Will Deacon a391263cd8 ARM: 8203/1: mm: try to re-use old ASID assignments following a rollover
Rather than unconditionally allocating a fresh ASID to an mm from an
older generation, attempt to re-use the old assignment where possible.

This can bring performance benefits on systems where the ASID is used to
tag things other than the TLB (e.g. branch prediction resources).

Stephen Boyd 6f0f2a9f0f ARM: 8196/1: vfp: Workaround bad MVFR1 register on some Kraits
Certain versions of the Krait processor don't report that they
support the fused multiply accumulate instruction via the MVFR1
register despite the fact that they actually do. Unfortunately we
use this register to identify support for VFPv4. Override the
hwcap on all Krait processors to indicate support for VFPv4 to
workaround this.

As best I can tell this issue existed for the set_tls ARM syscall
before commit fbfb872f5f "ARM: 8148/1: flush TLS and thumbee
register state during exec" consolidated the TLS manipulation code
into the set_tls helper function, but now that we're using it to flush
register state during execve, !MMU users encounter the oops at the
first exec.

Prevent CONFIG_MMU=n configurations from enabling
CONFIG_KUSER_HELPERS.

"ARM: integrator: make the Integrator multiplatform"
broke allmodconfig like this:

This is due to the fact that as we turned on multiplatform
for the Integrator, this enabled a lot of non-applicable
CPU's to be selected for its multiplatform images, due to
a lot of "depends on ARCH_INTEGRATOR" restrictions in
arch/arm/mm/Kconfig for the different ARM CPU types.

Fix this by restricting the CPU selections to respective
multiplatform config, which now becomes a subset of the
possible Integrator configurations, or alternatively the
non-multiplatform config plus ARCH_INTEGRATOR, i.e.:

   (ARCH_INTEGRATOR || ARCH_FOO ...)

Since the Integrator has been converted to multiplatform,
this will often take the short form:

Thomas Petazzoni 9ff0bb5ba6 ARM: 8180/1: mm: implement no-highmem fast path in kmap_atomic_pfn()
Since CONFIG_HIGHMEM got enabled on ARMv5 Kirkwood, we have noticed a
very significant drop in networking performance. The test were
conducted on an OpenBlocks A7 board. Without this patch, the outgoing
performance measured with iperf are:

 - highmem OFF, TSO OFF   544 Mbit/s
 - highmem OFF, TSO ON	  942 Mbit/s
 - highmem ON,  TSO OFF   306 Mbit/s
 - highmem ON,  TSO ON    246 Mbit/s

On this Kirkwood platform, the L2 cache is a Feroceon cache, and with
this cache, all the range operations have to be done on virtual
addresses and not physical addresses. Therefore, whenever
CONFIG_HIGHMEM is enabled, the cache maintenance operations call
kmap_atomic_pfn() and kunmap_atomic().

However, kmap_atomic_pfn() does not implement the same fast path for
non-highmem pages as the one implemented in kmap_atomic(), and this is
one of the reason for the performance drop. While this patch does not
fully restore the performances, it clearly improves them a lot:

 - highmem ON, TSO OFF   306 Mbit/s     387 Mbit/s
 - highmem ON, TSO ON    246 Mbit/s     434 Mbit/s

We're still far from the !CONFIG_HIGHMEM performances, but it does
improve a bit the situation.

message could also be changed to something like:

Fabio Estevam d0b92845e5 ARM: 8182/1: l2c: Make l2x0_cache_size_of_parse() return 'int'
Since commit f3354ab674 ("ARM: 8169/1: l2c: parse cache properties from
ePAPR definitions") the following error is seen on imx6q:

[    0.000000] PL310 OF: cache setting yield illegal associativity
[    0.000000] PL310 OF: -2147097556 calculated, only 8 and 16 legal

As imx6q does not pass the "cache-size" and "cache-sets" properties in DT, the function l2x0_cache_size_of_parse() returns early and keep the 'associativity' pointer uninitialized.

To fix this problem, return error codes inside l2x0_cache_size_of_parse() and only use the 'associativity' pointer result if l2x0_cache_size_of_parse() succeeds.

Kees Cook 80d6b0c2ee ARM: mm: allow text and rodata sections to be read-only
This introduces CONFIG_DEBUG_RODATA, making kernel text and rodata
read-only. Additionally, this splits rodata from text so that rodata can
also be NX, which may lead to wasted memory when aligning to SECTION_SIZE.
The read-only areas are made writable during ftrace updates and kexec.

