Arnd Bergmann 76a64921da isdn: autoconvert trivial BKL users to private mutex
All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.

None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.

Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

Dan Carpenter 60a5711db6 isdn/kcapi: return -EFAULT on copy_from_user errors
copy_from_user() returns the number of bytes remaining but we should
return -EFAULT here.  The error code gets returned to the user.  Both
old_capi_manufacturer() and capi20_manufacturer() had other places
that already returned -EFAULT so this won't break anything.

Tilman Schmidt 85a83560af isdn/capi: make reset_ctr op truly optional
The CAPI controller operation reset_ctr is marked as optional, and
not all drivers do implement it. Add a check to the kernel CAPI
whether it exists before trying to call it.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Jan Kiszka c6af043794 CAPI: Drop special controller lookup from capi20_put_message
This strange special rule to fall back to controller 1 cannot be derived
from the CAPI specs and looks a lot like it was once dedicated to some
out-of-tree driver, probably AVM's broken fcdsl2 (FRITZ!Card DSL v2.0).
I found no in-tree user that needs this check, and I'm now taking care
of the fcdsl2. So drop these bits from our stack.

Jan Kiszka dfbb84ffe9 CAPI: Fix locking around capiminor's output queue and drop workaround_lock
Introduce outlock as a spin lock that protects capiminor's outqueue,
outbytes and outskb (formerly known as ttyskb). outlock can be acquired
from soft-IRQ context via capinc_write, so make it bh-safe.

This finally removes the last reason for keeping the workaround lock
around (which was incomplete and partly broken anyway). And as we no
longer call handle_recv_skb in atomic context, gen_data_b3_resp_for can
use non-atomic allocation now.

Jan Kiszka 40fb2d0da7 CAPI: Dynamically register minor devices
Register capiminors dynamically with the TTY core so that udev can make
them show up as the NCCIs appear or disappear. This removes the need to
check if the capiminor requested in capinc_tty_open actually exists.

And this completely obsoletes capifs which will be scheduled for removal
in a later patch.

Jan Kiszka 05b4149433 CAPI: Rework locking of capidev members
Rename 'ncci_list_mtx' to 'lock', expressing that it now protects a
larger set of capidev members: the NCCI list, ap.applid (ie. the
registration of the application), and modifications of userflags.

We do not need to protect each and every check for ap.applid because,
once an application is registered, it will stay for the whole lifetime
of the device.

Also, there is no need to apply the capidev mutex during release (if
there could be concurrent users, we would crash them anyway by freeing
the device at the end of capi_release).

Jan Kiszka 88c896ef87 CAPI: Rework application locking
Drop the application rw-lock in favour of RCU. This synchronizes
capi20_release against capi_ctr_handle_message which may dereference an
application from (soft-)IRQ context. Any other access to the application
list is now protected by the capi_controller_lock as well. This also
allows to safely inspect applications for /proc dumping by holding
capi_controller_lock.

At this chance, drop some useless release_in_progress checks where we
obtained the application pointer from the list (which becomes NULL on
release_in_progress).

This patch applies the mutex so far only protecting the controller list
to (almost) all accesses of controller data structures. It also reworks
waiting on state changes in old_capi_manufacturer so that it no longer
poll and holds a module reference to the controller owner while waiting
(the latter was partly done already). Modification and checking of the
blocked state remains racy by design, the caller is responsible for
dealing with this.

Jan Kiszka ef69bb2ec6 CAPI: Rework controller state notifier
Another step towards proper locking: Rework the callback provided to
capidrv for controller state changes. This is so far attached to an
application, which would require us to hold the corresponding lock
across notification calls.

But there is no direct relation between a controller up/down event and
an application, so let's decouple them and provide a notifier call chain
for those events instead. This notifier chain is first of all used
internally. Here we request the highest priority to unsure that
housekeeping work is done before any other notifications. The chain is
exported via [un]register_capictr_notifier to our only user, capidrv, to
replace the racy and unfixable capi20_set_callback.

The CVS revisions dumped by all CAPI modules are meaningless today. And
that some CAPI module is loaded or removed does not necessarily deserve
a message. Just keep the message of the central module, capi.ko, drop

In several places, capidrv sends a CAPI message to the ISDN
device and then updates its internal state accordingly.
If the response message from the device arrives before the
state is updated, it may be rejected or processed incorrectly.
Avoid these races by updating the state before emitting the
message.

