sigp sense only returns the status of a cpu if it is non zero. If the
status of the sensed cpu is all zeros condition code 0 (accpeted) is
set and no status bits are returned.
The current code however assumes that a status was returned and tests
bits in it. This means uninitalized data is accessed with random
results.
Worst case is that the code that checks if cpu is offline on cpu
hotplug assumes that the target cpu is offline while it is still
running. This leads potentially to memory corruption since resources
that are still needed by the target cpu will be freed and could be
resused while still in use.

According to the architecture a cpu must not necessarily enter stopped
state after completion of a sigp instruction with "stop" order code.
So remove the BUG() statement after self sending sigp stop to avoid
that it ever gets reached.
Also add a sigp busy check to make sure that the order gets delivered.

Offlined cpus still have valid prefix register contents. Dumpers
will store the register contents of a cpu to the location where its
prefix register points to.
For offlined cpus the area (lowcore) has been freed and the dumper
would write the uninteresting contents of the offline cpu to a memory
location which might be in use by some other component and destroy
valueable information.
To fix this set the prefix register of offline cpus to absolute
address zero again. This prevents the current dumpers to write to
random memory locations.

The S3C64XX DMA implementation will work a lot better with the ability
to enqueue circular buffers as the hardware can do it's own linked-list
management.
Add a function s3c_dma_has_circular() to show that the system can do this
and a flag for the channel.
Update the s3c24xx/s3c64xx I2S DMA code to deal with this.

The function iommu_feature_disable is required on system
shutdown to disable the IOMMU but it is marked as __init.
This may result in a panic if the memory is reused. This
patch fixes this bug.

xen_setup_stackprotector() ends up trying to set page protections,
so we need to have vm_mmu_ops set up before trying to do so.
Failing to do so causes an early boot crash.

The EFI RTC functions are only available on 32 bit. commit 7bd867df
(x86: Move get/set_wallclock to x86_platform_ops) removed the 32bit
dependency which leads to boot crashes on 64bit EFI systems.
Add the dependency back.

Use preempt_schedule_irq to prevent infinite irq-entry and
eventual stack overflow problems with fast-paced IRQ sources.
This kind of problems has been observed on the PASemi Electra IDE
controller. We have to make sure we are soft-disabled before calling
preempt_schedule_irq and hard disable interrupts after that
to avoid unrecoverable exceptions.
This patch also moves the "clrrdi r9,r1,THREAD_SHIFT" out of
the #ifdef CONFIG_PPC_BOOK3E scope, since r9 is clobbered
and has to be restored in both cases.

This defconfig's purpose at this time is to help catch compile errors
between Book-3S and Book-3E support in ppc64. It is based on the
ppc64_defconfig with some things disabled that we dont support on
Book-3E right now (hugetlbfs, slices, etc.)

Prior to the arch/ppc -> arch/powerpc transition, xmon had support for single
stepping on 4xx boards. The functionality was lost when arch/ppc was removed.
This patch restores single step support for 44x boards, and Book-E in general.

The creation of the flattened device tree depended on the compiler
putting the constant strings for an object in a section with a
particular name. This was changed with recent compilers. Do this
explicitly instead.
Without this patch, iseries kernels may silently not boot when built with
some compilers.

The architecture defines that if MSR PR is set we are in problem state
irrespective of the HV bit. This fixes perf events to reflect this.
Also, on bare metal systems, samples taken in Linux will now be reported
as kernel rather than hypervisor.

The hugetlb dependencies presently depend on SUPERH && MMU while the
hugetlb page size definitions depend on CPU_SH4 or CPU_SH5. This
unfortunately allows SH-3 + MMU configurations to enable hugetlbfs
without a corresponding HPAGE_SHIFT definition, resulting in the build
blowing up.
As SH-3 doesn't support variable page sizes, we tighten up the
dependenies a bit to prevent hugetlbfs from being enabled. These days
we also have a shiny new SYS_SUPPORTS_HUGETLBFS, so switch to using
that rather than adding to the list of corner cases in fs/Kconfig.

Restoring %ebp after the call to audit_syscall_exit() is not
only unnecessary (because the register didn't get clobbered),
but in the sysenter case wasn't even doing the right thing: It
loaded %ebp from a location below the top of stack (RBP <
ARGOFFSET), i.e. arbitrary kernel data got passed back to user
mode in the register.