Kees Cook 1e6b48116a ARM: mm: allow non-text sections to be non-executable
Adds CONFIG_ARM_KERNMEM_PERMS to separate the kernel memory regions
into section-sized areas that can have different permisions. Performs
the NX permission changes during free_initmem, so that init memory can be
reclaimed.

This uses section size instead of PMD size to reduce memory lost to
padding on non-LPAE systems.

Rob Herring 836a241832 ARM: expand fixmap region to 3MB
With commit a05e54c103 ("ARM: 8031/2: change fixmap mapping region to
support 32 CPUs"), the fixmap region was expanded to 2MB, but it
precluded any other uses of the fixmap region. In order to support other
uses the fixmap region needs to be expanded beyond 2MB. Fortunately, the
adjacent 1MB range 0xffe00000-0xfff00000 is availabe.

Remove fixmap_page_table ptr and lookup the page table via the virtual
address so that the fixmap region can span more that one pmd. The 2nd
pmd is already created since it is shared with the vector page.

Mark Salter b615bbbff1 arm: use generic fixmap.h
ARM is different from other architectures in that fixmap pages are indexed
with a positive offset from FIXADDR_START.  Other architectures index with
a negative offset from FIXADDR_TOP.  In order to use the generic fixmap.h
definitions, this patch redefines FIXADDR_TOP to be inclusive of the
useable range.  That is, FIXADDR_TOP is the virtual address of the topmost
fixed page.  The newly defined FIXADDR_END is the first virtual address
past the fixed mappings.

Steve Capper b8cd51afe0 arm: mm: enable RCU fast_gup
Activate the RCU fast_gup for ARM.  We also need to force THP splits to
broadcast an IPI s.t.  we block in the fast_gup page walker.  As THP
splits are comparatively rare, this should not lead to a noticeable
performance degradation.

Some pre-requisite functions pud_write and pud_page are also added.

remapped with coherent attributes.  Factor out the the remapping code from
arm and put it in a common location to reduce code duplication.

As part of this, the arm APIs are now migrated away from
ioremap_page_range to the common APIs which use map_vm_area for remapping.
 This should be an equivalent change and using map_vm_area is more correct
as ioremap_page_range is intended to bring in io addresses into the cpu
space and not regular kernel managed memory.

Linus Walleij f3354ab674 ARM: 8169/1: l2c: parse cache properties from ePAPR definitions
When both 'cache-size' and 'cache-sets' are specified for a L2 cache
controller node, parse those properties and set up the
set size based on which type of L2 cache controller we are using.

Update the L2 cache controller Device Tree binding with the optional
'cache-size', 'cache-sets', 'cache-block-size' and 'cache-line-size'
properties. These come from the ePAPR specification.

Using the cache size, number of sets and cache line size we can
calculate desired associativity of the L2 cache. This is done
by the calculation:

    set size = cache size / sets
    ways = set size / line size
    way size = cache size / ways = sets * line size
    associativity = cache size / way size

Example output from the PB1176 DT that look like this:

Robin Murphy 5ca918e5e3 ARM: 8165/1: alignment: don't break misaligned NEON load/store
The alignment fixup incorrectly decodes faulting ARM VLDn/VSTn
instructions (where the optional alignment hint is given but incorrect)
as LDR/STR, leading to register corruption. Detect these and correctly

Will Deacon 2c553ac19e ARM: 8164/1: mm: clear SCTLR.HA instead of setting it for LPAE
SCTLR.HA (hardware access flag) is deprecated and not actually
implemented by any CPUs. Furthermore, it can confuse cr_alignment checks

TTBR1_OFFSET to TTRR1 was wrong:

        addls   ttbr1, ttbr1, #TTBR1_OFFSET
        adcls   tmp, tmp, #0

addls doesn't update flags, adcls adds carry from cmp above:

        cmp     ttbr1, tmp                    @ PHYS_OFFSET > PAGE_OFFSET?

Condition 'ls' means carry flag is clear or zero flag is set, thus only one
case is affected: when PHYS_OFFSET == PAGE_OFFSET.

It seems safer to remove this fixup. Bug is here for ages and nobody
complained. Let's fix it separately.

Mark Rutland 8586831317 ARM: 8128/1: abort: don't clear the exclusive monitors
The ARMv6 and ARMv7 early abort handlers clear the exclusive monitors
upon entry to the kernel, but this is redundant:

  - We clear the monitors on every exception return since commit
    200b812d00 ("Clear the exclusive monitor when returning from an
    exception"), so this is not necessary to ensure the monitors are
    cleared before returning from a fault handler.

  - Any dummy STREX will target a temporary scratch area in memory, and
    may succeed or fail without corrupting useful data. Its status value
    will not be used.

