Bruce Allan 464c85e37e e1000e: conditionally restart autoneg on 82577/8/9 when setting LPLU state
When setting the Low Power Link Up (LPLU, a.k.a. reverse auto-negotiation)
on 82577/8278/82579, do not restart auto-negotiation if reset of the Phy is
blocked by the Manageability Engine.

Bruce Allan 7f1557e142 e1000e: increase Rx PBA to prevent dropping received packets on 82566/82567
During bi-directional stress on some 82566/82567 devices, some received
packets were dropped.  Increasing the Receive Packet Buffer Allocation
resolves this.

Bruce Allan c077a9065b e1000e: ICHx/PCHx LOMs should use LPLU setting in NVM when going to Sx
When going to Sx with an ICHx/PCH device, the default Low Power Link Up
(LPLU, a.k.a. reverse auto-negotiation) behavior should be whatever is set
in the NVM.  However, the function e1000_suspend_workarounds_ich8lan()
called when going to Sx always enabled LPLU in all power states.

Bruce Allan 90b82984a4 e1000e: update workaround for 82579 intermittently disabled during S0->Sx
The workaround which toggles the LANPHYPC (LAN PHY Power Control) value bit
to force the MAC-Phy interconnect into PCIe mode from SMBus mode during
driver load and resume should always be done except if PHY resets are
blocked by the Manageability Engine (ME).  Previously, the toggle was done
only if PHY resets are blocked and the ME was disabled.

The rest of the patch is just indentation changes as a consequence of the
updated workaround.

Bruce Allan 79d4e9087a e1000e: disable Early Receive DMA on ICH LOMs
Internal stress testing with jumbo frames shows the reliability of ICH9 and
ICH10D devices is improved in certain corner cases by disabling the Early
Receive feature. To reduce the performance impact caused by disabling this
feature, the packet buffer sizes and relevant flow control settings are
modified accordingly.

Ben Hutchings f9c762500a sfc: Make all MAC statistics consistently 64 bits wide
Currently we use type u64 for byte counts, which can very quickly
exceed 2^32, and unsigned long for packet counts, which do not.  But
it can still take only 20-something minutes to send or receive 2^32
packets, and not all tools properly handle overflow even if they
sample more often than this.

The MAC statistics are all updated synchronously, so it costs very
little to make them all 64-bit regardless of native word size.

Ben Hutchings 55c5e0f85d sfc: Add hwmon driver for boards using SFC9000-family controllers
The SFC9000-family controllers have firmware to manage all board
peripherals including temperature, heat sink continuity and voltage
sensors.  The firmware reports sensor alarms, which we log, and
will shut down the board if necessary.

Some users may want to monitor their boards more closely, so add an
hwmon driver that exposes all sensors reported by the firmware.  Move
efx_mcdi_sensor_event() into the new file so it can share the array of
sensor labels with the hwmon driver.

Ben Hutchings 1646a6f352 sfc: Clean up test interrupt handling
Interrupts are normally generated by the event queues, moderated by
timers.  However, they may also be triggered by detection of a 'fatal'
error condition (e.g. memory parity error) or by the host writing to
certain CSR fields as part of a self-test.

The IRQ level/index used for these on Falcon rev B0 and Siena is set
by the KER_INT_LEVE_SEL field and cached by the driver in
efx_nic::fatal_irq_level.  Since this value is also relevant to
self-tests rename the field to just 'irq_level'.

Avoid unnecessary cache traffic by using a per-channel 'last_irq_cpu'
field and only writing to the per-controller field when the interrupt
matches efx_nic::irq_level.  Remove the volatile qualifier and use
ACCESS_ONCE in the places we read these fields.

Ben Hutchings f70d184734 Partly revert "sfc: Handle serious errors in exactly one interrupt handler"
This reverts commit 6369545945 in
drivers/net/ethernet/sfc/falcon.c.

Unlike the INT_ISR0 register on later controller revisions, the
NET_IVEC_INT_Q bits written to memory are only ever set for
interrupting event queues, not for any other interrupt sources.

By definition there can only be one legacy interrupt handler per
function, so there is no need to worry about detecting a fatal
interrupt more than once.

Ben Hutchings 0fb53faa2e sfc: Remove dependence on NAPI polling in efx_test_eventq_irq()
We cannot safely assume that the NAPI handler will complete within the
20 ms that we allow for the event self-test.  The handler may be
deferred for longer than this, particularly on realtime kernels.

Instead, check whether either an event has been handled or (as in the
old failure path) whether an interrupt has been received and an event
has been delivered but not yet handled.  Use napi_disable() to
synchronize with the NAPI handler before checking, since it will
clear events before updating eventq_read_ptr.

Remove the test result chan.N.eventq.poll, since it is not an error
if the NAPI handler does not run during the test.

Ben Hutchings cc180b69c0 sfc: Correct interrupt timer quantum for Siena (normal and turbo mode)
We currently assume that the timer quantum for Siena is 5 us, the same
as for Falcon.  This is not correct; timer ticks are generated on a
rota which takes a minimum of 768 cycles (each event delivery or other
timer change will delay it by 3 cycles).  The timer quantum should be
6.144 or 3.072 us depending on whether turbo mode is active.

