rxe does not have correct locking for its registration/unregistration
paths, use the core code to handle it instead. In this mode
ib_unregister_device will also do the dealloc, so rxe is required to do
clean up from a callback.
The core code ensures that unregistration is done only once, and generally
takes care of locking and concurrency problems for rxe.

Adapt to a umem API change which could return non-uniform sized SGEs due

The s_ack_queue is managed by two pointers into the ring:
r_head_ack_queue and s_tail_ack_queue. r_head_ack_queue is the index of
where the next received request is going to be placed and s_tail_ack_queue
is the entry of the request currently being processed. This works
perfectly fine for normal Verbs as the requests are processed one at a
time and the s_tail_ack_queue is not moved until the request that it
points to is fully completed.
In this fashion, s_tail_ack_queue constantly chases r_head_ack_queue and
the two pointers can easily be used to determine "queue full" and "queue
empty" conditions.
The detection of these two conditions are imported in determining when an
old entry can safely be overwritten with a new received request and the
resources associated with the old request be safely released.
When pipelined TID RDMA WRITE is introduced into this mix, things look
very different. r_head_ack_queue is still the point at which a newly
received request will be inserted, s_tail_ack_queue is still the
currently processed request. However, with pipelined TID RDMA WRITE
requests, s_tail_ack_queue moves to the next request once all TID RDMA
WRITE responses for that request have been sent. The rest of the protocol
for a particular request is managed by other pointers specific to TID RDMA
- r_tid_tail and r_tid_ack - which point to the entries for which the next
TID RDMA DATA packets are going to arrive and the request for which
the next TID RDMA ACK packets are to be generated, respectively.
What this means is that entries in the ring, which are "behind"
s_tail_ack_queue (entries which s_tail_ack_queue has gone past) are no
longer considered complete. This is where the problem is - a newly
received request could potentially overwrite a still active TID RDMA WRITE
request.
The reason why the TID RDMA pointers trail s_tail_ack_queue is that the
normal Verbs send engine uses s_tail_ack_queue as the pointer for the next
response. Since TID RDMA WRITE responses are processed by the normal Verbs
send engine, s_tail_ack_queue had to be moved to the next entry once all
TID RDMA WRITE response packets were sent to get the desired pipelining
between requests. Doing otherwise would mean that the normal Verbs send
engine would not be able to send the TID RDMA WRITE responses for the next
TID RDMA request until the current one is fully completed.
This patch introduces the s_acked_ack_queue index to point to the next
request to complete on the responder side. For requests other than TID
RDMA WRITE, s_acked_ack_queue should always be kept in sync with
s_tail_ack_queue. For TID RDMA WRITE request, it may fall behind
s_tail_ack_queue.

The RC retry timeout value is based on the estimated time for the
response packet to come back. However, for TID RDMA READ request, due
to the use of header suppression, the driver is normally not notified
for each incoming response packet until the last TID RDMA READ response
packet. Consequently, the retry timeout value should be extended to
cover the transaction time for the entire length of a segment (default
256K) instead of that for a single packet. This patch addresses the
issue by introducing new retry timer functions to account for multiple
packets and wrapper functions for backward compatibility.

TID entries are used by hfi1 hardware to receive data payload from
incoming packets directly into a user buffer and thus avoid data copying
by software. This patch implements the functions for TID allocation,
freeing, and programming TID RcvArray entries in hardware for kernel
clients. TID entries are managed via lists of TID groups similar to PSM.
Furthermore, to track TID resource allocation for each request, software
flows are also allocated and freed as needed. Since software flows
consume large amount of memory for tracking TID allocation and freeing,
it is generally desirable to allocate them dynamically in the send queue
and only for TID RDMA requests, but pre-allocate them for receive queue
because the send queue could have thousands of entries while the receive
queue has only a limited number of entries.

FRWR memory registration is done with a series of calls and WRs.
1. ULP invokes ib_dma_map_sg()
2. ULP invokes ib_map_mr_sg()
3. ULP posts an IB_WR_REG_MR on the Send queue
Step 2 generates an iova. It is permissible for ULPs to change this
iova (with certain restrictions) between steps 2 and 3.
rxe_map_mr_sg captures the MR's iova but later when rxe processes the
REG_MR WR, it ignores the MR's iova field. If a ULP alters the MR's iova
after step 2 but before step 3, rxe never captures that change.
When the remote sends an RDMA Read targeting that MR, rxe looks up the
R_key, but the altered iova does not match the iova stored in the MR,
causing the RDMA Read request to fail.