Non-PAE 32-bit dump kernels may wrap an address around 4G and
poke unwanted space. ptes there are 32-bit long, and since
pfn << PAGE_SIZE may exceed this limit, high pfn bits are
cropped and wrong address mapped by kmap_atomic_pfn in
copy_oldmem_page.
Don't allow this behavior in non-PAE kdump kernels by checking
pfns passed into copy_oldmem_page. In the case of failure,
userspace process gets EFAULT.

Commit 79e1dd05d1 "x86: Provide an alternative() based
cmpxchg64()" broke lguest, even on systems which have cmpxchg8b
support. The emulation code gets used until alternatives get
run, but it contains native instructions, not their paravirt
alternatives.
The simplest fix is to turn this code off except for 386 and 486
builds.

When CONFIG_FUNCTION_GRAPH_TRACER is enabled the function graph tracer
may patch return addresses on the stack with the address of
return_to_handler(). This really confuses the DWARF unwinder because it
will try find the caller of return_to_handler(), not the caller of the
real return address.
So teach the DWARF unwinder how to find the real return address whenever
it encounters return_to_handler().
This patch does not cope very well when multiple return addresses on the
stack have been patched. To make it work properly it would require state
to track how many return_to_handler()'s have been seen so that we'd know
where to look in current->curr_ret_stack[]. So for now, instead of
trying to handle this, just moan if more than one return address on the
stack has been patched.

This adds an __irq_entry annotation for do_IRQ() so that the IRQ
annotation in the function graph tracer works as advertized. We already
have the IRQENTRY section wired up, so this is just a trivial addition
to actually make use of it.

We were using GFP_DMA for masks other than 0xffffffff, which is
wrong when some masks are initialized to 0xffffffffffffffff.
This caused such masks to obtain memory from the precious DMA
pool.

The signal restarting code was placed on the user stack when OABI
compatibility is enabled. Unfortunately, with an EABI NX executable,
this results in an attempt to run code from the non-executable stack,
which segfaults the application.
Fix this by placing the code in the vectors page, along side the
signal return code, and directing the application to that code.

STACKPROTECTOR_ALL has a really high overhead (runtime and stack
footprint) and is not really worth it protection wise (the
normal STACKPROTECTOR is in effect for all functions with
buffers already), so lets just remove the option entirely.

This patch enables omap_serial_early_init() function for OMAP4430
SDP. Without this the bootup would throw oops in omap_serial_init().
Note that the ifndef CONFIG_ARCH_OMAP4 is split into two sections
to enable omap_serial_early_init(). This ifndef cannot be removed
until omap4 clock framework is implemented.

UART3 is in the PER powerdomain. If PER goes idle/inactive
independently of CORE, for UART3 to wakeup it must have its wakeup
enable bits setup in PM_WKEN_PER. This patch enables these bits.
The reason it works when PER and CORE work together is because when
CORE goes inactive/retention, the IOPAD wakeups are enabled and
trigger UART3 wakeup.
Without this patch, when the UART inactivity timer fires for UART3,
its clocks are disabled and it's unable to wakeup so will be unusable
until PER is awoken by another source.
Another way of testing is by keeping CORE on during suspend but
allowing PER to hit retention
# echo 3 > /debug/pm_debug/core_pwrdm/suspend
then enter suspend
# echo mem > /sys/power/state
Without this patch, UART3 will be unable to wakeup the system.

DSP public peripherals used to work on OMAP1510 based (or all OMAP1 class?)
machines as long as old dspgateway code were present in the l-o tree. For
several months it is no longer included, breaking support for McBSP1 based
audio on Amstrad Delta, for example.
This patch, derived from the old dspgateway code, corrects the problem for the
board by simply taking the DSP out of reset state, I guess. That way, things
should not break when a new dsp code is added to the tree, and the change can
be reverted then.
If there are any reports on McBSP1 or other DSP public peripherals not working
for other OMAP1 machines (I've not heard of any for now), I can prepare a more
general patch providing an extra include file with a helper function defined.

