Merge tag 'for-linus-5.9-rc1b-tag' of git://git.kernel.org/pub/scm/linux/kernel/git/xen/tip

Pull more xen updates from Juergen Gross:

 - Remove support for running as 32-bit Xen PV-guest.

32-bit PV guests are rarely used, are lacking security fixes for
Meltdown, and can be easily replaced by PVH mode. Another series for
doing more cleanup will follow soon (removal of 32-bit-only pvops
   functionality).

 - Fixes and additional features for the Xen display frontend driver.

Juergen Gross f2e39e8c4f x86/xen: drop tests for highmem in pv code
With support for 32-bit pv guests gone pure pv-code no longer needs to
test for highmem. Dropping those tests removes the need for flushing
in some places.

Juergen Gross a13f2ef168 x86/xen: remove 32-bit Xen PV guest support
Xen is requiring 64-bit machines today and since Xen 4.14 it can be
built without 32-bit PV guest support. There is no need to carry the
burden of 32-bit PV guest support in the kernel any longer, as new
guests can be either HVM or PVH, or they can use a 64 bit kernel.

Remove the 32-bit Xen PV support from the kernel.

Linus Torvalds 97d052ea3f A set of locking fixes and updates:
- Untangle the header spaghetti which causes build failures in various
situations caused by the lockdep additions to seqcount to validate that
the write side critical sections are non-preemptible.

   - The seqcount associated lock debug addons which were blocked by the
     above fallout.

seqcount writers contrary to seqlock writers must be externally
serialized, which usually happens via locking - except for strict per
CPU seqcounts. As the lock is not part of the seqcount, lockdep cannot
validate that the lock is held.

This new debug mechanism adds the concept of associated locks.
sequence count has now lock type variants and corresponding
initializers which take a pointer to the associated lock used for
writer serialization. If lockdep is enabled the pointer is stored and
write_seqcount_begin() has a lockdep assertion to validate that the
     lock is held.

Aside of the type and the initializer no other code changes are
required at the seqcount usage sites. The rest of the seqcount API is
unchanged and determines the type at compile time with the help of
_Generic which is possible now that the minimal GCC version has been
     moved up.

Adding this lockdep coverage unearthed a handful of seqcount bugs which
have been addressed already independent of this.

While generaly useful this comes with a Trojan Horse twist: On RT
kernels the write side critical section can become preemtible if the
writers are serialized by an associated lock, which leads to the well
known reader preempts writer livelock. RT prevents this by storing the
associated lock pointer independent of lockdep in the seqcount and
changing the reader side to block on the lock when a reader detects
that a writer is in the write side critical section.

  - Conversion of seqcount usage sites to associated types and initializers.

Peter Zijlstra 0cd39f4600 locking/seqlock, headers: Untangle the spaghetti monster
By using lockdep_assert_*() from seqlock.h, the spaghetti monster
attacked.

Attack back by reducing seqlock.h dependencies from two key high level headers:

 - <linux/seqlock.h>: -Remove <linux/ww_mutex.h>
 - <linux/time.h>: -Remove <linux/seqlock.h>
 - <linux/sched.h>: +Add <linux/seqlock.h>

The price was to add it to sched.h ...

Core header fallout, we add direct header dependencies instead of gaining them
parasitically from higher level headers:

 - <linux/dynamic_queue_limits.h>: +Add <asm/bug.h>
 - <linux/hrtimer.h>: +Add <linux/seqlock.h>
 - <linux/ktime.h>: +Add <asm/bug.h>
 - <linux/lockdep.h>: +Add <linux/smp.h>
 - <linux/sched.h>: +Add <linux/seqlock.h>
 - <linux/videodev2.h>: +Add <linux/kernel.h>

Arch headers fallout:

 - PARISC: <asm/timex.h>: +Add <asm/special_insns.h>
 - SH: <asm/io.h>: +Add <asm/page.h>
 - SPARC: <asm/timer_64.h>: +Add <uapi/asm/asi.h>
 - SPARC: <asm/vvar.h>: +Add <asm/processor.h>, <asm/barrier.h>
                                    -Remove <linux/seqlock.h>
 - X86: <asm/fixmap.h>: +Add <asm/pgtable_types.h>
                                    -Remove <asm/acpi.h>

There's also a bunch of parasitic header dependency fallout in .c files, not listed
separately.