The current code has two copies of the device name, ibdev->dev and
dev_name(&ibdev->dev), and they are setup at different times, which is
very confusing.
Set them both up at the same time and make dev_name() the lead name, which
is the proper use of the driver core APIs. To make it very clear that the
name is not valid until registration pass it in to the
ib_register_device() call rather than messing with ibdev->name directly.
Also the reorganization now checks that dev_name is unique even if it does

The post_send() path determines if it should post directly or, schedule
the post for later. The current logic is:
if the swqe ring is empty or (for hfi1) wqe->length <= piothreshold
post the send
else
schedule
This can allow large requests to call the send engine directly. Large
requests can potentially produce a large number of packets prior to
returning to the caller, blocking the caller from posting more requests,
and allowing better parallel processing.
Allow the driver(s) more say in this logic (pass call_send to the driver,
rather than examining a return value).
Update hfi1/qib logic to schedule the send engine if an RC or UC message

When a read request is retried for the remaining partial
data, the response may restart from read response first
or read response only. So support those cases.
Do not advance the comp psn beyond the current wqe's last_psn
as that could skip over an entire read wqe and will cause the
req_retry() logic to set an incorrect req psn.
An example sequence is as follows:
Write PSN 40 -- this is the current WQE.
Read request PSN 41
Write PSN 42
Receive ACK PSN 42 -- this will complete the current WQE
for PSN 40, and set the comp psn to 42 which is a problem
because the read request at PSN 41 has been skipped over.
So when req_retry() tries to retransmit the read request,
it sets the req psn to 42 which is incorrect.
When retrying a read request, calculate the number of psns
completed based on the dma resid instead of the wqe first_psn.
The wqe first_psn could have moved if the read request was
retried multiple times.
Set the reth length to the dma resid to handle read retries for
the remaining partial data.

Don't reset the resp opcode for a replayed read response.
The resp opcode could be in the middle of a write or send
sequence, when the duplicate read request was received.
An example sequence is as follows:
- Receive read request for 12KB PSN 20. Transmit read response
first, middle and last with PSNs 20,21,22.
- Receive write first PSN 23.
At this point the resp psn is 24 and resp opcode is write first.
- The sender notices that PSN 20 is dropped and retransmits.
Receive read request for 12KB PSN 20. Transmit read response
first, middle and last with PSNs 20,21,22. The resp opcode is
set to -1, the resp psn remains 24.
- Receive write first PSN 23. This is processed by duplicate_request().
The resp opcode remains -1 and resp psn remains 24.
- Receive write middle PSN 24. check_op_seq() reports a missing
first error since the resp opcode is -1.
When sending an ack for a duplicate send or write request,
use the psn of the previous ack sent. Do not use the psn
of a read response for the ack.
An example sequence is as follows:
- Receive write PSN 30. Transmit ACK for PSN 30.
- Receive read request 4KB PSN 31. Transmit read response with
PSN 31. The resp psn is now 32.
- The sender notices that PSN 30 is dropped and retransmits.
Receive write PSN 30. duplicate_request() sends an ACK with
PSN 31. That is incorrect since PSN 31 was a read request.

Since neither ib_post_send() nor ib_post_recv() modify the data structure
their second argument points at, declare that argument const. This change
makes it necessary to declare the 'bad_wr' argument const too and also to
modify all ULPs that call ib_post_send(), ib_post_recv() or
ib_post_srq_recv(). This patch does not change any functionality but makes
it possible for the compiler to verify whether the
ib_post_(send|recv|srq_recv) really do not modify the posted work request.
To make this possible, only one cast had to be introduce that casts away
constness, namely in rpcrdma_post_recvs(). The only way I can think of to
avoid that cast is to introduce an additional loop in that function or to
change the data type of bad_wr from struct ib_recv_wr ** into int
(an index that refers to an element in the work request list). However,
both approaches would require even more extensive changes than this

As defined in the base specification, a special Queue Pair, QP0 is defined

This patch replaces the ib_device_attr.max_sge with max_send_sge and
max_recv_sge. It allows ulps to take advantage of devices that have very
different send and recv sge depths. For example cxgb4 has a max_recv_sge
of 4, yet a max_send_sge of 16. Splitting out these attributes allows
much more efficient use of the SQ for cxgb4 with ulps that use the RDMA_RW
API. Consider a large RDMA WRITE that has 16 scattergather entries.
With max_sge of 4, the ulp would send 4 WRITE WRs, but with max_sge of
16, it can be done with 1 WRITE WR.