Replace EFX_IRQ_MOD_RESOLUTION with a timer_quantum_ns field in struct
efx_nic, initialised by the efx_nic_type::probe function.

While we're at it, replace EFX_IRQ_MOD_MAX with a timer_period_max
field in struct efx_nic_type.

Ben Hutchings 5f3f9d6c44 sfc: Consistently test DEBUG macro, not EFX_ENABLE_DEBUG
The netif_dbg() macro is defined in <linux/netdevice.h>.  If the DEBUG
macro is defined, it logs a message at 'debug' level, otherwise it
does nothing.

In net_driver.h we define DEBUG if EFX_ENABLE_DEBUG is defined, but
this is too late for those source files that already got a
definition of netif_dbg() by including <linux/netdevice.h>

Get rid of EFX_ENABLE_DEBUG, and only define and test DEBUG.

In mtd.c, we do not use DEBUG as a condition flag but are forced to
use the DEBUG macro-function from <linux/mtd/mtd.h>.  Undefine DEBUG
before including it.

Ben Hutchings 30b81cda95 sfc: Remove efx_nic_type::push_multicast_hash operation
Both implementations of efx_nic_type::reconfigure_mac operation
push the multicast hash filter to the hardware.  It is therefore
redundant to call efx_nic_type::push_multicast_hash as well.

efx_mcdi_mac_reconfigure() also uses this operation, but the
implementation for Siena just uses MCDI anyway.  Merge that into
efx_mcdi_mac_reconfigure().

Ben Hutchings 710b208dc2 sfc: Merge efx_mac_operations into efx_nic_type
No NICs need to switch efx_mac_operations at run-time, and the MAC
operations are fairly closely bound to NIC types.

Move efx_mac_operations::reconfigure to efx_nic_type::reconfigure_mac
and efx_mac_operations::check_fault fo efx_nic_type::check_mac_fault.
Change callers to call through efx->type or directly if the NIC type
is known.

Remove efx_mac_operations::update_stats.  The implementations for
Falcon used to fetch MAC statistics synchronously and this was used by
efx_register_netdev() to clear statistics after running self-tests.
However, it now only converts statistics that have already been
fetched (and that only for Falcon), and the call from
efx_register_netdev() has no effect.

Ben Hutchings 1cb345220f sfc: Hold efx_nic::stats_lock while reading efx_nic::mac_stats
efx_nic::stats_lock is used to serialise stats updates, but each
reader was dropping it before it finished reading efx_nic::mac_stats.

If there were concurrent stats reads using procfs, or one using procfs
and one using ethtool, an update could race with a read.  On a 32-bit
system, the reader could see word-tearing of 64-bit stats (32 bits of
the old value and 32 bits of the new).

Ben Hutchings 3f713bf4dd sfc: Make handling of MC reboot more reliable
When the MC reboots, either as part of a firmware upgrade or due to a
bug, it attempts to complete (with an error) any requests that were
outstanding before the reboot.  Since there is an inherent race
condition in checking this, it will also write to a status word in
shared memory.

If we look at each of these separately, we may detect each reboot
twice, resulting in a spurious command failure after a firmware
upgrade or frustrating recovery from a firmware bug.  Instead, if a
request completion indicates a reboot, we must poll and clear the
status word.

This bug was previously masked by use of an incorrect address for the
status word.  Fix that, using the definition now included in
mcdi_pcol.h.

Ben Hutchings e332bcb3d1 sfc: Remove fallback for invalid permanent MAC address
By the time we look at the MAC address in efx_probe_port(), either the
driver or the firmware has already validated the board configuration.
The possibility of having an invalid MAC address just isn't worth
considering.  It certainly isn't worth having a compile-time option
for this.

Eric Dumazet bb349bb4b1 be2net: allocate more headroom in incoming skbs
Allocation of 64 bytes in skb headroom is not enough if we have to pull
ethernet + ipv6 + tcp headers, and/or extra tunneling header.

Its currently not noticed because netdev_alloc_skb_ip_align(64) give us
more room, thanks to power-of-two kmalloc() roundups.

Make sure we ask for 128 bytes so that side effects of upcoming patches
from Ian Campbell dont decrease benet rx performance, because of extra
skb head reallocations.

Ariel Elior 95c6c6165e bnx2x: Recovery flow bug fixes
1. Sample mcp pulse and mcp sequence in nic load instead of in init_one
as they may change by the time we want to use them.

2. Allow cnic to access device during nic load (by adding a new "LOADING" state
to recovery flow). This prevents the unnecessary cnic timeout which resulted
by cnic attempting to access because nic is loading, but being blocked because
of the Recovery state.

3. Issue 'fake' driver load command to mcp when last driver unloads to prevent
mcp from taking ownership. When recovery is complete unload fake driver to
allow mcp to initialize the hardware before first driver loads.

