Make the LSO code work on BE platforms: parsing_data field of
a parsing BD (PBD) for 57712 was improperly composed which made FW read wrong
values for TCP header's length and offset and, as a result, the corresponding
PCI device was performing bad DMA reads triggering EEH.

NETIF_F_HW_CSUM is a superset of NETIF_F_IP_CSUM+NETIF_F_IPV6_CSUM, but
some drivers miss the difference. Fix this and also fix UFO dependency
on checksumming offload as it makes the same mistake in assumptions.

NIC partitioning is another flavor of multi function - having few
PCI functions share the same physical port. Unlike the currently
supported mode of multi-function which depends on the switch
configuration and uses outer-VLAN, the NPAR mode is switch independent
and uses the MAC addresses to distribute incoming packets to the different
functions. This patch adds the specific HW setting of the NPAR mode
and some distinctions between switch dependent (SD) and
switch independent (SI) multi-function (MF) modes where the configuration
is not the same.

Currently the skb->protocol field is used to setup checksum
offloading on transmit for the correct protocol. However, if
vlan offloading is disabled or otherwise not used, the protocol
field will be ETH_P_8021Q, not the actual protocol. This will
cause the checksum to be not computed correctly, even though the
hardware is capable of looking inside vlan tags. Instead,
look inside the header if necessary to determine the correct
protocol type.
To some extent this fixes a regression from 2.6.36 because it
was previously not possible to disable vlan offloading and this
error case was not exposed.

Many (but not all) drivers check to see whether there is a vlan
group configured before using a tag stored in the skb. There's
not much point in this check since it just throws away data that
should only be present in the expected circumstances. However,
it will soon be legal and expected to get a vlan tag when no
vlan group is configured, so remove this check from all drivers
to avoid dropping the tags.

Optimized the branching in the bnx2x_rx_int() based on the fact
that FP CQE will always have at least one of START or STOP flags set,
so if not both bits are set and START bit is not set,
then it's a STOP bit that is set.

Default number of rx buffers will be divided equally
between allocated queues. This will decrease amount of
pre-allocated buffers on systems with multiple CPUs.
User can override this behavior with ethtool -G.
Minimum amount of rx buffers per queue set to 128.

Make the needed changes for the new scheme especially around PHY
initialization, and link detection.
- The get_emac_base function was changed to enable setting different MAC
access setting for each PHY based of external configuration.
- As a part of the dual-media preparation, the save_spirom_version was
modified to accept a PHY specific version address to be saved in the
shmem. This will enable to save more than single spirom version in
different locations.
- The test_link function was changed also as a preparation for the
dual-media upcoming changes
- Duplicate initialization logic was removed from the
link_setting_status and link_initialize functions

Move the code into PHY oriented functions, and for that a new structure
is defines for each PHY which contain PHY properties and its own
functions. This also enables to encapsulate all PHY specific operations
into the PHY functions. During initialization, the PHYs will be probed
by the "bnx2x_phy_probe" function to detect which PHYs exist on-board,
and configure them accordingly. Note that the ext_phy_reset
implementation was incorporated in the ext_phy_init since it is actually
part of the PHY initialization procedure.

We can avoid setting again skb->ip_summed to CHECKSUM_NONE in drivers.
Introduce skb_checksum_none_assert() helper so that we keep this

PHY locking is required between two ports for some external PHYs. Since
initialization was done in the common init function (called only on the
first port initialization) rather than in the port init function, there
was in fact no PHY locking between the ports.

Newly created files have no functionality changes,
but includes some functionality from bnx2x_main.c which
is common for PF and coming in the future VF driver.

Store module parameters during initialization of main driver
structure. This will allow access to the parameters from different
files.

This commit includes files movement to newly created folder
using git-mv command and fixes references in cnic and bnx2x code
to each other.
files moved using following:
#!/bin/bash
mkdir drivers/net/bnx2x/
list=$(cd drivers/net/ && ls bnx2x*.[ch])
for f in $list; do
git mv -f drivers/net/$f drivers/net/bnx2x/$f
done