Commit 15ac408ee5 removed enabled_uart
and OMAP_TAG_UART. This works for mach-omap2, but causes issues on
mach-omap1 for some boards as the mach-omap1 serial.c was muxing
pins based on the enabled_uart flag for 15xx.
Fix this by muxing pins in board-*.c files for the 15xx boards for
the uart ports that had enabled_uart flag set before the commit
above.
Tested on Amsdtrad Delta only.
Note that in the future we should add support for powering down
the uarts with a timer like mach-omap2/serial.c does. Otherwise
the enabled uarts will be blocking retention-while-idle.

OMAP sDMA driver API omap_stop_dma() doesn't really stop the dma when used
in linking scenario.
The DMA channel needs to be disabled before resetting the chain.
Also fix clearing of the OMAP_DMA_ACTIVE status in the linked case.

When sending a NMI_VECTOR IPI using the UV_HUB_IPI_INT register,
we need to ensure the delivery mode field of that register has
NMI delivery selected.
This makes those IPIs true NMIs, instead of flat IPIs. It
matters to reboot sequences and KGDB, both of which use NMI
IPIs.

When renaming kernel_fpu_using to irq_fpu_usable, the semantics of the
function is changed too, from mesuring whether kernel is using FPU,
that is, the FPU is NOT available, to measuring whether FPU is usable,
that is, the FPU is available.
But the usage of irq_fpu_usable in aesni-intel_glue.c is not changed
accordingly. This patch fixes this.

Create an inline function to extract the pnode from a global
physical address and then convert the broadcast assist unit to
use the newly created uv_gpa_to_pnode function.
The open-coded code was wrong as well - it might explain a
few of our unexplained bau hangs.

The MCE initialization code explicitly says it doesn't handle
asymmetric configurations where different CPUs support different
numbers of MCE banks, and it prints a big warning in that case.
Therefore, printing the "mce: CPU supports <x> MCE banks"
message into the kernel log for every CPU is pure redundancy
that clutters the log significantly for systems with lots of
CPUs.

A few parts of the uv_hub_info structure are initialized
incorrectly.
- n_val is being loaded with m_val.
- gpa_mask is initialized with a bytes instead of an unsigned long.
- Handle the case where none of the alias registers are used.
Lastly I converted the bau over to using the uv_hub_info->m_val
which is the correct value.
Without this patch, booting a large configuration hits a
problem where the upper bits of the gnode affect the pnode
and the bau will not operate.

The open function got the BKL via the big push down. Replace it by
preempt_enable/disable as this is sufficient for an UP machine.
The ioctl can be unlocked because there is no functionality which
requires serialization. The usage by multiple callers is broken with
and without the BKL due to the local static variable addr.

Add code to handle the cache disabled case. Fixes breakage introduced by
37443ef3f0 ("sh: Migrate SH-4 cacheflush
ops to function pointers."). Without this patch configuring caches off
with CONFIG_CACHE_OFF=y makes kfr2r09 and migo-r lock up in fbdev
deferred io or early user space.

Presently The SH-4 cache flushing code uses flush_cache_4096() for most
of the real flushing work, which breaks down to a fixed 4096 unroll and
increment. Not only is this sub-optimal for larger page sizes, it's also
uncovered a bug in sh4_flush_dcache_page() when large page sizes are used
and we have no cache aliases -- resulting in only a part of the page's
D-cache lines being written back.

clock24xx.c is missing a omap2_init_clk_clkdm() in its
omap2_clk_init() function. Among other bad effects, this causes the
OMAP hwmod layer to oops on boot.

There is a possible race condition in clockdomain
code handling hw supported idle transitions.
When multiple autodeps dependencies are being added
or removed, a transition of still remaining dependent
powerdomain can result in false readings of the
state counter. This is especially fatal for off mode
state counter, as it could result in a driver not
noticing a context loss.
Fixed by disabling hw supported state transitions
when autodeps are being changed.

This small typo written by author causes that McBSP receiver is disabled on
OMAP2430 and OMAP3430 even if only transmitter is stopped. This was noted
with ALSA SoC where simultaneous recording halted if playback was stopped
first.

The I2C-1 bus frequency on RX-51 should be 2.2 MHz. The speed is limited
by TWL5030/GAIA; a higher speed could lead to errors on the interface. The
maximum speed depends on the system clock for GAIA: 2.2 MHz (if 19.2 MHz),
2.4 MHz (26 MHz) or 2.9 MHz (38.4 MHz).

