This patch removes chip specific operations from the common hardware
paths, as well as the Makefile change to accomodate the new files.

This patch defines the common operations to support multiple chips. It
includes common header file modifications to support the current chips
(T4 and T5). It also includes the following bug fixes:
- reconfirms the rnode state after an implicit logo.
- corrects the stats array size.
- sets up and checks flags correctly when coming up as master and finding
the card initialized

If kzalloc() or csio_hw_check_fwconfig() fail we may leave the
csio_hw_flash_config() function without freeing allocated memory or
firmware. This should take care of the leaks.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

It enables establishing active open connection using fw_ofld_connection work
request when cpl_act_open_rpl says TCAM full error which may be because
of LE hash collision. Current support is only for IPv4 active open connections.
Sets ntuple bits in active open requests. For T4 firmware greater than 1.4.10.0
ntuple bits are required to be set.
Adds nocong and enable_ecn module parameter options.
