser. This is
not the case anymore (for DEVICE_REMOVAL event). So avoid any possible
chance we cause iscsi_conn dereference after iscsi_conn was freed.
We also call iser_conn_terminate (safe to call multiple times) just
for the corner case of iscsi daemon stopping an old connection before
invoking endpoint removal (might happen if it was violently killed).
Notice we are unbinding under a lock - which is required.

give a grace period and continue cleanup if it expired. Have
iser_conn_release wait for full completion before freeing iser_conn.

iscsi daemon is in user-space, thus we can't rely on it to be invoked
at connection teardown (if not running or does not receive CPU time).
This patch addresses the issue by re-structuring iSER connection
teardown logic and CM events handling.
The CM events will dictate the RDMA resources destruction (ib_conn)
and iser_conn is kept around as long as iscsi_conn is left around
allowing iscsi/iser callbacks to continue after RDMA transport was
destroyed.
This patch introduces a separation in logic when handling CM events:
- DISCONNECTED_HANDLER, ADDR_CHANGED
This events indicate the start of teardown process.
Actions:
1. Terminate the connection: rdma_disconnect (send DREQ/DREP)
2. Notify iSCSI of connection failure
3. Change state to TERMINATING
4. Poll for all flush errors to be consumed
- TIMEWAIT_EXIT, DEVICE_REMOVAL
These events indicate the final stage of termination process and
we can free RDMA related resources.
Actions:
1. Call disconnected handler (we are not guaranteed that DISCONNECTED
event was invoked in the past)
2. Cleanup RDMA related resources
3. For DEVICE_REMOVAL return non-zero rc from cma_handler to
implicitly destroy the cm_id (Can't rely on user-space, make sure
we have forward progress)
We replace flush_completion (indicate all flushes were consumed) with
ib_completion (rdma resources were cleaned up).
The iser_conn_release_work will wait for teardown completions:
- conn_stop was completed (tasks were cleaned-up) - stop_completion
- RDMA resources were destroyed - ib_completion
And then will continue to free iser connection representation (iser_conn).

Put all connection IB related resources release in this routine. One
exception is the cm_id which cannot be destroyed as the routine is
protected by the state mutex. Also move its position to avoid forward
declaration. While at it fix qp NULL assignment.

Pull networking updates from David Miller:
"Most notable changes in here:
1) By far the biggest accomplishment, thanks to a large range of
contributors, is the addition of multi-send for transmit. This is
the result of discussions back in Chicago, and the hard work of
several individuals.
Now, when the ->ndo_start_xmit() method of a driver sees
skb->xmit_more as true, it can choose to defer the doorbell
telling the driver to start processing the new TX queue entires.
skb->xmit_more means that the generic networking is guaranteed to
call the driver immediately with another SKB to send.
There is logic added to the qdisc layer to dequeue multiple
packets at a time, and the handling mis-predicted offloads in
software is now done with no locks held.
Finally, pktgen is extended to have a "burst" parameter that can
be used to test a multi-send implementation.
Several drivers have xmit_more support: i40e, igb, ixgbe, mlx4,
virtio_net
Adding support is almost trivial, so export more drivers to
support this optimization soon.

Testing xmit_more support with netperf and connected UDP sockets,
I found strange dst refcount false sharing.
Current handling of IFF_XMIT_DST_RELEASE is not optimal.
Dropping dst in validate_xmit_skb() is certainly too late in case
packet was queued by cpu X but dequeued by cpu Y
The logical point to take care of drop/force is in __dev_queue_xmit()
before even taking qdisc lock.
As Julian Anastasov pointed out, need for skb_dst() might come from some
packet schedulers or classifiers.
This patch adds new helper to cleanly express needs of various drivers
or qdiscs/classifiers.
Drivers that need skb_dst() in their ndo_start_xmit() should call
following helper in their setup instead of the prior :
dev->priv_flags &= ~IFF_XMIT_DST_RELEASE;

Instead of using a single bit, we use two bits, one being
eventually rebuilt in bonding/team drivers.
The other one, is permanent and blocks IFF_XMIT_DST_RELEASE being
rebuilt in bonding/team. Eventually, we could add something
smarter later.

This patch explicitly disables TX completion interrupt coalescing logic
in isert_put_response() and isert_put_datain() that was originally added
as an efficiency optimization in commit 95b60f07.
It has been reported that this change can trigger ABORT_TASK timeouts
under certain small block workloads, where disabling coalescing was
required for stability. According to Sagi, this doesn't impact
overall performance, so go ahead and disable it for now.

