Following configuration is created with what driver exports:
iface.vlan_id = 65535
iface.vlan_priority = 255
iface.vlan_state = <empty>
vlan_state is empty as iscsiadm doesn't process "Disabled".
When applying this configuration, iscsiadm checks for if vlan_state is
"disable" if not it enables with value in vlan_id. 65535 not being valid
value, 0 is applied.
Use "enable" or "disable" for ISCSI_NET_PARAM.

The adapter file descriptor was previously cached within the kernel for
a given context in order to support performing a close on behalf of an
application. This is no longer needed as applications are now required
to perform a close on the adapter file descriptor.

Caching the adapter file descriptor and performing a close on behalf of
an application is a poor design. This is due to the fact that once a
file descriptor in installed, it is free to be altered without the
knowledge of the cxlflash driver. This can lead to inconsistencies
between the application and kernel. Furthermore, the nature of the
former design is more exploitable and thus should be abandoned.
To support applications performing a close on the adapter file that is
associated with a context, a new flag is introduced to the user API to
indicate to applications that they are responsible for the close
following the cleanup (detach) of a context. The documentation is also
updated to reflect this change in behavior.

Currently, context user references are tracked via the list of LUNs that
have attached to the context. While convenient, this is not intuitive
without a deep study of the code and is inconsistent with the existing
reference tracking patterns within the kernel. This design choice can
lead to future bug injection.
To improve code comprehension and better protect against future bugs,
add explicit reference counting to contexts and migrate the context
removal code to the kref release handler.

The context removal routine requires access to the owning adapter
structure to reset the context within the AFU as part of the tear down
sequence. In order to support kref adoption, the owning adapter must be
accessible from the release handler. As the kref framework only provides
the kref reference as the sole parameter, another means is needed to
derive the owning adapter.
As a remedy, the owning adapter reference is saved off within the
context during initialization.

Context information structures are protected by a mutex that is held
when accessing/manipulating the context. When the code that manages
these structures was authored, a decision was made to include taking the
mutex as part of the allocation/initialization sequence and also handle
the scenario where the mutex was already held when freeing the context.
While not a problem outright, this design decision has been deemed as
too flexible and the code should be made more rigid to avoid future
bugs. In addition, further review of the code yields that the existing
mutex manipulations in both of these context management paths are
superfluous.
This commit removes the obtaining of the context mutex in the context
initialization routine and assumes the mutex is not held in the context
free path.

FC-LS mandates that we should invalidate all sequences before sending a
LOGO. And we should set the event to RPORT_EV_STOP when a LOGO request
has been received to signal that all exchanges are terminated.

When running in point-to-multipoint mode PLOGI is done after FLOGI
completed. So when the PLOGI fails we should be sending a LOGO to the
remote port.

When receiving a PRLO it just means that the operating parameters have
changed, it does _not_ mean that the port doesn't want to communicate
with us. So instead of implicitly logging out we should be issueing a
PRLI to figure out the new operating parameters. We can always recover
once PRLI fails.

Use scsi_is_sas_rphy() instead of is_sas_attached() to decide whether we
should obtain the SAS address from a scsi device or not. This will
prevent us from tripping on the BUG_ON() in sas_sdev_to_rdev() if the
rphy isn't attached to the SAS transport class, like it is with hpsa's
logical devices.

Currently we free the resources backing the enclosure device before we
call device_unregister(). This is racy: during rmmod of low-level SCSI
drivers that hook into enclosure, we end up with a small window of time

Fortunately the solution is extremely simple: call device_unregister()
before we free the resources, and the race no longer exists. The driver
core holds a reference over ->remove_dev(), so AFAICT this is safe.

In aacraid's ioctl_send_fib() we do two fetches from userspace, one the
get the fib header's size and one for the fib itself. Later we use the
size field from the second fetch to further process the fib. If for some
reason the size from the second fetch is different than from the first
fix, we may encounter an out-of- bounds access in aac_fib_send(). We
also check the sender size to insure it is not out of bounds. This was
reported in https://bugzilla.kernel.org/show_bug.cgi?id=116751 and was
assigned CVE-2016-6480.