Pull overflow updates from Kees Cook:
"This adds the new overflow checking helpers and adds them to the
2-factor argument allocators. And this adds the saturating size
helpers and does a treewide replacement for the struct_size() usage.
Additionally this adds the overflow testing modules to make sure
everything works.
I'm still working on the treewide replacements for allocators with
"simple" multiplied arguments:
*alloc(a * b, ...) -> *alloc_array(a, b, ...)
and
*zalloc(a * b, ...) -> *calloc(a, b, ...)
as well as the more complex cases, but that's separable from this
portion of the series. I expect to have the rest sent before -rc1
closes; there are a lot of messy cases to clean up.

One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:
struct foo {
int stuff;
void *entry[];
};
instance = kmalloc(sizeof(struct foo) + sizeof(void *) * count, GFP_KERNEL);
Instead of leaving these open-coded and prone to type mistakes, we can
now use the new struct_size() helper:
instance = kmalloc(struct_size(instance, entry, count), GFP_KERNEL);
This patch makes the changes for kmalloc()-family (and kvmalloc()-family)
uses. It was done via automatic conversion with manual review for the
"CHECKME" non-standard cases noted below, using the following Coccinelle
script:
// pkey_cache = kmalloc(sizeof *pkey_cache + tprops->pkey_tbl_len *
// sizeof *pkey_cache->table, GFP_KERNEL);

Currently the driver doesn't support completion vectors. These
are used to indicate which sets of CQs should be grouped together
into the same vector. A vector is a CQ processing thread that
runs on a specific CPU.
If an application has several CQs bound to different completion
vectors, and each completion vector runs on different CPUs, then
the completion queue workload is balanced. This helps scale as more
nodes are used.
Implement CQ completion vector support using a global workqueue
where a CQ entry is queued to the CPU corresponding to the CQ's
completion vector. Since the workqueue is global, it's guaranteed
to always be there when queueing CQ entries; Therefore, the RCU
locking for cq->rdi->worker in the hot path is superfluous.
Each completion vector is assigned to a different CPU. The number of
completion vectors available is computed by taking the number of
online, physical CPUs from the local NUMA node and subtracting the
CPUs used for kernel receive queues and the general interrupt.
Special use cases:
* If there are no CPUs left for completion vectors, the same CPU
for the general interrupt is used; Therefore, there would only
be one completion vector available.
* For multi-HFI systems, the number of completion vectors available
for each device is the total number of completion vectors in
the local NUMA node divided by the number of devices in the same
NUMA node. If there's a division remainder, the first device to
get initialized gets an extra completion vector.
Upon a CQ creation, an invalid completion vector could be specified.
Handle it as follows:
* If the completion vector is less than 0, set it to 0.
* Set the completion vector to the result of the passed completion
vector moded with the number of device completion vectors
available.

All threads queuing CQ entries on different CQs are unnecessarily
synchronized by a spin lock to check if the CQ kthread worker hasn't
been destroyed before queuing an CQ entry.
The lock used in 6efaf10f16 ("IB/rdmavt: Avoid queuing work into a
destroyed cq kthread worker") is a device global lock and will have
poor performance at scale as completions are entered from a large
number of CPUs.
Convert to use RCU where the read side of RCU is rvt_cq_enter() to
determine that the worker is alive prior to triggering the
completion event.
Apply write side RCU semantics in rvt_driver_cq_init() and
rvt_cq_exit().

To handle this we bump the ABI version, as expected. However the user

Updated userspace will start to require the necessary kernel version.

rvt_mregion uses percpu_ref for reference counting and RCU to protect
accesses from lkey_table. When a rvt_mregion needs to be freed, it
first gets unregistered from lkey_table and then rvt_check_refs() is
called to wait for in-flight usages before the rvt_mregion is freed.
rvt_check_refs() seems to have a couple issues.
* It has a fast exit path which tests percpu_ref_is_zero(). However,
a percpu_ref reading zero doesn't mean that the object can be
released. In fact, the ->release() callback might not even have
started executing yet. Proceeding with freeing can lead to
use-after-free.
* lkey_table is RCU protected but there is no RCU grace period in the
free path. percpu_ref uses RCU internally but it's sched-RCU whose
grace periods are different from regular RCU. Also, it generally
isn't a good idea to depend on internal behaviors like this.
To address the above issues, this patch removes the fast exit and adds
an explicit synchronize_rcu().