Rearrange struct mlx5_caps so it has a "gen" field to represent the current
capabilities configured for the device. Max capabilities can also be queried
from the device. Also update capabilities struct to contain more fields as per
the latest revision if firmware specification.

Pull networking fixes from David Miller:
1) If the user gives us a msg_namelen of 0, don't try to interpret
anything pointed to by msg_name. From Ani Sinha.
2) Fix some bnx2i/bnx2fc randconfig compilation errors.
The gist of the issue is that we firstly have drivers that span both
SCSI and networking. And at the top of that chain of dependencies
we have things like SCSI_FC_ATTRS and SCSI_NETLINK which are
selected.
But since select is a sledgehammer and ignores dependencies,
everything to select's SCSI_FC_ATTRS and/or SCSI_NETLINK has to also
explicitly select their dependencies and so on and so forth.
Generally speaking 'select' is supposed to only be used for child
nodes, those which have no dependencies of their own. And this
whole chain of dependencies in the scsi layer violates that rather
strongly.
So just make SCSI_NETLINK depend upon it's dependencies, and so on
and so forth for the things selecting it (either directly or
indirectly).

We had several problems here. First, a race condition on QP1 mac
handling between mlx4_ib_update_qps and mlx4_ib_modify_qp, which is
fixed by taking the qp mutex in mlx4_ib_update_qps.
Also, qp->pri.smac_port was not updated in mlx4_ib_update_qps.
Last, in __mlx4_ib_modify_qp we did not properly handle the case where
the mac is zero, but port is non-zero.

Automatic Path Migration is not supported under RoCE. Therefore,
return a "not-supported" error if the caller attempts to set an
alternate path in a QP context.
In addition, if there are no IB ports configured, do not report
APM capability in the device flags returned by mlx4_ib_query_device.

The source MAC is needed in RoCE when building the QP1 header.
Currently, this is obtained from the source net device. However, the net
device may not yet exist, or can be destroyed in parallel to this QP1 send
operation (e.g through the VPI port change flow) so accessing it may cause
a kernel crash.
To fix this, we maintain a source MAC cache per port for the net device in
struct mlx4_ib_roce. This cached MAC is initialized to be the default MAC
address obtained during HCA initialization via QUERY_PORT. This cached MAC
is updated via the netdev event notifier handler.
Since the cached MAC is held in an atomic64 object, we do not need locking
when accessing it.

When device is being removed (e.g during VPI port link type change
from ETH to IB), tasks for gid table changes should not be executed.

When a RoCE port becomes active and the netdev of the port has upper

When reading the IPv6 addresses from the net-device, make sure to
avoid adding a duplicate entry to the GID table because of equality
between the default GID we generate and the default IPv6 link-local
address of the device.

When Ethernet netdev is not present for a port (e.g. when the link
layer type of the port is InfiniBand) it's possible to dereference a
null pointer when we do netdevice scanning.
To fix that, we move a section of code that needs to run only when
netdev is present to a proper if () statement.

When failing to allocate TX CQ we already allocated RX CQ, so we need to make
sure we release it. Also, when failing to register notification to the RX CQ
we currently leak both RX and TX CQs of the current index, fix that too.

Because of IP-based GIDs, userspace AHs must have MAC and VLAN ID
resolved separately. Presently, user AHs are broken for ocrdma. This
patch resolves L2 addresses while creating user AH and obtains the
right DMAC and VLAN ID before creating AH.

The static helper routine, __qib_get_user_pages(), accepts a vma arg,
but current use always passes NULL.
This has caused some confusion associated with the correct use of this
argument, but since the current use case doesn't require the
flexiblity, the best thing to do is to simplfy the code to always pass
NULL to get_user_pages().

In debugging an application that receives -ENOMEM from ib_reg_mr(), I
found that ib_umem_get() can fail because the pinned_vm count has
wrapped causing it to always be larger than the lock limit even with
RLIMIT_MEMLOCK set to RLIM_INFINITY.
The wrapping of pinned_vm occurs because the process that calls
ib_reg_mr() will have its mm->pinned_vm count incremented. Later a
different process with a different mm_struct than the one that
allocated the ib_umem struct ends up releasing it which results in
decrementing the new processes mm->pinned_vm count past zero and
wrapping.
I'm not entirely sure what circumstances cause a different process to
release the ib_umem than the one that allocated it but the kernel
stack trace of the freeing process from my situation looks like the

When ib_request_notify_cq() is called for the first time, ocrdma tries
to skip setting deffered_arm flag. This may lead CQ to an un-armed
state thus never generating a CQ event and leaving consumer hung.
This patch removes the part of code that skips setting deferred_arm.

