Update device_add_disk() to take an 'groups' argument so that
individual drivers can register a device with additional sysfs
attributes.
This avoids race condition the driver would otherwise have if these
groups were to be created with sysfs_add_groups().

In the quest to remove all stack VLA usage from the kernel[1], this
replaces struct crypto_skcipher and SKCIPHER_REQUEST_ON_STACK() usage
with struct crypto_sync_skcipher and SYNC_SKCIPHER_REQUEST_ON_STACK(),
which uses a fixed stack size.

Commit a46b53672b ("xen/blkfront: cleanup stale persistent grants")
added support for purging persistent grants when they are not in use. As
part of the purge, the grants were removed from the grant buffer, This
eventually causes the buffer to become empty, with BUG_ON triggered in
get_free_grant(). This can be observed even on an idle system, within
20-30 minutes.
We should keep the grants in the buffer when purging, and only free the
grant ref.

Prior patches ensured that all bios are now associated with some blkg.
This now makes bio->bi_css unnecessary as blkg maintains a reference to
the blkcg already.
This patch removes the field bi_css and transfers corresponding uses to

The final field of a floppy_struct is the field "name", which is a pointer
to a string in kernel memory. The kernel pointer should not be copied to
user memory. The FDGETPRM ioctl copies a floppy_struct to user memory,
including this "name" field. This pointer cannot be used by the user
and it will leak a kernel address to user-space, which will reveal the
location of kernel code and data and undermine KASLR protection.
Model this code after the compat ioctl which copies the returned data
to a previously cleared temporary structure on the stack (excluding the
name pointer) and copy out to userspace from there. As we already have
an inparam union with an appropriate member and that memory is already
cleared even for read only calls make use of that as a temporary store.

The supported added for zones in null_blk seem to assume that only rq
based operation is possible. But this depends on the queue_mode setting,
if this is set to 0, then cmd->bio is what we need to be operating on.
Right now any attempt to load null_blk with queue_mode=0 will
insta-crash, since cmd->rq is NULL and null_handle_cmd() assumes it to
always be set.
Make the zoned code deal with bio's instead, or pass in the
appropriate sector/nr_sectors instead.

In preparing to remove all stack VLA usage from the kernel[1], this
removes the discouraged use of AHASH_REQUEST_ON_STACK in favor of
the smaller SHASH_DESC_ON_STACK by converting from ahash-wrapped-shash
to direct shash. By removing a layer of indirection this both improves
performance and reduces stack usage. The stack allocation will be made
a fixed size in a later patch to the crypto subsystem.
The bulk of the lines in this change are simple s/ahash/shash/, but the
main logic differences are in drbd_csum_ee() and drbd_csum_bio(), which
externalizes the page walking with k(un)map_atomic() instead of using
scattergather.

If parent_get class method is not supported by the OSDs, fall back to
the legacy class method and assume that the parent is in the default
(i.e. "") namespace. The "use the child's image namespace" workaround
is no longer needed because creating images within namespaces will
require parent_get aware OSDs.

In preparation for the new parent_get and parent_overlap_get class
methods, factor out the fetching and decoding of parent data.
As a side effect, we now decode all four fields in the "no parent"
case.

syzbot reports a divide-by-zero off the NBD_SET_BLKSIZE ioctl.
We need proper validation of the input here. Not just if it's
zero, but also if the value is a power-of-2 and in a valid
range. Add that.

Add a periodic cleanup function to remove old persistent grants which
are no longer in use on the backend side. This avoids starvation in
case there are lots of persistent grants for a device which no longer
is involved in I/O business.

Persistent grants are allocated until a threshold per ring is being
reached. Those grants won't be freed until the ring is being destroyed
meaning there will be resources kept busy which might no longer be
used.
Instead of freeing only persistent grants until the threshold is
reached add a timestamp and remove all persistent grants not having
been in use for a minute.