This patch fixes RDMA/rxe over 802.1q VLAN devices.
Without it, I observed the following behavior:
a) adding a VLAN device to RXE via rxe_net_add() creates a non-functional
RDMA device. This is caused by the logic in enum_all_gids_of_dev_cb() /
is_eth_port_of_netdev(), which only considers networks connected to
"upper devices" of the configured network device, resulting in an empty
set of gids for a VLAN interface that is an "upper device" itself.
Later attempts to connect via this rdma device fail in cma_acuire_dev()
because no gids can be resolved.
b) adding the master device of the VLAN device instead seems to work
initially, target addresses via VLAN devices are resolved successfully.
But the connection times out because no 802.1q VLAN headers are
inserted in the ethernet packets, which are therefore never received.
This happens because the RXE layer sends the packets via the master
device rather than the VLAN device.
The problem could be solved by changing either a) or b). My thinking was
that the logic in a) was created deliberately, thus I decided to work on
b). It turns out that the information about the VLAN interface for the gid
at hand is available in the AV information. My patch converts the RXE code
to use this netdev instead of rxe->ndev. With this change, RXE over vlan
works on my test system.

The rxe driver works as follows:
* The send queue, receive queue and completion queues are implemented as
circular buffers.
* ib_post_send() and ib_post_recv() calls are serialized through a spinlock.
* Removing elements from various queues happens from tasklet
context. Tasklets are guaranteed to run on at most one CPU. This serializes
access to these queues. See also rxe_completer(), rxe_requester() and
rxe_responder().
* rxe_completer() processes the skbs queued onto qp->resp_pkts.
* rxe_requester() handles the send queue (qp->sq.queue).
* rxe_responder() processes the skbs queued onto qp->req_pkts.
Since rxe_drain_req_pkts() processes qp->req_pkts, calling
rxe_drain_req_pkts() from rxe_requester() is racy. Hence this patch.

The following sequence:
* Change queue pair state into IB_QPS_ERR.
* Post a work request on the queue pair.
Triggers the following race condition in the rdma_rxe driver:
* rxe_qp_error() triggers an asynchronous call of rxe_completer(), the function
that examines the QP send queue.
* rxe_post_send() posts a work request on the QP send queue.
If rxe_completer() runs prior to rxe_post_send(), it will drain the send
queue and the driver will assume no further action is necessary.
However, once we post the send to the send queue, because the queue is
in error, no send completion will ever happen and the send will get
stuck. In order to process the send, we need to make sure that
rxe_completer() gets run after a send is posted to a queue pair in an
error state. This patch ensures that happens.

The smp_read_barrier_depends() does nothing at all except on DEC Alpha,
and no current DEC Alpha systems use Infiniband:
lkml.kernel.org/r/20171023085921.jwbntptn6ictbnvj@tower
This commit therefore makes Infiniband depend on !ALPHA and removes
the now-ineffective invocations of smp_read_barrier_depends() from

Please note that this patch should not be construed as my saying that
InfiniBand's memory ordering is correct, but rather that this patch does

"This is a fairly plain pull request. Lots of driver updates across the
stack, a huge number of static analysis cleanups including a close to
50 patch series from Bart Van Assche, and a number of new features
inside the stack such as general CQ moderation support.
Nothing really stands out, but there might be a few conflicts as you
take things in. In particular, the cleanups touched some of the same
lines as the new timer_setup changes.
Everything in this pull request has been through 0day and at least two
days of linux-next (since Stephen doesn't necessarily flag new
errors/warnings until day2). A few more items (about 30 patches) from
Intel and Mellanox showed up on the list on Tuesday. I've excluded
those from this pull request, and I'm sure some of them qualify as
fixes suitable to send any time, but I still have to review them
fully. If they contain mostly fixes and little or no new development,
then I will probably send them through by the end of the week just to
get them out of the way.
There was a break in my acceptance of patches which coincides with the
computer problems I had, and then when I got things mostly back under
control I had a backlog of patches to process, which I did mostly last
Friday and Monday. So there is a larger number of patches processed in
that timeframe than I was striving for.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 11139
and resulted in the first patch in this series.
If that file was a */uapi/* path one, it was "GPL-2.0 WITH
Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 WITH Linux-syscall-note 930
and resulted in the second patch in this series.
- if a file had some form of licensing information in it, and was one
of the */uapi/* ones, it was denoted with the Linux-syscall-note if
any GPL family license was found in the file or had no licensing in
it (per prior point). Results summary:
SPDX license identifier # files
---------------------------------------------------|------
GPL-2.0 WITH Linux-syscall-note 270
GPL-2.0+ WITH Linux-syscall-note 169
((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21
((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17
LGPL-2.1+ WITH Linux-syscall-note 15
GPL-1.0+ WITH Linux-syscall-note 14
((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5
LGPL-2.0+ WITH Linux-syscall-note 4
LGPL-2.1 WITH Linux-syscall-note 3
((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3
((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1
and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Please do not apply this to mainline directly, instead please re-run the
coccinelle script shown below and apply its output.
For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining
ACCESS_ONCE() instances to {READ,WRITE}_ONCE(), using the following

