Linus Torvalds dfdf16ecfd SCSI misc on 20200806
This series consists of the usual driver updates (ufs, qla2xxx, tcmu,
 lpfc, hpsa, zfcp, scsi_debug) and minor bug fixes.  We also have a
 huge docbook fix update like most other subsystems and no major update
 to the core (the few non trivial updates are either minor fixes or
 removing an unused feature [scsi_sdb_cache]).

Christoph Hellwig b8f1d1e058 scsi: Wire up ata_scsi_dma_need_drain for SAS HBA drivers
We need ata_scsi_dma_need_drain for all drivers wired up to drive ATAPI
devices through libata.  That also includes the SAS HBA drivers in addition
to native libata HBA drivers.

Arnd Bergmann 75c0b0e118 compat_ioctl: scsi: handle HDIO commands from drivers
The ata_sas_scsi_ioctl() function implements a number of HDIO_* commands
for SCSI devices, it is used by all libata drivers as well as a few
drivers that support SAS attached SATA drives.

The only command that is not safe for compat ioctls here is
HDIO_GET_32BIT. Change the implementation to check for in_compat_syscall()
in order to do both cases correctly, and change all callers to use it
as both native and compat callback pointers, including the indirect
callers through sas_ioctl and ata_scsi_ioctl.

James Bottomley 42caa0edab scsi: aic94xx: fix module loading
The aic94xx driver is currently failing to load with errors like

sysfs: cannot create duplicate filename '/devices/pci0000:00/0000:00:03.0/0000:02:00.3/0000:07:02.0/revision'

Because the PCI code had recently added a file named 'revision' to every
PCI device.  Fix this by renaming the aic94xx revision file to
aic_revision.  This is safe to do for us because as far as I can tell,
there's nothing in userspace relying on the current aic94xx revision file
so it can be renamed without breaking anything.

Christoph Hellwig 2a3d4eb8e2 scsi: flip the default on use_clustering
Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page.  Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

Kees Cook 6396bb2215 treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

Hannes Reinecke cc199e7846 scsi: libsas: move bus_reset_handler() to target_reset_handler()
The bus reset handler is calling I_T Nexus reset, which logically is a
target reset as it need to specify both the initiator and the target.
So move it to target reset.

Greg Kroah-Hartman e14a396772 scsi: remove DRIVER_ATTR() usage
It's better to use the DRIVER_ATTR_RW() and DRIVER_ATTR_RO() macros to
explicitly show that this is a read/write or read/only sysfs file.  So
convert the remaining SCSI drivers that use the old style to use the
newer macros.

Bonus is that this removes some checkpatch.pl warnings :)

This is part of a series to drop DRIVER_ATTR() from the tree entirely.

Johannes Thumshirn c5ce0abeb6 scsi: sas: move scsi_remove_host call into sas_remove_host
Move scsi_remove_host call into sas_remove_host and remove it from SAS
HBA drivers, so we don't mess up the ordering. This solves an issue with
double deleting sysfs entries that was introduced by the change of sysfs
behaviour from commit bcdde7e221 ("sysfs: make __sysfs_remove_dir()
recursive").

Christoph Hellwig 64d513ac31 scsi: use host wide tags by default
This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

Jack Wang 4024593d5d aic94xx: remove SCSI host before detaching from SAS transport
commit cff549e486 ("scsi: proper state checking and module refcount
handling in scsi_device_get") the reference count of scsi device was
changed, which could lead to when rmmod with at least on drive attached,
SCSI error handle will run into infinite loop, and lockup the system.

Fix it by remove scsi host first, this way scsi core will not send
commands down after detaching SAS transport.

This is a follow up fix for Benjamin's fix for pm80xx.

All existing usages of IORESOURCE_CACHEABLE call ioremap() instead of
ioremap_nocache() if the resource is cacheable, however ioremap() is
uncached by default. Clearly none of the existing usages care about the
cacheability. Particularly devm_ioremap_resource() never worked as
advertised since it always fell back to plain ioremap().

Clean this up as the new direction we want is to convert
ioremap_<type>() usages to memremap(..., flags).

Christoph Hellwig 79855d1785 libsas: remove task_collector mode
The task_collector mode (or "latency_injector", (C) Dan Willians) is an
optional I/O path in libsas that queues up scsi commands instead of
directly sending it to the hardware.  It generall increases latencies
to in the optiomal case slightly reduce mmio traffic to the hardware.

Only the obsolete aic94xx driver and the mvsas driver allowed to use
it without recompiling the kernel, and most drivers didn't support it
at all.