The call to strlcpy in backing_dev_store is incorrect. It should take
the size of the destination buffer instead of the size of the source
buffer. Additionally, ignore the newline character (\n) when reading
the new file_name buffer. This makes it possible to set the backing_dev
as follows:
echo /dev/sdX > /sys/block/zram0/backing_dev
The reason it worked before was the fact that strlcpy() copies 'len - 1'
bytes, which is strlen(buf) - 1 in our case, so it accidentally didn't
copy the trailing new line symbol. Which also means that "echo -n
/dev/sdX" most likely was broken.

If zram supports writeback feature, it's no longer a
BD_CAP_SYNCHRONOUS_IO device beause zram does asynchronous IO operations
for incompressible pages.
Do not pretend to be synchronous IO device. It makes the system very
sluggish due to waiting for IO completion from upper layers.
Furthermore, it causes a user-after-free problem because swap thinks the
opearion is done when the IO functions returns so it can free the page
(e.g., lock_page_or_retry and goto out_release in do_swap_page) but in
fact, IO is asynchronous so the driver could access a just freed page
afterward.
This patch fixes the problem.

There is a lot of needless struct request_sense usage in the CDROM
code. These can all be struct scsi_sense_hdr instead, to avoid any
confusion over their respective structure sizes. This patch is a lot
of noise changing "sense" to "sshdr", but the final code is more
readable to distinguish between "sense" meaning "struct request_sense"
and "sshdr" meaning "struct scsi_sense_hdr".

The request mtime field is used all over ceph, and is currently
represented as a 'timespec' structure in Linux. This changes it to
timespec64 to allow times beyond 2038, modifying all users at the
same time.

The BTF conflicts were simple overlapping changes.
The virtio_net conflict was an overlap of a fix of statistics counter,
happening alongisde a move over to a bonafide statistics structure
rather than counting value on the stack.

Add and use a new op_stat_group() function for indexing partition stat
fields rather than indexing them by rq_data_dir() or bio_data_dir().
This function works similarly to op_is_sync() in that it takes the
request::cmd_flags or bio::bi_opf flags and determines which stats
should et updated.
In addition, the second parameter to generic_start_io_acct() and
generic_end_io_acct() is now a REQ_OP rather than simply a read or
write bit and it uses op_stat_group() on the parameter to determine
the stat group.
Note that the partition in_flight counts are not part of the per-cpu
statistics and as such are not indexed via this function. It's now
indexed by op_is_write().

Add a part_stat_read_accum macro to genhd.h to read and sum across
field entries. For example to sum up the number read and write
sectors completed. In addition to being ar reasonable cleanup by
itself this will make it easier to add new stat fields in the future.

c11f0c0b5b ("block/mm: make bdev_ops->rw_page() take a bool for
read/write") replaced @op with boolean @is_write, which limited the
amount of information going into ->rw_page() and more importantly
page_endio(), which removed the need to expose block internals to mm.
Unfortunately, we want to track discards separately and @is_write
isn't enough information. This patch updates bdev_ops->rw_page() to
take REQ_OP instead but leaves page_endio() to take bool @is_write.
This allows the block part of operations to have enough information
while not leaking it to mm.

If the server or network is misbehaving and we get an unexpected reply
we can sometimes miss the request not being started and wait on a
request and never get a response, or even double complete the same
request. Fix this by replacing the send_complete completion with just a
per command lock. Add a per command cookie as well so that we can know
if we're getting a double completion for a previous event. Also check
to make sure we dont have REQUEUED set as that means we raced with the
timeout handler and need to just let the retry occur.

We can race with the snd timeout and the per-request timeout and end up
requeuing the same request twice. We can't use the send_complete
completion to tell if everything is ok because we hold the tx_lock
during send, so the timeout stuff will block waiting to mark the socket
dead, and we could be marked complete and still requeue. Instead add a
flag to the socket so we know whether we've been requeued yet.

Adds support for exposing a null_blk device through the zone device
interface.
The interface is managed with the parameters zoned and zone_size.
If zoned is set, the null_blk instance registers as a zoned block
device. The zone_size parameter defines how big each zone will be.

