pci_get_bus_and_slot() is restrictive such that it assumes domain=0 as
where a PCI device is present. This restricts the device drivers to be
reused for other domain numbers.
Getting ready to remove pci_get_bus_and_slot() function in favor of
pci_get_domain_bus_and_slot().
Replace pci_get_bus_and_slot() with pci_get_domain_bus_and_slot()
and extract the domain number from struct pci_dev.

The changes introduced through commit 82ed4db499 assume that the
sense buffer pointer in struct scsi_request is initialized for all
requests - passthrough and filesystem requests. Hence make sure
that that pointer is initialized for filesystem requests. Remove
the memset() call that clears .cmd because the scsi_req_init()
call in ide_initialize_rq() already initializes the .cmd.

The 0day bot reports the below failure which happens occasionally, with
their randconfig testing (once every ~100 boots). The Code points at
the private pointer ->driver_data being NULL, which hints at a race of
sorts where the private driver_data descriptor has disappeared by the
time we get to run the workqueue.
So let's check that pointer before we continue with issuing the command
to the drive.
This fix is of the brown paper bag nature but considering that IDE is
long deprecated, let's do that so that random testing which happens to
enable CONFIG_IDE during randconfig builds, doesn't fail because of
this.
Besides, failing the TEST_UNIT_READY command because the drive private
data is gone is something which we could simply do anyway, to denote
that there was a problem communicating with the device.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.

files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Instead of referring from inside drivers/cdrom/Makefile to all the
drivers that use this driver, let these drivers select the cdrom
driver. This change makes the cdrom build code follow the approach
that is used for most other drivers, namely refer from the higher
layers to the lower layer instead of from the lower layer to the
higher layers.

Since we split the scsi_request out of struct request, while the
standard prep_rq_fn builds 10 byte cmds, it missed to invoke
scsi_req_init() to initialize certain fields of a scsi_request
structure (.__cmd[], .cmd, .cmd_len and .sense_len but no other
members of struct scsi_request).

We used to assign IRQs for all devices at boot-time, before any drivers
claimed devices. The following commits:
30fdfb929e ("PCI: Add a call to pci_assign_irq() in pci_device_probe()")
0e4c2eeb75 ("alpha/PCI: Replace pci_fixup_irqs() call with host bridge IRQ mapping hooks")
changed this so we now call pci_assign_irq() from pci_device_probe() when
we call a driver's probe method.
The ide_scan_pcibus() path (enabled by CONFIG_IDEPCI_PCIBUS_ORDER) bypasses
pci_device_probe() so it can guarantee devices are claimed in order of PCI
bus address. It calls the driver's probe method directly, so it misses the
pci_assign_irq() call (and other PCI initialization functions), which
causes failures like this:

Fix the IRQ allocation issue by calling pci_assign_irq() from
ide_scan_pcidev() before probing the IDE PCI drivers, so that IRQs for a
given PCI device are allocated for the IDE PCI drivers to use them for

Instead of explicitly calling scsi_req_init() after blk_get_request(),
call that function from inside blk_get_request(). Add an
.initialize_rq_fn() callback function to the block drivers that need
it. Merge the IDE .init_rq_fn() function into .initialize_rq_fn()
because it is too small to keep it as a separate function. Keep the
scsi_req_init() call in ide_prep_sense() because it follows a
blk_rq_init() call.

Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings. This patch
instead introduces a new blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning. Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.
For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.
blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

From the context where a SCSI command is submitted it is not always
possible to figure out whether or not the queue the command is
submitted to has struct scsi_request as the first member of its
private data. Hence introduce the flag QUEUE_FLAG_SCSI_PASSTHROUGH.

The parisc architecture recently reimplemented the memcpy function and
their reimplementation crashed when source and destination overlapped.
The crash happened in the function ide_complete_cmd where memcpy is called
with the same source and destination pointer. According to the C
specification, memcpy behavior is undefined if the source and destination
range overlaps. This patches fixes the undefined behavior.

ide_pm_execute_rq exectures a PM request synchronously, and in the failure
case where it calls __blk_end_request_all it never checks the error field
passed to the end_io callback, so don't bother setting it.

This passes on the scsi_cmnd result field to users of passthrough
requests. Currently we abuse req->errors for this purpose, but that
field will go away in its current form.
Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it. I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

The function only returns -EIO if rq->errors is non-zero, which is not
very useful and lets a large number of callers ignore the return value.
Just let the callers figure out their error themselves.

We are going to split <linux/sched/task_stack.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/task_stack.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.

The object palm_bk3710_port_info of type ide_port_info is never
referenced anywhere after initialization by palm_bk3710_probe. It is
also passed as a parameter to ide_host_add which is called from the init
function but this call doesn't store the object reference anywhere, and
it only dereferences the values of the fields. Therefore add __initdata
to its declaration.