Ariel Elior 889b9af34f bnx2x: Track active PFs with bitmap
The recovery register (to which a hardware lock has been added in previous
patch) is used amongst other things to track the active PFs. The old
implementation which used a per path counter is not viable in a virtualized
environment where a pf may increment the counter and then have the kernel
crash around it preventing the counter from ever reaching zero.
In the new implementation the scenario described will result in the PF timing
out against the mcp, which will clear the PF's bit in the bitmask allowing
recovery process to proceed.

Ariel Elior f16da43b5d bnx2x: Lock PF-common resources
Use hardware locks to protect resources common to several Physical Functions. In
a virtualized environment the RTNL lock only protects a PF's driver against
the PFs sharing it's VMs with regard to device resources. Other PFs may reside
in other VMs under other OSs, and are not subject to the lock. Such resources
which were previously protected implicitly by the RTNL lock must now be
protected explicitly with dedicated HW locks.

Ariel Elior d1e2d9660e bnx2x: Loaded Firmware Version Validation
In a virtualized environment it is possible for a loading driver to discover
that Firmware is already loaded to the device, and that this FW does not match
its own. This can happen for example if different Physical Functions are
Assigned to different VMs in which different driver versions are loaded. The
code in this patch ensures that only drivers with matching FW are loaded over
the device, and that in the case described above where the Firmware version
doesn't match the driver load is aborted.

Ariel Elior 127a425e8f bnx2x: Removing indirect register access
In virtualized environments indirect access to the device may not be supported
(depending on the Hypervisor type). Indirect device access was used since in
some harware contexts (i.e. certain chipset and BIOS) every access the driver
makes across the pci is followed by a BIOS initiated Zero Length Read to the
same address. When accessing widebus registers this zero length read corrupts
the serialization of the read/write sequence resulting with errors. To avoid
this problem widebus registers are always accessed via the DMAE or the indirect
interface. However, the 57712x and 578xx devices intercept the zero length read
and so using the indirect interface with these devices is not necessary. Since
PDA is only supported for 57712x and 578xx the indirect access to device was
restricted to 57710 and 57711x.

Ariel Elior 8d7b02783b bnx2x: Support Queue Per Cos in 5771xx devices
Enable the use of up to three hardware queues for transmission. The queues
are always dequed round robin (i.e. strict priority, PFC and ETS are not
supported). This does allow the allocation of a seperate HW queue for low
volume, high priority traffic which will be serviced more promptly.

Bruce Allan 6a92f732f5 e1000e: use default settings for Tx Inter Packet Gap timer
Use the default hardware values for TIPG except for 80003es2lan(*).  The
code that is removed in this patch is either unnecessarily writing the TIPG
register with the hardware default values for some devices (82571/2/3/4) or
writing the wrong value for others (ICH/PCH LOMs).  The only change in
functionality is setting the correct default TIPG for the latter devices.

(*) The correct value for 80003es2lan is already set properly in
e1000_init_hw_80003es2lan() and e1000_cfg_kmrn_{10_100|1000}_80003es2lan(),
and the unused flag FLAG_TIPG_MEDIUM_FOR_80003ESLAN is removed.

Bruce Allan c5083cf6d2 e1000e: convert head, tail and itr_register offsets to __iomem pointers
The Tx/Rx head and tail registers and itr_register are always at known
addresses based on the __iomem address at which the PCI region (from BAR 0)
is mapped and known offsets within the region for each of these registers.
Store and use the full address rather than just the region offset to reduce
unnecessary address calculations.  Also, change current u8 __iomem pointers
to void __iomem pointers.

Bruce Allan 70495a500d e1000e: add Receive Packet Steering (RPS) support
Enable RPS by default.  Disallow jumbo frames when both receive checksum
and receive hashing are enabled because the hardware cannot do both IP
payload checksum (enabled when receive checksum is enabled when using
packet split which is used for jumbo frames) and provide RSS hash at the
same time.

v2: added ethtool command to query flow hashing behavior per Ben Hutchings
    and changed the type of rsskey to cleanup the setting of the register
    array and avoid unnecessary casts (as pointed out by Joe Perches).
    The long error messages are not changed since there is nothing in
    the kernel ./Documentation that suggests the preferred method for
    dealing with long messages other than to never break strings; leaving
    them as-is for now.

tg3_start_xmit() makes the wrong assumption for TSOV6 that skb->head
doesnt include any payload data.

if (skb_is_gso_v6(skb))
	hdr_len = skb_headlen(skb) - ETH_HLEN;

This is not true anymore after commit f07d960df3 (tcp: avoid frag
allocation for small frames)

We should instead use : skb_transport_offset(skb) + tcp_hdrlen(skb)

Its also true for IPv4

Upon insertion of elements into the execution queue, it is validated
that there are enough credits to support additional vlan-macs,
and the credits are consumed. However, when removing a pending
command in `bnx2x_vland_mac_del_all' the consumed credits are not
released, which might cause leakage and eventually the inability to
add new vlan-macs in certain scenarios.