Currently mbps knob could only be set once before switching power knob to
on, after power knob has been set at least once, there is no way to set
mbps knob again due to -EBUSY.
As nullb is mainly used for testing, in order to make it flexible, this
removes the flag NULLB_DEV_FL_CONFIGURED so that mbps knob can be reset
when power knob is off, e.g.

echo 40 > /config/nullb/a/mbps
echo 1 > /config/nullb/a/power
So does other knobs under /config/nullb/a.

We have
struct drbd_requests { ... struct bio *private_bio; ... }
to hold a bio clone for local submission.
On local IO completion, we put that bio, and in case we want to use the
result later, we overload that member to hold the ERR_PTR() of the
completion result,
Which, before v4.3, used to be the passed in "int error",
so we could first bio_put(), then assign.
v4.3-rc1~100^2~21 4246a0b63b block: add a bi_error field to struct bio
changed that:
bio_put(req->private_bio);
- req->private_bio = ERR_PTR(error);
+ req->private_bio = ERR_PTR(bio->bi_error);
Which introduces an access after free,
because it was non obvious that req->private_bio == bio.
Impact of that was mostly unnoticable, because we only use that value
in a multiple-failure case, and even then map any "unexpected" error
code to EIO, so worst case we could potentially mask a more specific
error with EIO in a multiple failure case.
Unless the pointed to memory region was unmapped, as is the case with

While __atomic_add_unless() was originally intended as a building-block
for atomic_add_unless(), it's now used in a number of places around the
kernel. It's the only common atomic operation named __atomic*(), rather
than atomic_*(), and for consistency it would be better named
atomic_fetch_add_unless().
This lack of consistency is slightly confusing, and gets in the way of
scripting atomics. Given that, let's clean things up and promote it to
an official part of the atomics API, in the form of
atomic_fetch_add_unless().
This patch converts definitions and invocations over to the new name,
including the instrumented version, using the following script:

If NBD_DISCONNECT_ON_CLOSE is set on a device, then the driver will
issue a disconnect from nbd_release if the device has no remaining
bdev->bd_openers.

blk_mq_complete_request can only be called for blk-mq drivers, but when
removing the BLK_EH_HANDLED return value, two legacy request timeout
methods incorrectly got switched to call blk_mq_complete_request.
Call __blk_complete_request instead to reinstance the previous behavior.
For that __blk_complete_request needs to be exported.

Admin can leverage this information to catch cold|incompressible pages
of process with *pagemap* once part of heaps are swapped out.
I used the feature a few years ago to find memory hoggers in userspace
to notify them what memory they have wasted without touch for a long
time. With it, they could reduce unnecessary memory space. However, at
that time, I hacked up zram for the feature but now I need the feature
again so I decided it would be better to upstream rather than keeping it
alone. I hope I submit the userspace tool to use the feature soon.

Mark incompressible pages so that we could investigate who is the owner
of the incompressible pages once the page is swapped out via using
upcoming zram memory tracker feature.
With it, we could prevent such pages to be swapped out by using mlock.
Otherwise we might remove them.
This patch exposes new stat for huge pages via mm_stat.

swapped out.

Technically we should be able to get away with 0 as the
discard_alignment, but there's no way currently for the protocol to
indicate different alignments, and in real life most disks have
discard_alignment == discard_granularity. Just set our alignment to our
blocksize to make sure discards will actually work properly with 4k
drives.

There is a problem if we are going to unmap a rbd device and the
watch_dwork is going to queue delayed work for watch:

When IOCB_FLAG_IOPRIO is set on the iocb aio_flags field, then we set the
newly added kiocb ki_ioprio field to the value in the iocb aio_reqprio field.

When a userspace client requests a NBD device be disconnected, the
DISCONNECT_REQUESTED flag is set. While this flag is set, the driver
will not inform userspace when a connection is closed.
Unfortunately the flag was never cleared, so once a disconnect was
requested the driver would thereafter never tell userspace about a
closed connection. Thus when connections failed due to timeout, no
attempt to reconnect was made and eventually the device would fail.
Fix by clearing the DISCONNECT_REQUESTED flag (and setting the
DISCONNECTED flag) once all connections are closed.

