To split up APEIs in_nmi() path, the caller needs to always be
in_nmi(). KVM shouldn't have to know about this, pull the RAS plumbing
out into a header file.
Currently guest synchronous external aborts are claimed as RAS
notifications by handle_guest_sea(), which is hidden in the arch codes
mm/fault.c. 32bit gets a dummy declaration in system_misc.h.
There is going to be more of this in the future if/when the kernel
supports the SError-based firmware-first notification mechanism and/or
kernel-first notifications for both synchronous external abort and
SError. Each of these will come with some Kconfig symbols and a
handful of header files.
Create a header file for all this.
This patch gives handle_guest_sea() a 'kvm_' prefix, and moves the
declarations to kvm_ras.h as preparation for a future patch that moves
the ACPI-specific RAS code out of mm/fault.c.

Instead of generating a struct siginfo before calling arm64_notify_die
pass the signal number, tne sicode and the fault address into
arm64_notify_die and have it call force_sig_fault instead of
force_sig_info to let the generic code generate the struct siginfo.
This keeps code passing just the needed information into
siginfo generating code, making it easier to see what
is happening and harder to get wrong. Further by letting
the generic code handle the generation of struct siginfo
it reduces the number of sites generating struct siginfo
making it possible to review them and verify that all
of the fiddly details for a structure passed to userspace
are handled properly.

Currently external aborts are unsupported by the guest abort
handling. Add handling for SEAs so that the host kernel reports
SEAs which occur in the guest kernel.
When an SEA occurs in the guest kernel, the guest exits and is
routed to kvm_handle_guest_abort(). Prior to this patch, a print
message of an unsupported FSC would be printed and nothing else
would happen. With this patch, the code gets routed to the APEI
handling of SEAs in the host kernel to report the SEA information.

When we take a fault that can't be handled, we print out the page table
entries associated with the faulting address. In some cases we currently
print out the wrong entries. For a faulting TTBR1 address, we sometimes
print out TTBR0 table entries instead, and for a faulting TTBR0 address
we sometimes print out TTBR1 table entries. Fix this by choosing the
tables based on the faulting address.

printk_ratelimit() shares the ratelimiting state with other callers what
may lead to scenarios where at the time we want to print out debug
information we already limited, so nothing appears in the dmesg - this
makes exception-trace quite poor helper in debugging.
Additionally, we have imbalance with some messages limited with global
ratelimit state and other messages limited with their private state
defined via pr_*_ratelimited().
To address this inconsistency show_unhandled_signals_ratelimited()
macro is introduced and caller sites are converted to use it.

The documented semantics of flush_cache_all are not possible to provide
for arm64 (short of flushing the entire physical address space by VA),
and there are currently no users; KVM uses VA maintenance exclusively,
cpu_reset is never called, and the only two users outside of arch code
cannot be built for arm64.
While cpu_soft_reset and related functions (which call flush_cache_all)
were thought to be useful for kexec, their current implementations only
serve to mask bugs. For correctness kexec will need to perform
maintenance by VA anyway to account for system caches, line migration,
and other subtleties of the cache architecture. As the extent of this
cache maintenance will be kexec-specific, it should probably live in the
kexec code.
This patch removes flush_cache_all, and related unused components,
preventing further abuse.

Commit 7b6d864b48 (reboot: arm: change reboot_mode to use enum
reboot_mode) changed the way reboot is handled on arm, which has a
direct impact on arm64 as we share the reset driver on the VE platform.
The obvious fix is to move arm64 to use the same infrastructure.