  - Any other STREX in the kernel must be preceded by an LDREX, which
    will initialise the monitors consistently and will not depend on the
    earlier state of the monitors.

Therefore we have no reason to care about the initial state of the
exclusive monitors when a data abort is taken, and clearing the monitors
prior to exception return (as we already do) is sufficient.

This patch removes the redundant clearing of the exclusive monitors from
the early abort handlers.

Joonsoo Kim a254129e86 CMA: generalize CMA reserved area management functionality
Currently, there are two users on CMA functionality, one is the DMA
subsystem and the other is the KVM on powerpc.  They have their own code
to manage CMA reserved area even if they looks really similar.  From my
guess, it is caused by some needs on bitmap management.  KVM side wants
to maintain bitmap not for 1 page, but for more size.  Eventually it use
bitmap where one bit represents 64 pages.

When I implement CMA related patches, I should change those two places
to apply my change and it seem to be painful to me.  I want to change
this situation and reduce future code management overhead through this
patch.

This change could also help developer who want to use CMA in their new
feature development, since they can use CMA easily without copying &
pasting this reserved area management code.

In previous patches, we have prepared some features to generalize CMA
reserved area management and now it's time to do it.  This patch moves
core functions to mm/cma.c and change DMA APIs to use these functions.

There is no functional change in DMA APIs.

Konstantin Khlebnikov 811a2407a3 ARM: 8115/1: LPAE: reduce damage caused by idmap to virtual memory layout
On LPAE, each level 1 (pgd) page table entry maps 1GiB, and the level 2
(pmd) entries map 2MiB.

When the identity mapping is created on LPAE, the pgd pointers are copied
from the swapper_pg_dir.  If we find that we need to modify the contents
of a pmd, we allocate a new empty pmd table and insert it into the
appropriate 1GB slot, before then filling it with the identity mapping.

However, if the 1GB slot covers the kernel lowmem mappings, we obliterate
those mappings.

When replacing a PMD, first copy the old PMD contents to the new PMD, so
that we preserve the existing mappings, particularly the mappings of the
kernel itself.

For LPAE, we have the following means for encoding writable or dirty
ptes:

So we can't distinguish between writeable clean ptes and read only
ptes. This can cause problems with ptes being incorrectly flagged as
read only when they are writeable but not dirty.

This patch renumbers L_PTE_RDONLY from AP[2] to a software bit #58,
and adds additional logic to set AP[2] whenever the pte is read only
or not dirty. That way we can distinguish between clean writeable ptes
and read only ptes.

HugeTLB pages will use this new logic automatically.

We need to add some logic to Transparent HugePages to ensure that they
correctly interpret the revised pgprot permissions (L_PTE_RDONLY has
moved and no longer matches PMD_SECT_AP2). In the process of revising
THP, the names of the PMD software bits have been prefixed with L_ to
make them easier to distinguish from their hardware bit counterparts.

Russell King a11dd731f5 ARM: SWP emulation: always enable when SMP is enabled
SWP is deprecated in ARMv6 and ARMv7 CPUs, but more importantly, when
running on a SMP system, SWP doesn't guarantee atomicity.  This means
it can't really be used (by userspace) for locking purposes in a SMP
environment.

Currently, many configurations leave the SWP emulation disabled, which
means we never know if userspace executes this instruction on ARMv7
hardware.  Rectify this by enabling SWP emulation for ARMv7 with SMP
(where we can trap the instruction.)

The CP15 diagnostic register holds ARM errata bits on Cortex-A9, so it
needs to be saved/restored on suspend/resume.  Otherwise, the
effectiveness of errata workaround gets lost together with diagnostic
register bit across suspend/resume cycle.  And the CP15 power control
register of Cortex-A9 shares the same problem.

The patch adds a couple of Cortex-A9 specific suspend/resume functions
to save/restore these two Cortex-A9 CP15 registers across the
suspend/resume cycle.

Since pj4b suspend/resume routines are implemented based on generic
ARMv7 ones, instead of hard-coding cpu_pj4b_suspend_size, we should have
it be cpu_v7_suspend_size plus pj4b specific bytes.  Otherwise, if
cpu_v7_suspend_size gets updated alone, the pj4b suspend/resume will
likely be broken.

While at it, fix the comments in cpu_pj4b_do_resume, as we're restoring
CP15 registers rather than saving in there.

Russell King 1e7e321185 ARM: alignment: save last kernel aligned fault location
Save and report (via the procfs file) the last kernel unaligned fault
location.  This allows us to trivially inspect where the last fault
happened for cases which we don't expect to occur.

