Add manual tuning function in CSR atlas7 SoC. It is mainly used
for the UHS-I SD card working SDR50 SDR104 mode.
The tuning principle can be seen in SD spec part1 v3.01 4.2.4.5
(tuning command).
SD host send the cmd19 and set the delay value(0-127).
and the sdcard return 64 bytes data. If the data is same with
the tuning data. The delay value is valid. Execute this commmand
128 times. And calculate the longest window of the valid values.
The value in the middle of this window is the best value.

We're running into cases where our enabling of the SDIO interrupt in

dw_mci_enable_sdio_irq() unsafely does a read-modify-write of the
INTMASK register.
We can't just use the standard host->lock since that lock is not irq
safe and mmc_signal_sdio_irq() (called from interrupt context) calls
dw_mci_enable_sdio_irq(). Add a new irq-safe lock to protect INTMASK.
An alternate solution to this is to punt mmc_signal_sdio_irq() to the
tasklet and then protect INTMASK modifications by the standard host
lock. This seemed like a bit more of a high-latency change.

In the patch (9623b5b mmc: dw_mmc: Disable low power mode if SDIO
interrupts are used) I added code that disabled the low power mode of
dw_mmc when SDIO interrupts are used. That code worked but always
felt a little hacky because we ended up disabling low power as a side
effect of the first enable_sdio_irq() call. That wouldn't be so bad
except that disabling low power involves a complicated process of
writing to the CMD/CMDARG registers and that extra process makes it
difficult to cleanly the read-modify-write race in
dw_mci_enable_sdio_irq() (see future patch in the series).

In (3fcb027 ARM: MXC: mxcmmc: work around a bug in the SDHC busy line
handling) the optional init_card() callback was added. According to
the original change it was "for now only called from
mmc_sdio_init_card()".
This callback really ought to be called from the SD and MMC init
functions as well. One current user of this callback
(mxcmci_init_card) will not work as expected if you insert an SDIO
card, then eject it and put a normal SD card in. Specifically the
normal SD card will not get to run with 4-bit data.
I'd like to use the init_card() callback to handle a similar quirk on
dw_mmc when using SDIO Interrupts (the "low power" feature of the card
needs to be disabled), so that will add a second user of the function.

Sleep in atomic context happened on Trats2 board after inserting or
removing SD card because mmc_gpio_get_cd() was called under spin lock.
Fix this by moving card detection earlier, before acquiring spin lock.
The mmc_gpio_get_cd() call does not have to be protected by spin lock
because it does not access any sdhci internal data.
The sdhci_do_get_cd() call access host flags (SDHCI_DEVICE_DEAD). After
moving it out side of spin lock it could theoretically race with driver
removal but still there is no actual protection against manual card

In commit 5491ce3f79 ("mmc: sdhci-pxav3: add support for the Armada
38x SDHCI controller"), the sdhci-pxav3 driver was extended to include
support for the SDHCI controller found in the Armada 38x
processor. This mainly involved adding some MBus window related
configuration.
However, this configuration is currently done too early in ->probe():
it is done before clocks are enabled, while this configuration
involves touching the registers of the controller, which will hang the
SoC if the clock is disabled. It wasn't noticed until now because the
bootloader typically leaves gatable clocks enabled, but in situations
where we have a deferred probe (due to a CD GPIO that cannot be taken,
for example), then the probe will be re-tried later, after a clock
disable has been done in the exit path of the failed probe attempt of
the device. This second probe() will hang the system due to the clock
being disabled.
This can for example be produced on Armada 385 GP, which has a CD GPIO
connected to an I2C PCA9555. If the driver for the PCA9555 is not
compiled into the kernel, then we will have the following sequence of
events:
1. The SDHCI probes
2. It does the MBus configuration (which works, because the clock is
left enabled by the bootloader)
3. It enables the clock
4. It tries to get the CD GPIO, which fails due to the driver being
missing, so -EPROBE_DEFER is returned.
5. Before returning -EPROBE_DEFER, the driver cleans up what was
done, which includes disabling the clock.
6. Later on, the SDHCI probe is tried again.
7. It does the MBus configuration, which hangs because the clock is
no longer enabled.
This commit does the obvious fix of doing the MBus configuration after
the clock has been enabled by the driver.

