Highlights:
 
  - Enable support for memory protection keys aka "pkeys" on Power7/8/9 when
    using the hash table MMU.
 
  - Extend our interrupt soft masking to support masking PMU interrupts as well
    as "normal" interrupts, and then use that to implement local_t for a ~4x
    speedup vs the current atomics-based implementation.
 
  - A new driver "ocxl" for "Open Coherent Accelerator Processor Interface
    (OpenCAPI)" devices.
 
  - Support for new device tree properties on PowerVM to describe hotpluggable
    memory and devices.
 
  - Add support for CLOCK_{REALTIME/MONOTONIC}_COARSE to the 64-bit VDSO.
 
  - Freescale updates from Scott:
      "Contains fixes for CPM GPIO and an FSL PCI erratum workaround, plus a
       minor cleanup patch."

Michael Ellerman ebf0b6a8b1 Merge branch 'fixes' into next
Merge our fixes branch from the 4.15 cycle.

Unusually the fixes branch saw some significant features merged,
notably the RFI flush patches, so we want the code in next to be
tested against that, to avoid any surprises when the two are merged.

There's also some other work on the panic handling that was reverted
in fixes and we now want to do properly in next, which would conflict.

And we also fix a few other minor merge conflicts.

Dmitry Torokhov 9625e69a38 powerpc: make use of for_each_node_by_type() instead of open-coding it
Instead of manually coding the loop with of_find_node_by_type(), let's
switch to the standard macro for iterating over nodes with given type.

Also fixed a couple of refcount leaks in the aforementioned loops.

Benjamin Herrenschmidt 349524bc0d powerpc: Don't preempt_disable() in show_cpuinfo()
This causes warnings from cpufreq mutex code. This is also rather
unnecessary and ineffective. If we really want to prevent concurrent
unplug, we could take the unplug read lock but I don't see this being
critical.

David Gibson ab9dbf771f Revert "powerpc: Do not call ppc_md.panic in fadump panic notifier"
This reverts commit a3b2cb30f2.

That commit tried to fix problems with panic on powerpc in certain
circumstances, where some output from the generic panic code was being
dropped.

Unfortunately, it breaks things worse in other circumstances. In
particular when running a PAPR guest, it will now attempt to reboot
instead of informing the hypervisor (KVM or PowerVM) that the guest
has crashed. The crash notification is important to some
virtualization management layers.

Revert it for now until we can come up with a better solution.

Michael Ellerman 4e00374704 powerpc/64s: Replace CONFIG_PPC_STD_MMU_64 with CONFIG_PPC_BOOK3S_64
CONFIG_PPC_STD_MMU_64 indicates support for the "standard" powerpc MMU
on 64-bit CPUs. The "standard" MMU refers to the hash page table MMU
found in "server" processors, from IBM mainly.

Currently CONFIG_PPC_STD_MMU_64 is == CONFIG_PPC_BOOK3S_64. While it's
annoying to have two symbols that always have the same value, it's not
quite annoying enough to bother removing one.

However with the arrival of Power9, we now have the situation where
CONFIG_PPC_STD_MMU_64 is enabled, but the kernel is running using the
Radix MMU - *not* the "standard" MMU. So it is now actively confusing
to use it, because it implies that code is disabled or inactive when
the Radix MMU is in use, however that is not necessarily true.

So s/CONFIG_PPC_STD_MMU_64/CONFIG_PPC_BOOK3S_64/, and do some minor
formatting updates of some of the affected lines.

This will be a pain for backports, but c'est la vie.

Christian Lamparter 070e004912 powerpc/4xx: Fix compile error with 64K pages on 40x, 44x
The mmu context on the 40x, 44x does not define pte_frag entry. This
causes gcc abort the compilation due to:

  setup-common.c: In function ‘setup_arch’:
  setup-common.c:908: error: ‘mm_context_t’ has no ‘pte_frag’

This patch fixes the issue by removing the pte_frag initialization in
setup-common.c.

This is possible, because the compiler will do the initialization,
since the mm_context is a sub struct of init_mm. init_mm is declared
in mm_types.h as external linkage.

According to C99 6.2.4.3:
  An object whose identifier is declared with external linkage
  [...] has static storage duration.

C99 defines in 6.7.8.10 that:
  If an object that has static storage duration is not
  initialized explicitly, then:
  - if it has pointer type, it is initialized to a null pointer

Alexey Dobriyan 9b130ad5bb treewide: make "nr_cpu_ids" unsigned
First, number of CPUs can't be negative number.

Second, different signnnedness leads to suboptimal code in the following
cases:

1)
	kmalloc(nr_cpu_ids * sizeof(X));

"int" has to be sign extended to size_t.

2)
	while (loff_t *pos < nr_cpu_ids)

MOVSXD is 1 byte longed than the same MOV.

Other cases exist as well. Basically compiler is told that nr_cpu_ids
can't be negative which can't be deduced if it is "int".