Since we expect the kernel to generate misalignment faults (due to
the networking layer), even when warnings are enabled, we don't log
them for the kernel.

Russell King 6ebbf2ce43 ARM: convert all "mov.* pc, reg" to "bx reg" for ARMv6+
ARMv6 and greater introduced a new instruction ("bx") which can be used
to return from function calls.  Recent CPUs perform better when the
"bx lr" instruction is used rather than the "mov pc, lr" instruction,
and this sequence is strongly recommended to be used by the ARM
architecture manual (section A.4.1.1).

We provide a new macro "ret" with all its variants for the condition
code which will resolve to the appropriate instruction.

Rather than doing this piecemeal, and miss some instances, change all
the "mov pc" instances to use the new macro, with the exception of
the "movs" instruction and the kprobes code.  This allows us to detect
the "mov pc, lr" case and fix it up - and also gives us the possibility
of deploying this for other registers depending on the CPU selection.

Ensure that platform maintainers check the CPU part number in the right
manner: the CPU part number is meaningless without also checking the
CPU implement(e|o)r (choose your preferred spelling!)  Provide an
interface which returns both the implementer and part number together,
and update the definitions to include the implementer.

Mark the old function as being deprecated... indeed, using the old
function with the definitions will now always evaluate as false, so
people must update their un-merged code to the new function.  While
this could be avoided by adding new definitions, we'd also have to
create new names for them which would be awkward.

Russell King 6b076991dc ARM: DMA: ensure that old section mappings are flushed from the TLB
When setting up the CMA region, we must ensure that the old section
mappings are flushed from the TLB before replacing them with page
tables, otherwise we can suffer from mismatched aliases if the CPU
speculatively prefetches from these mappings at an inopportune time.

A mismatched alias can occur when the TLB contains a section mapping,
but a subsequent prefetch causes it to load a page table mapping,
resulting in the possibility of the TLB containing two matching
mappings for the same virtual address region.

to use memblock_get_current_limit for calculating the max_low pfn.
nommu targets never actually set a limit on memblock though which

value. Set the memblock_limit to be the end of DDR to make sure
bounds are calculated correctly.

Thomas Petazzoni 98ea2dba65 ARM: 8076/1: mm: add support for HW coherent systems in PL310 cache
When a PL310 cache is used on a system that provides hardware
coherency, the outer cache sync operation is useless, and can be
skipped. Moreover, on some systems, it is harmful as it causes
deadlocks between the Marvell coherency mechanism, the Marvell PCIe
controller and the Cortex-A9.

To avoid this, this commit introduces a new Device Tree property
'arm,io-coherent' for the L2 cache controller node, valid only for the
PL310 cache. It identifies the usage of the PL310 cache in an I/O
coherent configuration. Internally, it makes the driver disable the
outer cache sync operation.

Note that technically speaking, a fully coherent system wouldn't
require any of the other .outer_cache operations. However, in
practice, when booting secondary CPUs, these are not yet coherent, and
therefore a set of cache maintenance operations are necessary at this
point. This explains why we keep the other .outer_cache operations and
only ->sync is disabled.

While in theory any write to a PL310 register could cause the
deadlock, in practice, disabling ->sync is sufficient to workaround
the deadlock, since the other cache maintenance operations are only
used in very specific situations.

Contrary to previous versions of this patch, this new version does not
simply NULL-ify the ->sync member, because the l2c_init_data
structures are now 'const' and therefore cannot be modified, which is
a good thing. Therefore, this patch introduces a separate
l2c_init_data instance, called of_l2c310_coherent_data.

Currently hugepage migration is available for all archs which support
pmd-level hugepage, but testing is done only for x86_64 and there're
bugs for other archs.  So to avoid breaking such archs, this patch
limits the availability strictly to x86_64 until developers of other
archs get interested in enabling this feature.

Simply disabling hugepage migration on non-x86_64 archs is not enough to
fix the reported problem where sys_move_pages() hits the BUG_ON() in
follow_page(FOLL_GET), so let's fix this by checking if hugepage
migration is supported in vma_migratable().

Russell King ca8f0b0a54 ARM: ensure C page table setup code follows assembly code
Fix a long standing bug where, for ARMv6+, we don't fully ensure that
the C code sets the same cache policy as the assembly code.  This was
introduced partially by commit 11179d8ca2 ([ARM] 4497/1: Only allow
safe cache configurations on ARMv6 and later) and also by adding SMP
support.

This patch sets the default cache policy based on the flags used by the
assembly code, and then ensures that when a cache policy command line
argument is used, we verify that on ARMv6, it matches the initial setup.