Yanmin reported that both tbench and hackbench were significantly
hurt by trying to keep tasks local on these domains, esp on small
cache machines.
So disable it in order to promote spreading outside of the cache
domains.

Hypfs never worked on systems that only provide D204 subcode 6.
In these cases we nevertheless used subcode 7. With this fix, we
use subcode 6, if it is available and the system does not provide
subcode 7.

The 32-bit wide variant of "mov pc, reg" in Thumb-2 is unpredictable
causing improper handling of the undefined instructions not caught by
the kernel. This patch adds a movw_pc macro for such situations
(currently only used in call_fpe).

The resume_userspace path had TRACE_IRQS_OFF written incorrectly and so
never handled the transition properly. This was fixed once before but
seems to have made it back in the tree. Fix it for good.

The secondary CPU info was seeing corrupted results due to not entering
all of the setup paths taken by the boot CPU. So we just memcpy() the
boot cpu data over directly, and then fix up the per-CPU bits.

We need to align before the output section. Having the align inside
the output section causes the linker to put some filler in there,
which makes it a non-empty section, but this section isn't assigned to
a segment so you get a warning from the linker.

Profiling of a page fault scalability microbenchmark shows flush_hash_range
is not calling the batch hpte invalidate hcall (H_BULK_REMOVE).
It turns out we have a duplicate firmware feature for hcall-bulk and the
current setup code stops after finding the first match. This meant we never
batch and always do individual invalidates.
The patch below removes the duplicate and shifts FW_FEATURE_CMO to close
the gap. With the patch applied the single threaded page fault rate improves
from 217169 to 238755 per second on a POWER5 test box, a 10% improvement.

On pSeries, we always force the IO space to be mapped using 4K
pages even with a 64K base page size to cope with some limitations
in the HV interface to some devices.
However, the SLB miss handler code to discriminate between vmalloc
and ioremap space uses a CPU feature section such that the code
is nop'ed out when the processor support large pages non-cachable
mappings.
Thus, we end up always using the ioremap page size for vmalloc
segments on such processors, causing a discrepency between the
segment and the hash table, and thus a hang continously hashing
the page.
It works for the first segment of the vmalloc space since that
segment is "bolted" in by C code correctly, and thankfully we
almost never use the vmalloc space beyond the first segment,
but the new percpu code made the bug happen.
This fixes it by removing the feature section from the assembly,
we now always do the comparison between vmalloc and ioremap.

Since the change of how interrupts are disabled during suspend,
certain PowerBook models started exhibiting various issues during
suspend or resume from sleep.
I finally tracked it down to the code that runs various "platform"
functions (kind of little scripts extracted from the device-tree),
which uses our i2c and PMU drivers expecting interrutps to work,
and at a time where with the new scheme, they have been disabled.
This causes timeouts internally which for some reason results in
the PMU being unable to see the trackpad, among other issues, really
it depends on the machine. Most of the time, we fail to properly adjust
some clocks for suspend/resume so the results are not always
predictable.
This patch fixes it by using IRQF_TIMER for both the PMU and the I2C
interrupts. I prefer doing it this way than moving the call sites since
I really want those platform functions to still be called after all
drivers (and before sysdevs).
We also do a slight cleanup to via-pmu.c driver to make sure the
ADB autopoll mask is handled correctly when doing bus resets

The mod_return_to_handler needs to switch to the kernel TOC before
jumping to a the kernel code. It currently does this by looking
at the kernel function data and retrieves the TOC that way.
Not only is this inefficient, it also breaks with a relocatable kernel.
The PACA contains the kernel TOC and we can easily retrieve it that
way.

When the function graph tracer is enabled, it replaces the return address
with a hook back to the tracer. This makes back traces see the hook instead
of the actual return address.
The current code also shows the real address by checking if the return
address jumps to the return_to_handler. If it is, is also prints out
the saved real return address.
On powerpc64, some modules may return to mod_return_to_handler, which
is not checked. This patch will also show the real address if a return
is to mod_return_to_handler as well.

We had assumed that SAL firmware would return an error if it didn't
understand extended config space. Unfortunately, the SAL on the SGI 750
doesn't do that, it panics the machine. So, condition the extended PCI
config space accesses on SAL revision 3.2.

