In ESA mode STCKF is not defined even if the facility bit is enabled.
To prevent an illegal operation we must also check if we run a 64 bit kernel.
To make the check perform well add the STCKF bit to the machine flags.

The pgste_update_all / pgste_update_young and pgste_set_pte need to
check if the pte entry contains a valid page address before the storage
key can be accessed. In addition pgste_set_pte needs to set the access
key and fetch protection bit of the new pte entry, not the old entry.

The 802.1X EAPOL handshake hostapd does requires
knowing whether the frame was ack'ed by the peer.
Currently, we fudge this pretty badly by not even
transmitting the frame as a normal data frame but
injecting it with radiotap and getting the status
out of radiotap monitor as well. This is rather
complex, confuses users (mon.wlan0 presence) and
doesn't work with all hardware.
To get rid of that hack, introduce a real wifi TX
status option for data frame transmissions.
This works similar to the existing TX timestamping
in that it reflects the SKB back to the socket's
error queue with a SCM_WIFI_STATUS cmsg that has
an int indicating ACK status (0/1).
Since it is possible that at some point we will
want to have TX timestamping and wifi status in a
single errqueue SKB (there's little point in not
doing that), redefine SO_EE_ORIGIN_TIMESTAMPING
to SO_EE_ORIGIN_TXSTATUS which can collect more
than just the timestamp; keep the old constant
as an alias of course. Currently the internal APIs
don't make that possible, but it wouldn't be hard
to split them up in a way that makes it possible.

We use both the external call and emergency call IPIs to signal remote
cpus. Therefore it makes sense to account them differently withing
/proc/irqstats so we actually know what happened.

gmap_fault needs to walk the guest page table. However, parts of
that may change if some other thread does munmap. In that case
gmap_unmap_notifier will also unmap the corresponding parts from
the guest page table. We need to take mmap_sem in order to serialize
these operations.
do_exception now calls __gmap_fault with mmap_sem held which does
not get exported to modules. The exported function, which is called
from KVM, now takes mmap_sem.

The user space program can change its addressing mode between the
24-bit, 31-bit and the 64-bit mode if the kernel is 64 bit. Currently
the kernel always forces the standard amode on signal delivery and
signal return and on ptrace: 64-bit for a 64-bit process, 31-bit for
a compat process and 31-bit kernels. Change the signal and ptrace code
to allow the full range of addressing modes. Signal handlers are
run in the standard addressing mode for the process.
One caveat is that even an 31-bit compat process can switch to the
64-bit mode. The next signal will switch back into the 31-bit mode
and there is no room in the 31-bit compat signal frame to store the
information that the program came from the 64-bit mode.

Split out addressing mode bits from PSW_BASE_BITS, rename PSW_BASE_BITS
to PSW_MASK_BASE, get rid of psw_user32_bits, remove unused function
enabled_wait(), introduce PSW_MASK_USER, and drop PSW_MASK_MERGE macros.
Change psw_kernel_bits / psw_user_bits to contain only the bits that
are always set in the respective mode.

Add an explicit TIF_SYSCALL bit that indicates if a task is inside
a system call. The svc_code in the pt_regs structure is now only
valid if TIF_SYSCALL is set. With this definition TIF_RESTART_SVC
can be replaced with TIF_SYSCALL. Overall do_signal is a bit more
readable and it saves a few lines of code.

An instruction with an address right below the adress limit for the
current addressing mode will wrap. The instruction restart logic in
the protection fault handler and the signal code need to follow the
wrapping rules to find the correct instruction address.

For a ERESTARTNOHAND/ERESTARTSYS/ERESTARTNOINTR restarting system call
do_signal will prepare the restart of the system call with a rewind of
the PSW before calling get_signal_to_deliver (where the debugger might
take control). For A ERESTART_RESTARTBLOCK restarting system call
do_signal will set -EINTR as return code.
There are two issues with this approach:
1) strace never sees ERESTARTNOHAND, ERESTARTSYS, ERESTARTNOINTR or
ERESTART_RESTARTBLOCK as the rewinding already took place or the
return code has been changed to -EINTR
2) if get_signal_to_deliver does not return with a signal to deliver
the restart via the repeat of the svc instruction is left in place.
This opens a race if another signal is made pending before the
system call instruction can be reexecuted. The original system call
will be restarted even if the second signal would have ended the
system call with -EINTR.
These two issues can be solved by dropping the early rewind of the
system call before get_signal_to_deliver has been called and by using
the TIF_RESTART_SVC magic to do the restart if no signal has to be
delivered. The only situation where the system call restart via the
repeat of the svc instruction is appropriate is when a SA_RESTART
signal is delivered to user space.
Unfortunately this breaks inferior calls by the debugger again. The
system call number and the length of the system call instruction is
lost over the inferior call and user space will see ERESTARTNOHAND/
ERESTARTSYS/ERESTARTNOINTR/ERESTART_RESTARTBLOCK. To correct this a
new ptrace interface is added to save/restore the system call number
and system call instruction length.

This patch implements the crash_map_pages() function for s390.
KEXEC_CRASH_MEM_ALIGN is set to HPAGE_SIZE, in order to support
kernel mappings that use large pages. We also use HPAGE_SIZE alignment
for CONFIG_HUGETLB_PAGE=n in order to have the same 1 MiB alignment on
all s390 systems.

PSW restart can be triggered on offline CPUs. If this happens, currently
the PSW restart code fails, because functions like smp_processor_id()
do not work on offline CPUs. This patch fixes this as follows:
If PSW restart is triggered on an offline CPU, the PSW restart (sigp restart)
is done a second time on another CPU that is online and the old CPU is
stopped afterwards.

Implement sigp external call, which might be required for guests that

Fix up trivial conflict in fs/gfs2/file.c (llseek changes)
This was found by inspection while tracking a similar
bug in compat_statfs64, that has been fixed in mainline
since decemeber.
- This fixes a bug where not all of the f_spare fields
were cleared on mips and s390.
- Add the f_flags field to struct compat_statfs
- Copy f_flags to userspace in case someone cares.
- Use __clear_user to copy the f_spare field to userspace
to ensure that all of the elements of f_spare are cleared.
On some architectures f_spare is has 5 ints and on some
architectures f_spare only has 4 ints. Which makes
the previous technique of clearing each int individually
broken.
I don't expect anyone actually uses the old statfs system
call anymore but if they do let them benefit from having
the compat and the native version working the same.

