information.

The BUG_ON() recently introduced in lpfc_sli_ringtxcmpl_put() is hit in
the lpfc_els_abort() > lpfc_sli_issue_abort_iotag() >
lpfc_sli_abort_iotag_issue() function path [similar names], due to
'piocb->vport == NULL':

This happens because lpfc_sli_abort_iotag_issue() doesn't set the
'abtsiocbp->vport' pointer -- but this is not the problem.
Previously, lpfc_sli_ringtxcmpl_put() accessed 'piocb->vport' only if
'piocb->iocb.ulpCommand' is neither CMD_ABORT_XRI_CN nor
CMD_CLOSE_XRI_CN, which are the only possible values for
lpfc_sli_abort_iotag_issue():

So, this function path would not have hit this possible NULL pointer
dereference before.
In order to fix this regression, move the second part of the BUG_ON()
check prior to the pointer dereference that it does check for.
For reference, this is the stack trace observed. The problem happened
because an unsolicited event was received - a PLOGI was received after
our PLOGI was issued but not yet complete, so the discovery state
machine goes on to sw-abort our PLOGI.

Export fc_bsg_jobdone so drivers can use it directly instead of doing
the round-trip via struct fc_bsg_job::job_done() and use it in the
LLDDs. That way we can also unify the interfaces of fc_bsg_jobdone and
bsg_job_done.
As we've converted all LLDDs over to use fc_bsg_jobdone() directly, we
can remove the function pointer from struct fc_bsg_job as well.

A recent bugfix introduced a harmless warning in the lpfc driver:

drivers/scsi/lpfc/lpfc_logmsg.h:56:45: error: format '%ld' expects argument of type 'long int', but argument 9 has type 'size_t {aka const unsigned int}' [-Werror=format=]
'size_t' is always the same width as 'long' in the kernel, but the
compiler doesn't know that. The %z modifier is what the standard expects
to be used here, and this shuts up the warning.

Synchronize link speed with boot driver
Link speed settings set by the boot driver are reported by the hw.
Driver will attempt to read them, and if set, will respect their
values.
The driver can override the settings with its own if instructed by
user space (via bsg), with the new values being picked up by the
boot driver.

Correct panics with eh_timeout and eh_deadline
We were having double completions on our SLI-3 version of adapters.
Solved by clearing our command pointer before calling scsi_done.
The eh paths potentially ran simulatenously and would see the non-null
value and invoke scsi_done again.

When the switch is rebooted, the lpfc driver fails to log
into the fabric, and Unexpected timeout message is seen.

The default rpi completion handler does back to back puts to force the
removal of the ndlp. This ends up calling lpfc_unreg_rpi after the
reference count is at 0.
Fix: Check the reference count of the ndlp before getting the ref to
make sure we are not getting a reference on a removed object.

After a link bounce, when a remote port issues a LOGO while a REGLOGIN
is pending on that port, the driver does not clean up the ndlp
structure. May result in stack traces in the console log.

Several functions in lpfc have comments stating that the function must
be called with the hbalock (or hostlock, or ringlock) held. Add

This removes a redundant code block that will either be executed if the
ENABLE_FCP_RING_POLLING flag is set in phba->cfg_poll or not. The code
is just duplicated in both cases, hence we unify it again.

Fix the FLOGI discovery logic to comply with T11 standards
We weren't properly setting fabric parameters, such as R_A_TOV and E_D_TOV,
when we registered the vfi object in default configs and pt2pt configs.
Revise to now pass service params with the values to the firmware and
ensure they are reset on link bounce. Required reworking the call sequence
in the discovery threads.

This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags. We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

kmalloc() can return NULL and without checking we were dereferencing it.
Moreover if kmalloc succeeds but the function fails in other parts then
we were returning the error code but we missed freeing lcb_context.
While at it fixed one related checkpatch warning.