Re-tuning for HS400 mode must be done in HS200
mode. Currently there is no support for that.
That needs to be reflected in the code.
Specifically, if tuning is executed in HS400 mode
then return an error, and do not start the
tuning timer if HS200 tuning is being done prior
to switching to HS400.
Note that periodic re-tuning is not expected
to be needed for HS400 but re-tuning is still
needed after the host controller has lost power.
In the case of suspend/resume that is not necessary
because the card is fully re-initialised. That
just leaves runtime suspend/resume with no support

This time we have some more new material than we used to have during
the last couple of development cycles.
The most important part of it to me is the introduction of a unified
interface for accessing device properties provided by platform
firmware. It works with Device Trees and ACPI in a uniform way and
drivers using it need not worry about where the properties come
from as long as the platform firmware (either DT or ACPI) makes
them available. It covers both devices and "bare" device node
objects without struct device representation as that turns out to
be necessary in some cases. This has been in the works for quite
a few months (and development cycles) and has been approved by
all of the relevant maintainers.
On top of that, some drivers are switched over to the new interface
(at25, leds-gpio, gpio_keys_polled) and some additional changes are
made to the core GPIO subsystem to allow device drivers to manipulate
GPIOs in the "canonical" way on platforms that provide GPIO information
in their ACPI tables, but don't assign names to GPIO lines (in which
case the driver needs to do that on the basis of what it knows about
the device in question). That also has been approved by the GPIO
core maintainers and the rfkill driver is now going to use it.
Second is support for hardware P-states in the intel_pstate driver.
It uses CPUID to detect whether or not the feature is supported by
the processor in which case it will be enabled by default. However,
it can be disabled entirely from the kernel command line if necessary.
Next is support for a platform firmware interface based on ACPI
operation regions used by the PMIC (Power Management Integrated
Circuit) chips on the Intel Baytrail-T and Baytrail-T-CR platforms.
That interface is used for manipulating power resources and for
thermal management: sensor temperature reporting, trip point setting
and so on.
Also the ACPI core is now going to support the _DEP configuration
information in a limited way. Basically, _DEP it supposed to reflect
off-the-hierarchy dependencies between devices which may be very
indirect, like when AML for one device accesses locations in an
operation region handled by another device's driver (usually, the
device depended on this way is a serial bus or GPIO controller).
The support added this time is sufficient to make the ACPI battery
driver work on Asus T100A, but it is general enough to be able to
cover some other use cases in the future.
Finally, we have a new cpufreq driver for the Loongson1B processor.
In addition to the above, there are fixes and cleanups all over the
place as usual and a traditional ACPICA update to a recent upstream
release.
As far as the fixes go, the ACPI LPSS (Low-power Subsystem) driver
for Intel platforms should be able to handle power management of
the DMA engine correctly, the cpufreq-dt driver should interact
with the thermal subsystem in a better way and the ACPI backlight
driver should handle some more corner cases, among other things.
On top of the ACPICA update there are fixes for race conditions
in the ACPICA's interrupt handling code which might lead to some
random and strange looking failures on some systems.
In the cleanups department the most visible part is the series
of commits targeted at getting rid of the CONFIG_PM_RUNTIME
configuration option. That was triggered by a discussion
regarding the generic power domains code during which we realized
that trying to support certain combinations of PM config options
was painful and not really worth it, because nobody would use them
in production anyway. For this reason, we decided to make
CONFIG_PM_SLEEP select CONFIG_PM_RUNTIME and that lead to the
conclusion that the latter became redundant and CONFIG_PM could
be used instead of it. The material here makes that replacement
in a major part of the tree, but there will be at least one more
batch of that in the second part of the merge window.
Specifics:
- Support for retrieving device properties information from ACPI
_DSD device configuration objects and a unified device properties
interface for device drivers (and subsystems) on top of that.
As stated above, this works with Device Trees and ACPI and allows
device drivers to be written in a platform firmware (DT or ACPI)
agnostic way. The at25, leds-gpio and gpio_keys_polled drivers
are now going to use this new interface and the GPIO subsystem
is additionally modified to allow device drivers to assign names
to GPIO resources returned by ACPI _CRS objects (in case _DSD is
not present or does not provide the expected data). The changes
in this set are mostly from Mika Westerberg, Rafael J Wysocki,
Aaron Lu, and Darren Hart with some fixes from others (Fabio Estevam,
Geert Uytterhoeven).
- Support for Hardware Managed Performance States (HWP) as described
in Volume 3, section 14.4, of the Intel SDM in the intel_pstate
driver. CPUID is used to detect whether or not the feature is
supported by the processor. If supported, it will be enabled