598841ca99 ([S390] use gmap address
spaces for kvm guest images) changed kvm to use a separate address
space for kvm guests. This address space was switched in __vcpu_run
In some cases (preemption, page fault) there is the possibility that
this address space switch is lost.
The typical symptom was a huge amount of validity intercepts or
random guest addressing exceptions.
Fix this by doing the switch in sie_loop and sie_exit and saving the
address space in the gmap structure itself. Also use the preempt
notifier.

With this patch a new S390 shutdown trigger "restart" is added. If under
z/VM "systerm restart" is entered or under the HMC the "PSW restart" button
is pressed, the PSW located at 0 (31 bit) or 0x1a0 (64 bit) bit is loaded.
Now we execute do_restart() that processes the restart action that is
defined under /sys/firmware/shutdown_actions/on_restart. Currently the
following actions are possible: reipl (default), stop, vmcmd, dump, and
dump_reipl.

After changing all consumers of atomics to include <linux/atomic.h>, we
ran into some compile time errors due to this dependency chain:
linux/atomic.h
-> asm/atomic.h
-> asm-generic/atomic-long.h
where atomic-long.h could use funcs defined later in linux/atomic.h
without a prototype. This patches moves the code that includes
asm-generic/atomic*.h to linux/atomic.h.
Archs that need <asm-generic/atomic64.h> need to select
CONFIG_GENERIC_ATOMIC64 from now on (some of them used to include it
unconditionally).
Compile tested on i386 and x86_64 with allnoconfig.

This patch removes kvm-s390 internal assumption of a linear mapping
of guest address space to user space. Previously, guest memory was
translated to user addresses using a fixed offset (gmsor). The new
code uses gmap_fault to resolve guest addresses.

Add code that allows KVM to control the virtual memory layout that
is seen by a guest. The guest address space uses a second page table
that shares the last level pte-tables with the process page table.
If a page is unmapped from the process page table it is automatically
unmapped from the guest page table as well.
The guest address space mapping starts out empty, KVM can map any
individual 1MB segments from the process virtual memory to any 1MB
aligned location in the guest virtual memory. If a target segment in
the process virtual memory does not exist or is unmapped while a
guest mapping exists the desired target address is stored as an
invalid segment table entry in the guest page table.
The population of the guest page table is fault driven.

The alignment is missing for various global symbols in s390 assembly code.
With a recent gcc and an instruction like stgrl this can lead to a
specification exception if the instruction uses such a mis-aligned address.
Specify the alignment explicitely and while add it define __ALIGN for s390
and use the ENTRY define to save some lines of code.

The entry to / exit from sie has subtle dependencies to the first level
interrupt handler. Move the sie assembler code to entry64.S and replace
the SIE_HOOK callback with a test and the new _TIF_SIE bit.
In addition this patch fixes several problems in regard to the check for
the_TIF_EXIT_SIE bits. The old code checked the TIF bits before executing
the interrupt handler and it only modified the instruction address if it
pointed directly to the sie instruction. In both cases it could miss
a TIF bit that normally would cause an exit from the guest and would
reenter the guest context.

When running a kvm guest we can get intercepts for tprot, if the host
page table is read-only or not populated. This patch implements the
most common case (linux memory detection).
This also allows host copy on write for guest memory on newer systems.

Do not trace arch_local_save_flags(), arch_local_irq_*() and friends.
Although they are marked inline, gcc may still make a function out of
them and add it to the pool of functions that are traced by the function
tracer. This can cause undesirable results (kernel panic, triple faults,
etc).
Add the notrace notation to prevent them from ever being traced.

Replace the s390 specific rcu page-table freeing code with the
generic variant. This requires to duplicate the definition for the
struct mmu_table_batch as s390 does not use the generic tlb flush
code.
While we are at it remove the restriction that page table fragments
can not be reused after a single fragment has been freed with rcu
and split out allocation and freeing of page tables with pgstes.

The qdio SBAL entry flag is made-up of four different values that are
independent of one another. Some of the bits are reserved by the
hardware and should not be changed by qdio. Currently all four values
are overwritten since the SBAL entry flag is defined as an u32.
Split the SBAL entry flag into four u8's as defined by the hardware
and don't touch the reserved bits.

page_get_storage_key() and page_set_storage_key() expect a page address
and not its page frame number. This got inconsistent with 2d42552d
"[S390] merge page_test_dirty and page_clear_dirty".
Result is that we read/write storage keys from random pages and do not
have a working dirty bit tracking at all.
E.g. SetPageUpdate() doesn't clear the dirty bit of requested pages, which
for example ext4 doesn't like very much and panics after a while.

32bit and 64bit on x86 are tested and working. The rest I have looked
at closely and I can't find any problems.
setns is an easy system call to wire up. It just takes two ints so I
don't expect any weird architecture porting problems.
While doing this I have noticed that we have some architectures that are
very slow to get new system calls. cris seems to be the slowest where
the last system calls wired up were preadv and pwritev. avr32 is weird
in that recvmmsg was wired up but never declared in unistd.h. frv is
behind with perf_event_open being the last syscall wired up. On h8300
the last system call wired up was epoll_wait. On m32r the last system
call wired up was fallocate. mn10300 has recvmmsg as the last system
call wired up. The rest seem to at least have syncfs wired up which was
new in the 2.6.39.

Interrupt sources like pfault, sclp, dasd_diag and virtio all use the
service signal external interrupt subclass mask in control register 0
to enable and disable the corresponding interrupt.
Because no reference counting is implemented each subsystem thinks it
is the only user of subclass and sets and clears the bit like it wants.
This leads to case that unloading the dasd diag module under z/VM
causes both sclp and pfault interrupts to be masked. The result will
be locked up system sooner or later.
Fix this by introducing a new way to set (register) and clear
(unregister) the service signal subclass mask bit in cr0.
Also convert all drivers.

Rework the architecture page table functions to access the bits in the
page table extension array (pgste). There are a number of changes:
1) Fix missing pgste update if the attach_count for the mm is <= 1.
2) For every operation that affects the invalid bit in the pte or the
rcp byte in the pgste the pcl lock needs to be acquired. The function
pgste_get_lock gets the pcl lock and returns the current pgste value
for a pte pointer. The function pgste_set_unlock stores the pgste
and releases the lock. Between these two calls the bits in the pgste
can be shuffled.
3) Define two software bits in the pte _PAGE_SWR and _PAGE_SWC to avoid
calling SetPageDirty and SetPageReferenced from pgtable.h. If the
host reference backup bit or the host change backup bit has been
set the dirty/referenced state is transfered to the pte. The common
code will pick up the state from the pte.
4) Add ptep_modify_prot_start and ptep_modify_prot_commit for mprotect.
5) Remove pgd_populate_kernel, pud_populate_kernel, pmd_populate_kernel
pgd_clear_kernel, pud_clear_kernel, pmd_clear_kernel and ptep_invalidate.
6) Rename kvm_s390_test_and_clear_page_dirty to
ptep_test_and_clear_user_dirty and add ptep_test_and_clear_user_young.
7) Define mm_exclusive() and mm_has_pgste() helper to improve readability.