Registers are not saved anywhere when INIT comes during fsys mode and
we cannot know what happened when we investigate vmcore captured by
kdump. This patch adds new function finish_pt_regs() so registers can
be saved in such a case.

not linked for i.MX31 and produces an undefined reference error. Fix this

instead of mxc_gpio_setup_multiple_pins() because the latter

Bastian Blank reported a boot crash with stackprotector enabled,
and debugged it back to edx register corruption.
For historical reasons irq enable/disable/save/restore had special
calling sequences to make them more efficient. With the more
recent introduction of higher-level and more general optimisations
this is no longer necessary so we can just use the normal PVOP_
macros.
This fixes some residual bugs in the old implementations which left
edx liable to inadvertent clobbering. Also, fix some bugs in
__PVOP_VCALLEESAVE which were revealed by actual use.

This too follows the ARM change, given that the issue at hand applies to
all platforms that implement lazy D-cache writeback.
This fixes up the case when a page mapping disappears between the
flush_dcache_page() call (when PG_dcache_dirty is set for the page) and
the update_mmu_cache() call -- such as in the case of swap cache being
freed early. This kills off the mapping test in update_mmu_cache() and
switches to simply testing for PG_dcache_dirty.

On ARM, update_mmu_cache() does dcache flush for a page only if
it has a kernel mapping (page_mapping(page) != NULL). The correct
behavior would be to force the flush based on dcache_dirty bit only.
One of the cases where present logic would be a problem is when
a RAM based block device[1] is used as a swap disk. In this case,
we would have in-memory data corruption as shown in steps below:
do_swap_page()
{
- Allocate a new page (if not already in swap cache)
- Issue read from swap disk
- Block driver issues flush_dcache_page()
- flush_dcache_page() simply sets PG_dcache_dirty bit and does not
actually issue a flush since this page has no user space mapping yet.
- Now, if swap disk is almost full, this newly read page is removed
from swap cache and corrsponding swap slot is freed.
- Map this page anonymously in user space.
- update_mmu_cache()
- Since this page does not have kernel mapping (its not in page/swap
cache and is mapped anonymously), it does not issue dcache flush
even if dcache_dirty bit is set by flush_dcache_page() above.
<user now gets stale data since dcache was never flushed>

We want this to happen after the PCI quirks, which are now running at
the very end of the fs_initcalls.
This works around the BIOS problems which were originally addressed by
commit db8be50c43 ('USB: Work around BIOS
bugs by quiescing USB controllers earlier'), which was reverted in
commit d93a8f829f.

Thanks to tip form ARM folks and Russell King.
If flush_dcache_page() occurs on a swapin it will have a mapping
and we'll try to defer the flush by setting the dirty bit.
But when it hits update_dcache_page() we won't flush because the
page won't have a mapping any more. So remove the mapping
requirement in flush_dcache().

initialized twice. The value in the first case,
SMSC911X_IRQ_POLARITY_ACTIVE_LOW, is normally stored in the irq_polarity
field, so I have renamed the field in the first initialization to that.

on some system when acpi are enabled, acpi clears some BAR for some
devices without reason, and kernel will need to allocate devices for
them. It then apparently hits some undocumented resource conflict,
resulting in non-working devices.
Try to increase alignment to get more safe range for unassigned devices.

Bit testing (test, testset, testclear, testchange) for bit numbers
known at compile time returns a word with the tested-for bit set.
Change it to return a true boolean value so to make it consistent with
the out-of-line path and all the other bitops implementations.

dump_mem and dump_backtrace were both using multiple printk statements
to print each line. With DEBUG_LL enabled, this causes OOPS to become
very difficult to read. Solve this by only using one printk per line.

The clock generation system in the ep93xx uses two external oscillator's
and two internal PLLs to derive all the internal clocks. Many of these
internal clocks can be stopped to save power.
This introduces a "parent" hierarchy for the clocks so that the users
count can be correctly tracked for power management.
The "parent" for the video clock can either be one of the PLL outputs
or the external oscillator. In order to correctly track the "parent"
for the video clock calc_clk_div() needed to be modified. It now
returns an error code if the desired rate cannot be generated.