Per the IBTA spec, QP destroy shall fail if the QP is attached
to multicast groups, although the spec is silent on modify_qp
to reset state. It implies that ULP must deregister QP from
all mcast groups for destroy to succeed.
The faulty patch "IB/ipoib: Update broadcast object if PKey value
was changed in index 0" exposed two issues in rdmavt:
1. Rvt QP reset waits for qp references to go to zero.
This will hang if QP is attached to multicast groups.
2. The mcast group detach will fail for a QP in reset state
therefore preventing ULP from correcting the issue.
This patch moves the reference count wait to the the destroy QP
path and allows a QP mcast detach to work in the reset state.

calculated but before the data is copied.
By copying the data into the packet and then calculating the CRC directly
from the packet contents we eliminate the window.
This can be seen with qperf's ud_bi_bw test. This seems like very
strange/reckless client behavior, but whether the client has mangled its

To successfully match an IPv6 path, the path cookie must match. Store it
in the QP so that the IPv6 path can be reused.
Replace open-coded version of dst_check() with the actual call, fixing the
logic. The open-coded version skips the check call if dst->obsolete is 0
(DST_OBSOLETE_NONE), proceeding to replace the route. DST_OBSOLETE_NONE
means that the route may continue to be used, though.

The resource array is sized by max_dest_rd_atomic, not max_rd_atomic.
Iterating over max_rd_atomic entries of qp->resp.resources[] will cause
incorrect behavior when the two attributes are different (or even
crash if max_rd_atomic is larger).

This prevents the stack from accessing userspace objects while they
are being torn down.
One possible sequence of events:
- Userspace program exits
- ib_uverbs_cleanup_ucontext() runs, calling ib_destroy_qp(),
ib_destroy_cq(), etc. and releasing/freeing the UCQ
- The QP still has tasklets running, so it isn't destroyed yet
- The CQ is referenced by the QP, so the CQ isn't destroyed yet
- The UCQ is kfree()'d anyway
- A send work request completes
- rxe_send_complete() calls cq->ibcq.comp_handler()
- ib_uverbs_comp_handler() runs and crashes; the event queue is checked
for is_closed, but it has no way to check the ib_ucq_object before
accessing it
The reference counting on the CQ doesn't protect against this since the CQ
hasn't been destroyed yet.
There's no available interface to deregister the UCQ from the CQ, and it
didn't appear that attempting to add reference counting to the UCQ was
going to be a good way to go since this solution is much simpler.

The network stack will call nskb's destructor, rxe_skb_tx_dtor(), if the
packet gets dropped by ip_local_out()/ip6_local_out(). Thus we need to add
the QP ref before output to avoid extra dereferences during network
congestion. This could lead to unwanted destruction of the QP.
Fix up the skb_out accounting, too.

A destroy of an MR prior to destroying the QP can cause the following
diagnostic if the QP is referencing the MR being de-registered:
hfi1 0000:05:00.0: hfi1_0: rvt_dereg_mr timeout mr ffff8808562108
00 pd ffff880859b20b00
The solution is to when the a non-zero refcount is encountered when
the MR is destroyed the QPs needs to be iterated looking for QPs in
the same PD as the MR. If rvt_qp_mr_clean() detects any such QP
references the rkey/lkey, the QP needs to be put into an error state
via a call to rvt_qp_error() which will trigger the clean up of any
stuck references.
This solution is as specified in IBTA 1.3 Volume 1 11.2.10.5.

There are currently 3 spots in the qib and hfi1 driver that have
knowledge of the internal QP hash list that should only be in
scope to rdmavt QP code.
Add an iterator API for processing all QPs to hide the
nature of the RCU hashlist.
The API consists of:
- rvt_qp_iter_init()
* For iterating QPs one at a time for seq_file semantics
- rvt_qp_iter_next()
* For iterating QPs one at a time for seq_file semantics
- rvt_qp_iter()
* For iterating all QPs
The first two are used for things like seq_file prints.
The last is for code that just needs to iterate all QPs
in the system.