The page_clear_dirty primitive always sets the default storage key
which resets the access control bits and the fetch protection bit.
That will surprise a KVM guest that sets non-zero access control
bits or the fetch protection bit. Merge page_test_dirty and
page_clear_dirty back to a single function and only clear the
dirty bit from the storage key.
In addition move the function page_test_and_clear_dirty and
page_test_and_clear_young to page.h where they belong. This
requires to change the parameter from a struct page * to a page
frame number.

On cpu hot remove a PFAULT CANCEL command is sent to the hypervisor
which in turn will cancel all outstanding pfault requests that have
been issued on that cpu (the same happens with a SIGP cpu reset).
The result is that we end up with uninterruptible processes where
the interrupt that would wake up these processes never arrives.
In order to solve this all processes which wait for a pfault
completion interrupt get woken up after a cpu hot remove. The worst
case that could happen is that they fault again and in turn need to
wait again.

Implement arch specific irqsafe_cpu ops. The arch specific ops do not
disable/enable interrupts since that is an expensive operation. Instead
we disable preemption and perform a compare and swap loop.
Since on server distros (the ones we care about) preemption is disabled
the preempt_disable()/preempt_enable() pair is a nop.
In the end this code should be faster than the generic one.

The noexec support on s390 does not rely on a bit in the page table
entry but utilizes the secondary space mode to distinguish between
memory accesses for instructions vs. data. The noexec code relies
on the assumption that the cpu will always use the secondary space
page table for data accesses while it is running in the secondary
space mode. Up to the z9-109 class machines this has been the case.
Unfortunately this is not true anymore with z10 and later machines.
The load-relative-long instructions lrl, lgrl and lgfrl access the
memory operand using the same addressing-space mode that has been
used to fetch the instruction.
This breaks the noexec mode for all user space binaries compiled
with march=z10 or later. The only option is to remove the current
noexec support.

While debugging I stumbled over two problems in the code that protects module
pages.
First issue is that disabling the protection before freeing init or unload of
a module is not symmetric with the enablement. For instance, if pages are set
to RO the page range from module_core to module_core + core_ro_size is
protected. If a module is unloaded the page range from module_core to
module_core + core_size is set back to RW.
So pages that were not set to RO are also changed to RW.
This is not critical but IMHO it should be symmetric.
Second issue is that while set_memory_rw & set_memory_ro are used for
RO/RW changes only set_memory_nx is involved for NX/X. One would await that
the inverse function is called when the NX protection should be removed,
which is not the case here, unless I'm missing something.

Processes started with kernel_execve from a kernel thread will have
current->mm==NULL. Reading current->mm->context.alloc_pgste will
read a more or less random bit from lowcore in this case. If the
bit turns out to be set the whole process tree started this way
will allocate page table extensions although they have no need
for it.

Currently the diag10() function can only release one page. For exploiters
that have to call diag10 on a contiguous memory region this is suboptimal.
This patch replaces the diag10() function with diag10_range() that is
able to release multiple pages. In addition to that the new function now
allows to release memory with addresses higher than 2047 MiB. This was
due to a restriction of the diagnose implementation under z/VM prior to
release 5.2.

Implement the architecture backend for jump label support on s390.
For a shared kernel booted from a NSS silently disable jump labels
because the NSS is read-only. Therefore jump labels will be disabled
in a shared kernel and can't be activated.

minix bit operations are only used by minix filesystem and useless by
other modules. Because byte order of inode and block bitmaps is different
on each architecture like below:
m68k:
big-endian 16bit indexed bitmaps
h8300, microblaze, s390, sparc, m68knommu:
big-endian 32 or 64bit indexed bitmaps
m32r, mips, sh, xtensa:
big-endian 32 or 64bit indexed bitmaps for big-endian mode
little-endian bitmaps for little-endian mode
Others:
little-endian bitmaps
In order to move minix bit operations from asm/bitops.h to architecture
independent code in minix filesystem, this provides two config options.
CONFIG_MINIX_FS_BIG_ENDIAN_16BIT_INDEXED is only selected by m68k.
CONFIG_MINIX_FS_NATIVE_ENDIAN is selected by the architectures which use
native byte order bitmaps (h8300, microblaze, s390, sparc, m68knommu,
m32r, mips, sh, xtensa). The architectures which always use little-endian
bitmaps do not select these options.
Finally, we can remove minix bit operations from asm/bitops.h for all
architectures.

As the result of conversions, there are no users of ext2 non-atomic bit
operations except for ext2 filesystem itself. Now we can put them into
architecture independent code in ext2 filesystem, and remove from

Introduce little-endian bit operations by renaming native ext2 bit
operations. The ext2 bit operations are kept as wrapper macros using
little-endian bit operations to maintain bisectability until the
conversions are finished.

Move xchg() and cmpxchg() functions to own header file like some
other architectures have done.
With this we make sure that system.h now really looks like a place
where everything is gathered that doesn't fit anywhere else.

Implement write protection for kernel modules text and read-only
data sections by implementing set_memory_[ro|rw] on s390.
Since s390 has no execute bit in the pte's NX is not supported.
set_memory_[ro|rw] will only work on normal pages and not on
large pages, so in case a large page should be modified bail
out with a warning.

The cmpxchg_futex_value_locked API was funny in that it returned either
the original, user-exposed futex value OR an error code such as -EFAULT.
This was confusing at best, and could be a source of livelocks in places
that retry the cmpxchg_futex_value_locked after trying to fix the issue
by running fault_in_user_writeable().
This change makes the cmpxchg_futex_value_locked API more similar to the
get_futex_value_locked one, returning an error code and updating the
original value through a reference argument.

NET_SKB_PAD has been increased from 32 to 64 and later to
max(32, L1_CACHE_BYTES). This led to a 25% throughput decrease for
streaming workloads accompanied by a 37% CPU cost increase on s390.
Define a architecture specific NET_SKB_PAD with the old value of 32.

task_show_regs used to be a debugging aid in the early bringup days
of Linux on s390. /proc/<pid>/status is a world readable file, it
is not a good idea to show the registers of a process. The only
correct fix is to remove task_show_regs.

All architecture specific rwsem headers carry the same function
prototypes. Just x86 adds asmregparm, which is an empty define on all
other architectures. S390 has a stale rwsem_downgrade_write()
prototype.