The iser target is the RDMA requester and the iser initiator is the
RDMA responder. In order to determine the max inflight RDMA READ requests
to set on the QP (initiator_depth), it should take the min between the
initiator published initiator_depth and the max inflight rdma read
requests its local HCA support (max_qp_init_rd_atom).
The target will never handle incoming RDMA READ requests so no need to
set responder_resources.

rdma_disconnect may be called in 2 code flows:
- isert_wait_conn: disconnect initiated be the target
- disconnected_handler: disconnect invoked by the initiator
In case isert_conn->disconnect is true then rdma_disconnect

disconnected_handler is invoked on several CM events (such
as DISCONNECTED, DEVICE_REMOVAL, TIMEWAIT_EXIT...). Since
multiple events can occur while before isert_free_conn is
invoked, we might put all isert_conn references and free
the connection too early.

Changing the vlan stripping policy of the QP isn't supported by older
firmware versions for the INIT2RTR command. Nevertheless, we've used it.
Fix that by doing this policy change using INIT2RTR only if the firmware
supports it, otherwise, we call UPDATE_QP command to do the task.

Raw Ethernet QPs opened from user-space lack the proper setup to
recieve/handle VXLAN traffic when VXLAN offloads are enabled.
Fix that by adding a tunnel steering rule on top of the normal unicast
steering rule and set the tunnel_type field in the QP context.

Currently, the parent interface keeps sending broadcast group join
requests even if p_key index 0 is invalid, which is possible/common in
virtualized environments where a VF has been probed to VM but the
actual P_key configuration has not yet been assigned by the management
software. This creates unnecessary noise on the fabric and in the
kernel logs:
ib0: multicast join failed for ff12:401b:8000:0000:0000:0000:ffff:ffff, status -22
The original code run the multicast task regardless of the actual
P_key value, which can be avoided. The fix is to re-init resources and
bring interface up only if P_key index 0 is valid either when starting
up or on PKEY_CHANGE event.

The error flow of ipoib_ib_dev_open() invokes ipoib_ib_dev_stop() with
workqueue flushing enabled, which deadlocks if the open procedure
itself was called by a worker thread.
Fix this by adding a flush enabled flag to ipoib_ib_dev_open() and set
it accordingly from the locations where such a call is made.

The current code use a dedicated polling logic to determine when the P_Key
assigned to the ipoib device is present in HCA port table and act accordingly.
Move to use the code which acts upon getting PKEY_CHANGE event to handle this
task and remove the P_Key polling logic/thread as they add extra complexity
which isn't needed.

If the user creates a listening cm_id with backlog of 0 the IWCM ends
up not allowing any connection requests at all. The correct behavior
is for the IWCM to pick a default value if the user backlog parameter
is zero.
Lustre from version 1.8.8 onward uses a backlog of 0, which breaks
iwarp support without this fix.

From Documentation/scsi/scsi_mid_low_api.txt: "resid - an LLD should
set this signed integer to the requested transfer length (i.e.
'request_bufflen') less the number of bytes that are actually
transferred." This means that resid > 0 in case of an underrun and
also that resid < 0 in case of an overrun. Modify the SRP initiator
code such that it matches this requirement.

Instead of waiting for events and condition changes of the iser
connection state, we wait for explicit completion of connection
establishment and teardown.
Separate connection establishment wait object from the teardown object
to avoid a situation where racing connection establishment and
teardown may concurrently wakeup each other.
ep_poll will wait for up_completion invoked by
iser_connected_handler() and iser release worker will wait for
flush_completion before releasing the connection.
Bound the completion wait with a 30 seconds timeout for cases where
iscsid (the user space iscsi daemon) is too slow or gone.

The iser connection state lookups and transitions are not fully protected.
Some transitions are protected with a spinlock, and in some cases the
state is accessed unprotected due to specific assumptions of the flow.
Introduce a new mutex to protect the connection state access. We use a
mutex since we need to also include a scheduling operations executed
under the state lock.
Each state transition/condition and its corresponding action will be
protected with the state mutex.
The rdma_cm events handler acquires the mutex when handling connection
events. Since iser connection state can transition to DOWN
concurrently during connection establishment, we bailout from
addr/route resolution events when the state is not PENDING.
This addresses a scenario where ep_poll retries expire during CMA
connection establishment. In this case ep_disconnect is invoked while
CMA events keep coming (address/route resolution, connected, etc...).

iser connection needs asynchronous cleanup completions which are
triggered in ep_disconnect. As a result we are keeping the
corresponding iscsi_endpoint structure hanging for no good reason. In
order to avoid that, we seperate iser_conn from iscsi_endpoint storage
space to have their destruction being independent.
iscsi_endpoint will be destroyed at ep_disconnect stage, while the
iser connection will wait for asynchronous completions to be released
in an orderly fashion.

