direct_gbpages can be force enabled as an early parameter
but not really have taken effect when DEBUG_PAGEALLOC
or KMEMCHECK is enabled. You can also enable direct_gbpages
right now if you have an x86_64 architecture but your CPU
doesn't really have support for this feature. In both cases
PG_LEVEL_1G won't actually be enabled but direct_gbpages is used
in other areas under the assumptions that PG_LEVEL_1G
was set. Fix this by putting together all requirements
which make this feature sensible to enable under, and only
enable both finally flipping on PG_LEVEL_1G and leaving
PG_LEVEL_1G set when this is true.
We only enable this feature then to be possible on sensible
builds defined by the new ENABLE_DIRECT_GBPAGES. If the
CPU has support for it you can either enable this by using
the DIRECT_GBPAGES option or using the early kernel parameter.
If a platform had support for this you can always force disable
it as well.

This effectively unexports set_memory_ro() and set_memory_rw()
functions, and thus reverts:
a03352d2c1 ("x86: export set_memory_ro and set_memory_rw").
They have been introduced for debugging purposes in e1000e, but
no module user is in mainline kernel (anymore?) and we
explicitly do not want modules to use these functions, as they
i.e. protect eBPF (interpreted & JIT'ed) images from malicious
modifications or bugs.
Outside of eBPF scope, I believe also other set_memory_*()
functions should be unexported on x86 for modules.

Now, we have prepared to avoid using debug-pagealloc in boottime. So
introduce new kernel-parameter to disable debug-pagealloc in boottime, and
makes related functions to be disabled in this case.
Only non-intuitive part is change of guard page functions. Because guard
page is effective only if debug-pagealloc is enabled, turning off
according to debug-pagealloc is reasonable thing to do.

"The biggest change is full PAT support from Jürgen Gross:
The x86 architecture offers via the PAT (Page Attribute Table) a
way to specify different caching modes in page table entries. The
PAT MSR contains 8 entries each specifying one of 6 possible cache
modes. A pte references one of those entries via 3 bits:
_PAGE_PAT, _PAGE_PWT and _PAGE_PCD.
The Linux kernel currently supports only 4 different cache modes.
The PAT MSR is set up in a way that the setting of _PAGE_PAT in a
pte doesn't matter: the top 4 entries in the PAT MSR are the same
as the 4 lower entries.
This results in the kernel not supporting e.g. write-through mode.
Especially this cache mode would speed up drivers of video cards
which now have to use uncached accesses.
OTOH some old processors (Pentium) don't support PAT correctly and
the Xen hypervisor has been using a different PAT MSR configuration
for some time now and can't change that as this setting is part of
the ABI.
This patch set abstracts the cache mode from the pte and introduces
tables to translate between cache mode and pte bits (the default
cache mode "write back" is hard-wired to PAT entry 0). The tables
are statically initialized with values being compatible to old
processors and current usage. As soon as the PAT MSR is changed
(or - in case of Xen - is read at boot time) the tables are changed
accordingly. Requests of mappings with special cache modes are
always possible now, in case they are not supported there will be a
fallback to a compatible but slower mode.
Summing it up, this patch set adds the following features:
- capability to support WT and WP cache modes on processors with
full PAT support
- processors with no or uncorrect PAT support are still working as
today, even if WT or WP cache mode are selected by drivers for
some pages
- reduction of Xen special handling regarding cache mode
Another change is a boot speedup on ridiculously large RAM systems,

Introduces lookup_pmd_address() to get the address of the pmd entry
related to a virtual address in the current address space. This
function is needed for support of a virtual mapped sparse p2m list
in xen pv domains, as we need the address of the pmd entry, not the
one of the pte in that case.

pte_pfn() returns a PFN of long (32 bits in 32-PAE), so "long <<
PAGE_SHIFT" will overflow for PFNs above 4GB.
Due to this issue, some Linux 32-PAE distros, running as guests on Hyper-V,
with 5GB memory assigned, can't load the netvsc driver successfully and
hence the synthetic network device can't work (we can use the kernel parameter
mem=3000M to work around the issue).

Add the next level of the pagetable populating function, we handle
chunks around a 1G boundary by mapping them with the lower level
functions - otherwise we use 1G pages for the mappings, thus using as
less amount of pagetable pages as possible.

with a PUD page. The next population level is a dummy macro which will
be removed by the next patch and it is added here to keep the patch
small and easily reviewable but not break bisection, at the same time.

When CONFIG_DEBUG_PAGEALLOC is set page table updates made by
kernel_map_pages() are not made visible (via TLB flush)
immediately if lazy MMU is on. In environments that support lazy
MMU (e.g. Xen) this may lead to fatal page faults, for example,
when zap_pte_range() needs to allocate pages in
__tlb_remove_page() -> tlb_next_batch().

Commit:
a8aed3e075 ("x86/mm/pageattr: Prevent PSE and GLOABL leftovers to confuse pmd/pte_present and pmd_huge")
introduced a valid fix but one location that didn't trigger the bug that
lead to finding those (small) problems, wasn't updated using the
right variable.
The wrong variable was also initialized for no good reason, that
may have been the source of the confusion. Remove the noop
initialization accordingly.
Commit a8aed3e075 also erroneously removed one canon_pgprot pass meant
to clear pmd bitflags not supported in hardware by older CPUs, that
automatically gets corrected by this patch too by applying it to the right
variable in the new location.

So basically we're generating the pte_t * from a struct page and
we're handing it down to the __split_large_page() internal version
which then goes and gets back struct page * from it because it
needs it.
Change the caller to hand down struct page * directly and the
callee can compute the pte_t itself.
Net save is one virt_to_page() call and simpler code. While at
it, make __split_large_page() static.