The rwsem initializers and related macros and functions are mostly the
same. Some of them lack the lockdep initializer, but having it in
place does not matter for architectures which do not support lockdep.
powerpc, sparc, x86: No functional change
sh, s390: Removes the duplicate init_rwsem (inline and #define)
alpha, ia64, xtensa: Use the lockdep capable init function in
lib/rwsem.c which is just uninlining the init
function for the LOCKDEP=n case

The difference between these declarations is the data type of the
count member and the lack of lockdep in some architectures/
long is equivivalent to signed long and the #ifdef guarded dep_map
member does not hurt anyone.

Consider stack address randomization when calulating mmap_base for
flexible mmap layout . Because of address randomization the stack
address can be up to 8MB lower than STACK_TOP.
When calculating mmap_base this isn't taken into account, which could
lead to the case that the gap between the real stack top and mmap_base
is lower than what ulimit specifies for the maximum stack size.

Get rid of messages that indicate if a cpu went online or offline.
There is nothing special about this anymore and these messages might
flood the kernel log buffer which makes debugging harder since more
important messages might be overwritten.

The spinning mutex implementation uses cpu_relax() in busy loops as a
compiler barrier. Depending on the architecture, cpu_relax() may do more
than needed in this specific mutex spin loops. On System z we also give
up the time slice of the virtual cpu in cpu_relax(), which prevents
effective spinning on the mutex.
This patch replaces cpu_relax() in the spinning mutex code with
arch_mutex_cpu_relax(), which can be defined by each architecture that
selects HAVE_ARCH_MUTEX_CPU_RELAX. The default is still cpu_relax(), so
this patch should not affect other architectures than System z for now.

Call init_idle() which (re-)initializes the idle task structure before
it gets used on a new cpu.
That way we can also get rid of the odd preempt_enable_no_resched()
call we have in the cpu offline path within cpu_idle(). That call
prevented preempt count imbalances between cpu hotplug operations.

Normal I/O operations through the DASD device driver give only access
to the data fields of an ECKD device even for track based I/O.
This patch extends the DASD device driver to give access to whole
ECKD tracks including count, key and data fields.

If a DASD device has been reserved by a Linux system, and later
this reservation is ‘stolen’ by a second system by means of an
unconditional reserve, then the first system receives a
notification about this fact. With this patch such an event can
be either ignored, as before, or it can be used to let the device
fail all I/O request, so that the device will not block anymore.

arch_needs_cpu() gets always executed on the current cpu. Therefore
the cpu parameter can be ignored it is possible to use __get_cpu_var()
instead of per_cpu() to access the per_cpu variable, which will
generate better code.

Introduce a scan treshold for the qdio outbound queues. By setting the
threshold the driver can tell qdio after how much used SBALs qdio
should schedule the outbound tasklet that scans the queue for finished
SBALs. The threshold is specific by the drivers because a
Hipersockets device is much faster in utilizing outbound buffers than a
ZFCP or OSA device.
The default values after how many used SBALs the tasklet should run are:
OSA: > 31 SBALs
Hipersockets: > 7 SBALs
zfcp: > 55 SBALs

Use an early init call to initialize pfault. That way it is possible to
use the register_external_interrupt() instead of the early variant.
No need to enable pfault any earlier since it has only effect if user
space processes are running.

Up to now /proc/interrupts only has statistics for external and i/o
interrupts but doesn't split up them any further.
This patch adds a line for every single interrupt source so that it
is possible to easier tell what the machine is/was doing.

Let local_tick_enable/disable() reprogram the clock comparator so the
function names make semantically more sense.
Also that way the functions are more symmetric since normally each
local_tick_enable() call usually would have a subsequent call to
set_clock_comparator() anyway.

The saved interrupt mask and the saved control registers are only
relevant while single stepping is set up. A secondary kprobe while
kprobe single stepping is active may not occur. That makes is safe
to remove the save and restore of kprobe_saved_imask / kprobe_save_ctl
from save_previous_kprobe and restore_previous_kprobe.
Move all single step related code to two functions, enable_singlestep
and disable_singlestep.

Function qeth_l3_arp_query now queries for IPv6 addresses, too, if
QETH_QARP_WITH_IPV6 is passed as parameter to the ioctl. HiperSockets
and GuestLAN in HiperSockets mode provide corresponding entries.

The spinning mutex implementation uses cpu_relax() in busy loops as a
compiler barrier. Depending on the architecture, cpu_relax() may do more
than needed in this specific mutex spin loops. On System z we also give
up the time slice of the virtual cpu in cpu_relax(), which prevents
effective spinning on the mutex.
This patch replaces cpu_relax() in the spinning mutex code with
arch_mutex_cpu_relax(), which can be defined by each architecture that
selects HAVE_ARCH_MUTEX_CPU_RELAX. The default is still cpu_relax(), so
this patch should not affect other architectures than System z for now.

The BIODASDSNID ioctl executes a 'Sense Path Group ID'
command on a DASD ECKD device. The returned path group data
allows user space programs to determine path state and
path group ID of the channel paths to the device.

The taskstats interface uses microsecond granularity for the user and
system time values. The conversion from cputime to the taskstats values
uses the cputime_to_msecs primitive which effectively limits the
granularity to milliseconds. Add the cputime_to_usecs primitive for
architectures that have better, more precise CPU time values. Remove
cputime_to_msecs primitive because there are no more users left.

Export the cpu configuration topology via sysinfo. Two new lines are
introduced:
CPU Topology HW: 0 0 0 4 6 4
CPU Topology SW: 0 0 0 0 4 24
The HW line describes the cpu topology nesting levels when the maximum
nesting level is used to get the corresponding SYSIB.
The SW line describes what Linux is actually using. In this case it
supports only two levels (CONFIG_SCHED_BOOK off) and therefore the
hardware folded the two lower levels in the SYSIB response block.

The ieee_instruction_pointer can not be read from user space anymore
since git commit 613e1def6b, the ptrace
interface always returns zero. Remove it from the thread_struct. It
is still present in the user_regs_struct for compatability reasons.

So move the function to pgtable.h on s390 as well.
Fixes a compile error introduced with "hugetlb: hugepage migration core"

If the previous task was a kernel thread there is no need to save the
contents of the fpu and access registers since they aren't used in
kernel mode.
For the same reason it is not necessary to restore these registers if
the next task is a kernel thread.

Improve performance of the sske operation by using the nonquiescing
variant if the affected page has no mappings established. On machines
with no support for the new sske variant the mask bit will be ignored.