The iser initiator is the RDMA responder so it should publish to the
target the max inflight rdma read requests its local HCA can handle in
responder_resources (max_qp_rd_atom).
The iser target should take the min of that and its local HCA max
inflight oustanding rdma read requests (max_qp_init_rd_atom).
We keep initiator_depth set to 1 in order to compat with old targets.

In case the DISCONNECTED event is not delivered after rdma_disconnect
is called, the CM waits TIMEWAIT seconds and delivers the
TIMEWAIT_EXIT local event. We use this as the notification needed to
continue in the teardown and release sequence.

If any mailbox command reports timeout, save the state in the driver,
to prevent issuing any more commands to the HW. Do proper cleanup
even if FW is in error state.

During cable pull test with a mount over NFS/RDMA, the driver was
reporting error completions when there were no pending requests in the
SQ and RQ. This was triggering a host crash because of reporting
wrong work req id. Avoid this crash by adding a check for SQ and RQ
empty condition and prevent reporting completions if queues are empty.

be2net driver sends a shutdown event to ocrdma during shutdown/reboot.
As part of event processing, ocrdma calls close() and remove() to free
all the resources associated with ocrdma. This also frees irqs used by
ocrdma.

This patch implements routine to query the PFC priority from the
adapter port.
Following are the changes implemented:
* A new FW command is implemented to query the operational/admin DCBX
configuration from the FW and obtain active priority(service
level).
* Adds support for the async event reported by FW when the PFC
priority changes. Service level is re-initialized during modify_qp
or create_ah, based on this event.
* Maintain SL value in ocrdma_dev structure and refer that as and
when needed.

The function __flush_qp() always calls the ULP's CQ completion handler
functions even if the CQ was not armed. This can crash the system if
the function pointer is NULL. The iSER ULP behaves this way: no
completion handler and never arm the CQ for notification. So now we
track whether the CQ is armed at flush time and only call the
completion handlers if their CQs were armed.
Also, if the RCQ and SCQ are the same CQ, the completion handler is
getting called twice. It should only be called once after all SQ and
RQ WRs are flushed from the QP. So rearrange the logic to fix this.

In the event flow, we currently pass only a port number in the
void *data argument. Rather than pass a pointer to the event handlers,
we should use an "unsigned long" parameter, and pass the port number
value directly.
In the future, if necessary for some events, we can use the unsigned long
parameter to pass a pointer.

In preparation for a new mlx5 device which is VPI (i.e., ports can be
either IB or ETH), move the pci device functionality from mlx5_ib
to mlx5_core.
This involves the following changes:
1. Move mlx5_core_dev struct out of mlx5_ib_dev. mlx5_core_dev
is now an independent structure maintained by mlx5_core.
mlx5_ib_dev now has a pointer to that struct.
This requires changing a lot of places where the core_dev
struct was accessed via mlx5_ib_dev (now, this needs to
be a pointer dereference).
2. All PCI initializations are now done in mlx5_core. Thus,
it is now mlx5_core which does pci_register_device (and not
mlx5_ib, as was previously).
3. mlx5_ib now registers itself with mlx5_core as an "interface"
driver. This is very similar to the mechanism employed for
the mlx4 (ConnectX) driver. Once the HCA is initialized
(by mlx5_core), it invokes the interface drivers to do
their initializations.
4. There is a new event handler which the core registers:
mlx5_core_event(). This event handler invokes the
event handlers registered by the interfaces.

This commit enhances the iwarp driver to optionally keep a log of rdma
work request timining data for kernel mode QPs. If iw_cxgb4 module option
c4iw_wr_log is set to non-zero, each work request is tracked and timing
data maintained in a rolling log that is 4096 entries deep by default.
Module option c4iw_wr_log_size_order allows specifing a log2 size to use
instead of the default order of 12 (4096 entries). Both module options
are read-only and must be passed in at module load time to set them. IE:
modprobe iw_cxgb4 c4iw_wr_log=1 c4iw_wr_log_size_order=10
The timing data is viewable via the iw_cxgb4 debugfs file "wr_log".
Writing anything to this file will clear all the timing data.
Data tracked includes:
- The host time when the work request was posted, just before ringing
the doorbell. The host time when the completion was polled by the
application. This is also the time the log entry is created. The delta
of these two times is the amount of time took processing the work request.
- The qid of the EQ used to post the work request.
- The work request opcode.
- The cqe wr_id field. For sq completions requests this is the swsqe
index. For recv completions this is the MSN of the ingress SEND.
This value can be used to match log entries from this log with firmware
flowc event entries.
- The sge timestamp value just before ringing the doorbell when
posting, the sge timestamp value just after polling the completion,
and CQE.timestamp field from the completion itself. With these three
timestamps we can track the latency from post to poll, and the amount
of time the completion resided in the CQ before being reaped by the
application. With debug firmware, the sge timestamp is also logged by
firmware in its flowc history so that we can compute the latency from
posting the work request until the firmware sees it.

