Different boards may use differenct UART ports for debugging.
To be able to add different debug UART configurations (e.g. UART2),
rename the existing general DEBUG_IMX6Q_UART to DEBUG_IMX6Q_UART4.

Now that the DEBUG_LL UART can be selected by a Kconfig choice,
simplify the #ifdefery in debug-macro.S and add entries to the
top-level Kconfig.debug instead.

Some platforms (like OMAP not to name it) are doing rather complicated
hacks just to determine the base UART address to use. Let's give their
addruart macro some slack by providing an extra work register which will
allow for much needed cleanups.
This is basically a no-op as this commit is only adding the extra argument
to the macro but no one is using it yet.

Commit fad107086d fixed the wrong test for MX51
as the MX51 addresses are wrong for MX50 and MX53 but now it's MX51 only,
UART_PADDR is not defined anymore when building for MX50/MX53.

Since support for mxc91231 was introduced 2009 it only saw patches that
were part of (mxc or arm) global cleanups. The only supported machine
only had 4 devices (2x UART, sdhc, watchdog).

Rather than checking the MMU status in every instance of addruart, do it
once in kernel/debug.S, and change the existing addruart macros to
return both physical and virtual addresses. The main debug code can then
select the appropriate address to use.
This will also allow us to retreive the address of a uart for the MMU
state that we're not current in.

Otherwise more complicated uart configuration won't be possible.
We can use r1 for tmp register for both head.S and debug.S.
NOTE: This patch depends on another patch to add the the tmp register
into all debug-macro.S files. That can be done with:
$ sed -i -e "s/addruart,rx|addruart, rx/addruart, rx, tmp/"
arch/arm/*/include/*/debug-macro.S

Apart from MXC91231_IO_ADDRESS itself this was the only usage of
MXC91231_AIPS1_IO_ADDRESS. Now MXC91231_IO_ADDRESS can be recoded with
IMX_IO_ADDRESS and all helper macros can go away.