Info values in the 0x00xx range are defined in the CAPI standard
as "Informational, message processed successfully". Therefore a
CONNECT_B3_CONF message with an Info value in that range should

The dereferencing of the private pointer cmsg->m in capi_cmsg2str() may
cause an Oops in case of an error, which is particularly inconvenient
as that function is typically used to format an error message. Add a

Wrap access to task credentials so that they can be separated more easily from
the task_struct during the introduction of COW creds.

Change most current->(|e|s|fs)[ug]id to current_(|e|s|fs)[ug]id().

Change some task->e?[ug]id to task_e?[ug]id().  In some places it makes more
sense to use RCU directly rather than a convenient wrapper; these will be
addressed by later patches.

Alan Cox a352def21a tty: Ldisc revamp
Move the line disciplines towards a conventional ->ops arrangement.  For
the moment the actual 'tty_ldisc' struct in the tty is kept as part of
the tty struct but this can then be changed if it turns out that when it
all settles down we want to refcount ldiscs separately to the tty.

Pull the ldisc code out of /proc and put it with our ldisc code.

This can be broken down into these major areas:
 - Documentation updates (language translations and fixes, as
   well as kobject and kset documenatation updates.)
 - major kset/kobject/ktype rework and fixes.  This cleans up the
   kset and kobject and ktype relationship and architecture,
   making sense of things now, and good documenation and samples
   are provided for others to use.  Also the attributes for
   kobjects are much easier to handle now.  This cleaned up a LOT
   of code all through the kernel, making kobjects easier to use
   if you want to.
 - struct bus_type has been reworked to now handle the lifetime
   rules properly, as the kobject is properly dynamic.
 - struct driver has also been reworked, and now the lifetime
   issues are resolved.
 - the block subsystem has been converted to use struct device
   now, and not "raw" kobjects.  This patch has been in the -mm
   tree for over a year now, and finally all the issues are
   worked out with it.  Older distros now properly work with new
   kernels, and no userspace updates are needed at all.
 - nozomi driver is added.  This has also been in -mm for a long
   time, and many people have asked for it to go in.  It is now
   in good enough shape to do so.
 - lots of class_device conversions to use struct device instead.
   The tree is almost all cleaned up now, only SCSI and IB is the
   remaining code to fix up...

Karsten Keil eb36f4fc01 fix oops on rmmod capidrv
Fix overwriting the stack with the version string
(it is currently 10 bytes + zero) when unloading the
capidrv module. Safeguard against overwriting it
should the version string grow in the future.

Should fix Kernel Bug Tracker Bug 9696.

Jesper Juhl e8a285b7b1 isdn: guard against a potential NULL pointer dereference in old_capi_manufacturer()
In drivers/isdn/capi/kcapi.c::old_capi_manufacturer(), if the call to
get_capi_ctr_by_nr(ldef.contr); in line 823 returns NULL, then we'll be
dereferencing a NULL pointer in the very next line.

(Found by Coverity checker as bug #402)

Jesper Juhl b1b2e7cf4a fix possible NULL deref on low memory condition in capidrv.c::send_message()
If we fail to allocate an skb in
drivers/isdn/capi/capidrv.c::send_message(), then we'll end up
dereferencing a NULL pointer.
Since out of memory conditions are not unheard of, I believe it
is better to print a error message and just return rather than
bring down the whole kernel.
Sure, doing this may upset some application, but that's still
better than crashing the whole system.

Of course, the code here might actually be buggy, in which case this patch
should not be applied?

Answer:

  No this field is ignored inside linux kernel.Yes this is ugly, but it's
  the CAPI spec for all OS.

  CAPI DATA_B3 Request/Indication CAPI Message has a mandatory field which
  represent the 32 bit buffer address of the payload data.  In linux the
  payload data do not use a sperate buffer, data follows directely after the
  CAPI Message in the same skb and we use this assumption inside the drivers,
  so we can ignore this field.

  Inside the linux CAPI implemetation we never use this field, so it could
  also have no value, but since random data in a message is bad as well (e.g.
  displayed in CAPI traces) we set is to the most adequate value.

  Outside the kernel the capi20 library sets the correct addresses (there is
  an optional second field for 64 bit adresses for 64 bit systems, we do not
  use here).