Sathya Perla df505eb804 be2net: create RSS rings even in multi-channel configs
Currently RSS rings are not created in a multi-channel config.
RSS rings can be created on one (out of four) interfaces per port in a
multi-channel config. Doing this insulates the driver from a FW bug wherin
multi-channel config is wrongly reported even when not enabled. This also
helps performance in a multi-channel config, as one interface per port gets
RSS rings.

Darren Hart 2b53d07891 pch_gbe: Do not abort probe on bad MAC
If the MAC is invalid or not implemented, do not abort the probe. Issue
a warning and prevent bringing the interface up until a MAC is set manually
(via ifconfig $IFACE hw ether $MAC).

Tested on two platforms, one with a valid MAC, the other without a MAC. The real
MAC is used if present, the interface fails to come up until the MAC is set on
the other. They successfully get an IP over DHCP and pass a simple ping and
login over ssh test.

This is meant to allow the Inforce SYS940X development board:
http://www.inforcecomputing.com/SYS940X_ECX.html
(and others suffering from a missing MAC) to work with the mainline kernel.
Without this patch, the probe will fail and the interface will not be created,
preventing the user from configuring the MAC manually.

This does not make any attempt to address a missing or invalid MAC for the
pch_phub driver.

Roopa Prabhu ca2b721de2 enic: rearrange some of the port profile code
This patch rearranges some of the port profile code in enic_probe.
It moves out some lines of port profile related code currently
inside CONFIG_PCI_IOV. This is only done to move all port profile
related code together so that it can help isolate the port profile
handling code under a separate #ifdef in our internal build scripts.

Matt Carlson c3b5003b62 tg3: Fix single-vector MSI-X code
Kdump kernels leave MSI-X interrupts (as setup by the crashed kernel)
enabled.  However, kdump only enables one CPU in the new environment,
thus causing tg3 to abort MSI-X setup.  When the driver attempts to
enable INTA or MSI interrupt modes on a kdump kernel, interrupt
delivery fails.

This patch attempts to workaround the problem by forcing the driver
to enable a single MSI-X interrupt.  In such a configuration, the
device's multivector interrupt mode must be disabled.

Yaniv Rosner 11b2ec6b73 bnx2x: Fix Super-Isolate mode for BCM84833
The Super-Isolate mode comes to isolate the BCM84833 PHY from the
outside world.  Not doing it correctly, made link partner see the link
before the driver was loaded.

This patch also involves SPIROM version fixes since it is used to
determine whether the common init of the PHY was already executed, and
the common init of this PHY is partially responsible for setting the
Super-Isolate mode.

Manfred Rudigier 9c4886e5e6 gianfar: Fix invalid TX frames returned on error queue when time stamping
When TX time stamping for PTP messages is enabled on a socket, a time
stamp is returned on the socket error queue to the user space application
after the frame was transmitted. The transmitted frame is also returned on
the error queue so that an application knows to which frame the time stamp
belongs.

In the current implementation the TxFCB is immediately followed by the
frame. Since the eTSEC inserts the TX time stamp 8 bytes after the TxFCB,
parts of the frame have been overwritten and an invalid frame was returned
on the socket error queue.

This patch fixes the described problem by adding additional 16 padding
bytes between the TxFCB and the frame for all messages sent from a time
stamping enabled socket (other sockets are not affected).

Manfred Rudigier db83d136d7 gianfar: Fix missing sock reference when processing TX time stamps
When there is not enough headroom in the skb a private copy will be made.
However, the private copy had no reference to the socket and consequently
no time stamp could be queued on the socket error queue during the
skb_tstamp_tx function. This patch fixes this issue by also stealing the
sock reference from the original skb after making the private copy.

Ben Hutchings cdb08f8fd8 sfc: Set default parallelism to per-core by default
The previous default of per-package can be more CPU-efficient, but
users generally seem to prefer per-core.  It should also allow
accelerated RFS to direct packets more precisely, if IRQ affinity
is properly spread out.

Francois Romieu 7ab87ff4c7 via-rhine: move work from irq handler to softirq and beyond.
- Tx processing is moved from the irq handler to NAPI poll
- link events and obscure event processing is moved to its proper work queue

Locking rules undergo some changes through the driver.

- the driver offers the usual lock-free Tx path
- besides the IRQ handler, the link event task schedules the napi handler.
  The driver thus adds some internal locking to prevent a loop when both
  must be disabled.
- the reset task keeps being scheduled from the Tx watchdog handler, thus
  with implicit Tx queue disabling. It does not need to care about irq,
  only napi softirq and competing task.
- it is not worth to add a dedicated lock between {g, s}et_wol and
  rhine_shutdown. It should not hurt no narrow it down a bit though.
- rhine_reset_task must keep its huge spin_lock_bh protected section due
  to :
  - races for the CAM registers (see rhine_vlan_rx_{add, kill}_vid)
  - implicit use of napi_enable (see init_registers)
  - use of the same lock for stats read / update exclusion between
    napi rx processing and rhine_get_stats
- rhine_resume requires a softirq disabled section for the same reason
  as rhine_reset_task
- {free, request}_irq have been replaced with IntrEnable actions in
  rhine_{suspend, resume}. It is hidden behind init_registers for the
  latter.