Remove the giant blob of code to allow better optimizations for scsi-mq
in the future.

All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.

Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq.  This means even

Dan Williams 9508a66f89 [SCSI] libsas: async ata scanning
libsas ata error handling is already async but this does not help the
scan case.  Move initial link recovery out from under host->scan_mutex,
and delay synchronization with eh until after all port probe/recovery
work has been queued.

Device ordering is maintained with scan order by still calling
sas_rphy_add() in order of domain discovery.

Since we now scan the domain list when invoking libata-eh we need to be
careful to check for fully initialized ata ports.

Dan Williams b91bb29618 [SCSI] libsas: use ->set_dmamode to notify lldds of NCQ parameters
sas_discover_sata() notifies lldds of sata devices twice.  Once to allow
the 'identify' to be sent, and a second time to allow aic94xx (the only
libsas driver that cares about sata_dev.identify) to setup NCQ
parameters before the device becomes known to the midlayer.  Replace
this double notification and intervening 'identify' with an explicit
->lldd_ata_set_dmamode notification.  With this change all ata internal
commands are issued by libata, so we no longer need sas_issue_ata_cmd().

The data from the identify command only needs to be cached in one
location so ata_device.id replaces domain_device.sata_dev.identify.

When an lldd invokes ->notify_port_event() it can trigger a chain of libsas
events to:

  1/ form the port and find the direct attached device

  2/ if the attached device is an expander perform domain discovery

A call to flush_workqueue() will only flush the initial port formation work.
Currently libsas users need to call scsi_flush_work() up to the max depth of
chain (which will grow from 2 to 3 when ata discovery is moved to its own
discovery event).  Instead of open coding multiple calls switch to use
drain_workqueue() to flush sas work.

drain_workqueue() does not handle new work submitted during the drain so
libsas needs a bit of infrastructure to hold off unchained work submissions
while a drain is in flight.  A lldd ->notify() event is considered 'unchained'
while a sas_discover_event() is 'chained'.  As Tejun notes:

  "For now, I think it would be best to add private wrapper in libsas to
   support deferring unchained work items while draining."

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

James Bottomley 63edf49e67 [SCSI] aic94xx: plumb in I_T_nexus_reset task management function
Currently aic94xx has no exported I_T_nexus_reset function.  This is a
bit of a huge problem, since sas_ata relies on this function to
perform an ATA phy reset and also it means that if abort fails, we
really have no bigger hammer to hit everything with.

Plumb in the I_T_nexus_reset by quiescing the sequencer, sending the
correct phy reset (link for ATA and hard for SAS) and then carefully
resuming the sequencer again.

Gilbert Wu 1237c98db2 [SCSI] aic94xx: update BIOS image from user space.
1. Create a file "update_bios" in sysfs to allow user to update bios
    from user space.

 2. The BIOS image file can be downloaded from web site

"http://www.adaptec.com/en-US/downloads/bios_fw/bios_fw_ver?productId=SAS-48300&dn=Adaptec+Serial+Attached+SCSI+48300"
    and copy the BIOS image into /lib/firmware folder.

 3. The aic994xx will accept "update bios_file" and "verify bios_file"
    commands to perform update and verify BIOS image .

    For example:

     Type "echo "update asc483c01.ufi" > /sys/devices/.../update_bios"
          to update BIOS image from /lib/firmware/as483c01.ufi file into
          HBA's flash memory.

     Type "echo "verify asc483c01.ufi" > /sys/devices/.../update_bios"
          to verify BIOS image between /lib/firmware/asc48c01.ufi file
and
          HBA's flash memory.

 4. Type "cat  /sys/devices/.../update_bios" to view the status or
result
    of updating BIOS.

Matthew Wilcox f01abb362f [SCSI] aic94xx: Free scsi host on error
If an error occurred during initialisation, we would sometimes fail to
call scsi_host_put() and thus end up with a leaked scsi_host.  It was
also possible to miss calling scsi_remove_host().

Paul Mundt 20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

Auke Kok 44c10138fd PCI: Change all drivers to use pci_device->revision
Instead of all drivers reading pci config space to get the revision
ID, they can now use the pci_device->revision member.

This exposes some issues where drivers where reading a word or a dword
for the revision number, and adding useless error-handling around the
read. Some drivers even just read it for no purpose of all.

In devices where the revision ID is being copied over and used in what
appears to be the equivalent of hotpath, I have left the copy code
and the cached copy as not to influence the driver's performance.

Compile tested with make all{yes,mod}config on x86_64 and i386.
