Isaku Yamahata 03f511dd02 ia64/pv_ops: implement binary patching optimization for native.
implement binary patching optimization for pv_cpu_ops.
With this optimization, indirect call for pv_cpu_ops methods can be
converted into inline execution or direct call.

Obvious inclusion of kernel.h doesn't fix it, because of circular dependencies
involving fls.h and log2(). Fixing the latter requires some serious header surgery,
it seems, so just remove BUILD_BUG_ON for now.

Isaku Yamahata 93fe10b670 [IA64] ia64/pv_ops/pv_cpu_ops: fix _IA64_REG_IP case.
pv_cpu_ops.getreg(_IA64_REG_IP) returned constant.
But the returned ip valued should be the one in the caller, not of the callee.
This patch fixes that.

Tony Luck 7f30491ccd [IA64] Move include/asm-ia64 to arch/ia64/include/asm
After moving the the include files there were a few clean-ups:

1) Some files used #include <asm-ia64/xyz.h>, changed to <asm/xyz.h>

2) Some comments alerted maintainers to look at various header files to
make matching updates if certain code were to be changed. Updated these
comments to use the new include paths.

3) Some header files mentioned their own names in initial comments. Just
deleted these self references.