Ingo Molnar 13c01139b1 x86/headers: Remove APIC headers from <asm/smp.h>
The APIC headers are relatively complex and bring in additional
header dependencies - while smp.h is a relatively simple header
included from high level headers.

Remove the dependency and add in the missing #include's in .c
files where they gained it indirectly before.

On Xen PV, #DB doesn't use IST. It still needs to be correctly routed

Vitaly Kuznetsov 998c2034c6 xen: Move xen_setup_callback_vector() definition to include/xen/hvm.h
Kbuild test robot reports the following problem on ARM:

  for 'xen_setup_callback_vector' [-Wmissing-prototypes]
1664 | void xen_setup_callback_vector(void) {}
| ^~~~~~~~~~~~~~~~~~~~~~~~~

The problem is that xen_setup_callback_vector is a x86 only thing, its
definition is present in arch/x86/xen/xen-ops.h but not on ARM. In
events_base.c there is a stub for !CONFIG_XEN_PVHVM but it is not declared
as 'static'.

On x86 the situation is hardly better: drivers/xen/events/events_base.c
doesn't include 'xen-ops.h' from arch/x86/xen/, it includes its namesake
from include/xen/ which also results in a 'no previous prototype' warning.

Currently, xen_setup_callback_vector() has two call sites: one in
drivers/xen/events_base.c and another in arch/x86/xen/suspend_hvm.c. The
former is placed under #ifdef CONFIG_X86 and the later is only compiled
in when CONFIG_XEN_PVHVM.

Resolve the issue by moving xen_setup_callback_vector() declaration to
arch neutral 'include/xen/hvm.h' as the implementation lives in arch
neutral drivers/xen/events/events_base.c.

  - Emit the ASM stub with DECLARE_IDTENTRY

The handler stubs need to stay in ASM code as they need corner case handling
and adjustment of the stack pointer.

Provide a new C function which invokes the entry/exit handling and calls
into the XEN handler on the interrupt stack if required.

The exit code is slightly different from the regular idtentry_exit() on
non-preemptible kernels. If the hypercall is preemptible and need_resched()
is set then XEN provides a preempt hypercall scheduling function.

Move this functionality into the entry code so it can use the existing
idtentry functionality.

Mike Rapoport e31cf2f4ca mm: don't include asm/pgtable.h if linux/mm.h is already included
Patch series "mm: consolidate definitions of page table accessors", v2.

The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.

Most of these definitions are actually identical and typically it boils
down to, e.g.

static inline unsigned long pmd_index(unsigned long address)
{
        return (address >> PMD_SHIFT) & (PTRS_PER_PMD - 1);
}

static inline pmd_t *pmd_offset(pud_t *pud, unsigned long address)
{
        return (pmd_t *)pud_page_vaddr(*pud) + pmd_index(address);
}

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.

For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.

These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.

This patch (of 12):

The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.

The include statements in such cases are remove with a simple loop:

	for f in $(git grep -l "include <linux/mm.h>") ; do
		sed -i -e '/include <asm\/pgtable.h>/ d' $f
	done

Borislav Petkov a9a3ed1eff x86: Fix early boot crash on gcc-10, third try
... or the odyssey of trying to disable the stack protector for the
function which generates the stack canary value.

The whole story started with Sergei reporting a boot crash with a kernel
built with gcc-10:

Kernel panic — not syncing: stack-protector: Kernel stack is corrupted in: start_secondary
  CPU: 1 PID: 0 Comm: swapper/1 Not tainted 5.6.0-rc5—00235—gfffb08b37df9 #139
Hardware name: Gigabyte Technology Co., Ltd. To be filled by O.E.M./H77M—D3H, BIOS F12 11/14/2013
  Call Trace:
    dump_stack
    panic
    ? start_secondary
    __stack_chk_fail
    start_secondary
    secondary_startup_64
-—-[ end Kernel panic — not syncing: stack—protector: Kernel stack is corrupted in: start_secondary

This happens because gcc-10 tail-call optimizes the last function call
in start_secondary() - cpu_startup_entry() - and thus emits a stack
canary check which fails because the canary value changes after the
boot_init_stack_canary() call.