Thomas Petazzoni 1c8c3cf0b5 ARM: 8060/1: mm: allow sub-architectures to override PCI I/O memory type
Due to a design incompatibility between the PCIe Marvell controller
and the Cortex-A9, stressing PCIe devices with a lot of traffic
quickly causes a deadlock.

One part of the workaround for this is to have all PCIe regions mapped
as strongly-ordered (MT_UNCACHED) instead of the default
MT_DEVICE. While the arch_ioremap_caller() mechanism allows
sub-architecture code to override ioremap(), used to map PCIe memory
regions, there isn't such a mechanism to override the behavior of
pci_ioremap_io().

This commit adds the arch_pci_ioremap_mem_type variable, initialized
to MT_DEVICE by default, and that sub-architecture code can
override. We have chosen to expose a single variable rather than
offering the possibility of overriding the entire pci_ioremap_io(),
because implementing pci_ioremap_io() requires calling functions
(get_mem_type()) that are private to the arch/arm/mm/ code.

Russell King 4374d64933 ARM: l2c: add automatic enable of early BRESP
The AXI bus protocol requires that a write response should only be
sent back to the master when the last write has been accepted.  Early
BRESP allows the L2C-310 to send the write response as soon as the
store buffer accepts the write address.

Cortex-A9 processors can signal to the L2C-310 that they wish to be
notified early, and if this optimisation is enabled, the L2C-310 can
signal an early write response.

When Linux is running in the non-secure world, any write to a secure
L2C register will generate an abort.  Platforms normally have to call
firmware to work around this.  Provide a hook for them to intercept
any L2C secure register write.

l2c_write_sec() avoids writes to secure registers which are already set
to the appropriate value, thus avoiding the overhead of needlessly
calling into the secure monitor.

Back in the mists of time, someone decided that it would be a good idea
to group like functions together - so all the save functions in one
place, all the resume functions in another, all the OF parsing functions
some place else.

This makes it difficult to get an overview on what a particular
implementation is doing - grouping an implementations specific functions
together makes more sense, because you can see what it's doing without
the clutter of other implementations.

Organise it according to implementation.

Rather than having a boolean and other tricks to disable some bits of
l2x0_init(), split this function into two parts: a common part shared
between OF and non-OF, and the non-OF part.

The common part can take a block of function pointers, and the cache
ID (to cope with Aurora's DT specified ID.)  Eliminate the redundant
setting of l2x0_base in the OF case, moving it to the non-OF init
function.

This allows us to localise the OF-specific initialisation handling
from the non-OF handling.

Victor Kamensky 72e6ae285a ARM: 8043/1: uprobes need icache flush after xol write
After instruction write into xol area, on ARM V7
architecture code need to flush dcache and icache to sync
them up for given set of addresses. Having just
'flush_dcache_page(page)' call is not enough - it is
possible to have stale instruction sitting in icache
for given xol area slot address.

Introduce arch_uprobe_ixol_copy weak function
that by default calls uprobes copy_to_page function and
than flush_dcache_page function and on ARM define new one
that handles xol slot copy in ARM specific way

flush_uprobe_xol_access function shares/reuses implementation
with/of flush_ptrace_access function and takes care of writing
instruction to user land address space on given variety of
different cache types on ARM CPUs. Because
flush_uprobe_xol_access does not have vma around
flush_ptrace_access was split into two parts. First that
retrieves set of condition from vma and common that receives
those conditions as flags.

Note ARM cache flush function need kernel address
through which instruction write happened, so instead
of using uprobes copy_to_page function changed
code to explicitly map page and do memcpy.

Note arch_uprobe_copy_ixol function, in similar way as
copy_to_user_page function, has preempt_disable/preempt_enable.

Russell King deace4a6b4 ARM: dma-mapping: avoid calling dma_cache_maint_page() on dev=>cpu
Avoid calling dma_cache_maint_page() when unmapping a DMA_TO_DEVICE
buffer.  The L1 cache ops never do anything in this circumstance, nor
do they ever need to - all that matters for this case is that the data
written is visible to the device before DMA starts.  What happens during
the transfer (provided the buffer is not written to) is of no real
consequence.

We already do this optimisation for the L2 cache.

Rob Herring d1552ce449 of/fdt: move memreserve and dtb memory reservations into core
Move the /memreserve/ processing and dtb memory reservations into
early_init_fdt_scan_reserved_mem. This converts arm, arm64, and powerpc
as they are the only users of early_init_fdt_scan_reserved_mem.

memblock_reserve is safe to call on the same region twice, so the
reservation check for the dtb in powerpc 32-bit reservations is safe to
remove.