Update the ep93xx i2c support:
1) The platform init code passes the configuration data for the
i2c-gpio driver. This allows any gpio pin do be used for the
sda and scl pins. It also allows the platform to specify the
udelay and timeout.
2) Program the gpio configuration register to enable/disable the
open drain drivers. Note that this really only works if the
sda and scl pins are set to EP93XX_GPIO_LINE_EEDAT and
EP93XX_GPIO_LINE_EECLK.
3) Update the edb93xx.c platform init to use the new support.

Bound the wait time for the ptcg_sem by using similar idea to the
ticket spin locks. In this case we have only one instance of a
spinaphore, so make it 8 bytes rather than try to squeeze it into
4-bytes to keep the code simpler (and shorter).

There is an erratum for IOMMU hardware which documents
undefined behavior when forwarding SMI requests from
peripherals and the DTE of that peripheral has a sysmgt
value of 01b. This problem caused weird IO_PAGE_FAULTS in my
case.
This patch implements the suggested workaround for that
erratum into the AMD IOMMU driver. The erratum is
documented with number 63.

The real bug producing LatencyTop latencies has been fixed in:
f5dc375: sched: Update the clock of runqueue select_task_rq() selected
And the commit being reverted here triggers local timer processing
from every device IRQ. If device IRQs come in at a high frequency,
this could cause a performance regression.
The commit being reverted here purely 'fixed' the reported latency
as a side effect, because CPUs were being moved out of idle more
often.

Currently, we've got the less than ideal situation where if we need to
allocate a 256MB mapping we'll allocate four entries like so,
entry 1: 128MB
entry 2: 64MB
entry 3: 16MB
entry 4: 16MB
This is because as we execute the loop in pmb_remap() we will
progressively try mapping the remaining address space with smaller and
smaller sizes. This isn't good because the size we use on one iteration
may be the perfect size to use on the next iteration, for instance when
the initial size is divisible by one of the PMB mapping sizes.
With this patch, we now only need two entries in the PMB to map 256MB of
address space,
entry 1: 128MB
entry 2: 128MB

Now that range timers and deferred timers are common, I found a
problem with these using the "perf timechart" tool. Frans Pop also
reported high scheduler latencies via LatencyTop, when using
iwlagn.
It turns out that on x86, these two 'opportunistic' timers only get
checked when another "real" timer happens. These opportunistic
timers have the objective to save power by hitchhiking on other
wakeups, as to avoid CPU wakeups by themselves as much as possible.
The change in this patch runs this check not only at timer
interrupts, but at all (device) interrupts. The effect is that:
1) the deferred timers/range timers get delayed less
2) the range timers cause less wakeups by themselves because
the percentage of hitchhiking on existing wakeup events goes up.
I've verified the working of the patch using "perf timechart", the
original exposed bug is gone with this patch. Frans also reported
success - the latencies are now down in the expected ~10 msec
range.

The patch added a special get_unmapped_area for framebuffer which
was hooked to the file ops in drivers/video/fbmem.c.
This is needed since v2.6.29-rc1 where nommu vma management was
updated, and mmap of framebuffer caused kernel BUG panic. You may turn
on "Debug the global anon/private NOMMU mapping region tree" config to
such message.
As Documentation/nommu-mmap.txt said,
"To provide shareable character device support, a driver must provide
a file->f_op->get_unmapped_area() operation. The mmap() routines will
call this to get a proposed address for the mapping."
With this change, user space should call mmap for framebuffer using
shared map. Or it can try shared map first, then private map if
failed. This shared map usage is now consistent between mmu and nommu.
The sys_ file may not be a good place for this patch. But there is a
similar one for sparc. I tested a similar patch on nios2nommu, though
I don't have a blackfin board to test.

Linus pointed out that other people have spent large amounts of time
and effort to optimize the layout of frequently used structures. Often
these have embedded locks, and the assumption is that a lock takes
4 bytes. Linus also pointed out how to work with the limited options
for atomic instructions on Itanium.

Both iPaqs h3100 and h3600 currently share the same source
file - h3600.c But Makefile builds it only if CONFIG_SA1100_H3600
selected, so selecting just CONFIG_SA1100_H3100 results in
"no machine record defined" message and aborted compilation.
Fix it.

If sparsemem is enabled, the start_pfn passed to the free_memmap()
function corresponds to an area of memory not known to the kernel and
pfn_to_page returns a wrong value. The (start_pfn - 1), however, is
known to the kernel.