To fix that, the initial attempt was to mark the one function which
generates the stack canary with:

  __attribute__((optimize("-fno-stack-protector"))) ... start_secondary(void *unused)

however, using the optimize attribute doesn't work cumulatively
as the attribute does not add to but rather replaces previously
supplied optimization options - roughly all -fxxx options.

The key one among them being -fno-omit-frame-pointer and thus leading to
not present frame pointer - frame pointer which the kernel needs.

The next attempt to prevent compilers from tail-call optimizing
the last function call cpu_startup_entry(), shy of carving out
start_secondary() into a separate compilation unit and building it with
-fno-stack-protector, was to add an empty asm("").

This current solution was short and sweet, and reportedly, is supported
by both compilers but we didn't get very far this time: future (LTO?)
optimization passes could potentially eliminate this, which leads us
to the third attempt: having an actual memory barrier there which the
compiler cannot ignore or move around etc.

That should hold for a long time, but hey we said that about the other
two solutions too so...

Miroslav Benes c3881eb58d x86/xen: Make the secondary CPU idle tasks reliable
The unwinder reports the secondary CPU idle tasks' stack on XEN PV as
unreliable, which affects at least live patching.
cpu_initialize_context() sets up the context of the CPU through
VCPUOP_initialise hypercall. After it is woken up, the idle task starts
in cpu_bringup_and_idle() function and its stack starts at the offset
right below pt_regs. The unwinder correctly detects the end of stack
there but it is confused by NULL return address in the last frame.

Introduce a wrapper in assembly, which just calls
cpu_bringup_and_idle(). The return address is thus pushed on the stack
and the wrapper contains the annotation hint for the unwinder regarding
the stack state.

Miroslav Benes 2f62f36e62 x86/xen: Make the boot CPU idle task reliable
The unwinder reports the boot CPU idle task's stack on XEN PV as
unreliable, which affects at least live patching. There are two reasons
for this. First, the task does not follow the x86 convention that its
stack starts at the offset right below saved pt_regs. It allows the
unwinder to easily detect the end of the stack and verify it. Second,
startup_xen() function does not store the return address before jumping
to xen_start_kernel() which confuses the unwinder.

Amend both issues by moving the starting point of initial stack in
startup_xen() and storing the return address before the jump, which is
exactly what call instruction does.

Juergen Gross 99bcd4a6e5 x86/ioperm: Add new paravirt function update_io_bitmap()
Commit 111e7b15cf ("x86/ioperm: Extend IOPL config to control ioperm()
as well") reworked the iopl syscall to use I/O bitmaps.

Unfortunately this broke Xen PV domains using that syscall as there is
currently no I/O bitmap support in PV domains.

Add I/O bitmap support via a new paravirt function update_io_bitmap which
Xen PV domains can use to update their I/O bitmaps via a hypercall.

Kees Cook 9038ec99ce x86/xen: Distribute switch variables for initialization
Variables declared in a switch statement before any case statements
cannot be automatically initialized with compiler instrumentation (as
they are not part of any execution flow). With GCC's proposed automatic
stack variable initialization feature, this triggers a warning (and they
don't get initialized). Clang's automatic stack variable initialization
(via CONFIG_INIT_STACK_ALL=y) doesn't throw a warning, but it also
doesn't initialize such variables[1]. Note that these warnings (or silent
skipping) happen before the dead-store elimination optimization phase,
so even when the automatic initializations are later elided in favor of
direct initializations, the warnings remain.

To avoid these problems, move such variables into the "case" where
they're used or lift them up into the main function body.

Thomas Gleixner eec399dd86 x86/vdso: Move VDSO clocksource state tracking to callback
All architectures which use the generic VDSO code have their own storage
for the VDSO clock mode. That's pointless and just requires duplicate code.

X86 abuses the function which retrieves the architecture specific clock
mode storage to mark the clocksource as used in the VDSO. That's silly
because this is invoked on every tick when the VDSO data is updated.

Move this functionality to the clocksource::enable() callback so it gets
invoked once when the clocksource is installed. This allows to make the
clock mode storage generic.