- Fixes for race conditions in the ACPICA's interrupt handling

- ACPI core fix for an RCU-related issue in the ioremap() regions

been allowed to enter idle states even if they'd had RCU callbakcs
queued and triggered some problems in certain proprietary graphics

that code with synchronize_rcu_expedited() which makes the issue
go away. From Konstantin Khlebnikov.
- ACPI LPSS (Low-Power Subsystem) driver fix to handle power
management of the DMA engine included into the LPSS correctly.
The problem is that the DMA engine doesn't have ACPI PM support

having ACPI PM support goes into D3cold. To work around that,
the PM domain used by the ACPI LPSS driver is redesigned so at
least one device with ACPI PM support will be on as long as the
DMA engine is in use. From Andy Shevchenko.

Pull ACPI and power management updates from Rafael Wysocki:
"This time we have some more new material than we used to have during
the last couple of development cycles.
The most important part of it to me is the introduction of a unified
interface for accessing device properties provided by platform
firmware. It works with Device Trees and ACPI in a uniform way and
drivers using it need not worry about where the properties come from
as long as the platform firmware (either DT or ACPI) makes them
available. It covers both devices and "bare" device node objects
without struct device representation as that turns out to be necessary
in some cases. This has been in the works for quite a few months (and
development cycles) and has been approved by all of the relevant
maintainers.
On top of that, some drivers are switched over to the new interface
(at25, leds-gpio, gpio_keys_polled) and some additional changes are
made to the core GPIO subsystem to allow device drivers to manipulate
GPIOs in the "canonical" way on platforms that provide GPIO
information in their ACPI tables, but don't assign names to GPIO lines
(in which case the driver needs to do that on the basis of what it
knows about the device in question). That also has been approved by
the GPIO core maintainers and the rfkill driver is now going to use
it.
Second is support for hardware P-states in the intel_pstate driver.
It uses CPUID to detect whether or not the feature is supported by the
processor in which case it will be enabled by default. However, it
can be disabled entirely from the kernel command line if necessary.

The SET_PM_RUNTIME_PM_OPS() and SET_RUNTIME_PM_OPS() macros are

However, after commit b2b49ccbdd (PM: Kconfig: Set PM_RUNTIME if

of these macros is now redundant.
For this reason, replace SET_PM_RUNTIME_PM_OPS() with