In _scsih_io_done() we test if the ioc->logging_level does _not_ have
the MPT_DEBUG_REPLY bit set and if it hasn't we print the debug
messages. This unfortunately is the wrong way around.
Note, the actual bug is older than af0094115 but this commit removed the
CONFIG_SCSI_MPT3SAS_LOGGING Kconfig option which hid the bug.

Trivial non-functional changes for a couple annoying things:
1) Functions local to files are not declared static, which is
frustrating when reading the code because it's non-obvious at first
glance what's actually called from other files.
2) Set-but-unused variables abound, presumably to mask -Wunused-result
errors in the past. None of these are flagged today though (with one
exception noted below), so remove them.
Fixing (2) exposed the fact that we improperly ignore the return value
of scsi_device_reprobe() in _scsih_reprobe_lun(). Fixing the calling
code to deal with the potential error is non-trivial, so for now just
WARN().

We blindly trust the hardware to give us NUL-terminated strings, which
is a bad idea because it doesn't always do that. For example:

In this case, connector_name is four spaces. We got lucky here because
the 2nd byte beyond our character array happens to be a NUL. Fix this by
explicitly writing '\0' to the end of the string to ensure we don't run
off the edge of the world in printk().

This crash is due to the AFU not being mapped when the shutdown
notification routine is called and is a regression that was inserted
recently with Commit 704c4b0ddc ("cxlflash: Shutdown notify support
for CXL Flash cards").
As a fix, shutdown notification should only occur when the AFU is

When running in VN2VN mode there is no central instance which would send
out any FIP VLAN discovery notifications. So this patch adds a new sysfs
attribute 'fip_vlan_responder' which will activate a FIP VLAN discovery
responder.

I wish the OSD code could simply use blk_rq_map_* helpers like
everyone else, but the complex nature of deciding if we have
DATA IN and/or DATA OUT buffers might make this impossible
(at least for a mere human like me).
But using blk_rq_append_bio at least allows sharing the setup code
between request with or without dat a buffers, and given that this
is the last user of blk_make_request it allows getting rid of that
somewhat awkward interface.

Remove the CXL_KERNEL_API and CXL_EEH Kconfig options, as they were only
needed to coordinate the merging of the cxlflash driver. Also remove the
stub implementation of cxl_perst_reloads_same_image() in cxlflash which is
only used if CXL_EEH isn't defined (i.e. never).

These only work if the we use the same initiator ID for all path,
which might not be true if we use different protocols, or even just
different HBAs.
Instead dm-mpath will grow support to register all path manually
later in this series.

The SA controller spins down RAID drive spares.
A REGNEWD event causes an inquiry to be sent to all physical
drives. This causes the SA controller to spin up the spare.
The controller suspends all I/O to a logical volume until
the spare is spun up. The spin-up can take over 50 seconds.
This can result in one or both of the following:
- SML sends down aborts and resets to the logical volume
and can cause the logical volume to be off-lined.
- a negative impact on the logical volume's I/O performance
each time a REGNEWD is triggered.

During hot swap of PCI device, there can be PCI error on device,
during normal driver unload. The race between normal driver unload and
driver unload due to PCI error, can lead to system crash.Fix is to check
if there is unload going on and allow that function to unload the driver.

When VP decoupling enabled, there could be a window where, FLOGI from initiators
can be dropped before VP0 is enabled, causing link level recovery.

The _scsih_pci_mmio_enabled called if scsih_pci_error_detected returns
PCI_ERS_RESULT_CAN_RECOVER, at this point, read/write to the device
still works, no need to reset slot.
Or the mpt3sas_base_map_resources in scsih_pci_slot_reset will fail,
and iounamp ioc->chip, then we will meet issue when read ioc->chip
in mpt3sas_base_get_iocstate from _base_fault_reset_work.

qla2xxx first calls request_irq() and then does the setup of the queue
entry data needed in the interrupt handlers in when using MSI-X. This
could lead to a NULL pointer dereference when an IRQ fires between the
request_irq() call and the assignment of the qentry data structure to
the rsp->msix field. A possible case for such a race would be in the
kdump case when the HBA's IRQs are still enabled but the driver is
undergoing a new initialisation and thus is not aware of already
activated IRQs in the HBA.