Linus Torvalds 1a2a76c268 A set of fixes for X86:
- Ensure that the PIT is set up when the local APIC is disable or
configured in legacy mode. This is caused by an ordering issue
introduced in the recent changes which skip PIT initialization when the
TSC and APIC frequencies are already known.

  - Handle malformed SRAT tables during early ACPI parsing which caused an
    infinite loop anda boot hang.

  - Fix a long standing race in the affinity setting code which affects PCI
devices with non-maskable MSI interrupts. The problem is caused by the
non-atomic writes of the MSI address (destination APIC id) and data
(vector) fields which the device uses to construct the MSI message. The
non-atomic writes are mandated by PCI.

If both fields change and the device raises an interrupt after writing
address and before writing data, then the MSI block constructs a
inconsistent message which causes interrupts to be lost and subsequent
    malfunction of the device.

The fix is to redirect the interrupt to the new vector on the current
CPU first and then switch it over to the new target CPU. This allows to
observe an eventually raised interrupt in the transitional stage (old
CPU, new vector) to be observed in the APIC IRR and retriggered on the
new target CPU and the new vector. The potential spurious interrupts
caused by this are harmless and can in the worst case expose a buggy
driver (all handlers have to be able to deal with spurious interrupts as
they can and do happen for various reasons).

  - Add the missing suspend/resume mechanism for the HYPERV hypercall page
which prevents resume hibernation on HYPERV guests. This change got
    lost before the merge window.

  - Mask the IOAPIC before disabling the local APIC to prevent potentially
stale IOAPIC remote IRR bits which cause stale interrupt lines after
    resume.

Thomas Gleixner 979923871f x86/timer: Don't skip PIT setup when APIC is disabled or in legacy mode
Tony reported a boot regression caused by the recent workaround for systems
which have a disabled (clock gate off) PIT.

On his machine the kernel fails to initialize the PIT because
apic_needs_pit() does not take into account whether the local APIC
interrupt delivery mode will actually allow to setup and use the local
APIC timer. This should be easy to reproduce with acpi=off on the
command line which also disables HPET.

Due to the way the PIT/HPET and APIC setup ordering works (APIC setup can
require working PIT/HPET) the information is not available at the point
where apic_needs_pit() makes this decision.

To address this, split out the interrupt mode selection from
apic_intr_mode_init(), invoke the selection before making the decision
whether PIT is required or not, and add the missing checks into
apic_needs_pit().

Ingo Molnar eb243d1d28 x86/mm/pat: Rename <asm/pat.h> => <asm/memtype.h>
pat.h is a file whose main purpose is to provide the memtype_*() APIs.

PAT is the low level hardware mechanism - but the high level abstraction
is memtype.

So name the header <memtype.h> as well - this goes hand in hand with memtype.c
and memtype_interval.c.

Pull x86 iopl updates from Ingo Molnar:
"This implements a nice simplification of the iopl and ioperm code that
Thomas Gleixner discovered: we can implement the IO privilege features
of the iopl system call by using the IO permission bitmap in
permissive mode, while trapping CLI/STI/POPF/PUSHF uses in user-space
if they change the interrupt flag.

This implements that feature, with testing facilities and related
  cleanups"

[ "Simplification" may be an over-statement. The main goal is to avoid
the cli/sti of iopl by effectively implementing the IO port access
parts of iopl in terms of ioperm.

This may end up not workign well in case people actually depend on
cli/sti being available, or if there are mixed uses of iopl and
ioperm. We will see.. - Linus ]

   - Cross-arch changes to move the linker sections for NOTES and

   - Switch the x86 linker fill byte from x90 (NOP) to 0xcc (INT3), to
trap jumps into the middle of those padding areas instead of
     sliding execution. (Kees Cook)

   - A thorough cleanup of symbol definitions within x86 assembler code.

     (hopefully) straightforward naming scheme:

        SYM_START(name, linkage, align...)

Jan Beulich 922eea2ce5 x86/xen/32: Simplify ring check in xen_iret_crit_fixup()
This can be had with two instead of six insns, by just checking the high
CS.RPL bit.

Also adjust the comment - there would be no #GP in the mentioned cases, as
there's no segment limit violation or alike. Instead there'd be #PF, but
that one reports the target EIP of said branch, not the address of the
branch insn itself.

