Boris Ostrovsky aec03f89e9 ACPI/NUMA: Do not map pxm to node when NUMA is turned off
acpi_map_pxm_to_node() unconditially maps nodes even when NUMA is turned
off. So acpi_get_node() might return a node > 0, which is fatal when NUMA
is disabled as the rest of the kernel assumes that only node 0 exists.

Expose numa_off to the acpi code and return NUMA_NO_NODE when it's set.

Yijing Wang eee46b3de7 Fix build error for numa_clear_node() under IA64
numa_clear_node() function is not implemented under IA64,
it will be called in unmap_cpu_on_node() in mm/memory_hotplug.c.
This cause build error under IA64, this patch adds numa_clear_node()
in IA64 to fix this problem.

[Added __cpuinit notation to numa_clear_node() to keep linker happy -Tony]

Jianguo Wu 7cd10a60c3 [IA64] Fix a node distance bug
In arch ia64, has following definition:
extern u8 numa_slit[MAX_NUMNODES * MAX_NUMNODES];
#define node_distance(from,to) (numa_slit[(from) * num_online_nodes() + (to)])

num_online_nodes() is a variable value, it can be changed after hot-remove/add
a node.

In my practice, I found node distance is wrong after offline
a node in IA64 platform. For example system has 4 nodes:
node distances:
node   0   1   2   3
  0:  10  21  21  32
  1:  21  10  32  21
  2:  21  32  10  21
  3:  32  21  21  10

linux-drf:/sys/devices/system/node/node0 # cat distance
10  21  21  32
linux-drf:/sys/devices/system/node/node1 # cat distance
21  10  32  21

After offline node2:
linux-drf:/sys/devices/system/node/node0 # cat distance
10 21 32
linux-drf:/sys/devices/system/node/node1 # cat distance
32 21 32	--------->expected value is: 21  10  21

Lee Schermerhorn 4e25b2576e hugetlb: add generic definition of NUMA_NO_NODE
Move definition of NUMA_NO_NODE from ia64 and x86_64 arch specific headers
to generic header 'linux/numa.h' for use in generic code.  NUMA_NO_NODE
replaces bare '-1' where it's used in this series to indicate "no node id
specified".  Ultimately, it can be used to replace the -1 elsewhere where
it is used similarly.

Tony Luck 7f30491ccd [IA64] Move include/asm-ia64 to arch/ia64/include/asm
After moving the the include files there were a few clean-ups:

1) Some files used #include <asm-ia64/xyz.h>, changed to <asm/xyz.h>

2) Some comments alerted maintainers to look at various header files to
make matching updates if certain code were to be changed. Updated these
comments to use the new include paths.

3) Some header files mentioned their own names in initial comments. Just
deleted these self references.