Set ARCH_HAS_SG_CHAIN for scatter lists on s390. Without this flag the
SCSI code limits the maximum number of segments, so set it to make

If the zero page is mapped to virtual user space addresses that differ
only in bit 2^12 or 2^13 we get L1 cache synonyms which can affect
performance. Follow the mips model and use multiple zero pages to avoid
the synonyms.

As suggested by Christian, we should expose headers to user space with
information that might be valuable there. The s390 virtio interface is
one of those cases. It defines an ABI between hypervisor and guest, so
it should be exposed to user space.

The one big missing feature in s390-virtio was hotplugging. This is no more.
This patch implements hotplug add support, so you can on the fly add new devices
in the guest.
Keep in mind that this needs a patch for qemu to actually leverage the
functionality.

Currenty the ext_param field only distinguishes between "config change" and
"vring interrupt". We can do a lot more with it though, so let's enable a
full byte of possible values and constants to #defines while at it.

This patch converts s390 to use asm-generic/ioctls.h instead of its
own version.
The differences between the arch-specific version and the generic
version are as follows:
- S390 defines its own value for FIOQSIZE, asm-generic/ioctls.h keeps it
- The generic version adds TIOCGRS485 and TIOCGRS485, which are unused
by any driver available on this architecture
- The generic version adds support for termiox

Provide a mechanism that allows running code in IRQ context. It is
most useful for NMI code that needs to interact with the rest of the
system -- like wakeup a task to drain buffers.
Perf currently has such a mechanism, so extract that and provide it as
a generic feature, independent of perf so that others may also
benefit.
The IRQ context callback is generated through self-IPIs where
possible, or on architectures like powerpc the decrementer (the
built-in timer facility) is set to generate an interrupt immediately.
Architectures that don't have anything like this get to do with a
callback from the timer tick. These architectures can call
irq_work_run() at the tail of any IRQ handlers that might enqueue such
work (like the perf IRQ handler) to avoid undue latencies in
processing the work.

Fix the IRQ flag handling naming. In linux/irqflags.h under one configuration,
it maps:
local_irq_enable() -> raw_local_irq_enable()
local_irq_disable() -> raw_local_irq_disable()
local_irq_save() -> raw_local_irq_save()
...
and under the other configuration, it maps:
raw_local_irq_enable() -> local_irq_enable()
raw_local_irq_disable() -> local_irq_disable()

This is quite confusing. There should be one set of names expected of the
arch, and this should be wrapped to give another set of names that are expected
by users of this facility.
Change this to have the arch provide:

compat_alloc_user_space() expects the caller to independently call
access_ok() to verify the returned area. A missing call could
introduce problems on some architectures.
This patch incorporates the access_ok() check into
compat_alloc_user_space() and also adds a sanity check on the length.
The existing compat_alloc_user_space() implementations are renamed
arch_compat_alloc_user_space() and are used as part of the
implementation of the new global function.
This patch assumes NULL will cause __get_user()/__put_user() to either
fail or access userspace on all architectures. This should be
followed by checking the return value of compat_access_user_space()
for NULL in the callers, at which time the access_ok() in the callers
can also be removed.

Extend the qdio API to allow polling in the upper-layer driver. This
is needed by qeth to use NAPI.
To use the new interface the upper-layer driver must specify the
queue_start_poll(). This callback is used to signal the upper-layer
driver that is has initiative and must process the inbound queue by
calling qdio_get_next_buffers(). If the upper-layer driver wants to
stop polling it calls qdio_start_irq().
Since adapter interrupts are not completely stoppable qdio implements
a software bit QDIO_QUEUE_IRQS_DISABLED to safely disable interrupts for an
input queue.
The old interface is preserved and will be used as is by zfcp.

The tlb flushing code uses the mm_users field of the mm_struct to
decide if each page table entry needs to be flushed individually with
IPTE or if a global flush for the mm_struct is sufficient after all page
table updates have been done. The comment for mm_users says "How many
users with user space?" but the /proc code increases mm_users after it
found the process structure by pid without creating a new user process.
Which makes mm_users useless for the decision between the two tlb
flusing methods. The current code can be confused to not flush tlb
entries by a concurrent access to /proc files if e.g. a fork is in
progres. The solution for this problem is to make the tlb flushing
logic independent from the mm_users field.

These are the changes needed for the kernel to support
LINEMODE in the server.
There is a new bit in the termios local flag word, EXTPROC.
When this bit is set, several aspects of the terminal driver
are disabled. Input line editing, character echo, and mapping
of signals are all disabled. This allows the telnetd to turn
off these functions when in linemode, but still keep track of
what state the user wants the terminal to be in.
New ioctl:
TIOCSIG Generate a signal to processes in the
current process group of the pty.
There is a new mode for packet driver, the TIOCPKT_IOCTL bit.
When packet mode is turned on in the pty, and the EXTPROC bit
is set, then whenever the state of the pty is changed, the
next read on the master side of the pty will have the TIOCPKT_IOCTL
bit set. This allows the process on the server side of the pty
to know when the state of the terminal has changed; it can then
issue the appropriate ioctl to retrieve the new state.
Since the original BSD patches accompanied the source code for telnet
I've left that reference here, but obviously the feature is useful for
any remote terminal protocol, including ssh.
The corresponding feature has existed in the BSD tty driver since 1989.

Fix up trivial conflicts in fs/nilfs2/super.c
Provide an INIT_MM_CONTEXT intializer macro which can be used to
statically initialize mm_struct:mm_context of init_mm. This way we can
get rid of code which will do the initialization at run time (on s390).
In addition the current code can be found at a place where it is not
expected. So let's have a common initializer which architectures
can use if needed.

Newer (guest) kernels use sigp sense running in their spinlock
implementation to check if the other cpu is running before yielding
the processor. This revealed some wrong guest settings, causing
unnecessary exits for every sigp sense running.

On 64bit, local_t is of size long, and thus we make local64_t an alias.
On 32bit, we fall back to atomic64_t. (architecture can provide optimized
32-bit version)

This is the first half of the attempt to use asm-generic/scatterlist.h
on every architecture.
There are only two ways to define scatterlist structure. So it's easy
to convert every architecture to use asm-generic/scatterlist.h.
This patch:
The trick for ISA_DMA_THRESHOLD in asm-generic/scatterlist.h doesn't work
for powerpc. This lets architectures defin ISA_DMA_THRESHOLD.
Hopefully, we can remove ISA_DMA_THRESHOLD in the future; we can do better
to decide if the bouncing is necessary or not.