Jan Beulich 29b810f5a5 x86/xen/32: Make xen_iret_crit_fixup() independent of frame layout
Now that SS:ESP always get saved by SAVE_ALL, this also needs to be
accounted for in xen_iret_crit_fixup(). Otherwise the old_ax value gets
interpreted as EFLAGS, and hence VM86 mode appears to be active all the
time, leading to random "vm86_32: no user_vm86: BAD" log messages alongside
processes randomly crashing.

Since following the previous model (sitting after SAVE_ALL) would further
complicate the code _and_ retain the dependency of xen_iret_crit_fixup() on
frame manipulations done by entry_32.S, switch things around and do the
adjustment ahead of SAVE_ALL.

Jiri Slaby e9b9d020c4 x86/asm: Annotate aliases
_key_expansion_128 is an alias to _key_expansion_256a, __memcpy to
memcpy, xen_syscall32_target to xen_sysenter_target, and so on. Annotate
them all using the new SYM_FUNC_START_ALIAS, SYM_FUNC_START_LOCAL_ALIAS,
and SYM_FUNC_END_ALIAS. This will make the tools generating the
debuginfo happy as it avoids nesting and double symbols.

Boris Ostrovsky c6875f3aac x86/xen: Return from panic notifier
Currently execution of panic() continues until Xen's panic notifier
(xen_panic_event()) is called at which point we make a hypercall that
never returns.

This means that any notifier that is supposed to be called later as
well as significant part of panic() code (such as pstore writes from
kmsg_dump()) is never executed.

There is no reason for xen_panic_event() to be this last point in
execution since panic()'s emergency_restart() will call into
xen_emergency_restart() from where we can perform our hypercall.

Nevertheless, we will provide xen_legacy_crash boot option that will
preserve original behavior during crash. This option could be used,
for example, if running kernel dumper (which happens after panic
notifiers) is undesirable.

Today the EFI runtime functions are setup in architecture specific
code (x86 and arm), with the functions themselves living in drivers/xen
as they are not architecture dependent.

As the setup is exactly the same for arm and x86 move the setup to
drivers/xen, too. This at once removes the need to make the single

Andrew Cooper 83b584d9c6 x86/paravirt: Drop {read,write}_cr8() hooks
There is a lot of infrastructure for functionality which is used
exclusively in __{save,restore}_processor_state() on the suspend/resume
path.

cr8 is an alias of APIC_TASKPRI, and APIC_TASKPRI is saved/restored by
lapic_{suspend,resume}(). Saving and restoring cr8 independently of the
rest of the Local APIC state isn't a clever thing to be doing.

Delete the suspend/resume cr8 handling, which shrinks the size of struct
saved_context, and allows for the removal of both PVOPS.

Commit 7457c0da02 ("x86/alternatives: Add int3_emulate_call()

which could be used for inserting call return address.

This gap is missed in XEN PV int3 exception entry path, then below panic
triggered:

For 64bit PV guests, Xen's ABI enters the kernel with using SYSRET, with
%rcx/%r11 on the stack. To convert back to "normal" looking exceptions,
the xen thunks do 'xen_*: pop %rcx; pop %r11; jmp *'.

E.g. Extracting 'xen_pv_trap xenint3' we have:
xen_xenint3:
 pop %rcx;
 pop %r11;
 jmp xenint3

As xenint3 and int3 entry code are same except xenint3 doesn't generate
a gap, we can fix it by using int3 and drop useless xenint3.

Zhenzhong Duan bef6e0ae74 x86/xen: Add "nopv" support for HVM guest
PVH guest needs PV extentions to work, so "nopv" parameter should be
ignored for PVH but not for HVM guest.

If PVH guest boots up via the Xen-PVH boot entry, xen_pvh is set early,
we know it's PVH guest and ignore "nopv" parameter directly.

If PVH guest boots up via the normal boot entry same as HVM guest, it's
hard to distinguish PVH and HVM guest at that time. In this case, we
have to panic early if PVH is detected and nopv is enabled to avoid a
worse situation later.

Remove static from bool_x86_init_noop/x86_op_int_noop so they could be
used globally. Move xen_platform_hvm() after xen_hvm_guest_late_init()
to avoid compile error.