The bug is happened in cpu_v7_set_pte_ext(ptep, pte):
An LPAE PTE is a 64bit quantity, passed to cpu_v7_set_pte_ext in the r2 and r3 registers.
On an LE kernel, r2 contains the LSB of the PTE, and r3 the MSB.
On a BE kernel, the assignment is reversed.

Unfortunately, the current code always assumes the LE case,
leading to corruption of the PTE when clearing/setting bits.

This patch fixes this issue much like it has been done already in the
cpu_v7_switch_mm case.

Liu Hua a05e54c103 ARM: 8031/2: change fixmap mapping region to support 32 CPUs
In 32-bit ARM systems, the fixmap mapping region can support no more
than 14 CPUs(total: 896k; one CPU: 64K). And we can configure NR_CPUS
up to 32. So there is a mismatch.

This patch moves fixmapping region downwards to region 0xffc00000-
0xffe00000. Then the fixmap mapping region can support up to 32 CPUs.

Liu Hua 4221e2e6b3 ARM: 8031/1: fixmap: remove FIX_KMAP_BEGIN and FIX_KMAP_END
It seems that these two macros are not used by non architecture
specific code. And on ARM FIX_KMAP_BEGIN equals zero.

This patch removes these two macros. Instead, using FIX_KMAP_NR_PTES to
tell the pte number belonged to fixmap mapping region. The code will
become clearer when I introduce a bugfix on fixmap mapping region.

Arnd Bergmann 027f3f9696 ARM: cache-tauros2: remove ARMv6 code
When building a kernel with support for both ARMv6 and ARMv7 but
no MMU, the call from tauros2_internal_init to adjust_cr causes
a link error. While that could probably be resolved, we don't
actually support cache-tauros2 on ARMv6 any more. All PJ4 CPU
implementations support both ARMv6 and ARMv7 and we already assume
that we are using them only in ARMv7 mode.

Removing the ARMv6 code path reduces the code size and avoids
the linker error.

Marek Szyprowski 68efd7d2fb arm: dma-mapping: remove order parameter from arm_iommu_create_mapping()
The 'order' parameter for IOMMU-aware dma-mapping implementation was
introduced mainly as a hack to reduce size of the bitmap used for
tracking IO virtual address space. Since now it is possible to dynamically
resize the bitmap, this hack is not needed and can be removed without any
impact on the client devices. This way the parameters for
arm_iommu_create_mapping() becomes much easier to understand. 'size'
parameter now means the maximum supported IO address space size.

The code will allocate (resize) bitmap in chunks, ensuring that a single
chunk is not larger than a single memory page to avoid unreliable
allocations of size larger than PAGE_SIZE in atomic context.

Andreas Herrmann 4d852ef8c2 arm: dma-mapping: Add support to extend DMA IOMMU mappings
Instead of using just one bitmap to keep track of IO virtual addresses
(handed out for IOMMU use) introduce an array of bitmaps. This allows
us to extend existing mappings when running out of iova space in the
initial mapping etc.

If there is not enough space in the mapping to service an IO virtual
address allocation request, __alloc_iova() tries to extend the mapping
-- by allocating another bitmap -- and makes another allocation
attempt using the freshly allocated bitmap.

This allows arm iommu drivers to start with a decent initial size when
an dma_iommu_mapping is created and still to avoid running out of IO
virtual addresses for the mapping.

Steven Capper 6ea41c8011 ARM: 7979/1: mm: Remove hugetlb warning from Coherent DMA allocator
The Coherant DMA allocator allocates pages of high order then splits
them up into smaller pages.

This splitting logic would run into problems if the allocator was
given compound pages. Thus the Coherant DMA allocator was originally
incompatible with compound pages existing and, by extension, huge
pages. A compile #error was put in place whenever huge pages were
enabled.

Compatibility with compound pages has since been introduced by the
following commit (which merely excludes GFP_COMP pages from being
requested by the coherant DMA allocator):

When huge page support was introduced to ARM, the compile #error in
dma-mapping.c was replaced by a #warning when it should have been
removed instead.

Will Deacon b6ccb9803e ARM: 7954/1: mm: remove remaining domain support from ARMv6
CPU_32v6 currently selects CPU_USE_DOMAINS if CPU_V6 and MMU. This is
because ARM 1136 r0pX CPUs lack the v6k extensions, and therefore do
not have hardware thread registers. The lack of these registers requires
the kernel to update the vectors page at each context switch in order to
write a new TLS pointer. This write must be done via the userspace
mapping, since aliasing caches can lead to expensive flushing when using
kmap. Finally, this requires the vectors page to be mapped r/w for
kernel and r/o for user, which has implications for things like put_user
which must trigger CoW appropriately when targetting user pages.