When a loop block device encounters a writeback error, that error will
get propagated to the bd_inode's wb_err field. If we then detach the
backing file from it, attach another and fsync it, we'll get back the
writeback error that we had from the previous backing file.
This is a bit of a grey area as POSIX doesn't cover loop devices, but it
is somewhat counterintuitive.
If we detach a backing file from the loopdev while there are still
unreported errors, take it as a sign that we're no longer interested in
the previous file, and clear out the wb_err in the loop blockdev.

bd_invalidated is kind of a pain wrt partitions as it really only
triggers the partition rescan if it is set after bd_ops->open() runs, so
setting it when we reset the device isn't useful. We also sporadically
would still have partitions left over in some disconnect cases, so fix
this by always setting bd_invalidated on open if there's no
configuration or if we've had a disconnect action happen, that way the
partition table gets invalidated and rescanned properly.

This is what the ioctl based nbd disconnect does as well. Without this
the device will just sit there and wait for the connection to go away
(or IO to occur) before the device gets torn down. Instead clear
everything up on our end so the configuration goes away as quickly as
possible.

When we stopped relying on the bdev everywhere I broke updating the
block device size on the fly, which ceph relies on. We can't just do
set_capacity, we also have to do bd_set_size so things like parted will
notice the device size change.

I messed up changing the size of an NBD device while it was connected by
not actually updating the device or doing the uevent. Fix this by
updating everything if we're connected and we change the size.

Found a bug (with ASAN) where we were passing a bio to bio_copy_data()
with bi_next not NULL, when it should have been - a driver had left
bi_next set to something after calling bio_endio().
Since the normal case is only copying single bios, split out
bio_list_copy_data() to avoid more bugs like this in the future.

Reading to the end of a 720K disk results in an IO error instead of EOF
because the block layer thinks the disk has 2880 sectors. (Partly this
is a result of inverted logic of the ONEMEG_MEDIA bit that's now fixed.)
Initialize the density and head count in swim_add_floppy() to agree
with the device size passed to set_capacity() during drive probe.
Call set_capacity() again upon device open, after refreshing the density
and head count values.

The SWIM chip is compatible with GCR-mode Sony 400K/800K drives but
this driver only supports MFM mode. Therefore only Sony FDHD drives
are supported. Skip incompatible drives.

The Sony drive status bits use active-low logic. The swim_readbit()
function converts that to 'C' logic for readability. Hence, the
sense of the names of the status bit macros should not be inverted.
Mostly they are correct. However, the TWOMEG_DRIVE, MFM_MODE and
TWOMEG_MEDIA macros have inverted sense (like MkLinux). Fix this
inconsistency and make the following patches less confusing.
The same problem affects swim3.c so fix that too.
No functional change.
The FDHD drive status bits are documented in sonydriv.cpp from MAME
and in swimiii.h from MkLinux.

The 'eject' shell command may send various different ioctl commands.
This leads to error messages on the console even though the FDEJECT
ioctl succeeds.

Add an option to turn off discard and write zeroes offload support to
avoid deprovisioning a fully provisioned image. When enabled, discard
requests will fail with -EOPNOTSUPP, write zeroes requests will fall
back to manually zeroing.

currently, the rbd_wait_state_locked() will wait forever if we
can't get our state locked. Example:
rbd map --exclusive test1 --> /dev/rbd0
rbd map test1 --> /dev/rbd1
dd if=/dev/zero of=/dev/rbd1 bs=1M count=1 --> IO blocked
To avoid this problem, this patch introduce a timeout design
in rbd_wait_state_locked(). Then rbd_wait_state_locked() will
return error when we reach a timeout.
This patch allow user to set the lock_timeout in rbd mapping.

We ran into an issue with loop and btrfs, where btrfs would complain about
checksum errors. It turns out that is because we don't handle short reads
at all, we just zero fill the remainder. Worse than that, we don't handle
the filling properly, which results in loop trying to advance a single
bio by much more than its size, since it doesn't take chaining into
account.
Handle short reads appropriately, by simply retrying at the new correct
offset. End the remainder of the request with EIO, if we get a 0 read.