Nicholas Piggin a3b2cb30f2 powerpc: Do not call ppc_md.panic in fadump panic notifier
If fadump is not registered, and no other crash or debug handlers are
registered, the powerpc panic handler stops the guest before the
generic panic code can push out debug information to the console.

Currently, system reset injection causes the guest to silently stop.

Stop calling ppc_md.panic in the panic notifier. crash_fadump already
does rtas_os_term() to terminate the guest if fadump is registered.

Remove ppc_md.panic. Move fadump panic notifier into fadump code.

Aneesh Kumar K.V 79cc38ded1 powerpc/mm/hugetlb: Add support for reserving gigantic huge pages via kernel command line
With commit aa888a7497 ("hugetlb: support larger than MAX_ORDER") we added
support for allocating gigantic hugepages via kernel command line. Switch
ppc64 arch specific code to use that.

W.r.t FSL support, we now limit our allocation range using BOOTMEM_ALLOC_ACCESSIBLE.

We use the kernel command line to do reservation of hugetlb pages on powernv
platforms. On pseries hash mmu mode the supported gigantic huge page size is
16GB and that can only be allocated with hypervisor assist. For pseries the
command line option doesn't do the allocation. Instead pseries does gigantic
hugepage allocation based on hypervisor hint that is specified via
"ibm,expected#pages" property of the memory node.

Michael Neuling 64ebb9a208 powerpc: Fix /proc/cpuinfo revision for POWER9 DD2
The P9 PVR bits 12-15 don't indicate a revision but instead different
chip configurations.  From BookIV we have:
   Bits      Configuration
    0 :    Scale out 12 cores
    1 :    Scale out 24 cores
    2 :    Scale up  12 cores
    3 :    Scale up  24 cores

DD1 doesn't use this but DD2 does. Linux will mostly use the "Scale
out 24 core" configuration (ie. SMT4 not SMT8) which results in a PVR
of 0x004e1200. The reported revision in /proc/cpuinfo is hence
reported incorrectly as "18.0".

This patch fixes this to mask off only the relevant bits for the major
revision (ie. bits 8-11) for POWER9.

Aneesh Kumar K.V 92d9dfda8b powerpc/mm/4k: Limit 4k page size config to 64TB virtual address space
Supporting 512TB requires us to do a order 3 allocation for level 1 page
table (pgd). This results in page allocation failures with certain workloads.
For now limit 4k linux page size config to 64TB.

Currently we assume that if the cpu_spec has a pvr_mask then it must also have a

Now that crashkernel parameter parsing and vmcoreinfo related code is
moved under CONFIG_CRASH_CORE instead of CONFIG_KEXEC_CORE, remove
dependency with CONFIG_KEXEC for CONFIG_FA_DUMP.  While here, get rid of
definitions of fadump_append_elf_note() & fadump_final_note() functions
to reuse similar functions compiled under CONFIG_CRASH_CORE.

Michael Ellerman 7644d5819c powerpc: Create asm/debugfs.h and move powerpc_debugfs_root there
powerpc_debugfs_root is the dentry representing the root of the
"powerpc" directory tree in debugfs.

Currently it sits in asm/debug.h, a long with some other things that
have "debug" in the name, but are otherwise unrelated.

Pull it out into a separate header, which also includes linux/debugfs.h,
and convert all the users to include debugfs.h instead of debug.h.

Aneesh Kumar K.V f4ea6dcb08 powerpc/mm: Enable mappings above 128TB
Not all user space application is ready to handle wide addresses. It's
known that at least some JIT compilers use higher bits in pointers to
encode their information. It collides with valid pointers with 512TB
addresses and leads to crashes.

To mitigate this, we are not going to allocate virtual address space
above 128TB by default.

But userspace can ask for allocation from full address space by
specifying hint address (with or without MAP_FIXED) above 128TB.

If hint address set above 128TB, but MAP_FIXED is not specified, we try
to look for unmapped area by specified address. If it's already
occupied, we look for unmapped area in *full* address space, rather than
from 128TB window.

This approach helps to easily make application's memory allocator aware
about large address space without manually tracking allocated virtual
address space.

This is going to be a per mmap decision. ie, we can have some mmaps with
larger addresses and other that do not.

Alexey Kardashevskiy 88f54a3581 powerpc/iommu: Pass mm_struct to init/cleanup helpers
We are going to get rid of @current references in mmu_context_boos3s64.c
and cache mm_struct in the VFIO container. Since mm_context_t does not
have reference counting, we will be using mm_struct which does have
the reference counter.

This changes mm_iommu_init/mm_iommu_cleanup to receive mm_struct rather
than mm_context_t (which is embedded into mm).

This should not cause any behavioral change.

Kevin Hao b92a226e52 powerpc: Move cpu_has_feature() to a separate file
We plan to use jump label for cpu_has_feature(). In order to implement
this we need to include the linux/jump_label.h in asm/cputable.h.