Fix for discovery failure in PT2PT when FLOGI's ELS ACC response gets aborted
Change login state machine to:
- Restart FLOGI if prior is ABTS'd
- Reject incoming FLOGIs if we have one pending
The above ensures that we always finish FLOGI processing, regardless
of who initated FLOGI, before processing PLOGI's.

lpfc_send_rscn_event() allocates data for sizeof(struct
lpfc_rscn_event_header) + payload_len, but claims that the data has size
of sizeof(struct lpfc_els_event_header) + payload_len. That leads to
buffer overruns.

Currently the module parameter lpfc_sg_seg_count does not have effect
for sli3 devices.
In lpfc_sli_driver_resource_setup(), which is used for sli3, the code
writes the configured sg_seg_cnt into lpfc_template.sg_tablesize.
But lpfc_template is the template used for sli4 only. Thus the value should
correctly be written to lpfc_template_s3->sg_tablesize.
This patch is for kernel 4.1-rc5, but is tested with lpfc 10.2.405.26 only.

If the bf_get() call in lpfc_mbx_cmpl_rdp_page_a2() does succeeds, execution
continues normally and mp gets kfree()d.
If the subsequent call to lpfc_sli_issue_mbox() fails execution jumps to the
error label where lpfc_mbuf_free() is called with mp->virt and mp->phys as
function arguments. This is the use after free. Following the use after free mp
gets kfree()d again which is a double free.

The domain controller PLOGI's concurrent with prior LOGO's/unreg_rpi's
completing created a race condition where driver rpi ref count can
inadvertantly hit 0 and the rpi attempted to be freed. This error
sometimes resulted in Warning messages indicating kref.h via
lfpc_nlp_get+0x128.
Correct by dropping any new PLOGI until the prior nport state has settled.

TMF's were getting error messages on FCP_RSP errors (underrun). Underruns
aren't meaningful in the scenario. Change the error message to filter out
these response check errors, and don't unconditionally mark the cmd as
in error.

The driver inadvertainly constrained the id space to 8 bits, when its
a full 16 bits from hw. This resulted in bad id's being written to the
hardware. Resulted in symptoms such as incomplete initialization, no
detection of link events, etc.

With blk-mq support in the mid-layer, lpfc can do IO steering based
on the information in the request tag. This patch allows lpfc to use
blk-mq if enabled. If not enabled, we fall back into the emulex-internal
affinity mappings.
This feature can be turned on via CONFIG_SCSI_MQ_DEFAULT or passing
scsi_mod.use_blk_mq=Y as a parameter to the kernel.

When a remote nport changes it's DID, a new ndlp is used.
However, we left the old ndlp state unchanged and still in
a discovery state. The may stall discovery resulting in some
devices not being discovered. Correct by swapping the state
of the 2 ndlp's when a DID swap is detected.

If a vport was deleted while in the middle of discovery, we weren't
clearing the nport discovery flag. Correct by clearing the flag and
cancelling our discovery timeout timer.

LCB requests to set Beacon would fail if the beacon was already enabled
internally as the mailbox command used to query the state failes with
an already-set status.
Correct by enhancing the check so we don't fail if if the already set
status comes back.

The order (it's a shall, but hard to dictate after the fact) is given in
FC-SCM - kind of. SCM indicates what shall be implemented, lists it as (a),
(b), (c), but actually doesn't say it has to be in that order. The only hard
requirement, called out in FCP-4, is that you must register your FC-4 Type
(via RFT_ID) before registering FC-4 Type Features (via RFF_ID), which makes
sense. We obviously violated this and there were some switches (or newer fw in
them) that enforced it. The other rule of thumbs are: register your data with
the switch first, then register for SCRs, then do queries about the fabric,
with the SCRs telling you of changes post the queries.

This reverts 4fbdf9cb it breaks LPFC on POWER7 machine, big endian kernel.

Currently, the driver plays off the fact that older sli4 adapters have a
different rpi access pattern that allowed for the rpi reference to be
released earlier in the teardown sequence, allowing the driver to recycle
the rpi value sooner. Newer sli4 adapters have a different access pattern that
requires us to wait for a later mailbox completion. This changes the put
call location on the newer sli4 adapters.
Symptoms of the error are "0110 ELS" and the "0372 iotag" errors.