Michael Chan 23021c2105 cnic: Improve error recovery on bnx2x devices
When a bnx2x device encounters parity errors, it will not respond to all
SPQ messages.  As a result, the shutdown sequence before reset can take
a long time as the ulp drivers (bnx2i/bnx2fc) have to wait for timeout
of all such messages.

To improve this scenario, when bnx2x returns error on the SPQ, we'll send
an immediate response to the ulp drivers to avoid such lengthy timeouts.

Adjust the return code of relevant functions to return error only if
the message cannot be sent on the SPQ so that we'll generate an error
completion to the ulp drivers.

Koki Sanagi a95a07445e igb: reset PHY after recovering from PHY power down
According to 82576_Datasheet.pdf, PHY setting is lost after PHY power down.
So resetting PHY is needed when recovering from PHY power down to set a default
setting to PHY register.
Owing to this lack, NIC doesn't link up in some rare situation.

Yan, Zheng 749ab2cd12 igb: add basic runtime PM support
Use the runtime power management framework to add basic runtime PM support
to the igb driver. Namely, make the driver suspend the device when the link
is off and set it up for generating a wakeup event after the link has been
detected again. This feature is disabled by default.

Based on e1000e's runtime PM code.

Changes since v1:
Don't suspend the device when shutting down the interface.
Avoid race between runtime suspending and ethtool operations.

Jesse Brandeburg 3a3847e007 e1000: fix lockdep splat in shutdown handler
As reported by Steven Rostedt, e1000 has a lockdep splat added
during the recent merge window.  The issue is that
cancel_delayed_work is called while holding our private mutex.

There is no reason that I can see to hold the mutex during pci
shutdown, it was more just paranoia that I put the mutex_lock
around the call to e1000_down.

In a quick survey lots of drivers handle locking differently when
being called by the pci layer.  The assumption here is that we
don't need the mutexes' protection in this function because
the driver could not be unloaded while in the shutdown handler
which is only called at reboot or poweroff.

Javier Martinez Canillas 6386994e03 net/smsc911x: Check if PHY is in operational mode before software reset
SMSC LAN generation 4 chips integrate an IEEE 802.3 ethernet physical layer.
The PHY driver for this integrated chip enable an energy detect power-down mode.
When the PHY is in a power-down mode, it prevents the MAC portion chip to be
software reseted.

That means that if we compile the kernel with the configuration option SMSC_PHY
enabled and try to bring the network interface up without an cable plug-ed the
PHY will be in a low power mode and the software reset will fail returning -EIO
to user-space:

root@igep00x0:~# ifconfig eth0 up
ifconfig: SIOCSIFFLAGS: Input/output error

This patch disable the energy detect power-down mode before trying to software
reset the LAN chip and re-enables after it was reseted successfully.

The ethernet driver for this family of devices needs to access the SMSC PHY

Sascha Hauer 86d8c07ff2 net/davinci: do not use all descriptors for tx packets
The driver uses a shared pool for both rx and tx descriptors.
During open it queues fixed number of 128 descriptors for receive
packets. For each received packet it tries to queue another
descriptor. If this fails the descriptor is lost for rx.
The driver has no limitation on tx descriptors to use, so it
can happen during a nmap / ping -f attack that the driver
allocates all descriptors for tx and looses all rx descriptors.
The driver stops working then.
To fix this limit the number of tx descriptors used to half of
the descriptors available, the rx path uses the other half.

Tested on a custom board using nmap / ping -f to the board from
two different hosts.

This is because fec.h uses CONFIG_SOC_IMX28 to determine the register
layout of the core which makes sense since the MX23 does not have a fec.
However, Kconfig uses the broader ARCH_MXS symbol and this way even
makes the fec-driver default for MX23. Adapt Kconfig to use the more
precise SOC_IMX28 as well.

Rx mode should be reset after resming, so unconditionally updating rx
mode rather than conditionally updating based on the value we
remembered, otherwise unexpected value may be used by the nic after
resuming.

btw. I find and test this when debugging guest hibernation in qemu, as
I did not have a 8139cp card in hand, this patch is untested in a
physical 8139cp card, plase review it carefully.

Ajit Khaparde b236916a68 be2net: query link status in be_open()
be2net gets an async link status notification from the FW when it creates
an MCC queue. There are some cases in which this gratuitous notification
is not received from FW. To cover this explicitly query the link status
in be_open().

Rogerio Pimentel c3b084c24c net: fec: Adjust ENET MDIO timeouts
On extensive NFS boots on a mx6qsabrelite board it was noted that "FEC: MDIO read timeout" were occuring,
which caused failure on loading the FEC driver.

The original FEC_MII_TIMEOUT was set to 1 ms, which is too low when passed to the usecs_to_jiffies macro.