Voltage Switch Procedure
This patch is to fix an issue found on mb86s7x platforms.
[symptom]
There are some UHS-1 SD memory cards sometimes cannot be detected correctly,
e.g., Transcend 600x SDXC 64GB UHS-1 memory card.
During Signal Voltage Switch Procedure, failure to switch is indicated
by the card holding DAT[3:0] low.
[analysis]
According to SD Host Controller Simplified Specification Version 3.00
chapter 3.6.1, the Signal Voltage Switch Procedure should be:
(1) Check S18A; (2) Issue CMD11; (3) Check CMD 11 response;
(4) Stop providing SD clock; (5) Check DAT[3:0] should be 0000b;
(6) Set 1.8V Signal Enable; (7) Wait 5ms; (8) Check 1.8V Signal Enable;
(9) Provide SD Clock; (10) Wait 1ms; (11) Check DAT[3:0] should be 1111b;
(12) error handling
With CONFIG_MMC_CLKGATE=y, sometimes there is one more gating/un-gating
SD clock between (2) and (3). In this case, some UHS-1 SD cards will hold
DAT[3:0] 0000b at (11) and thus fails Signal Voltage Switch Procedure.
[solution]
By mmc_host_clk_hold() before CMD11, the additional gating/un-gating
SD clock between (2) and (3) can be prevented and thus no failure at (11).
It has been verified with many UHS-1 SD cards on mb86s7x platforms and
works correctly.

Commit f1d2736c81 (mmc: dw_mmc: control card read threshold) added
dw_mci_ctrl_rd_thld() with an unconditional write to the CDTHRCTL
register at offset 0x100. However before version 240a, the FIFO region
started at 0x100, so the write messes with the FIFO and completely
breaks the driver.
If the version id < 240A, return early from dw_mci_ctl_rd_thld() so as
not to hit this problem.

In order to use the mvsdio driver for sdio, it has been necessary to
use a module parameter to disable DMA so to force PIO is used. It is
then possible to use wireless LAN devices like mwifiex found on
topkick and mirabox. However, accessing an MMC SD card does work with
DMA.
Investigation has shown that MMC block device accesses are always
aligned to 64 byte boundaries, where as transfers from mwifiex are
rarely more than word aligned. It has also been determined that card
to host transfers work with DMA for SDIO devices, but host to card
transfers with DMA have problems.
This patch extends the current checks for buffers which are not word
aligned or multiple of words. All host to card transfers which are not
64 byte aligned are now also performed via PIO. This should not affect
the performance of SD cards, but allow sdio devices to work out of the
box, and they are likely to be more efficient since DMA will be used
for card to host transfers.

Currently the driver imposes a limit of 256 total minor numbers,
apparently based on the historic Unix/Linux limit. This is quite
restrictive, particularly if we raise the maximum number of
partitions per card to 256 to match sd.
In order to make the full minor number space available we would
have to replace the static dev_use and name_use arrays with struct
ida. But we can at least allow use of 256 cards rather than just
256 minors, with only a small change.

mmc_do_hw_reset(), mmc_power_up() and mmc_power_off() all set similar
initial values for bus_mode, bus_width, chip_select and timing. Let's
make this handling simpler and more consistent by sticking them
together in a common function. This will introduce small changes in
behavior in the following places:
mmc_power_off():
For SPI hosts, explicitly set bus_mode = MMC_BUSMODE_PUSHPULL and
chip_select = MMC_CS_HIGH, before we left them as they were.
For non-SPI hosts, set bus_mode = MMC_BUSMODE_PUSHPULL instead of
MMC_BUSMODE_OPENDRAIN as before.
These two changes should not be a problem since the device will be
powered off anyway.
mmc_do_hw_reset():
Always set bus_mode = MMC_BUSMODE_PUSHPULL, as required by SD/SDIO
cards. MMC cards require MMC_BUSMODE_OPENDRAIN, but this is taken
care of by mmc_init_card() and mmc_attach_mmc().

omap_hsmmc supports only one slot per controller, see OMAP_MMC_MAX_SLOTS.
This unnecessary indirection leads to confusion in the omap_hsmmc driver.
For example the card_detect callback is not installed by platform code
but from the driver probe function. So it should be a field of
omap_hsmmc_host. But since it is declared under the platform slot while
the drivers struct omap_hsmmc_host has no slot abstraction, this looks
like a bug, especially when not familiar that this driver only supports