Commit 2d1d4c1e59 made loop_get_status() drop lo_ctx_mutex before
returning, but the loop_get_status_old(), loop_get_status64(), and
loop_get_status_compat() wrappers don't call loop_get_status() if the
passed argument is NULL. The callers expect that the lock is dropped, so
make sure we drop it in that case, too.

Remove ZRAM's enforced "huge object" value and use zsmalloc huge-class
watermark instead, which makes more sense.
TEST
- I used a 1G zram device, LZO compression back-end, original
data set size was 444MB. Looking at zsmalloc classes stats the

As we can see, we reduced the number of objects stored in class-4096,
because a huge number of objects which we previously forcibly stored in
class-4096 now stored in non-huge class-3264. This results in lower
memory consumption:
- zsmalloc now uses 47424 physical pages, which is less than 48787 pages
zsmalloc used before.
- objects that we store in class-3264 share zspages. That's why overall
the number of pages that both class-4096 and class-3264 consumed went
down from 10924 to 9564.

As part of the effort to remove VLAs from the kernel[1], this moves
the literal values into the stack array calculation instead of using a

Currently we request the latest osdmap only if ceph_pg_poolid_by_name()
fails with -ENOENT. This is effective with newly created pools, but we
also want to avoid attempting to map from pools that were recently
deleted and report "pool does not exist" instead. (Such an attempt
eventually fails in the OSD client after map check code kicks in, but
the error message is confusing.)
Request the latest osdmap unconditionally after bumping a ref on an
existing client in rbd_client_find().

If the layout is "fancy", we need to be able to rearrange the provided
bio_vecs in stripe unit chunks to make it possible for the messenger to
read/write directly from/to the provided data buffer, without employing
a temporary data buffer for assembling the result.
Higher level bio_vec arrays are generally immutable, so this requires
copying into a private array. Only the bio_vecs themselves are shuffled
around, not the actual data. OWN_BVECS doesn't own any pages.

rbd_parent_request_create() takes a ref on obj_req for child_img_req.
There is no point in doing that because child_img_req is created on
behalf of obj_req -- obj_req is the initiator and can't be completed
before child_img_req.
Open-code the rest of rbd_parent_request_create() and remove it along
with rbd_parent_request_destroy().

A whole-object layered discard is implemented as a truncate rather
than a delete: a dummy object is needed to prevent the CoW machinery
from kicking in. However, a truncate on a non-existent object is
a no-op. If the object doesn't exist in HEAD, a discard request is
effectively ignored, which violates our "discard zeroes data" promise
and breaks REQ_OP_WRITE_ZEROES implementation.
A non-exclusive create on an existing object is also a no-op, so the
fix is to do a compound create+truncate instead.

In preparation for rbd "fancy" striping, replace obj_req->img_offset
with obj_req->img_extents. A single starting offset isn't sufficient
because we want only one OSD request per object and will merge adjacent
object extents in ceph_file_to_extents(). The final object extent may
map into multiple different byte ranges in the image.

Do away with partial request completions and all the associated
complexity. Individual object requests no longer need to be completed
in order -- when the last one becomes ready, we complete the entire
higher level request all at once.
This also wraps up the conversion to a state machine model and
eliminates the recursion described in commit 6d69bb536b ("rbd:
prevent kernel stack blow up on rbd map").

No need to pass rbd_dev and op_type to rbd_osd_req_create(): there are
no standalone (!IMG_DATA) object requests anymore and osd_req->r_flags
can be set in rbd_osd_req_format_{read,write}().

The notable changes are:
- instead of explicitly stat'ing the object to see if it exists before
issuing the write, send the write optimistically along with the stat
in a single OSD request
- zero copyup optimization
- all object requests are associated with an image request and have
a valid ->img_request pointer; there are no standalone (!IMG_DATA)
object requests anymore
- code is structured as a state machine (vs a bunch of callbacks with
implicit state)

In preparation for rbd "fancy" striping which requires bio_vec arrays,
wire up BVECS data type and kill off PAGES data type. There is nothing
wrong with using page vectors for copyup requests -- it's just less
iterator boilerplate code to write for the new striping framework.