Since function tables are a common target for attackers, it's best to keep
them in read-only memory. As such, this makes the CDROM device ops tables
const. This drops additionally n_minors, since it isn't used meaningfully,
and sets the only user of cdrom_dummy_generic_packet explicitly so the
variables can all be const.

Instead of keeping two levels of indirection for requests types, fold it
all into the operations. The little caveat here is that previously
cmd_type only applied to struct request, while the request and bio op
fields were set to plain REQ_OP_READ/WRITE even for passthrough
operations.
Instead this patch adds new REQ_OP_* for SCSI passthrough and driver
private requests, althought it has to add two for each so that we
can communicate the data in/out nature of the request.

Currently the legacy ide driver defines several request types of it's own,
which is in the way of removing that field entirely.
Instead add a type field to struct ide_request and use that to distinguish
the different types of IDE-internal requests.
It's a bit of a mess, but so is the surrounding code..

We only need this code to support scsi, ide, cciss and virtio. And at
least for virtio it's a deprecated feature to start with.
This should shrink the kernel size for embedded device that only use,
say eMMC a bit.

This humble patch was sent one or two months before, and had no actions,
except for a colleague reply which friendly pointed out some formatting
problems (which were solved in a second message).
It relates to an old code, the legacy IDE driver, but the bug it
addresses is real. The code, although rarely used, is
still there to be compiled if one chooses to do so (like me).
Also, the fix has a very low risk of present collateral effects IMHO.
It is already compiled and tested in some embedded machines.
So, again IMHO, it is worth be fixed.
This email is a second trial with it. I hope it can help the one or two
guys out there which are still running the legacy IDE driver and
haven't noticed the former email.

A lot of the REQ_* flags are only used on struct requests, and only of
use to the block layer and a few drivers that dig into struct request
internals.
This patch adds a new req_flags_t rq_flags field to struct request for
them, and thus dramatically shrinks the number of common requests. It
also removes the unfortunate situation where we have to fit the fields
from the same enum into 32 bits for struct bio and 64 bits for
struct request.

There was clearly supposed to be a break statement here. Currently we
use the k2 ata timings instead of sh ata ones we intended. Probably no
one has this hardware anymore so it likely doesn't make a difference
beyond the static checker warning.

According to the HPT366 data sheet, PCI config space dword 0x40-0x43
bits 11:8 specify the primary drive cmd_high_time, however,
currently just 3 bits of the 4 are being used because the mask
is 0x07 and not 0x0f. Fix the mask, allowing for the 40MHz clock
to be detected.
Also add in missing space between switch and parenthesis to clean
up a checkpatch warning.

Add __init attribute on a function that is only called from other __init
functions and that is not inlined, at least with gcc version 4.8.4 on an
x86 machine with allyesconfig. Currently, the function is put in the
.text.unlikely segment. Declaring it as __init will cause it to be put in
the .init.text and to disappear after initialization.
The result of objdump -x on the function before the change is as follows:

Done with the help of Coccinelle. The semantic patch checks for local

This patch converts the IDE specific LED trigger to a generic disk
activity LED trigger. The libata core is now a trigger source just
like before the IDE disk driver. It's merely a replacement of the
string ide by disk.
The patch is taken from http://dev.gentoo.org/~josejx/ata.patch and is
widely used by any ibook/powerbook owners with great satisfaction.
Likewise, it is very often used successfully on different ARM platforms.
Unlike the original patch, the existing 'ide-disk' trigger is still
available for backward compatibility. That reduce the amount of patches
in affected device trees out of the mainline kernel. For further
development, the new name 'disk-activity' should be used.

The icside_probe function can be called for hotplugged devices, so
its reference to the __initconst icside_v6_port_info variable
may be invalid, as Kbuild points out:

Interestingly, this problem only shows up with clang but not
with gcc, which optimizes out the __initconst variable.

Drivers should include asm/pci-bridge.h only when they need the arch-
specific things provided there. Outside of the arch/ directories, the only
drivers that actually need things provided by asm/pci-bridge.h are the
powerpc RPA hotplug drivers in drivers/pci/hotplug/rpa*.
Remove the includes of asm/pci-bridge.h from the other drivers, adding an
include of linux/pci.h if necessary.

The Kconfig for this support is currently:

...meaning that it currently is not being built as a module by anyone.
Lets change the initcall to be the equivalent device_initcall, so that
when reading the driver code, there is no doubt it is builtin-only.
Unlike other similar changes, we leave the module.h header to be
included since this code interacts with other drivers and needs to
know what a struct module is.

Back in the day we used to just say this code was root only so it was
ok that the bounds checking was sloppy. These days it annoys static
checkers so we fix it.
In the original code "c > INT_MAX" was never true since "c" was an int.
I am not sure what was intended so I left it alone. But because I made
"c" unsigned it means we don't have a warning any more.
The second warning is that we cap "i" but allow negatives leading to an
underflow of the ide_disks_chs[] array. The third set of warnings is
because these values come from the user and we cap most of the upper
bounds but allow negative values. Negative cylinders doesn't make
sense.