With ingress WRITE or READ RESPONSE errors, HW provides the offending
stag from the packet. This patch adds logic to log the parsed TPTE
in this case. cxgb4 now exports a function to read a TPTE entry
from adapter memory.

Advertise a larger max read queue depth for qps, and gather the resource limits
from fw and use them to avoid exhaustinq all the resources.
Design:
cxgb4:
Obtain the max_ordird_qp and max_ird_adapter device params from FW
at init time and pass them up to the ULDs when they attach. If these
parameters are not available, due to older firmware, then hard-code
the values based on the known values for older firmware.
iw_cxgb4:
Fix the c4iw_query_device() to report these correct values based on
adapter parameters. ibv_query_device() will always return:
max_qp_rd_atom = max_qp_init_rd_atom = min(module_max, max_ordird_qp)
max_res_rd_atom = max_ird_adapter
Bump up the per qp max module option to 32, allowing it to be increased
by the user up to the device max of max_ordird_qp. 32 seems to be
sufficient to maximize throughput for streaming read benchmarks.
Fail connection setup if the negotiated IRD exhausts the available
adapter ird resources. So the driver will track the amount of ird
resource in use and not send an RI_WR/INIT to FW that would reduce the
available ird resources below zero.

We need to only register with the iwpm core once. Currently it is
being done for every adapter, which causes a failure for each adapter
but the first, making multiple adapters unusable.

The status page is mapped to user processes and allows sharing the
device state between the kernel and user processes. This state isn't
getting initialized and thus intermittently causes problems. Namely,
the user process can mistakenly think the user doorbell writes are
disabled which causes SQ work requests to never get fetched by HW.

Change logic which determines our Physical Function at PCI Probe time.
Now we read the PL_WHOAMI register and get the Physical Function.
Pass Physical Function to Upper Layer Drivers in lld_info structure in the
new field "pf" added to lld_info. This is useful for the cases where the
PF, say PF4, is attached to a Virtual Machine via some form of "PCI
Pass Through" technology and the PCI Function shows up as PF0 in the VM.

In case protection information exists over the wire
iscsi header data length is required to include it.
Use protection information aware scsi helpers to set
the correct transfer length.
In order to avoid breakage, remove iser transfer length
checks for each task as they are not always true and
somewhat redundant anyway.

In case the transport is iser we should not include the
iscsi target info in the sendtargets text response pdu.
This causes sendtargets response to include the target
info twice.
Modify iscsit_build_sendtargets_response to filter

In case the discovery session is carried over iser, we can't
access the assumed network portal since the default portal is
used. In this case we don't really need to allocate the fastreg
pool, just prepare to the text pdu that will follow.

Fixed a bug that shows up with recv window sizes that exceed the size of
the RCV_BUFSIZ field in opt0 (>= 1024K). If the recv window exceeds
this, then we specify the max possible in opt0, add add the rest in via
a RX_DATA_ACK credits.

Select the appropriate hw mtu index and initial sequence number to optimize
hw memory performance.
Add new cxgb4_best_aligned_mtu() which allows callers to provide enough
information to be used to [possibly] select an MTU which will result in the
TCP Data Segment Size (AKA Maximum Segment Size) to be an aligned value.
If an RTR message exhange is required, then align the ISS to 8B - 1 + 4, so
that after the SYN the send seqno will align on a 4B boundary. The RTR
message exchange will leave the send seqno aligned on an 8B boundary.
If an RTR is not required, then align the ISS to 8B - 1. The goal is
to have the send seqno be 8B aligned when we send the first FPDU.

Currently indirect interrupts for RDMA CQs funnel through the LLD's RDMA
RXQs, which also handle direct interrupts for offload CPLs during RDMA
connection setup/teardown. The intended T4 usage model, however, is to
have indirect interrupts flow through dedicated IQs. IE not to mix
indirect interrupts with CPL messages in an IQ. This patch adds the
concept of RDMA concentrator IQs, or CIQs, setup and maintained by the
LLD and exported to iw_cxgb4 for use when creating CQs. RDMA CPLs will
flow through the LLD's RDMA RXQs, and CQ interrupts flow through the
CIQs.
Design:
cxgb4 creates and exports an array of CIQs for the RDMA ULD. These IQs
are sized according to the max available CQs available at adapter init.
In addition, these IQs don't need FL buffers since they only service
indirect interrupts. One CIQ is setup per RX channel similar to the
RDMA RXQs.
iw_cxgb4 will utilize these CIQs based on the vector value passed into
create_cq(). The num_comp_vectors advertised by iw_cxgb4 will be the
number of CIQs configured, and thus the vector value will be the index
into the array of CIQs.