This is needed because applications using the sys_cacheflush system call
can pass a memory range which isn't mapped yet even though the
corresponding vma is valid. The patch also adds unwinding annotations
for correct backtraces from the coherent_user_range() functions.

The cplbinfo was using d_path() to figure out which cpu/cplb was being
parsed. As Al pointed out, this isn't exactly reliable as it assumes the
static VFS path to be unchanged, and it's just poor form. So use the
proc_create_data() to properly (and internally) pass the exact cpu/cplb
requested to the parser function.

Bill Gatliff & David Brownell pointed out we were missing some
copyrights, and licensing terms in some of the files in
./arch/blackfin, so this fixes things, and cleans them up.
It also removes:
- verbose GPL text(refer to the top level ./COPYING file)
- file names (you are looking at the file)
- bug url (it's in the ./MAINTAINERS file)
- "or later" on GPL-2, when we did not have that right
It also allows some Blackfin-specific assembly files to be under a BSD
like license (for people to use them outside of Linux).

Convert Blackfin to use GENERIC_TIME via the arch_getoffset()
infrastructure, reducing the amount of arch specific code we need to
maintain.
I've taken my best swing at converting this, but I'm not 100% confident
I got it right. My cross-compiler is now out of date (gcc4.2) so I
wasn't able to check if it compiled. Any assistance from arch
maintainers or testers to get this merged would be great.

I was using Coccinelle with the mutex_unlock semantic patch, and it
unconvered this problem. It appears to be a valid missing unlock issue.
This change should correct it by moving the unlock below the label.
This patch is against the mainline kernel.

Some architectures such as Sparc, ARM and MIPS (basically
everything with flush_dcache_page()) need to deal with dcache
aliases by carefully placing pages in both kernel and user maps.
These architectures typically have to use vmalloc_user() for this.
However, on other architectures, vmalloc() is not needed and has
the downsides of being more restricted and slower than regular
allocations.

This patch adds an EX_TABLE entry to mvc{p|s|os} usercopy functions that
may be called with KERNEL_DS. In combination with collaborative memory
management, kernel pages marked as unused may trigger an adressing exception
in the usercopy functions. This fixes an unhandled addressing exception bug
where strncpy_from_user() is used with len > strnlen and KERNEL_DS, crossing
a page boundary to an unused page.

The time a system has been suspended should not show up in any
of the cputime accounting fields. The time of inactivity is definitly
not any form of real cputime nor is it idle time.

The function graph tracer used to have a protection against NMI
while entering a function entry tracing. But this is useless now,
the tracer is reentrant and the ring buffer supports NMI tracing.

When udelay() gets called with a delay that would expire before the
next clock event it reprograms the clock comparator.
When the interrupt happens the clock comparator won't be resetted
therefore the interrupt condition doesn't get cleared.
The result is an endless timer interrupt loop until the next clock
event would expire (stored in lowcore).
So udelay() usually would wait much longer for small delays than it
should.
Fix this by disabling the local tick which makes sure that the clock
comparator will be resetted when a timer interrupt happens.

The s390 version of module_frob_arch_sections allocates additional
syminfos for got and plt offsets. These syminfos are freed on
sucessful module load. If the module fails to load (e.g. missing
dependency when using insmod instead of modprobe) this area is not
freed.
This patch lets module_free free this area. Please note, we have to
set the pointer to NULL since module_free is called several times
from the generic code.

The original driver was written with the KEY() macro defined as (col,
row) instead of (row, col) as defined by the matrix keypad
infrastructure. So the keymap was defined accordingly. Since the
driver that was merged upstream uses the matrix keypad infrastructure,
modify the keymap accordingly.
While we are at it, fix the comments in twl4030.h and define
PERSISTENT_KEY as (r,c) instead of (c, r)
Tested on a RX51 (N900) device.

Lock DPLL5 at 120MHz at boot. The USBHOST 120MHz f-clock and
USBTLL f-clock are the only users of this DPLL, and 120MHz is
is the only recommended rate for these clocks.
With this patch, the 60 MHz ULPI clock is generated correctly.
Tested on an OMAP3430 SDP.