There was an issue reported by Lucz Geza on Dell Perc 6i. As per issue
reported, megaraid_sas driver goes into an infinite error reporting loop
as soon as there is a change in the status of one of the
arrays (degrade, resync online etc ). Below are the error logs reported

This issue is very much specific to controllers which do not support
DCMD- MR_DCMD_PD_LIST_QUERY. In case of any hotplugging/rescanning of
drives, AEN thread will be scheduled by driver and fire DCMD-
MR_DCMD_PD_LIST_QUERY and if this DCMD is failed then driver will fail
this event processing and will not go ahead for further events. This
will cause infinite loop of same event getting retried infinitely and
causing above mentioned logs.
Fix for this problem is: not to fire DCMD MR_DCMD_PD_LIST_QUERY for
controllers which do not support it and send DCMD SUCCESS status to AEN
function so that it can go ahead with other event processing.

In the receive path libfc extracts a cpu number from the ox_id in the
fiber channel header and uses that to do a per_cpu_ptr conversion. If,
for some reason, a frame is received with an invalid ox_id, per_cpu_ptr
will return an invalid pointer and the libfc receive path will panic the
system trying to use it.
I'm currently looking at such a case, and I don't yet know why a cpu
number > nr_cpu_ids is appearing in an exchange id. But adding a sanity
check in libfc prevents a system panic, and seems like good idea when
dealing with frames coming in from the network.

The driver creates its own per-CPU threads which are updated based on
CPU hotplug events. It is also possible to use kworkers and remove some
of the kthread infrastrucure.
The code checked ->thread to decide if there is an active per-CPU
thread. By using the kworker infrastructure this is no longer
possible (or required). The thread pointer is saved in `kthread' instead
of `thread' so anything trying to use thread is caught by the
compiler. Currently only the bnx2fc driver is using struct fcoe_percpu_s
and the kthread member.
After a CPU went offline, we may still enqueue items on the "offline"
CPU. This isn't much of a problem. The work will be done on a random
CPU. The allocated crc_eof_page page won't be cleaned up. It is probably
expected that the CPU comes up at some point so it should not be a
problem. The crc_eof_page memory is released of course once the module
is removed.
This patch was only compile-tested due to -ENODEV.

The fc_get_host_stats() function contains a complex conversion from
jiffies to timespec to seconds. As we try to get rid of uses of struct
timespec, we can clean this up and replace it with a simpler
computation.
Simply dividing the difference in jiffies by HZ is not only much more
efficient, it also avoids a problem that causes the
seconds_since_last_reset value to be incorrect if jiffies has overrun
since the 'boot_time' value was recorded.

Some CXL Flash cards need notification of device shutdown in order to
flush pending I/Os.
A PCI notification hook for shutdown has been added where the driver
notifies the card and returns. When the device is removed in the PCI
remove path, notification code will wait for shutdown processing to
complete.

Device dependent flags are needed to support functions that are specific
to a particular device.
One such case is - some CXL Flash cards need to be notified of device
shutdown. For other CXL devices, this feature does not prove to be
useful yet. Such distinct features need to be identified in the driver
to bypass or invoke specific functionality.
In this patch, a member 'flags' has been added to device dependent
values. These flags will be used and expanded in the future to support
various device specific functions.

The problem here is that the problem space area is unmapped while the
application issues the DK_CXLFLASH_RECOVER_AFU ioctl.
This is the order I observe:

1) sg_reset
2) ioctl(DK_CXLFLASH_RECOVER_AFU)
3) sg_reset again
causing a PSA unmap
4) continues RECOVER_AFU processing
The resolution to this problem is to have the reset handler drain all
outstanding user space initiated ioctls before proceeding. It is safe
to drain after the state has been changed to STATE_RESET. Also since
drain_ioctls() was static, it had to be moved up a bit to be before
cxlflash_eh_host_reset_handler().

author shows some kernel infrastructure complaining about a sleep in an
invalid context. Remove offending call to vmalloc(). Instead of using
kzalloc() which reviewers didn't like, use a bucket system (64 bytes on
the stack) and potentially multiple calls to sg_pcopy_from_buffer() to
construct the 'data-in' buffer for the SCSI REPORT LUNS command.