This patch adds iWARP Port Mapper (IWPM) Version 2 support. The iWARP
Port Mapper implementation is based on the port mapper specification
section in the Sockets Direct Protocol paper -

Existing iWARP RDMA providers use the same IP address as the native
TCP/IP stack when creating RDMA connections. They need a mechanism to
claim the TCP ports used for RDMA connections to prevent TCP port
collisions when other host applications use TCP ports. The iWARP Port
Mapper provides a standard mechanism to accomplish this. Without this
service it is possible for RDMA application to bind/listen on the same
port which is already being used by native TCP host application. If
that happens the incoming TCP connection data can be passed to the
RDMA stack with error.
The iWARP Port Mapper solution doesn't contain any changes to the
existing network stack in the kernel space. All the changes are
contained with the infiniband tree and also in user space.
The iWARP Port Mapper service is implemented as a user space daemon

The iWARP driver (port mapper client) sends to the IWPM service the
local IP address and TCP port it has received from the RDMA
application, when starting a connection. The IWPM service performs a
socket bind from user space to get an available TCP port, called a
mapped port, and communicates it back to the client. In that sense,
the IWPM service is used to map the TCP port, which the RDMA
application uses to any port available from the host TCP port
space. The mapped ports are used in iWARP RDMA connections to avoid
collisions with native TCP stack which is aware that these ports are
taken. When an RDMA connection using a mapped port is terminated, the
client notifies the IWPM service, which then releases the TCP port.
The message exchange between the IWPM service and the iWARP drivers
(between user space and kernel space) is implemented using netlink
sockets.
1) Netlink interface functions are added: ibnl_unicast() and
ibnl_mulitcast() for sending netlink messages to user space
2) The signature of the existing ibnl_put_msg() is changed to be more
generic
3) Two netlink clients are added: RDMA_NL_NES, RDMA_NL_C4IW
corresponding to the two iWarp drivers - nes and cxgb4 which use
the IWPM service
4) Enums are added to enumerate the attributes in the netlink
messages, which are exchanged between the user space IWPM service
and the iWARP drivers

Avoid that closing /dev/infiniband/umad<n> or /dev/infiniband/issm<n>
triggers a use-after-free. __fput() invokes f_op->release() before it
invokes cdev_put(). Make sure that the ib_umad_device structure is
freed by the cdev_put() call instead of f_op->release(). This avoids
that changing the port mode from IB into Ethernet and back to IB
followed by restarting opensmd triggers the following kernel oops:

The ports kobject isn't being released during error flow in device
registration. This patch refactors the ports kobject cleanup into a
single function called from both the error flow in device registration
and from the unregistration function.
A couple of attributes aren't being deleted (iw_stats_group, and
ib_class_attributes). While this may be handled implicitly by the
destruction of their kobjects, it seems better to handle all the
attributes the same way.

The i386 ABI disagrees with most other ABIs regarding alignment of
data types larger than 4 bytes: on most ABIs a padding must be added
at end of the structures, while it is not required on i386.
So for most ABI struct c4iw_alloc_ucontext_resp gets implicitly padded
to be aligned on a 8 bytes multiple, while for i386, such padding is
not added.
The tool pahole can be used to find such implicit padding:

When boundary check will be implemented, the x86_64 kernel will refuse
to write past the i386 userspace provided buffer and the uverbs will
fail.
If the structure is on a page boundary and the next page is not
mapped, ib_copy_to_udata() will fail and the uverb will fail.
Additionally, as reported by Dan Carpenter, without the implicit
padding being properly cleared, an information leak would take place
in most architectures.
This patch adds an explicit padding to struct c4iw_alloc_ucontext_resp,
and, like 92b0ca7cb1 ("IB/mlx5: Fix stack info leak in
mlx5_ib_alloc_ucontext()"), makes function c4iw_alloc_ucontext()
not writting this padding field to userspace. This way, x86_64 kernel
will be able to write struct c4iw_alloc_ucontext_resp as expected by
unpatched and patched i386 libcxgb4.

When encountering an error during the add_port function, adding a port
to sysfs, the port kobject is freed without being deleted from sysfs.
Instead of freeing it directly, the patch uses kobject_put to release
the kobject and delete it.