This reverts commit b3b77c8cae, which was
also totally broken (see commit 0d2daf5cc8 that reverted the crc32
version of it). As reported by Stephen Rothwell, it causes problems on
big-endian machines:

Linux does not define __BYTE_ORDER in its endian header files which makes
some header files bend backwards to get at the current endian. Lets
#define __BYTE_ORDER in big_endian.h/litte_endian.h to make it easier for
header files that are used in user space too.
In userspace the convention is that
1. _both_ __LITTLE_ENDIAN and __BIG_ENDIAN are defined,
2. you have to test for e.g. __BYTE_ORDER == __BIG_ENDIAN.

WARN() is used in some places to report firmware or hardware bugs that
are then worked-around. These bugs do not affect the stability of the
kernel and should not set the flag for TAINT_WARN. To allow for this,
add WARN_TAINT() and WARN_TAINT_ONCE() macros that take a taint number
as argument.
Architectures that implement warnings using trap instructions instead
of calls to warn_slowpath_*() now implement __WARN_TAINT(taint)
instead of __WARN().

Copy the last breaking event address from the lowcore to a new
field in the thread_struct on each system entry. Add a new
ptrace request PTRACE_GET_LAST_BREAK and a new utrace regset
REGSET_LAST_BREAK to query the last breaking event.
This is useful for debugging wild branches in user space code.

A machine check can interrupt the i/o and external interrupt handler
anytime. If the machine check occurs while the interrupt handler is
waking up from idle vtime_start_cpu can get executed a second time
and the int_clock / async_enter_timer values in the lowcore get
clobbered. This can confuse the cpu time accounting.
To fix this problem two changes are needed. First the machine check
handler has to use its own copies of int_clock and async_enter_timer,
named mcck_clock and mcck_enter_timer. Second the nested execution
of vtime_start_cpu has to be prevented. This is done in s390_idle_check
by checking the wait bit in the program status word.

The system call path in entry[64].S is run with interrupts enabled.
Remove the irq tracing check from the system call exit code. If a
program check interrupted a context enabled for interrupts do a
call to trace_irq_off_caller in the program check handler before
branching to the system call exit code.
Restructure the system call and io interrupt return code to avoid
avoid the lpsw[e] to disable machine checks.

Commit "timekeeping: Fix clock_gettime vsyscall time warp" (0696b711e)
introduced the new parameter "mult" to update_vsyscall(). This parameter
contains the internal NTP adjusted clock multiplier.
The s390x vdso did not use this adjusted multiplier. Instead, it used
the constant clock multiplier for gettimeofday() and clock_gettime()
variants. This may result in observable time warps as explained in
commit 0696b711e.
Make the NTP adjusted clock multiplier available to the s390x vdso
implementation and use it for time calculations.

The default size of the vmalloc area is currently 1 GB. The memory resource
controller uses about 10 MB of vmalloc space per gigabyte of memory. That
turns a system with more than ~100 GB memory unbootable with the default
vmalloc size. It costs us nothing to increase the default size to some
more adequate value, e.g. 128 GB.

To save the registers for all CPUs a sigp "store status" is done that
stores the registers to address absolute zero. To access storage at
absolute zero, normally the address of the prefix register of the
accessing CPU has to be used. This does not work when large pages are
active (currently only under LPAR). In order to fix that problem,
instead of memcpy memcpy_real is used, which switches to real mode
where prefixing works.

While in theory user_enable_single_step/user_disable_single_step/
user_enable_blockstep could also be provided as an inline or macro there's
no good reason to do so, and having the prototype in one places keeps code
size and confusion down.

As to the arch changes, there is only one thought I'd add to the
record. It was always my thinking that for an arch where
PTRACE_SINGLESTEP does text-modifying breakpoint insertion,
user_enable_single_step() should not be provided. That is,
arch_has_single_step()=>true means that there is an arch facility with
"pure" semantics that does not have any unexpected side effects.
Inserting a breakpoint might do very unexpected strange things in
multi-threaded situations. Aside from that, it is a peculiar side
effect that user_{enable,disable}_single_step() should cause COW
de-sharing of text pages and so forth. For PTRACE_SINGLESTEP, all these
peculiarities are the status quo ante for that arch, so having
arch_ptrace() itself do those is one thing. But for building other
things in the future, it is nicer to have a uniform "pure" semantics
that arch-independent code can expect.
OTOH, all such arch issues are really up to the arch maintainer. As
of today, there is nothing but ptrace using user_enable_single_step() et
al so it's a distinction without a practical difference. If/when there
are other facilities that use user_enable_single_step() and might care,
the affected arch's can revisit the question when someone cares about
the quality of the arch support for said new facility.

On an architecture that supports 32-bit compat we need to override the
reported machine in uname with the 32-bit value. Instead of doing this
separately in every architecture introduce a COMPAT_UTS_MACHINE define in
<asm/compat.h> and apply it directly in sys_newuname().

The glibc vdso code for s390 uses the version string 2.6.29, the
kernel uses the version string 2.6.26. No wonder the vdso code
is never used. The first kernel version to contain the vdso code
is 2.6.29 which makes this the correct version.

Saves around 30kb on a defconfig kernel.

Use asm offsets to make sure the offset defines to struct _lowcore and
its layout don't get out of sync.
Also add a BUILD_BUG_ON() which checks that the size of the structure
is sane.
And while being at it change those sites which use odd casts to access
the current lowcore. These should use S390_lowcore instead.

This patch introduces a new function that checks the running status
of a cpu in a hypervisor. This status is not virtualized, so the check
is only correct if running in an LPAR. On acquiring a spinlock, if the
cpu holding the lock is scheduled by the hypervisor, we do a busy wait
on the lock. If it is not scheduled, we yield over to that cpu.

Remove a memset hack that relied on the internal layout of the
qdio_irq struct and move the per device statistics data into an own
cache line to avoid cache line bashing between the inbound and the
outbound queue tasklets. Also reduce the number of allocated queues
from 32 to 4 which is the current maximum. That saves a cache line
in struct qdio_irq.

Rename signal_processor* functions to sigp*.
Add raw variants of each version, so we can get rid of the hacks played
in smp code which establish temporary cpu logical mappings so they could
call the sigp functions.

Always reboot on logical cpu 0. This makes sure that the IPL cpu is
always the same and usually avoids strange numbering schemes between
physical and logical cpus.

Remove support to be able to dump 31 bit systems with a 64 bit dumper.
This is mostly useless since no distro ships 31 bit kernels together
with a 64 bit dumper.

Drop support to compile the kernel with gcc versions older than 3.3.3.
This allows us to use the "Q" inline assembly contraint on some more
inline assemblies without duplicating a lot of complex code (e.g. __xchg
and __cmpxchg). The distinction for older gcc versions can be removed
which saves a few lines and simplifies the code.