The dw_mmc driver had a bunch of code that ran whenever a card was
ejected and inserted. However, this code was old and crufty and
should be removed. Some evidence that it's really not needed:
1. Is is supposed to be legal to use 'cd-gpio' on dw_mmc instead of
using the built-in card detect mechanism. The 'cd-gpio' code
doesn't run any of the crufty old code but yet still works.
2. While looking at this, I realized that my old change (369ac86 mmc:
dw_mmc: don't queue up a card detect at slot startup) actually
castrated the old code a little bit already and nobody noticed.
Specifically "last_detect_state" was left as 0 at bootup. That
means that on the first card removal none of the crufty code ran.
3. I can run "while true; do dd if=/dev/mmcblk1 of=/dev/null; done"
while ejecting and inserting an SD Card and the world doesn't
explode.
If some of the crufty old code is actually needed, we should justify
it and also put it in some place where it will be run even with
"cd-gpio".
Note that in my case I'm using the "cd-gpio" mechanism but for various
reasons the hardware triggers a dw_mmc "card detect" at bootup. That
was actually causing a real bug. The card detect workqueue was
running while the system was trying to enumerate the card. The
"present != slot->last_detect_state" triggered and we were doing all
kinds of crazy stuff and messing up enumeration. The new mechanism of
just asking the core to check the card is much safer and then the
bogus interrupt doesn't hurt.

We've already got a reset of DMA after it's done. Add one before we
start DMA too. This fixes a data corruption on Rockchip SoCs which
will get bad data when doing a DMA transfer after doing a PIO transfer.
We tested this on an Exynos 5800 with HS200 and didn't notice any
difference in sequential read throughput.

In most of the cases mmc_get|set_drvdata() didn't simplify code, which
should be the primary reason for such macros.
Let's remove them and convert to the common device_driver macros,
dev_set|get_drvdata() instead.

The struct mmc_driver adds an extra layer on top of the struct
device_driver. That would be fine, if there were a good reason, but
that's not the case.
Let's simplify code by converting to the common struct device_driver
instead and thus also removing superfluous overhead.

added MMC_DDR52 as eMMC's DDR mode to be distinguished from SD-UHS.
While the differentation may be useful, pxav3 SDHCI controller lacks

While starting the bkops the previously prepared request should be canceled
and restarted after the bkops. As the prepared resource might already
setup the dma channels and ready to be started. Now with the arrival of bkops
request this prepared request can be serviced ONLY after the bkops. So
holding on to the prepared request in the host driver is confusing at
this point in time, so it makes sense to cleanup such dangling requests and
reissue this request once bkops is done.
Canceling the prepared request would give opportunity to the host drivers
to perform cleanup on the prepared request.
Without this patch host drivers like mmci gets confused when a blocking
request like send_ext_csd(CMD8) is issued while there is already a prepared
request. With the help of this patch, the driver can better manage such
blocking requests and cleanup the prepared requests which are not started yet.
Without this patch I hit below crash on Qualcomm APQ8064 based IFC6410 board
with mmci host driver.

go to "err" label, which skips removing of newly allocated card structure,
that will actually not be used. Fix that by using proper "free_card" label.

Add the case of SET_BLOCK_COUNT command error to the error conditions
check for making a controller reset at request handling finish.
Otherwise, if the SET_BLOCK_COUNT command failed, e.g. with a timeout,
the controller state was not reset, and the next command failed too.
In the case of data error the controller reset is already done in
finish_data() function before sending stop command (if present),
so the finish tasklet should make a reset after data error only
if no stop command existed in the request.
Also, fix the indentation of this condition check to make it more logical.