The upshot of all this is that a v6/v7 kernel makes use of domains to
segregate kernel and user memory accesses. This has the nasty
side-effect of making device mappings executable, which has been
observed to cause subtle bugs on recent cores (e.g. Cortex-A15
performing a speculative instruction fetch from the GIC and acking an
interrupt in the process).

This patch solves this problem by removing the remaining domain support
from ARMv6. A new memory type is added specifically for the vectors page
which allows that page (and only that page) to be mapped as user r/o,
kernel r/w. All other user r/o pages are mapped also as kernel r/o.

Jason Gunthorpe 0f054e3cea ARM: 7949/1: feroceon: Log a FW_BUG if the L2 cache is turned on at boot
Booting on feroceon CPUS requires the L2 cache to be turned off. With
some kernel configurations (notably CONFIG_ARM_PATCH_PHYS_VIRT
disabled) the kernel will boot even if the L2 is turned on.

However there may be subtle breakage, and when PATCH_PHYS_VIRT is
enabled it is very likely that booting with L2 will crash at early
boot before any kernel diagnostic output.

The diagnostic message is intended to discourage people from shipping
bootloaders that leave the L2 turned on.

The issue on feroceon is that the L2 is bypassed when the L1 caches
are disabled. So the decompressor will place parts of the kernel image
into the L2 and the early cache-off boot code in head.S will write to
parts of the kernel image, bypassing the L2 and creating inconsistency.

Tested on ARM Kirkwood.

The A12 behaves as the A7/A15 does with respect to setting the SMP bit, and
doesn't require TLB ops broadcasting to be explicitly enabled like the A9 does.

Note that as the ACTLR cannot (usually) be written from non-secure, it is the
responsibility of the bootloader/firmware to set this bit per core - it is
done here in Linux as last resort in case of bad firmware.

During __v{6,7}_setup, we invalidate the TLBs since we are about to
enable the MMU on return to head.S. Unfortunately, without a subsequent
dsb instruction, the invalidation is not guaranteed to have completed by
the time we write to the sctlr, potentially exposing us to junk/stale
translations cached in the TLB.

This patch reworks the init functions so that the dsb used to ensure
completion of cache/predictor maintenance is also used to ensure
completion of the TLB invalidation.

Christoffer Dall 4d9c5b89cf ARM: 7950/1: mm: Fix stage-2 device memory attributes
The stage-2 memory attributes are distinct from the Hyp memory
attributes and the Stage-1 memory attributes.  We were using the stage-1
memory attributes for stage-2 mappings causing device mappings to be
mapped as normal memory.  Add the S2 equivalent defines for memory
attributes and fix the comments explaining the defines while at it.

Add a prot_pte_s2 field to the mem_type struct and fill out the field
for device mappings accordingly.

Santosh Shilimkar cfb665864e arch/arm/mm/init.c: use memblock apis for early memory allocations
Switch to memblock interfaces for early memory allocator instead of
bootmem allocator.  No functional change in beahvior than what it is in
current code from bootmem users points of view.

Archs already converted to NO_BOOTMEM now directly use memblock
interfaces instead of bootmem wrappers build on top of memblock.  And
the archs which still uses bootmem, these new apis just fallback to
exiting bootmem APIs.

Mel Gorman aec6a8889a mm, show_mem: remove SHOW_MEM_FILTER_PAGE_COUNT
Commit 4b59e6c473 ("mm, show_mem: suppress page counts in
non-blockable contexts") introduced SHOW_MEM_FILTER_PAGE_COUNT to
suppress PFN walks on large memory machines.  Commit c78e93630d ("mm:
do not walk all of system memory during show_mem") avoided a PFN walk in
the generic show_mem helper which removes the requirement for
SHOW_MEM_FILTER_PAGE_COUNT in that case.

This patch removes PFN walkers from the arch-specific implementations
that report on a per-node or per-zone granularity.  ARM and unicore32
still do a PFN walk as they report memory usage on each bank which is a
much finer granularity where the debugging information may still be of
use.  As the remaining arches doing PFN walks have relatively small
amounts of memory, this patch simply removes SHOW_MEM_FILTER_PAGE_COUNT.

Will Deacon a7a0410506 ARM: 7925/1: mm: keep track of last ASID allocation to improve bitmap searching
Since we only clear entries in the ASID bitmap on a rollover event, the
bitmap tends to consist of a block of consecutive set bits followed by
a block of consecutive clear bits. The exception to this rule is for
ASIDs which have been carried over from a previous generation, but
these are bound by the number of CPUs.