On ARM one jiffy is 10ms, so use a timeout of 30ms, which corresponds to 3 jiffies.

After running extensive NFS boots, the MDIO timeouts do not occur anymore with this change.

Joshua Kinard c0d2b8376a net: meth: Add set_rx_mode hook to fix ICMPv6 neighbor discovery
SGI IP32 (O2)'s ethernet driver (meth) lacks a set_rx_mode function, which
prevents IPv6 from working completely because any ICMPv6 neighbor
solicitation requests aren't picked up by the driver.  So the machine can
ping out and connect to other systems, but other systems will have a very
hard time connecting to the O2.

Rusty Russell eb93992207 module_param: make bool parameters really bool (net & drivers/net)
module_param(bool) used to counter-intuitively take an int.  In
fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy
trick.

It's time to remove the int/unsigned int option.  For this version
it'll simply give a warning, but it'll break next kernel version.

(Thanks to Joe Perches for suggesting coccinelle for 0/1 -> true/false).

Matt Carlson 90415477bf tg3: Make the RSS indir tbl admin configurable
This patch adds the ethtool callbacks necessary to change the rss
indirection table from userspace.  Should the number of interrupts
change (e.g.  across a close / open call, or through a reset) and
any one of the indirection table values fall out-of-range, the driver
will reset the indirection table to a default layout.

[Integrated many suggestions made by Ben Hutchings.]

bnx2-mips-06-6.2.3 and bnx2-mips-09-6.2.1.b

New firmware fixes iSCSI problems with some LeftHand targets that don't
set TTT=0xffffffff for Data-In according to spec.  Firmware generates
exception warnings for this condition and becomes very slow.  This is
fixed by suppressing these warnings when using default error mask.

Padmanabh Ratnakar 93c86700c0 be2net: Fix INTx processing for Lancer
Lancer does not have HW registers to indicate the EQ causing the INTx
interrupt. As a result EQE entries of one EQ may be consumed when interrupt
is caused by another EQ. Fix this by arming CQs at the end of NAPI poll
routine to regenerate the EQEs.

Yevgeny Petrilin 8d0fc7b611 mlx4_core: Changing link sensing logic
New FW can give clues to driver regarding default port type
and whether or not we should default to link sensing on the port.

2 bits are added to QUERY_PORT command:
1. suggested_type: This bit gives a hint whether the default port type should be
   IB or Ethernet.
   The driver will use this hint in case the user didn't specify explicitly the link layer
   type he wants to set.
2. default_sense: If this bit is set, we would sense the port type on start-up
   and default the port to link sensing

reported it supports the feature.

Ben Hutchings 278bc4296b ethtool: Define and apply a default policy for RX flow hash indirection
All drivers that support modification of the RX flow hash indirection
table initialise it in the same way: RX rings are assigned to table
entries in rotation.  Make that default policy explicit by having them
call a ethtool_rxfh_indir_default() function.

In the ethtool core, add support for a zero size value for
ETHTOOL_SRXFHINDIR, which resets the table to this default.

indirectional table size.  Use this to validate the user buffer size

the number of RX rings, and validate the contents of the new
indirection table before calling set_rxfh_indir.  Remove this
validation from drivers.

Jack Morgenstein ab9c17a009 mlx4_core: Modify driver initialization flow to accommodate SRIOV for Ethernet
1. Added module parameters sr_iov and probe_vf for controlling enablement of
   SRIOV mode.
2. Increased default max num-qps, num-mpts and log_num_macs to accomodate
   SRIOV mode
3. Added port_type_array as a module parameter to allow driver startup with
   ports configured as desired.
   In SRIOV mode, only ETH is supported, and this array is ignored; otherwise,
   for the case where the FW supports both port types (ETH and IB), the
   port_type_array parameter is used.
   By default, the port_type_array is set to configure both ports as IB.
4. When running in sriov mode, the master needs to initialize the ICM eq table
   to hold the eq's for itself and also for all the slaves.
5. mlx4_set_port_mask() now invoked from mlx4_init_hca, instead of in mlx4_dev_cap.
6. Introduced sriov VF (slave) device startup/teardown logic (mainly procedures
   mlx4_init_slave, mlx4_slave_exit, mlx4_slave_cap, mlx4_slave_exit and flow
   modifications in __mlx4_init_one, mlx4_init_hca, and mlx4_setup_hca).
   VFs obtain their startup information from the PF (master) device via the
   comm channel.
7. In SRIOV mode (both PF and VF), MSI_X must be enabled, or the driver
   aborts loading the device.
8. Do not allow setting port type via sysfs when running in SRIOV mode.
9. mlx4_get_ownership:  Currently, only one PF is supported by the driver.
   If the HCA is burned with FW which enables more than one PF, only one
   of the PFs is allowed to run.  The first one up grabs a FW ownership
   semaphone -- all other PFs will find that semaphore taken, and the
   driver will not allow them to run.