__GFP_WAIT was used to signal that the caller was in atomic context and
could not sleep. Now it is possible to distinguish between true atomic
context and callers that are not willing to sleep. The latter should
clear __GFP_DIRECT_RECLAIM so kswapd will still wake. As clearing
__GFP_WAIT behaves differently, there is a risk that people will clear the
wrong flags. This patch renames __GFP_WAIT to __GFP_RECLAIM to clearly
indicate what it does -- setting it allows all reclaim activity, clearing
them prevents it.

This driver uses 'struct timeval' which we are trying to remove since
32 bit time types will break in the year 2038 by replacing it with
ktime_t.
This patch changes do_gettimeofday() to ktime_get() because
ktime_get() returns a ktime_t while do_gettimeofday() returns struct
timeval.
This patch also uses ktime_us_delta() to get the elapsed time.

speed module address lookup. He found some abusers of the module lock
doing that too.
A little bit of parameter work here too; including Dan Streetman's breaking
up the big param mutex so writing a parameter can load another module (yeah,
really). Unfortunately that broke the usual suspects, !CONFIG_MODULES and
!CONFIG_SYSFS, so those fixes were appended too.

Most code already uses consts for the struct kernel_param_ops,
sweep the kernel for the last offending stragglers. Other than
include/linux/moduleparam.h and kernel/params.c all other changes
were generated with the following Coccinelle SmPL patch. Merge
conflicts between trees can be handled with Coccinelle.
In the future git could get Coccinelle merge support to deal with
patch --> fail --> grammar --> Coccinelle --> new patch conflicts
automatically for us on patches where the grammar is available and
the patch is of high confidence. Consider this a feature request.

ppc has special instruction forms to efficiently load and store values
in non-native endianness. These can be accessed via the arch-specific
{ld,st}_le{16,32}() inlines in arch/powerpc/include/asm/swab.h.
However, gcc is perfectly capable of generating the byte-reversing
load/store instructions when using the normal, generic cpu_to_le*() and
le*_to_cpu() functions eaning the arch-specific functions don't have much
point.
Worse the "le" in the names of the arch specific functions is now
misleading, because they always generate byte-reversing forms, but some
ppc machines can now run a little-endian kernel.
To start getting rid of the arch-specific forms, this patch removes them
from all the old Power Macintosh drivers, replacing them with the
generic byteswappers.

Use jiffies_to_msecs for converting jiffies as it handles all of the corner
cases reliably and also helps readability. The printk format is fixed up
as jiffies_to_msecs returns unsigned int not unsigned long.

drivers. They touch a lot of files, but they are "simple" changes, just
removing a line in a structure.
Other than that, a few minor driver core and debugfs changes. There are
some ath9k patches coming in through this tree that have been acked by
the wireless maintainers as they relied on the debugfs changes.
Everything has been in linux-next for a while.

Clear QUEUE_FLAG_ADD_RANDOM in all block drivers that set
QUEUE_FLAG_NONROT.
Historically, all block devices have automatically made entropy
contributions. But as previously stated in commit e2e1a148 ("block: add
sysfs knob for turning off disk entropy contributions"):
- On SSD disks, the completion times aren't as random as they
are for rotational drives. So it's questionable whether they
should contribute to the random pool in the first place.
- Calling add_disk_randomness() has a lot of overhead.
There are more reliable sources for randomness than non-rotational block
devices. From a security perspective it is better to err on the side of
caution than to allow entropy contributions from unreliable "random"
sources.

The blk_get_request function may fail in low-memory conditions or during
device removal (even if __GFP_WAIT is set). To distinguish between these
errors, modify the blk_get_request call stack to return the appropriate
ERR_PTR. Verify that all callers check the return status and consider
IS_ERR instead of a simple NULL pointer check.
For consistency, make a similar change to the blk_mq_alloc_request leg
of blk_get_request. It may fail if the queue is dead, or the caller was
unwilling to wait.

This was used in the olden days, back when onions were proper
yellow. Basically it mapped to the current buffer to be
transferred. With highmem being added more than a decade ago,
most drivers map pages out of a bio, and rq->buffer isn't
pointing at anything valid.
Convert old style drivers to just use bio_data().
For the discard payload use case, just reference the page
in the bio.

As far as I know, the CS5520 and CS5530 chipsets were only used with
32-bit x86 Geode processors, so I think their drivers are only needed
on this architecture, except for build testing purpose.
While we're here, simplify the dependencies for the CS5535 driver.