This patch optimises our bitmap searching strategy, so that we search
from the last successful allocation, rather than search from index 1
each time we allocate a new ASID.

Will Deacon e1a5848e33 ARM: 7924/1: mm: don't bother with reserved ttbr0 when running with LPAE
With the new ASID allocation algorithm, active ASIDs at the time of a
rollover event will be marked as reserved, so active mm_structs can
continue to operate with the same ASID as before. This in turn means
that we don't need to worry about allocating a new ASID to an mm that
is currently active (installed in TTBR0).

Since updating the pgd and ASID is atomic on LPAE systems (by virtue of
the two being fields in the same hardware register), we can dispose of
the reserved TTBR0 and rely on whatever tables we currently have live.

Lorenzo Pieralisi 70f665fe77 ARM: 7919/1: mm: refactor v7 cache cleaning ops to use way/index sequence
Set-associative caches on all v7 implementations map the index bits
to physical addresses LSBs and tag bits to MSBs. As the last level
of cache on current and upcoming ARM systems grows in size,
this means that under normal DRAM controller configurations, the
current v7 cache flush routine using set/way operations triggers a
DRAM memory controller precharge/activate for every cache line
writeback since the cache routine cleans lines by first fixing the
index and then looping through ways (index bits are mapped to lower
physical addresses on all v7 cache implementations; this means that,
with last level cache sizes in the order of MBytes, lines belonging
to the same set but different ways map to different DRAM pages).

Given the random content of cache tags, swapping the order between
indexes and ways loops do not prevent DRAM pages precharge and
activate cycles but at least, on average, improves the chances that
either multiple lines hit the same page or multiple lines belong to
different DRAM banks, improving throughput significantly.

This patch swaps the inner loops in the v7 cache flushing routine
to carry out the clean operations first on all sets belonging to
a given way (looping through sets) and then decrementing the way.

Benchmarks showed that by swapping the ordering in which sets and
ways are decremented in the v7 cache flushing routine, that uses
set/way operations, time required to flush caches is reduced
significantly, owing to improved writebacks throughput to the DRAM
controller.

Benchmarks results vary and depend heavily on the last level of
cache tag RAM content when cache is cleaned and invalidated, ranging
from 2x throughput when all tag RAM entries contain dirty lines
mapping to sequential pages of RAM to 1x (ie no improvement) when
all tag RAM accesses trigger a DRAM precharge/activate cycle, as the
current code implies on most DRAM controller configurations.

Russell King ebd4922ecc ARM: implement basic NX support for kernel lowmem mappings
Add basic NX support for kernel lowmem mappings.  We mark any section
which does not overlap kernel text as non-executable, preventing it
from being used to write code and then execute directly from there.

This does not change the alignment of the sections, so the kernel
image doesn't grow significantly via this change, so we can do this
without needing a config option.

Peter reports that OMAP audio broke with the recent fix for these
checks, caused by OMAP audio using a 64-bit DMA mask.  We should
allow 64-bit DMA masks even with 32-bit dma_addr_t if we can be sure
the amount of RAM we have won't allow the 32-bit dma_addr_t to
overflow.  Unfortunately, the checks to detect overflow were not
correct.

reserve space for crash kernel to have one less dependency with nobootmem
allocator wrapper.

Grygorii Strashko 8e58caefd9 ARM: mm: Don't allow resizing of memblock data until "low" memory is not mapped
If allowed by call to memblock_allow_resize() - The Memblock core will
try to allocate additional memory and rearrange its internal data in
case, if there are more then INIT_MEMBLOCK_REGIONS(128) memory regions
of any type have been allocated. If this happens before Low memory is
mapped (which is done now by map_lowmem()) the system will hang, because
the Memblock core will try to operate with virtual addresses which
aren't mapped yet.

In ARM code, the memblock resizing is allowed (memblock_allow_resize())
from arm_memblock_init() which is called before map_lowmem(), so
this may lead to an error as described above.

Hence, allow Memblock resizing later during init, from bootmem_init()
when all appropriate mappings are ready.

Santosh Shilimkar b3ba41f28f ARM: mm: Fix max_mapnr with recent max*pfn updates
With commit  26ba47b1 {ARM: 7805/1: mm: change max*pfn to include
the physical offset of memory}, the max_pfn already contain
PHYS_PFN_OFFSET, so it shouldn't be taken into account again.

While at it, use use set_max_mapnr() helper.