Unfortunately if we do that it leads to an include loop. The root of the
problem seems to be that reg.h needs cputable.h (for CPU_FTRs), and then
cputable.h via jump_label.h eventually pulls in hw_irq.h which needs
reg.h (for MSR_EE).

So move cpu_has_feature() to a separate file on its own.

Alexander Graf 9178ba294b powerpc: Convert power off logic to pm_power_off
The generic Linux framework to power off the machine is a function pointer
called pm_power_off. The trick about this pointer is that device drivers can
potentially implement it rather than board files.

Today on powerpc we set pm_power_off to invoke our generic full machine power
off logic which then calls ppc_md.power_off to invoke machine specific power
off.

However, when we want to add a power off GPIO via the "gpio-poweroff" driver,
this card house falls apart. That driver only registers itself if pm_power_off
is NULL to ensure it doesn't override board specific logic. However, since we
always set pm_power_off to the generic power off logic (which will just not
power off the machine if no ppc_md.power_off call is implemented), we can't
implement power off via the generic GPIO power off driver.

To fix this up, let's get rid of the ppc_md.power_off logic and just always use
pm_power_off as was intended. Then individual drivers such as the GPIO power off
driver can implement power off logic via that function pointer.

With this patch set applied and a few patches on top of QEMU that implement a
power off GPIO on the virt e500 machine, I can successfully turn off my virtual
machine after halt.

The general idea is that each core will release all of its
threads into the secondary thread startup code, which will
eventually wait in the secondary core holding area, for the
appropriate bit in the PACA to be set. The kick_cpu function
pointer will set that bit in the PACA, and thus "release"
the core/thread to boot. We also need to do a few things that
U-Boot normally does for CPUs (like enable branch prediction).

Scott Wood 6663a4fa67 powerpc: Don't skip ePAPR spin-table CPUs
Commit 59a53afe70 "powerpc: Don't setup
CPUs with bad status" broke ePAPR SMP booting.  ePAPR says that CPUs
that aren't presently running shall have status of disabled, with
enable-method being used to determine whether the CPU can be enabled.

Fix by checking for spin-table, which is currently the only supported
enable-method.

Michael Neuling 59a53afe70 powerpc: Don't setup CPUs with bad status
OPAL will mark a CPU that is guarded as "bad" in the status property of the CPU
node.

Unfortunatley Linux doesn't check this property and will put the bad CPU in the
present map.  This has caused hangs on booting when we try to unsplit the core.

This patch checks the CPU is avaliable via this status property before putting
it in the present map.

Tang Yuantian d2deebabae booke/watchdog: refine and clean up the codes
Basically, this patch does the following:
1. Move the codes of parsing boot parameters from setup-common.c
   to driver. In this way, code reader can know directly that
   there are boot parameters that can change the timeout.
2. Make boot parameter 'booke_wdt_period' effective.
   currently, when driver is loaded, default timeout is always
   being used in stead of booke_wdt_period.
3. Wrap up the watchdog timeout in device struct and clean up
   unnecessary codes.

Michael Ellerman 5853aef1ac powerpc: Add threads_per_subcore
On POWER8 we have a new concept of a subcore. This is what happens when
you take a regular core and split it. A subcore is a grouping of two or
four SMT threads, as well as a handfull of SPRs which allows the subcore
to appear as if it were a core from the point of view of a guest.

Unlike threads_per_core which is fixed at boot, threads_per_subcore can
change while the system is running. Most code will not want to use
threads_per_subcore.

Gautham R. Shenoy 2299d03a63 powerpc: powernv: Framework to show the correct clock in /proc/cpuinfo
Currently, the code in setup-common.c for powerpc assumes that all
clock rates are same in a smp system. This value is cached in the
variable named ppc_proc_freq and is the value that is reported in
/proc/cpuinfo.

However on the PowerNV platform, the clock rate is same only across
the threads of the same core. Hence the value that is reported in
/proc/cpuinfo is incorrect on PowerNV platforms. We need a better way
to query and report the correct value of the processor clock in
/proc/cpuinfo.

The patch achieves this by creating a machdep_call named
get_proc_freq() which is expected to returns the frequency in Hz. The
code in show_cpuinfo() can invoke this method to display the correct
clock rate on platforms that have implemented this method. On the

Benjamin Herrenschmidt 36ae37e343 powerpc: Make boot_cpuid common between 32 and 64-bit
Move the definition to setup-common.c and set the init value
to -1 on both 32 and 64-bit (it was 0 on 64-bit).

Additionally add a check to prom.c to garantee that the init
value has been udpated after the DT scan.

Anton Blanchard 01666c8ee2 powerpc: Fix endian issue in setup-common.c
During on LE boot we see:

    Partition configured for 1073741824 cpus, operating system maximum is 2048.

Clearly missing a byteswap here.