On m68k, host->get_lock is used to both lock and register the interrupt
that the IDE host shares with other device drivers. Registering the
IDE interrupt handler in ide-probe.c results in duplicating the
interrupt registered (once via host->get lock, and also via init_irq()),
and may result in IDE accepting interrupts even when another driver has
locked the interrupt hardware. This opens the whole locking scheme up
to races.
host->get_lock is set on m68k only, so other drivers' behaviour is not
changed.

Replace direct inclusions of <acpi/acpi.h>, <acpi/acpi_bus.h> and
<acpi/acpi_drivers.h>, which are incorrect, with <linux/acpi.h>
inclusions and remove some inclusions of those files that aren't
necessary.
First of all, <acpi/acpi.h>, <acpi/acpi_bus.h> and <acpi/acpi_drivers.h>
should not be included directly from any files that are built for
CONFIG_ACPI unset, because that generally leads to build warnings about
undefined symbols in !CONFIG_ACPI builds. For CONFIG_ACPI set,
<linux/acpi.h> includes those files and for CONFIG_ACPI unset it
provides stub ACPI symbols to be used in that case.
Second, there are ordering dependencies between those files that always
have to be met. Namely, it is required that <acpi/acpi_bus.h> be included
prior to <acpi/acpi_drivers.h> so that the acpi_pci_root declarations the
latter depends on are always there. And <acpi/acpi.h> which provides
basic ACPICA type declarations should always be included prior to any other
ACPI headers in CONFIG_ACPI builds. That also is taken care of including
<linux/acpi.h> as appropriate.

ZTWO_VADDR() converts from physical to virtual I/O addresses, so it should
return "void __iomem *" instead of "unsigned long".
This allows to drop several casts, but requires adding a few casts to
accomodate legacy driver frameworks that store "unsigned long" I/O

Modify struct acpi_dev_node to contain a pointer to struct acpi_device
associated with the given device object (that is, its ACPI companion
device) instead of an ACPI handle corresponding to it. Introduce two
new macros for manipulating that pointer in a CONFIG_ACPI-safe way,
ACPI_COMPANION() and ACPI_COMPANION_SET(), and rework the
ACPI_HANDLE() macro to take the above changes into account.
Drop the ACPI_HANDLE_SET() macro entirely and rework its users to
use ACPI_COMPANION_SET() instead. For some of them who used to
pass the result of acpi_get_child() directly to ACPI_HANDLE_SET()
introduce a helper routine acpi_preset_companion() doing an
equivalent thing.
The main motivation for doing this is that there are things
represented by struct acpi_device objects that don't have valid
ACPI handles (so called fixed ACPI hardware features, such as
power and sleep buttons) and we would like to create platform
device objects for them and "glue" them to their ACPI companions
in the usual way (which currently is impossible due to the
lack of valid ACPI handles). However, there are more reasons
why it may be useful.
First, struct acpi_device pointers allow of much better type checking
than void pointers which are ACPI handles, so it should be more
difficult to write buggy code using modified struct acpi_dev_node
and the new macros. Second, the change should help to reduce (over
time) the number of places in which the result of ACPI_HANDLE() is
passed to acpi_bus_get_device() in order to obtain a pointer to the
struct acpi_device associated with the given "physical" device,
because now that pointer is returned by ACPI_COMPANION() directly.
Finally, the change should make it easier to write generic code that
will build both for CONFIG_ACPI set and unset without adding explicit
compiler directives to it.

There are several places in the tree where ACPI_STATE_D3 is used
instead of ACPI_STATE_D3_COLD which should be used instead for
clarity. Modify them all to use ACPI_STATE_D3_COLD as appropriate.
[The definition of ACPI_STATE_D3 itself cannot go away at this point
as it is part of ACPICA.]

Calling dev_set_name with a single paramter causes it to be handled as a
format string. Many callers are passing potentially dynamic string
content, so use "%s" in those cases to avoid any potential accidents,
including wrappers like device_create*() and bdi_register().

The only part of proc_dir_entry the code outside of fs/proc
really cares about is PDE(inode)->data. Provide a helper
for that; static inline for now, eventually will be moved
to fs/proc, along with the knowledge of struct proc_dir_entry
layout.

The CONFIG_EXPERIMENTAL config item has not carried much meaning for a
while now and is almost always enabled by default. As agreed during the
Linux kernel summit, remove it from any "depends on" lines in Kconfigs.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

This patch fixes a regresion introduced by commit 0998d063 (device-core: Ensure
drvdata = NULL when no driver is bound).
Suspend oopses in generic_ide_suspend() because dev_get_drvdata()
returns NULL (dev->p->driver_data == NULL) and this function is not
prepared for this.
Fix is based on Alan Stern's suggestion.

IO_DATA_PATH_WIDTH_8 is 0, so a bit-and with it is always false. The
value IO_DATA_PATH_WIDTH covers the bits of the IO_DATA_PATH constants, so
first pick those bits and then make the test using !=.
