asm/page.h uses READ_IMPLIES_EXEC from linux/personality.h but does not
explicitly include it causing build failures in -next where whatever was
causing it to be implicitly included has changed to remove that
inclusion. Add an explicit inclusion to fix this.

This patch turns on the 16K page support in the kernel. We
support 48bit VA (4 level page tables) and 47bit VA (3 level
page tables).
With 16K we can map 128 entries using contiguous bit hint
at level 3 to map 2M using single TLB entry.
TODO: 16K supports 32 contiguous entries at level 2 to get us
1G(which is not yet supported by the infrastructure). That should
be a separate patch altogether.

Move the kernel pagetable (both swapper and idmap) definitions
from the generic asm/page.h to a new file, asm/kernel-pgtable.h.
This is mostly a cosmetic change, to clean up the asm/page.h to
get rid of the arch specific details which are not needed by the
generic code.
Also renames the symbols to prevent conflicts. e.g,
BLOCK_SHIFT => SWAPPER_BLOCK_SHIFT

The page size and the number of translation levels, and hence the supported
virtual address range, are build-time configurables on arm64 whose optimal
values are use case dependent. However, in the current implementation, if
the system's RAM is located at a very high offset, the virtual address range
needs to reflect that merely because the identity mapping, which is only used
to enable or disable the MMU, requires the extended virtual range to map the
physical memory at an equal virtual offset.
This patch relaxes that requirement, by increasing the number of translation
levels for the identity mapping only, and only when actually needed, i.e.,
when system RAM's offset is found to be out of reach at runtime.

The core mm code will provide a default gate area based on
FIXADDR_USER_START and FIXADDR_USER_END if
!defined(__HAVE_ARCH_GATE_AREA) && defined(AT_SYSINFO_EHDR).
This default is only useful for ia64. arm64, ppc, s390, sh, tile, 64-bit
UML, and x86_32 have their own code just to disable it. arm, 32-bit UML,
and x86_64 have gate areas, but they have their own implementations.
This gets rid of the default and moves the code into ia64.
This should save some code on architectures without a gate area: it's now
possible to inline the gate_area functions in the default case.

This patch allows support for 3 levels of page tables with 64KB page
configuration allowing 48-bit VA space. The pgd is no longer a full
PAGE_SIZE (PTRS_PER_PGD is 64) and (swapper|idmap)_pg_dir are not fully
populated (pgd_alloc falls back to kzalloc).

This patch implements 4 levels of translation tables since 3 levels
of page tables with 4KB pages cannot support 40-bit physical address
space described in [1] due to the following issue.
It is a restriction that kernel logical memory map with 4KB + 3 levels
(0xffffffc000000000-0xffffffffffffffff) cannot cover RAM region from
544GB to 1024GB in [1]. Specifically, ARM64 kernel fails to create
mapping for this region in map_mem function since __phys_to_virt for
this region reaches to address overflow.
If SoC design follows the document, [1], over 32GB RAM would be placed
from 544GB. Even 64GB system is supposed to use the region from 544GB
to 576GB for only 32GB RAM. Naturally, it would reach to enable 4 levels
of page tables to avoid hacking __virt_to_phys and __phys_to_virt.
However, it is recommended 4 levels of page table should be only enabled
if memory map is too sparse or there is about 512GB RAM.

Currently we place swapper_pg_dir and idmap_pg_dir below the kernel
image, between PHYS_OFFSET and (PHYS_OFFSET + TEXT_OFFSET). However,
bootloaders may use portions of this memory below the kernel and we do
not parse the memory reservation list until after the MMU has been
enabled. As such we may clobber some memory a bootloader wishes to have
preserved.
To enable the use of all of this memory by bootloaders (when the
required memory reservations are communicated to the kernel) it is
necessary to move our initial page tables elsewhere. As we currently
have an effectively unbound requirement for memory at the end of the
kernel image for .bss, we can place the page tables here.
This patch moves the initial page table to the end of the kernel image,
after the BSS. As they do not consist of any initialised data they will
be stripped from the kernel Image as with the BSS. The BSS clearing
routine is updated to stop at __bss_stop rather than _end so as to not
clobber the page tables, and memory reservations made redundant by the
new organisation are removed.

This patch adds support for the handling of the MMU faults (exception
entry code introduced by a previous patch) and page table management.
The user translation table is pointed to by TTBR0 and the kernel one
(swapper_pg_dir) by TTBR1. There is no translation information shared or
address space overlapping between user and kernel page tables.