Originally libfc would just be initializing the refcount to '1', and
using the disc_mutex to synchronize if and when the final put should be
happening. This has a race condition as the mutex might be delayed,
causing other threads to access an invalid structure. This patch
updates the rport reference counting to increase the reference every
time 'rport_lookup' is called, and decreases the reference
correspondingly. This removes the need to hold 'disc_mutex' when
removing the structure, and avoids the above race condition.

If _scsih_sas_host_add's call to mpt3sas_config_get_sas_iounit_pg0
fails, ioc->sas_hba.parent_dev may be left uninitialized. A later
device probe could invoke mpt3sas_transport_port_add which will call
sas_port_alloc_num [scsi_transport_sas] with a NULL parent_dev pointer.

In _scsih_sas_host_add, the number of HBA phys are determined and then
later used to allocate an array of struct _sas_phy's. If the routine
sets ioc->sas_hba.num_phys, but then fails to allocate the
ioc->sas_hba.phy array (by kcalloc error or other intermediate
error/exit path), ioc->sas_hba is left in a dangerous state: all readers
of ioc->sas_hba.phy[] do so by indexing it from 0..ioc->sas_hba.num_phys
without checking that the space was ever allocated.
Modify _scsih_sas_host_add to set ioc->sas_hba.num_phys only after
successfully allocating ioc->sas_hba.phy[].

Commit b704f70ce2 ("SCSI: fix bug in scsi_dev_info_list matching")
changed the way vendor- and model-string matching was carried out in the
routine that looks up entries in a SCSI devinfo list. The new matching
code failed to take into account the case of a maximum-length string; in
such cases it could end up testing for a terminating '\0' byte beyond
the end of the memory allocated to the string. This out-of-bounds bug
was detected by UBSAN.
I don't know if anybody has actually encountered this bug. The symptom
would be that a device entry in the blacklist might not be matched
properly if it contained an 8-character vendor name or a 16-character
model name. Such entries certainly exist in scsi_static_device_list.
This patch fixes the problem by adding a check for a maximum-length
string before the '\0' test.

The untagged command case in the 53c700 driver has been broken since
host wide tags were enabled because the replaced scsi_find_tag()
function had a special case for the tag value SCSI_NO_TAG to retrieve
sdev->current_cmnd. The replacement function scsi_host_find_tag() has
no such special case and returns NULL causing untagged commands to
trigger a BUG() in the driver. Inspection shows that the 53c700 is the
only driver using this SCSI_NO_TAG case, so a local fix in the driver
suffices to fix this problem globally.

sas_ata_strategy_handler() adds the works of the ata error handler to
system_unbound_wq. This workqueue asynchronously runs work items, so the
ata error handler will be performed concurrently on different CPUs. In
this case, ->host_failed will be decreased simultaneously in
scsi_eh_finish_cmd() on different CPUs, and become abnormal.
It will lead to permanently inequality between ->host_failed and
->host_busy, and scsi error handler thread won't start running. IO
errors after that won't be handled.
Since all scmds must have been handled in the strategy handler, just
remove the decrement in scsi_eh_finish_cmd() and zero ->host_busy after
the strategy handler to fix this race.

This patch drops the compat definition of req_op where it matches
the rq_flag_bits definitions, and drops the related old and compat
code that allowed users to set either the op or flags for the operation.
We also then store the operation in the bi_rw/cmd_flags field similar
to how we used to store the bio ioprio where it sat in the upper bits
of the field.

We currently set REQ_WRITE/WRITE for all non READ IOs
like discard, flush, writesame, etc. In the next patches where we
no longer set up the op as a bitmap, we will not be able to
detect a operation direction like writesame by testing if REQ_WRITE is
set.
This patch converts the drivers and cgroup to use the
op_is_write helper. This should just cover the simple

For historic reasons, io_opt is in bytes and max_sectors in block layer
sectors. This interface inconsistency is error prone and should be
fixed. But for 4.4--4.7 let's make the unit difference explicit via a
wrapper function.

Only SRC-based adapters support the AifReqEvent function, so there is no

sd_check_events() is called asynchronously, and might race
with device removal. So always take a disk reference when
processing the event to avoid the device being removed while
the event is processed.