The ib_core module will call kobject_get on the parent object of each
kobject it creates. This is redundant since kobject_add does that
anyway.
As a side effect, this patch should fix leaking the ports kobject and
the device kobject during unregister flow, since the previous code
didn't seem to take into account the kobject_get calls on behalf of
the child kobjects.

This patch addresses a bug where an early exception for SCSI WRITE
with ImmediateData=Yes was missing the target_put_sess_cmd() call
to drop the extra se_cmd->cmd_kref reference obtained during the
normal iscsit_setup_scsi_cmd() codepath execution.
This bug was manifesting itself during session shutdown within
isert_cq_rx_comp_err() where target_wait_for_sess_cmds() would
end up waiting indefinately for the last se_cmd->cmd_kref put to
occur for the failed SCSI WRITE + ImmediateData descriptors.
This fix follows what traditional iscsi-target code already does
for the same failure case within iscsit_get_immediate_data().

Modify the various routines used to allocate memory resources which
serve QPs in mlx4 to get an input GFP directive. Have the Ethernet
driver to use GFP_KERNEL in it's QP allocations as done prior to this
commit, and the IB driver to use GFP_NOIO when the IB verbs
IB_QP_CREATE_USE_GFP_NOIO QP creation flag is provided.

This addresses a problem where NFS client writes over IPoIB connected
mode may deadlock on memory allocation/writeback.
The problem is not directly memory reclamation. There is an indirect
dependency between network filesystems writing back pages and
ipoib_cm_tx_init() due to how a kworker is used. Page reclaim cannot
make forward progress until ipoib_cm_tx_init() succeeds and it is
stuck in page reclaim itself waiting for network transmission.
Ordinarily this situation may be avoided by having the caller use
GFP_NOFS but ipoib_cm_tx_init() does not have that information.
To address this, take a general approach and add a new QP creation
flag that tells the low-level hardware driver to use GFP_NOIO for the
memory allocations related to the new QP.
Use the new flag in the ipoib connected mode path, and if the driver
doesn't support it, re-issue the QP creation without the flag.

This patch creates a Makefile in hw/ and ulp/ and moves each
corresponding parts of drivers/infiniband/Makefile in the new
Makefiles.
It should not break build except if some hw/ drivers or ulp/ were
allowed previously to be built while CONFIG_INFINIBAND is set to 'n',
but according to drivers/infiniband/Kconfig, it's not possible. So it
should be safe to apply.

The “affinity hint” mechanism is used by the user space
daemon, irqbalancer, to indicate a preferred CPU mask for irqs.
Irqbalancer can use this hint to balance the irqs between the
cpus indicated by the mask.
We wish the HCA to preferentially map the IRQs it uses to numa cores
close to it. To accomplish this, we use cpumask_set_cpu_local_first(), that
sets the affinity hint according the following policy:
First it maps IRQs to “close” numa cores. If these are exhausted, the
remaining IRQs are mapped to “far” numa cores.

SRP defines pr_fmt(fmt) to be "PFX fmt", and then includes a bunch of
header files before it gets around to defining PFX. This causes
problems if any of the header files do a pr_... and use pr_fmt().
Fix this by using KBUILD_MODNAME instead of the private PFX.

IB_PORT_SM capability mask is not cleared in ib_umad_sm_open() if
nonseekable_open() fails.
Since container_of() never returns NULL, remove the code that tests
whether container_of() returns NULL.
Moving the kref_get() call from the start of ib_umad_*open() to the
end is safe since it is the responsibility of the caller of these
functions to ensure that the cdev pointer remains valid until at least
when these functions return.

This commit adds the sysfs interface for enabling QP0 on VFs for
selected VF/port.
By default, no VFs are enabled for QP0 operation.
To enable QP0 operation on a VF/port, under
/sys/class/infiniband/mlx4_x/iov/<b:d:f>/ports/x there are two new entries:
- smi_enabled (read-only). Indicates whether smi is currently
enabled for the indicated VF/port
- enable_smi_admin (rw). Used by the admin to request that smi
capability be enabled or disabled for the indicated VF/port.
0 = disable, 1 = enable.
The requested enablement will occur at the next reset of the
VF (e.g. driver restart on the VM which owns the VF).

This commit adds the infrastructure for enabling selected VFs to
operate SMI (QP0) MADs without restriction.
Additionally, for these enabled VFs, their QP0 proxy and tunnel QPs
are MLX QPs. As such, they operate over VL15. Therefore, they are
not affected by "credit" problems or changes in the VLArb table (which
may shut down VL0).
Non-enabled VFs may only create UD proxy QP0 qps (which are forced by
the hypervisor to send packets using the q-key it assigns and places
in the qp-context). Thus, non-enabled VFs will not pose a security
risk. The hypervisor discards any privileged MADs it receives from
these non-enabled VFs.
By default, all VFs are NOT enabled, and must explicitly be enabled
by the administrator.
The sysfs interface which operates the VF enablement infrastructure
is provided in the next commit.