As a follow-up of commit
"mmc: sdhci: Balance vmmc regulator_enable(), and always enable vqmmc"
vmmc regulator disable is also not needed in sdhci_remove_host.
The regulator is completely controlled by mmc_power_up and mmc_power_off

Extra regulator_disable call in sdhci_remove_host is unbalanced and

Since commit 89168b4899 ("mmc: core: restore detect line inversion
semantics"), the SD card on i.MX28 (and possibly other) devices isn't
detected and booting stops at:
[ 4.120617] Waiting for root device /dev/mmcblk0p3...
This is caused by the MMC_CAP2_CD_ACTIVE_HIGH flag being set incorrectly
when the host controller doesn't use a GPIO for card detection (but
instead uses a dedicated pin). In this case mmc_gpiod_request_cd() will
return before assigning to the gpio_invert variable, leaving the
variable uninitialized. The variable then gets used to set the flag.
This patch fixes the issue by making sure gpio_invert is set to false
when a GPIO isn't used. After this patch, i.MX28 boots fine.
The MMC_CAP2_RO_ACTIVE_HIGH (write protect) flag is also set incorrectly
for the exact same reason (it uses the same uninitialized variable), so
this patch fixes that too.

Braswell eMMC host controller specifies an incorrect
timeout clock frequncy in the capabilities registers.
The correct value is 1 MHz. A similar fix was done
for sdhci-pci, however in the sdhci-acpi case the
HID/UID is not unique so the capabilities register
values are matched also.

Clear QUEUE_FLAG_ADD_RANDOM in all block drivers that set
QUEUE_FLAG_NONROT.
Historically, all block devices have automatically made entropy
contributions. But as previously stated in commit e2e1a148 ("block: add
sysfs knob for turning off disk entropy contributions"):
- On SSD disks, the completion times aren't as random as they
are for rotational drives. So it's questionable whether they
should contribute to the random pool in the first place.
- Calling add_disk_randomness() has a lot of overhead.
There are more reliable sources for randomness than non-rotational block
devices. From a security perspective it is better to err on the side of
caution than to allow entropy contributions from unreliable "random"
sources.

"mmc: host: switch OF parser to use gpio descriptors"
switched the semantic behaviour of card detect and read
only flags such that the inversion capability flag would
only be set if inversion was explicitly specified in the
device tree, in the hopes that no-one was using double
inversion.
It turns out that the XOR:ing between the explicit
inversion was indeed in use, so we need to restore the
old semantics where both ways of inversion are checked
and the end result XOR:ed.

As of 528bc7808f ("mmc: atmel-mci: Release mmc resources on failure in probe"),
the atmci_probe() function calls atmci_cleanup_slot in the failure path.
This causes a new warning whenever the driver is built:

As soon as the CD IRQ is requested, it can trigger, since it's an
externally controlled event. If it does, delayed_work host->detect will
be scheduled.
Many host controller probe()s are roughly structured as:
*_probe() {
host = sdhci_pltfm_init();
mmc_of_parse(host->mmc);
rc = sdhci_add_host(host);
if (rc) {
sdhci_pltfm_free();
return rc;
}
In 3.17, CD IRQs can are enabled quite early via *_probe() ->
mmc_of_parse() -> mmc_gpio_request_cd() -> mmc_gpiod_request_cd_irq().
Note that in linux-next, mmc_of_parse() calls mmc_gpio*d*_request_cd()
rather than mmc_gpio_request_cd(), and mmc_gpio*d*_request_cd() doesn't
call mmc_gpiod_request_cd_irq(). However, this issue still exists if
mmc_gpio_request_cd() is called directly before mmc_start_host().
sdhci_add_host() may fail part way through (e.g. due to deferred
probe for a vmmc regulator), and sdhci_pltfm_free() does nothing to
unrequest the CD IRQ nor cancel the delayed_work. sdhci_pltfm_free() is
coded to assume that if sdhci_add_host() failed, then the delayed_work
cannot (or should not) have been triggered.
This can lead to the following with CONFIG_DEBUG_OBJECTS_* enabled, when
kfree(host) is eventually called inside sdhci_pltfm_free():
WARNING: CPU: 2 PID: 6 at lib/debugobjects.c:263 debug_print_object+0x8c/0xb4()
ODEBUG: free active (active state 0) object type: timer_list hint: delayed_work_timer_fn+0x0/0x18
The object being complained about is host->detect.
There's no need to request the CD IRQ so early; mmc_start_host() already
requests it. For most SDHCI hosts at least, the typical call path that
does this is: *_probe() -> sdhci_add_host() -> mmc_add_host() ->
mmc_start_host(). Therefore, remove the call to mmc_gpiod_request_cd_irq()
from mmc_gpio_request_cd(). This also matches mmc_gpio*d*_request_cd(),
which already doesn't call mmc_gpiod_request_cd_irq().
However, some host controller drivers call mmc_gpio_request_cd() after
mmc_start_host() has already been called, and assume that this will also
call mmc_gpiod_request_cd_irq(). Update those drivers to explicitly call
mmc_gpiod_request_cd_irq() themselves. Ideally, these drivers should be
modified to move their call to mmc_gpio_request_cd() before their call
to mmc_add_host(). However that's too large a change for stable.
This solves the problem (eliminates the kernel error message above),
since it guarantees that the IRQ can't trigger before mmc_start_host()
is called.
The critical point here is that once sdhci_add_host() calls
mmc_add_host() -> mmc_start_host(), sdhci_add_host() is coded not to
fail. In other words, if there's a chance that mmc_start_host() may have
been called, and CD IRQs triggered, and the delayed_work scheduled,
sdhci_add_host() won't fail, and so cleanup is no longer via
sdhci_pltfm_free() (which doesn't free the IRQ or cancel the work queue)
but instead must be via sdhci_remove_host(), which calls mmc_remove_host()
-> mmc_stop_host(), which does free the IRQ and cancel the work queue.