When SCSI was written, all commands coming from the filesystem
(REQ_TYPE_FS commands) had data. This meant that our signal for needing
to complete the command was the number of bytes completed being equal to
the number of bytes in the request. Unfortunately, with the advent of
flush barriers, we can now get zero length REQ_TYPE_FS commands, which
confuse this logic because they satisfy the condition every time. This
means they never get retried even for retryable conditions, like UNIT
ATTENTION because we complete them early assuming they're done. Fix
this by special casing the early completion condition to recognise zero
length commands with errors and let them drop through to the retry code.

iscsi_sw_tcp_data_ready() and iscsi_sw_tcp_state_change() were
using read_lock(&sk->sk_callback_lock) which is fine if caller
disabled BH.
TCP stack no longer has this requirement and can run from
process context.
Use read_lock_bh() variant to restore previous assumption.
Ideally this code could use RCU instead...

Some ethernet adapter vendors are supplying products which support optional
(payed license) features. On some adapters this includes a hardware iscsi
initiator. The same adapters in a normal (no extra licenses) mode of
operation can be used as a software iscsi initiator. In addition, software
iscsi boot initiators are becoming a standard part of many vendors uefi
implementations. This is creating difficulties during early boot/install
determining the proper configuration method for these adapters when they
are used as a boot device.
The attached patch creates sysfs entries to expose information from the
acpi header of the ibft table. This information allows for a method to
easily determining if an ibft table was created by a ethernet card's
firmware or the system uefi/bios. In the case of a hardware initiator this
information in combination with the pci vendor and device id can be used
to ascertain any vendor specific behaviors that need to be accommodated.

The iBFT table only specifies a prefix length, not a netmask.
And the netmask is pretty much pointless for IPv6.
So introduce a new attribute 'prefix-len'.
Some older user-space code might rely on the netmask attribute
being present, so we should always display it.

Replaced mpt3sas_base_flush_reply_queues() with
mpt3sas_base_sync_reply_irqs(),as mpt3sas_base_flush_reply_queues()
skips over reply queues that are currently busy (i.e. being handled by
interrupt processing in another core). If a reply queue is busy, then
call to synchronize_irq()in mpt3sas_base_sync_reply_irqs()make sures the
other core has finished flushing the queue and completed any calls to
the mid-layer scsi_done() routine.

In-order to handle this 'MPI2_EVENT_ACTIVE_CABLE_EXCEPTION' event,
driver need to follow below steps,
1. Unmask the 'MPI2_EVENT_ACTIVE_CABLE_EXCEPTION' event,
so that FW can notify this event to host driver.
2. After receiving this event, add this event to AEN event queue,
for notifying this event to applications.
3. Then Print below message in kernel logs if the event data's reason
code is zero,
"Currently an active cable with ReceptacleID <ID_Value> cannot be powered
and devices connected to this active cable will not be seen. This active
cable requires <PowerValue_in_mW> of power"
This event is only for Intruder/Cutlass HBAs.

The device ID obtained from the inquiry can only be of a single type.
The original code places a check for TYPE_ZBC right after the check for
TYPE_DISK. Logically, if the first if statement sees a device of a
TYPE_DISK and moves on to the second statement checking if not TYPE_ZBC,
it will always hit the continue.

Not every device will return a useable VPD identification, but still
might support ALUA. Rather than disable ALUA support we should be
allowing the device identification to be empty and attach individual
ALUA device handler to each devices.

Permit changing of a LU name from a (fake) IEEE registered NAA (5) to a
locally assigned UUID. Using a UUID (RFC 4122) for a SCSI designation
descriptor (e.g. a LU name) was added in spc5r08.pdf (a draft INCITS
standard) on 25 January 2016. Add parameter uuid_ctl to use a separate
UUID for each LU (storage device) name. Additional option for all LU
names to have the same UUID (since their storage is shared). Previous
action of using NAA identifier for LU name remains the default.

The caller assumes that "itnim" is NULL on error and non-NULL on success

When a cxlflash adapter goes into EEH recovery and multiple processes
(each having established its own context) are active, the EEH recovery
can hang if the processes attempt to recover in parallel. The symptom

The hang is because of a 3 way dead-lock:
Process A holds the recovery mutex, and waits for eehd to complete.
Process B holds the semaphore and waits for the recovery mutex.
eehd waits for semaphore.
The fix is to have Process B above release the semaphore before
attempting to acquire the recovery mutex. This will allow
eehd to proceed to completion.