Jack Morgenstein d81c7186aa mlx4_core: adjust catas operation for SRIOV mode
When running in SRIOV mode, driver should not automatically start/stop
the mlx4_core upon sensing an HCA internal error -- doing this disables/enables
sriov, which will cause the hypervisor to hang if there are running VMs with
attached VFs.

In addition, on VMs the catas process should not run at all, since the HCA
error buffer is not available to VMs in the BARs.

1. order     - log(mtt segments), 'mtt segment' groups several mtts together.
2. first_seg - segment location relative to mtt table.
In the current implementation:
1. order     - log(mtts) rather than segments
2. offset    - mtt index in mtt table

Note: The actual mtt allocation is made in segments but it is
      transparent to callers.

Rational: The mtt resource holders are not interested on how the allocation
          of mtt is done, but rather on how they will use it.

To enable internal loopback, always fill DMAC in control segment
when transmitting the packet, once this is done, the packet is subject
for loopback for if the DMAC mathces one of the multicast/unicast addresses
registered on the physical port.
In receive path if source MAC is our own MAC and we are not in selftest,
or not in force LB mode - drop this packet.

virtualization unaware

Move handling qp functionality to mlx4_get_eth_qp/mlx4_put_eth_qp
including reserve/release range and add/release unicast steering.
Let mlx4_register/unregister_mac deal only with MAC (un)registration.

Eli Cohen c82e9aa0a8 mlx4_core: resource tracking for HCA resources used by guests
The resource tracker is used to track usage of HCA resources by the different
guests.

Virtual functions (VFs) are attached to guest operating systems but
resources are allocated from the same pool and are assigned to VFs. It is
essential that hostile/buggy guests not be able to affect the operation of
other VFs, possibly attached to other guest OSs since ConnectX firmware is not
tolerant to misuse of resources.

The resource tracker module associates each resource with a VF and maintains
state information for the allocated object. It also defines allowed state
transitions and enforces them.

Relationships between resources are also referred to. For example, CQs are
pointed to by QPs, so it is forbidden to destroy a CQ if a QP refers to it.

ICM memory is always accessible through the primary function and hence it is
allocated by the owner of the primary function.

When a guest dies, an FLR is generated for all the VFs it owns and all the
resources it used are freed.

The tracked resource types are: QPs, CQs, SRQs, MPTs, MTTs, MACs, RES_EQs,
and XRCDNs.

Jack Morgenstein acba2420f9 mlx4_core: Add wrapper functions and comm channel and slave event support to EQs
Passing async events to slaves:
In SRIOV mode, each slave creates its own async EQ, but only the master can
register directly with the FW to receive async events.  Async events which
should be passed to slaves (such as a WQ_ACCESS_ERROR for a QP owned by a slave)
are generated at the slave by the master using the GEN_EQE FW command.

Wrapper functions: mlx4_MAP_EQ_wrapper
Only the master can map an EQ. The slave commands to map their EQs arrive
at the master via the comm channel.  The master then invokes the wrapper
function to do the work (and enter the resource in the tracking database).

New events: COMM_CHANNEL and FLR
The COMM_CHANNEL event arrives only at the master, and signals that
a slave has posted a command on the comm channel.
The FLR event is generated by the FW when a guest operating a VF
unexpectedly goes down.

Marcel Apfelbaum 5cc914f108 mlx4_core: Added FW commands and their wrappers for supporting SRIOV
The following commands are added here:
1. QUERY_FUNC_CAP and its wrapper.  This function is used by VFs when
   they start up to receive configuration information from the PF, such
   as resource quotas for this VF, which ports should be used (currently
   two), what protocol is running on the port (currently Ethernet ONLY,
   or port not active).

2. QUERY_PORT and its wrapper. Previously, this FW command was invoked directly
   by the ETH driver (en_port.c) using mlx4_cmd_box. Virtualization is now
   required here (the VF's MAC address must be substituted for the PFs
   MAC address returned by the FW). We changed the invocation
   in the ETH driver to use mlx4_QUERY_PORT, and added the wrapper.

3. QUERY_HCA. Used by the VF to determine how the HCA was initialized.
   For now, we need only the multicast table member entry size
   (log2_mc_table_entry_sz, in the ConnectX PRM).  No wrapper is needed
   here, because the data may be passed as is to the VF without modification).

   In this command, we have added a GLOBAL_CAPS field for passing required
   configuration information from FW to a VF (this field is to allow safely
                   adding new SRIOV capabilities which require support in VF drivers, too).
   Bits will set here by FW in response to PF-driver configuration commands which
   will activate as yet undefined new SRIOV features. The VF will test to see that
   all required capabilities indicated by this field are supported (i.e., if a bit
   is set and the VF driver does not recognize that bit, it must abort
   its initialization).  Currently, no bits are set.

4. Added a CLOSE_PORT wrapper.  The PF context needs to keep track of how many VF contexts
   have the port open.  The PF context will not actually issue the FW close port command
   until the last port user issues a CLOSE_PORT request.

