Replace printk(KERN_DEBUG ...) with netdev_dbg and dev_dbg. Remove

Since the kernel already has a function for hex dumps, use that instead
of the driver's own versions. The function supports dynamic debugging,
so also remove some unnecessary debug macros.

None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>. Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

The gdm72xx driver needs to have either the USB or SDIO implementation
enabled to provide useful functionalities, so the driver should depend
on either USB or MMC. This patch makes WIMAX_GDM72XX depend on either
USB or MMC.
Also, WIMAX_GDM72XX needs to be built as a module if its dependent
interface, either USB or MMC, is built as a module. This patch enforces
that in the WIMAX_GDM72XX_USB and WIMAX_GDM72XX_SDIO dependency.

Commit 84ebc10294 (USB: remove
CONFIG_USB_SUSPEND option) failed to remove all of the usages of
USB_SUSPEND throughout the kernel. This patch (as1677) removes the
remaining instances of that symbol.

alloc failures already get standardized OOM
messages and a dump_stack.
For the affected mallocs around these OOM messages:
Converted kzallocs with multiplies to kcalloc.
Converted kmallocs with multiplies to kmalloc_array.
Converted a kmalloc/strlen/strncpy to kstrdup.
Moved a spin_lock below a removed OOM message and
removed a now unnecessary spin_unlock.
Neatened alignment and whitespace.

Pull big execve/kernel_thread/fork unification series from Al Viro:
"All architectures are converted to new model. Quite a bit of that
stuff is actually shared with architecture trees; in such cases it's
literally shared branch pulled by both, not a cherry-pick.
A lot of ugliness and black magic is gone (-3KLoC total in this one):
- kernel_thread()/kernel_execve()/sys_execve() redesign.
We don't do syscalls from kernel anymore for either kernel_thread()
or kernel_execve():
kernel_thread() is essentially clone(2) with callback run before we
return to userland, the callbacks either never return or do
successful do_execve() before returning.
kernel_execve() is a wrapper for do_execve() - it doesn't need to
do transition to user mode anymore.
As a result kernel_thread() and kernel_execve() are
arch-independent now - they live in kernel/fork.c and fs/exec.c
resp. sys_execve() is also in fs/exec.c and it's completely
architecture-independent.
- daemonize() is gone, along with its parts in fs/*.c
- struct pt_regs * is no longer passed to do_fork/copy_process/
copy_thread/do_execve/search_binary_handler/->load_binary/do_coredump.
- sys_fork()/sys_vfork()/sys_clone() unified; some architectures
still need wrappers (ones with callee-saved registers not saved in
pt_regs on syscall entry), but the main part of those suckers is in
kernel/fork.c now."

commit cf4ca4874f removed the definition of NIPQUAD and NIPQUAD_FMT,
and NIP6 also is out of date.
commit 2874762b31 replace deprecated NIPQUAD marco to C code, but we can use %pI4 to
print IPv4 address more simply. And remove constant condition judge.
Because DEBUG_SDU is not defined in gdm_wimax.h, no error message when compiling.

This patch applies spinlock to protect access to rx / tx structs in
certain call sites, which fixes the following crash in gdm_suspend.
It also fixes usb_set_intfdata() in gdm_usb_probe to avoid setting an
already freed phy_dev.