The qla1280 driver sets the scsi_host_template's can_queue field to 0xfffff
which results in an allocation failure when allocating the block layer tags
for the driver's queues. This was introduced with the change for host wide
tags in commit 64d513ac31 - "scsi: use host wide tags by default".
Reduce can_queue to MAX_OUTSTANDING_COMMANDS (512) to solve the allocation
error.

It's possible to use "err" without initializing it. If it happens to be
a 2 which is SCSI_DH_RETRY then that could cause a bug. Bart Van Assche
pointed out that we should probably re-initialize it for every iteration
through the retry loop.

When a negative value was placed in the delay parameter, a tasklet was
scheduled. Change the tasklet to a work queue. Previously a delay of -1
scheduled a high priority tasklet; since there are no high priority work
queues, treat -1 like other negative values in delay and schedule a work
item.

The driver supports two command delay interfaces, the original one whose
unit is a jiffy, and a newer one whose unit is a nanosecond. Each had
different implementations. Keep both interfaces but simplify the
implemenation to use a single delay mechanism based on high resolution
timers.

The driver was calling scsi_scan_host before enabling interrupts.
This has gone unnoticed except for customers running in intx mode.
Calling scsi_scan_host before interrupts are enabled causes
"irq XX: nobody cared" messages and the driver to hang.
This patch enables interrupts before the call to scsi_scan_host.

When KDUMP is triggered the driver first talks to the firmware in INTX
mode, but the adapter firmware is still in MSIX mode. Therefore the first
driver command hangs since the driver is waiting for an INTX response and
firmware gives a MSIX response. If when the OS is installed on a RAID
drive created by the adapter KDUMP will hang since the driver does not
receive a response in sync mode.
Fixed by: Change the firmware to INTX mode if it is in MSIX mode before
sending the first sync command.

Currently driver completes double completed or spurious interrupted fibs.
This is not necessary and causes the SCSI mid layer to issue aborts and
resets, since completing a fib prematurely might trigger a race condition
resulting in the driver not calling the scsi_done callback.
Fixed by removing the call to fib complete.

Typically under error conditions, it is possible for aac_command_thread()
to miss the wakeup from kthread_stop() and go back to sleep, causing it
to hang aac_shutdown.
In the observed scenario, the adapter is not functioning correctly and so
aac_fib_send() never completes (or time-outs depending on how it was
called). Shortly after aac_command_thread() starts it performs
aac_fib_send(SendHostTime) which hangs. When aac_probe_one
/aac_get_adapter_info send time outs, kthread_stop is called which breaks
the command thread out of it's hang.
The code will still go back to sleep in schedule_timeout() without
checking kthread_should_stop() so it causes aac_probe_one to hang until
the schedule_timeout() which is 30 minutes.
Fixed by: Adding another kthread_should_stop() before schedule_timeout()

aac_fib_send has a special function case for initial commands during
driver initialization using wait < 0(pseudo sync mode). In this case,
the command does not sleep but rather spins checking for timeout.This
loop is calls cpu_relax() in an attempt to allow other processes/threads
to use the CPU, but this function does not relinquish the CPU and so the
command will hog the processor. This was observed in a KDUMP
"crashkernel" and that prevented the "command thread" (which is
responsible for completing the command from being timed out) from
starting because it could not get the CPU.
Fixed by replacing "cpu_relax()" call with "schedule()"

Current driver checks for NULL return from aac_fib_alloc_tag, but it not
possible for it to return NULL.

Some drives set the ILI flag together with MEDIUM ERROR sense code.
Clear the ILI flag in this case so that the medium error will be
handled. The problem was reported by Maurizio Lombardi.

Firmware events are queued up using the fw_event_work's struct work, not
its delayed_work member. The initial driver for SAS2 controllers had
handled firmware reset using the rescan barrier and was later redesigned
through "mpt2sas: [Resend] Host Reset code cleanup". The delayed_work
variables are now unused and may provoke CONFIG_DEBUG_OBJECTS_TIMERS
"assert_init not available" false warnings in
_scsih_fw_event_cleanup_queue.
Cleanup fw_event_work's unused entries, update its kerneldoc, and
update _scsih_fw_event_cleanup_queue accordingly.