Some parts of cio do not shift PAGE_DEFAULT_KEY correctly and end up
with an incorrect key in their data structures.
Since the default key is zero this doesn't really matter. However if
somebody would use key-controlled protection for debugging purposes
it would be quite helpful if all of this would work as expected.
Also remove a stale declaration.

To fetch a pending channel report word (crw) we use a kernel
thread which triggers stcrw and sleeps on a semaphore. The s390
machine check handler uses crw_handle_channel_report to handle
one crw if needed.
This patch replaces the semaphore with a waitqueue (to block the
kernel thread) and an atomic_t (to count the number of pending
requests).
By this we achieve the ability to force this thread to check for
a pending crw (independent on when it is triggered by the machine
check handler) and wait for this action to finish.

On VIVT ARM, when we have multiple shared mappings of the same file
in the same MM, we need to ensure that we have coherency across all
copies. We do this via make_coherent() by making the pages
uncacheable.
This used to work fine, until we allowed highmem with highpte - we
now have a page table which is mapped as required, and is not available
for modification via update_mmu_cache().
Ralf Beache suggested getting rid of the PTE value passed to
update_mmu_cache():
On MIPS update_mmu_cache() calls __update_tlb() which walks pagetables
to construct a pointer to the pte again. Passing a pte_t * is much
more elegant. Maybe we might even replace the pte argument with the
pte_t?
Ben Herrenschmidt would also like the pte pointer for PowerPC:
Passing the ptep in there is exactly what I want. I want that
-instead- of the PTE value, because I have issue on some ppc cases,
for I$/D$ coherency, where set_pte_at() may decide to mask out the
_PAGE_EXEC.
So, pass in the mapped page table pointer into update_mmu_cache(), and
remove the PTE value, updating all implementations and call sites to
suit.

Most implementations of arch_syscall_addr() are the same, so create a
default version in common code and move the one piece that differs (the
syscall table) to asm/syscall.h. New arch ports don't have to waste
time copying & pasting this simple function.
The s390/sparc versions need to be different, so document why.

The TIF_USEDFPU bit is always 0 for s390 and it is not tested anywhere.
Remove the bit. At the same time remove the calls to clear_used_math()
as well. The PF_USED_MATH bit is never set for s390 either.

Fixed non-conflicting clean merge of modpost.c as per comments from
Stephen Rothwell (modpost.c had grown an include of linux/autoconf.h
that needed to be changed to generated/autoconf.h)
Currently all architectures but microblaze unconditionally define
USE_ELF_CORE_DUMP. The microblaze omission seems like an error to me, so
let's kill this ifdef and make sure we are the same everywhere.

The raw_spin* namespace was taken by lockdep for the architecture
specific implementations. raw_spin_* would be the ideal name space for
the spinlocks which are not converted to sleeping locks in preempt-rt.
Linus suggested to convert the raw_ to arch_ locks and cleanup the
name space instead of using an artifical name like core_spin,
atomic_spin or whatever

The simplest method was to add an extra asm-offsets.h
file in arch/$ARCH/include/asm that references the generated file.
We can now migrate the architectures one-by-one to reference
the generated file direct - and when done we can delete the
temporary arch/$ARCH/include/asm/asm-offsets.h file.

Trivial conflicts due to new Kconfig options in arch/Kconfig and kernel/Makefile
We dont need the dirty bit if a write access is done via the kernel
mapping. In that case SetPageDirty and friends are used anyway, no
need to do that a second time. We can use the change-recording
overide function for the kernel mapping, if available.

Git commit ea43546750 changed the
definition of atomic_t and atomic64_t for s390 by adding the volatile
modifier to the counter field. This has an unfortunate side effect
with newer versions of the gcc. The typeof operator now picks up the
volatile modifier from the expression. This causes the compiler to
think that it has to store the two temporary variable old_val and
new_val in the __CS_LOOP for the different atomic operations to the
stack as the variables are now volatile. Both stores are superfluous.
The hack to replace typeof(ptr->counter) with int in __CS_LOOP and
and long long in __CSG_LOOP avoids the two stores. A better solution
would be to drop the volatile from the counter field of the atomic_t
and atomic64_t definition. But that is a touchy subject ..

The pagetable walk usercopy functions have used a modified copy of the
do_exception() function for fault handling. This lead to inconsistencies
with recent changes to do_exception(), e.g. performance counters. This
patch changes the pagetable walk usercopy code to call do_exception()
directly, eliminating the redundancy. A new parameter is added to
do_exception() to specify the fault address.

Introduce user_mode to replace the two variables switch_amode and
s390_noexec. There are three valid combinations of the old values:
1) switch_amode == 0 && s390_noexec == 0
2) switch_amode == 1 && s390_noexec == 0
3) switch_amode == 1 && s390_noexec == 1
They get replaced by
1) user_mode == HOME_SPACE_MODE
2) user_mode == PRIMARY_SPACE_MODE
3) user_mode == SECONDARY_SPACE_MODE
The new kernel parameter user_mode=[primary,secondary,home] lets
you choose the address space mode the user space processes should
use. In addition the CONFIG_S390_SWITCH_AMODE config option
is removed.

Split setting (driver wants feature enabled) and status (feature
setup was successful) for PGID related ccw device features so that
setup errors can be detected. Previously, incorrectly handled setup
errors could in rare cases lead to erratic I/O behavior and
permanently unusuable devices.

This patch moves s390 processor status word into the base kvm_run
struct and keeps it up-to date on all userspace exits.
The userspace ABI is broken by this, however there are no applications
in the wild using this. A capability check is provided so users can
verify the updated API exists.

Mtdblock driver doesn't call flush_dcache_page for pages in request. So,
this causes problems on architectures where the icache doesn't fill from
the dcache or with dcache aliases. The patch fixes this.
The ARCH_IMPLEMENTS_FLUSH_DCACHE_PAGE symbol was introduced to avoid
pointless empty cache-thrashing loops on architectures for which
flush_dcache_page() is a no-op. Every architecture was provided with this
flush pages on architectires where ARCH_IMPLEMENTS_FLUSH_DCACHE_PAGE is
equal 1 or do nothing otherwise.

commit 892a7c67 (locking: Allow arch-inlined spinlocks) implements the
selection of which lock functions are inlined based on defines in
arch/.../spinlock.h: #define __always_inline__LOCK_FUNCTION
Despite of the name __always_inline__* the lock functions can be built
out of line depending on config options. Also if the arch does not set
some inline defines the generic code might set them; again depending on
config options.
This makes it unnecessary hard to figure out when and which lock
functions are inlined. Aside of that it makes it way harder and
messier for -rt to manipulate the lock functions.
Convert the inlining decision to CONFIG switches. Each lock function
is inlined depending on CONFIG_INLINE_*. The configs implement the
existing dependencies. The architecture code can select ARCH_INLINE_*
to signal that it wants the corresponding lock function inlined.
ARCH_INLINE_* is necessary as Kconfig ignores "depends on"
restrictions when a config element is selected.
No functional change.