The lockup happens because _PAGE_GLOBAL and _PAGE_PROTNONE
shares the same bit, and pageattr leaves _PAGE_GLOBAL set on a
kernel pte which is then mistaken as _PAGE_PROTNONE (so
pte_present returns true by mistake and the kernel fault then
gets confused and loops).
With THP the same can happen after we taught pmd_present to
check _PAGE_PROTNONE and _PAGE_PSE in commit
027ef6c878 ("mm: thp: fix pmd_present for
split_huge_page and PROT_NONE with THP"). THP has the same
problem with _PAGE_GLOBAL as the 4k pages, but it also has a
problem with _PAGE_PSE, which must be cleared too.
After the patch is applied copy_user correctly returns -EFAULT
and doesn't lockup anymore.

When memory is removed, the corresponding pagetables should alse be
removed. This patch introduces some common APIs to support vmemmap
pagetable and x86_64 architecture direct mapping pagetable removing.
All pages of virtual mapping in removed memory cannot be freed if some
pages used as PGD/PUD include not only removed memory but also other
memory. So this patch uses the following way to check whether a page
can be freed or not.
1) When removing memory, the page structs of the removed memory are
filled with 0FD.
2) All page structs are filled with 0xFD on PT/PMD, PT/PMD can be
cleared. In this case, the page used as PT/PMD can be freed.
For direct mapping pages, update direct_pages_count[level] when we freed
their pagetables. And do not free the pages again because they were
freed when offlining.
For vmemmap pages, free the pages and their pagetables.
For larger pages, do not split them into smaller ones because there is
no way to know if the larger page has been split. As a result, there is
no way to decide when to split. We deal the larger pages in the
following way:
1) For direct mapped pages, all the pages were freed when they were
offlined. And since menmory offline is done section by section, all
the memory ranges being removed are aligned to PAGE_SIZE. So only need
to deal with unaligned pages when freeing vmemmap pages.
2) For vmemmap pages being used to store page_struct, if part of the
larger page is still in use, just fill the unused part with 0xFD. And
when the whole page is fulfilled with 0xFD, then free the larger page.

This is necessary because __pa() does not work on some kinds of
memory, like vmalloc() or the alloc_remap() areas on 32-bit
NUMA systems. We have some functions to do conversions _like_
this in the vmalloc() code (like vmalloc_to_page()), but they
do not work on sizes other than 4k pages. We would potentially
need to be able to handle all the page sizes that we use for
the kernel linear mapping (4k, 2M, 1G).
In practice, on 32-bit NUMA systems, the percpu areas get stuck
in the alloc_remap() area. Any __pa() call on them will break
and basically return garbage.
This patch introduces a new function slow_virt_to_phys(), which
walks the kernel page tables on x86 and should do precisely
the same logical thing as __pa(), but actually work on a wider
range of memory. It should work on the normal linear mapping,
vmalloc(), kmap(), etc...

We are going to map ram only, so under max_low_pfn_mapped,
between 4g and max_pfn_mapped does not mean mapped at all.

When I made an attempt at separating __pa_symbol and __pa I found that there
were a number of cases where __pa was used on an obvious symbol.
I also caught one non-obvious case as _brk_start and _brk_end are based on the
address of __brk_base which is a C visible symbol.
In mark_rodata_ro I was able to reduce the overhead of kernel symbol to
virtual memory translation by using a combination of __va(__pa_symbol())
instead of page_address(virt_to_page()).

Other than ix86, x86-64 on EFI so far didn't set the
{g,s}et_wallclock accessors to the EFI routines, thus
incorrectly using raw RTC accesses instead.
Simply removing the #ifdef around the respective code isn't
enough, however: While so far early get-time calls were done in
physical mode, this doesn't work properly for x86-64, as virtual
addresses would still need to be set up for all runtime regions
(which wasn't the case on the system I have access to), so
instead the patch moves the call to efi_enter_virtual_mode()
ahead (which in turn allows to drop all code related to calling
efi-get-time in physical mode).
Additionally the earlier calling of efi_set_executable()
requires the CPA code to cope, i.e. during early boot it must be
avoided to call cpa_flush_array(), as the first thing this
function does is a BUG_ON(irqs_disabled()).
Also make the two EFI functions in question here static -
they're not being referenced elsewhere.
History:
This commit was originally merged as bacef661ac ("x86-64/efi:
Use EFI to deal with platform wall clock") but it resulted in some
ASUS machines no longer booting due to a firmware bug, and so was
reverted in f026cfa82f. A pre-emptive fix for the buggy ASUS
firmware was merged in 03a1c254975e ("x86, efi: 1:1 pagetable
mapping for virtual EFI calls") so now this patch can be
reapplied.

This commit has been found to cause serious regressions on a number of
ASUS machines at the least. We probably need to provide a 1:1 map in
addition to the EFI virtual memory map in order for this to work.

When (no)bootmem finish operation, it pass pages to buddy
allocator. Since debug_pagealloc_enabled is not set, we will do
not protect pages, what is not what we want with
CONFIG_DEBUG_PAGEALLOC=y.
To fix remove debug_pagealloc_enabled. That variable was
introduced by commit 12d6f21e "x86: do not PSE on
CONFIG_DEBUG_PAGEALLOC=y" to get more CPA (change page
attribude) code testing. But currently we have CONFIG_CPA_DEBUG,
which test CPA.

It's forbidden to take the page_table_lock with the irq disabled
or if there's contention the IPIs (for tlb flushes) sent with
the page_table_lock held will never run leading to a deadlock.
Nobody takes the pgd_lock from irq context so the _irqsave can be
removed.

Xen want page table pages read only.
But the initial page table (from head_*.S) live in .data or .bss.
That was broken by 64edc8ed5f. There is
absolutely no reason to force these pages RW after they have already
been marked RO.