Instead of removing removing the #endif at the end of the file,
the #endif before #else should have been removed.
But we cannot have multiple #else statements as pointed out by
Alistair Buxton <a.j.buxton@gmail.com>. So the fix is to:
- remove the non-multi-omap special handling, as we need to
detect between omap730 and omap850 anyways.
- add the missing #endif back to the end of the file

The 'pwrdm_for_each()' function walks powerdomains with a spinlock
locked, so the the callbacks cannot do anything which may sleep.
This patch introduces a 'pwrdm_for_each_nolock()' helper which does

Currently, only GPIOs in the wakeup domain (GPIOs in bank 0) are
enabled as wakups. This patch also enables GPIOs in the PER
powerdomain (banks 2-6) to be used as possible wakeup sources.
In addition, this patch ensures that all GPIO wakeups can wakeup
the MPU using the PM_MPUGRPSEL_<pwrdm> registers.
NOTE: this doesn't enable the individual GPIOs as wakeups, this simply
enables the per-bank wakeups at the powerdomain level.
This problem was discovered by Mike Chan when preventing the CORE
powerdomain from going into retention/off. When CORE was allowed to
hit retention, GPIO wakeups via IO pad were working fine, but when
CORE remained on, GPIO module-level wakeups were not working properly.
To test, prevent CORE from going inactive/retention/off, thus
preventing the IO chain from being armed:

This ensures that GPIO wakeups happen via module-level wakeups and
not via IO pad.
Tested on 3430SDP using the touchscreen GPIO (gpio 2, in WKUP)
Tested on Zoom2 using the QUART interrup GPIO (gpio 102, in PER)

USBHOST module has 2 fclocks (for HOST1 and HOST2), only one iclock
and only a single bit in the WKST register to indicate a wakeup event.
Because of the single WKST bit, we cannot know whether a wakeup event
was on HOST1 or HOST2, so enable both fclocks before clearing the
wakeup event to ensure both hosts can properly clear the event.

Clearing wakeup sources is now only done when the PRM indicates a
wakeup source interrupt. Since we don't handle any other types of
PRCM interrupts right now, warn if we get any other type of PRCM
interrupt. Either code needs to be added to the PRCM interrupt
handler to react to these, or these other interrupts should be masked
off at init.

PM_WKST register contents should be ANDed with the contents of the
MPUGRPSEL registers. Otherwise the MPU PRCM interrupt handler could
wind up clearing wakeup events meant for the IVA PRCM interrupt
handler. A future revision to this code should be to read a cached
version of MPUGRPSEL from the powerdomain code, since PRM reads are
relatively slow.

There are two scenarios where a race condition could result in a hang
in the prcm_interrupt handler. These are:
1). Waiting for PRM_IRQSTATUS_MPU register to clear.
Bit 0 of the PRM_IRQSTATUS_MPU register indicates that a wake-up event
is pending for the MPU. This bit can only be cleared if the all the
wake-up events latched in the various PM_WKST_x registers have been
cleared. If a wake-up event occurred during the processing of the prcm
interrupt handler, after the corresponding PM_WKST_x register was
checked but before the PRM_IRQSTATUS_MPU was cleared, then the CPU
would be stuck forever waiting for bit 0 in PRM_IRQSTATUS_MPU to be
cleared.
2). Waiting for the PM_WKST_x register to clear.
Some power domains have more than one wake-up source. The PM_WKST_x
registers indicate the source of a wake-up event and need to be cleared
after a wake-up event occurs. When the PM_WKST_x registers are read and
before they are cleared, it is possible that another wake-up event
could occur causing another bit to be set in one of the PM_WKST_x
registers. If this did occur after reading a PM_WKST_x register then
the CPU would miss this event and get stuck forever in a loop waiting
for that PM_WKST_x register to clear.
This patch address the above race conditions that would result in a
hang.

Since mmap_sem can be held at arbitrary times by another thread this
also means that any page faults from kernel mode are invalid if no
exception handler is defined for them, regardless whether mmap_sem is
held at the time of fault.
To easier detect code that can trigger the above error, add a check
also for the case where mmap_sem is acquired. As this has an overhead
make it a VM debug check.

this flag notify that the host physical page we are pointing to from
the spte is write protected, and therefore we cant change its access
to be write unless we run get_user_pages(write = 1).