The reason we clone bios is to be able to give each object request
(and consequently each ceph_osd_data/ceph_msg_data item) its own
pointer to a (list of) bio(s). The messenger then initializes its
cursor with cloned bio's ->bi_iter, so it knows where to start reading
from/writing to. That's all the cloned bios are used for: to determine
each object request's starting position in the provided data buffer.
Introduce ceph_bio_iter to do exactly that -- store position within bio
list (i.e. pointer to bio) + position within that bio (i.e. bvec_iter).

Even after the previous patch to drop lo_ctl_mutex while calling
vfs_getattr(), there are other cases where we can end up sleeping for a
long time while holding lo_ctl_mutex. Let's avoid the uninterruptible
sleep from the ioctls.

We hit an issue where a loop device on NFS was stuck in
loop_get_status() doing vfs_getattr() after the NFS server died, which
caused a pile-up of uninterruptible processes waiting on lo_ctl_mutex.
There's no reason to hold this lock while we wait on the filesystem;
let's drop it so that other processes can do their thing. We need to
grab a reference on lo_backing_file while we use it, and we can get rid
of the check on lo_device, which has been unnecessary since commit
a34c0ae9ebd6 ("[PATCH] loop: remove the bio remapping capability") in
the linux-history tree.

It happens often while I'm preparing a patch for a block driver that
I'm wondering: is a definition of SECTOR_SIZE and/or SECTOR_SHIFT
available for this driver? Do I have to introduce definitions of these
constants before I can use these constants? To avoid this confusion,
move the existing definitions of SECTOR_SIZE and SECTOR_SHIFT into the
<linux/blkdev.h> header file such that these become available for all
block drivers. Make the SECTOR_SIZE definition in the uapi msdos_fs.h
header file conditional to avoid that including that header file after
<linux/blkdev.h> causes the compiler to complain about a SECTOR_SIZE
redefinition.
Note: the SECTOR_SIZE / SECTOR_SHIFT / SECTOR_BITS definitions have
not been removed from uapi header files nor from NAND drivers in
which these constants are used for another purpose than converting
block layer offsets and sizes into a number of sectors.

The following commit:
commit aa4d86163e ("block: loop: switch to VFS ITER_BVEC")
replaced __do_lo_send_write(), which used ITER_KVEC iterators, with
lo_write_bvec() which uses ITER_BVEC iterators. In this change, though,
the WRITE flag was lost:
- iov_iter_kvec(&from, ITER_KVEC | WRITE, &kvec, 1, len);
+ iov_iter_bvec(&i, ITER_BVEC, bvec, 1, bvec->bv_len);
This flag is necessary for the DAX case because we make decisions based on
whether or not the iterator is a READ or a WRITE in dax_iomap_actor() and
in dax_iomap_rw().
We end up going through this path in configurations where we combine a PMEM
device with 4k sectors, a loopback device and DAX. The consequence of this
missed flag is that what we intend as a write actually turns into a read in
the DAX code, so no data is ever written.
The very simplest test case is to create a loopback device and try and
write a small string to it, then hexdump a few bytes of the device to see
if the write took. Without this patch you read back all zeros, with this
you read back the string you wrote.
For XFS this causes us to fail or panic during the following xfstests:
xfs/074 xfs/078 xfs/216 xfs/217 xfs/250
For ext4 we have a similar issue where writes never happen, but we don't
currently have any xfstests that use loopback and show this issue.
Fix this by restoring the WRITE flag argument to iov_iter_bvec(). This
causes the xfstests to all pass.

This task tried to open the cdrom device, the sr_block_open function
acquired the global sr_mutex lock. The call to check_disk_change()
then saw an event flag indicating a possible media change and tried
to flush any cached data for the device.
As part of the flush, it tried to acquire the super_block->s_umount
lock associated with the cdrom device.
This was the same super_block as created and locked by the previous task.
The first task acquires the s_umount lock and then the sr_mutex_lock;
the second task acquires the sr_mutex_lock and then the s_umount lock.
This patch fixes the issue by moving check_disk_change() out of
cdrom_open() and let the caller take care of it.