Yevgeny Petrilin e8f081aacd net/mlx4_core: Implement the master-slave communication channel
When SRIOV is enabled, pf and vfs communicate via shared comm channel.
The vf gets its side of the comm channel via a VF BAR.
Each VF (slave) creates its vHCR (virtual HCA Command Register),
Its DMA address is passed to the PF (master) using Communication Channel Register.
The same Register is used to notify the master of commands posted by the
slaves and for the master to pass events to the slaves, such as command completions
and asynchronous events.

The vHCR format is identical to the HCR format, except for the 'go' and 't' bits,
which are reserved in the vHCR. Posting commands to the vHCR is identical to
the way it is done with the HCR, albeit that the function/PF token fields are
used instead of the HCR go bit.
Specifically:
- When the function prepares a new command in the vHCR, it issues the Post_vHCR_cmd
  communication channel command and toggles the value of the function token;
  when PF token has an equal value, the command has been accepted and a new command may be posted.
- When the PF detects a Post_vHCR_cmd command, it concludes that a new command is available in the vHCR;
  after processing the command, the PF toggles the PF token to match the function token.

When the 'e' bit is not set, the completion of a Post_vHCR_cmd command also indicates
the completion the vHCR command. If, however, the 'e' bit is set, the completion of a
Post_vHCR_cmd command only indicates that the vHCR command has been accepted for execution by the PF.

Function commands are processed by the PF as follows:
-DMA (using the ACCESS_MEM command) the vHCR image into a shadow buffer.
-Validate that the opcode is non-privileged, and that the opcode- and input-modifiers are legal.
-DMA the in-box (if required) into a shadow buffer.
-Validate the command:
	o Resource ranges (e.g., QP ranges).
	o Partition key.
	o Ranges of referenced resources (e.g., CQs within QP contexts).
-If the 'e' bit is set
	o complete the Post_vHCR_cmd command
-Execute the command on the HCR.
-DMA the results to the vHCR out-box (if required).
-If the 'e' bit is set
	o Indicate command completion by generating a completion event using the GEN_EQE command
-Otherwise
	o DMA the command status to the vHCR
	o Complete the Post_vHCR_cmd command

Jack Morgenstein f5311ac109 mlx4_core: Reduce number of PD bits to 17
When SRIOV is enabled on the chip (at FW burning time),
the HCA uses only 17 bits for the PD. The remaining 7 high-order bits
are ignored.

Change the allocator to return only 17 bits for the PD.  The MSB 7
bits will be used to encode the slave number for consistency
checking later on in the resource tracker.

Jack Morgenstein f9baff509f mlx4_core: Add "native" argument to mlx4_cmd and its callers (where needed)
For SRIOV, some Hypervisor commands can be executed directly (native = 1).
Others should go through the command wrapper flow (for tracking resource
usage, for example, or for changing some HCA configurations that slaves
need to be notified of).

This patch sets the groundwork for this capability -- adding the correct
value of "native" in each case.

Note that if SRIOV is not activated, this parameter has no effect.

- use adapter->num_vfs (and not the module param) to store the actual
number of vfs created. Use the same variable to reflect SRIOV
enable/disable state. So, drop the adapter->sriov_enabled field.

- use for_all_vfs() macro in VF configuration code

- drop the "vf_" prefix for the fields of be_vf_cfg; the prefix is
redundant and removing it helps reduce line wrap

Sathya Perla 110b82bc62 be2net: fix ethtool ringparam reporting
The ethtool "-g" option is supposed to report the max queue length and
user modified queue length for RX and TX queues.  be2net doesn't support
user modification of queue lengths. So, the correct values for these
would be the max numbers.
be2net incorrectly reports the queue used values for these fields.

Dmitry Kravkov 7185bb335a bnx2x: properly initialize L5 features
The code is missing initialization of NO_FCOE_FLAG and NO_ISCSI*FLAGS
when CONFIG_CNIC is not selected.
This causes panic during driver load since commit
1d187b34da where NO_FCOE tested
unconditionally (outside #ifdef BCM_CNIC  structure) and
accessed fp[FCOE_IDX] which is not allocated.

Jiri Pirko 8e586137e6 net: make vlan ndo_vlan_rx_[add/kill]_vid return error value
Let caller know the result of adding/removing vlan id to/from vlan
filter.

In some drivers I make those functions to just return 0. But in those
where there is able to see if hw setup went correctly, return value is
set appropriately.

Lothar Waßmann e163cc97f9 net/fec: fix the .remove code
The .remove code is broken in several ways.
 - mdiobus_unregister() is called twice for the same object in case of dual FEC
 - phy_disconnect() is being called when the PHY is already disconnected
 - the requested IRQ(s) are not freed
 - fec_stop() is being called with the inteface already stopped

 All of those lead to kernel crashes if the remove function is actually used.

Lothar Waßmann 589efdc7f7 net/fec: don't munge MAC address from platform data
When the MAC address is supplied via platform_data it should be OK as
it is and should not be modified in case of a dual FEC setup.
Also copying the MAC from platform_data to the single 'macaddr'
variable will overwrite the MAC for the first interface in case of a
dual FEC setup.