Currently, VFs in SRIOV VFs are denied QP0 access. The main reason
for this decision is security, since Subnet Management Datagrams
(SMPs) are not restricted by network partitioning and may affect the
physical network topology. Moreover, even the SM may be denied access
from portions of the network by setting management keys unknown to the
SM.
However, it is desirable to grant SMI access to certain privileged
VFs, so that certain network management activities may be conducted
within virtual machines instead of the hypervisor.
This commit does the following:
1. Create QP0 tunnel QPs for all VFs.
2. Discard SMI mads sent-from/received-for non-privileged VFs in the
hypervisor MAD multiplex/demultiplex logic. SMI mads from/for
privileged VFs are allowed to pass.
3. MAD_IFC wrapper changes/fixes. For non-privileged VFs, only
host-view MAD_IFC commands are allowed, and only for SMI LID-Routed
GET mads. For privileged VFs, there are no restrictions.
This commit does not allow privileged VFs as yet. To determine if a VF
is privileged, it calls function mlx4_vf_smi_enabled(). This function
returns 0 unconditionally for now.
The next two commits allow defining and activating privileged VFs.

mlx4_ib_modify_port is invoked in IB for resetting the Q_Key violations
counters and for modifying the IB port capability flags.
For example, when opensm is started up on the hypervisor,
mlx4_ib_modify_port is called to set the port's IsSM flag.
In multifunction mode, the SET_PORT command used in this flow should
be wrapped (so that the PF port capability flags are also tracked,
thus enabling the aggregate of all the VF/PF capability flags to be
tracked properly).
The procedure mlx4_SET_PORT() in main.c is also renamed to mlx4_ib_SET_PORT()
to differentiate it from procedure mlx4_SET_PORT() in port.c.
mlx4_ib_SET_PORT() is used exclusively by mlx4_ib_modify_port().
Finally, the CM invokes ib_modify_port() to set the IsCMSupported flag
even when running over RoCE. Therefore, when RoCE is active,
mlx4_ib_modify_port should return OK unconditionally (since the
capability flags and qkey violations counter are not relevant).

Commit 297e0dad72 ("IB/mlx4: Handle Ethernet L2 parameters for IP
based GID addressing") introduced a bug where is_mcast is now no
longer initialized on the non-multicast condition and so it can be
any random value from the stack. This issue was detected by cppcheck:
[drivers/infiniband/hw/mlx4/ah.c:103]: (error) Uninitialized
variable: is_mcast
Simple fix is to initialise is_mcast to zero.

from the user, but the driver operates on only the extended
diagpkt. This patch specifically initializes the extended diagpkt

The i386 ABI disagrees with most other ABIs regarding alignment of
data type larger than 4 bytes: on most ABIs a padding must be added at
end of the structures, while it is not required on i386.
So for most ABIs struct mlx5_ib_create_srq gets implicitly padded to be
aligned on a 8 bytes multiple, while for i386, such padding is not
added.
Tool pahole could be used to find such implicit padding:

When boundary check will be implemented, the x86_64 kernel will
refuse to read past the i386 userspace provided buffer and the
uverb will fail.
Anyway, if the structure lay in memory on a page boundary and
next page is not mapped, ib_copy_from_udata() will fail and the
uverb will fail.
This patch makes create_srq_user() takes care of the input
data size to handle the case where no padding was provided.
This way, x86_64 kernel will be able to handle struct mlx5_ib_create_srq
as sent by unpatched and patched i386 libmlx5.

For memory regions that are allocated using reg_umr, the suffix of
mlx5_core_create_mkey isn't being called. Instead the creation is
completed in a callback function (reg_mr_callback). This means that
these MRs aren't being added to the MR radix tree. Add them in the
callback.

If ib_post_send fails when posting the UMR work request in reg_umr,
the code doesn't release the temporary pas buffer allocated, and

No need for repetition format pattern in case the data and protection
are already interleaved in the memory domain since the pattern
already exists. A single key entry is sufficient and may save some
extra fetch ops.

When the data and protection are interleaved in the memory domain, no
need to expand the mkey total length.
At the moment no Linux user works (iSER initiator & target) in
interleaved mode. This may change in the future as for SCSI
pass-through devices there is no real point in target performing
de-interleaving and re-interleaving of the protection data in the PT
stage. Regardless, signature verbs support this mode.