Allow the architecture to request a normal jiffy tick when the system
goes idle and tick_nohz_stop_sched_tick is called . On s390 the hook is
used to prevent the system going fully idle if there has been an
interrupt other than a clock comparator interrupt since the last wakeup.
On s390 the HiperSockets response time for 1 connection ping-pong goes
down from 42 to 34 microseconds. The CPU cost decreases by 27%.

Create a new socket level option to report number of queue overflows
Recently I augmented the AF_PACKET protocol to report the number of frames lost
on the socket receive queue between any two enqueued frames. This value was
exported via a SOL_PACKET level cmsg. AFter I completed that work it was
requested that this feature be generalized so that any datagram oriented socket
could make use of this option. As such I've created this patch, It creates a
new SOL_SOCKET level option called SO_RXQ_OVFL, which when enabled exports a
SOL_SOCKET level cmsg that reports the nubmer of times the sk_receive_queue
overflowed between any two given frames. It also augments the AF_PACKET
protocol to take advantage of this new feature (as it previously did not touch
sk->sk_drops, which this patch uses to record the overflow count). Tested
successfully by me.
Notes:
1) Unlike my previous patch, this patch simply records the sk_drops value, which
is not a number of drops between packets, but rather a total number of drops.
Deltas must be computed in user space.
2) While this patch currently works with datagram oriented protocols, it will
also be accepted by non-datagram oriented protocols. I'm not sure if thats
agreeable to everyone, but my argument in favor of doing so is that, for those
protocols which aren't applicable to this option, sk_drops will always be zero,
and reporting no drops on a receive queue that isn't used for those
non-participating protocols seems reasonable to me. This also saves us having
to code in a per-protocol opt in mechanism.
3) This applies cleanly to net-next assuming that commit
977750076d (my af packet cmsg patch) is reverted

Currently, when the physical resume CPU is not equal to the physical suspend
CPU, we swap the CPUs logically, by modifying the logical/physical CPU mapping.
This has two major drawbacks: First the change is visible from user space (e.g.
CPU sysfs files) and second it is hard to ensure that nowhere in the kernel
the physical CPU ID is stored before suspend.
To fix this, we now really swap the physical CPUs, if the resume CPU is not
the pysical suspend CPU. We restart the suspend CPU and stop the resume CPU
using SIGP restart and SIGP stop. If the suspend CPU is no longer available,
we write a message and load a disabled wait PSW.

pfn_is_nosave doesn't return the correct value for the second lowcore
page if lowcore protection is enabled. Make sure it always returns
the correct value.
While at it simplify the whole thing.
NSS special handling is done by the tprot check like it already works
for DCSS as well. So remove the extra code for NSS.

Add a flag for mmap that will be used to request a huge page region that
will look like anonymous memory to user space. This is accomplished by
using a file on the internal vfsmount. MAP_HUGETLB is a modifier of
MAP_ANONYMOUS and so must be specified with it. The region will behave
the same as a MAP_ANONYMOUS region using small pages.
The patch also adds the MAP_STACK flag, which was previously defined only
on some architectures but not on others. Since MAP_STACK is meant to be a
hint only, architectures can define it without assigning a specific
meaning to it.

Bye-bye Performance Counters, welcome Performance Events!
In the past few months the perfcounters subsystem has grown out its
initial role of counting hardware events, and has become (and is
becoming) a much broader generic event enumeration, reporting, logging,
monitoring, analysis facility.
Naming its core object 'perf_counter' and naming the subsystem
'perfcounters' has become more and more of a misnomer. With pending
code like hw-breakpoints support the 'counter' name is less and
less appropriate.
All in one, we've decided to rename the subsystem to 'performance
events' and to propagate this rename through all fields, variables
and API names. (in an ABI compatible fashion)
The word 'event' is also a bit shorter than 'counter' - which makes
it slightly more convenient to write/handle as well.
Thanks goes to Stephane Eranian who first observed this misnomer and
suggested a rename.
User-space tooling and ABI compatibility is not affected - this patch
should be function-invariant. (Also, defconfigs were not touched to
keep the size down.)

a Linux kernel tree over to the new naming. We tried to time this
change to the point in time where the amount of pending patches
is the smallest: the end of the merge window.
Namespace clashes were fixed up in a preparatory patch - and some
stylistic fallout will be fixed up in a subsequent patch.
( NOTE: 'counters' are still the proper terminology when we deal
with hardware registers - and these sed scripts are a bit
over-eager in renaming them. I've undone some of that, but
in case there's something left where 'counter' would be
better than 'event' we can undo that on an individual basis
instead of touching an otherwise nicely automated patch. )

Get rid of the PAGE_STATES config option and enable guest page hinting
by default.
It can be disabled by specifying "cmma=off" at the command line.

The page_table_lock is already acquired in __pmd_alloc (mm/memory.c) and
it tries to populate the pud/pgd with a new pmd allocated. If another
thread populates it before we get a chance, we free the pmd using
pmd_free().
On s390x, pmd_free(even pud_free ) is #defined to crst_table_free(),
which acquires the page_table_lock to protect the crst_table index updates.
Hence this ends up in a recursive locking of the page_table_lock.
The solution suggested by Dave Hansen is to use a new spin lock in the mmu
context to protect the access to the crst_list and the pgtable_list.

Append scpdata to the kernel boot command line. If scpdata starts
with the equal sign (=), the kernel boot command line is replaced.
(For consistency with zIPL and IPL PARM parameters.)
To use scpdata for the kernel boot command line, scpdata must consist
of ascii characters only. If scpdata contains other characters,
scpdata is not appended to the kernel boot command line.
In addition, re-IPL is extended for setting scpdata for the next
Linux reboot.

Introduce get_clock_monotonic() function which can be used to get a
(fast) timestamp. Resolution is the same as for get_clock(). The
only difference is that the timestamps are monotonic and don't jump
backward or forward.

All scsw helper functions are very short and usage of them shouldn't
result in function calls. Therefore we move them to a separate header
file.
Also saves a lot of EXPORT_SYMBOLs.

This patch relocates the variables kvm-s390 uses to track guest mem addr/size.
As discussed dropping the variables at struct kvm_arch level allows to use the
common vcpu->request based mechanism to reload guest memory if e.g. changes
via set_memory_region.
The kick mechanism introduced in this series is used to ensure running vcpus
leave guest state to catch the update.