Currently the MMCI driver will only handle GPIO descriptors
implicitly through the device tree probe glue in mmc_of_init(),
but devices instatiated other ways such as through board files
and passing descriptors using the GPIO descriptor table will
not be able to exploit descriptors.
Augment the driver to look for a GPIO descriptor if device
tree is not used for the device, and if that doesn't work,
fall back to platform data GPIO assignment using the old
API. The end goal is to get rid of the platform data integer
GPIO assingments from the kernel.

else if (host and device can both support 1.8v IO)

It is fully legal for a controller to start handling busy-end interrupt
before it has signaled that the command has completed. So make sure
we do things in the proper order, Or it results that command interrupt

The default sdhci driver write 0xE into timeout counter register to
set the maximum timeout. The value is not correct for uSDHC since the
max counter value for uSDHC is 0xF.
Instead of using common timeout code in sdhci, we implement esdhc_set_timeout
to handle the difference between eSDHC and uSDHC.
Currently we simply set the max timeout value as before.
But in the future, we probably may implement IMX specific timeout
setting algorithm and use suitable timeout for different CMDs.

The operation conditions register (OCR) stores the voltage
profile of the card, however the list of possible voltages
is restricted by the voltage range supported by the supply
used as VCC/VDD. So in mmc_vddrange_to_ocrmask() a OCR mask
is obtained to filter the not supported voltages, from the
value read in the host controller OCR register.
For fixed regulators, regulator_list_voltage() returns the
fixed output for the first selector but this doesn't happen
for switch (FET) regulators that obtain their voltage from
their parent supply. A call to regulator_get_voltage() is
needed in this case so the regulator core can return the
FET's parent supply voltage output.
This change is consistent with the fact that for other
fixed regulators (that are not FETs) the OCR mask is
returned even when mmc_regulator_set_ocr() checks if the
regulator is fixed before calling regulator_set_voltage().
Without this patch, the following warning is reported when
a FET is used as a vmmc-supply:

Some eMMC and SD cards implement a DSR register that allows to tune
raise/fall times and drive strength of the CMD and DATA outputs.
The values to use depend on the card in use and the host.
It might be needed to reduce the drive strength to prevent voltage peaks
above the host's specification.
Implement a 'dsr' devicetree property that allows to specify the value
to set the DSR to. For non-dt setups the new members of mmc_host can be
set by board code.
